Background: Exercise is a safe and beneficial way of improving physical and psychological well-being in postpartum people; yet, postpartum people are less likely to meet the recommended guidelines for physical activity. Identifying the barriers to exercise in the postpartum period may assist health care professionals to better understand how to counsel this population on strategies for exercise. Objective: To synthesize the current literature on barriers to exercise in postpartum people. Study Design: A mixed-methods systematic review. Methods: A systematic literature search was conducted in MEDLINE/PubMed, EMBASE, Scopus, and CINAHL databases of both qualitative and quantitative studies that examined the barriers to exercise among postpartum people. The articles were selected on the basis of predetermined inclusion and exclusion criteria. The Downs and Black Checklist of Methodological Quality was used to assess study quality. Results: Ten studies met the inclusion criteria (7 qualitative and 3 quantitative). Barriers were categorized into the following categories: intrapersonal, interpersonal, sociocultural/demographic, physical environment, and health care environment. The most reported barriers in each category were tiredness and/or lack of sleep (7 out of 10 mentioned), time and/or unpredictable routines/schedule and busy with domestic chores/care/responsibilities (both 8 out of 10 mentioned), lack of support from family, friends, and other mothers (9 out of 10 mentioned), weather (7 out of 10 mentioned), and breastfeeding (3 out of 10 mentioned), respectively. Conclusion: Physical therapists and other health care professionals should be aware of the major reported barriers to exercise among postpartum people, as this will help guide them in providing meaningful education and counseling strategies to increase exercise in this unique population. Further research is needed to capture a more diverse group of postpartum people, as well as considering how policy may affect exercise postpartum.

A review of public health guidelines for postpartum physical activity and sedentary behavior from around the world

Abstract:Background: The period following pregnancy is a critical time window when future habits with respect to physical activity (PA) and sedentary behavior (SB) are established; therefore, it warrants guidance. The purpose of this scoping review was to summarize public health-oriented country-specific postpartum PA and SB guidelines worldwide. Methods: To identify guidelines published since 2010, we performed a (a) systematic search of 4 databases (CINAHL, Global Health, PubMed, and SPORTDiscus), (b) structured repeatable web-based search separately for 194 countries, and (c) separate web-based search. Only the most recent guideline was included for each country. Results: We identified 22 countries with public health-oriented postpartum guidelines for PA and 11 countries with SB guidelines. The continents with guidelines included Europe (n = 12), Asia (n = 5), Oceania (n = 2), Africa (n = 1), North America (n = 1), and South America (n = 1). The most common benefits recorded for PA included weight control/management (n = 10), reducing the risk of postpartum depression or depressive symptoms (n = 9), and improving mood/well-being (n = 8). Postpartum guidelines specified exercises to engage in, including pelvic floor exercises (n = 17); muscle strengthening, weight training, or resistance exercises (n = 13); aerobics/general aerobic activity (n = 13); walking (n = 11); cycling (n = 9); and swimming (n = 9). Eleven guidelines remarked on the interaction between PA and breastfeeding; several guidelines stated that PA did not impact breast milk quantity (n = 7), breast milk quality (n = 6), or infant growth (n = 3). For SB, suggestions included limiting long-term sitting and interrupting sitting with PA. Conclusion: Country-specific postpartum guidelines for PA and SB can help promote healthy behaviors using a culturally appropriate context while providing specific guidance to public health practitioners.

Abstract:Exercise is a component of physical activity done with the intention of getting physically fit to carry out daily activities without undue stress. Habitual participation in exercise is beneficial to all categories of people, including pregnant women. The pregnancy period is regarded as a period of inactivity, but exercise during pregnancy has significant positive health benefits for the mother and the fetus. Despite the positive gains of regular engagement in exercise, many pregnant women do not participate in exercise. To increase participation of pregnant women in exercise, there is a need for women to understand the positive effects of exercise on women during pregnancy and to know exercise protocols for all the stages of pregnancy and the postpartum period. It is believed that if women are aware of the gains of participation in correct exercise regimen during pregnancy, they will be encouraged to engage in it regularly. Through this study, exercise professionals will be kept abreast of the latest research discussions in this area.

