Abstract:Background: Transcatheter aortic valve replacement (TAVR) is as an alternative treatment to surgical AVR, but the long-term outcomes of TAVR remain unclear. This study aimed to analyze long-term outcomes following TAVR using meta-analysis. Methods: A literature search was performed with MEDLINE, EMBASE, Cochrane Library, Web of Science, and Google Scholar through December 2022; studies reporting clinical outcomes of TAVR with follow-up periods of {greater than or equal to} 8 years were included. The outcomes of interest were overall survival and/or freedom from structural valve deterioration (SVD). Surgical risk was assessed with the Society of Thoracic Surgeons (STS) predicted risk of mortality (PROM) score. A subgroup analysis was conducted for intermediate-/high-surgical risk patients only. Results: Eleven studies including 5,458 patients were identified and analyzed. The mean age was 82.0 {plus minus} 6.5 years, and mean STS PROM score ranged from 2.9 to 10.6%. Survival rate at 5 and 10 years was 47.7% {plus minus} 1.4% and 12.1 {plus minus} 2.0%. Five studies including 1,509 patients were analyzed for SVD. Freedom from SVD at 5 and 8 years was 95.5 {plus minus} 0.7% and 84.9 {plus minus} 3.1%. Similar results for survival and SVD were noted in the subgroup analysis of intermediate-/high-risk patients. Conclusions: Following TAVR, approximately 88% of patients died within 10 years, whereas 85% were free from SVD at 8 years. These date suggest that baseline patient demographic have the greatest impact on survival, and SVD does not seem to have a prognostic impact in this population. Further investigations on longer-term outcomes of younger and lower-risk patients are warranted.

Abstract:The use of transcatheter aortic valve replacement (TAVR) has expanded as an alternative to surgical aortic valve replacement and has become a widely accepted treatment option for patients with severe aortic stenosis who are considered intermediate- and high-risk surgical candidates. The information concerning the long-term durability of TAV devices is currently limited. The interplay between estimated life expectancy and prosthetic heart valve durability is a key consideration in these discussions. The aim of this study was to determine the clinical outcomes, survival and the factors predicting mortality after TAVR Methods From April 2008 to December 2020, the auto-expandible and baloom-expandable prostheses were implanted in 991 patients with symptomatic severe aortic stenosis with intermediate, high and deemed risk. Results The mean age was 79.43±6.7 years. The logistic EuroSCORE II and STS score were 5.92±5% and 5.22±3.7%, respectively. The implantation success rate was 98.7%. In-hospital mortality was 3.3%, and the combined endpoint of death, vascular complications, myocardial infarction or stroke had a rate of 17.2%. The late mortality (beyond 30 days) was 43.2%. Survival at 1, 3, 5, 7, 9 and ten years were 88.2%, 68.2%, 50,1%, 38.1%, 26.3% and 16.4% respectively, after a mean follow-up of 44.8±31 months. The NYHA functional class improved from 3.3±0.7 to 1.76±0.5 in the follow-up. At 7 years, 9 patients had severe prosthetic valve dysfunction (severe stenosis and moderate transvalvular regurgitationI. In 7 patients were treated again with TAVR. The predictors of cumulative mortality were: Charlson index [HR 1.167 (95% CI 1.078–1.264), p=0.001], Readmision Heart Failure [HR 2.793 (95% CI 1.948–4.005), p=0.001], stroke post-TAVR [HR 2.919 (95% CI 1.965–4.336), p=0.001], Left ventricular ejection fraction [HR −1.012 (95% CI 1.002–1.022) p=0.021] and Karnosfky index [HR −0.984 (95% CI 0.975–0.993), p=0,001] but the requirement of pacemaker or residual aortic regurgitation after TAVR were not predictors [HR 0,979 (CI 95% 0.678–1.415), p 0.911] and [HR 1.087 (CI 95% 0.913–1.294), p=0.349]. Conclusions TAVR is associated with excellent clinical results at follow-up. Survival, predictive factors, and causes of mortality following transcatheter aortic valve implantation

