Abstract:Objectives: The majority of the previous research has focused on the impact of average sleep parameters on longevity. In this study, we aimed to investigate the associations of day-to-day deviations in sleep parameters with biological ages among 6052 adults participating in the 2011-2014 waves of the US National Health and Nutrition Examination Survey. Methods: Sleep parameters, including sleep duration, efficiency, midpoint, and day-to-day deviations in sleep parameters, including standard deviation of sleep duration (sleep variability), standard deviation of sleep midpoint (sleep irregularity), catch-up sleep, and social jetlag, were obtained from 4 to 7 days of 24-h accelerometer recording. We used physiological data to compute measurements of biological aging according to 3 published algorithms: PhenoAge, Klemera-Doubal method Biological Age, and homeostatic dysregulation. Results: After adjustment of multiple covariates, we observed that all parameters of day-to-day deviations in sleep were significantly associated with biological aging with larger sleep variability, larger sleep irregularity, more catch-up sleep, and more social jetlag linked with more advanced biological aging. The significant associations of sleep irregularity, catch-up sleep, and social jetlag with biological aging indices remained even after adjustment for sleep duration, efficiency, and midpoint. Conclusion: In this study, we found that day-to-day deviations in sleep parameters are independently associated with biological aging in US general population. Since day-to-day deviation in sleep is a modifiable behavioral factor, our finding suggests that intervention aiming at increasing regularity in sleep patterns may be a novel approach for extending a healthy life span.
Country differences in nocturnal sleep variability: Observations from a large-scale, long-term sleep wearable study

Abstract:Study objectives Country or regional differences in sleep duration are well-known, but few large-scale studies have specifically evaluated sleep variability, either across the work week, or in terms of differences in weekday and weekend sleep. Methods Sleep measures obtained over 50 million night's sleep, from ∼220,000 wearable device users in 35 countries were analysed. Each person contributed an average of ∼242 nights of data. Multiple regression was used to assess the impact country of residence had on a sleep duration, timing, efficiency, weekday sleep variability, weekend sleep extension and social jetlag. Results Nocturnal sleep was shorter and had a later onset in Asia than other regions. Despite this, sleep efficiency was lower and weekday sleep duration was higher. Weekend sleep extension was longer in Europe and the USA than in Asia, and was only partially related to weekday sleep duration. There were also cross-country differences in social jetlag although the regional differences were less distinct than for weekend sleep extension. Conclusions In addition to regional differences in sleep duration, cross-country differences in sleep variability and weekend sleep extension suggest that using the latter as an indicator of sleep debt may need to be reconsidered. In countries exhibiting both short sleep and high weekday sleep variability, a culturally different means of coping with inadequate sleep is likely. Country or region differences in culture, particularly those related to work, merit closer examination as factors influencing the variability in normative sleep patterns around the world.

Abstract:Limited information exists on age and racial disparities in sleep duration and mortality in the United States (US) population. This study compared the association between mortality and sleep duration within distinct races and age groups in the US. This study used data on 26,915 US citizens (≥ 18 years) from the 2004 wave of the National Health Interview Survey, linked to the National Death Index prospective mortality through 2019. Cox proportional hazard models were used to obtain hazard ratios (HRs) and 95% confidence intervals (CIs) for mortality by sleep duration, race (Whites, Black/African Americans, and Others (AIAN, Asian, and Native Hawaiian or other Pacific Islander)), and age (< 40, 40–60, and ≥ 60 years), while controlling for covariates such as sex, education, smoking status, disease history, and other vital factors. Race and age significantly modified the sleep duration-mortality relationship. Compared to other races, White participants exhibited higher mortality risks at all hours except at 5–6 h [HR: 0.993, 95% CI: 0.923–1.069]. Likewise, sleep duration associated mortality risks varied by age. Those at greater risk included < 40 years sleeping for 1–4 h [HR: 2.461, 95% CI: 1.446–4.187], 40–< 60 years sleeping for less than 7 h and more than 8 h, and ≥ 60 years sleeping for 9 h [HR: 1.309, 95% CI: 1.162–1.475] and ≥ 10 h [HR: 1.662, 95% CI: 1.486–1.858]. Age and race were significant effect modifiers in the sleep duration-mortality relationship. Thus, it is important to consider these factors when evaluating mortality risks associated with sleep patterns.

