Abstract:Fitness training is becoming an increasingly popular way of maintaining overall health and preventing illness. However, in some cases the training could be risky and fitness-related injuries have increased by 48% in the USA. The training itself will not cause hurt, but if it is performed in a crucial-improper form (using the wrong technique), it will injure the exerciser. Research has explored the potential of using wearables to monitor fitness training, but the consideration of proper/improper form is not included. In this paper, we propose WearCoach, a wearable based fitness training assistant, which acquires the user’s fitness form information and generates real-time feedback during training. WearCoach differs from previous work of training assistant in 1) it employs a two-phase tracking algorithm to achieve accurate and real-time tracking of body motion, 2) it analyzes the captured arm posture and generates training guidance based on the user’s form, 3) it uses joint orientation as an exercise classification feature to improve recognition accuracy. We conducted experiments with eight participants and nine exercises. Three kinds of feedback are generated, including injury alert, movement correction and symmetry analysis.
Configurable, wearable sensing and vibrotactile feedback system for real-time postural balance and gait training: proof-of-concept

Abstract:Background Postural balance and gait training is important for treating persons with functional impairments, however current systems are generally not portable and are unable to train different types of movements. Methods This paper describes a proof-of-concept design of a configurable, wearable sensing and feedback system for real-time postural balance and gait training targeted for home-based treatments and other portable usage. Sensing and vibrotactile feedback are performed via eight distributed, wireless nodes or “Dots” (size: 22.5 × 20.5 × 15.0 mm, weight: 12.0 g) that can each be configured for sensing and/or feedback according to movement training requirements. In the first experiment, four healthy older adults were trained to reduce medial-lateral (M/L) trunk tilt while performing balance exercises. When trunk tilt deviated too far from vertical (estimated via a sensing Dot on the lower spine), vibrotactile feedback (via feedback Dots placed on the left and right sides of the lower torso) cued participants to move away from the vibration and back toward the vertical no feedback zone to correct their posture. A second experiment was conducted with the same wearable system to train six healthy older adults to alter their foot progression angle in real-time by internally or externally rotating their feet while walking. Foot progression angle was estimated via a sensing Dot adhered to the dorsal side of the foot, and vibrotactile feedback was provided via feedback Dots placed on the medial and lateral sides of the mid-shank cued participants to internally or externally rotate their foot away from vibration. Results In the first experiment, the wearable system enabled participants to significantly reduce trunk tilt and increase the amount of time inside the no feedback zone. In the second experiment, all participants were able to adopt new gait patterns of internal and external foot rotation within two minutes of real-time training with the wearable system. Conclusion These results suggest that the configurable, wearable sensing and feedback system is portable and effective for different types of real-time human movement training and thus may be suitable for home-based or clinic-based rehabilitation applications.

Abstract:The trajectory of the whole body center of mass (CoM) is useful as a reliable metric of postural stability. If the evaluation of a subject-specific CoM were available outside of the laboratory environment, it would improve the assessment of the effects of physical rehabilitation. This paper develops a method that enables tracking CoM position using low-cost sensors that can be moved around by a therapist or easily installed inside a patient's home. Here, we compare the accuracy of a personalized CoM estimation using the statically equivalent serial chain (SESC) method and measurements obtained with the Kinect to the case of a SESC obtained with high-end equipment (Vicon). We also compare these estimates to literature-based ones for both sensors. The method was validated with seven able-bodied volunteers for whom the SESC was identified using 40 static postures. The literature-based estimation with Vicon measurements had a average error 24.9 ± 3.7 mm; this error was reduced to 12.8 ± 9.1 mm with the SESC identification. When using Kinect measurements, the literature-based estimate had an error of 118.4 ± 50.0 mm, while the SESC error was 26.6 ± 6.0 mm. The subject-specific SESC estimate using low-cost sensors has an equivalent performance as the literature-based one with high-end sensors. The SESC method can improve CoM estimation of elderly and neurologically impaired subjects by considering variations in their mass distribution.