Abstract:Objective: Transcatheter aortic valve implantation (TAVI) is considered the standard of care for patients with severe symptomatic aortic stenosis unsuitable for surgery. However, short- and long-term mortality after TAVI are still relatively high. The aim of this study was to establish survival, predictive factors, and causes of mortality after TAVI at early and midterm follow-up. Methods: Between December 2007 and May 2012, a total of 119 patients with symptomatic severe aortic stenosis underwent 121 TAVI procedures. The mean ± SD age was 81 ± 9 years, and 59% were men. The mean ± SD logistic European System for Cardiac Operative Risk Evaluation was 22 ± 15. Seventy-five patients (63%) were in New York Heart Association functional class III to IV. The transfemoral approach was used in 76% of the patients. One hundred ten patients (91%) had the CoreValve prosthesis, and 11 (9%) had an Edwards SAPIEN valve. Baseline characteristics, procedural complications, and outcomes were collected prospectively. Clinical outcomes were defined according to the Valve Academic Research Consortium criteria. Follow-up was completed for 100% of the patients at a median of 1.3 years (range, 0-4.5). Results: The total number of deaths was 36 (30%). One-month mortality was 4.2%. Actuarial survival was 83.2%, 76.5%, and 68.2% at 1, 2, and 3 years, respectively. Acute kidney injury occurred in 12.3% of the patients, none of whom required dialysis during hospitalization. Twenty-one patients (17.6%) had new conduction abnormalities that required permanent pacemaker implantation before hospital discharge. The incidence of major vascular injury and stroke was 2.4% and 4.1%, respectively. Survival was significantly adversely affected by preprocedural left ventricular dysfunction (P = 0.04), history of atrial fibrillation (P = 0.03), prior heart block (P < 0.01), and critical preoperative state (P < 0.01). Twelve (33%) of the 36 deaths were due to bronchopneumonia. In 12 (33%) of the 36 patients who died, mortality was related to cardiac causes. When a death occurred within the first 30 days, it was mainly cardiac in nature (80%). Twelve patients (34%) died because of a variety of other reasons such as pulmonary embolism, stroke, cancer, renal failure, and sepsis. Conclusions: Preprocedural left ventricular dysfunction, atrial fibrillation, and heart block are independent predictive factors of all-cause mortality. Early mortality was mainly cardiac in origin. Most of the late deaths were caused by noncardiac reasons, with bronchopneumonia being reported as the most common cause of late mortality.

Abstract:Background: Treatment of severe aortic stenosis with transcatheter aortic valve implantation (TAVI) was introduced in 2002. Since then, TAVI has become the primary treatment approach worldwide for advanced-age patients and younger patients with severe comorbidities. We aimed to evaluate the changes in patient demographics, complications, and mortality rates within 13 years. Methods: This retrospective observational study included 867 patients who underwent TAVI at the University Hospital of North Norway in Tromsø from 2008 to 2021. The 13-year period was divided into period 1 (2008–2012), period 2 (2013–2017), and period 3 (2018–2021). The primary objective was to evaluate the changes in periprocedural (30 days), early (30–365 days), and late mortality rates (>365 days) between the periods. The secondary objective was to evaluate late mortality rates by sex and age groups: <70 years, 70–79 years, 80–89 years, and ≥90 years. Results: The periprocedural mortality rates for periods 1, 2, and 3 were 10.3%, 2.9%, and 1.2%, respectively ( P < 0.001). The early mortality rates were 5.6%, 5.8%, and 6.5%, respectively. No significant differences were observed in late mortality by sex or age group (<70, 70–79, and 80–89 years) with a median survival of 5.3–5.6 years. The median survival in patients aged ≥90 years was 4.0 years ( P = 0.018). Conclusion: Our findings indicate that most patients are octogenarians, and the burden of their comorbidities should be highly considered compared to their age when evaluating the procedural outcomes. As the incidence of most complications related to TAVI has decreased, the rates of permanent pacemaker implantation remain high. Important advancements in diagnostics, valve technology, and procedural techniques have improved the periprocedural mortality rates; however, early mortality remains unchanged and poses a clinical challenge that needs to be addressed in the future.

