Abstract:Recently, many deep learning methods have been successfully used to diagnose Alzheimer’s disease (AD) using brain imaging. However, structural magnetic resonance imaging (sMRI) of AD detects relatively small lesion areas, and it is difficult to distinguish the early lesions. These factors make it difficult to extract the key features in the subtle regions for discrimination in many studies. To address these issues, an attention-based and micro designed EfficientNetB2 (EB2) approach is proposed in this study to classify AD, mild cognitive impairment (MCI), and normal control (NC). First, the front portion of the EB2 uses a global attention mechanism (GAM) to improve the classification results by capturing significant features in three dimensions. Next, coordination attention (CA) was added to the EB2 model. The CA can automatically extract channel and location information from the sMRI two-dimensional slice data. The location information is critical for constructing spatial attention maps that help the model to identify the lesion areas accurately, thereby helping the model to extract features that are useful for classification. Finally, the model uses the micro design (MD) method of the ConvNeXt network, which investigates the effect of activation function and batch normalization layer on the model at the microscopic scale. MD can reduce the model complexity while improving classification ability considerably. The accuracy of the proposed method is 93.30%, 92.42% and 92.03% for AD/NC, AD/MCI and MCI/NC dichotomous data, respectively. Finally, the proposed method outperforms the existing convolutional neural networks, such as AlexNet, GoogleNet, MobileNetV2, MobileNetV3, and DenseNet.

Abstract:Alzheimer's disease (AD) leads to memory loss and impairment, which may cause further symptoms. It affects lives of patients seriously and is not curable, but early confirmation of AD may be helpful to start proper treatment so as to avoid further brain damage. Over the past decades, machine learning methods have been applied to the classification of AD with results based on manually prepared features and a classifier having a multiple-step architecture. Recently, with the development of deep learning, the end to- end process of neural networks has been employed for pattern classification. In this paper, we focus on early diagnosis of AD based on convolutional neural networks (ConvNets) by using magnetic resonance imaging (MRI). Image slices of gray matter and white matter from MRI have been used as the inputs for classification. Ensemble learning methods have been employed after the convolutional operations for improving the classification by combining outputs of deep learning classifiers [27]. Three base ConvNets were designed, implemented, and compared in this paper. Our method was evaluated based on a dataset from the Alzheimer's Disease Neuroimaging Initiative for the early diagnosis of this illness. In particular, the accuracy rates of our classifications have reached up to 97.65% for AD/mild cognitive impairment and 88.37% for mild cognitive impairment/normal control.
Polar-Net: A Clinical-Friendly Model for Alzheimer's Disease Detection in OCTA Images

Abstract:Optical Coherence Tomography Angiography (OCTA) is a promising tool for detecting Alzheimer's disease (AD) by imaging the retinal microvasculature. Ophthalmologists commonly use region-based analysis, such as the ETDRS grid, to study OCTA image biomarkers and understand the correlation with AD. However, existing studies have used general deep computer vision methods, which present challenges in providing interpretable results and leveraging clinical prior knowledge. To address these challenges, we propose a novel deep-learning framework called Polar-Net. Our approach involves mapping OCTA images from Cartesian coordinates to polar coordinates, which allows for the use of approximate sector convolution and enables the implementation of the ETDRS grid-based regional analysis method commonly used in clinical practice. Furthermore, Polar-Net incorporates clinical prior information of each sector region into the training process, which further enhances its performance. Additionally, our framework adapts to acquire the importance of the corresponding retinal region, which helps researchers and clinicians understand the model's decision-making process in detecting AD and assess its conformity to clinical observations. Through evaluations on private and public datasets, we have demonstrated that Polar-Net outperforms existing state-of-the-art methods and provides more valuable pathological evidence for the association between retinal vascular changes and AD. In addition, we also show that the two innovative modules introduced in our framework have a significant impact on improving overall performance.

