Abstract:Background of the study: Catheter-associated urinary tract infection (CAUTI) is the most prevalent healthcare-associated infection worldwide, according to reports. Nurses are also accountable to acquire appropriate knowledge and practices of catheter care that will prevent UTI. Nurses’ knowledge is seen to be one of the most important factors in their practice. Aim of the study: To assess the nurse’s knowledge toward the prevention of catheter-associated urinary tract infection in public hospitals at Amran City. Methods: A descriptive cross-sectional study was conducted among nurses at public hospitals in Amran City, Yemen. A convenience sampling technique was of the study consisted of 93 nurses from different public hospitals that were participated in this study. Self-administered questionnaire was used to collect data from March 2021 to April 2021. Results: The distribution of nurses’ ware female 65.6%, belonged to age group between (25 to <30 years) 43.0%. The Diploma degree was the most available qualification 88.2%, 81.7% between 1 - 5 years’ experience as nurses. Knowledge regarding prevention of catheter-associated urinary tract infection was fair 72%, while 18.3% had a good. There is a significant statistical difference between knowledge level regarding prevention of CAUTI, and sex (P-value = 0.042). Conclusion: Most of nurses had fair knowledge level regarding prevention of catheter-associated urinary tract infection. Recommendations: We recommended increasing the knowledge of nursing staff through the courses training, workshop, and curriculums.
Compliance With Guideline Statements for Urethral Catheterization in an Iranian Teaching Hospital

Abstract:Background: It is believed that healthcare staff play an important role in minimizing complications related to urethral catheterization. The purpose of this study was to determine whether or not healthcare staff complied with the standards for urethral catheterization. Methods: This study was conducted in Imam Reza teaching hospital, Tabriz, Iran, from July to September 2013. A total of 109 catheterized patients were selected randomly from surgical and medical wards and intensive care units (ICUs). A questionnaire was completed by healthcare staff for each patient to assess quality of care provided for catheter insertion, while catheter in situ, draining and changing catheter bags. Items of the questionnaire were obtained from guidelines for the prevention of infection. Data analysis was performed with SPSS 16. Results: The mean age of the patients was 50.54±22.13. Of the 109 patients, 56.88% were admitted to ICUs. The mean duration of catheter use was 15.86 days. Among the 25 patients who had a urinalysis test documented in their hospital records, 11 were positive for urinary tract infection (UTI). The lowest rate of hand-washing was reported before bag drainage (49.52%). The closed drainage catheter system was not available at all. Among the cases who had a daily genital area cleansing, in 27.63% cases, the patients or their family members performed the washing. In 66.35% of cases, multiple-use lubricant gel was applied; single-use gel was not available. The rate of documentation for bag change was 79%. Conclusion: The majority of the guideline statements was adhered to; however, some essential issues, such as hand hygiene were neglected. And some patients were catheterized routinely without proper indication. Limiting catheter use to mandatory situations and encouraging compliance with guidelines are recommended.

