Abstract:The mechanisms that appear to account for the effects of ionizing radiation on the growth of plants are reviewed. Preliminary results are reported from a systematic study of the effects of gamma radiation on a terrestrial ecosystem and its components. The ecosystem chosen for this experiment supports a stana of the Long Island pine-oak forest. A 9500 c Cs/sup 137/ source, designed so it can be shielded when desired. was centrally located. Rates of exposure vary from several thousand r/day within a few Meters to about 2 r/day at 130 meters. The source has been exposed 20 hr/day since Nov. 22, 1961 Prior to installation of the source, a detailed survey was made on the species composition of the vegetation and on the size and vigor of individual plants. The early effects of radiation exposure are described. (C.H.)
Effect of Music Therapy on Anxiety in Pregnancy: A Systematic Review of Randomized Controlled Trials

Abstract:Pregnancy can heighten anxiety levels, impacting both maternal and fetal well-being. This systematic review synthesizes evidence from 33 randomized controlled trials exploring the effects of music therapy on anxiety in pregnant women. The studies included diverse populations, ranging from women undergoing in vitro fertilization (IVF) to those facing high-risk pregnancies. Music therapy interventions varied widely, including virtual reality experiences, classical music, lullabies, and patient-selected music, administered at different pregnancy stages such as IVF treatments, elective cesarean sections, and high-risk hospitalizations. The findings consistently demonstrated that music therapy significantly reduces anxiety levels in pregnant women. Positive outcomes included reductions in both state and trait anxiety, improved pregnancy rates, and enhanced maternal-fetal parameters. Additionally, music therapy showed promise in reducing anxiety during labor, cesarean deliveries, and high-risk hospital stays. These varied interventions and their positive outcomes highlight the potential of music therapy as an effective, non-pharmacological approach to managing pregnancy-related anxiety. This review provides a comprehensive overview of the existing evidence on music therapy's efficacy in alleviating anxiety during pregnancy. It underscores the need for further research to standardize interventions and incorporate music therapy into routine prenatal care. Effect of Listening Music on Nulliparous Singleton Pregnancies Who Underwent Induction of Labor: A Randomized Clinical Trial

Abstract:Objective: A recent randomized clinical trial (RCT) showed that listening to music reduces the pain level and anxiety levels in women with spontaneous labor at term. The effect on pregnant women undergoing induction of labor is still unclear.This study aimed to test the hypothesis that in nulliparous women with singleton pregnancies, undergoing induction of labor at term, listening to music would reduce the pain level during labor. Study design: Parallel group, non-blinded, RCT conducted at a single center in Italy. Nulliparous women with singleton pregnancies and vertex presentation, admitted for induction of labor with either oral or vaginal prostaglandins, between 370/7 and 420/7 weeks, were randomized in a 1:1 ratio to receive music during induction of labor or no music during induction. The endpoints of the trials were the pain level during induction, and in the active phase of labor, recorded using the visual analog scale for pain, ranging from 0 (no pain) to 10 (unbearable pain). The effect of music use on each outcome was quantified as the mean difference (MD) with a 95% confidence interval (CI). Results: During the study period, 30 women agreed to take part in the study, underwent randomization, and were enrolled and followed up. Fifteen women were randomized in the music group and 15 in the control group. No patients were lost to follow-up for the primary outcome. Pain level during the induction procedure was 8.8 ± 0.9 in the music group, and 9.8 ± 0.3 in the control group (MD -2.60 points, 95% CI -3.94 to -1.26; p < 0.01). Music during labor and delivery was also associated with decreased anxiety during the induction procedure (MD -3.80 points, 95% CI -5.53 to -2.07; p < 0.01). Conclusion: In nulliparous women, listening to music during the induction of labor reduces pain and anxiety levels. Abstract:OBJECTIVE: Music interventions are widely used, but have not yet gained a place in guidelines for pediatric surgery or pediatric anesthesia. In this systematic review and meta-analysis we examined the effects of music interventions on pain, anxiety and distress in children undergoing invasive surgery.DATA SOURCES: We searched 25 electronic databases from their first available date until October 2014.STUDY SELECTION: Included were all randomized controlled trials with a parallel group, crossover or cluster design that included pediatric patients from 1 month to 18 years old undergoing minimally invasive or invasive surgical procedures, and receiving either live music therapy or recorded music.DATA EXTRACTION AND SYNTHESIS: 4846 records were retrieved from the searches, 26 full text reports were evaluated and data was extracted by two independent investigators.MAIN OUTCOME MEASURES: Pain was measured with the Visual Analogue Scale, the Coloured Analogue Scale and the Facial Pain Scale. Anxiety and distress were measured with an emotional index scale (not validated), the Spielberger short State Trait Anxiety Inventory and a Facial Affective Scale.RESULTS: Three RCTs were eligible for inclusion encompassing 196 orthopedic, cardiac and day surgery patients (age of 1 day to 18 years) receiving either live music therapy or recorded music. Overall a statistically significant positive effect was demonstrated on postoperative pain (SMD -1.07; 95%CI-2.08; -0.07) and on anxiety and distress (SMD -0.34 95% CI -0.66; -0.01 and SMD -0.50; 95% CI -0.84; - 0.16.CONCLUSIONS AND RELEVANCE: This systematic review and meta-analysis indicates that music interventions may have a statistically significant effect in reducing post-operative pain, anxiety and distress in children undergoing a surgical procedure. Evidence from this review and other reviews suggests music therapy may be considered for clinical use.