Abstract:Background Transcatheter Aortic valve Replacement (TAVR) has emerged as an alternative to surgical aortic valve replacement. It is widely known the short and mid-term outcomes, however, is limited about long-term outcomes in according to age. The 2021 ESC/EACTS guidelines for the management of valvular heart disease recommended TAVR in older patients ≥75 years or in those who are high risk. The aim of this study was to determine the survival and the factors predicting mortality after TAVR in according to age. Methods From April 2008 to December 2020, a total of 991 patients with symptomatic severe aortic stenosis underwent TAVR with balloon-expandable or self-expanding valves were included. Primary outcomes and survival analysed on base to age <75 years and ≥75 years old. Results The mean age in patients <75 compared with ≥75 years, was 68.2±7.5 vs. 81.4±3.8 years and STS score were 4.24±3.1% vs. 5.4±3.1%. In-hospital mortality was 3.8% vs. 3.1%, (OR= 0.825, 95% confidence interval [CI] 0.034–2.037, p=0.676, and the combined endpoint of death, vascular complications, myocardial infarction, major bleeding or stroke was 13.4% vs. 17.8%, OR = 1.39 95% CI 0.842–2.309) p=0.195, respectively. When compared both groups (75 compared with ≥75 years) in the follow-up, there were no differences in tbleeding 5.7% vs. 3.8% (HR = 0.584 [IC95% 0.278–21.226], p=0.155), myocardial infarction 5% vs. 2.1% (HR = 0.447 [IC95% 0.209–1,090], p=0.08), stroke 5.7% vs. 9.9% (HR = 1.617 [IC95% 0.812–3.219], p=0.171) and mortality 47.2% vs. 44.8% (HR=0.862 [IC95% 0.672–1.105], p=0.241) and there was difference in between groups by hospitalizations for heart failure 16.4% vs. 10.3% (HR = 0.583 [IC95% 0.373–0.912], p=0.018 Survival at 1, 3, 5, and 8 years were similar in both groups (75 compared with ≥75 years): 74.8% vs. 83.4%, 61.1 vs. 69.6%, 45 vs. 51.1% and 35% vs. 31.8% log Rank 1.551, p=0.213, respectively, after a mean follow-up of 44.8±31 months. The predictors of cumulative mortality were in patients <75 years: Charlson index (HR 1.283 [95% CI 1.037–1.587], p=0.022) and STS score (HR 1.224 [95% CI 1.095–1.368], p=0.001) and the predictors in patients with ≥75 years in addition to Charlson index and STS score, were: Stroke postTAVR (HR 2.692 [95% CI 1.380–5.228], p=0.003), Frailty (HR 1.634 [95% CI 1.159–2.034], p=0.005), reduced left ventricular ejection fraction (HR 0.407 [95% CI 0.191–01.867], p=0,020], COPD (HR 1.472 [95% CI 1.043–2.077], p=0,003], mitral regurgitation post-TAVR (HR 1.190 [1.006–1.409], p=0.043. Conclusions TAVR is associated with significant survival benefit throughout 3.29 years of follow-up in both groups. Survival during follow-up was similar in patients with <75 compared with ≥75 years old, but they present some differences in the predictors of mortality, with a greater impact of complications post-TAVR in the elderly. Abstract:Background Transaortic valvular implantation (TAVI) has revolutionised the management of aortic stenosis (AS). Uptake in Asia has caught up in recent years, after an initial lag due to expensive cost, limited subsidies, and scarce accessibility. Hence, longitudinal outcomes of TAVI remain largely unknown in Asian patients. Purpose This study aims to present the 5-year mortality endpoints in an Asian cohort followed up over a decade. Methods 253 patients with symptomatic severe aortic stenosis status post TAVI were retrospectively enrolled at a tertiary academic hospital from 2010-2021. The main outcomes were defined as short-term (30-day), mid-term (1-year) and long-term (5-year) all-cause mortality. Baseline demographics, procedural findings and clinical outcomes were analysed. Kaplan-Meier curves and multiple Cox regression analyses were constructed. Results The mean age was 75.2 + 10.2 years. 120 (47.4%) patients were female and 167 (66%) were Chinese. 168 patients (66.7%) received balloon-expandable valves, 80 patients (31.7%) underwent self-expandable valves, and 4 patients (1.6%) had mechanical-expandable valves. Malignancy was associated with worse short-term mortality at 30-days (HR 3.65, p = 0.054), while pulmonary disease and previous stroke were associated with worse long-term mortality at 5-years (HR 2.27, p = 0.012 and HR 2.02, p = 0.064). Worse all-cause mortality at 30-days, 1-year and 5-years were associated with high NT-Pro BNP levels (HR 1.04, p = 0.046; HR 1.05, p = 0.002 and HR 1.04, p < 0.001), the presence of peripheral arterial disease (PAD) (HR 4.32, p = 0.056; HR 4.48, p = 0.006 and HR 2.43, p = 0.023) and chronic kidney disease (CKD) (HR 6.19, p = 0.006; HR 6.77, p < 0.001 and HR 2.09, p = 0.008). Society of Thoracic Surgery (STS) score (HR 1.11, p = 0.001; HR 1.13, p < 0.001 and HR 1.12, p < 0.001) and Valve Academic Research Consortium (VARC) III endpoints of acute kidney injury (AKI) (HR 6.68, p = 0.047, HR 10.8, p < 0.001 and HR 6.99, p < 0.001), cardiogenic shock (HR 26.3, p < 0.001, HR 12.0, p < 0.001 and HR 4.29, p = 0.011) and structural complications (HR 40.3, p < 0.001, HR 18.1, p < 0.001 and HR 5.18, p = 0.008) were associated with worse all –cause mortality at time-points of 30-days, 1-year and 5-years respectively. On multivariate analyses adjusted for comorbidities and outcomes, STS score and structural complications were independent predictors of 30-day, 1-year, and 5-year all-cause mortality (HR 1.16; p = 0.003, HR 1.09, p = 0.003 and HR 1.10, p < 0.001) and (HR 62.7, p < 0.007; HR 18.9, p < 0.001 and HR 10.6, p = 0.001) respectively. Angiotensin converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) use was protective of survival within 1-year (HR 0.37, p = 0.022). Conclusions STS score and VARC III endpoints of structural complications were reliable predictors of all-cause mortality in Asian patients with TAVI. A Unified Framework for Optimization under Uncertainty TutORials in Operations Research