Abstract:Abstract Background Excess gestational weight gain has long- and short-term implications for women and children, and postpartum weight retention is associated with an increased risk of long-term obesity. Despite the existence of dietary and exercise guidelines, many women struggle to return to pre-pregnancy weight. Experiences of women in tackling postpartum weight loss are poorly understood. We undertook this study to explore experiences related to nutrition, exercise and weight in the postpartum in women in Ontario, Canada. Methods This was a nested qualitative study within The Be Healthy in Pregnancy Study, a randomized controlled trial. Women randomized to the control group were invited to participate. Semi-structured focus groups were conducted at 4–6 months postpartum. Focus groups were audio recorded, transcribed verbatim, coded and analyzed thematically using a constructivist grounded theory approach. Results Women experienced a complex relationship with their body image, due to unrealistic expectations related to their postpartum body. Participants identified barriers and enablers to healthy habits during pregnancy and postpartum. Gestational weight gain guidelines were regarded as unhelpful and unrealistic. A lack of guidance and information about weight management, healthy eating, and exercise in the postpartum period was highlighted. Conclusion Strategies for weight management that target the unique characteristics of the postpartum period have been neglected in research and in patient counselling. Postpartum women may begin preparing for their next pregnancy and support during this period could improve their health for subsequent pregnancies. However, the last topic about exercise in the postpartum period has some inconsistencies concerning abdominal strengthening exercises, and the two references supporting this paragraph, from our research group where I (P.M.) am the first author, do not concur with what is written. Abstract:2013, 15(3): 123-130. Effects of Exercise on the Health-related Fitness, Postpartum Depression, and Body Weight in Puerperium Period during Pregnancy. [INTRODUCTION] The purpose of this study was to compare health-related fitness, postpartum depression, and body weight changes between two groups of women who had regularly exercised during pregnancy and who had not exercised. This study may also provide useful information for exercise prescription in pregnancy women and contribute their health during pregnancy and physical and mental recovery after delivery. [METHOD] Puerperium women who were being served at the postpartum care center located in B Metropolitan City completed a questionnaire to identify their demographic and delivery-related characteristics. Total 22 puerperium women participated in this study and divided into exercise and non-exercise group. Exercise group (n=9) was those who had regularly exercised for more than 5 months during their pregnancy, while non-exercise group (n=13) was those who had not participated in any exercise program during their pregnancy. Exercise group performed pregnancy yoga program 2 days/week, which was composed of 5-10 min warm-up, 30-40 min main exercise, and 5-10 min cool-down. Submaiximal oxygen consumption, handgrip test, partial curl-ups, back scratch, and body composition were measured for health-related fitness. Postpartum depression was tested using by self-rating depression scale. Difference between the exercise and non-exercise groups was examined by t -test. Difference between the two groups in weight change in accordance with the time point of weight measurement was analyzed by two-way repeated ANOVA. All statistical analyses was peformed by using a statistical software SPSS Ver. 12.0 at the statistical significance level of α =.05. [RESULT] Main Findings of the study were that the exercise group had significantly higher health-related fitness than the non-exercise group, especially in cardiovascular fitness, upper-limb flexibility, and lean body mass. Also, exercise group tended to show lower postpartum depression than non-exercise group, which was not statistically significant. [CONCLUSION] In conclusion, regular exercise during pregnancy was needed to promote the improvement of physical and mental fitness during pregnancy and after delivery. Therefore, it is needed to develop a well-organized and appropriate exercise program during pregnancy for provide pregnant women.

Recommendations for postpartum athletes returning to sport: the past, present, and future

Abstract:Introduction: There is a growing percentage of elite female athletes who choose to start a family during their athletic careers. Current guidelines to manage postpartum elite athletes returning to sport are weakly rooted in athlete-centered evidence and/or are restricted by small sample sizes. The purpose of this review was to collect and compare existing protocols and guidelines for elite athletes returning to sport following childbirth and to highlight the current culture surrounding postpartum athletes. Methods: Online databases including PubMed and BioMed Central were searched from September 2023 to May 2024. Studies of any design were included if they contained information on pregnancy and postpartum. Peer-reviewed research studies, systematic reviews, case reports, and data from organizational websites (American College of Obstetricians and Gynecologists, the World Health Organization, and the Official Olympics) were included. Results: Athlete-mothers face numerous challenges in their transition back to competitive sport following childbirth, including the societal expectations of a 'good' mother, potential loss of financial sponsorships, and limited guidance on postpartum training. Additionally, the healthcare community historically managed postpartum athletes in a reactionary manner by treating symptoms from pregnancy and childbirth as they arise. Recent literature is pointing toward adopting a preventive and proactive model of care to optimize an athlete's health prior to pregnancy and therefore support their safe return to sport postpartum. Conclusion: Increased support for female participation in sports must also be met by increased support for pregnant and postpartum athletes. There is a continued need for research regarding return-to-sport guidelines for postpartum athletes, and their proper implementation.

