Abstract:Healthcare is undergoing a transformation, and it is imperative to leverage new technologies to generate new data and support the advent of precision medicine (PM). Recent scientific breakthroughs and technological advancements have improved our understanding of disease pathogenesis and changed the way we diagnose and treat disease leading to more precise, predictable and powerful health care that is customized for the individual patient. Genetic, genomics, and epigenetic alterations appear to be contributing to different diseases. Deep clinical phenotyping, combined with advanced molecular phenotypic profiling, enables the construction of causal network models in which a genomic region is proposed to influence the levels of transcripts, proteins, and metabolites. Phenotypic analysis bears great importance to elucidat the pathophysiology of networks at the molecular and cellular level. Digital biomarkers (BMs) can have several applications beyond clinical trials in diagnostics—to identify patients affected by a disease or to guide treatment. Digital BMs present a big opportunity to measure clinical endpoints in a remote, objective and unbiased manner. However, the use of "omics" technologies and large sample sizes have generated massive amounts of data sets, and their analyses have become a major bottleneck requiring sophisticated computational and statistical methods. With the wealth of information for different diseases and its link to intrinsic biology, the challenge is now to turn the multi-parametric taxonomic classification of a disease into better clinical decision-making by more precisely defining a disease. As a result, the big data revolution has provided an opportunity to apply artificial intelligence (AI) and machine learning algorithms to this vast data set. The advancements in digital health opportunities have also arisen numerous questions and concerns on the future of healthcare practices in particular with what regards the reliability of AI diagnostic tools, the impact on clinical practice and vulnerability of algorithms. AI, machine learning algorithms, computational biology, and digital BMs will offer an opportunity to translate new data into actionable information thus, allowing earlier diagnosis and precise treatment options. A better understanding and cohesiveness of the different components of the knowledge network is a must to fully exploit the potential of it.

Abstract:According to the World Health Organization (WHO), the worldwide obesity rate has tripled since 1975. In Europe, more than half of the population is overweight and obese. Around 2.8 million people die each year worldwide as a result of conditions linked to being overweight or obese. This study aimed to analyze the policies, approaches, and solutions that address the social and health unmet needs of obese patients, at different levels, in order to simulate the definition of an integrated approach, and to provide and share examples of innovative solutions supporting health promotion, disease prevention, and integration of services to improve the collaboration between the different health and care stakeholders involved across the country and in the lives of obese patients. A collaborative approach involving various levels of government and regional experts from different European countries was applied to identify, explore, and evaluate different aspects of the topic, from the innovation perspective and focusing on a European and a regional vision. Currently, people prefer more foods rich in fats, sugars, and salt/sodium than fruits, vegetables, and fiber. This behavior leads to a significant negative impact on their health-related quality of life. Changes in healthcare systems, healthy policy, and approaches to patient care and better implementation of the different prevention strategies between all the stakeholders are needed, taking advantage of the digital transformation of health and care. Such changes can support obese patients in their fight against an unhealthy lifestyle and at the same time reduce healthcare costs.

Abstract:The current state of the development of Medicine today is changing dramatically. Previously, data of the patient's health were collected only during a visit to the clinic. These were small chunks of information obtained from observations or experimental studies by clinicians, and were recorded on paper or in small electronic files. The advances in computer power development, hardware and software tools and consequently design an emergence of miniature smart devices for various purposes (flexible electronic devices, medical tattoos, stick-on sensors, biochips etc.) can monitor various vital signs of patients in real time and collect such data comprehensively. There is a steady growth of such technologies in various fields of medicine for disease prevention, diagnosis, and therapy. Due to this, clinicians began to face similar problems as data scientists. They need to perform many different tasks, which are based on a huge amount of data, in some cases with incompleteness and uncertainty and in most others with complex, non-obvious connections between them and different for each individual patient (observation) as well as a lack of time to solve them effectively. These factors significantly decrease the quality of decision making, which usually affects the effectiveness of diagnosis or therapy. That is why the new concept in Medicine, widely known as Data-Driven Medicine, arises nowadays. This approach, which based on IoT and Artificial Intelligence, provide possibilities for efficiently process of the huge amounts of data of various types, stimulates new discoveries and provides the necessary integration and management of such information for enabling precision medical care. Such approach could create a new wave in health care. It will provide effective management of a huge amount of comprehensive information about the patient's condition; will increase the speed of clinician's expertise, and will maintain high accuracy analysis based on digital tools and machine learning. The combined use of different digital devices and artificial intelligence tools will provide an opportunity to deeply understand the disease, boost the accuracy and speed of its detection at early stages and improve the modes of diagnosis. Such invaluable information stimulates new ways to choose patient-oriented preventions and interventions for each individual case.