Abstract:Alzheimer's Disease (AD) is characterized by the gradual degeneration and decline of brain cells, leading to irreversible neurological changes. This study investigates advanced image enhancement techniques for improving AD diagnosis using brain MRI. The methods used include CLAHE (Contrast Limited Adaptive Histogram Equalization) to enhance local image contrast and ESRGAN (Enhanced Super-Resolution Generative Adversarial Networks) to improve image resolution. These preprocessing methods improve MRI images and classification accuracy. An ensemble model of MobileNetV2 and DenseNet121, two efficient deep-learning models with feature extraction capabilities, were used as classifiers. This approach addresses challenges in AD diagnosis by leveraging deep learning for more accurate classification of brain tissue images. The model achieved an accuracy of 80.31% for MobileNetV2 and 89.22% for DenseNet121 when no enhancements were utilized. The model achieved accuracies of 92.34% and 89.38% for MobileNetV2 and DenseNet121, respectively, when enhancement methods were utilized, indicating strong dependability with the Kaggle dataset of MRI images. These findings underscore the efficacy of advanced image processing and deep learning in the early and accurate detection of Alzheimer's disease.

Beyond the Eye: A Relational Model for Early Dementia Detection Using Retinal OCTA Images

Abstract:Early detection of dementia, such as Alzheimer's disease (AD) or mild cognitive impairment (MCI), is essential to enable timely intervention and potential treatment. Accurate detection of AD/MCI is challenging due to the high complexity, cost, and often invasive nature of current diagnostic techniques, which limit their suitability for large-scale population screening. Given the shared embryological origins and physiological characteristics of the retina and brain, retinal imaging is emerging as a potentially rapid and cost-effective alternative for the identification of individuals with or at high risk of AD. In this paper, we present a novel PolarNet+ that uses retinal optical coherence tomography angiography (OCTA) to discriminate early-onset AD (EOAD) and MCI subjects from controls. Our method first maps OCTA images from Cartesian coordinates to polar coordinates, allowing approximate sub-region calculation to implement the clinician-friendly early treatment of diabetic retinopathy study (ETDRS) grid analysis. We then introduce a multi-view module to serialize and analyze the images along three dimensions for comprehensive, clinically useful information extraction. Finally, we abstract the sequence embedding into a graph, transforming the detection task into a general graph classification problem. A regional relationship module is applied after the multi-view module to excavate the relationship between the sub-regions. Such regional relationship analyses validate known eye-brain links and reveal new discriminative patterns.

Abstract:Alzheimer's disease is a type of neurodegenerative disorder that is characterized by the progressive degeneration of brain cells, leading to cognitive decline and memory loss. It is the most common cause of dementia and affects millions of people worldwide. While there is currently no cure for Alzheimer's disease, early detection and treatment can help to slow the progression of symptoms and improve quality of life. This research presents a diagnostic tool for classifying mild cognitive impairment and Alzheimer's diseases using feature-based machine learning applied to optical coherence tomographic angiography images (OCT-A). Several features are extracted from the OCT-A image, including vessel density in five sectors, the area of the foveal avascular zone, retinal thickness, and novel features based on the histogram of the range-filtered OCT-A image. To ensure effectiveness for a diverse population, a large local database for our study was collected. The promising results of our study, with the best accuracy of 92.17,% will provide an efficient diagnostic tool for early detection of Alzheimer's disease.

Abstract:Mild cognitive impairment (MCI) is a syndrome that occurs in the preclinical stage of Alzheimer's disease (AD) and is also an early signal of the onset of AD. Early detection and accurate differentiation between MCI and AD populations, and providing them with effective intervention and treatment, are of great significance for preventing or delaying the onset of AD. In this paper, we propose a deep learning model, SE-DenseNet, that combines channel attention and dense connectivity networks and apply it to the field of magnetic resonance imaging (MRI) data recognition for the diagnosis of AD and MCI. First, to extract MRI features with high quality, a slicing algorithm based on two-dimensional image information entropy is proposed to obtain AD brain lesion features with stronger representation ability. Second, in terms of model structure, SENet is introduced as a channel attention module and redistribute the weight of image features in the channel dimension; use DenseNet as the main architecture to maximize information flow, and each layer is directly interconnected with subsequent layers. It enables the network to learn and extract relevant features from the input data and improve the classification ability of the network. Finally, our proposed model is validated on the Alzheimer's Disease Neuroimaging Initiative (ADNI) dataset, the results have shown that the accuracy for the four classification tasks of AD-NC, AD-MCI, NC-MCI, and AD-NC-MCI can reach 98.12%, 97.42%, 97.42%, and 95.24%, respectively. At the same time, the sensitivity and specificity have also achieved satisfactory results, exhibited a high performance in comparison with the classic machine learning algorithm and several existing state-of-the-art deep learning methods, demonstrating the proposed method is a powerful tool for the early diagnosis and detection of AD.

