Abstract:Objective: To investigate the time and postoperative binocular vision of strabismus surgery for children with intermittent exotropia (X(T)). Methods: A retrospective investigation was conducted in 80 child patients with intermittent exotropia. Pre- and postoperative angles of deviation fixating at near (33 cm) and distant targets (6 m) were measured with the prolonged alternate cover testing. The binocular function was assessed with synoptophore. Twenty-one patients took the postoperative synoptophore exercise. Results: (1) A week after surgery, 96.2% of the 80 patients had binocular normotopia, while a year after surgery, 91.3% of the 80 patients had binocular normotopia; (2) Preoperatively, 58 patients had near stereoacuity, while postoperatively, 72 patients achieved near stereoacuity (P < 0.05); (3) Preoperatively, 64 patients had Grade I for the synoptophore evaluation and postoperatively, 76 patients achieved Grade I. Meanwhile, 55 patients had Grade II preoperatively and 72 achieved Grade II postoperatively. For Grade III, there were 49 patients preoperatively and 64 patients postoperatively (P < 0.05); (4) Patients of 5-8 years old had a significantly better recovery rate of binocular vision than those of 9-18 years old (P < 0.05); (5) Patients taking postoperative synoptophore exercise had a better binocular vision than those taking no exercise (P < 0.05). Conclusions: (1) Strabismus surgery can help to preserve or restore the binocular vision for intermittent exotropia; (2) Receiving the surgery at young ages may develop better postoperative binocular vision; (3) The postoperative synoptophore exercise can help to restore the binocular vision.
Corneal Topographical Changes Following Strabismus Surgery.

Abstract:PURPOSE:To study corneal topographical changes after strabismus surgery.METHODS:Computer-aided corneal topography was used in 43 strabismus patients (45 eyes) one or two days prior to and six or seven days after strabismus surgery. The spherical and cylindrical equivalents were calculated based on the simulated keratometry.RESULTS:After the surgery, only the changes at 3 mm in the inferior quadrant were statistically significant. The changes at 3 mm in the rest quadrants and the changes at 7 mm were not significant. Significant changes in spherical equivalent were found post-operatively. Neither the horizontal nor the vertical meridional equivalent showed significant changes after surgery.CONCLUSIONS:The results of corneal topographical changes following strabismus surgery in our preliminary study indicated the little effect of strabismus surgery on corneal curvature and corneal astigmatism.

Abstract:The primary goal of strabismus treatment is to correct and reverse a pathologic condition. Successful treatment is associated with relief from visual confusion or diplopia, a very high incidence of sensory binocular vision, an expanded peripheral visual field, and improved psychosocial function. Binocularity before and after strabismus surgery should be well understood. The stereopsis or sensory fusion should be examined routinely. The measurements should be repeated with the angle of strabismus neutralized with loose prisms. Eye misalignment is measured using the alternate prism and cover test, Eye alignment is measured using the simultaneous prism and cover test. The recent find is that the maximum horizontal deviation that will support true stereopsis is 4(Delta). The functional benefit can usually be obtained in the form of improved binocularity and, consequently, better stability of ocular alignment.
Factors Determining Improvement in Stereopsis and Binocularity After Good Postoperative Alignment in Patients With Childhood-Onset Strabismus