Abstract:The estimation of joint contact forces in musculoskeletal multibody dynamics models typically requires the use of expensive and time-consuming technologies, such as reflective marker-based motion capture (Mocap) system. In this study, we aim to propose a more accessible and cost-effective solution that utilizes the dual smartphone videos (SPV)-driven musculoskeletal multibody dynamics modeling workflow to estimate the lower limb mechanics. Twelve participants were recruited to collect marker trajectory data, force plate data, and motion videos during walking and running. The smartphone videos were initially analyzed using the OpenCap platform to identify key joint points and anatomical markers. The markers were used as inputs for the musculoskeletal multibody dynamics model to calculate the lower limb joint kinematics, joint contact forces, and ground reaction forces, which were then evaluated by the Mocap-based workflow. The root mean square error ( RMSE ), mean absolute deviation ( MAD ), and Pearson correlation coefficient ( ρ ) were adopted to evaluate the results. Excellent or strong Pearson correlations were observed in most lower limb joint angles ( ρ = 0.74 ~ 0.94). The averaged MADs and RMSEs for the joint angles were 1.93 ~ 6.56° and 2.14 ~ 7.08°, respectively. Excellent or strong Pearson correlations were observed in most lower limb joint contact forces and ground reaction forces ( ρ = 0.78 ~ 0.92). The averaged MADs and RMSEs for the joint lower limb joint contact forces were 0.18 ~ 1.07 bodyweight ( BW ) and 0.28 ~ 1.32 BW , respectively. Overall, the proposed smartphone video-driven musculoskeletal multibody dynamics simulation workflow demonstrated reliable accuracy in predicting lower limb mechanics and ground reaction forces, which has the potential to expedite gait dynamics analysis in a clinical setting.

Abstract:In clinical, the center of pressure (CoP) is commonly used for accessing the stability of a person's postural control, which is highly associated with various neurological diseases and movement disorders such as Alzheimer's disease, Parkinson's disease, chronic ankle instability. Such a disease usually has a long development or rehabilitation process which requires long-term CoP monitoring. The current CoP evaluation process does not meet the requirement, as it is often complicated and expensive through either the lab-based equipment or the clinical evaluation procedure. Different wearable sensor-based systems with less cost and restrictions have emerged, but their way of CoP calculation requires deliberate calibration of the positions of their sensors, which are not feasible in daily CoP monitoring. In this study, we developed a long-term CoP monitoring system in a smart-shoe form. First, a thin and flexible smart insole with optimal sensor locations was designed to be compact and energy sufficient for a whole-day usage. Then, a user-friendly app on the smartphone with a cloud-based data managing system was developed for applications in both clinical and home environments. Additionally, a simplified CoP estimation model was created without the need for calibration. Lastly, a machine learning-based human activity recognition method was incorporated to make the CoP detection process more automatic. Through a thorough validation test with the clinical level lab equipment, our system can generate the CoP measurements with high accuracy.

A Data-Driven Approach to Estimate Human Center of Mass State During Perturbed Locomotion Using Simulated Wearable Sensors

Abstract:Center of mass (COM) state, specifically in a local reference frame (i.e., relative to center of pressure), is an important variable for controlling and quantifying bipedal locomotion. However, this metric is not easily attainable in real time during human locomotion experiments. This information could be valuable when controlling wearable robotic exoskeletons, specifically for stability augmentation where knowledge of COM state could enable step placement planners similar to bipedal robots. Here, we explored the ability of simulated wearable sensor-driven models to rapidly estimate COM state during steady state and perturbed walking, spanning delayed estimates (i.e., estimating past state) to anticipated estimates (i.e., estimating future state). We used various simulated inertial measurement unit (IMU) sensor configurations typically found on lower limb exoskeletons and a temporal convolutional network (TCN) model throughout this analysis. We found comparable COM estimation capabilities across hip, knee, and ankle exoskeleton sensor configurations, where device type did not significantly influence error. We also found that anticipating COM state during perturbations induced a significant increase in error proportional to anticipation time. Delaying COM state estimates significantly increased accuracy for velocity estimates but not position estimates. All tested conditions resulted in models with R2 > 0.85, with a majority resulting in R2 > 0.95, emphasizing the viability of this approach. Broadly, this preliminary work using simulated IMUs supports the efficacy of wearable sensor-driven deep learning approaches to provide real-time COM state estimates for lower limb exoskeleton control or other wearable sensor-based applications, such as mobile data collection or use in real-time biofeedback.