Abstract:Objective. Mild cognitive impairment (MCI) is a precursor stage of dementia characterized by mild cognitive decline in one or more cognitive domains, without meeting the criteria for dementia. MCI is considered a prodromal form of Alzheimer's disease (AD). Early identification of MCI is crucial for both intervention and prevention of AD. To accurately identify MCI, a novel multimodal 3D imaging data integration graph convolutional network model is designed in this paper. Approach. The proposed model utilizes 3D-VGGNet to extract three-dimensional features from multimodal imaging data (such as sMRI and FDG-PET), which are then fused into feature vectors as the node features of a population graph. Non-imaging features of participants are combined with the multimodal imaging data to construct a population sparse graph. Additionally, in order to optimize the connectivity of the graph, we employed the Pairwise Attribute Estimation (PAE) method to compute the edge weights based on non-imaging data, thereby enhancing the effectiveness of the graph structure. Subsequently, a population-based graph convolutional network (GCN) integrates the structural and functional features of different modal images into the features of each participant for MCI classification. Main results. Experiments on the ADNI demonstrated accuracies of 98.57%, 96.03%, and 96.83% for the NC-EMCI, NC-LMCI, and EMCI-LMCI classification tasks, respectively. The AUC, specificity, sensitivity, and F1-score are also superior to state-of-the-art models, demonstrating the effectiveness of the proposed model. Furthermore, the proposed model is applied to the ABIDE dataset for autism diagnosis, achieving an accuracy of 91.43% and outperforming the state-of-the-art models, indicating excellent generalization capabilities of the proposed model. Significance. This study demonstrate the proposed model's ability to integrate multimodal imaging data and its excellent ability to recognize MCI. This will help achieve early warning for AD and intelligent diagnosis of other brain neurodegenerative diseases.

Abstract:Mild cognitive impairment (MCI) is the clinical precursor of Alzheimer's disease (AD), which is considered the most common neurodegenerative disease in the elderly. Some MCI patients tend to remain stable over time and do not evolve to AD. It is essential to diagnose MCI in its early stages and provide timely treatment to the patient. In this study, we propose a neuroimaging approach to identify MCI using a deep learning method and functional near-infrared spectroscopy (fNIRS). For this purpose, fifteen MCI subjects and nine healthy controls (HCs) were asked to perform three mental tasks: N-back, Stroop, and verbal fluency (VF) tasks. Besides examining the oxygenated hemoglobin changes (ΔHbO) in the region of interest, ΔHbO maps at 13 specific time points (i.e., 5, 10, 15, 20, 25, 30, 35, 40, 45, 50, 55, 60, and 65 s) during the tasks and seven temporal feature maps (i.e., two types of mean, three types of slope, kurtosis, and skewness) in the prefrontal cortex were investigated. A four-layer convolutional neural network (CNN) was applied to identify the subjects into either MCI or HC, individually, after training the CNN model with ΔHbO maps and temporal feature maps above. Finally, we used the 5-fold cross-validation approach to evaluate the performance of the CNN. The results of temporal feature maps exhibited high classification accuracies: The average accuracies for the N-back task, Stroop task, and VFT, respectively, were 89.46, 87.80, and 90.37%. Notably, the highest accuracy of 98.61% was achieved from the ΔHbO slope map during 20–60 s interval of N-back tasks. Our results indicate that the fNIRS imaging approach based on temporal feature maps is a promising diagnostic method for early detection of MCI and can be used as a tool for clinical doctors to identify MCI from their patients.