Abstract:Introduction: The purpose of the present study was to determine the factors that affect the outcome of strabismus surgery in terms of improvement in stereopsis and binocularity. Methods: Data were collected prospectively from patients with childhood-onset, concomitant, constant strabismus greater than 30 prism diopters (PD) with postoperative alignment within 10 PD. Pre- and postoperative stereopsis and binocularity testing were performed using the Titmus fly test, random dot test, Bagolini striated glass test, and Worth four dot test at one, four, and 12 weeks postoperatively. Results: A total of 73 patients (55% males and 45% females) who underwent surgery at our center were studied. The mean age at the time of surgery was 16 ± 7.7 years (range: 5-35 years). We found that factors such as age of strabismus onset, type of deviation, and amblyopia had a statistically significant influence on the postoperative surgical outcome. A statistically insignificant relationship was noted with gender, refractive error, and duration of strabismus. Patients who had strabismus after six months of age noticed a significant improvement in stereopsis (p-value = 0.000) than those who had strabismus before six months of age (p-value = 0.660). Further, there was a statistically significant improvement in patients having exotropia (p-value = 0.018) or combined horizontal and vertical deviations (p-value = 0.000), but there was no significant improvement in patients with esotropia (p-value = 0.180). Moreover, non-amblyopes had a significantly better postoperative stereopsis than amblyopes (p-value = 0.006). Although there was no association between preoperative deviation and improvement in stereopsis (p-value = 0.081), patients having preoperative deviation in the range of 31-45 PD had a statistically significant improvement in stereopsis (p-value = 0.000). There was no significant difference between postoperative residual deviation and final stereopsis (p-value > 0.05). All the results were the same for both the Titmus test and the random dot test. Binocular fusion was observed in 34 subjects, and uniocular suppression was noted in 38 subjects preoperatively. It was observed that only one patient gained binocular single vision postoperatively. Conclusion: The presence of amblyopia, esotropia, early onset of strabismus (within six months of age), and a larger preoperative deviation (>45 PD) were associated with poorer stereopsis. In patients with horizontal strabismus, the coexistence of vertical deviation had a positive impact on the postoperative stereopsis. Gender, refractive error, and duration of strabismus did not influence the final stereopsis in our study.

Abstract:Objective To observe the eye correction,the binocular vision rebuilding after surgery in the adult concomitant strabismus. Methods 78 patients(aged 18 to 54 years old)with a diagnosis of strabismus conform to the inclusion and exclusion standard were selected in our hospital from October 2013 to May 2015.The eye position and the stereopsis were observed and analyzed half year after surgery. ResuIts All patients was satisfactory with their eye position,59 cases(85.94%)were cured,19 patients were improved and were satisfied with their appearance.At six months after operation,there was a great improvement in 47 patients(60.26%)getting stereopsis and synoptophore in this group. ConcIusion The eye correction and the cosmetic effect was satisfactory,the binocular vision has been improved in different degrees postoperatively for adult concomitant strabismus after surgery .
Factors Influencing Stereopsis Outcomes in Adults Following Strabismus Surgery

Abstract:Objectives: The aim of the study was to evaluate binocular vision after adult strabismus surgery and to investigate the predictive factors on improvement stereoacuity. Methods: Patients aged upper from 16 years who underwent strabismus surgery in our hospital reviewed retrospectively. Age, existence of amblyopia, pre-operative and postoperatively fusion ability, stereoacuity, and deviation angle were recorded. Patients were divided into two groups according to final stereoacuity; 200 sn/arc and lower: Good stereopsis (Group 1), upper 200 sn/arc: Poor stereopsis (Group 2). Characteristics were compared between groups. Results: A total of 49 patients, who were 16-56 years of age, were included in the study. The mean follow-up time was 37.8 months (range 12-72 months). Of patients, 26 had improvement in stereopsis scores after surgery (53.0%). Group 1 includes 200 sn/arc and lower (n=18, 36.7%) and Group 2 includes higher than 200 sn/arc (n=31, 63.3%). The presence of amblyopia and higher refraction error was frequent significantly in Group 2 (p=0.01 and p=0.02, respectively). The existence of fusion postoperatively was significantly frequent in Group 1 (p=0.02). Type of strabismus and the amount of deviation angle were not found in a relationship with good stereopsis. Conclusion: In adults, surgical correction of horizontal deviation improves stereoacuity. Having no amblyopia, having fusion after surgery, and low refraction error are predictive for the improvement in stereoacuity.
Clinical Study and Discussion of the Monocular Fixation Field Before and after Concomitant Horizontal Strabismus Surgery