Risk Factors Associated with Short Sleep Duration among Chinese School-Aged Children

Abstract:Objectives: To examine risk factors regarding short sleep duration among Chinese school-aged children.Methods: A random sample of 20,778 children aged around 5-11 years participated in a cross-sectional survey, which was conducted in eight cities of China in 2005. A parent-administered questionnaire was used to collect information on children's sleep duration and possible related factors from eight domains. Short sleep duration was defined as total sleep duration <9 h per day.Results: In all, 28.3% of the sampled children slept <9 h per day. The multivariate logistic regression identified, after adjusting for demographic and socioeconomic variables, factors associated with short sleep duration: more television viewing during weekdays (OR = 1.21, p = 0.004), more frequent computer/internet using (OR = 1.17, p = 0.006), earlier school starting time (OR = 1.10, p = 0.020), more time on homework during weekdays (OR = 1.66, p < 0.001) and weekends (OR = 1.14, p = 0.001), poor bedtime hygiene (e.g., having drinks with caffeine after 6:00PM [OR = 1.22, p < 0.001], doing exciting activities during bedtime [OR = 1.16, p < 0.001], and irregular bedtime [OR = 1.55, p < 0.001]), and shorter sleep duration of parents (mother: OR = 1.31, p < 0.001 for sleep duration <6 h and OR = 1.24, p = 0.006 for 6-8 h; father: OR = 1.52, p < 0.001 for <6 h and OR = 1.19, p < 0.001 for 6-8 h).Conclusions: Factors associated with sleep duration covered multidimensional domains among school-aged children. Compared to sleep environments and chronic health problems, school schedules, lifestyle patterns, and parents' sleep habits had greater impact on children's sleep duration, indicating the existing chronic sleep loss in school children could be, at least partly, intervened by reducing the use of visual technologies, by changing the school schedules, by improving the sleep hygiene routine, and by regulating parents' sleep habits. Abstract:Abstract Introduction Epidemiological evidence indicates that short, long and variable sleep duration are associated with cardiometabolic outcomes. However, due to the ubiquitous role of sleep, one would expect to see effects on a broad range of human diseases. Using electronic health records (EHR) and wearable sleep data from the All of Us Research Program, we conducted a cross-sectional analysis of sleep duration and variability with the risk of over 3530 chronic conditions across the human phenome. Methods All of Us participants (≥18 years) with available EHR data and at least 90 days of continuous monitoring using Fitbit devices under a ‘bring your own device’ model were included. Average (mean) and variability (standard deviation) of sleep duration were derived based on device-generated data. Chronic conditions were determined based on mapping of International Classification of Disease 10 codes available in the EHR to phecodes, an ontology representing clinically meaningful concepts. Associations between sleep traits and phecodes were assessed using multiple test corrected logistic regression adjusted for age, sex, and race. Results A total of 8,256 participants with available Fitbit data were included. Participants were 89% White, 72% female, 72% college educated and had median (IQR) age of 51 (36.6-52.0) years, BMI 28.3 (24.4-33.0) kg/m2, and 1,566 (860-2307) days with sleep measurements. The median average nightly sleep duration was 6.34 (5.67-6.91) hours and the median nightly sleep variability was 1.63 (1.33-1.95) hours. In adjusted analyses, participants with greater sleep duration had significantly lower odds of obesity (OR; 95%CI = 0.85; 0.80-0.91, obstructive sleep apnea (0.74; 0.74-0.88), type 2 diabetes (0.68, CI 0.58-0.80), and essential hypertension (0.86; 0.81-0.92). Conversely, participants with greater sleep duration variability had higher odds of major depressive disorder (2.15; 1.84-2.51), anxiety disorder (1.75; 1.51-2.03), gastroesophageal reflux disease (1.61; 1.39-1.86), and abnormal electrocardiogram (1.80; 1.47-2.19). Conclusion Our findings suggest important associations between sleep duration patterns and chronic conditions, emphasizing the importance of sleep health in disease prevention and management. Important limitations include analysis of a selected population with access to wearable devices, which may not represent the general population enrolled in All of Us. Abstract:Continuous monitoring of cortical blood flow provides real time information of CBF-changes during neurosurgical operations and on the neurointensive care unit. Laser Doppler flowmetry is a continuous, noninvasive technique suitable for measurement of the cortical microcirculation. In a number of experimental studies we have analyzed the characteristics of this method for cortical blood flow monitoring. The high spatial resolution of laser Doppler flowmetry and the heterogeneity of the cortical microvascular network results in a scatter of flow values over a wide range depending on the site of measurement. Data collection from different spots and calculation of frequency histogram may serve as a measure of cortical blood flow. For clinical application instead of single fiber probes a multispot measurement approach will provide a reliable cortical blood flow monitoring. The use of continuous techniques will lead to a better understanding of cerebral hemodynamics under pathological conditions.

