Abstract:Avulsion occurs when the tooth is completely knocked out of its alveolar socket. The maxillary central incisors are more vulnerable to avulsion due to their prominent position in the dental arch. This case report describes a successful permanent maxillary incisor replantation in an 11-year-old child. The replanted tooth was stabilized in the socket using orthodontic wire, followed by root canal treatment and composite restoration within a two-week interval. Clinical and radiographic follow-up was done at one and six months. Successful management of an avulsed tooth requires educating the patient about different storage mediums and emergency management after an avulsion. This case report concluded that the avulsed tooth result is highly dependent on the patient's understanding of avulsion and how to approach it.
Sequelae of delayed replantation of maxillary permanent incisors after avulsion: A case series with 24-month follow-up and clinical review

Abstract:Replantation of avulsed incisors in young children is a successful treatment modality. Almost all replanted teeth exhibit ankylosis followed by inflammatory or replacement resorption, as immediate replantation is practically rare. The purpose of the review is to report a series of cases of prolonged delay in replantation of avulsed incisors and discuss its sequelae, leading to different patterns of root resorption after a minimum follow-up period of 24 months. The present case series is a follow-up of five cases of delayed replantation (more than 24 h delay) without any root surface treatment. Extraoral endodontic therapy was performed before replantation. The avulsed teeth were stabilized using an acid-etch composite resin splint for 4 weeks. The patients were followed up at 3, 6, and 12 months interval and half-yearly thereafter, for examination of the replanted teeth clinically and radiographically. After 24-month follow-up, the replanted teeth were evaluated for gingival changes and clinical mobility. The radiographs were evaluated for external root resorption or inflammatory resorption, osseous root replacement, or replacement resorption. The case series concludes that avulsed teeth transported in dry as well as dessicated conditions and replanted after a delay of 24 h have a survival rate of more than 24 months, though there is no promising long-term prognosis. The sequelae in most of the cases are surface resorption followed by inflammatory resorption or resorption due to pulpal infection or replacement resorption.
Reimplantation of an avulsed mature permanent tooth after 6 days: a 1-year follow-up

Abstract:Tooth avulsion in children and adolescents demands emergency treatment to increase the chances of successful reimplantation. The treatment prognosis depends on intrinsic and extrinsic factors, such as the extra-alveolar time until reimplantation, storage medium, handling, and periodontal ligament condition of the avulsed tooth as well as the patient's general health. This case report describes the treatment of an 11-year-old boy who suffered an avulsion of the maxillary right central incisor followed by delayed reimplantation after the tooth was stored in a dry medium for 6 days. Although the tooth was kept in extremely unfavorable conditions, tooth reimplantation was attempted because the patient was young and it was desirable to avoid psychological trauma as well as esthetic and functional problems. The treatment performed followed the guidelines of the International Association of Dental Traumatology for an avulsed tooth with a closed apex and extraoral dry time exceeding 60 minutes. At a 1-year follow-up appointment, the tooth exhibited clinical function, no mobility or pain symptoms, and mild signs of ankylosis. A radiographic image showed resorption by substitution. Although the long-term prognosis is uncertain, this treatment approach was advantageous for the patient because it maintained esthetics, function, and alveolar bone height.
16-Year follow-up of an avulsed maxillary central incisor after replantation following 10-h storage: An unusual case

Abstract:Tooth avulsion is a type of dental injury defined as the complete displacement of a tooth out of the alveolar socket, and the lack of prompt treatment measures can result in the loss of function, poor quality of life, and psychological and social problems. However, several factors may not permit the immediate replantation of an avulsed tooth; therefore, delayed replantation has emerged as an alternative to meet the esthetic, functional, and psychological demands of patients. Here it was described that the successful replantation of an avulsed maxillary central incisor in a 9-year-old boy who presented at the clinic with the tooth stored in unfavorable conditions as dry and then in olive oil-milk mixture almost 10 h after the event. The tooth has remained in its socket healthy for 16 years after treatment. The patient was satisfied with both esthetics and function.
Dissimilar Sequelae Caused by Single Dental Trauma: A Case Report with Long-Term Follow-up

