Abstract:Indoor environment is an important repository of microbiome. Environmental microbiome can influence immune protection through changing the commensal flora of the respiratory tract. The interaction between environment airborne microbiome and human microbiome contributes to human microbial diversity, provides immunomodulatory effects, and thus reduces the risk of respiratory diseases. Lack of immune modulation acquired through microbial exposure may lead to risk of chronic respiratory diseases. Clinical parameters, such as susceptibility, progression, and severity of respiratory disease, are associated with microbial changes. Chronic obstructive pulmonary disease, cystic fibrosis, asthma, and lung cancers are associated with airway microbial dysbiosis. The changes of microbiome during the disease period are mainly characterized by a decrease in overall bacterial diversity, a decrease in the number of commensal bacteria, and an increase in the number of potential pathogenic bacteria. Based on the perception that pathogenic or conditional pathogenic microbiome is responsible for the infection and transmission of respiratory and non-respiratory diseases, public health policies have long favored keeping indoors at lower concentrations of microbiome or even absolutely sterile. However, in the past decade, an in-depth understanding of the microbiology of the environment and human health has shifted researchers' view of indoor microbiome from a purely pathogenic and infectious negative role to a potentially protective, preventive positive role. Facing with the dual health effects of indoor airborne microbiome, the ideal control strategy should be to increase the microbial diversity and the proportion of beneficial microbiome, while reducing the concentration of pathogenic microbiome for regions, seasons, and sensitive populations. However, there is still no paradigm shift in built environment microbiology due to technical bottlenecks that prevent researchers from establishing a causal relationship between indoor microbiome and health effects. Currently, the types and properties of indoor airborne microbiome are yet to be fully screened and understood. The characteristics of indoor microbiome when they exert health effects are yet to quantify, including microbial metabolization and reproduction in the respiratory tract, corresponding protective effects, and dose-response effects and individual variation in health effects. The answer to the question "What is a healthy indoor air microbial environment", that is, the characterization of indoor airborne microbiome based on respiratory health effects, is hindered by respiratory microbial sampling, in-depth analysis of microbial data, and the accuracy of in vitro experiments. Subsequent research needs to focus on the development of the following techniques: Microbial sampling tools with low invasiveness, high efficiency and low cost; statistical tools with complex source identification, interference factor quantification and causal relationship judgment functions; and experimental tools capable of simulating real, complex respiratory environments and revealing molecular mechanisms of action in vivo. "What is a healthy indoor air microbial environment" will gradually become clear as these questions are resolved. A clear and quantitative mechanism will serve as a scientific basis to guide the improvement of indoor microbial control standards and the rational use of practical interventions.
Indoor microbiome, air pollutants and asthma, rhinitis and eczema in preschool children - A repeated cross-sectional study

Abstract:Background: Indoor microbiome exposure is associated with asthma, rhinitis and eczema. However, no studies report the interactions between environmental characteristics, indoor microbiome and health effects in a repeated cross-sectional framework. Methods: 1,279 and 1,121 preschool children in an industrial city (Taiyuan) of China were assessed for asthma, rhinitis and eczema symptoms in 2012 and 2019 by self-administered questionnaires, respectively. Bacteria and fungi in classroom vacuum dust were characterized by culture-independent amplicon sequencing. Multi-level logistic/linear regression was performed in two cross-sectional and two combined models to assess the associations. Results: The number of observed species in bacterial and fungal communities in classrooms increased significantly from 2012 to 2019, and the compositions of the microbial communities were drastically changed (p < 0.001). The temporal microbiome variation was significantly larger than the spatial variation within the city (p < 0.001). Annual average outdoor SO2 concentration decreased by 60.7%, whereas NO2 and PM10 concentrations increased by 63.3% and 40.0% from 2012 to 2019, which were both associated with indoor microbiome variation (PERMANOVA p < 0.001). The prevalence of asthma (2.0% to 3.3%, p = 0.06) and rhinitis (28.0% to 25.3%, p = 0.13) were not significantly changed, but the prevalence of eczema was increased (3.6% to 7.0%; p < 0.001). Aspergillus subversicolor, Collinsella and Cutibacterium were positively associated with asthma, rhinitis and eczema, respectively (p < 0.01). Prevotella, Lactobacillus iners and Dolosigranulum were protectively (negatively) associated with rhinitis (p < 0.01), consistent with previous studies in the human respiratory tract. NO2 and PM10 concentrations were negatively associated with rhinitis in a bivariate model, but a multivariate mediation analysis revealed that Prevotella fully mediated the health effects. Conclusions: This is the first study to report the interactions between environmental characteristics, indoor microbiome and health in a repeated cross-sectional framework. The mediating effects of indoor microorganisms suggest incorporating biological with chemical exposure for a comprehensive exposure assessment.
Indoor metabolites and chemicals outperform microbiome in classifying childhood asthma and allergic rhinitis