Abstract:Objective To study eye movement and change in binocular balance and the characteristics of this change after stra- bismus surgery; to discuss the relationship between the patterns of strabismus surgery and the amount of muscle involved in the opera- tion. This study included the fixation fields of both normal persons and patients with horizontal strabismus and research on the fixation field before and after strabismus surgery. To identify factors that may improve ocular motility after surgery. Methods Two groups of 20 subjects each with a standard BCVA of ≥4.8 were included in the study. Group A was the control group, with an average age of 24(15~45) years, no heterotropia and normal ocular motility. Group B was the experimental group , with an average age of 19(10~ 42) years, with horizontal herterotropia. The monocular fixation field was examined in each group using an arc campimeter and Humphrey campimeter. The subjects in group B were examined 6 weeks preoperatively and 6 weeks postoperatively. SPSS 11.5 soft- ware was used for statistical analysis. Results ①Monocular field fixation: The area of the monocular field fixation was widest in the lateral-inferior area in both groups. There was no distinct difference in preoperative monocular fixation field between groups A and B. ②The relationship between the pattern used in the operation and alteration of the monocular fixation field: For concomitant horizontal strabismus patients, there was a routine recession of a single rectus in 7 eyes, recession with antagonist muscle resection in 16 eyes, and no prominent difference was found in their monocular fixation fields before and after the operation. In the group that had reces- sion with antagonist muscle resection, in the 6 weeks after the op- eration, 4 of 16 eyes showed a reduction in the fixation field using an arc campimeter. This outcome was confirmed with a Humphrey campimeter. The reduction of the fixation field in 4 eyes showed a prominent difference, but no prominent difference was found after 6 weeks of postoperative follow -up. ③ The relationship between the size of the area affected by the operation and change in the monocular fixation field: In group B, with routine surgery, there was no significant difference in the fixation field between pre- and postoperation; for 2 subjects who underwent an operation that was more complex than routine, the fixation field decreased in a lateral direction compared to preoperation, and lateral movement was re- stricted. Conclusion Examination of the monocular fixation field before and after a strabismus operation can objectively reveal the function of eye muscles and ocular motility quantitatively. The changes in monocular fixation field may be correlated with the pat- tern used in strabismus surgery and the extent that the surgery in- volves the muscles.

Abstract:ABSTRACT In vitro interactions of tacrolimus, a calcineurin inhibitor, and azoles, including itraconazole, voriconazole, and posaconazole, against planktonic cells and biofilms of Exophiala dermatitidis were assessed via a broth microdilution checkerboard technique. A total of 16 clinical isolates were studied. The results revealed favorable synergistic inhibitory activity between tacrolimus and itraconazole, voriconazole, or posaconazole against 68.8%, 87.5%, and 100% of tested strains of planktonic E. dermatitidis, respectively.However, limited synergism was observed against biofilms of E. dermatitidis. No antagonism was observed in all combinations.
Experience with Strabologic Surgery on Pediatric Patients with Divergence Excess Exotropia

Abstract:Aim: This retrospective clinical study evaluates the results of surgical treatment of patients diagnosed with intermittent exotropia of the divergence excess type. The study compares the results of surgery delayed due to the Covid-19 pandemic (patients underwent the surgery after the age of eight), versus a group of younger children. Materials and methods: The objective angle of deviation and quality of binocular functions were examined. The follow-up period was six months after surgery. The patients were divided into two groups according to age: 4-7 years and 8-13 years. Each group included 20 patients. The second group comprised patients whose surgery was postponed due to restrictions on elective surgery during the ongoing pandemic. The surgery took place at the University Hospital in Pilsen. Angle of deviation and binocular functions were examined before surgery, postoperatively, and three and six months after surgery. Data were collected retrospectively and statistically processed. The main values were plotted in charts. Results: Accordance between the angle of deviation before surgery in both groups was statistically demonstrated. During the six-month follow-up period, the median angle of deviation was statistically without proof of disparity between the groups. Six months after surgery, an objective angle of deviation within the limit ±5 degrees was achieved in 65% of patients from the first group and in 75% from the second group. Binocular vision before surgery was statistically without proof of difference between both groups. However the statistical processing demonstrated a difference between the two groups six months after the surgery. Better binocular functions were achieved by the younger children in the first group. Conclusion: The objective deviation angle was comparable in both study groups prior to surgery as well as six months after the surgery. The first group of younger patients attained a higher quality of binocular functions within the six-month follow-up period compared to children operated on after the age of eight. This claim was statistically verified.
Strabismus Surgery of Consecutive Exotropia