Abstract:Stochastic optimization, also known as optimization under uncertainty, is studied by over a dozen communities, often (but not always) with different notational systems and styles, typically motivated by different problem classes (or sometimes different research questions) which often lead to different algorithmic strategies. This resulting “jungle of stochastic optimization” has produced a highly fragmented set of research communities which complicates the sharing of ideas. This tutorial unifies the modeling of a wide range of problems, from dynamic programming to stochastic programming to multiarmed bandit problems to optimal control, in a common mathematical framework that is centered on the search for policies. We then identify two fundamental strategies for finding effective policies, which leads to four fundamental classes of policies which span every field of research in stochastic optimization.
Long-Term Prognosis and Predictors of Mortality in Patients Undergoing Transcatheter Aortic Valve Replacement: A Retrospective Analysis

Abstract:BACKGROUND: Aortic valve disease is a common and impactful disorder that imposes significant health burdens and is associated with increased mortality rates. Particularly noteworthy is the emergence of transcatheter aortic valve replacement (TAVR), a minimally invasive procedure that has revolutionized the management of aortic valve disease. However, there remain certain unresolved questions and ongoing research regarding the long-term effectiveness and suitability of TAVR in various patient populations, underscoring the need for further investigation and clinical scrutiny.OBJECTIVE: This retrospective analysis aimed to investigate the long-term outcomes and predictors of mortality in 500 patients who underwent transcatheter aortic valve replacement (TAVR).METHODS: This retrospective analysis included individuals who received transcatheter aortic valve replacement (TAVR) at Sri Venkata Sai (SVS) Medical College, Mahabubnagar, Telangana, India, between January 2020 and July 2023. Demographic characteristics, including age, gender, and comorbidities, were recorded, and long-term outcomes after TAVR were assessed, including the incidence of survival rates and major adverse cardiac events (MACE). Predictors of mortality were also identified using Cox proportional hazards regression analysis.RESULTS: The study group exhibited an average age of 75.6 years (standard deviation (SD): 6.8), with 58% male and 42% female patients. Hypertension (74%), coronary artery disease (CAD) (68%), diabetes mellitus (DM) (42%), and chronic kidney disease (CKD) stage ≥ 3 (36%) were prevalent comorbidities. The median follow-up duration was 5.2 years (interquartile range (IQR): 4.3-6.8 years). The overall long-term survival rate after TAVR was 73.5% (95% confidence interval (CI): 69.8%-77.1%). Additionally, MACE occurred in 21% of patients throughout the follow-up period. The cumulative incidence of MACE at one year, three years, and five years was 6.8% (95% CI: 4.2%-9.5%), 14.2% (95% CI: 10.6%-18.7%), and 21.8% (95% CI: 17.3%-26.7%), respectively. The study found that higher age (hazard ratio (HR): 1.08, 95% CI: 1.04-1.12, p < 0.001), male gender (HR: 1.48, 95% CI: 1.15-1.91, p = 0.002), and the presence of CAD (HR: 1.72, 95% CI: 1.29-2.30, p < 0.001) were linked to an elevated risk of mortality. Additionally, diabetes mellitus (HR: 1.39, 95% CI: 1.05-1.85, p = 0.022) and CKD stage ≥ 3 (HR: 1.96, 95% CI: 1.47-2.61, p < 0.001) emerged as notable predictors of mortality. Conversely, a history of prior coronary artery bypass grafting (CABG) (HR: 0.62, 95% CI: 0.46-0.84, p = 0.003) was associated with a reduced risk of mortality. No significant associations were found between mortality and hypertension (HR: 1.12, 95% CI: 0.88-1.43, p = 0.360) or prior percutaneous coronary intervention (PCI) (HR: 1.21, 95% CI: 0.88-1.67, p = 0.245).CONCLUSION: Age, male gender, CAD, DM, and CKD stage ≥ 3 were significant indicators of mortality risk in TAVR patients. Risk stratification and individualized management are crucial in optimizing long-term outcomes following TAVR procedures.
Three-year Outcomes of Transcatheter Aortic Valve Implantation for Bicuspid Versus Tricuspid Aortic Stenosis.

