Abstract:epiphysis of the left femur within the pelvic cavity with luxation of the left hip (Fig. 1). An ultrasound examination demonstrated a large collection in the lower abdomen, which was seen to communicate with the left hip joint. During surgery, around 250 ml of abscess was drained and a left hip arthrotomy was performed. Any oating, damaged, left femoral capital epiphysis was removed. The uid analysis was negative for acid-fast bacilli. However, Pseudomonas grew on a culture of the uid. Slippage of the capital femoral epiphysis is a wellrecognized occurrence in children and adolescents and is usually found in boys between the ages of 10 and 17 y and in girls between the ages of 8 and 15 y (1). On rare occasions, a slipped epiphysis may occur in a younger child or neonate who has had severe trauma or disorders such as malnutrition, developmental dysphasia of the hip, leg Calvé-Perthes disease, or tuberculosis (2). Trauma is an important precipitating event in slipped epiphysis in infants and young children. Adolescent growth spurt is a well-recognized factor for slipped epiphysis (3). Obesity increases the shearing stress on the growth plate and can lead to slippage of the epiphysis during normal activity (4). The femoral head in the acetabulum is located in a posterior and medical direction with respect to the remainder of the femur. Although a posteromedioinferior slippage of the capital femoral epiphysis is typical, the epiphysis can also move anteriorly and superiorly, or in a valgus orientation. In the present case, the condition could have started as a coxitis secondary to the septicaemia. As there is a documented communication between the hip joint cavity and the iliopsoas bursa, it is possible that the infection spread into the pelvis and lower abdomen. The trauma to the hip, with subsequent reduction of the dislocated hip, could have lead to a traumatic physiolysis and free migration of the femoral capital epiphysis into the pelvic cavity. Septic arthritis can occur by the haematogenous route, contiguous spread, direct implantation and after surgery. Predisposing factors include serious chronic illness, prior antibiotic treatment, immunosuppressive therapy and underlying joint pathology (5). Complications of septic arthritis include avascular necrosis, epiphyseal slip, complete disorganization of the joints, etc. We conclude the rarity of this case, which possibly was a sequelae of septic arthritis and subsequent dislocation owing to trauma resulting in migration of the capital femoral epiphysis.