Abstract:Abstract Introduction Sleep fragmentation by arousals and sleep-disordered breathing (SDB) is known to increase with age and differ by gender, although the magnitude of these effects is largely unknown. Population studies allow large collection of continuous nightly data to explore these effects. Methods Sleep fragmentation index (SFI, arousals/hour) and SDB were analyzed by a commercially available home monitoring device (Sleeptracker-AI Monitor, Fullpower Technologies Inc., California, USA) that passively monitors sleep using piezo-electric sensors. Validated sleep/respiratory parameters were derived from device data. De-identified data were analyzed, following review and exemption of the study (#57681) from Stanford University IRB. SFI was assessed using an estimated arousal index, estimating individual arousal events from passive sensor data, with validation against PSG. Data 8/16/2023-12/15/2023 were reviewed. Individuals with birth year and gender provided and age 20-79 years in 2023 were included in the analytic dataset. Results 117,846 individuals (64,049 men, 48.2±12.6 years; 53,797 women, 48.3±12.6 years) with 10,090,778 recorded nights met the inclusion criteria. For AHI< 5, the SFI mean±SE for women and men were 13.6±0.16 and 12.6±0.13 at 20-24 years, respectively, and increased with age to 15.0±0.26 and 16.0±0.24 for 75-79 years. For AHI< 5, men showed a significantly greater SFI during REM sleep than women across all ages above 24 years, but not during NREM sleep. For AHI< 5, SFI significantly increased with age during NREM sleep (women 11.9±0.15 to 14.6±0.17, men 10.8±0.12 to 15.1±0.26), but for REM sleep increased up to 25-34 years (women 18.8±0.12, men 19.6±0.09), but then steadily decreased with age to 75-79 years (women 16.1±0.27, men 18.1±0.24), and a similar trend was found for AHI≥5. For AHI≥5 compared to AHI< 5, SFI was significantly higher by 6-13 arousals/hour across all ages. Conclusion Use of a noninvasive in-home monitoring device enabling continuous nightly collection/analysis of a large sample of sleep/respiratory data revealed differences in sleep fragmentation across age and by gender. Greater sleep fragmentation was observed for men across age during REM sleep, and fragmentation during REM sleep decreased with age after 34 years, which is unexpected and warrants further study given that overall quantities of NREM and REM sleep decrease with age. Abstract:IntroductionPoor mental health in adolescents has been linked to more variable sleep. However, most studies use self-reported measures of sleep. There is also a lack of research on sex differences in these associations, which is important given that poor emotional health is more common in female than male adolescents. The current study examined whether sleep variability measured through actigraphy was associated differently with self-reported mood in female versus male adolescents in between-person analyses.MethodsData were collected from a micro-longitudinal substudy of the age 15 wave of the Fragile Families and Child Wellbeing Study. Adolescents wore a wrist-actigraphy device and completed daily surveys for approximately one week (mean=5.6 actigraphy days; n range: 577-604 depending on the mood variable) where they reported levels of loneliness, happiness, excitement, and anger on a five-point Likert scale (0, not at all, to 4, extremely). Separate mixed models assessed whether sex moderated the association between mean mood and actigraphy-measured sleep variability measures including residual standard deviation (riSD) of sleep duration, onset, and offset; sleep regularity index (SRI), and social jetlag (|midpoint of sleep on school nights – midpoint of sleep on free nights|; n for social jetlag ranged from 363-376 depending on the mood variable).ResultsIn both sexes, greater average happiness (β=−.11, p=.009) and excitement (β=−.11, p=006) were associated with less variability in sleep duration, and greater average excitement was associated with higher SRI (β=.13, p=.001). Male adolescents reporting greater average happiness and excitement had less variability (riSD) in sleep onset (happiness β=−.15, p=.008; excitement β=−.14, p=.016) and lower levels of social jetlag (happiness β=−.25, p .20). Neither loneliness nor anger were associated with sleep variability measures in either sex (all p>.15).ConclusionGreater positive moods are associated with lower sleep variability, which may protect mental and physical health. Boosting positive mood in adolescents may assist in normalizing sleep schedules, and decreasing sleep variability may aid in the maintenance of positive emotional health, particularly for males. Abstract:Abstract Introduction Adolescents often obtain short sleep duration, which has been linked to poor grades and greater behavioral issues at school. There is limited research on the association between other dimensions of sleep and school-related outcomes, and few studies use objective measures of sleep. The current study examined associations of multiple actigraphic sleep dimensions with school-related outcomes among adolescents. Methods Data were collected from a sub-study of the age 15 wave of the Fragile Families and Child Wellbeing Study (N=774-782 depending on the outcome), a national sample of diverse teens. Adolescents wore a wrist-actigraphy device for approximately one week (mean=6.6 nights) and completed a survey reporting current grades and current or past problems at school. Separate linear and logistic regression models assessed whether actigraphic sleep duration, timing, maintenance efficiency, variability (SD) in duration and timing, and sleep regularity index (SRI) were associated with grades and behavioral issues at school. Analyses adjusted for demographic and household characteristics, depressive symptoms, and anxious symptoms. Results Greater variability in sleep onset was associated with higher odds of receiving a D or F in any course (OR=1.29, p=.040) and of ever failing a course (OR=1.26, p=.038). Later sleep onset (β=−.07, p=.029) and offset (β=−.08, p=.014) and greater variability in sleep duration (β=−.07, p=.035) were associated with fewer A grades. Later sleep offset was also associated with lower average GPA (β=−.07, p=.033). Later sleep onset (OR=.91, p=.049) and offset (OR=.91, p=.035) and greater variability in sleep offset (OR=.83, p=.029) were associated with lower odds of ever taking an honors course. Later sleep offset (OR=1.11, p=.034) and greater variability in sleep duration (OR=1.31, p=.012) and onset (OR=1.42, p=.004) were associated with higher odds of ever being suspended or expelled in the past two years. Greater variability in sleep duration was associated with greater behavioral problems in school (β=.13, p<.001). Sleep duration, maintenance efficiency, and SRI were not associated with school-related outcomes (p≥.05). Conclusion Later sleep timing and greater sleep variability are risk factors for school-related problems among adolescents. Promoting earlier sleep timing and reducing sleep variability across the week may improve academic and behavioral outcomes among adolescents in school. Support (if any) R01HD073352