Abstract:Indoor microorganisms impact asthma and allergic rhinitis (AR), but the associated microbial taxa often vary extensively due to climate and geographical variations. To provide more consistent environmental assessments, new perspectives on microbial exposure for asthma and AR are needed. Home dust from 97 cases (32 asthma alone, 37 AR alone, 28 comorbidity) and 52 age- and gender-matched controls in Shanghai, China, were analyzed using high-throughput shotgun metagenomic sequencing and liquid chromatography-mass spectrometry. Homes of healthy children were enriched with environmental microbes, including Paracoccus, Pseudomonas, and Psychrobacter, and metabolites like keto acids, indoles, pyridines, and flavonoids (astragalin, hesperidin) (False Discovery Rate < 0.05). A neural network co-occurrence probability analysis revealed that environmental microorganisms were involved in producing these keto acids, indoles, and pyridines. Conversely, homes of diseased children were enriched with mycotoxins and synthetic chemicals, including herbicides, insecticides, and food/cosmetic additives. Using a random forest model, characteristic metabolites and microorganisms in Shanghai homes were used to classify high and low prevalence of asthma/AR in an independent dataset in Malaysian schools (N = 1290). Indoor metabolites achieved an average accuracy of 74.9% and 77.1% in differentiating schools with high and low prevalence of asthma and AR, respectively, whereas indoor microorganisms only achieved 51.0% and 59.5%, respectively. These results suggest that indoor metabolites and chemicals rather than indoor microbiome are potentially superior environmental indicators for childhood asthma and AR. This study extends the traditional risk assessment focusing on allergens or air pollutants in childhood asthma and AR, thereby revealing potential novel intervention strategies for these diseases.
Indoor microbiome, microbial and plant metabolites, chemical compounds, and asthma symptoms in junior high school students: a multicentre association study in Malaysia

Abstract:Background: Indoor microbial exposure is associated with asthma, but the health effects of indoor metabolites and chemicals have not been comprehensively assessed. Methods: We collected classroom dust from 24 junior high schools in three geographically distanced areas in Malaysia (Johor Bahru, Terengganu and Penang), and conducted culture-independent high-throughput microbiome and untargeted metabolomics/chemical profiling. Results: 1290 students were surveyed for asthma symptoms (wheeze). In each centre, we found significant variation in the prevalence of wheeze among schools, which could be explained by personal characteristics and air pollutants. Large-scale microbial variations were observed between the three centres; the potential protective bacteria were mainly from phyla Actinobacteria in Johor Bahru, Cyanobacteria in Terengganu and Proteobacteria in Penang. In total, 2633 metabolites and chemicals were characterised. Many metabolites were enriched in low-wheeze schools, including plant secondary metabolites flavonoids/isoflavonoids (isoliquiritigenin, formononetin, astragalin), indole and derivatives (indole, serotonin, 1H-indole-3-carboxaldehyde), and others (biotin, chavicol). A neural network analysis showed that the indole derivatives were co-occurring with the potential protective microbial taxa, including Actinomycetospora, Fischerella and Truepera, suggesting these microorganisms may pose health effects by releasing indole metabolites. A few synthetic chemicals were enriched in high-wheeze schools, including pesticides (2(3H)-benzothiazolethione), fragrances (2-aminobenzoic acid, isovaleric acid), detergents and plastics (phthalic acid), and industrial materials (4,4-sulfonyldiphenol). Conclusions: This is the first association study between high-throughput indoor chemical profiling and asthma symptoms. The consistent results from the three centres indicate that indoor metabolites/chemicals could be a better indicator than the indoor microbiome for environmental and health assessments, providing new insights for asthma prediction, prevention and control.