Abstract:To date, the human balance assessment is mainly achieved by measuring the center of pressure (COP) trajectory of upright posture. However, COP-based metrics provide inconclusive results under certain circumstances, due to the lack of relevance with the body balance mechanism. This paper developed a novel framework to measure the variability in body sway based on the Poincaré analysis to enhance the balance assessment. It provided nonlinear parameters SD1, SD2, and their ratio SD1/SD2 as quantitative indicators of the local and global balance regulation capacities. 136 healthy adults were categorized into young and elderly groups and received test on a force platform. COP signals were recorded for 30 s under eyes-opened and eyes-closed vision conditions. The results were analyzed using the two-way mixed-design analysis of variance and show that the technology is able to reveal the effect of age and vision on the balance control by SD1, SD2, and SD1/SD2. SD1 mainly responded to vision, and SD2 was more influenced by age. Due to the information about the subtle adjustment during quiet standing, these Poincaré indexes surpassed the conventional COP measurements in demonstrating the balance control behavior. It can be concluded that the supposed method of estimating the local and global variabilities is a valuable approach for the balance assessment.
Estimation of CoM and CoP Trajectories During Human Walking Based on a Wearable Visual Odometry Device

Abstract:Estimation of center of mass (CoM) and center of pressure (CoP) is critical for lower limb exoskeletons, prostheses, and legged robots. To meet the demand in these fields, this study presents a novel CoM and CoP estimation method for human walking through a wearable visual odometry (VO) device. This method is named VO-based estimation of CoM and CoP (VOECC). The methodology of VOECC is that the VO provides CoM trajectory estimation and the inherent walking dynamics model is exploited as prior knowledge for CoP trajectory estimation during human walking. Gait cycle is estimated based on the frequency analysis of the CoM trajectory, which is cropped into segments. Each segment mainly includes a half gait cycle. The segments are designed to be sliding to mitigate the disturbance of double-stance phase. For each segment, a quadratic programming (QP) problem is formulated to fit the CoM measurement with the theoretical walking dynamics model. The solution to this QP problem is an optimal gait parameters estimation, including CoP. Based on this solution, the human walking model with the CoM trajectory and CoP excursion is reconstructed. VOECC is evaluated experimentally where human walks on level ground and upstairs with VO device attached in front of the chest. The ground truth of CoM and CoP position is directly measured by the motion capture system and fully instrumented treadmill, respectively, and compared with the VOECC results. The proposed method is demonstrated to be effective in terms of wearable and extensible functionalities compared with the existing methods. Root-mean-squared errors between the CoP measured by fully instrumented treadmill and the CoP estimated by VOECC are evaluated and compared. This method has the potential to be extensible in lower limb rehabilitation, prosthetic, and legged locomotion fields. Note to Practitioners—This article addresses the problem of estimating center of mass (CoM) and center of pres-ure (CoP) trajectories using a minimum number of wearable sensors and reliable algorithms during human daily walking. Estimation of CoM and CoP trajectories is critical for lower limb exoskeletons, prostheses, and legged robots. In this study, a novel method named VO-based estimation of CoM and CoP (VOECC) is presented that utilizes a walking model as prior knowledge and integrates it with visual odometry data, which estimates the trajectory of the wearable visual device. Compared with the inertia measurement unit (IMU)-based method, VOECC only uses one wearable visual device and thus significantly reduces the cost and system complexity. In addition, the VOECC outperforms the motion capture system and force plate since it is not limited to space constraints and has the potential to be applicable for daily life locomotion tasks. VOECC is wearable and untethered and therefore can be directly amounted on lower limb exoskeletons, prostheses, and legged robots. In the experiments, motion capture system and force plates are used to measure CoM and CoP as ground truth to demonstrate the effectiveness of the proposed VOECC. Practical limitations include failure from fast turning and synchronization of multichannel sensors. These limitations will be addressed as our future research directions.