Inverted U-shaped relationship between sleep duration and phenotypic age in US adults: a population-based study

Abstract:Sleep is a modifiable behavior that can be targeted in interventions aimed at promoting healthy aging. This study aims to (i) identify the sleep duration trend in US adults; (ii) investigate the relationship between sleep duration and phenotypic age; and (iii) explore the role of exercise in this relationship. Phenotypic age as a novel index was calculated according to biomarkers collected from US adults based on the National Health and Nutrition Examination Survey (NHANES). Sleep information was self-reported by participants and discerned through individual interviews. The principal analytical method employed was weighted multivariable linear regression modeling, which accommodated for the complex multi-stage sampling design. The potential non-linear relationship was explored using a restricted cubic spline (RCS) model. Furthermore, subgroup analyses evaluated the potential effects of sociodemographic and lifestyle factors on the primary study outcomes. A total of 13,569 participants were finally included in, thereby resulting in a weighted population of 78,880,615. An examination of the temporal trends in sleep duration revealed a declining proportion of individuals with insufficient and markedly deficient sleep time since the 2015–2016 cycle. Taken normal sleep group as a reference, participants with extreme short sleep [β (95% CI) 0.582 (0.018, 1.146), p = 0.044] and long sleep [β (95% CI) 0.694 (0.186, 1.203), p = 0.010] were both positively associated with phenotypic age using the fully adjusted model. According to the dose–response relationship between sleep duration and phenotypic age, long sleep duration can benefit from regular exercise activity, whereas short sleep duration with more exercise tended to have higher phenotypic age. There is an inverted U-shaped relationship between short and long sleep durations and phenotypic age. This study represents an important step forward in our understanding of the complex relationship between sleep and healthy aging. By shedding light on this topic and providing practical exercise recommendations for promoting healthy sleep habits, researchers can help individuals live longer, healthier, and more fulfilling lives.