Abstract:Objectives To examine the impact of receptive music therapy on maternal anxiety both during and after the process of childbirth. Methods In this experimental study, 217 women were divided into the receptive music therapy and control groups. The first group were exposed to music at intervals of 20 minutes for a duration of 30 minutes during labor. Data were collected using the Pregnant Information Form, the State Anxiety Inventory (STAI), Visual Analogue Scale and Edinburgh postnatal depression scale. Results The pregnant women who participated in the music group exhibited reduced scores of STAI, both during the active time (46.42 ± 11.69 vs. 50.21 ± 11.14, 44.37 ± 10.38 vs. 47.56 ± 11.46, P<0.05) and two hours after giving birth(26.32 ± 6.23 vs. 29.55 ± 8.9, 30.38 ± 7.15 vs. 33.08 ± 9.45, P<0.05). At the first stage of labor, pregnant women in the music group experienced dramatically reduced score of pain in active phase (6.39 ± 1.00 vs. 6.91 ± 0.99, P<0.05) and Edinburgh postnatal depression scale at discharged from the hospital (6.68 ± 3.36 vs. 7.66 ± 3.54, P<0.05). Conclusion Receptive music therapy is effective in reducing pain during labor and anxiety during prenatal and postnatal periods. The use of receptive music therapy in obstetric care can be an effective tool in preventing anxiety-induced complications.