Abstract:Center of pressure (COP) serves as a widely utilized indicator for evaluating balance-related issues, e.g., gait quality of neurological disorders, fall risk of the elderly, and recovery of the injured. Existing methods for acquiring COP mostly rely on expensive force platforms or wearable force-sensing sensors that are relatively difficult to deploy. We propose a novel low-cost strategy to estimate COP with only visual information. Specifically, we first collect a database with walking videos. The plantar pressure of the subjects during walking is synchronously collected for calculating COP ground truth. Then, we propose an encoder-decoder model for estimating COPs using human pose and body shape as inputs. A perceptual codebook is used to tolerate the error in human pose estimation to improve the accuracy of the COP estimation. Experiments demonstrate the effectiveness of our proposed method. Our proposal achieves a correlation coefficient of 0.962, which is 0.8% better than the baseline model using Multi-layer Perceptron (MLP). The normalized RMSEs are improved by 13.9% and 19.0% in the anterior-posterior and medial-lateral directions, respectively. Compared to existing methods using wearable sensors or plantar pressure plates, our method is cheaper and easier to deploy.

Abstract:Myalgia, which often manifests as pain or soreness in skeletal muscles, is among the most salient adverse events associated with 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins). Clinical issues related to statin-associated myotoxicity include (1) incidence in randomized controlled trials and occurrence in postmarketing surveillance databases; (2) potential differences between statins in their associations with such adverse events; and (3) diagnostic and treatment strategies to prevent, recognize, and manage these events. Data from systematic reviews, meta-analyses, clinical and observational trials, and post-marketing surveillance indicate that statin-associated myalgia typically affects approximately 5.0% of patients, as myopathy in 0.1% and as rhabdomyolysis in 0.01%. However, studies also suggest that myalgia is among the leading reasons patients discontinue statins (particularly high-dose statin monotherapy) and that treatment with certain statins (eg, fluvastatin) is unlikely to result in such adverse events. This review presents a clinical algorithm for monitoring and managing statin-associated myotoxicity. The algorithm highlights risk factors for muscle toxicity and provides recommendations for (1) creatine kinase measurements and monitoring; (2) statin dosage reduction, discontinuation, and rechallenge; and (3) treatment alternatives, such as extended-release fluvastatin with or without ezetimibe, low-dose or alternate-day rosuvastatin, or ezetimibe with or without colesevelam. The algorithm should help to inform and enhance patient care and reduce the risk of myalgia and other potentially treatment-limiting muscle effects that might undermine patient adherence and compromise the overall cardioprotective benefits of statins.
A Smartphone App-Based Application Enabling Remote Assessments of Standing Balance During the COVID-19 Pandemic and Beyond

Abstract:Assessments of postural control provide important insights into health. We created a smartphone App-based assessment of standing posture—completed with the phone placed in the user's pocket—to enable remote monitoring of function while minimizing the need for in-person assessment or contact. We tested the reliability of App-derived postural sway metrics, as well as their sensitivity to age and task conditions. Fifteen older and 15 younger adults completed two separate laboratory visits. They followed multi-media instructions provided by the App to complete three 30-second trials of eyes-open (EO), eyes-closed (EC) and dual-task (DT) standing. Sway data were recorded by the App and a force plate. Participants also used the App to complete the assessment in their homes on three separate days. Sway path length and root-mean-square were derived from the angular velocity and acceleration acquired from phone's internal motion sensor, and from the center-of-pressure signal from force plate. App-derived path length and root-mean-square of acceleration and angular velocity across conditions demonstrated moderate-to-excellent test-retest reliability (Intraclass Correlation Coefficients (ICCs)=0.69–0.97) for both younger and older adults. Reliability was comparable to metrics derived from the force plate (ICCs=0.44–0.93). As expected, App-derived sway outcomes exhibited somewhat greater variability when tested across days at home (ICCs=0.14–0.79). All App-derived metrics were sensitive to age (F>43.8,p<0.001) and testing condition (F>31.8,p<0.001) in both laboratory and home settings, revealing that the smartphone App enabled reliable and sensitive assessment of standing posture in both healthy younger and older adults.