Abstract:Immediate care and accurate diagnosis are essential for treating dental trauma. This report presents a 7-year-old boy suffered a bicycle accident with direct trauma to the mouth and extrusive luxation of teeth #11 and #21. The emergency treatment was performed within the first hour. Clinical examinations showed that both teeth exhibited 3 mm extrusion from comparing their incisal edges to their neighboring teeth, had edematous gingiva, showed grade 3 mobility and were painful to percussion. Radiographic exams showed incomplete root formation, widened periodontal spaces along their entire lengths and absence of bone or root fractures. The treatment consisted of immediate repositioning and semi-rigid splinting. After 15 days, tooth #11 exhibited no clinical signor symptom and tooth #21 was diagnosed with pulp necrosis, with an acute dentoalveolar abscess and intraoral edema. Immediate endodontic treatment was initiated (intracanal dressing with calcium hydroxide paste). At 30 days, another pulp sensibility test was performed on tooth #11, which had a negative response. In the same visit, the intracanal dressing of tooth #21 was replaced. The Ca(OH)2 medication was replaced every 6 months until the formation of an apical barrier. The final dressing consisted of gutta-percha and Grossman's sealer and was placed two and a half years after the first dressing appointment. The extrusive luxation resulted in two different sequelae: complete root obliteration in tooth #11 and anomalous root development after the formation of an apical barrier in tooth #21. The patient received orthodontic treatment after 6 years from the initial treatment with no negative outcome. After 18 years follow-up, both teeth were stabilized with no symptom or sign. It's important to follow the evolution of every trauma case continuously because the sequelae can be unpredictable.
An immediate implant approach to replace failing maxillary anterior dentition due to orthodontically induced severe root resorption

Abstract:Incomplete orthodontic therapy can lead to severe root resorption, resulting in mobile and non-restorable teeth. This clinical report presents the diagnosis, treatment planning and oral rehabilitation of a young woman with failing dentition in the anterior maxilla due to orthodontically induced root resorption. The patient's chief complaint was mobile maxillary anterior teeth 2 years after discontinuing orthodontic treatment. Radiographic and clinical evaluations revealed a missing right first premolar and left premolars and grade III mobility from the right canine to the left lateral incisor. Due to a hopeless prognosis, extraction of the maxillary anterior teeth was planned, followed by grafting procedures. Four implants were immediately placed in the fresh sockets of the canine and central sites, and a removable provisional appliance was delivered to contour the soft tissues involved. The final restorations consisted of two three-unit layered zirconia implant-supported fixed dental prostheses. Well-planned immediate implant therapy and zirconia restorations can successfully replace mobile teeth with severe root resorption caused by external surface resorption from incomplete orthodontic treatment. Combining grafting procedures during implant placement can replace hard tissue lost due to extractions, whereas provisional restorations can re-establish optimal tissue architecture in the aesthetic zone. The present case offers insight into effective strategies for treating non-compliant or uncooperative patients with failing dentition due to orthodontically induced root resorption.
Successful Management of an Extensive Class V Carious Lesion With Dehiscence and Loss of the Attachment Apparatus: A Case Report With Six-Month Follow-Up

Abstract:The present case report presents a rare instance of a Class V carious lesion combined with alveolar dehiscence in a non-vital maxillary central incisor without gingival recession. A 25-year-old male exhibited symptoms, including a blackened tooth (#21), pus discharge, and tenderness, with clinical examination revealing a Class V carious lesion and partial dehiscence of the labial cortical plate, confirmed by radiographic and cone-beam computed tomography (CBCT) imaging. Treatment involved a two-phase approach: endodontic therapy with root canal instrumentation and obturation using a bioceramic sealer, followed by surgical intervention that included carious lesion removal, restoration with resin-modified glass ionomer cement (RMGIC), and application of a platelet-rich fibrin (PRF) membrane. PRF was selected for its regenerative properties, promoting bone healing and tissue repair, and was particularly beneficial in extensive lesions. Post-treatment follow-up at six months demonstrated complete soft tissue healing, reduced probing depth, and significant bone regeneration. This case illustrates the effective management of a complex dental condition through a combined endodontic and periodontal approach enhanced by PRF therapy, yielding favorable clinical and radiographic outcomes.

Abstract:A clinical case of traumatic dental avulsion with endodontics, replantation and bracket fixation is presented. Subsequent follow-up led to extraction of the replanted teeth. The edentulous space was restored with a Maryland bridge. The factors which contribute to a successful tooth replantation as well as choosing the right time to begin orthodontic treatment in the case of tooth avulsion and replantation are described.