Abstract:Pyostomatitis pyodermatitis vegetans (PSPDV) is an inflammation of the skin and mucous membranes associated commonly with inflammatory bowel disease, particularly ulcerative colitis. Ocular adnexal involvement with this condition is rare. We describe a case of PSPDV presenting with conjunctival involvement. A 50-year-old woman was referred to our clinic for repair of upper lid cicatricial entropions and nodules following 4 years of recurrent upper eyelid inflammation (Figs 1,2). On her first episode, she had associated flu-like symptoms with a history of ulcerative colitis controlled by a combination of azathioprine and mesalamine. She denied oral or skin complications. Her first ophthalmologist incised both upper lids for suspected chalazia but this caused increased conjunctival ulceration and scarring. Conjunctival biopsies found a mixed inflammatory cell infiltrate with prominent eosinophilia and neutrophilia, with lymphocytes in deeper layers (Fig. 3). Immunofluorescent stains were negative for IgG, IgA, IgM, C3 and fibrin. Ocular cicatricial pemphigoid was suspected, and dapsone was added at 50 mg/day, increasing gradually to 100 mg/day. This additional medication appeared to provide some control of the inflammation but necrosis and scarring developed nonetheless. Her disease settled after 18 months of therapy with combination azathioprine, mesalamine and dapsone. Several short pulses of oral prednisone were given throughout this period for flare-ups of her colitis. She was referred to our clinic 3 years later for repair of a left upper eyelid nodule and cicatricial entropion. Her unusual history prompted review of her condition. At the time of initial symptoms her full blood count was normal apart from an eosinophilia (0.8 giga/L, normal less than 0.5). She was noted to have snail track scarring in the buccal mucosa and her diagnosis changed to PSPDV (Fig. 4). The left upper lid nodule with associated trichiasis was excised with a full thickness wedge and direct closure, using perioperative oral corticosteroid cover. Pyostomatitis pyodermatitis vegetans is an inflammatory disorder of unknown aetiology affecting the mucosal surfaces of the mouth, the intertriginous skin and rarely the conjunctiva. Characteristically the process begins as small pustules in each location that coalesce into ‘snail tracks’. Vegetating lesions of the skin may erupt. Typical histopathology includes intraepithelial and subepithelial abscesses, a preponderance of eosinophils diminishing with time, underlying lymphocytic and neutrohpilic infiltrate and possible acanthosis and hyperkeratosis of overlying epithelium. An acute peripheral eosinophilia is present in 90% of cases. Ocular adnexal involvement with PSPDV is rare. Leibovitch et al. described a case masquerading as folliculitis and reviewed two previous papers describing ocular involvement. The diagnosis may be challenging as no diagnostic test exists for PSPDV. It is differentiated from pemphigobullous disease based on clinical features together with supportive histology and negative immunofluorescence. Biopsy results may vary based on the site and timing of the process. An early eosinophilia gives way to a mixed chronic inflammatory cell infiltrate and eventually tissue necrosis, fibrosis and scarring. Inflammatory bowel disease is well known to be associated with a variety of dermatites including Sweet’s syndrome, Pyoderma Gangrenosum, erythema elevatum diutinum and granuloma faciale. There has been considerable debate over the classification of this clinical spectrum with recent trend moving towards unification under the term neutrophilic dermatoses. Some authors postulate that differing levels of skin involvement by the same pathogenic process confers the different clinical appearances of this group. All share infiltration of the skin by normal neutrophils with tissue inflammation and necrosis. PSPDV may be distinct from these disorders but has considerable clinical overlap. Our case bears some similarities to a series of pyoderma gangrenosum affecting the eyelids. The authors describe a nodular inflammatory process followed by necrosis with predilection for the central 1/3 of the upper eyelid with sparing of the tarsal margin. However, we favour the diagnosis of PSPDV based on the painless nature of the lesions, the presence of local and peripheral eosinophilia, the characteristic oral findings and the primary involvement of the conjunctiva rather than the lid skin. Atypical features of our case are that our patient denies ever having skin involvement elsewhere and that she had minimal stomatitis, recognized only in retrospect by the presence of scarring. Her tempered presentation may have resulted from baseline immunosuppression with azathioprine, mesalasine, dapsone and, some months later, periodic oral prednisone pulses. All these agents are known to suppress PSPDV and other neutrophilic dermatoses. Systemic steroids are perhaps the most potent agent in the short term. It is unsure whether more aggressive earlier therapy might have spared some of the ultimate deformity of the eyelids. Ophthalmologists should be aware of the cutaneous manifestations of inflammatory bowel disease for several reasons. Although ocular involvement is rare, findings are potentially reversible with immunosuppression. Music intervention to relieve anxiety and pain in adults undergoing cardiac surgery: a systematic review and meta-analysis