Abstract:Purpose: The purpose of this study was to determine the surgical outcome, dose-effect (DE), and degree of binocularity in patients undergoing surgery for consecutive exotropia following initial surgery of esotropia. Patients/methods: Twenty-one patients were identified. We analyzed the mean angle of deviation pre- and postoperatively as measured with the alternate prism cover test, DE, and binocularity. Results: All patients had had previous strabismus surgery. The surgery for consecutive exotropia had been performed at a mean age of 35.92 ± 18.26 years. In 19 of these patients, surgery of consecutive exotropia involved at least one previously operated extraocular muscle, and the mean interval to the previous surgery was 25.67 ± 16.14 years. The mean angle of deviation (DE) at distance and in the primary position was - 33.43 ± 12.75 prism diopters (PD) preoperatively, + 0.76 ± 7.91 PD 1 week after surgery, and - 7.24 ± 12.14 PD 3 months after surgery. The mean DE was 3.58 ± 1.53 mm/PD at 1 week and 2.70 ± 1.78 mm/PD at 3 months post-surgery. Postoperatively, 62% patients had a binocularity of at least Bagolini positive, 33% had either a positive TNO or Titmus Test, and 24% were Lang I positive (550″). Conclusion: Performing strabismus surgery with consecutive exotropia results in restoration of some binocularity in a large number of patients, even in adults, and should be considered as a possibility. The dose-effect is comparable to conventional surgery of exotropia.
Clinical study on preoperative and postoperative binocular visual function of children with intermittent exotropia

Abstract:Objective To investigate the recovery of postoperative binocular visual function of children with intermittent exotropia.Methods A total of 168 children with intermittent exotropia were taken corrective procedures to correct strabismus.Synoptophore was used to detect visual function at I stage and at Ⅱ stage.Titmus stereogram was used to detect visual function at Ⅲ stage.All data were statistically analyzed.Results In 168 children with intermittent exotropia,There were 82 children with visual function at I stage preoperatively,and 130 children postoperatively,there was statistical difference(P<0.05).There were 54 children with visual function at Ⅱstage preoperatively,and 130 children postoperatively,there was statistical difference(P<0.05).Titmus stereogram showed that there were 19 cases with distant stereopsis preoperatively and 71 children postoperatively,82 children with near stereopsis preoperatively and 136 children postoperatively,there were statistical differences(P<0.05,P<0.01).Conclusion Operation is good for reconstruction of binocular visual function for children with intermittent exotropia.
Short-Term Near Stereoacuity Improvements Following Favorable Surgical Alignment in Exotropic and Esotropic Patients.

Abstract:Purpose Surgical treatment for strabismus is to promote binocular vision, and the study is to evaluate the effect of surgical realignment on near stereopsis in exotropic and esotropic patients. Methods The records of patients who underwent strabismus surgery were retrospectively reviewed. Visual acuity, ocular deviations, fusion control, ocular motility, strabismus subtype and duration, surgery procedures, and stereopsis at before and at one month after surgery were collected. RMANOVA was performed to analyze stereopsis improvement by surgery. A logistic analysis was used to investigate the factors for stereopsis improvement. Results 143 exotropic and 40 esotropic patients were enrolled. The stereopsis was not significantly improved in patients with exotropia (p > .05), but not in esotropic patients (p < .01). Small degree of near deviation (beta = 0.01), without vertical surgery (beta = 0.11), and the high pre-surgery Titmus (beta = -0.44) were related factors for stereopsis improvement in exotropia patients; male (beta = 0.21) and the high pre-surgery Titmus (beta = -0.36) were related factors for stereopsis improvement in esotropia patients. Conclusion The loss of stereopsis was significantly restored by surgery in esotropia patients and the extent of stereopsis recovery by surgery depends on the deviation at near distance, vertical surgery, and pre-surgery stereopsis in exotropes and on sex and pre-surgery Titmus in esotropes.
[The surgical management of strabismus following orbitotomy]

