Abstract:Identifying arrhythmias from electrocardiogram(ECG) signals remains an intractable challenge. This study aims to develop an effective and non-invasive approach to realize the recognition of arrhythmias based on 12-lead ECG for the PhysioNet/Computing in Cardiology Challenge2020. To this end, we propose a deep learning-based diagnosis approach, called EASTNet which captures the characteristics of cardiac abnormalities and correlation between heartbeats sampled randomly from 12-lead ECG records by a 34-layer 1D-deep squeeze-and-excitation network. Experimenting in the multi-label arrhythmia classification task, our team, EASTBLUE, was unable to rank and score in the hidden validation and test sets, but achieved diagnostic performance with 0.7030 ± 0.0090 metric score using 5-fold cross-validation on the training set. We also investigate the effect of beat sampling on diagnostic performance, and find that the beat sampling plays a role in data augmentation that effectively alleviates network overfitting. These results demonstrate that our approach has good potential application prospects in clinical practice, especially in the auxiliary diagnosis of abnormalities.

Abstract:There are three primary challenges in the automatic diagnosis of arrhythmias by electrocardiogram (ECG): the significant variation among individual patients, the multiple pathologies in the ECG signal and the high cost in annotating clinical ECG with the corresponding labels. Traditional ECG processing approaches rely heavily on prior knowledge, such as those from feature extraction and waveform analysis. The preprocessing for prior knowledge incurs computational overhead. Furthermore, standard deep learning methods do not fully consider the dynamic temporal, spatial and multi-labeling characteristics of ECG data. In clinical ECG waveforms, it is common to see multi-labeling in which a patient is labeled with multiple classes of arrhythmias. However, multiclass approaches in current research mainly solve the multi-label machine learning problem, ignoring the correlation between diseases, resulting in information loss. In this paper, an arrhythmia detection and classification scheme called multi-label fusion deep learning is proposed. The objective is to build a unified system with automatic feature learning which supports effective multi-label classification. First, a multi-label ECG-based feature selection method is combined with a matrix decomposition and sparse learning theory. The optimal feature subset is selected as a preprocessing algorithm for ECG data. A multi-label classifier is then constructed by fusing CNN and RNN networks to fully exploit the interactions and features of the time and space dimensions. The experimental result demonstrates that the proposed method can achieve a state-of-the-art performance compared to other algorithms in multi-label database experiments.

Abstract:Cardiovascular disease (CVD) is one of the most serious diseases that harm human life and gives a huge burden to the health care system. Recent advances in deep learning have achieved great success in object detection, speech and image recognition. Although deep learning has been applied to the detection of arrhythmia, detection accuracy is limited because of three major issues: 1. Each ECG signal maybe contains more than one-label information; 2. It is hard to classify ECG with different lengths; 3. Data imbalance problem is severe for arrhythmia detection. In this paper, we present a multi-label learning algorithm to address the class imbalance and detection on ECGs with different durations. We utilize Deep Convolutional Generative Adversarial Networks (DCGANs) and Wasserstein GAN-Gradient Penalty (WGAN-GP) to generate new positive samples and use two losses to balance the importance between positive samples and negative samples. Moreover, we construct a Squeeze and Excitation-ResNet (SE-ResNet) module for normal rhythm and arrhythmia detection. In order to solve the multi-label classification problem, we train nine different binary classifiers for each category and determine which types of rhythm the ECG signals belong to. Experimental results on The ECG Intelligence Challenge 2019 dataset demonstrate that our multi-label learning method achieves competitive performance in multi-label ECGs classification.