Abstract:Aerosolized particles with a biological origin are called bioaerosols. Bioaerosols from plants, animals, fungi, bacteria and viruses are an important class of environmental exposures that are clinically relevant to asthma. However, there are important differences in the pathways by which various bioaerosols impact asthma. Additionally, differences in individual susceptibility to different bioaerosols impact exposure reduction and mitigation strategies. Strategies to reduce exposures to potential triggers of asthma are routinely considered as part of standard clinical care and asthma management guidelines. Ventilation standards in buildings may reduce bioaerosol exposure for everyone, but are not necessarily designed specifically to protect patients with asthma. Direct measurement of a bioaerosol is not generally necessary for practical applications where the relevant source of the bioaerosol has been identified. Different types of bioaerosols can be controlled with similar strategies that prioritize source control (e.g. reducing resuspension, integrated pest management, controlling moisture), and can be supplemented by enhancing air filtration. The goal of this review is to summarize the latest information on bioaerosols, including allergens, fungi, bacteria and viruses, that have been associated with adverse asthma outcomes and to discuss mitigation options.

Classroom microbiome, functional pathways and sick-building syndrome (SBS) in urban and rural schools - Potential roles of indoor microbial amino acids and vitamin metabolites

Abstract:Sick building symptoms (SBS) are defined as non-specific symptoms related to indoor exposures, including mucosal symptoms in eye, nose, throat, and skin, and general symptoms as headache and tiredness. Indoor microbial composition is associated with SBS symptoms, but the impact of microbial functional genes and potential metabolic products has not been characterized. We conducted a shotgun microbial metagenomic sequencing for vacuum dust collected in urban and rural schools in Shanxi province, China. SBS symptoms in students were surveyed, and microbial taxa and functional pathways related to the symptoms were identified using a multi-level linear regression model. SBS symptoms were common in students, and the prevalence of ocular and throat symptoms, headache, and tiredness was higher in urban than in rural areas (p < 0.05). A significant higher microbial α-diversity was found in rural areas than in urban areas (Chao1, p = 0.001; ACE, p = 0.002). Also, significant variation in microbial taxonomic and functional composition (β-diversity) was observed between urban and rural areas (p < 0.005). Five potential risk Actinobacteria species were associated with SBS symptoms (p < 0.01); students in the classrooms with a higher abundance of an unclassified Geodermatophilaceae, Geodermatophilus, Fridmanniella luteola, Microlunatus phosphovorus and Mycetocola reported more nasal and throat symptoms and tiredness. Students with a higher abundance of an unclassified flavobacteriaceae reported fewer throat symptoms and tiredness. The abundance of microbial metabolic pathways related to the synthesis of B vitamins (biotin and folate), gamma-aminobutyric acid (GABA), short-chain fatty acids (SCFAs), and peptidoglycan and were protectively (negatively) associated with SBS symptoms (FDR < 0.05). The result is consistent with human microbiota studies, which reported that these microbial products are extensively involved in immunological processes and anti-inflammatory effects. This is the first study to report the functional potential of the indoor microbiome and the occurrence of SBS, providing new insights into the potential etiologic mechanisms in chronic inflammatory diseases.