Abstract:Background: We propose a method for estimating the timing of in-bed intervals using objective data in a large representative U.S. sample, and quantify the association between these intervals and age, sex, and day of the week. Methods: The study included 11,951 participants six years and older from the National Health and Nutrition Examination Survey (NHANES) 2003-2006, who wore accelerometers to measure physical activity for seven consecutive days. Participants were instructed to remove the device just before the nighttime sleep period and put it back on immediately after. This nighttime period of non-wear was defined in this paper as the objective bedtime (OBT), an objectively estimated record of the in-bed-interval. For each night of the week, we estimated two measures: the duration of the OBT (OBT-D) and, as a measure of the chronotype, the midpoint of the OBT (OBT-M). We estimated day-of-the-week-specific OBT-D and OBT-M using gender-specific population percentile curves. Differences in OBT-M (chronotype) and OBT-D (the amount of time spent in bed) by age and sex were estimated using regression models. Results: The estimates of OBT-M and their differences among age groups were consistent with the estimates of chronotype obtained via self-report in European populations. The average OBT-M varied significantly by age, while OBT-D was less variable with age. The most pronounced differences were observed between OBT-M of weekday and weekend nights. Conclusions: The proposed measures, OBT-D and OBT-M, provide useful information of time in bed and chronotype in NHANES 2003-2006. They identify within-week patterns of bedtime and can be used to study associations between the bedtime and the large number of health outcomes collected in NHANES 2003-2006.

Few studies have investigated the intersection of other sociodemographic characteristics with race/ethnicity on sleep duration prevalence.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>Longitudinal retrospective analysis of continental U.S. cohort, the <u>RE</u>asons for <u>G</u>eographic <u>A</u>nd <u>R</u>acial <u>D</u>ifferences in <u>S</u>troke (REGARDS)</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Participants</h3><p>Black (<em>n</em> = 7,547) and white (<em>n</em> = 12,341) adults, 56% women, ≥45 years</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Measurements</h3><p>At baseline (2003–07), participants reported age, sex, race, education, income, marital status, U.S. region, and employment status. The weighted average of reported sleep duration on weekdays and weekends, assessed at follow-up (2008–10), was categorized as <6, 6.0–6.99, 7.0–7.99 [reference], 8.0–8.99, and ≥9 h. Multinomial logistic regression models examined the independent and multivariable associations of sociodemographic factors with sleep duration. Interactions terms between race with education, income, region, and sex were examined.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Average sleep duration was 7.0 h (SD=1.3). Prevalence of short (<6 h) and long (≥9 h) sleep duration was 11.4% (<em>n</em> = 2,260) and 7.0% (<em>n</em> = 1,395), respectively. In the multivariable model, interactions terms race*income, race*sex, and race*region were significant (<em>P</em> < .05). Relative to white adults, black adults, were most likely to have short sleep duration. The magnitude of that likelihood increased across greater levels of household income, but with greatest odds among black adults living outside of the Southeast and Appalachian United States, particularly for men (≥$75k; black men OR = 5.47, 95%CI: 3.94,7.54; black women OR = 4.28, 95%CI: 3.08, 5.96).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Race in the context of socioeconomic, sex, and regional factors should be examined as key modifiers of sleep duration.</p>

Different Expression Profiles of Bioactive Peptides in Pelophylax nigromaculatus from Distinct Regions

Abstract:Amphibian skin is an abundant repository of bioactive peptides, important components of the defensive system. The variability of the bioactive peptide repertoires of individual species remains unclear. In this study, dark-spotted frogs were collected from Kunming in Yunnan Province, China and Guiyang in Guizhou Province, China to determine whether the bioactive peptides in amphibian skin differ between the two regions. Eight antimicrobial peptides and an antioxidant peptide were identified by screening of cDNA library. Among the identified peptides, three antimicrobial peptides (pelophylaxin-2GY, temporin-1GY, and temporin-1KM) and an antioxidant peptide (antioxidin-PN) are reported here for the first time. Nigrocin-1, nigrocin-2, and pelophylaxin-2 were expressed by frogs in both regions. Pelophylaxin-2GY and temporin-1GY were found only in the frogs from Guiyang, whereas antioxidin-PN, esculetin-1, esculetin-2, and temporin-1KM were found only in those from Kunming. This difference was confirmed by allele-specific RT-PCR. The bioactive peptides expressed clearly varied between these populations of the same species.