Balance Assessment Using a Handheld Smartphone with Principal Component Analysis for Anatomical Calibration

Abstract:Most balance assessment studies using inertial measurement units (IMUs) in smartphones use a body strap and assume the alignment of the smartphone with the anatomical axes. To replace the need for a body strap, we have used an anatomical alignment method that employs a calibration maneuver and Principal Component Analysis (PCA) so that the smartphone can be held by the user in a comfortable position. The objectives of this study were to determine if correlations existed between angular velocity scores derived from a handheld smartphone with PCA functional alignment vs. a smartphone placed in a strap with assumed alignment, and to analyze acceleration score differences across balance poses of increasing difficulty. The handheld and body strap smartphones exhibited moderately to strongly correlated angular velocity scores in the calibration maneuver (r = 0.487–0.983, p < 0.001). Additionally, the handheld smartphone with PCA functional calibration successfully detected significant variance between pose type scores for anteroposterior, mediolateral, and superoinferior acceleration data (p < 0.001).

Abstract:Assessment of balance by means of posturographic analysis is frequently used in the clinical practice for evaluating the risk of falls or as an indicator of balance-related disorders. The development of automatic, affordable and accurate systems for gauging balance capabilities in the elderly is deemed a crucial step towards the adoption of prevention strategies and the reduction of associated social costs, especially in a context of growing average age of population. In this article we propose to exploit signals that can be collected from sensors on board of common consumer-grade smartphones for posturographic analysis. To this aim, we introduce several processing algorithms for extracting useful information from the acceleration data streams, and we also present an assessment framework based on the comparison of the trajectory of the body center of gravity, estimated from embedded triaxial accelerometers, with a homologous counterpart, estimated from the reference plate force, thus adding to the consistency of the whole process. Experimental results confirm the effectiveness of the proposed system in terms of its capability of achieving signals and posturographic features which agree with those obtained by means of balance board platforms, potentially opening the way to novel research studies and applications of mobile technology in this field.

Abstract:BACKGROUND: Standing is a basic human function that healthy adults take for granted, yet it is a complex perceptual-motor process that requires sensation of position and motion from the sensory systems. OBJECTIVE: We assessed agreement between center of pressure data from a laboratory force-platform and head position data from an HTC Vive head-mounted display (HMD) for the evaluation of standing postural control. We investigated the impact of different statistical choices when assessing the relationship between two measurements. Specifically: 1) How does correlation and agreement statistics relate before and after logarithmic transformation? 2) Is there systemic or proportional bias between the force-platform and HMD measurements? METHODS: We tested 37 adults (26 controls, 11 with unilateral vestibular hypofunction) standing on foam, observing a static or dynamic visual scene projected from the HMD. We quantified anterior-posterior and medio-lateral sway via Directional Path, Root Mean Square Velocity, Variance, and Power Spectral Density (PSD) from a force-platform and the HMD. RESULTS: Intra-class correlations (ICCs) were moderate-to-good for the non-transformed data and good-to-excellent after logarithmic transformation for all outcomes except for PSD above 1 Hz. Correlations were higher than ICCs. Bland-Altman plots indicated proportional bias but not after logarithmic transformation. CONCLUSIONS: Both devices correlated linearly, and measure people's postural responses but cannot be used interchangeably, mostly because they appear to diverge with larger sway as evident on Bland-Altman plots of non-transformed data. Agreement between devices was excellent for low frequency movement but poor for high frequency small corrective movements.

The Wii Balance Board is employed to sense the CoP, and vibration motors are used for haptic feedback. To provide a comparison, inductions were also performed using visual and auditory feedback, and the time required for induction was measured. Additionally, after the experiments, a questionnaire survey was conducted.