Abstract:(1) Background: There is increasing awareness that the quality of the indoor environment affects our health and well-being. Indoor air quality (IAQ) in particular has an impact on multiple health outcomes, including respiratory and cardiovascular illness, allergic symptoms, cancers, and premature mortality. (2) Methods: We carried out a global systematic literature review on indoor exposure to selected air pollutants associated with adverse health effects, and related household characteristics, seasonal influences and occupancy patterns. We screened records from six bibliographic databases: ABI/INFORM, Environment Abstracts, Pollution Abstracts, PubMed, ProQuest Biological and Health Professional, and Scopus. (3) Results: Information on indoor exposure levels and determinants, emission sources, and associated health effects was extracted from 141 studies from 29 countries. The most-studied pollutants were particulate matter (PM2.5 and PM10); nitrogen dioxide (NO2); volatile organic compounds (VOCs) including benzene, toluene, xylenes and formaldehyde; and polycyclic aromatic hydrocarbons (PAHs) including naphthalene. Identified indoor PM2.5 sources include smoking, cooking, heating, use of incense, candles, and insecticides, while cleaning, housework, presence of pets and movement of people were the main sources of coarse particles. Outdoor air is a major PM2.5 source in rooms with natural ventilation in roadside households. Major sources of NO2 indoors are unvented gas heaters and cookers. Predictors of indoor NO2 are ventilation, season, and outdoor NO2 levels. VOCs are emitted from a wide range of indoor and outdoor sources, including smoking, solvent use, renovations, and household products. Formaldehyde levels are higher in newer houses and in the presence of new furniture, while PAH levels are higher in smoking households. High indoor particulate matter, NO2 and VOC levels were typically associated with respiratory symptoms, particularly asthma symptoms in children. (4) Conclusions: Household characteristics and occupant activities play a large role in indoor exposure, particularly cigarette smoking for PM2.5, gas appliances for NO2, and household products for VOCs and PAHs. Home location near high-traffic-density roads, redecoration, and small house size contribute to high indoor air pollution. In most studies, air exchange rates are negatively associated with indoor air pollution. These findings can inform interventions aiming to improve IAQ in residential properties in a variety of settings.

Abstract:Background: Previous research has found relationships between specific indoor environmental exposures and exacerbation of asthma. Objectives: In this review we provide an updated summary of knowledge from the scientific literature on indoor exposures and exacerbation of asthma. Methods: Peer-reviewed articles published from 2000 to 2013 on indoor exposures and exacerbation of asthma were identified through PubMed, from reference lists, and from authors' files. Articles that focused on modifiable indoor exposures in relation to frequency or severity of exacerbation of asthma were selected for review. Research findings were reviewed and summarized with consideration of the strength of the evidence. Results: Sixty-nine eligible articles were included. Major changed conclusions include a causal relationship with exacerbation for indoor dampness or dampness-related agents (in children); associations with exacerbation for dampness or dampness-related agents (in adults), endotoxin, and environmental tobacco smoke (in preschool children); and limited or suggestive evidence for association with exacerbation for indoor culturable Penicillium or total fungi, nitrogen dioxide, rodents (nonoccupational), feather/down pillows (protective relative to synthetic bedding), and (regardless of specific sensitization) dust mite, cockroach, dog, and dampness-related agents. Discussion: This review, incorporating evidence reported since 2000, increases the strength of evidence linking many indoor factors to the exacerbation of asthma. Conclusions should be considered provisional until all available evidence is examined more thoroughly. Conclusion: Multiple indoor exposures, especially dampness-related agents, merit increased attention to prevent exacerbation of asthma, possibly even in nonsensitized individuals. Additional research to establish causality and evaluate interventions is needed for these and other indoor exposures.