Abstract:Objective: To analyze the clinical features and therapeutic outcome in patients with strabismus due to orbitotomy. Methods: Retrospective case series. Nine cases of strabismus following orbital operation seen between March 1998 and July 2003 in Zhongshan Ophthalmic Center, Sun Yat-sen University were reviewed retrospectively. Results: All of the 9 cases were treated by strabismus surgery. Among them, 5 cases were male and 4 were female, with their age ranging from 12 to 40 years (average 26.4 years). The right eye was involved in 4 cases and the left one in 5 cases. Most of them belonged to restrictive strabismus secondary to the formation of surgical scar, accounting for 66.7% of them. The methods of surgery included the excision of fibrous tissue around the involved extraocular muscle, the routine recession and resection and the Jensen procedure, combined with the fixation suture. Six cases obtained excellent results. Two cases were slightly undercorrected and one case was remarkably undercorrected. Of the 4 cases with diplopia before operation, the diplopia disappeared in 2 cases after surgery. The other 2 cases had diplopia only in one direction far away from the primary position. All of the ocular movement improved obviously after strabismus surgery. Conclusions: Most of the deviation happened after orbitotomy was restrictive strabismus caused by the muscle fibrosis and scar formation around the muscle. Excision of fibrotic tissue, combined with the routine recession and resection, Jensen procedure, and the fixation suture can have quite satisfactory results.
Binocular vision training after intermittent exotropia surgery

Abstract:·AIM:To observe the clinical effect of binocular vision train-ing for patients with intermittent exotropia after surgery.·METHODS:Totally 121 patients with intermittent exotropia were divided into training group and control group randomly.After surgery,patients in training group were treated by using binocular vision training software,no training for patients in control group.Binocular vision function was examined before surgery,1 week and 12 months after surgery.·RESULTS:Twelve months after surgery,recovery rate of binocular vision of patients in training group was higher than that of patients in control group.Recession rate of eye position of patients in training group was lower than that of patients in control group.·CONCLUSION:Binocular vision training for patients with intermittent exotropia after surgery is helpful for recovery of binocular vision.Reconstruction of binocular vision is useful in keeping eye position.·

Binocular photophobia after surgical treatment in intermittent exotropia

Abstract:Purpose: This study examined the rate of binocular photophobia in intermittent exotropia patients before surgical correction, and the factors affecting this rate. The study also investigated the factors affecting postsurgical disappearance of binocular photophobia. Methods: The study involved 162 basic type intermittent exotropia patients who underwent surgical correction between March 2001 and January 2006. The study did not include patients with entropion, congenital cataract, or retinal abnormalities. Parents of the patients were questioned to determine which patients had monocular eye closure in bright light and which had disappeared after bilateral lateral rectus recession. Data was analyzed to determine the factors affecting monocular eye closure before surgery and disappearance after surgery. Results: Of 162 patients, 96 patients had binocular photophobia (60.2%). Photophobia was found to be associated with an angle of exodeviation at distance >25 Prism Diopter (Delta) (p = 0.02), and stereoacuity worse than 60 s (p = 0.02). We defined satisfactory surgical outcome as a deviation of <10 Delta. Of the 81 patients with satisfactory outcomes, 46 showed disappearance of binocular photophobia (53.5%). No factor was found to be associated with loss of binocular photophobia. Conclusion: The angle of strabismus and stereoacuity were found to affect the occurrence of binocular photophobia. Successful surgery treated binocular photophobia in 53.5% of patients. Regardless of the surgical result, clinicians should educate patients in terms of the possibility of persistence of postsurgical photophobia.