Abstract:Objective: Arrhythmia detection and classification are challenging because of the imbalanced ratio of normal heartbeats to arrhythmia heartbeats and the complicated combinations of arrhythmia types. Arrhythmia classification on wearable electrocardiogram monitoring devices poses a further unique challenge: unlike clinically used electrocardiogram monitoring devices, the environments in which wearable devices are deployed are drastically different from the carefully controlled clinical environment, leading to significantly more noise, thus making arrhythmia classification more difficult. Methods: We propose a novel hierarchical model based on CNN+BiLSTM with Attention to arrhythmia detection, consisting of a binary classification module between normal and arrhythmia heartbeats and a multi-label classification module for classifying arrhythmia events across combinations of beat and rhythm arrhythmia types. We evaluate our method on our proprietary dataset and compare it with various baselines, including CNN+BiGRU with Attention, ConViT, EfficientNet, and ResNet, as well as previous state-of-the-art frameworks. Results: Our model outperforms existing baselines on the proprietary dataset, resulting in an average accuracy, F1-score, and AUC score of 95%, 0.838, 0.906 for binary classification, and 88%, 0.736, 0.875 for multi-label classification. Conclusions: Our results validate the ability of our model to detect and classify real-world arrhythmia. Our framework could revolutionize arrhythmia diagnosis by reducing the burden on cardiologists, providing more personalized treatment, and achieving emergency intervention of patients by allowing real-time monitoring of arrhythmia occurrence.
Integrated Approach for ECG Signal Classification Using Fused Slantlet Transform and Autoregressive Features

Abstract:Sudden cardiac death remains a significant concern, emphasizing the critical need for intelligent diagnostic techniques in cardiac disease detection. Cardiologic Vascular Diseases (CVD) pose a significant global health challenge, often associated with unhealthy lifestyle habits. Electrocardiogram (ECG) monitoring plays a crucial role in assessing heart function, but its interpretation can be challenging due to signal complexity. This study proposes a novel approach utilizing fused Slantlet-Spatial features alongside Auto Regression (AR) coefficients to classify electrocardiogram (ECG) signals. Leveraging the Slantlet Transform (SLT) for its superior time localization capabilities and the simplicity of AR coefficients, this methodology aims to enhance the accuracy of cardiac illness classification. Nineteen distinct features were extracted from 300 ECG signals, each comprising 512 samples, enriching the classification scheme's efficacy. The fused feature extraction technique demonstrated success in improving overall accuracy. Employing Binary Particle Swarm Optimization (BPSO), irrelevant features were pruned, resulting in a refined feature vector of only 14 relevant features. This reduction in feature dimensionality significantly influenced the classification accuracy. Subsequently, the refined feature vector was introduced to the classification stage, employing four distinct classifiers: Support Vector Machine (SVM) with various kernel functions, K-Nearest Neighbors (K-NN), Naïve Bayes (NB), and Decision Tree (DT). Simulation results revealed the feasibility of achieving an average classification accuracy of 99.67% across ECG signal categories. Notably, the polynomial kernel function-SVM classifier, with only 2-fold cross-validation, yielded optimal performance. This study emphasizes the potential of fused Slantlet-Spatial and AR features in enhancing the accuracy of cardiac disease classification. Moreover, the efficacy of BPSO in feature selection and the superior performance of the SVM classifier highlight promising avenues for further research in intelligent cardiac diagnosis systems. The proposed approach outperforms existing methods and contributes to the early detection of cardiovascular abnormalities, potentially improving patient outcomes.

Abstract:Arrhythmia automatic analysis techniques provide convenience for the prevention and diagnosis of cardiac disease. Aiming at classify cardiac abnormalities into 27 classes with either 12-lead, 6-lead, 4-lead, 3-lead or 2-lead multi-label ECG recordings, we develop a deep 1-Dimensional Convolutional Neural Network (1D CNN) with residual block and squeeze-and-excitation (SE) attention mechanism (namely 1D RANet). First, we introduce residual block and SE attention block into 1D CNN to extract deep features adaptively and avoid vanishing gradient or network degradation. Second, to improve the robustness, we use data augmentation techniques such as band-pass and wavelet-based filter, noise addition, time-frequency switch and so on. For the convenience of training, all recordings are cropped or padded to 60 seconds with resampled rates of 300 Hz. Our classifiers received challenge scores of 0.73, 0.53, 0.59, 0.57 and 0.47 for the 12-lead, 6-lead, 4-lead, 3-lead, and 2-lead versions of the public training set, 0.56, 0.54, 0.56, 0.54 and 0.55 for the hidden validation set and 0.52, 0.49, 0.51, 0.50 and 0.49 for the hidden test sets (Team name: Proton). However, our entries did not rank since we missed the preprint submitting deadline of the Computing in Cardiology Conference.
Multi-Label ECG Classification using Temporal Convolutional Neural Network