Abstract:Indoor microbial diversity and composition are suggested to affect the prevalence and severity of asthma. In this study, we collected floor dust and environmental characteristics from 21 classrooms, and health data related to asthma symptoms from 309 students, in junior high schools in Johor Bahru, Malaysia. Bacterial and fungal composition was characterized by sequencing 16s rRNA gene and internal transcribed spacer (ITS) region, and the absolute microbial concentration was quantified by qPCR. In total, 326 bacterial and 255 fungal genera were characterized. Five bacterial (Sphingobium, Rhodomicrobium, Shimwellia, Solirubrobacter, Pleurocapsa) and two fungal (Torulaspora and Leptosphaeriaceae) taxa were protective for asthma severity. Two bacterial taxa, Izhakiella and Robinsoniella, were positively associated with asthma severity. Several protective bacterial taxa including Rhodomicrobium, Shimwellia and Sphingobium has been reported as protective microbes in previous studies, whereas other taxa were first time reported. Environmental characteristics, such as age of building, size of textile curtain per room volume, occurrence of cockroaches, concentration of house dust mite allergens transferred from homes by the occupants, were involved in shaping the overall microbial community but not asthma-associated taxa; whereas visible dampness and mold, which did not change the overall microbial community for floor dust, decreased the concentration of protective bacteria Rhodomicrobium (\b{eta}=-2.86, p=0.021) of asthma, indicating complex interactions between microbes, environmental characteristics and asthma symptoms. Overall, this is the first indoor microbiome study to characterize the asthma-associated microbes and their environmental determinant in tropical area, promoting the understanding of microbial exposure and respiratory health in this region.

Abstract:Indoor environments serve as habitat for humans and are replete with various reservoirs and niches for microorganisms. Microorganisms enter indoor spaces with their human and non-human hosts, as well as via exchange with outdoor sources, such as ventilation and plumbing. Once inside, many microorganisms do not survive, especially on dry, barren surfaces. Even reduced, this microbial biomass has critical implications for the health of human occupants. As urbanization escalates, exploring the intersection of the indoor environment with the human microbiome and health is increasingly vital. The indoor microbiome, a complex ecosystem of microorganisms influenced by human activities and environmental factors, plays a pivotal role in modulating infectious diseases and fostering healthy immune development. Recent advancements in microbiome research shed light on this unique ecological system, highlighting the need for innovative approaches in creating health-promoting living spaces. In this Review, we explore the microbial ecology of built environments — places where humans spend most of their lives — and its implications for immune, endocrine and neurological health. We further propose strategies to harness the indoor microbiome for better health outcomes.

Abstract:The use of HVAC (the heating, ventilation and air conditioning) system which leads to the inadequate circulation of indoor air, together with the popularization of family pets, creates an appropriate environment for survival and reproduction of air-borne microbes. Microbes with their toxin and allergen in air often adhere to saliva and particular matters, landing to the ground or floating in air for long time. Potential adverse health consequences may be led to subjects exposed to the biological contaminants. In particular, the disorders from the respiratory and allergic response were often reported. The progresses of studies on the biological contaminants during recent ten years were reviewed in the paper.

Abstract:Modern living conditions and the recent surge in global urbanization have transformed the human microbiome. This transformation is believed to be a significant factor in the recent spike of common chronic inflammatory diseases like asthma and allergies worldwide, evident in both developed and developing nations. Immigrants from less developed regions who settle in highly urbanized and affluent areas present an ideal demographic for research. Investigating immigrant populations can yield valuable insights, particularly when studying microbiome changes that occur as individuals transition from areas with low asthma prevalence to regions with a high prevalence of the condition. The application of prebiotics and probiotics as potential treatments for asthma and allergies faces challenges. This is due to the complex interplay of numerous factors that contribute to their aetiology. Exploring the interaction between the human microbiome and potential epigenetic changes in specific populations, such as immigrants adapting to new, urbanized environments, may offer crucial insights. Such research could underscore the role of prebiotics and probiotics in preventing allergic conditions. Recognizing the changes in the human microbiome in the context of a Western/modern environment might be essential in addressing the increasing prevalence of allergic diseases. Persistent research in this domain is pivotal for devising effective interventions such as dietary supplementation with prebiotics and probiotics.