Abstract:Introduction Healthcare-acquired infections are infections that patients acquire while receiving treatment for a medical or surgical condition and can occur in all care facilities. Hospital-acquired infections and the spread of antimicrobial resistance can be reduced by implementing proper preventive measures, including hand hygiene. Aim This study aimed to assess nurses’ knowledge and attitudes toward hand hygiene guidelines in Najran city, determine compliance levels, identify factors contributing to non-compliance, and provide recommendations for interventions to improve hand hygiene practices and reduce healthcare-associated infections risk. Subject and methods This cross-sectional study was conducted among nurses working in the selected government hospitals in Najran City, Saudi Arabia. A self-administered questionnaire was distributed among the targeted nurses using an online survey. The questionnaire includes socio-demographic characteristics such as age, gender, and marital status. The questionnaire had 25 items to measure knowledge, 10 to measure attitude, 6 to measure practices, and 4 to measure the impact of COVID-19 on hand hygiene practices. Results Among the 386 nurses recruited, 88.3% were females, and 25.6% were aged between 31 to 35 years old. Overall, good knowledge, positive attitude, and good practice levels were found in 42.5, 48.4, and 94%, respectively. The common factor influencing hand hygiene practice was the prevention of cross-infection (88.1%). The total knowledge score mean was 18 ± 3.4 (highest possible score: 25). The total attitude score mean was 37.5 ± 6.1 (highest possible score: 50). The total practice score mean was 26.9 ± 2.8 (highest possible score: 30). A higher score indicates higher KAP of hand hygiene. Factors associated with increased KAP were being older in age ( Z = 6.446; p < 0.001), gender female ( Z = 9.869; p < 0.001), being a Filipino nurse ( H = 117.8; p < 0.001), working in a surgery department ( H = 28.37; p < 0.001), having more than 10 years of experience ( Z = 6.903; p < 0.001), living in shared accommodation ( H = 87.22; p < 0.001), having associated chronic disease ( Z = 4.495; p < 0.001), and receiving formal training in hand hygiene ( Z = 2.381; p = 0.017). There was a positive highly statistically significant correlation between knowledge score and attitude score ( r = 0.556), between knowledge score and practices score ( r = 0.303), and between attitude score and practices score ( r = 0.481). Conclusion In light of the results, we can say that the nurses’ knowledge, attitude, and practice in regards to hand hygiene were deemed acceptable. We noticed that female nurses who were older and had more experience, as well as those who had received formal hand hygiene training, displayed better KAP compared to their counterparts. Moreover, we found a significant and positive correlation between the scores for knowledge, attitude, and practice. Nonetheless, additional research is necessary to establish the extent of KAP concerning hand hygiene.

Abstract:Abstract Background: Infections acquired in healthcare facilities have become an increasingly challenging issue worldwide. The prevention of infectious diseases is a critical issue since healthcare workers develop acquired infections while providing services to patients. However, infection prevention and control are challenging, especially in healthcare facilities. Thus, this study aimed to determine the level of knowledge, attitudes, and practices among healthcare workers in King Khaled Eye Specialist Hospital (KKESH). Methodology: A cross-sectional survey was conducted at KKESH, total of 285 healthcare workers, including physicians, nurses, optometrists, radiographers, and ophthalmic assistants were investigated. A close-ended, self-administered questionnaire was used to assess knowledge, attitudes, and practices toward infection control prevention measures. Data analysis was performed using SPSS version 20. Chi-squared was computed to identify associations between factors within the knowledge, attitudes, and practices of infection prevention. Results: The study showed that 81% of respondents had good knowledge, 82% had a positive attitude, and only 59% had safe infection prevention practices. Doctors were more knowledgeable than other staff, and nurses showed a high level of positive attitude and safe practices. There was a statistically significant correlation between age, occupation, education, and years of experience with infection prevention practices among healthcare providers (P<0.05). The availability of infection prevention guidelines was associated with excellent knowledge, a positive attitude, and safe practices. Conclusion: The findings of this study revealed that most healthcare workers had adequate knowledge and displayed a positive attitude, but practices toward infection control were not sufficient. Therefore, regular training, that contain educational programs, should be supported to maintain the highest level of practice.

Abstract:Background and objectives: The emergence of life-threatening infections has highlighted the need for effective infection control programs in all health care settings. These programs are used to support hospitals in reducing the risk of healthcare-associated infection or nosocomial infection, and this study describes the knowledge, attitude, and practice (KAP) of standard infection control precautions among Health Care Workers in 3 teaching hospitals in Sudan. Methods: A cross-sectional study was conducted between March and August 2018 among healthcare workers in 3 teaching hospitals: Omdurman Teaching Hospital, Khartoum Bahri Teaching Hospital and Khartoum Teaching Hospital in Sudan. Data for 254 nurses and 283 working physicians were collected via a self-structured questionnaire to assess the KAP core components of standard precautions. The percentage of professional knowledge and practice scores was calculated and occupational differences in the average professional knowledge and practice scores were ascertained. Result: A total of 429 out of 537 participating healthcare workers completed the questionnaire with a response rate of (79.9%), of whom (52.9%) were physicians and (47.1%) were nurses. (39.6%) of doctors and (53.5%) of nurses have a good level of knowledge, about (96%) of doctors and (97%) of nurses wash their hands after handling any bodily fluids, and this indicates that nearly And the doctors have a similar level of practice as well as a positive attitude. (91.1%) of doctors and (85.6%) of nurses think that they can be the source of transmission. Conclusion: Based on the findings of our study, the majority of nurses had good knowledge compared to fair knowledge physicians, with positive attitudes and practices of both physicians and nurses towards the infection control program. Policies that promote training of health care workers in standard precautions and ensure that infection control and prevention resources in health facilities are regularly provided are required in Sudan. Rates of urinary catheter-associated urinary tract infection in Saudi MOH hospitals: a 2-year multi-centre study