Abstract:Myriad digital health interventions, applications, devices and technologies have, and are, being developed to help refine and personalise medicine from the patient, healthcare professional (HCP), healthcare system and industry perspectives. At a gathering of leaders in digital health, discussion included the current landscape of such digital health tools (DHTs), with specific examples from cardiology and respiratory medicine, and both the benefits and sometime downfalls of such tools. While DHTs can help patients and HCPs detect and monitor health conditions, the experts discussed how adoption of DHTs may be hampered by issues such as access to technology; data privacy and security concerns; technology integration into current healthcare systems; cost and reimbursement; and lack of guidelines and regulatory hurdles. The experts suggested solutions to such issues, including wider availability of healthcare 'booths' local to a patient; easy to understand and use phone applications; patient and HCP incentives to use DHTs and clear paths to adoption within a healthcare system. These should help with integration of DHTs into the healthcare system to aid shared decision-making and, ultimately, streamline and personalise healthcare for all.

Abstract:Objective: Solutions for improving management of chronic conditions are under the attention of healthcare systems, due to the increasing prevalence caused by demographic change and better survival, and the relevant impact on healthcare expenditures. The objective of this study was to propose a comprehensive architecture of a mHealth system aimed at boosting the active and informed participation of patients in their care process, while at the same time overcoming the current technical and psychological/clinical issues highlighted by the existing literature. Methods: After having studied the current challenges outlined in the literature, both in terms of technological and human requirements, we focused our attention on some specific psychological aspects with a view to providing patients with a comprehensive and personalized solution. Our approach has been reinforced through the results of a preliminary assessment we conducted on 22 patients with chronic conditions. The main goal of such an assessment was to provide a preliminary understanding of their needs in a real context, both in terms of self-awareness and of their predisposition toward the use of IT solutions. Results: According to the specific needs and features, such as mindfulness and gamification, which were identified through the literature and the preliminary assessment, we designed a comprehensive open architecture able to provide a tailor-made solution linked to specific individuals' needs. Conclusion: The present study represents the preliminary step towards the development of a solution aimed at enhancing patients' actual perception and encouraging self-management and self-awareness for a better lifestyle. Future work regards further identification of pathology-related needs and requirements through focus groups including all stakeholders in order to describe the architecture and functionality in greater detail.