Abstract:While the birth of a child is a joyous moment within a family, there can be repercussions for the immediate and long-term mental and physical health of mothers. Many women struggle with mental health duress leading to postpartum depression and anxiety, with symptoms aggravated by lack of sleep, psychological distress, and physiological changes. 1 Physical activity (PA) is a valuable tool to assist in coping with physical and mental challenges postpartum. However, many postpartum women are hesitant to engage in PA because of vague recommendations for what is safe to engage in, 2 highlighting a lack of guidance in understanding how to safely engage in PA after childbirth. Moreover, self-efficacy for PA tends to be lower in the postpartum period 3 and is associated with inactivity. 4 Therefore, it is necessary to support mothers in increasing their self-efficacy for PA postpartum to increase PA behavior. 5 Guided by Bronfenbrenner's social ecological model, 6 the aim of this commentary was to provide recommendations on how to support postpartum women's PA engagement through a health equity lens.

Feasibility of a Supervised Postpartum Exercise Program and Effects on Maternal Health and Fitness Parameters—Pilot Study

Abstract:The postpartum period is marked by profound changes in women at physical, psychological, and physiological levels. Many of these changes persist after four to six weeks postpartum, and most women do not resume their levels of physical activity, which increases the risk of remaining inactive for many years. It is crucial to implement effective programs that promote exercise during the postpartum period. The objective of this study was to test the feasibility and analyze the effects of a structured and supervised postpartum exercise program on maternal health and fitness parameters. To analyze the potential effects of the intervention, the level of physical activity, quality of life, pelvic girdle and low back pain, fatigue, depression, and the level of functional and physical fitness were assessed at baseline, after 8 weeks, and after 16 weeks of intervention. Feedback on the exercise program was collected after the final assessment. The results showed that a structured and supervised postpartum exercise program was feasible and safe and produced positive effects on selected maternal health and fitness parameters. These results will encourage a study protocol with a larger sample in order to prove its effectiveness, improve the guidelines for postpartum exercise, and incorporate this program into a routine healthcare setting.

Abstract:Background: Given the increased occurrence of pre-gravid obesity in recent years, and the implications of maternal obesity for maternal and offspring health, it is evident that there is a continued need to investigate antenatal and postnatal weight management strategies and to provide evidence-based advice for exercise-based interventions. Given the small number of studies (<i>n</i> = 5) included in an original systematic review by our group in 2015, it was important to add to the dataset by assessing data published since 2015, in order to provide a more in-depth view of current knowledge.Objective: To provide an updated systematic review on studies employing exercise interventions for weight management in pregnant and postpartum women.Methods: A systematic review of randomised controlled trials evaluating the effects of an exercise intervention on gestational weight gain and postpartum weight management in normal weight women, and women with overweight and obesity was conducted. PubMed, Scopus, Central Register of Controlled Trials and Web of Science were searched for studies published between September 2013 and June 2021. No restrictions were set on type, intensity, duration, or frequency of exercise intervention. Only studies that targeted body weight or mass as a primary outcome were included.Results: Thirteen studies were included in this review: 11 during and two following pregnancy. Exercise significantly reduced gestational weight gain in five of the pregnancy studies and induced significant weight loss in one of the postpartum studies. Across studies, there were large disparities in exercise modality, frequency, and duration, although moderate intensity exercise was consistently employed.Conclusions: Some studies showed positive effects of exercise on weight management during and following pregnancy. While there is still no consensus on effective exercise intervention approaches, it is crucial that physical activity, of any safe form, is recommended to encourage healthy weight management during this time.