Abstract:Objectives Previous studies have reported beneficial effects of perioperative music on patients’ anxiety and pain. We performed a systematic review and meta-analysis of randomised controlled trials investigating music interventions in cardiac surgery. Methods Five electronic databases were systematically searched. Primary outcomes were patients’ postoperative anxiety and pain. Secondary outcomes were hospital length of stay, opioid use, vital parameters and time on mechanical ventilation. PRISMA guidelines were followed and PROSPERO database registration was completed (CRD42020149733). A meta-analysis was performed using random effects models and pooled standardised mean differences (SMD) with 95% confidence intervals were calculated. Results Twenty studies were included for qualitative analysis (1169 patients) and 16 (987 patients) for meta-analysis. The first postoperative music session was associated with significantly reduced postoperative anxiety (SMD = –0.50 (95% CI –0.67 to –0.32), p<0.01) and pain (SMD = –0.51 (95% CI –0.84 to –0.19), p<0.01). This is equal to a reduction of 4.00 points (95% CI 2.56 to 5.36) and 1.05 points (95% CI 0.67 to 1.41) on the State-Trait Anxiety Inventory and Visual Analogue Scale (VAS)/Numeric Rating Scale (NRS), respectively, for anxiety, and 1.26 points (95% CI 0.47 to 2.07) on the VAS/NRS for pain. Multiple days of music intervention reduced anxiety until 8 days postoperatively (SMD = –0.39 (95% CI –0.64 to –0.15), p<0.01). Conclusions Offering recorded music is associated with a significant reduction in postoperative anxiety and pain in cardiac surgery. Unlike pharmacological interventions, music is without side effects so is promising in this population.
The effect of music on pain management in preterm infants during daily painful procedures: a systematic review and meta-analysis

Abstract:Background: The present systematic review and meta-analysis of randomized controlled trials (RCTs) was conducted to investigate the effects of music on pain management in preterm neonates during painful procedures. Methods: The PubMed, Embase, Web of Science, EBSCO and Cochrane Library databases were searched to identify relevant articles published from their inception to September 2023. The study search strategy and all other processes were implemented in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. Results: Four RCTs that satisfied the inclusion criteria were included in this meta-analysis. The music group had significantly lower Premature Infant Pain Profile (PIPP) scores during (RR = −1.21; 95% CI = −2.02–−0.40, p = 0.0032) and after painful procedures (RR = −0.65; 95% CI = −1.06–−0.23, p = 0.002). The music group showed fewer changes in PIPP scores after invasive operations than did the control group (RR = −2.06; 95% CI −3.16–−0.96; p = 0.0002). Moreover, our results showed that music improved oxygen saturation during (RR = 3.04, 95% CI = 1.64–4.44, p < 0.0001) and after painful procedures (RR = 3.50, 95% CI = 2.11–4.90, p < 0.00001). However, the change in peak heart rate during and after painful procedures was not statistically significant (RR = −12.14; 95% CI = −29.70−5.41 p = 0.18; RR = −10.41; 95% CI = −22.72−1.90 p = 0.10). Conclusion: In conclusion, this systematic review demonstrated that music interventions are effective for relieving procedural pain in preterm infants. Our results indicate that music can reduce stress levels and improve blood oxygen saturation. Due to the current limitations, large-scale, prospective RCTs should be performed to validate the present results.

Abstract:Background: Hospitalized patients undergoing surgery or procedures may experience negative symptoms. Music is a nonpharmacological complementary approach and is used as an intervention to reduce anxiety, stress, and pain in these patients. Recently, music has been used conveniently in clinical situations with technology devices, and the mode of providing music is an important factor in technology-based music interventions. However, many reviews have focused only on the effectiveness of music interventions. Objective: We aimed to review randomized controlled trials (RCTs) of technology-based music interventions for reducing anxiety and pain among patients undergoing surgery or procedures. We examined the clinical situation, devices used, delivery methods, and effectiveness of technology-based music interventions in primary articles. Methods: The search was performed in the following 5 electronic databases: PubMed, MEDLINE (OvidSP), CINAHL complete, PSYCINFO, and Embase. This systematic review focused on technology-based music interventions. The following articles were included: (1) RCTs, (2) studies using interactive technology (eg, smartphones, mHealth, tablets, applications, and virtual reality), (3) empirical studies reporting pain and anxiety outcomes, and (4) English articles published from 2018 to 2023 (as of January 18, 2023). The risk of bias was assessed using the Cochrane Risk of Bias tool version 2. Results: Among 292 studies identified, 21 met the inclusion criteria and were included. Of these studies, 9 reported that anxiety scores decreased after music interventions and 7 reported that pain could be decreased before, during, and after procedures. The methodology of the music intervention was important to the results on anxiety and pain in the clinical trials. More than 50% (13/21, 62%) of the studies included in this review allowed participants to select themes themselves. However, it was difficult to distinguish differences in effects depending on the device or software used for the music interventions. Conclusions: Technology-based music interventions could help reduce anxiety and pain among patients undergoing surgery or procedures. The findings of this review could help medical teams to choose a practical methodology for music interventions. Future studies should examine the effects of advanced technology-based music interventions using smart devices and software that promote interactions between medical staff and patients.