Abstract:Global innovation is expanding rapidly, fuelled by the ongoing development of various communication and data sharing tools. This revolution can also be seen in the creation of specific databases, the result of collaboration between numerous experts from all over the world, representing different nationalities and medical specialities. A striking example of this dynamic is the exponential growth in citations on PUBMED, which currently stands at 36 million, demonstrating a remarkable acceleration in the field of research. In this profusion of research activities, certain medical specialities stand out for their significant involvement. Cardiology, for example, has 525 819 results on PUBMED, closely followed by pneumology with 351 998 results and rheumatology is holding its own with 181 828 results. These impressive figures demonstrate the extent of the efforts made by these disciplines to push back the boundaries of medical knowledge. However, at the heart of this wave of medical innovation lies a crucial question: what about innovation in the management of chronic diseases in rheumatology? Rheumatic diseases, which are often chronic and disabling, require special attention from medical research. While rheumatology is registering a large number of results on PUBMED, it is essential to develop specific advances in the monitoring and management of rheumatic diseases. Abstract:Precision medicine—of which precision prescribing is a core component—is becoming a new frontier in today's healthcare. Both artificial intelligence (AI) and machine learning (ML) have the potential to enhance our understanding of data and therefore our ability to accurately diagnose and treat patients. By leveraging these technologies and processes, we can uncover associations between a person's genomic makeup and their health, identify biomarkers associated with diseases, fine-tune patient selection for clinical trials, reduce costs, and accelerate drug discovery and vaccine development. Although real-world data pose challenges in terms of collection, representation, and missing or inaccurate data sets, the integration of precision medicine into healthcare is critical. Clearly, precision medicine can benefit from health information innovations that empower decision-making at the patient level. Healthcare fusion is an example of an innovative framework and process [K Zhai et al., 2022, pp. 179–192] [K Zhai et al. ECKM 2022, 20(3), pp. 179–192]. Data science and process improvement are also expected to play a role in resource planning and operational efficiency for optimal patient-centered care. Driving this transformation are advances in 'omics' technologies, digital devices, and imaging capabilities, along with an arsenal of powerful analytics tools working across a multitude of institutions and stakeholders. Encompassing this entire ecosystem, medicine will be evidence-based and driven by three key components: (1) Data curation through clinical diagnostics and behavioral apps that capture health and disease states; (2) Individualized solutions driven by advanced data analytics and personalized therapies; and (3) Business models that deliver value and incentivize growth. Abstract:Introduction: Digital transformation and technological innovation which have influenced several areas of social and productive life in recent years, are now also a tangible and concrete reality in the vast and strategic sector of public healthcare. The progressive introduction of digital technologies and their widespread diffusion in many segments of the population undoubtedly represent a driving force both for the evolution of care delivery methods and for the introduction of new organizational and management methods within clinical structures. Methods: The CS Clinical Engineering of the "Spedali Civili Hospital in Brescia" decided to design a path that would lead to the development of a software for the management of biomedical technologies within its competence inside the hospital. The ultimate aim of this path stems from the need of Clinical Engineering Department to have up-to-date, realistic, and systematic control of all biomedical technologies present in the company. "Spedali Civili Hospital in Brescia" is not just one of the most important corporate realities in the city, but it is also the largest hospital in Lombardy and one of the largest in Italy. System development has followed the well-established phases: requirement analysis phase, development phase, release phase and evaluating and updating phase. Results: Finally, cooperation between the various figures involved in the multidisciplinary working group led to the development of an innovative management software called "SIC Brescia". Discussion: The contribution of the present paper is to illustrate the development of a complex implementation model for the digitization of processes, information relating to biomedical technologies and their management throughout the entire life cycle. The purpose of sharing this path is to highlight the methodologies followed for its realization, the results obtained and possible future developments. This may enable other realities in the healthcare context to undertake the same type of pathway inspired by an accomplished model. Furthermore, future implementation and data collection related to the proposed Key Performance Indicators, as well as the consequent development of new operational management models for biomedical technologies and maintenance processes will be possible. In this way, the Clinical Risk Management concept will also be able to evolve into a more controlled, safe, and efficient system for the patient and the user.

Abstract:Advances in Internet of Things (IoT) devices and in Machine Learning (ML) applications can provide valuable insights and predictions on personal health by optimizing data generation and processing.Nevertheless, the flow of data about the health status of a patient brings a variety of technical, legal and economic challenges that need to be addressed through an interdisciplinary approach.In this context, based on the action research methodology, the paper introduces an exemplary health-related activity recognition platform based on IoT, developed as a part of European-funded project Horizon 2020 in collaboration with academia and industry.The platform proposes innovative solutions on how personal healthcare data can be processed and analysed, protecting users' privacy. The main strength of the platform is the interdisciplinary approach used within a triple-helix model, involving a variety of institutions, companies and researchers from different academic fields. In this perspective, the paper shows the potential that the integration of IoT and ML models have to offer and the main challenges that still need to be addressed.

Abstract:Big data and artificial intelligence are currently two of the most important and trending pieces for innovation and predictive analytics in healthcare, leading the digital healthcare transformation. Keralty organization is already working on developing an intelligent big data analytic platform based on machine learning and data integration principles. We discuss how this platform is the new pillar for the organization to improve population health management, value-based care, and new upcoming challenges in healthcare. The benefits of using this new data platform for community and population health include better healthcare outcomes, improvement of clinical operations, reducing costs of care, and generation of accurate medical information. Several machine learning algorithms implemented by the authors can use the large standardized datasets integrated into the platform to improve the effectiveness of public health interventions, improving diagnosis, and clinical decision support. The data integrated into the platform come from Electronic Health Records (EHR), Hospital Information Systems (HIS), Radiology Information Systems (RIS), and Laboratory Information Systems (LIS), as well as data generated by public health platforms, mobile data, social media, and clinical web portals. This massive volume of data is integrated using big data techniques for storage, retrieval, processing, and transformation. This paper presents the design of a digital health platform in a healthcare organization in Colombia to integrate operational, clinical, and business data repositories with advanced analytics to improve the decision-making process for population health management.