Abstract:Physical exercise should be part of an active lifestyle during pregnancy and the puerperium, as shown by growing evidence on its benefits for the health of pregnant women and newborns. Appropriate exercise testing and exercise prescription are needed to tailor effective and safe exercise programs. Exercise testing and prescription in pregnancy is the plan of exercise and fitness-related activities designed to meet the health and fitness goals and motivations of the pregnant woman. It should address the health-related fitness components and the pregnancy-specific conditions, based on previous health and exercise assessments, and take into account the body adaptations and the pregnancy-related symptoms of each stage of pregnancy and postpartum, in order to provide safe and effective exercise. This chapter reviews the guidelines for exercise testing and prescription of pregnant and postpartum women to be developed by exercise professionals, following the health screening and medical clearance for exercise by healthcare providers.

Abstract:PURPOSE To evaluate the imaging performance characteristics of four multidetector-row helical computed tomography (CT) and assess improvement in volume coverage speed over that with single multidetector-row helical CT. MATERIALS AND METHODS The section-sensitivity profile and image noise of a four multidetector-row scanner were measured with phantom scans and compared with predictions from theoretic models. Nominal section thickness ranged from 1.25 to 10.00 mm, beam collimation from 1.25 to 5.00 mm, and table speed from 3.75 to 30.00 mm per rotation. Image artifacts with four and single multidetector-row helical CT were compared in both a phantom study and a subjective rating analysis of clinical images. RESULTS Compared with single multidetector-row helical CT, the volume coverage speed of four multidetector-row helical CT (range, 3.75-30.00 mm per rotation) is at least twice as fast as that with single multidetector-row helical CT (1.0-10.0 mm per rotation) with fully comparable image quality or, in many cases, three times as fast with diagnostically comparable image quality. CONCLUSION Compared with single multidetector-row helical CT, four multidetector-row helical CT provides a two- to threefold improvement in volume coverage speed with comparable diagnostic image quality.

Abstract:This study aimed to describe the beliefs and perceptions of pregnant women and healthcare providers about physical activity during pregnancy. Using a qualitative approach, 30 pregnant women and the 14 healthcare providers caring for them were interviewed in the second trimester of pregnancy. We included women who maintained, decreased, or stopped physical activity since becoming pregnant. They were divided into low (≤ 8 years) and high schooling (> 8 years). Semi-structured, in-depth interviews were conducted and guided by three key questions: (1) When does physical activity during pregnancy start to be considered a wrong behavior?; (2) What are the main barriers (biological or others) to physical activity?; and (3) Do the actions of healthcare providers and people close to pregnant women reinforce barriers? Interviews were audio recorded, transcribed, and analyzed based on recurring themes. All women changed their physical activity behavior (decreased or stopped) when they discovered their pregnancy. Fear of miscarriage, contractions, bleeding, and of causing malformations in the baby were the most reported reasons for decreasing or stopping physical activity. Participants also lacked access to consistent information and healthcare providers’ support on the benefits of physical activity. Despite the current international recommendations to regular physical activity during pregnancy, uncertainty regarding its benefits remains. Interventions to promote physical activity during this period should include the training of healthcare providers so they can advise and discard ideas contrary to mother-child health benefits.

Abstract:Background: The extent of the benefits of exercise training during pregnancy on maternal, fetal, and neonatal health outcomes has not been sufficiently addressed. While aerobic exercise training has been determined as safe and efficacious throughout pregnancy, the effects of other training modes on fetal health and development as well as any continued benefits for the neonate, especially with regards to cardiovascular development and function, is largely unknown. In the ENHANCED by Mom study we aim to determine the effects of different modes of exercise training (aerobic, circuit, and resistance) throughout pregnancy on childhood health by controlling individual exercise programs and assessing the effects of each on fetal and neonatal health adaptations.Methods/Design: ENHANCED by mom is a cross sectional comparison study utilizing 3 intervention groups in comparison to a control group. Participants will complete three 5 min warmup + 45 min sessions weekly from 16 weeks to 36 weeks gestation of aerobic, resistance, or circuit training, in comparison to non-exercising controls. Maternal physical measurements will occur every 4 weeks throughout the intervention period. Fetal morphometric and heart measurements will occur at 34 weeks gestation. Neonatal measurements will be acquired at birth and at 1 month, 6 months, and 12 months.Discussion: A better understanding on the effects of exercise training during pregnancy on fetal and neonatal health could have a profound impact on the prevention and development of chronic diseases such as obesity, hypertension, and diabetes.