Abstract:Labor pain is a physiological condition that can hinder the delivery process. Excessive pain can disturb the body's homeostasis which can result in excessive and useless energy. Reducing the pain scale can be done by fermacological and non-fermacological measures. It's normal for pregnant women and mothers to experience pain. Measuring the pain scale using the Numerical Rating Scale (NRS) and quasi-experiments. Music therapy is a form of implementation that uses music and songs in an integrated and directed manner to guide mothers in pregnancy with the aim that pregnant women feel relaxed, early stimulation of the fetus. Effective intrapersonal communication between a midwife can help a mother overcome depression during pregnancy and childbirth. It can also help patients stay positive throughout the pregnancy and delivery process, making it fun and easy to deal with issues that arise while raising a child. The anxiety level score in the pre-test measurement is higher than the post-test measurement, indicated by the mean value in the pre-test (22.67) higher than the post-test (16.77) which means that there is a decrease in the anxiety level score in the respondents before and after intervention. The results of this study were that there was a significant effect of classical music therapy on reducing the anxiety level of third-trimester pregnant women. This was evident from the statistical test used, the Wilcoxon Match Pairs Test, with a result (p=0.000) less than (<0.05). The researcher's assumption is that music therapy can distract pregnant women so as to reduce maternal anxiety in welcoming labor and effective communication between midwives and pregnant women can motivate pregnant women to be brave in facing labor and reduce anxiety in welcoming labor.

Abstract:Pollen tubes transport two immotile sperm cells in their cytoplasm during growth through the pistil, delivering one to the egg cell to form the zygote and one to the central cell to create the endosperm. Pollen tubes grow through the pistil’s complex extracellular environment and target ovules by following a gradient of small protein attractants, which are secreted by two specialized synergid cells within the ovule. This image shows Arabidopsis thaliana pollen tubes growing in vitro, and illustrates how pollen tubes gain the capacity to locate and enter ovules only after interaction with the pistil tissue. When pollen grains expressing red fluorescent protein (bottom) are placed directly on media in vitro, they exhibit random growth and are unable to target ovules (shown in white; middle). In contrast, when pollen expressing green fluorescent protein (top) is grown through an explant of stigma and style, pollen tubes grow more rapidly and efficiently target and enter ovules. Pollen transcriptional programs are rapidly induced during growth through the pistil, which prepares the pollen tube for interaction with the synergid cells. The molecular components that allow perception of the chemoattractants, which are similarly transcriptionally regulated, are beginning to be identified. Pollen growth in the pistil initiates pollen tube differentiation, maximizing the capacity of pollen to deliver sperm.

Abstract:Background: Music-based intervention has been advocated as a nonpharmacologic approach for the perioperative control of pain and anxiety in surgical patients. However, its impact on patients with preoperative anxiety has not been clearly established. Our study aimed to examine the impact of music-based intervention administered before, during, and after surgery on postoperative opioid consumption and pain levels, as well as preoperative anxiety, depression, and pain catastrophizing. We hypothesized that, compared to a control group, music-based intervention would be effective in reducing opioid requirements and mood disorders. Methods: This study was a single-center, prospective, single-blinded, randomized controlled trial. Inclusion criteria isame-day or observation surgery. Exclusion criteria included American Society of Anesthesiologists physical status IV, use of spinal anesthesia, PROMIS Anxiety T-scores ≤ 57.4 and ≥74.1, preoperative chronic opioid use, transgender surgery, and history of drug or alcohol abuse. Music-based intervention was developed by a certified music therapist. Each patient was randomized to receive standard of care (SC) or SC plus music-based intervention before, during, and after surgery. The primary end point was postoperative oral morphine equivalents (OMEs) over 5 days following surgery using the area under the curve (AUC)Secondary end points were PROMIS Anxiety, PROMIS Depression scores Pain Catastrophizing Scale scores, postoperative nausea and vomiting, time of hospital discharge, and patient satisfaction (0 = totally unsatisfied to 10 = completely satisfied). Results: A total of 75 patients were randomized to a music-based intervention (n = 33) or control (n = 42) group. Patients in the music-based intervention group consumed 56.7% less opioids than those in the control group (AUC was 2.8 in the music-based intervention group vs. 6.4 in the control group, absolute standardized mean difference (aSMD) = 0.34 (−0.17, 0.85)). No difference in pain scores was recorded between groups. Music-based intervention also reduced anxiety on postoperative day (POD)2 (aSMD = 0.38 (−0.16, 0.91)), depression on POD2 (aSMD = 0.31 (−0.23, 0.84)) and POD4 (aSMD = 0.24 (−0.29, 0.77)), and pain catastrophizing on POD1 (aSMD = 0.24 (−0.3, 0.77)). Conclusions: Our data support the use of music-based intervention to reduce postoperative opioid requirements. Music-based intervention may also reduce anxiety, depression, and pain catastrophizing.