Abstract:Heart failure (HF) is a leading cause of morbidity worldwide, imposing a significant burden on deaths, hospitalizations, and health costs. Anticipating patients' deterioration is a cornerstone of HF treatment: preventing congestion and end organ damage while titrating HF therapies is the aim of the majority of clinical trials. Anyway, real-life medicine struggles with resource optimization, often reducing the chances of providing a patient-tailored follow-up. Telehealth holds the potential to drive substantial qualitative improvement in clinical practice through the development of patient-centered care, facilitating resource optimization, leading to decreased outpatient visits, hospitalizations, and lengths of hospital stays. Different technologies are rising to offer the best possible care to many subsets of patients, facing any stage of HF, and challenging extreme scenarios such as heart transplantation and ventricular assist devices. This article aims to thoroughly examine the potential advantages and obstacles presented by both existing and emerging telehealth technologies, including artificial intelligence.

Abstract:The Italian Government planned to invest €15 billion of European funds on National Health Service digitalization and primary care enhancement. The critical burden brought by the pandemic upon hospital care mean these investments could no longer be delayed, considering the extraordinary backlogs of many treatments and the ordinary gaps of fragmented long-term care, in Italy and abroad. National guidelines have been published to standardize interventions across the Italian regions, and telemedicine is frequently mentioned as a key innovation to achieve both goals. The professional resources needed to run the facilities introduced in primary care are defined with great precision, but no details are given on how digitalization and remote care technologies must be implemented in this context. Building on this policy case, this paper focuses on what contribution digitalization and telemedicine can offer to specific primary care innovations, drawing from implemented technology-driven policies which may support the effective stratification, prevention and management of chronic patient needs, including anticipatory healthcare, population health management, adjusted clinical groups, chronic care management, quality and outcomes frameworks, patient-reported outcomes and patient-reported experience. All these policies can benefit significantly from digitalization and remote care technology, provided that some risks and limitations are considered by design.

Abstract:The COVID-19 pandemic exposed and exacerbated longstanding inefficiencies and deficiencies in chronic disease management and treatment in the United States, such as a fragmented healthcare experience and system, narrowly focused services, limited resources beyond office visits, expensive yet low quality care, and poor access to comprehensive prevention and non-pharmacological resources. It is feared that the addition of COVID-19 survivors to the pool of chronic disease patients will burden an already precarious healthcare system struggling to meet the needs of chronic disease patients. Digital health and telemedicine solutions, which exploded during the pandemic, may address many inefficiencies and deficiencies in chronic disease management, such as increasing access to care. However, these solutions are not panaceas as they are replete with several limitations, such as low uptake, poor engagement, and low long-term use. To fully optimize digital health and telemedicine solutions, we argue for the gamification of digital health and telemedicine solutions through a pantheoretical framework—one that uses personalized, contextualized, and behavioral science algorithms, data, evidence, and theories to ground treatments.

Abstract:The interest in new and more advanced technological solutions is paving the way for the diffusion of innovative and revolutionary applications in healthcare organizations. The application of an artificial intelligence system to medical research has the potential to move toward highly advanced e-Health. This analysis aims to explore the main areas of application of big data in healthcare, as well as the restructuring of the technological infrastructure and the integration of traditional data analytical tools and techniques with an elaborate computational technology that is able to enhance and extract useful information for decision-making. We conducted a literature review using the Scopus database over the period 2010–2020. The article selection process involved five steps: the planning and identification of studies, the evaluation of articles, the extraction of results, the summary, and the dissemination of the audit results. We included 93 documents. Our results suggest that effective and patient-centered care cannot disregard the acquisition, management, and analysis of a huge volume and variety of health data. In this way, an immediate and more effective diagnosis could be possible while maximizing healthcare resources. Deriving the benefits associated with digitization and technological innovation, however, requires the restructuring of traditional operational and strategic processes, and the acquisition of new skills.