A Novel Detection Tool for Mild Cognitive Impairment Patients Based on Eye Movement and Electroencephalogram

Abstract:Background: Detecting subtle changes in visual attention from electroencephalography (EEG) and the perspective of eye movement in mild cognitive impairment (MCI) patients can be of great significance in screening early Alzheimer's disease (AD) in a large population at primary care. Objective: We proposed an automatic, non-invasive, and quick MCI detection approach based on multimodal physiological signals for clinical decision-marking. Methods: The proposed model recruited 152 patients with MCI and 184 healthy elderly controls (HC) who underwent EEG and eye movement signal recording under a visual stimuli task, as well as other neuropsychological assessments. Forty features were extracted from EEG and eye movement signals by linear and nonlinear analysis. The features related to MCI were selected by logistic regression analysis. To evaluate the efficacy of this MCI detection approach, we applied the same procedures to achieve the Clinical model, EEG model, Eye movement model, EEG+ Clinical model, Eye movement+ Clinical model, and Combined model, and compared the classification accuracy between the MCI and HC groups with the above six models. Results: After the penalization of logistic regression analysis, five features from EEG and eye movement features exhibited significant differences (p < 0.05). In the classification experiment, the combined model resulted in the best accuracy. The average accuracy for the Clinical/EEG/Eye movement/EEG+ Clinical/Eye movement+ Clinical/Combined model was 68.69%, 61.79%, 73.13%, 69.46%, 75.61%, and 81.51%, respectively. Conclusion: These results suggest that the proposed MCI detection tool has the potential to screen MCI patients from HCs and may be a powerful tool for personalized precision MCI screening in the large-scale population under primary care condition.
Detection of mild cognitive impairment based on attention mechanism and parallel dilated convolution

Abstract:Mild cognitive impairment (MCI) is a precursor to neurodegenerative diseases such as Alzheimer's disease, and an early diagnosis and intervention can delay its progression. However, the brain MRI images of MCI patients have small changes and blurry shapes. At the same time, MRI contains a large amount of redundant information, which leads to the poor performance of current MCI detection methods based on deep learning. This article proposes an MCI detection method that integrates the attention mechanism and parallel dilated convolution. By introducing an attention mechanism, it highlights the relevant information of the lesion area in the image, suppresses irrelevant areas, eliminates redundant information in MRI images, and improves the ability to mine detailed information. Parallel dilated convolution is used to obtain a larger receptive field without downsampling, thereby enhancing the ability to acquire contextual information and improving the accuracy of small target classification while maintaining detailed information on large-scale feature maps. Experimental results on the public dataset ADNI show that the detection accuracy of the method on MCI reaches 81.63%, which is approximately 6.8% higher than the basic model. The method is expected to be used in clinical practice in the future to provide earlier intervention and treatment for MCI patients, thereby improving their quality of life.

Abstract:Alzheimer's disease (AD) is a global healthcare challenge lacking a simple and affordable detection method. We propose a novel deep learning framework, Eye-AD, to detect Early-onset Alzheimer's Disease (EOAD) and Mild Cognitive Impairment (MCI) using OCTA images of retinal microvasculature and choriocapillaris. Eye-AD employs a multilevel graph representation to analyze intra- and inter-instance relationships in retinal layers. Using 5751 OCTA images from 1671 participants in a multi-center study, our model demonstrated superior performance in EOAD (internal data: AUC = 0.9355, external data: AUC = 0.9007) and MCI detection (internal data: AUC = 0.8630, external data: AUC = 0.8037). Furthermore, we explored the associations between retinal structural biomarkers in OCTA images and EOAD/MCI, and the results align well with the conclusions drawn from our deep learning interpretability analysis. Our findings provide further evidence that retinal OCTA imaging, coupled with artificial intelligence, will serve as a rapid, noninvasive, and affordable dementia detection.

Abstract:Background and objectives. The accurate identification of people with Mild Cognitive Impairment (MCI) who may develop Alzheimer's disease (AD) holds significant importance in facilitating timely intervention and treatment. However, current classification methods have yet to be effective due to the subtle nature of the features involved. This research aims to improve the performance of the MCI classification by enhancing the feature representation in brain MRI. Methods We propose an integrated model that combines Group Shuffle Depth-wise Convolution (GSDW), Global Context Network (GCN), Hybrid Multi-Focus Attention Block (HMAB), and EfficientNet-B0 architecture for MCI classification. This model extracts fine-grained features, contextual information, and long-range dependencies and aggregates low-level details with high-level context information to learn discriminative features. Results Our comprehensive evaluation demonstrates significant improvements in the classification performance for both progressive MCI (pMCI) and stable MCI (sMCI), surpassing existing approaches. The proposed model achieved a notable accuracy of 77.2%. Conclusions This study introduces a novel feature fusion technique that combines global contextual representations and cross-dimensional dependencies to enhance classification results. These findings highlight the potential of the proposed framework for early identification and intervention in individuals at risk of cognitive decline.