Abstract:Background: The published rates of catheter-associated urinary tract infection (CAUTI) in Saudi Arabia were derived from single-centre studies or nationally non-representative studies. Objectives: The aim of this study was to estimate CAUTI rates and urinary catheter utilisation (UCU) ratios in 99 Saudi Ministry of Health (MOH) hospitals from all regions. Methods: A prospective surveillance study was conducted in 15 different types of intensive care units (ICUs) between January 2018 and December 2019. The data were entered into the Health Electronic Surveillance Network (HESN) programme. The methods of the US National Healthcare Safety Network (NHSN) and the Gulf Cooperation Council (GCC) centre for infection control were used. Results: During 2 years of surveillance covering 919,615 patient-days and 573,720 urinary catheter days, a total of 965 CAUTI events were identified. The overall MOH CAUTI rate was 1.68 (95%confidence interval (CI) 1.58–1.79) per 1,000 urinary catheter days, and the overall UCU was 0.624 (95%CI 0.623–0.625). CAUTI rates were highest in paediatric medical (5.73) and adult medical (2.02) ICUs. UCU were highest in neurosurgical (0.795) and medical surgical (0.743) ICUs. CAUTI standardised infection ratios across different ICUs in Saudi MOH hospitals were 16% lower than NHSN hospitals, 47% lower than GCC hospitals and 69% lower than International Nosocomial Infection Control Consortium (INICC) hospitals. Urinary catheter standardised utilisation ratios across different ICUs in Saudi MOH hospitals were 15% higher than NHSN hospitals, 9% higher than INICC hospitals and 13% lower than GCC hospitals. Conclusions: The current study is a unique national CAUTI benchmark that can potentially help in the fight against healthcare-associated infection.
Epidemiology and microbiology of catheter-associated urinary tract infections: A 14-year surveillance study at a tertiary care center in Lebanon

Abstract:Background Catheter-associated urinary tract infections (CAUTIs) are the most common device-associated healthcare-acquired infections and pose a significant burden on patients and healthcare systems worldwide. However, there is a paucity of data on CAUTI epidemiology and microbiology in the Middle East and North Africa (MENA) region, including Lebanon. Methods This 14-year retrospective cohort study was conducted at a tertiary care center in Lebanon. It analyzed data on all adult patients diagnosed with CAUTI between January 2009 and December 2022 in intensive care units (ICUs) and between June 2011 and December 2022 in regular units. Incidence rates, urinary catheter utilization ratios, and microbiological profiles were collected and analyzed. Results A total of 620 CAUTI cases were identified during the study period. The overall CAUTI rate was 2.4 per 1000 catheter-days, with higher rates in ICUs (3.2 per 1000 catheter-days) compared to regular units (1.4 per 1000 catheter-days). No significant changes in the rates were noted despite implementing many interventions. The most common pathogens were Gram-negative bacteria, with Escherichia coli , Pseudomonas aeruginosa , and Klebsiella pneumoniae being predominant. Multidrug-resistant organisms represented 48% of all isolates. Enterobacterales were largely extended-spectrum β-lactamase (ESBL) producing, and most Acinetobacter baumannii isolates showed multidrug resistance. Conclusions This study provides important insights into CAUTI epidemiology and microbiology in a tertiary care center in Lebanon, addressing the knowledge gap in this area in the MENA region. Despite implementing prevention measures, CAUTI rates remained stable over the 14-year period. The findings highlight the need for continuous improvement in infection prevention practices, diagnostic stewardship, and antimicrobial stewardship, especially given the rising threat of antimicrobial resistance. These results can serve as a guide for the development of targeted preventive strategies to reduce the burden of CAUTIs, particularly in low- and middle-income countries where antimicrobial resistance is a major issue.