Abstract:Two fractured maxillary central incisors were restored via the crown fragment reattachment technique with adhesive systems and composite resin material. A long-term successful outcome was achieved after 4 years of follow-up. The patient presented with fractured maxillary central incisors, and a comprehensive intraoral and radiographic examination revealed that both teeth had extended crown fractures with no pulpal exposure (Class II, Ellis and Davey). The tooth fragments were thoroughly cleaned and inspected before being repositioned. Both fragments were in excellent condition with almost no pieces missing. Therefore, the proposed treatment was fragment reattachment. After verifying the repositioning and adaptation of the fractured fragments, a flowable composite was used to perform the reattachment procedure under rubber dam isolation. With modern dental adhesives, the reattachment of tooth fragments in good condition is considered a more conservative option for restoring tooth morphology, esthetics, and function. Although one tooth showed signs of irreversible pulpitis and required root canal treatment 2 weeks after the restoration, the clinical and radiographic evaluations at the 4-year follow-up visits confirmed the success of the restorative treatment. The restorations showed good functional and esthetic outcomes after 4 years of follow-up.
Assessment of Survival and Success of Patients with Avulsed Tooth Undergoing Tooth Replantation : A Retrospective Study

Abstract:Background: Tooth avulsion cases occurring because of these injuries require immediate treatment to restore and maintain their position and function in the jaws. Once the avulsed tooth is replanted, it requires stabilization by a process known as splinting of teeth along with natural teeth for support. Avoiding the complications forms the mainstay of treatment modalities for tooth avulsion cases. Materials and Methods: Data were collected from 2005 to 2010 of 80 patients retrospectively, reporting to the outpatient department with a chief complaint of avulsion of the permanent tooth due to trauma and accidental injuries. Only those cases of permanent tooth avulsion were included for the study for whom follow-up details of a minimum of 1 year was available. Assessment of the cases was done based on the extraoral time between the avulsion and replantation and based on the type of transporting media. Chi-square test was used to measure the level of significance. P < 0.001 was considered as significant. Results: We found that out of 80 cases of replantation, 48 showed successful results without any pathologic change or mobility at the end of 1-year follow-up. We observed that patients, in which extraoral time was minimal, had maximum number of successful replantation results on 1-year follow-up (P > 0.001). We also observed that only cases where milk was used as a transporting media showed maximum successful results as compared to other groups (P > 0.001). Conclusion: From this study, we conclude that avulsed tooth should be replanted as soon as possible for a good prognosis. Replantation not only restores functional stability but also establishes the esthetics shortly following injury.

Abstract:The therapeutic mainstay in the treatment of acral lentiginous melanoma is excision with suffi cient surgical margins in order to prevent possible local tumor recurrence [ 1 ] . At the transition between the sole and the toes in particular, primary closure is rarely feasible. In addition to full-thickness and split-thickness grafts, options for defect closure include free fl ap repair. Secondary intention healing may also be considered but is relatively time-consuming. Given the tissue loss involved, the excision defects are frequently too deep to allow for skin grafting to be performed during the same procedure. In this context, vacuum sealing can be used to induce granulation [ 2–4 ] . However, this technique may not be practical if the plantar excision defects extend as far as the metatarsophalangeal joints. Here, achieving adequate airtightness, which is a sine qua non for vacuum therapy, is challenging. Herein, we like to introduce the ‘fl ipper’ vacuum seal as a suitable technique for the hermetic sealing of plantar excision defects.

Abstract:The clinical report describes the oral rehabilitation of a 25-year-old male patient who lost lower incisors, right canine, and a significant amount of anterior mandibular bony and soft tissue following severe dentoalveolar trauma due to a car accident. The patient's young age, anterior aesthetic zone in the lower jaw, previous mandibular fracture, extended bony and soft tissue defect hindering ideal three-dimensional implant placement oriented the therapeutic plan towards a staged approach, with several reconstructive surgical procedures before implant rehabilitation. The treatment involved: the labio-buccal vestibule and lingual sulcus deepening to correct the cicatricial shrinkage due to the previous surgical fixation of the mandibular fracture, the vertical guided bony augmentation to regenerate adequate volumes of bone, free gingival graft to achieve sufficient height and thickness of peri-implant soft tissues, and prosthetic-driven surgical procedure to place implants in a good functional and esthetic position. This therapeutic approach restored function and aesthetics and achieved the outcomes' stability at three years of follow-up.