Abstract:Automated analysis of 12-lead electrocardiogram (ECG) plays a crucial role in the early screening and management of cardiovascular diseases (CVDs). In practice, it is common to see multiple co-occurring cardiac disorders, i.e., multi-label or multimorbidity in patients with CVDs, which increases the risk for mortality. Most current research focuses on the single-label ECG classification, i.e., each ECG record corresponds to one cardiac disorder, ignoring ECG records with multi-label phenomenon. In this paper, we propose an ensemble of attention-based temporal convolutional neural network (ATCNN) models for the multi-label classification of 12-lead ECG records. Specifically, a set of ATCNN-based single-lead binary classifiers are trained one for each cardiac disorder, and the predictions from these classifiers with simple thresholding generate the final multi-label decisions. The ATCNN contains a stack of TCNN layers followed by temporal and spatial attention layers. The TCNN layers operate at different dilation rates to represent the multi-scaled pathological ECG features dynamics, and attention layers emphasize/reduce the diagnostically relevant/redundant 12-lead ECG information. The proposed framework is evaluated on the PTBXL-2020 dataset and achieved an average F1-score of 76.51%. Moreover, the spatial and temporal attention weights visualization provides the optimal ECG-lead subset selection for each disease and model interpretability results, respectively. The improved performance and interpretability of the proposed approach demonstrate its ability to screen multimorbidity patients and help clinicians initiate timely treatment.

Abstract:Since ECG contains key characteristic information of arrhythmias, extracting this information is crucial for identifying arrhythmias. Based on this, in order to effectively extract ECG data features and realize automatic detection of arrhythmia, a multi-classification method of arrhythmia based on multi-scale residual neural network and multi-channel data fusion is proposed. First, the features of single-lead ECG signals are extracted and converted into two-dimensional images, and the feature data sets are labeled and divided according to different types of arrhythmias. The improved residual neural network is trained on the training set to obtain the classification model of the neural network. Finally, the classification model is applied to the automatic detection of arrhythmias during exercise. The accuracy of the classification model of this method is as high as 99.60%, and it has high accuracy and generalization ability. The automatic identification of arrhythmia also contributes to the research and development of future wearable devices.

rECGnition_v1.0: Arrhythmia detection using cardiologist-inspired multi-modal architecture incorporating demographic attributes in ECG

Abstract:A substantial amount of variability in ECG manifested due to patient characteristics hinders the adoption of automated analysis algorithms in clinical practice. None of the ECG annotators developed till date consider the characteristics of the patients in a multi-modal architecture. We employed the XGBoost model to analyze the UCI Arrhythmia dataset, linking patient characteristics to ECG morphological changes. The model accurately classified patient gender using discriminative ECG features with 87.75% confidence. We propose a novel multi-modal methodology for ECG analysis and arrhythmia classification that can help defy the variability in ECG related to patient-specific conditions. This deep learning algorithm, named rECGnition_v1.0 (robust ECG abnormality detection Version 1), fuses Beat Morphology with Patient Characteristics to create a discriminative feature map that understands the internal correlation between both modalities. A Squeeze and Excitation based Patient characteristic Encoding Network (SEPcEnet) has been introduced, considering the patient's demographics. The trained model outperformed the various existing algorithms by achieving the overall F1-score of 0.986 for the ten arrhythmia class classification in the MITDB and achieved near perfect prediction scores of ~0.99 for LBBB, RBBB, Premature ventricular contraction beat, Atrial premature beat and Paced beat. Subsequently, the methodology was validated across INCARTDB, EDB and different class groups of MITDB using transfer learning. The generalizability test provided F1-scores of 0.980, 0.946, 0.977, and 0.980 for INCARTDB, EDB, MITDB AAMI, and MITDB Normal vs. Abnormal Classification, respectively. Therefore, with a more enhanced and comprehensive understanding of the patient being examined and their ECG for diverse CVD manifestations, the proposed rECGnition_v1.0 algorithm paves the way for its deployment in clinics.