Abstract:BACKGROUND:Transcatheter aortic valve implantation (TAVI) might be a feasible treatment option for more patients with bicuspid aortic valve (BAV) stenosis. However, long-term follow-up data in this population are scarce.AIMS:The aim of this study was to evaluate three-year outcomes after TAVI in patients with BAV.METHODS:A total of 246 consecutive patients who underwent TAVI at a single centre in China between March 2013 and February 2018 were enrolled in this study. Clinical outcomes, health status and echocardiography were followed and recorded for three years.RESULTS:Among 109 (44.3%) BAV patients, 61.5% were Type 0 and 36.7% were Type 1 BAV patients. BAV patients were younger (75 vs 77 years, p=0.041) and had a lower Society of Thoracic Surgeons (STS) score (5.09 vs 6.00, p=0.026) compared to tricuspid aortic valve (TAV) patients. There were no differences in three-year survival rates between bicuspid and tricuspid patients (87.1% vs 79.5%, log-rank p=0.126). Multivariate Cox regression analysis adjusting for confounding factors revealed a similar risk of all-cause mortality in the BAV population (hazard ratio [HR] 0.86, 95% confidence interval [CI]: 0.44-1.70, p=0.666). Except for the rate of permanent pacemaker implantation that was lower in BAV patients (11.9% vs 21.9%, p=0.041), the incidence of other clinical adverse events was comparable between the two groups. Both BAV and TAV patients showed an obvious improvement in valve haemodynamics, which was sustained for three years. In addition, similar left ventricular reverse remodelling was found during follow-up.CONCLUSIONS:BAV patients showed similar satisfactory three-year clinical outcomes, persistent valve haemodynamics improvement, and obvious cardiac reverse remodelling after TAVI compared to TAV patients.

Abstract:Objective: To determine the 5-year and temporal performance of TAVR versus SAVR. Background: TAVR has become a valuable treatment for severe aortic stenosis but the long-term safety and efficacy remain unclear. Methods: Databases were searched until October 6, 2019 for randomized trials with >= 5 years' follow-up. Primary outcome was all-cause mortality. Odds ratios (ORs) with 95% confidence intervals (CIs) were pooled with random-effects models. Results: We included 4 trials with 3,758 patients. TAVR was associated with a significantly higher 5-year all-cause mortality than SAVR (OR, 1.19; 95% CI, 1.03-1.37; P = 0.02). Landmark analysis showed no significant difference within 2 years (OR, 0.92; 95% CI, 0.79-1.08; P = 0.33) but a statistically higher mortality in TAVR between 2 and 5 years (OR, 1.32; 95% CI, 1.14-1.52; P = 0.0002), with significant difference between these 2 temporal phases (P for interaction = 0.001). Similar interaction was found for cardiovascular mortality and several other outcomes. Rates of all-cause mortality or disabling stroke, permanent pacemaker implantation, aortic-valve rehospitalization, and reintervention were higher, but rates of major bleeding and new-onset fibrillation were lower in TAVR at 5 years. The incidences of myocardial infarction, stroke, and transient ischemic attack were not statistically different between TAVR and SAVR. Conclusions: TAVR was associated with a significantly higher all-cause mortality at 5 years compared with SAVR. Of note, all-cause mortality presented a characteristic temporal pattern showing increased risk between 2 and 5 years but not within 2 years. Longer-term follow-up data are warranted.
Extent of Cardiac Damage and Mortality in Patients Undergoing Transcatheter Aortic Valve Implantation

Abstract:(1) Aims: We sought to assess the impact of the extent of cardiac damage on survival among real-world patients with severe aortic stenosis (AS) undergoing transcatheter aortic valve implantation (TAVI). (2) Methods: A staging classification was applied to 262 patients from the EffecTAVI Registry at baseline and re-assessed within 30-days after TAVI. The primary endpoint of the study was all-cause mortality at 1-year. Secondary endpoints included cerebrovascular accident, myocardial infarction, permanent pacemaker implantation, endocarditis, and re-hospitalization for all causes. (3) Results: At baseline, 23 (8.7%) patients were in Stage 0/1 (no cardiac damage/left ventricular damage), 106 (40.4%) in Stage 2 (left atrial or mitral valve damage), 59 (22.5%) in Stage 3 (pulmonary vasculature or tricuspid valve damage) and 74 (28.3%) in Stage 4 (right ventricular damage). At 30-days after TAVI, a lower prevalence of advanced stages of cardiac damage than baseline, mainly driven by a significant improvement in left ventricular diastolic parameters and right ventricular function, was reported. At 1-year, a stepwise increase in mortality rates was observed according to staging at baseline: 4.3% in Stage 0/1, 6.6% in Stage 2, 18.6% in Stage 3 and 21.6% in Stage 4 (p = 0.08). No differences were found in secondary endpoints. (4) Conclusions: TAVI has an early beneficial impact on the left ventricular diastolic and right ventricular function. However, the extent of cardiac damage at baseline significantly affects the risk of mortality at 1-year after the procedure.