Abstract:AimTo assess health care workers' compliance with infection prevention and control measures in different health care sectors in Qatar during COVID-19 pandemic. BackgroundBeing the first line of defense against COVID-19 infection, health care workers are particularly at increased risk of getting infected. Compliance with infection prevention and control measures is essential for their safety and the safety of patients. MethodsA web-based national survey was conducted between November 2020 and January 2021 targeting all health care workers in governmental, semi-governmental, and private health care sectors. ResultsOf 1757 health care workers, 49.9% were between 30-39 years of age, majority (47.5%) were nurses. Participants reported a significant increase in the median self-rated compliance scores during the pandemic compared to before it (p<0.001). During the pandemic, 49.7% of health care workers were fully compliant with personal protective equipment (PPE) use, 83.1% were fully compliant with hand hygiene. Overall, 44.1% were fully compliant with infection prevention and control measures (PPE and hand hygiene). Nationality, health sector, profession, and frequency of interactions with suspected or confirmed COVID-19 cases were significantly associated with compliance with overall infection prevention and control measures. The most reported barriers were work overload, and shortages of PPE and handwashing agents. ConclusionsCompliance of health care workers with infection prevention and control measures needs further improvement. Implications for Nursing ManagementFrequent quality checks, provision of adequate supplies, and behavior change interventions are recommended strategies for hospital and nursing administrators to improve health care workers' compliance.

Healthcare Workers' Perception and Compliance on Personal Protective Equipment Use in Critical Care Units of a Tertiary Care Hospital in Bahrain

Abstract:Introduction Contact precautions or the use of personal protective equipment (PPE), specifically gowns and gloves, has become a common practice in intensive care units (ICUs) as part of robust infection prevention and control efforts. Hence, a positive attitude toward PPE used among healthcare workers (HCWs) is critical to prevent healthcare-associated infections (HAIs). Aim This study aimed to determine the perception and compliance of HCWs working at critical care units regarding PPE used at government hospitals (GHs) in Bahrain. Subject and methods A cross-sectional study was conducted among HCWs working in critical units at GHs, Bahrain. A self-administered questionnaire consisting of three sections was sent to the targeted HCWs. The questionnaire consisted of socio-demographic characteristics (i.e., age, gender, job category, etc.), previous participation in PPE training, and a 15-item questionnaire to assess the perception and compliance of HCWs regarding PPE use. Results Among the 119 HCWs enrolled, 73.1% were females, and 48.7% were between 31 and 40 years old. The results of this study suggested that although 95.8% of participants received education about PPE use, only 48.7% had a good perception of PPE, and an even smaller percentage (12.6%) demonstrated good compliance. A positive, highly statistically significant correlation was found between perception and compliance scores (p<0.001). Increased perception and compliance scores were associated with increasing age, non-Bahrainis, and years of experience. Interestingly, doctors were more likely to demonstrate lower perception and compliance scores toward PPE use. Conclusion The perception of HCWs regarding PPE use was adequate, but their actual adherence was unsatisfactory. However, doctors' perceptions and compliance with PPE use were significantly lower than other HCWs. Further larger studies are required to establish the level of perception and compliance among HCWs in our region.