Abstract:Abstract: The effects of hypoxia on release of endogenous 3,4‐dihydroxyphenylethylamine (DA, dopamine) were investigated in mouse striatal slices. Following a 60‐min pre‐incubation, potassium increased DA release 12 times between zero and 1 min. By 10 min, uptake processes exceeded release and DA levels in the media decreased. Hypoxia (low oxygen) and anoxia (no oxygen) increased DA in the media by 120 and 205%, respectively, but did not alter dihydroxyphenylacetic acid concentrations. Under similar conditions, anoxia increased [3H]DA uptake eightfold. For the uptake studies, the amount of DA added to the media was critical; in the presence of high concentrations of DA, anoxia reduced reuptake. Regardless of exogenous DA, hypoxia and anoxia increased extracellular DA, which may play a role in ischemic cell damage.
Cleaning Indoor Air- What Works for Respiratory Health: An Updated Literature Review and Recommendations

Abstract:Indoor air pollution is a growing public health concern globally and is associated with increased respiratory symptoms and morbidity. Individuals spend most of their time indoors, and pollutant-related health effects are often driven by the indoor environment. Understanding effective interventions to improve indoor air quality (IAQ) and their impact on respiratory outcomes is key to decreasing the burden of air pollution for high-risk populations across the lifespan. This review applies a hierarchy of interventions framework specific to respiratory health effects and focuses on recent studies of interventions to improve IAQ among high-risk populations with chronic respiratory disease published in the past three years. While policy and source control interventions are likely the most effective and equitable approaches to improve IAQ and benefit population health, these were less extensively investigated. Engineering interventions, such as air cleaner interventions, were the most widely studied. Several studies, including those focused on asthma and COPD, demonstrated improvement in symptoms and medication use with interventions in both home and school-based settings. Combined multilevel interventions with engineering and behavioral interventions led to improved respiratory outcomes in some, but not all, studies. Placing the recent work in the context of the broader literature, we identify gaps in research. Further research is needed to understand intervention effectiveness over time and an increased focus on policy and source control interventions that can mitigate risk in vulnerable populations.
Environmental determinants of household microbial and allergen levels in the Greater Taipei Area

Abstract:People spend most of their time indoors, where they may be exposed to various microorganisms and allergens. In this cross-sectional study, we investigated the relationship between indoor microbial and allergen levels and indoor and outdoor household environmental characteristics. Participants were recruited from an ongoing cohort study in the Greater Taipei Area. Questionnaire administration and environmental sampling were conducted during home visits. Samples were collected from children's bedrooms, living rooms, kitchens, and outdoor areas (balconies) and analyzed for fungal spores, house dust mite allergens (Der p 1 and Der f 1), endotoxin, and cockroach allergens (Bla g 1). Indoor temperature and relative humidity were also measured, and the land-use characteristics and points of interest surrounding the residences were evaluated within specified radii. The mean levels of outdoor air pollutants were then estimated, and multiple regression analysis was performed to investigate the predictors of indoor microbial and allergen levels. In total, 136 homes were analyzed. The results indicated that dog ownership, air purifier use, and floor cleaning frequency were major predictors of indoor microbial and allergen levels. Moreover, proximity to parks, roads, and forests was associated with elevated levels of indoor microorganisms and allergens. Our results demonstrated that indoor concentrations of certain microorganisms and allergens tended to increase in households located within neighborhoods with particular characteristics. Overall, our research findings contribute to housing intervention protocols and policies aimed at reducing exposure to indoor microorganisms and allergens. Future studies should longitudinally investigate the health effects of indoor microorganisms and allergens.

Indoor/Outdoor airborne microbiome characteristics in residential areas across four seasons and its indoor purification

Abstract:Bioaerosols are more likely to accumulate in the residential environment, and long-term inhalation may lead to a variety of diseases and allergies. Here, we studied the distribution, influencing factors and diffusion characteristics of indoor and outdoor microbiota pollution in six residential buildings in Guangzhou, southern China over a period of one year. The results showed that the particle sizes of bioaerosol were mainly in the range of inhalable particle size (<4.7 μm) with a small difference among four seasons (74.61 % ± 2.17 %). The microbial communities showed obvious seasonal differences with high abundance in summer, but no obvious geographical differences. Among them, the bacteria were more abundant than the fungi. The dominant microbes in indoor and outdoor environments were similar, with Anoxybacillu, Brevibacillus and Acinetobacter as the dominant bacteria, and Cladosporium, Penicillium and Alternaria as the dominant fungi. The airborne microbiomes were more sensitive to temperature and particulate matter (PM2.5, PM10) concentrations. Based on the Sloan neutral model, bacteria were more prone to random diffusion than fungi, and the airborne microbiome can be randomly distributed in indoor and outdoor environments and between the two environments in each season. Bioaerosol in indoor was mainly from outdoor. The health risk evaluation showed that the indoor inhalation risks were higher than those outdoor. The air purifier had a better removal efficiency on 1.1-4.7 μm microorganisms, and the removal efficiency on Gram-negative bacteria was better than that on Gram-positive bacteria. This study is of great significance for the risk assessment and control of residential indoor bioaerosol exposure.