Trends in Transcatheter and Surgical Aortic Valve Replacement Among Older Adults in the United States

Abstract:Background: Recent trends, including survival beyond 30 days, in aortic valve replacement (AVR) following the expansion of indications for transcatheter aortic valve replacement (TAVR) are not well-understood. Objectives: The authors sought to characterize the trends in characteristics and outcomes of patients undergoing AVR. Methods: The authors analyzed Medicare beneficiaries who underwent TAVR and SAVR in 2012 to 2019. They evaluated case volume, demographics, comorbidities, 1-year mortality, and discharge disposition. Cox proportional hazard models were used to assess the annual change in outcomes. Results: Per 100,000 beneficiary-years, AVR increased from 107 to 156, TAVR increased from 19 to 101, whereas SAVR declined from 88 to 54. The median [interquartile range] age remained similar from 77 [71-83] years to 78 [72-84] years for overall AVR, decreased from 84 [79-88] years to 81 [75-86] years for TAVR, and decreased from 76 [71-81] years to 72 [68-77] years for SAVR. For all AVR patients, the prevalence of comorbidities remained relatively stable. The 1-year mortality for all AVR decreased from 11.9% to 9.4%. Annual change in the adjusted odds of 1-year mortality was 0.93 (95% CI: 0.92-0.94) for TAVR and 0.98 (95% CI: 0.97-0.99) for SAVR, and 0.94 (95% CI: 0.93-0.95) for all AVR. Patients discharged to home after AVR increased from 24.2% to 54.7%, primarily driven by increasing home discharge after TAVR. Conclusions: The advent of TAVR has led to about a 60% increase in overall AVR in older adults. Improving outcomes in AVR as a whole following the advent of TAVR with increased access is a reassuring trend.