Abstract:Abstract Background Knowledge of infection prevention and control (IPC) procedures among healthcare workers (HCWs) is crucial for effective IPC. Compliance with IPC measures has critical implications for HCWs safety, patient protection and the care environment. Aims To discuss the body of available literature regarding HCWs' knowledge of IPC and highlight potential factors that may influence compliance to IPC precautions. Design A systematic review. A protocol was developed based on the Preferred Reporting Items for Systematic reviews and Meta-Analysis [PRISMA] statement. Data sources Electronic databases (PubMed, CINAHL, Embase, Proquest, Wiley online library, Medline, and Nature) were searched from 1 January 2006 to 31 January 2021 in the English language using the following keywords alone or in combination: knowledge, awareness, healthcare workers, infection, compliance, comply, control, prevention, factors . 3417 papers were identified and 30 papers were included in the review. Results Overall, the level of HCW knowledge of IPC appears to be adequate, good, and/or high concerning standard precautions, hand hygiene, and care pertaining to urinary catheters. Acceptable levels of knowledge were also detected in regards to IPC measures for specific diseases including TB, MRSA, MERS-CoV, COVID-19 and Ebola. However, gaps were identified in several HCWs' knowledge concerning occupational vaccinations, the modes of transmission of infectious diseases, and the risk of infection from needle stick and sharps injuries. Several factors for noncompliance surrounding IPC guidelines are discussed, as are recommendations for improving adherence to those guidelines. Conclusion Embracing a multifaceted approach towards improving IPC-intervention strategies is highly suggested. The goal being to improve compliance among HCWs with IPC measures is necessary.

Abstract:Background: The placement of a urinary catheter is the single most important predisposing factor for Catheter-associated Urinary Tract Infections, which causes significant morbidity in hospitalized patients, including pain, fever, and malaise. Urinary tract infections are responsible for around 13,000 fatalities each year. Catheter-associated Urinary Tract Infections complications lead to an increase in length of stay, patient pain, higher healthcare expenses, and death. Urinary tract infections are the second most frequent illness after respiratory infections. This has an impact on patients' health outcomes and periods of hospital stay. The aim of this study is to improve nurses' knowledge and practices by involving them in an educational program after assessing nurses' knowledge regarding preventing male catheter-associated urinary tract infections. Methods: A pre-experimental design had been applied with the use of one group pre-test/post-test design of samples of 45 nurses to assess of nursing staff's knowledge, and practices toward the prevention of indwelling catheter-associated urinary tract infection in Al-Diwaniyah Teaching Hospital. The reliability of the questionnaire was achieved through a pilot study and then presented to experts to prove its validity.
Compliance and Knowledge of Healthcare Workers Regarding Hand Hygiene and Use of Disinfectants: A Study Based in Karachi

Abstract:Background Hand hygiene is the cardinal step in combating various healthcare-associated infections. These infections are a cause of 37,000 deaths in Europe and 100,000 deaths in the United States annually. Thus, prevention of their spread is of utmost importance today. A study conducted in a tertiary care center in Karachi found that 17% of the medical professionals were aware of the World Health Organization (WHO) guidelines on hand hygiene while only 4.9% followed these hand-washing techniques. Lack of hand hygiene practice and awareness has raised a need to reassess infection control in hospitals. There is currently undisputed proof that adherence to hand cleanliness diminishes the danger of transmission of various infections. Methods A questionnaire-based cross-sectional study was conducted at Dr. Ruth K.M. Pfau Civil Hospital, Karachi in January 2019. Data from 212 participants who met the inclusion criteria were analyzed. A three-part questionnaire was used for the hospital staff who had been present at the hospital for at least six hours and had attended to the patients during the last three continuous working days. Staff members who visited the hospital but did not attend to any patients or those who had been present at the hospital for less than six hours were excluded. Collected data were analyzed using Statistical Package for Social Science (SPSS) version 23.0 (IBM, Armonk, NY). Results A total of 212 individuals (74 doctors, 66 nurses, 52 technicians, and 20 ward assistants) agreed to participate in our study, of which 124 were females. The compliance with hand disinfectant use before and after every patient contact was found to be 12.3%. The use of disinfectant was found to be more among males than females (mean 7.88 times for males vs. 6.20 for females) and the younger individuals were more compliant with hand hygiene practices; 62.73% of participants were aware of the WHO guidelines regarding hand hygiene and 65.56% were aware of hospital-acquired infections. However, nearly half of the participants (45.75%) had never attended a formal lecture on the subject and more than half (62.26%) of the participants were unenlightened about the complications of hospital-acquired infections. Conclusions Hand hygiene is a basic requirement for every medic and paramedic in a hospital setting today. Keeping in mind the drastic consequences of the spread of hospital-associated infections, it is evident that hand hygiene should be stressed upon. The rising incidence of nosocomial infections and their complications can be prevented by raising awareness about hand hygiene practices. There is a need to further investigate the application of and adherence to the basic guidelines on hand hygiene. Our results indicate that this issue should be tackled through a multidimensional approach.
Effectiveness of Evidence-Based Guidelines on Catheter Associated Urinary Tract Infection Rate among Pediatric Intensive Care Children