Abstract:Current evidence and professional guidance recommend sleeping between 7 and 9 h in a 24-hour period for optimal health. The present study examines the association between sleep duration and mortality and assesses whether this association varies by racial/ethnic identity for a large and diverse sample of United States adults. We use data on 274,836 adults, aged 25 and older, from the 2004–2014 waves of the National Health Interview Survey (NHIS) linked to prospective mortality through 2015 (23,382 deaths). Cox proportional hazards models were used in multi-variable regressions to estimate hazard ratios for mortality by sleep duration and racial/ethnic identity, controlling for sociodemographic, socioeconomic, and psychological distress variables. We find elevated risks of mortality from any cause for adults who sleep less than 5 h or more than 9 h in a 24-hour period after all adjustments. Further, we find evidence that these elevated risks for mortality are more pronounced for some racial/ethnic groups and less pronounced for others. Improved understanding of differences in sleep duration and sleep health can facilitate more effective and culturally-tailored interventions around sleep health, improving overall well-being and enhancing longevity.

Abstract:Background: Suboptimal rest-activity patterns in adolescence are associated with worse health outcomes in adulthood. Understanding sociodemographic factors associated with rest-activity rhythms may help identify subgroups who may benefit from interventions. This study aimed to investigate the association of rest-activity rhythm with demographic and socioeconomic characteristics in adolescents. Methods: Using cross-sectional data from the nationally representative National Health and Nutrition Examination Survey (NHANES) 2011-2014 adolescents (N = 1814), this study derived rest-activity profiles from 7-day 24-hour accelerometer data using functional principal component analysis. Multiple linear regression was used to assess the association between participant characteristics and rest-activity profiles. Weekday and weekend specific analyses were performed in addition to the overall analysis. Results: Four rest-activity rhythm profiles were identified, which explained a total of 82.7% of variance in the study sample, including (1) High amplitude profile; (2) Early activity window profile; (3) Early activity peak profile; and (4) Prolonged activity/reduced rest window profile. The rest-activity profiles were associated with subgroups of age, sex, race/ethnicity, and household income. On average, older age was associated with a lower value for the high amplitude and early activity window profiles, but a higher value for the early activity peak and prolonged activity/reduced rest window profiles. Compared to boys, girls had a higher value for the prolonged activity/reduced rest window profiles. When compared to Non-Hispanic White adolescents, Asian showed a lower value for the high amplitude profile, Mexican American group showed a higher value for the early activity window profile, and the Non-Hispanic Black group showed a higher value for the prolonged activity/reduced rest window profiles. Adolescents reported the lowest household income had the lowest average value for the early activity window profile. Conclusions: This study characterized main rest-activity profiles among the US adolescents, and demonstrated that demographic and socioeconomic status factors may shape rest-activity behaviors in this population.