Abstract:Labor pain is often severe, and analgesic medication may not be indicated. In this randomized controlled trial we examined the effects of music on sensation and distress of pain in Thai primiparous women during the active phase of labor. The gate control theory of pain was the theoretical framework for this study. Randomization with a computerized minimization program was used to assign women to a music group (n = 55) or a control group (n = 55). Women in the intervention group listened to soft music without lyrics for 3 hours starting early in the active phase of labor. Dual visual analog scales were used to measure sensation and distress of pain before starting the study and at three hourly posttests. While controlling for pretest scores, one-way repeated measures analysis of covariance indicated that those in the music group had significantly less sensation and distress of pain than did the control group (F (1, 107) = 18.69, p <.001, effect size =.15, and F (1, 107) = 14.87, p <.001, effect size =.12), respectively. Sensation and distress significantly increased across the 3 hours in both groups (p <.001), except for distress in the music group during the first hour. Distress was significantly lower than sensation in both groups (p <.05). In this controlled study, music--a mild to moderate strength intervention--consistently provided significant relief of severe pain across 3 hours of labor and delayed the increase of affective pain for 1 hour. Nurses can provide soft music to laboring women for greater pain relief during the active phase when contractions are strong and women suffer.
Clinical benefits of music-based interventions on preterm infants' health: A systematic review of randomised trials

Abstract:Aim: This systematic review aimed to differentiate and isolate the results of different music-based interventions used with preterm infants in the neonatal intensive care unit and explore their clinical benefits. Methods: The last search was performed on 5 July 2021 on Web of Science, Scopus, EMBASE, PsycINFO, CINAHL, LILACS and CENTRAL. Only randomised clinical trials that explored the health benefits of music-based interventions were considered. Results: A total of 39 studies were included. All music-based interventions were divided into music medicine and music therapy. The overall results suggested that music medicine interventions were associated with a significant improvement in pain relief; in turn, improvements in cardiac and respiratory function, weight gain, eating behaviour, and quiet alert and sleep states were more consistent in studies that followed a music therapy approach with the presence of a music therapist. Conclusion: This review supports the beneficial effects of music-based interventions on the health of preterm infants in a neonatal intensive care unit; however, it also offers suggestions for future studies in order to increase the number of interventions with music therapists, since the results of music therapy approaches were more consistent for physiological and behavioural outcomes.