Abstract:The prevalence of allergic diseases and asthma has dramatically increased over the last decades, resulting in a high burden for patients and healthcare systems. Thus, there is an unmet need to develop preventative strategies for these diseases. Epidemiological studies show that reduced exposure to environmental bacteria in early life (eg, birth by cesarean section, being formula-fed, growing up in an urban environment or with less contact to various persons) is associated with an increased risk to develop allergies and asthma later in life. Conversely, a reduced risk for asthma is consistently found in children growing up on traditional farms, thereby being exposed to a wide spectrum of microbes. However, clinical studies with bacteria to prevent allergic diseases are still rare and to some extent contradicting. A detailed mechanistic understanding of how environmental microbes influence the development of the human microbiome and the immune system is important to enable the development of novel preventative approaches that are based on the early modulation of the host microbiota and immunity. In this mini-review, we summarize current knowledge and experimental evidence for the potential of bacteria and their metabolites to be used for the prevention of asthma and allergic diseases.

Abstract:A 67-year-old right-handed man was referred to a tertiary care center for headaches, changes in cognition, and gait imbalance. His symptoms began 3 months prior when he developed an unremitting, left-sided, parietal headache. The headache was unusual for him and was not associated with nausea, photophobia, or phonophobia. He had a history of prior headaches that began after minor head trauma in a skiing accident. However, those headaches were infrequent and invariably resolved after 1 day. Shortly after the onset of headache, he began experiencing intermittent scotomas and scintillations in the right visual field. The visual phenomenon would resolve after several minutes. Around the same time, he began having word-finding difficulties and deficits in his short-term memory. There were no clear deficits in long-term memory or changes in mood, behavior, or emotional stability. Within a few weeks, he began to feel unstable when walking. He had several falls due to what he described as a feeling of “being stuck to the floor” and then falling forward when he was finally able to move. His family noted a shuffling gait but no listing to either side. He had to start using a walker. Two months after the onset of symptoms, his visual phenomenon resolved without any intervention. However, he then developed stereotyped, olfactory hallucinations consisting of 20-second episodes of smelling a foul odor. His gait also continued to decline with increased bradykinesia and shuffling. The headaches were still persistent. He was seen at a local hospital 2 weeks after the onset of symptoms, where he underwent a lumbar puncture as part of his workup. Following the lumbar puncture, his gait temporarily improved. His past medical history was significant for diabetes, hyperlipidemia, and hypertension. There was no significant family history. He had smoked 1 pack per day for 37 years, but quit 5 months prior to presentation. There were no systemic symptoms including fevers, night sweats, or rashes.

Abstract:Air pollution and climate change have a significant impact on human health and well‐being and contribute to the onset and aggravation of allergic rhinitis and asthma among other chronic respiratory diseases. In Westernized countries, households have experienced a process of increasing insulation and individuals tend to spend most of their time indoors. These sequelae implicate a high exposure to indoor allergens (house dust mites, pets, molds, etc.), tobacco smoke and other pollutants, which have an impact on respiratory health. Outdoor air pollution derived from traffic and other human activities not only has a direct negative effect on human health but also enhances the allergenicity of some plants and contributes to global warming. Climate change modifies the availability and distribution of plant‐ and fungal‐derived allergens and increases the frequency of extreme climate events. This review summarizes the effects of indoor air pollution, outdoor air pollution and subsequent climate change on asthma and allergic rhinitis in children and adults and addresses the policy adjustments and lifestyle changes required to mitigate their deleterious effects.