Abstract:Background Transcatheter aortic valve implantation (TAVI) is the accepted state-of-the-art treatment for elderly patients suffering from severe symptomatic aortic stenosis (AS). Co-morbidities at baseline are of great impact not only for individual peri-procedural risk stratification but also for the determination of long-term prognosis. The latter is of certain clinical interest, since a variety of co-existing disorders can be effectively treated in addition to TAVI. Purpose The current study aimed to elucidate the prognostic value of a wide range of baseline characteristics and co-morbidities with respect to long-term survival of TAVI patients in a prospective real-world single-center registry study. Methods A total of 445 patients with severe AS, which were treated by transfemoral TAVI, were included. A wide range of clinical, laboratory, functional and imaging parameters were prospectively assessed at baseline prior to TAVI. Mortality was recorded at 30 days, 1 year and 2 years after TAVI. Results The mean age of patients in this typical TAVI cohort was 78.7±7.3 years, 52% were female and the mean STS score was 5±3.9%. The mortality rates were as follows: 3.6% after 30 days, 16.4% after 1 and 22.6% after 2 years. Multivariate analysis could identify the following independent predictors of 2-year mortality assessed at baseline: sex, age, AS entity other than high-gradient, atrial fibrillation (Afib), renal function, relevant TR, systolic pulmonary artery pressure (PAPsys) and six-minute walk distance (SMWD). Among those, the strongest predictive value could be shown for Afib (OR 2.505, CI 1.509–4.157, p<0.001) and TR (OR 2.179, CI 1.105–4.299, p=0.025). Kaplan-Meier survival analysis displayed significantly worse 2-year survival rates in patients suffering from relevant TR (31.6% versus 17.4%, p<0.001) and Afib (29.4% versus 14.8%, p<0.001). Conclusions Taken together, the results of the current study demonstrate the prognostic value of cardiovascular co-morbidities assessed prior to TAVI. We identified relevant TR and Afib as the strongest independent predictors of long-term mortality in our cohort. Since both conditions are effectively treatable, special emphasis should be placed on the question, which patient might benefit from treatment, e.g., by transcatheter edge-to-edge repair of TR or rhythm control, in addition to TAVI. Abstract:Background Symptomatic patients with severe aortic stenosis (AS) at high risk for surgical aortic valve replacement (SAVR) sustain comparable improvements in health status over 5 years after transcatheter aortic valve replacement (TAVR) or SAVR. Whether a similar long-term benefit is observed among intermediate-risk AS patients is unknown. Objectives The purpose of this study was to assess health status outcomes through 5 years in intermediate risk patients treated with a self-expanding TAVR prosthesis or SAVR using data from the SURTAVI (Surgical Replacement and Transcatheter Aortic Valve Implantation) trial. Methods Intermediate-risk patients randomized to transfemoral TAVR or SAVR in the SURTAVI trial had disease-specific health status assessed at baseline, 30 days, and annually to 5 years using the Kansas City Cardiomyopathy Questionnaire (KCCQ). Health status was compared between groups using fixed effects repeated measures modelling. Results Of the 1,584 patients (TAVR, n = 805; SAVR, n = 779) included in the analysis, health status improved more rapidly after TAVR compared with SAVR. However, by 1 year, both groups experienced large health status benefits (mean change in KCCQ-Overall Summary Score (KCCQ-OS) from baseline: TAVR: 20.5 ± 22.4; SAVR: 20.5 ± 22.2). This benefit was sustained, albeit modestly attenuated, at 5 years (mean change in KCCQ-OS from baseline: TAVR: 15.4 ± 25.1; SAVR: 14.3 ± 24.2). There were no significant differences in health status between the cohorts at 1 year or beyond. Similar findings were observed in the KCCQ subscales, although a substantial attenuation of benefit was noted in the physical limitation subscale over time in both groups. Conclusions In intermediate-risk AS patients, both transfemoral TAVR and SAVR resulted in comparable and durable health status benefits to 5 years. Further research is necessary to elucidate the mechanisms for the small decline in health status noted at 5 years compared with 1 year in both groups. Abstract:Background Aortic stenosis (AS) is a progressive disease, which untreated leads to irreversible myocardial damage and potentially death. Patients with AS may present with acute decompensation (ADAS) even during active surveillance. Purpose In this TAVI-NOR sub-study, we investigate the clinical profile, risk factors, epidemiology and long-term outcomes of patients treated with TAVI after presenting with ADAS compared to stable patients who underwent elective TAVI (no ADAS). Method A total of 600 patients with severe AS consecutively treated with TAVI between January 2012 and July 2019 were enrolled in TAVI-NOR study. Predictors and long-term clinical outcomes were explored in binary logistic and Cox regression models. Results A total of 68 patients (11.3%) received urgent TAVI due to acute hospitalization with ADAS. Age (80±8.0 years vs 81±6.0 years, p=0.662), gender (56% males vs 50% males, p=0.361) and other comorbidities at baseline such as cardiovascular disease, chronic lung disease and atrial fibrillation (AF) were comparable in patients with ADAS and no ADAS (n=532) (p>0.05 for all). However, patients with ADAS had greater symptom burden (NYHA III-IV 81% vs 51%, p<0.001), lower blood pressure, higher heart rate and lower estimated glomeruli filtration rate (52.0±10.8 vs 54.6±9.8 mL/min/1.73m2, p<0.045). They had also higher prevalence of ECG left ventricular (LV) hypertrophy (40% vs 27%, p=0.027), more often reduced LV ejection fraction (<50%) (41% vs 16%, p<0.001), lower stroke volume (39±11 ml/m2 vs 42±11 ml/m2 vs, p=0.030) a trend of abnormal ECG (p=0.055) and longer hospital stay (11.5 ±8.7 days 6.6± 4.9 days, p<0.001). Patients with ADAS were comparable with no ADAS in terms of early short-term ( 0.05). Although 1-year mortality was comparable (1.5% in ADAS vs 2.1% in no ADAS, p>0.99), mid- to late-term mortality rates were significantly higher in patient with ADAS receiving urgent TAVI, with 3-, 5- and 7 years mortality of 22% vs 11%, p=0.008; 46% vs 29%, p=0.005; 53% vs 39%, p=0.033, respectively. In a Kaplan-Meier analysis, event-free survival was significantly reduced for patients with ADAS undergoing urgent TAVI compared with stable patients undergoing elective TAVI HR 1.63, 95% CI 1.15-2.31, p=0.006), confirmed by a multivariable-adjusted model (HR 1.54; 95% CI 1.05-2.25, p=0.026) independent of age, gender, AF, LV ejection fraction and time from diagnosis to TAVI (Fig). Conclusion Patients with AS presenting with acute decompensation and treated with urgent TAVI had excellent short-term survival. This was irrespective of increased symptom burden, adversely remodeled and functionally reduced LV at baseline. These findings might explain the observed higher mid- and long-term mortality compared with stable patients undergoing elective TAVI. It is essential to identify patients at risk of acute decompensation and treat them timely before irreversible myocardial damage occur.

Abstract:Background: We aimed to perform a meta-analysis of randomized trials comparing long-term outcomes of patients undergoing transcatheter aortic valve replacement (TAVR) vs surgical aortic valve replacement (SAVR) for severe aortic stenosis. The short-term efficacy and safety of TAVR are proven, but long-term outcomes are unclear. Methods: We included randomized controlled trials comparing TAVR vs SAVR at the longest available follow-up. The primary end point was death or disabling stroke. Secondary end points were all-cause mortality, cardiac mortality, stroke, pacemaker implantation, valve thrombosis, valve gradients, and moderate-to-severe paravalvular leaks. The study is registered with PROSPERO (CRD42023481856). Results: Seven trials (N = 7785 patients) were included. Weighted mean trial follow-up was 5.76 ± 0.073 years. Overall, no significant difference in death or disabling stroke was observed with TAVR vs SAVR (HR, 1.02; 95% CI, 0.93-1.11; P = .70). Mortality risks were similar. TAVR resulted in higher pacemaker implantation and moderate-to-severe paravalvular leaks compared to SAVR. Results were consistent across different surgical risk profiles. As compared to SAVR, self-expanding TAVR had lower death or stroke risk (P interaction = .06), valve thrombosis (P interaction = .06), and valve gradients (P interaction < .01) but higher pacemaker implantation rates than balloon-expandable TAVR (P interaction < .01). Conclusions: In severe aortic stenosis, the long-term mortality or disabling stroke risk of TAVR is similar to SAVR, but with higher risk of pacemaker implantation, especially with self-expanding valves. As compared with SAVR, the relative reduction in death or stroke risk and valve thrombosis was greater with self-expanding than with balloon-expandable valves.