Abstract:Background and aims: Childbirth is a multifaceted and distressing event that can profoundly affect mothers' psychological and physical well-being. Noninvasive interventions like music therapy have been explored to improve labor outcomes by decreasing pain and anxiety levels, promoting relaxation, and enhancing maternal-fetal health. This study aimed to investigate the effect of Iranian traditional music on cervical dilation and labor progress in the active stage of term pregnancy. Methods: This study was a nonrandomized controlled trial that included pregnant women between 20 and 35, with a gravidity of two or three. Participants were assigned to either the intervention group, which received traditional Iranian music therapy during labor, or the control group, which received routine care without the music intervention. The primary outcome was the effect of music therapy on cervical dilation from 4 cm to full dilation (10 cm) during term pregnancy. The secondary outcomes included changes in fetal heart rate, maternal anxiety, and pain scores before and after the intervention. Statistical analyses were performed using appropriate methods, and the results were reported using descriptive and inferential statistics. Results: The mean age of the intervention group was 28.32 ± 6.13 years, and that of the control group was 28.31 ± 3.47 years (p = 0.890). The median duration of labor was not significantly different during the active stage, with 120 min in the intervention group and 137 min in the control group (p = 0.505). However, significant differences were observed in fetal heart rate, anxiety score, and pain score changes before and after the intervention between the two groups (p < 0.05). Conclusion: Using traditional Iranian music therapy as a noninvasive intervention during labor can potentially improve maternal and fetal outcomes by reducing anxiety and pain perception. Further research is needed to explore the potential benefits of traditional music therapy in clinical settings.

Abstract:We investigate the propagation of light impulse in aperiodic systems including Thue-Morse(TM) and Fibonacci sequence. Both refractive index and optical thickness are considered. At the same time, we calculate the transmission in TM sequence with negative refractive index by using the transfer matrix. We find many completely transparent electromagnetic states appear and the states' corresponding frequency distribution has a self-similar structure, it is similar to the normal TM photonic crystals, depending on its crystals structure and around a particular center frequency at the midgap frequency of a quarter-wavelength, the transmission is more sensitive to the optical thickness modulation.

Abstract:Aim Patients undergoing surgery experience perioperative anxiety and pain. Music has been shown to reduce perioperative anxiety, pain and medication requirement. This study assessed the feasibility and effectiveness of implementing a perioperative music intervention. Method A prospective pre‐ and post‐implementation pilot study was conducted to assess adherence to the intervention and the initial effect of music on postoperative pain scores (Numerical Rating Scale, 0–10) compared to a control group. Secondary outcomes encompassed pain scores throughout hospital admission, anxiety levels, medication usage, complications and hospital stay length. Results Adherence to the music intervention was preoperative 95.2%, intraoperative 95.7%, postoperative 31.9% and overall 29.7%. The intervention did influence postoperative pain. Patient's willingness to receive music was high (73%), they appreciated the intervention (median 8.0, interquartile range 7.0–9.0) and healthcare professionals were willing to apply the intervention. Music significantly reduced postoperative anxiety (2.0 vs. 3.0, p = 0.02) and the consumption of benzodiazepines on the first postoperative day (number of patients: zero [0%] vs. five [10%], p = 0.04). Conclusion Implementation of music resulted in reduced postoperative anxiety and decreased consumption of benzodiazepines, and the strategy was feasible, but adjustments are needed to improve postoperative adherence. Both patients and healthcare professionals had a positive attitude towards the intervention.

Investigating the effect of music on labor pain and progress in the active stage of first labor

Abstract:DESIGN AND PURPOSE: The purpose of this study is to investigate the effects of music-therapy on labor pain and progress in parturient primipara. Music-therapy during labor increases tolerance to pain; decreasing anxiety, it increases paturition and uterus activity and shorten labor duration. Subjects and methods: The subjects of this research were 30 women, selected voluntarily and they have been put in two experimental and control group. This research has been conducted in the form of pre-test and post-test design. The experimental group listened to a relaxing music for 30 minutes in each hour for a two-hour period a nd the control group was not exposed to music during this period. For the purpose of gathering data in both groups, the pain scales (verbal, numeric and visual) was used to measure pain. The independent variable in this research is relaxing music and the dependent variables are the pain level and delivery progress. Findings: The independent t for sensations of pain in the experimental and control group before intervention has been (p = 0.875) 0.601 in numeric and visual pain and (p < 0.01) 2.92 in verbal pain, and one hour after intervention, it has been (p < 0.0001) 8.527 in visual and numeric pain and (p < 0.0001) 11.824 in verbal pain. Also, the equal value of independent t for the duration of delivery in control group before and after intervention shows that music has not had any effect on the rate of serotonin. Conclusion: The results of statistical analysis show the effect of music on the decrease of sensation of pain in the experimental group as compared with the control group.