A Velostat-Based Pressure-Sensitive Mat for Center-of-Pressure Measurements: A Preliminary Study

Abstract:Center-of-pressure (CoP) displacements play a key role in studies assessing postural stability. The accepted instrument to measure CoP trajectories is the force platform, but pressure-sensitive mats (PSMs) are an alternative composed of a matrix of sensitive cells. A typical cell comprises two electrodes with piezoresistive material in between, while a force platform has a force sensor at each of its corners. In this paper, we compare a homemade Velostat-based PSM and an affordable commercial mat with a commercial force platform in a test series with 42 healthy volunteers in single-legged trials (29 males, 13 females; height 1.74 (0.09) m, weight 74.3 (16.34) kg, age 31.21 (12.66) years). The aim of the research was to perform a preliminary study of the performance of our prototype to measure CoP, and more specifically, the standard deviation of the CoP path on both axes, the medial–lateral and anterior–posterior. We could thus discover several improvements for future clinical applications. The intraclass correlation coefficient (ICC) for agreement in the base experiment showed a moderate value for the prototype (0.38 to 0.63) and lower values for the commercial mat (0.11 to 0.12). However, we identified several factors that were relevant to improve ICC and reduce error by considering several processing options: (i) the known crosstalk problem between cells that appears in this kind of mats must be eliminated; (ii) the response time of the sensor has to be taken into account; and (iii) increasing the mat resolution also improves agreement. Therefore, as future work, we plan to test the improved version of the prototype in a clinical environment.

Abstract:Balance control with an upright posture is affected by many factors. This study was undertaken to investigate the effects of real-time visual feedback training, provided by smart wearable devices for COP changes for healthy females, on static stance. Thirty healthy female college students were randomly divided into three groups (visual feedback balance training group, non-visual feedback balance training group, and control group). Enhanced visual feedback on the screen appeared in different directions, in the form of fluctuations; the visual feedback balance training group received real-time visual feedback from the Podoon APP for training, while the non-visual feedback balance training group only performed an open-eye balance, without receiving real-time visual feedback. The control group did not do any balance training. The balance training lasted 4 weeks, three times a week for 30 min each time with 1–2 day intervals. After four weeks of balance training, the results showed that the stability of human posture control improved for the one leg static stance for the visual feedback balance training group with smart wearable devices. The parameters of COP max displacement, COP velocity, COP radius, and COP area in the visual feedback balance training group were significantly decreased in the one leg stance (p < 0.05). The results showed that the COP real-time visual feedback training provided by smart wearable devices can better reduce postural sway and improve body balance ability than general training, when standing quietly.

Abstract:This study explores the efficacy of integrating stroboscopic glasses with smartphone-based applications to evaluate postural control, offering a cost-effective alternative to traditional forceplate technology. Athletes, particularly those with visual and visuo-oculomotor enhancements due to sports, often suffer from injuries that necessitate reliance on visual inputs for balance—conditions that can be simulated and studied using visual perturbation methods such as stroboscopic glasses. These glasses intermittently occlude vision, mimicking visual impairments that are crucial in assessing dependency on visual information for postural stability. Participants performed these tasks under three visual conditions: full vision, partial vision occlusion via stroboscopic glasses, and no vision (eyes closed), on foam surfaces to induce postural instability. The use of a smartphone app to measure postural sway was validated against traditional force plate measurements, providing a comparative analysis of both tools under varied sensory conditions. We investigated postural parameters like anterior–posterior and medial–lateral sway ranges, root mean square values, 95% confidence ellipse area, and sway velocity and median dominant sway frequency from both the smartphone and the force plates. Our findings indicate that force plates exhibit high sensitivity to various visual conditions, as evidenced by significant differences observed in certain postural parameters, which were not detected by smartphone-based measurements. Overall, our findings indicate that smartphones show promise as a cost-effective alternative to force plate measurements for routine monitoring of postural control in sports, although they may not achieve the same level of accuracy as force plates. The integration of stroboscopic glasses further refined the assessment by effectively simulating visual impairments, thereby allowing precise evaluation of an individual's ability to maintain balance under visually perturbed conditions.