Abstract:AimsTo establish the impact of adult strabismus surgery on clinical and psychosocial well-being and determine who experiences the greatest benefit from surgery and how one could intervene to improve quality of life post-surgery.MethodsA longitudinal study, with measurements taken pre-surgery and at 3 and 6 months post-surgery. All participants completed the AS-20 a disease specific quality of life scale, along with measures of mood, strabismus and appearance-related beliefs and cognitions and perceived social support. Participants also underwent a full orthoptic assessment at their preoperative visit and again 3 months postoperatively. Clinical outcomes of surgery were classified as success, partial success or failure, using the largest angle of deviation, diplopia and requirement for further therapy.Results210 participants took part in the study. Strabismus surgery led to statistically significant improvements in psychosocial and functional quality of life. Those whose surgery was deemed a partial success did however experience a deterioration in quality of life. A combination of clinical variables, high expectations, and negative beliefs about the illness and appearance pre-surgery were significant predictors of change in quality of life from pre- to post-surgery.ConclusionsStrabismus surgery leads to significant improvements in quality of life up to 6 months postoperatively. There are however a group of patients who do not experience these benefits. A series of clinical and psychosocial factors have now been identified, which will enable clinicians to identify patients who may be vulnerable to poorer outcomes post-surgery and allow for the development of interventions to improve quality of life after surgery.

Predictive factors of stereopsis outcomes following strabismus surgery

Abstract:Purpose: To determine the predictive factors of post-operative stereopsis in patients with strabismus. Method: In this retrospective study, records of patients who received surgical treatment for strabismus were reviewed. All types of strabismus were included. Pre- and post-treatment stereoacuity were measured using the Titmus Stereo Fly test, and predictive factors of stereopsis were evaluated. Results: A total of 194 patients (132 females and 62 males) with a mean age of 14.8 ± 8.4 years were included. There was a statistically significant improvement in stereopsis following surgery (p value < 0.001). Patients with a higher amount of deviation at baseline had poorer stereopsis on the final examination (p value < 0.001). Stereopsis improvement was more prominent in the pure horizontal strabismus group, compared to combined horizontal and vertical deviations. Baseline and the final stereopsis were higher in the "exotropia" group as compared to the "esotropia" group (p value = 0.003 and 0.0155, respectively); however, the within group change of stereopsis was not significantly different between these two groups (p value = 0.144). Post surgical residual deviation was associated with a poorer stereopsis (p value = 0.002, r = 0.251). A longer duration of strabismus before surgery was associated with poorer final stereopsis levels (p value = 0.026). The presence of amblyopia before surgery was associated with poorer stereopsis on last examination (p value < 0.001 for both correlations). Conclusion: Based on the result of this study, final stereopsis after strabismus surgery could be affected by the type, duration, and the amount of deviation before surgery, amblyopia, and post operative ocular deviation.
Binocular Summation With Quantitative Contrast Sensitivity Function: A Novel Parameter to Evaluate Binocular Function in Intermittent Exotropia

Abstract:Purpose: Intermittent exotropia (IXT) is the most common form of strabismus. Surgery can potentially improve binocular function in patients with IXT. We aimed to evaluate binocular function using a novel parameter-binocular summation ratio (BSR), measured using quantitative contrast sensitivity function (CSF) in patients with IXT before and after surgery.Methods: Prospective study of 63 patients with IXT and 41 healthy controls were consecutively enrolled and underwent quantitative CSF testing binocularly and monocularly. BSR was calculated by dividing the CSF of the binocular value by the better monocular value. Forty-eight patients with IXT underwent strabismus surgery. BSR, stereoacuity, fusion ability, and strabismus questionnaires were assessed pre-operatively and 2 months postoperatively.Results: Sixty-three patients with IXT (median age = 9 years) compared with 41 healthy controls showed a worse mean BSR based on all CSF metrics at baseline (the area under the log CSF [AULCSF], spatial frequency [SF] cutoff, and contrast sensitivity at 1.0-18.0 cpd SF). All 48 patients with IXT showed successful alignment after surgery, and there were significant improvements in BSR based on the AULCSF, SF cutoff, and contrast sensitivity at 6.0, 12.0, and 18.0 cpd SF, respectively. The distance stereoacuity and fusion ability also improved after surgery, and a better BSR was associated with better stereoacuity and fusion. For strabismus questionnaires, the psychosocial subscale scores improved postoperatively, whereas the functional subscale scores did not change.Conclusions: BSR based on quantitative CSF can characterize binocular function across a range of spatial frequencies and can be used as a supplemental measurement for monitoring binocularity in patients with IXT in clinical settings.