Abstract:Background: In the real world, medical practice is changing hand in hand with the development of new Artificial Intelligence (AI) systems and problems from different areas have been successfully solved using AI algorithms. Specifically, the use of AI techniques in setting up or building precision medicine is significant in terms of the accuracy of disease discovery and tailored treatment. Moreover, with the use of technology, clinical personnel can deliver a very much efficient healthcare service. Objective: This article reviews AI state-of-the-art in cardiovascular disease management, focusing on diagnostic and therapeutic improvements. Methods: To that end, we conducted a detailed PubMed search on AI application from distinct areas of cardiology: heart failure, arterial hypertension, atrial fibrillation, syncope and cardiovascular rehabilitation. Particularly, to assess the impact of these technologies in clinical decision-making, this research considers technical and medical aspects. Results: : On one hand, some devices in heart failure, atrial fibrillation and cardiac rehabilitation represent an inexpensive, not invasive or not very invasive approach to long-term surveillance and management in these areas. On the other hand, the availability of large datasets (big data) is a useful tool to predict the development and outcome of many cardiovascular diseases. In summary, with this new guided therapy, the physician can supply prompt, individualised, and tailored treatment and the patients feel safe as they are continuously monitored, with a significant psychological effect. Conclusion: Soon, tailored patient care via telemonitoring can improve clinical practice because AI-based systems support cardiologists in daily medical activities, improving disease detection and treatment. However, the physician-patient relationship remains a pivotal step.

Abstract:Abstract Digital and technological solutions (DTS) in cardiovascular nursing are profoundly transforming the landscape of patient care by integrating advanced data-driven approaches. DTS help to enhance patient outcomes and streamline clinical workflows, supporting the shift of the traditional roles of healthcare providers and patients towards more engaged and collaborative care processes. This article presents a perspective in this regard. The adoption of DTS, including mobile health applications and wearable devices, enables continuous monitoring and management of patient health, fostering significant improvements in cardiovascular health management. However, the rapid incorporation of such technologies presents various challenges, such as robust data standardization, the development of digital literacy among healthcare professionals, and addressing privacy and security concerns. Localization of NADPH-diaphorase/nitric oxide synthase activity in the rat cerebellar cortex: a light and electron microscopical study.

Abstract:The activity of reduced nicotinamide adenine dinucleotide phosphate-diaphorase (NADPH-d) was examined histochemically in the rat cerebellar cortex at the light and electron microscopical level. Different staining patterns were observed depending on the technique used. Electron dense reaction product was seen on distinct membrane portions of the endoplasmic reticulum including the nuclear envelope in most of the neurons and in endothelial cells. Electron microscopically no activity staining was seen in glial cells, including Bergmann cells. The addition of the detergent Triton X-100, usually applied in the light microscopical diaphorase histochemistry, led to a striking diminution in membrane staining. In such preparations inspected electron microscopically formazan formed small granules which were evenly distributed over the cytoplasm.

Abstract:Abstract Background This article is focused on the understanding of the key points and their importance and impact on the future of early disease predictive models, accurate and fast diagnosis, patient management, optimise treatment, precision medicine, and allocation of resources through the applications of Big Data (BD) and Artificial Intelligence (AI) in healthcare. Content BD and AI processes include learning which is the acquisition of information and rules for using the information, reasoning which is using rules to reach approximate or definite conclusions and self-correction. This can help improve the detection of diseases, rare diseases, toxicity, identifying health system barriers causing under-diagnosis. BD combined with AI, Machine Learning (ML), computing and predictive-modelling, and combinatorics are used to interrogate structured and unstructured data computationally to reveal patterns, trends, potential correlations and relationships between disparate data sources and associations. Summary Diagnosis-assisted systems and wearable devices will be part and parcel not only of patient management but also in the prevention and early detection of diseases. Also, Big Data will have an impact on payers, devise makers and pharmaceutical companies. BD and AI, which is the simulation of human intelligence processes, are more diverse and their application in monitoring and diagnosis will only grow bigger, wider and smarter. Outlook BD connectivity and AI of diagnosis-assisted systems, wearable devices and smartphones are poised to transform patient and to change the traditional methods for patient management, especially in an era where is an explosion in medical data.

Abstract:Digital innovations provide novel opportunities to individualize a person's care to best match their lifestyle needs and circumstances and to support them as they live their daily lives with diabetes. These innovations also serve to provide actionable data and insights for the care team giving them a "Webb telescope-like" view into their individual self-management journey, allowing them to see what cannot be seen during infrequent and limited office visits, thereby facilitating collaboration and communication to optimize the care plan on a timely basis. Technology advances are enabling diabetes care to transition from episodic, synchronous, primarily in-person care to include synchronous virtual care options and to continuous, on-demand, data-informed, asynchronous digital care better matching the demands of living with a relentless 24/7 chronic condition. In this paper we will discuss the critical elements and considerations in designing and implementing successful diabetes digital health tools in clinical practice.