Abstract:Introduction and Objective: There is no doubt that engaging in regular physical activity is beneficial for individuals of all ages. Pregnancy is a special time of life for most women. This period presents a unique opportunity for them to improve their lifestyle choices and adopt healthier habits such as exercise. The aim of this article is to find out if regular physical activity during pregnancy has a positive impact for both the mother and the baby.Review and Methods: Review and summary of available studies and meta-analyses found in open-access formats on Google Scholar and PubMed.State of Knowledge: The WHO defines physical activity as any bodily movement requiring energy expenditure, including during leisure, transport, work, and domestic activities. Regular exercise reduces the risk of noncommunicable diseases (NCDs) and poor health outcomes, while sedentary behavior contributes to this and burdens healthcare systems. For pregnant and postpartum women, physical movement decreases the risk of various complications, with no negative effects on birthweight or stillbirth risk. Conclusions: Studies show that physical activity decreased risk of pre-eclampsia, gestational hypertension, gestational diabetes, excessive gestational weight gain, delivery complications, postpartum depression and newborn complications.
'It's just one step too far': Negotiating physical activity for perinatal mental health

Abstract:Background: Physical activity is recommended as a self-help strategy for some mild to moderate perinatal mental illnesses. Despite this, we know very little about how women remain active, or take up physical activity, in the context of changing family life and perinatal mental illness. We seek to explore: a) how women negotiate physical activity for their mental health during transitions into parenthood and the early years; and b) the experiences of women with perinatal mental illness in relation to physical activity. Methods: An anonymous UK-wide qualitative online survey was used to better understand how physical activity may be used for perinatal mental health, barriers to activity and changes over time. 186 women with babies and children up to four years completed the survey. Reflexive thematic analysis was used to analyse the qualitative data and generate themes. Results: Themes generated from this data were: 1) negotiating being active for perinatal mental health (the benefits and how these women remained active); 2) barriers to PA associated with mental illness (some women were active but still experienced general barriers to further PA, and there were several emotional barriers from mental illness); and 3) PA, guilt and the importance of valuing walking (guilt about not being active enough, guilt and feelings of failure exacerbated by questions about PA levels without support offered; regular walking not valued as PA exacerbating feelings of guilt). Conclusions and implications: This study provides new empirical data on the experiences of new mothers', physical activity and mental health with important implications for physical activity messaging and support for women during this time through maternity care. Individualised conversations are important and the value of walking and incidental activity from activities that mothers and families find enjoyable should be promoted.
[Physical activity in pregnancy and in breast-feeding period in obese mothers]

Abstract:Background: Regular physical activity is an important component of a health pregnancy. Being physically active during pregnancy often creates uncertainty and leads to numerous questions: How much and which kind of sports is possible? In pregnant woman a lot of physiological changes are taking place in haemodynamics, in the respiratory system, in the muculoskeletal system, in the glucose metabolism, and in further endocrinological feedback systems besides the psyche and bring about changes in fitness and physical performance. Discussion: There is evidence that the most active women show the lowest prevalence for gestational diabetes (GDM) and, moreover a lower incidence for obesity and diabetes in both mother and child. Physically active women rarely develop not only GDM but also pre-eclampsia. The protective effect of physical activity can be explained by an enhanced placental growth and vascularity, by decreased oxidative stress, reduced inflammation and an adaption of the disease-related endothelial dysfunction. Maternal obesity increases the frequencies of infertility and miscarriage. Weight loss programmes with nutritional advice and activity counsellings represent a cost-effective infertility treatment. Moreover the possibility of health problems during pregnancy are limited thereby. A high degree of fitness before pregnancy and regular physical activity before conception can prevent the excessive weight gain during pregnancy and influence the weight at the very best. Conclusions: Considering common recommendations for training, as well as careful measures and contraindications, a moderate individual training to maintain physical and psychic fitness is desirable. Many kinds of sports like jogging, nordic walking, swimming and cycling, for example, can be carried out in a pregnancy without any risks and furthermore promote the health of the future mother and child.