Abstract:Abstract Introduction Connections between sleep and cardiovascular risk and health are reported in adults. The sleep regularity index (SRI), a measure of day-to-day variability in sleep-wake times, has been shown to be associated with cardiovascular risk in older adults. The connection between SRI and cardiovascular health, the American Heart Association (AHA) derived concept of positive health promotion across the lifespan, has yet to be reported. The aim of this work is to investigate associations between SRI and cardiovascular risk and health. Methods Using National Health and Nutrition Examination Survey (NHANES) 2011-2014 data in US adults aged 40-75 years of age who are free of cardiovascular disease, the SRI was calculated from actigraphy across seven days using the ActiGraph model GT3X+, manufactured by ActiGraph of Pensacola, FL. Life’s Simple 7 (LS7) and Life’s Essential 8 (LE8) scores were calculated based on AHA. The 10-year risk of atherosclerotic cardiovascular disease (ASCVD) was calculated using the pooled cohort equations defined by the American College of Cardiology/AHA guidelines. The four-year interview weight was used for all complex sample analyses. SPSS complex sample general linear models were used to assess the association between SRI, ASCVD score, LE8, LS7, and demographic variables. Results Among the NHANES sample (n=5589; 52.8 % female; mean age = 54.5, SE = 0.17), the mean SRI score was 63.0 (SE = 0.52), the average ASCVD score was 0.12 (SE = .003), the mean LE8 and LS7 scores were 57.1 (SE = 0.46) and 6.6 (SE = 0.04). SRI was not significantly related to ASCVD score (β = -0.001, p = .062). Individuals with higher SRI scores reported higher LE8 (β = 0.265, p < .001) and LS7 (β = 0.035, p < .001) scores. After controlling for gender, race ethnicity, household food security, and SNAP participation status, SRI remained as a significant predictor to LE8 (β = 0.219, p < .001) and LS7 (β = 0.029, p <.001 Conclusion Among U.S. adults aged 40 to 75 years of age, sleep regularity is positively associated with cardiovascular health. Abstract:Abstract Introduction Abnormal sleep duration is an established risk factor for obesity. However, the potential adverse effects of sleep's nightly variability, including fluctuations in duration, remain less explored. Irregular sleep duration may increase circadian disruption and subsequently contribute to metabolic alterations, behavioral and mood changes, and obesity development. Methods From 2013-2015, a subsample of the UK Biobank (UKB) participants wore accelerometers for a week. Our longitudinal study included 8,438 participants (mean age: 64 years) with ≥5 days of accelerometer data and repeated weight measurements who had BMI< 30 kg/m2 at baseline. Irregular sleep duration was assessed by the within-person standard deviation (SD) of 7-night accelerometer-measured sleep duration. Weight was objectively measured at UKB center visits or extracted from UK general practice records at two timepoints. The baseline measurement was taken in a period spanning from two years before to one year after the accelerometer study. The follow-up measurement was obtained on average 3.6 years after the baseline measurement (interquartile range: 1.8-5.0) and at least 6 months after the accelerometer study. Poisson regression with robust error variance was used to estimate relative risk (RR) for incident obesity (BMI≥30 kg/m2) according to sleep duration-SD categories. Results A total of 419 participants developed obesity at the follow-up measurement. Compared with participants with a sleep duration-SD < 30 minutes, the RR (95% CI) for incident obesity was 1.22 (0.86, 1.73) for 31-45 minutes, 1.60 (1.14, 2.26) for 46-60 minutes, and 2.01 (1.45, 2.80) for >60 minutes (p-trend=0.0024), after adjusting for age, sex, race, and follow-up period. Additional adjustment for lifestyle (smoking, physical activity, diet, and alcohol use), co-morbidities (dyslipidemia, hypertension, and depression), and other sleep-related factors including average sleep duration resulted in a weaker but statistically significant association (RR comparing >60 versus < 30 min: 1.73; 95% CI: 1.24, 2.41). The association was similar in men and women (P-interaction in the fully-adjusted model=0.84). Conclusion Irregular sleep duration was associated with higher risk of incident obesity in older adults, independent of average sleep duration and other obesogenic behaviors. Abstract:Abstract Introduction Racial and ethnic minority populations are at an increased risk of having shorter sleep duration and poorer sleep quality. This nationally-representative study examines whether groups differ on their typical sleep opportunity window, and whether this depends on typical work hours. Methods Data were obtained from adults who participated in the National Health And Nutrition Examination Survey (NHANES), overseen by the CDC, for the 2017-2018 and 2019-2020 (March, pre-pandemic) waves. These data were combined and weighted using NHANES sample weights. Habitual sleep timing was assessed as self-reported typical bedtime and wake time, which was converted to minutes from midnight. Race/ethnicity was self-reported as non-Hispanic White, Black/African-American, Mexican-American, Other Latinx, Asian, or Multiracial/Other. Work time was categorized as None, 20 or fewer hours, 21-39 hours, 40 hours (reference), 41-59 hours, or 60 or more hours. Covariates included demographics (age, sex) and socioeconomics (education, income-poverty ratio). Results Overall, compared to Non-Hispanic Whites, Blacks/African-Americans went to bed 14 minutes later, Mexican Americans went to bed 18 minutes earlier and got out of bed 33 minutes earlier, and Asians went to bed 13 minutes later and woke up 16 minutes later. And compared to those who worked 40 hours, those who worked less than 40 hours went to bed later and woke up later, and those who worked 60+ hours went to bed later. There was a significant race/ethnicity by work hours interaction. The relationship between race/ethnicity and sleep timing differed depending on which work hours category respondents were in. Overall, Black-White differences in bedtime were seen in those who worked 41-59 hours. Mexican-White differences in bedtime were seen for non-working and 40 hours and for waketime were seen for all <=40 hours groups. Asian-White differences were seen for bedtime for 41-59 hours and for waketime in non-working, 40 hours, and 60+ hours. Multiracial-White differences were seen for bedtime in non-working 21-39 hours, and for waketime in 21-39 hours. Conclusion Even after accounting for other elements of sleep, Blacks/African-Americans, and Asians still exhibit delayed sleep. Future research is required to further explore disparities in circadian rhythm and culturally targeted interventions for sleep health. Support (if any)