Abstract:Background: Transcatheter aortic valve replacement (TAVR) being currently employed in low surgical risk patients with severe symptomatic aortic stenosis (AS). The durability and extended outcomes of TAVR as compared to surgical aortic valve replacement (SAVR) in low-risk patients remains uncertain. Methods: We selected randomized controlled trials (RCT) comparing outcomes of TAVR vs. SAVR in low surgical risk patients having severe AS using online databases. The primary outcome was all-cause death. The secondary outcomes were composite of all-cause death & disabling stroke, cardiovascular (CV) death, stroke, myocardial infarction (MI), permanent pacemaker (PPM) placement, new onset atrial fibrillation (AF), valve re-intervention and valve thrombosis. The outcomes were stratified at short- (1-year) and intermediate-term (≤5 years) follow-up. We used a random effect model to report outcomes as relative risk (RR) with a 95 % confidence interval (CI). Results: The analysis consisted of six RCTs comprising 5,122 subjects with a mean age of 75.4 years. At short-term follow up, there was a significant reduction in all-cause death (RR: 0.62, 0.46-0.82, p = 0.001) and composite of all-cause death and disabling stroke (RR: 0.62, 0.45-0.83, p = 0.002) in patients undergoing TAVR. At intermediate-term follow-up, there was no significant difference in survival (RR:0.95, 0.73-1.24, p = 0.71) and composite outcome (RR: 0.95, 0.74-1.22, p = 0.71). TAVR patients had lower incidence of new onset AF, however, higher PPM placement. Conclusion: In patients with severe AS having low-surgical risk, patients undergoing TAVR had improved short-term survival as compared to SAVR. This survival advantage was absent at intermediate-term follow-up. The long-term outcomes remain uncertain.

Abstract:Background/Objective: Transcatheter aortic valve replacement (TAVR) is indicated for severe aortic stenosis patients with a prohibitive surgical risk. However, its use has been expanding in recent years to include intermediate- and low-risk patients. Thus, registry data describing changes in patient characteristics and outcomes are needed. The aim of this study was to analyse the temporal changes in patient profiles and clinical outcomes of all-comer TAVR. Methods: Baseline characteristics and VARC-3 outcomes of 1632 consecutive patients undergoing TAVR between 2008 and 2021 were analysed. Results: The annual rate of TAVR increased from 30 procedures in 2008–2009 to 398 in 2020–2021. Over the follow-up period, patient age decreased from 85 ± 4 to 80 ± 6.8 (p < 0.001) and the STS score decreased from 5.9% to 2.8% (p < 0.001). Procedural characteristics significantly changed, representing a shift into a minimally invasive approach: adoption of local anaesthesia (none to 48%, p < 0.001) and preference of transfemoral access (74% in 2011–2012 vs. 94.5% in 2020–2021, p < 0.001). The rates of almost all procedural complications decreased, including major vascular and bleeding complications, acute kidney injury (AKI) and in-hospital heart failure. There was a striking decline in rates of complete atrioventricular block (CAVB) and the need for a permanent pacemaker (PPM). PPM rates, however, remain high (17.8%). Thirty-day and one-year mortality significantly declined to 1.8% and 8.3%, respectively. Multivariable analysis shows that AKI, bleeding and stroke are strong predictors of one-year mortality (p < 0.001). Conclusions: The TAVR procedure has changed dramatically during the last 14 years in terms of patient characteristics, procedural aspects and device maturity. These shifts have led to improved procedural safety, contributing to improved short- and long-term patient outcomes.

Abstract:An effective multiport measurement system for the characterization of N-port nonlinear microwave devices is proposed. The proposed measurement system has the capability of measuring the S-parameters of linear microwave circuits and the amplitude-modulation-to-amplitude-modulation (AM/AM) and amplitude-modulation-to-phase-modulation (AM/PM) conversions for fundamental and harmonic frequencies for nonlinear microwave devices on single-step measurement without any need for reconnection or change in the calibration technique or measurement setup. It is found suitable that the proposed system can accurately quantify the impact of crosstalk on the performance of multibranch amplifiers and multiport nonlinear microwave systems in terms of power efficiency and signal distortion.