Abstract:Avulsion is a complex injury affecting the pulp, periodontal ligament and the alveolar bone. Avulsed permanent teeth can survive following replantation. However, post-traumatic external root resorption eventually resulting in loss of the traumatized tooth is a frequent finding. After replantation of the avulsed teeth, esthetic requirements can be needed. Treatment options include porcelain laminate veneers, metal-ceramic restorations and all-ceramic crowns as well as minimally invasive procedures such as direct resin composite bonding. This article describes the restoration of avulsed and replanted teeth with direct resin composite laminate veneers. Because of the fact that lost fragments were recovered with the loss of anterior esthetic, and bearing in mind the patient's psychologically affected, we considered direct resin composite laminate veneer restoration of the avulsed and replanted teeth as the best therapeutic option. The patient was satisfied with the final result.

Abstract:Fractured anterior teeth from accidental falls and sports injuries are very common amongst teenagers and young adults. Management of such critical emergencies is time sensitive for preserving the health of traumatized teeth. Timely management also helps in reassuring the patient and instilling confidence in regards to ideal esthetic outcome and patient satisfaction. However, there is no standard treatment that can be applied to all clinical case scenarios and the treatment has to be innovative and adaptive to the specific case. This case report, specifically aims to highlight a clinical case that was managed by re-attaching the fractured segment on a permanent central incisor. A 24-year-old young man, visiting the town for a wedding, walked in with a piece of broken front tooth in his hand. After a clinical and radiographic exam, though presented with advanced treatment options of a crown or a veneer for long term success, patient insisted on keeping the broken piece and "gluing" it back. Would a direct composite restoration be successful as an interim procedure until the patient returns to his primary dentist? And even if it does, would the patient be happy disposing off his broken piece of tooth? This presents a paradox between clinician’s judgement for best outcome and patient satisfaction. The success of direct composite restorations is never disappointing in a skilled clinician's hands. Let's delve into some literature review, step by step management and conclusion of this clinical situation with a WIN-WIN outcome.
Regeneration of Pulp and Radicular Tissues in a Nonvital Avulsed Tooth with Open Apex: A Case Report

Abstract:Aim and background: The management of avulsed immature teeth poses a significant challenge to clinicians, as prolonged dry time before reimplantation may lead to replacement resorption or ankylosis. Case description: Recent studies have shown promising results with the use of an intracanal proprietary pharmaceutical preparation, followed by sealing of the orifice with mineral trioxide aggregate (MTA). Angiography suggested the presence of vascularized tissue after 1 year, indicating successful regeneration of pulpal tissue in the radicular area. Conclusion: Despite an unfavorable crown-root fracture leading to tooth extraction after 9 months, histological examination revealed regular root dentin formation, indicating a favorable outcome. Clinical significance: These findings offer hope for the management of avulsed immature teeth and underscore the importance of early intervention and appropriate treatment selection. Abstract:Dental trauma requires immediate treatment. However, some conventional treatments (such as an implant and/or partial fixed denture) will not always be immediately possible, due to the patient's age (mainly in pediatric cases) or lack of immediate financial resources. In such situations, the technique of adhesive reattachment plays an important role as a treatment option in the interim. This case report describes the use of adhesive fragment reattachment in the rehabilitation of a maxillary central incisor with a complicated crown-root fracture. A 1-year follow-up affirmed that the adhesive reattachment efficiently resolved the crown-root fracture, allowing for the retention of the tooth structure. This more conservative multidisciplinary approach restored function, esthetics, and self-esteem to the patient, while preserving all bone and gingival architecture for future implant treatment.

Abstract:Replacing both missing maxillary anterior teeth is particularly challenging, especially in compromised sockets. This case report describes the management of an 18-year-old female patient, who suffered avulsion of both maxillary central incisors at 7 years of age. This multidisciplinary implant technique, called Immediate Dentoalveolar Restoration (IDR), included extraction of the injured teeth and a single procedure for immediate implant placement and restoration of the compromised sockets after root fracture and periapical lesion development were detected during orthodontic treatment. Successful esthetic and functional outcomes and reestablishment of the alveolar process after bone reconstruction were observed during the 3-year follow-up period. The predictable esthetic outcomes and soft- and hard-tissue stability that can be achieved following IDR are demonstrated.