Abstract:Alzheimer’s disease (AD) is a progressive neurode-generative disease. Identifying the mild cognitive impairment (MCI) subjects who will convert to AD is essential for early intervention to slow the irreversible brain damage and cognitive decline. In this paper, we propose a novel double-attention assisted multi-task framework for the MCI conversion prediction task. By introducing an auxiliary grey matter segmentation task along with an adaptive dynamic weight average strategy to balance the impact of each task. Then, a double-attention module is incorporated to leverage both the classification and the segmentation attention information to guide the network to focus more on the structural alteration regions for better discrimination of AD pathology, as well as increase the interpretability of the network. Extensive experiments on a publicly available dataset demonstrate that the proposed method significantly outperforms the approaches using the same image modality.

Abstract:Alzheimer's disease (AD) poses a substantial public health challenge, demanding accurate screening and diagnosis. Identifying AD in its early stages, including mild cognitive impairment (MCI) and healthy control (HC), is crucial given the global aging population. Structural magnetic resonance imaging (sMRI) is essential for understanding the brain's structural changes due to atrophy. While current deep learning networks overlook voxel long-term dependencies, vision transformers (ViT) excel at recognizing such dependencies in images, making them valuable in AD diagnosis. Our proposed method integrates convolution-attention mechanisms in transformer-based classifiers for AD brain datasets, enhancing performance without excessive computing resources. Replacing multi-head attention with lightweight multi-head self-attention (LMHSA), employing inverted residual (IRU) blocks, and introducing local feed-forward networks (LFFN) yields exceptional results. Training on AD datasets with a gradient-centralized optimizer and Adam achieves an impressive accuracy rate of 94.31% for multi-class classification, rising to 95.37% for binary classification (AD vs. HC) and 92.15% for HC vs. MCI. These outcomes surpass existing AD diagnosis approaches, showcasing the model's efficacy. Identifying key brain regions aids future clinical solutions for AD and neurodegenerative diseases. However, this study focused exclusively on the AD Neuroimaging Initiative (ADNI) cohort, emphasizing the need for a more robust, generalizable approach incorporating diverse databases beyond ADNI in future research.

Convolutional neural network to identify symptomatic Alzheimer’s disease using multimodal retinal imaging

Abstract:Background/Aims To develop a convolutional neural network (CNN) to detect symptomatic Alzheimer’s disease (AD) using a combination of multimodal retinal images and patient data. Methods Colour maps of ganglion cell-inner plexiform layer (GC-IPL) thickness, superficial capillary plexus (SCP) optical coherence tomography angiography (OCTA) images, and ultra-widefield (UWF) colour and fundus autofluorescence (FAF) scanning laser ophthalmoscopy images were captured in individuals with AD or healthy cognition. A CNN to predict AD diagnosis was developed using multimodal retinal images, OCT and OCTA quantitative data, and patient data. Results 284 eyes of 159 subjects (222 eyes from 123 cognitively healthy subjects and 62 eyes from 36 subjects with AD) were used to develop the model. Area under the receiving operating characteristic curve (AUC) values for predicted probability of AD for the independent test set varied by input used: UWF colour AUC 0.450 (95% CI 0.282, 0.592), OCTA SCP 0.582 (95% CI 0.440, 0.724), UWF FAF 0.618 (95% CI 0.462, 0.773), GC-IPL maps 0.809 (95% CI 0.700, 0.919). A model incorporating all images, quantitative data and patient data (AUC 0.836 (CI 0.729, 0.943)) performed similarly to models only incorporating all images (AUC 0.829 (95% CI 0.719, 0.939)). GC-IPL maps, quantitative data and patient data AUC 0.841 (95% CI 0.739, 0.943). Conclusion Our CNN used multimodal retinal images to successfully predict diagnosis of symptomatic AD in an independent test set. GC-IPL maps were the most useful single inputs for prediction. Models including only images performed similarly to models also including quantitative data and patient data.