Abstract:Objective To investigate whether sleep disparities vary by birthplace among non-Hispanic White (NHW) and Hispanic/Latino adults in the USA and to investigate language preference as an effect modifier. Design Cross-sectional. Setting USA. Participants 254 699 men and women. Methods We used pooled 2004–2017 National Health Interview Survey data. Adjusting for sociodemographic and behavioural/clinical characteristics, survey-weighted Poisson regressions with robust variance estimated prevalence ratios (PRs) and 95% CIs of self-reported sleep characteristics (eg, sleep duration, trouble staying asleep) among (1) foreign-born NHW adults and Hispanic/Latino heritage groups versus US-born NHW adults and (2) Hispanic/Latino heritage groups versus foreign-born NHW adults. We further stratified by language preference in comparisons of Hispanic/Latino heritage groups with the US-born NHW group. Results Among 254 699 participants with a mean age±SE 47±0.9 years, 81% self-identified their race/ethnicity as NHW, 12% Mexican, 2% Puerto Rican, 1% Cuban, 1% Dominican and 3% Central/South American. Compared with US-born NHW adults, foreign-born NHW adults were more likely to report poor sleep quality (eg, PR trouble staying asleep =1.27 (95% CI: 1.17 to 1.37)), and US-born Mexican adults were no more likely to report non-recommended sleep duration while foreign-born Mexican adults were less likely (eg, PR ≤5-hours =0.52 (0.47 to 0.57)). Overall, Mexican adults had lower prevalence of poor sleep quality versus US-born NHW adults, and PRs were lowest for foreign-born Mexican adults. US-born Mexican adults were more likely than foreign-born NHW adults to report shorter sleep duration. Regardless of birthplace, Puerto Rican adults were more likely to report shorter sleep duration versus NHW adults. Generally, sleep duration and quality were better among Cuban and Dominican adults versus US-born NHW adults but were similar versus foreign-born NHW adults. Despite imprecision in certain estimates, Spanish language preference was generally associated with increasingly better sleep among Hispanic/Latino heritage groups compared with US-born NHW adults. Conclusion Sleep disparities varied by birthplace, Hispanic/Latino heritage and language preference, and each characteristic should be considered in sleep disparities research.

Abstract:Objectives To assess the contemporary prevalence and decade-long trends of sleep duration, sleep disorders and trouble sleeping among adults in the United States, as well as their risk factors, from 2005 to 2018. Materials and methods We used National Health and Nutrition Examination Survey data to calculate the sleep duration and weighted prevalence of sleep disorders and trouble sleeping in adults aged 20 years or older. Sleep duration, sleep disorders and trouble sleeping were assessed by questionnaire. Results A total of 27,399 people were included in the survey on sleep duration, with a weighted percentage of normal sleep (7–8 h/night) of 56.33 % (95 % CI, 53.06–59.60 %) and a weighted percentage of short sleep (5–6 h/night) of 31.73 %. In stratified descriptions, participants aged 40–49 years were more likely to sleep less than five hours, while women aged 80 years and older were more likely to sleep longer and blacks were more likely to sleep shorter. A total of 27,406 participants were included in the survey for sleep disorders. The weighted proportion of the population with sleep disorders was 8.44 % (95 % CI, 7.79–9.8 %). Independent risk factors for sleep disorders were being 40–69 years old, being white, having a high education level, smoking, having hypertension, diabetes, heart disease, and BMI ≥ 25. From 2005 to 2014, the prevalence of sleep disorders increased year by year, from 7.44 % in 2005–2006 to 10.40 % in 2013–2014 ( P for Trend<0.001). A total of 38,165 participants were included in the survey on trouble sleeping. The weighted proportion of the population with troubled sleeping was 27.30 % (25.70–28.90 %). Independent risk factors for troubled sleeping were being 30–79 years old, being white, having a high education level, smoking, drinking, having hypertension, diabetes, heart disease and BMI ≥ 25. From 2005 to 2018, the prevalence of trouble sleeping increased annually, from 24.44 % in 2005–2006 to 30.58 % in 2017–2018 ( P for trend<0.001). Conclusion Adults in the United States are likely to have abnormal sleep durations, and the prevalence of sleep disorders and troubled sleeping is on the rise.