Abstract:BackgroundElectrocardiogram automated arrhythmia detection plays a crucial role in the early prevention and diagnosis of cardiovascular diseases. However, previous research relies on noise removal algorithms and extracting solid features from raw ECGs. Besides, existing heartbeat classifiers ignore underlying complementary information of various scales, and intra-patient paradigms often lead to biased results.MethodsWe constructed a novel end-to-end Multi-Scale Convolutional Neural Network-Sequence to Sequence architecture for heartbeat classification to address these issues. We have verified this approach on the clinical data collected by wearable devices and two heterogeneous datasets.ResultsThe proposed model can effectively capture multi-granularity frequency and longitudinal temporal information by fusion representation and sequence learning. The overall F1 score of our approach was achieved at 99.57%, which exceeded the reference pure cascade model by 4.36%.

Abstract:The 12-lead electrocardiogram (ECG) is widely used for diagnosing cardiovascular diseases in clinical practice. Recently, deep learning methods have become increasingly effective for automatically classifying ECG signals. However, most current research simply combines the 12-lead ECG signals into a matrix without fully considering the intrinsic relationships between the leads and the heart's structure. To better utilize medical domain knowledge, we propose a multi-branch network for multi-label ECG classification and introduce an intuitive and effective lead grouping strategy. Correspondingly, we design multi-branch networks where each branch employs a multi-scale convolutional network structure to extract more comprehensive features, with each branch corresponding to a lead combination. To better integrate features from different leads, we propose a feature weighting fusion module. We evaluate our method on the PTB-XL dataset for classifying 4 arrhythmia types and normal rhythm, and on the China Physiological Signal Challenge 2018 (CPSC2018) database for classifying 8 arrhythmia types and normal rhythm. Experimental results on multiple multi-label datasets demonstrate that our proposed multi-branch network outperforms state-of-the-art networks in multi-label classification tasks

Abstract:Recently, classification from compressed physiological signals in compressed sensing has been successfully applied to cardiovascular disease monitoring. However, in real-time wearable electrocardiogram (ECG) monitoring, it is very difficult to directly obtain the heartbeats information from compressed ECG signals. Thus arrhythmia classification from compressed ECG signals has to be handled in fixed-length segments instead of individual heartbeats. An inevitable issue is that a fixed-length ECG segment may contain multiple different types of arrhythmia. As a result, it is not appropriate to represent the multi-type real arrhythmia with a single label. In this paper, we first introduce multiple labels into fixed-length compressed ECG segments to challenge the arrhythmia classification issue. Then, we propose a deep learning model, which can directly classify multiple different types of arrhythmia from fixed-length compressed ECG segments with the advantages of low time cost for data processing and relatively high classification accuracy at a high compression ratio. Experimental results on the MIT-BIH arrhythmia database show that the exact match rate of our proposed method has reached 96.03% at CR(Compression Ratio)=70%, 94.99% at CR=80% and 93.19% at CR=90%.
Automatic Detection of Arrhythmia Based on Multi-Resolution Representation of ECG Signal

Abstract:Automatic detection of arrhythmia is of great significance for early prevention and diagnosis of cardiovascular disease. Traditional feature engineering methods based on expert knowledge lack multidimensional and multi-view information abstraction and data representation ability, so the traditional research on pattern recognition of arrhythmia detection cannot achieve satisfactory results. Recently, with the increase of deep learning technology, automatic feature extraction of ECG data based on deep neural networks has been widely discussed. In order to utilize the complementary strength between different schemes, in this paper, we propose an arrhythmia detection method based on the multi-resolution representation (MRR) of ECG signals. This method utilizes four different up to date deep neural networks as four channel models for ECG vector representations learning. The deep learning based representations, together with hand-crafted features of ECG, forms the MRR, which is the input of the downstream classification strategy. The experimental results of big ECG dataset multi-label classification confirm that the F1 score of the proposed method is 0.9238, which is 1.31%, 0.62%, 1.18% and 0.6% higher than that of each channel model. From the perspective of architecture, this proposed method is highly scalable and can be employed as an example for arrhythmia recognition.