Abstract:Background: Urinary tract infection is the most common healthcare associated infection (HCAI) that usually results from prolonged use of urinary catheter that results in prolonged hospital stay. Prevention of catheter associated urinary tract infection (CAUTI) in pediatric patients regarded now as an important hospital policy worldwide. Aim: The aim of this study was examining the effectiveness of evidence-based guidelines on catheter associated urinary tract infection rate among pediatric intensive care children. Design: A quasi-experimental design was utilized. Setting: The study was conducted in acute care units (medical pediatric intensive care unit “PICU”, surgical PICU and cardiac PICU) at Mansoura University Children's Hospital. Subjects: The subjects of the study included 84 nurses who are working at the above-mentioned study settings &102 pediatric patients who are divided randomly into 2 identical groups, case& control groups, each group consists of (51) pediatric patients. Tools: I: A Structured self-administered questionnaire, II: evidence-based guidelines on Urinary catheter maintenance and removal observational checklists, III: A structured questionnaire of pediatric patients' health condition & IV: Catheter-associated urinary tract infection criteria checklist. Results: There was an improvement in the mean scores of nurses' total knowledge and practice with statistically significant difference (P<0.001) in the post intervention compared to pre intervention stage, additionally, the positive symptoms and signs suggesting CAUTI were more evident in control group compared to case group with significant difference (P <0.05 & P <0.001) post intervention. Conclusion: the implementation of evidence-based guidelines about CAUTI prevention for pediatric patients with urinary catheter has positive significant improvement on the prevention of CAUTI rate. Recommendations: there is a requisite to educate the pediatric nurse to avoid routinely use of urinary catheters and remove them when they are no longer required. The use of urinary catheter evidence-based guidelines checklists should be kept in the pediatric patient file and reviewed regularly by pediatric nurses.
Knowledge, attitudes, anxiety, and preventive behaviours towards COVID-19 among health care providers in Yemen: an online cross-sectional survey

Abstract:Background: The growing incidence of coronavirus (COVID-19) continues to cause fear, anxiety, and panic amongst the community, especially for healthcare providers (HCPs), as the most vulnerable group at risk of contracting this new SARS-CoV-2 infection. To protect and enhance the ability of HCPs to perform their role in responding to COVID-19, healthcare authorities must help to alleviate the level of stress and anxiety amongst HCPs and the community. This will improve the knowledge, attitude and practice towards COVID-19, especially for HCPs. In addition, authorities need to comply in treating this virus by implementing control measures and other precautions. This study explores the knowledge, attitude, anxiety, and preventive behaviours among Yemeni HCPs towards COVID-19. Methods: A descriptive, web-based-cross-sectional study was conducted among 1231 Yemeni HCPs. The COVID-19 related questionnaire was designed using Google forms where the responses were coded and analysed using the Statistical Package for the Social Sciences software package (IBM SPSS), version 22.0. Descriptive statistics and Pearson's correlation coefficient test were also employed in this study. A p-value of < 0.05 with a 95% confidence interval was considered as statistically significant. The data collection phase commenced on 22nd April 2020, at 6 pm and finished on 26th April 2020 at 11 am. Results: The results indicated that from the 1231 HCPs participating in this study, 61.6% were male, and 67% were aged between 20 and 30 years with a mean age of 29.29 ± 6.75. Most (86%) held a bachelor's degree or above having at least 10 years of work experience or less (88.1%). However, while 57.1% of the respondents obtained their information via social networks and news media, a further 60.0% had never attended lectures/discussions about COVID-19. The results further revealed that the majority of respondents had adequate knowledge, optimistic attitude, moderate level of anxiety, and high-performance in preventive behaviours, 69.8, 85.10%, 51.0 and 87.70%, respectively, towards COVID-19. Conclusion: Although the Yemeni HCPs exhibited an adequate level of knowledge, optimistic attitude, moderate level of anxiety, and high-performance in preventive behaviours toward COVID-19, the results highlighted gaps, particularly in their knowledge and attitude towards COVID-19.