A Prospective Study of One-Muscle Surgery in 15-25 Prism Diopters Horizontal Comitant Strabismus in Adults

Abstract:Purpose: To prospectively evaluate the results of one-muscle surgery in 15-25 prism diopters (PD) horizontal comitant strabismus in adults. Patients and methods: 15-25 PD horizontal strabismus patients, comprising 25 exotropic (XT) patients and 11 esotropic (ET) patients, who underwent one-muscle recession by a single surgeon with a fixed surgical dosage were included in the study with a minimum follow-up of 3 months. The main outcome was a surgical success, which is defined as a residual deviation of <8 PD at the last examination. Other parameters include postoperative lateral incomitance and patient satisfaction. Postoperative lateral incomitance indicated a difference in deviation of >5 PD between the primary position and lateral gaze, or duction limitation at any visit. Patient satisfaction regarding cosmetic and functional outcomes was assessed by using a 5-point Likert scale. Results: Successful alignment was obtained in 19 (76%) and 9 (81.81%) XT and ET patients (p = 1.00), with a median and interquartile range (IQR) of follow-up time of 3, 3-3 months and 3, 3-7 months, respectively. Postoperative lateral incomitance was observed in 8 (32%) XT patients and in 1 (9.09%) ET patient (p = 0.22). One XT patient and one ET patient who developed incomitance reported diplopia, which disappeared 3 months after surgery. One XT patient reported diplopia while gazing towards the operated eye whilst one ET patient reported diplopia occasionally which was unrelated to gaze direction. Patient satisfaction was based on Likert scale scores with 4.12 ± 0.97 in XT patients and 4.73 ± 0.65 in ET patients (p = 0.07). Conclusion: One-muscle recession with a fixed surgical dosage was effective for treating small angle horizontal strabismus with a high rate of surgical success and patient satisfaction. Although postoperative lateral incomitance occurred, only a few patients developed symptomatic diplopia, which completely resolved 3 months after surgery.

Abstract:BACKGROUND/AIMS: To evaluate the role of achieving immediate target angles on the success of strabismus surgery.METHODS: We performed a retrospective cohort study of all patients over 12 years old undergoing strabismus surgery with either adjustable or non-adjustable sutures. Target angle for patients with esotropia and vertical deviation was within 4 prism dioptres (PD) of orthotropia and for patients with exotropia between orthotropia and 8 PD of esotropia. Success was defined as alignment within 10 PD for horizontal rectus surgery and within 5 PD of orthotropia for vertical rectus surgery, without diplopia or reoperations. The main outcome measure was surgical success rate.RESULTS: Three hundred and fifty-three patients were included in the study with mean follow-up of 13.9 (4-132) months. Patients achieving the target angle immediately postoperatively had higher success rate than patients who did not (83.6% vs 63.7%, p<0.0001, OR 2.9, 95% CI 1.8 to 4.9). When target angle was achieved, adjustable and non-adjustable sutures had similar success (84.8% and 80.9%, respectively, p=0.46, OR 1.3, CI 0.58 to 2.9). However, patients undergoing adjustable surgery were more likely to obtain the target angle (75.5% vs 54%, p<0.0001, OR 2.7, CI 1.7 to 4.2). Success for exotropia surgery was significantly higher when the immediate target angle was achieved (86.4% vs 58.7%, p<0.0001, OR 4.47, CI 2.3 to 8.6). For esotropia and vertical deviations, a similar beneficial effect was not shown (p=0.31, OR 1.6, CI 0.65 to 4.0 and p=0.33, respectively). On multiple logistic regressions, sex, amblyopia, binocularity and reoperations were not significant factors in surgical success.CONCLUSIONS: Our results suggest that achieving the immediate target angle is the most significant factor in the success of strabismus surgery for exotropia. Adjustable suture surgery results in higher proportion of patients achieving this target angle.