Abstract:Cardiovascular diseases have become more and more prominent in recent years, which have proven to be a major threat to people's health. Accurate detection of arrhythmia in patients has important implications for clinical treatment. The aim of this study was to propose a novel automatic classification method for arrhythmia in order to improve classification accuracy. The electrocardiogram (ECG) signal was subjected preprocessing for denoising purposes using a wavelet transform. Then, the local and global characteristics of the beat, which contained RR interval features according with the clinical diagnosis criterion, morphology features based on wavelet packet decomposition and statistical features along with kurtosis coefficient, skewness coefficient and variance are exploited and fused. Meanwhile, the dimensionality of wavelet packet coefficients were reduced via principal component analysis (PCA). Finally, these features were used as the input of the random forest classifier to train the model and were then compared with the support vector machine (SVM) and back propagation (BP) neural networks. Based on 100,647 beats from the MIT-BIH database, the proposed method achieved an average accuracy, specificity and sensitivity of 99.08%, 99.00% and 89.31%, respectively, using the intra-patient beats, and 92.31%, 89.98% and 37.47%, respectively, using the inter-patient beats. Moreover, two classification schemes, namely, inter-patient and intra-patient scheme, were validated. Compared with the other methods referred to in this paper, the performance of the novel method yielded better results.

Abstract:Cardiovascular diseases are a pervasive global health concern, contributing significantly to morbidity and mortality rates worldwide. Among these conditions, arrhythmia, characterized by irregular heart rhythms, presents formidable diagnostic challenges. This study introduces an innovative approach utilizing deep learning techniques, specifically Convolutional Neural Networks (CNNs), to address the complexities of arrhythmia classification. Leveraging multi-lead Electrocardiogram (ECG) data, our CNN model, comprising six layers with a residual block, demonstrates promising outcomes in identifying five distinct heartbeat types: Left Bundle Branch Block (LBBB), Right Bundle Branch Block (RBBB), Atrial Premature Contraction (APC), Premature Ventricular Contraction (PVC), and Normal Beat. Through rigorous experimentation, we highlight the transformative potential of our methodology in enhancing diagnostic accuracy for cardiovascular arrhythmias. Arrhythmia diagnosis remains a critical challenge in cardiovascular care, often relying on manual interpretation of ECG signals, which can be time-consuming and prone to subjectivity. To address these limitations, we propose a novel approach that leverages deep learning algorithms to automate arrhythmia classification. By employing advanced CNN architectures and multi-lead ECG data, our methodology offers a robust solution for precise and efficient arrhythmia detection. Through comprehensive evaluation, we demonstrate the effectiveness of our approach in facilitating more accurate clinical decision-making, thereby improving patient outcomes in managing cardiovascular arrhythmias.

Abstract:Arrhythmia is a life-threatening cardiac condition characterized by irregular heart rhythm. Early and accurate detection is crucial for effective treatment. However, single-lead electrocardiogram (ECG) methods have limited sensitivity and specificity. This study propose an improved ensemble learning approach for arrhythmia detection using multi-lead ECG data. Proposed method, based on a boosting algorithm, namely Fine Tuned Boosting (FTBO) model detects multiple arrhythmia classes. For the feature extraction, introduce a new technique that utilizes a sliding window with a window size of 5 R-peaks. This study compared it with other models, including bagging and stacking, and assessed the impact of parameter tuning. Rigorous experiments on the MIT-BIH arrhythmia database focused on Premature Ventricular Contraction (PVC), Atrial Premature Contraction (PAC), and Atrial Fibrillation (AF) have been performed. The results showed that the proposed method achieved high sensitivity, specificity, and accuracy for all three classes of arrhythmia. It accurately detected Atrial Fibrillation (AF) with 100% sensitivity and specificity. For Premature Ventricular Contraction (PVC) detection, it achieved 99% sensitivity and specificity in both leads. Similarly, for Atrial Premature Contraction (PAC) detection, proposed method achieved almost 96% sensitivity and specificity in both leads. The proposed method shows great potential for early arrhythmia detection using multi-lead ECG data.