Abstract:Catheter-associated Urinary Tract Infection (CAUTI) has been studied worldwide as part of device-related healthcare infection. CAUTI is one of the most commonly reported infections in the literature, which occurs during clinical practice in Intensive Care Units (ICUs) and non-ICU departments. Many studies have covered the CAUTI rate in Saudi Arabia mainly in the ICU covering single or multiple hospitals as surveillance for device-related infections in the ICU. Few studies have conducted an interventional approach that examined the implementation of infection control protocols and then compared them to international practice as a standard. This review aims to explore the literature to provide insight into the infection control practices that have been reported in Saudi Arabia during the last two decades on the preventive measures and clinical consequences of CAUTIs. Very few studies have recorded the pattern of resistant microorganisms that burdens clinical practice in ICUs concerning CAUTIs. Only one study compared the type of catheter materials and discussed the effect of these materials on reducing CAUTIs. It is essential to cover catheter utilization and to understand how a sufficient infection control protocol with educational programs for healthcare personnel can transform practice, for the better, regarding CAUTI rates in Saudi hospitals. There is a demand for more interventional and epidemiological studies on the causes and factors affecting the rate of CAUTI in the area. Studies may help reduce the CAUTI incidence rate, which consequently reduces the costs and morbidity associated with this type of infection and other healthcare-related infections.

Isolation and identification of Candida species from catheter-associated urinary tract infection in Thamar city hospitals, Yemen

Abstract:Background The incidence of urinary tract infections associated with Candida is increasing in Yemeni public hospitals. Objectives The primary objective of this research was to isolate specific Candida species responsible for catheter-associated urinary tract infections (UTIs) and to examine the antifungal sensitivity of these Candida isolates. Patients and methods A total of 200 samples were collected from patients with catheters admitted to multiple hospitals of Thamar city (Yemen). There were 50 positive samples with Candida out of 200 samples. We conducted the primary identification process using the established protocols. Before isolation and identification, all yeast isolates underwent sub-culturing on Sabouraud dextrose agar. We employed the standard microbiological procedures such as Gram staining, colonial morphology analysis, lactophenol cotton blue assay, germ tube formation assessment, colony staining on chrom agar Candida medium, and incubation at 37 °C for 48 h. The assessment of cultures was conducted by evaluating their predominant species. All Candida isolates were tested for antifungal susceptibility using the disk diffusion technique, as indicated by the Clinical and Laboratory Standards Institute (CLSI) M44-A document recommendations. Results In this study, the prevalence of Candida species obtained from catheter-associated UTIs was shown to be the highest among individuals aged 51–60 years (28.0%) and the lowest was among those aged 10–20 years (8.0%). Males exhibit higher rates than females, with males accounting for 56.0 and females for 44.0%, respectively. The predominant strain from catheters linked to urinary tract infections was Candida albicans . The Candida isolates had the highest susceptibility to itraconazole, with fluconazole and nystatin at sensitivity rates of 64, 60, and 50%, respectively. Amphotericin B and ketoconazole exhibited the most elevated concentrations. The p value of duration of catheterization < 5 was significant ( p = 0.01), as well as significant in anti-fungal susceptibility testing of itraconazole, ketoconazole, and nystatin which are ( p = 0.03), ( p = 0.04), and ( p = 0.03) respectively. Conclusion Urinary tract infection due to candiduria was more common in patients with indwelling urinary catheter. The catheter-associated urinary tract infection caused by Candida species occurred mainly in old male patients. Candia albicans was the predominant Candida species isolated from urinary tract infection associated with urinary catheter at Thamar city hospitals. This study determined that diabetes and antibiotic use are significant predisposing factors associated with isolation of Candida in specimens submitted by patients at Thamar city hospitals.