Attention-based 3D CNN with Multi-layer Features for Alzheimer's Disease Diagnosis using Brain Images

Abstract:Structural MRI and PET imaging play an important role in the diagnosis of Alzheimer's disease (AD), showing the morphological changes and glucose metabolism changes in the brain respectively. The manifestations in the brain image of some cognitive impairment patients are relatively inconspicuous, for example, it still has difficulties in achieving accurate diagnosis through sMRI in clinical practice. With the emergence of deep learning, convolutional neural network (CNN) has become a valuable method in AD-aided diagnosis, but some CNN methods cannot effectively learn the features of brain image, making the diagnosis of AD still presents some challenges. In this work, we propose an end-to-end 3D CNN framework for AD diagnosis based on ResNet, which integrates multi-layer features obtained under the effect of the attention mechanism to better capture subtle differences in brain images. The attention maps showed our model can focus on key brain regions related to the disease diagnosis. Our method was verified in ablation experiments with two modality images on 792 subjects from the ADNI database, where AD diagnostic accuracies of 89.71% and 91.18% were achieved based on sMRI and PET respectively, and also outperformed some state-of-the-art methods.

Abstract:Alzheimer's Disease (AD) is a widespread, chronic, irreversible, and degenerative condition, and its early detection during the prodromal stage is of utmost importance. Typically, AD studies rely on single data modalities, such as MRI or PET, for making predictions. Nevertheless, combining metabolic and structural data can offer a comprehensive perspective on AD staging analysis. To address this goal, this paper introduces an innovative multi-modal fusion-based approach named as Dual-3DM3-AD. This model is proposed for an accurate and early Alzheimer's diagnosis by considering both MRI and PET image scans. Initially, we pre-process both images in terms of noise reduction, skull stripping and 3D image conversion using Quaternion Non-local Means Denoising Algorithm (QNLM), Morphology function and Block Divider Model (BDM), respectively, which enhances the image quality. Furthermore, we have adapted Mixed-transformer with Furthered U-Net for performing semantic segmentation and minimizing complexity. Dual-3DM3-AD model is consisted of multi-scale feature extraction module for extracting appropriate features from both segmented images. The extracted features are then aggregated using Densely Connected Feature Aggregator Module (DCFAM) to utilize both features. Finally, a multi-head attention mechanism is adapted for feature dimensionality reduction, and then the softmax layer is applied for multi-class Alzheimer's diagnosis. The proposed Dual-3DM3-AD model is compared with several baseline approaches with the help of several performance metrics. The final results unveil that the proposed work achieves 98% of accuracy, 97.8% of sensitivity, 97.5% of specificity, 98.2% of f-measure, and better ROC curves, which outperforms other existing models in multi-class Alzheimer's diagnosis.

Abstract:Multimodality neuroimages have been widely applied to diagnose mild cognitive impairment (MCI). However, the missing data problem is unavoidable. Most previously developed methods first train a generative adversarial network (GAN) to synthesize missing data and then train a classification network with the completed data. These methods independently train two networks with no information communication. Thus, the resulting GAN cannot focus on the crucial regions that are helpful for classification. To overcome this issue, we propose a hybrid deep learning method. First, a classification network is pretrained with paired MRI and PET images. Afterward, we use the pretrained classification network to guide a GAN by focusing on the features that are helpful for classification. Finally, we synthesize the missing PET images and use them with real MR images to fine-tune the classification model to make it better adapt to the synthesized images. We evaluate our proposed method on the ADNI dataset, and the results show that our method improves the accuracies obtained on the validation and testing sets by 3.84 and 5.82%, respectively. Moreover, our method increases the accuracies for the validation and testing sets by 7.7 and 9.09%, respectively, when we synthesize the missing PET images via our method. An ablation experiment shows that the last two stages are essential for our method. We also compare our method with other state-of-the-art methods, and our method achieves better classification performance.