Abstract:The heart disease is one of the most serious health problems in today’s world. Over 50 million persons have cardiovascular diseases around the world. Our proposed work based on 744 segments of ECG signal is obtained from the MIT-BIH Arrhythmia database (strongly imbalanced data) for one lead (modified lead II), from 29 people. In this work, we have used long-duration (10 s) ECG signal segments (13 times less classifications/analysis). The spectral power density was estimated based on Welch’s method and discrete Fourier transform to strengthen the characteristic ECG signal features. Our main contribution is the design of a novel three-layer (48 + 4 + 1) deep genetic ensemble of classifiers (DGEC). Developed method is a hybrid which combines the advantages of: (1) ensemble learning, (2) deep learning, and (3) evolutionary computation. Novel system was developed by the fusion of three normalization types, four Hamming window widths, four classifiers types, stratified tenfold cross-validation, genetic feature (frequency components) selection, layered learning, genetic optimization of classifiers parameters, and new genetic layered training (expert votes selection) to connect classifiers. The developed DGEC system achieved a recognition sensitivity of 94.62% (40 errors/744 classifications), accuracy = 99.37%, specificity = 99.66% with classification time of single sample = 0.8736 (s) in detecting 17 arrhythmia ECG classes. The proposed model can be applied in cloud computing or implemented in mobile devices to evaluate the cardiac health immediately with highest precision.

Abstract:Arrhythmia accounts for more than 80% of sudden cardiac death, and its incidence rate has increased rapidly recently. Nowadays, many studies have applied artificial intelligence (AI) methods to arrhythmia detection. Deep learning approaches can improve model performance but lack the capability to mine the relationship between the electrocardiogram (ECG) samples and final results. Furthermore, most arrhythmia diagnosis methods aim at only multiclassification problems. However, in many cases, an ECG recording may have more than one arrhythmia disease. In this article, we develop an accurate and interpretable model for multilabel ECG signals, which is called dual-level attentional convolutional long short-term memory neural network (DLA-CLSTM). Convolutional layers, bidirectional long short-term memory layers, and attention mechanisms are used for extracting beat- and rhythm-level information. Notably, the attention mechanism helps the model be more interpretable. In addition, we design a weighted multilabel classifier to address multilabel and imbalance problems. Finally, the MIT-BIH Arrhythmia Database and the 1st China Physiological Signal Challenge dataset are utilized to evaluate DLA-CLSTM. Compared with existing methods, DLA-CLSTM can improve the accuracy by 22.50% and the F1-macro by 20.51% on average. Further analysis shows that the areas focused by DLA-CLSTM match the clinical judgment basis to some extent. Collectively, DLA-CLSTM has good potential for assisting clinicians in real applications.

Abstract:The timely detection of abnormal electrocardiogram (ECG) signals is vital for preventing heart disease. However, traditional automated cardiology diagnostic methods have the limitation of being unable to simultaneously identify multiple diseases in a segment of ECG signals, and do not consider the potential correlations between the 12-lead ECG signals. To address these issues, this paper presents a novel network architecture, denoted as Branched Convolution and Channel Fusion Network (BCCF-Net), designed for the multi-label diagnosis of ECG cardiology to achieve simultaneous identification of multiple diseases. Among them, the BCCF-Net incorporates the Channel-wise Recurrent Fusion (CRF) network, which is designed to enhance the ability to explore potential correlation information between 12 leads. Furthermore, the utilization of the squeeze and excitation (SE) attention mechanism maximizes the potential of the convolutional neural network (CNN). In order to efficiently capture complex patterns in space and time across various scales, the multi branch convolution (MBC) module has been developed. Through extensive experiments on two public datasets with seven subtasks, the efficacy and robustness of the proposed ECG multi-label classification framework have been comprehensively evaluated. The results demonstrate the superior performance of the BCCF-Net compared to other state-of-the-art algorithms. The developed framework holds practical application in clinical settings, allowing for the refined diagnosis of cardiac arrhythmias through ECG signal analysis.