Abstract:Distribution of harringtonine positively and negatively charged liposome (HL(+), HL(-)) and harringtonine free drug (FH) in rat tissues were measured by HPLC. Their LD50 in mice were compared. The results showed that distribution of HL(+), HL(-), In vivo may be changed, that the amount of HL(+), HL(-) was increased in the liver, lung, and spleen and in these tissues it was 2-30-fold higher than that of HF after iv 2 h. HL(+), HL(-) may aid to permeate through the blood-brain, blood-testicle barrier, and to reduce acute lethal toxicity. Areas under the time curve of HL(+), HL(-) in brain and testis within 2 h were 2-4.5 times as much as those of HF. There were significant differences in the fate between negatively and positively charged liposomes in vivo.

Abstract:Background: According to the literature analysis, the majority of the studies focused primarily on public health institutions. Although assessing the compliance of healthcare workers in private and public institutions would give comprehensive evidence on existing problems and appropriate prevention method, as a result, research on adherence to standard precautions are still required. Rely on existing research, to the best of the investigator’s knowledge, compliance with standard precautions in hospitals of Bahir Dar town has not been assessed. Therefore, this study will contribute to narrowing these gaps and determining the scope of problems with standard precautions. Methods: An institution-based cross-sectional study design was conducted among 442 healthcare workers working in hospitals from June 10 to 30, 2021. A stratified random sampling technique was employed to select the study participants. Pre-tested and structured questionnaires and an observational checklist were used to collect the required data. The data were entered into EpiData and analyzed using SPSS version 22. Bivariate and multivariable analyses were used to assess the association between independent and outcome variables. Odd ratios at 95% CI were used to measure the strength of the association between the outcome and explanatory variables. Finally, a P-value of <.05 was considered as a cut-off point for statistical significance. Results: Of the 442 healthcare workers who participated in the study, 41% were compliant with standard precautions. Furthermore, 68.1% and 51.8% of the respondents had good knowledge and a positive attitude toward infection prevention, respectively. Consistent water supply availability (AOR = 1.92 and 95% CI = 1.63, 6.27), and access to infection prevention guidelines (AOR = 1.73 and 95% CI = 1.08, 2.77), and availability of personal protective equipment (AOR = 2.32 and 95% CI = 1.35, 3.98) were some of the factors significantly associated with health care workers’ compliance. Conclusions: The current study found that only about two-fifths of the healthcare workers complied with standard precautions. The study suggests that there is a significant risk of developing an infection. Therefore, the concerned organizations; Bahir Dar Zonal Health Office, and respective sectors including Amhara Regional Health Office and the Federal Ministry of Health must take appropriate measures to improve the implementation of safety practices.

Abstract:Catheter-associated urinary tract infections (CAUTIs) are some of the most common hospital-acquired infections (HAIs). Prolonged hospitalization, invasive devices such as catheters, and irrational use of antimicrobial agents are believed to be the major causes of high rates of HAIs. Infections such as pyelonephritis, urethritis, cystitis, and prostatitis are the main concerns in catheterized ICU patients. In these cases, Gram-negative bacteria are the most common bacteria. The present study was undertaken to determine the frequency, antibiograms, disease pattern, and risk factors involved in providing an advocacy recommendation to prevent CAUTI. A total of 1078 patients were admitted to the hospital ICU, out of which healthcare-associated infection was reported in 316 patients. CAUTI was reported only in 70 patients. Klebsiella pneumoniae (20%) was the predominant isolate, with Serratia (3%) and Providencia (3%) species being the least common isolates in this study. The present study provides CAUTI incidence rates in a tertiary care hospital in Hail, Saudi Arabia. Furthermore, information on the risk factors of common associated CAUTI causative organisms and their antibiogram patterns are also presented. This study provides vital information that can be used to formulate an effective antibiotic stewardship program that can be implemented throughout the kingdom.
