Abstract:Abstract Children with cleft lip and/or palate show early delays in speech and vocabulary development that may have an impact on later communication and social development. While delays in the complexity of babbling may put children at risk for later delays in speech and language development, there is considerable variability in development. This study focused on the rate of children's communication acts, canonical vocalizations, and word use as they made the transition from the pre-linguistic to linguistic development. The study included 15 children with non-syndromic cleft lip and/or palate who were seen at three time points between 17–34 months age. Communication rates were calculated from parent–child language samples collected during play activities. Assignment to linguistic stages was based on the children's expressive vocabulary, as reported on the MacArthur Communicative Development Inventory: Words and Sentences. From the pre-linguistic to linguistic level, the children's average rate per minute of: communicative acts overall increased significantly from 1.49 to 3.07 per minute; canonical vocalizations from 0.21 to 0.90 per minute; and word use from 0.16 to 3.61 per minute. Rates of communicative acts were associated with later word use. It appears that children with clefts rely on non-verbal communicative acts when verbal development is delayed.

Abstract:Pneumonia is regarded as the top killer of children amongst all other infectious diseases by causing nearly 700,000 deaths to children aged under five every year. The list also includes the elderly (aged 65 and over) in addition to people with pre-existing health issues. However, detection of pneumonia at early stage has a huge impact on saving lives. Chest X-rays imaging technique is typically used for the identification of this disease. Nonetheless, examination of pneumonia is not a straightforward task, not even for an expert radiologist. Consequently, there is always an imperative need for automated diagnosis of pneumonia to assist radiologists confirm their diagnosis. This paper introduces a novel pneumonia classification methodology of Chest X ray images by integrating three optimized pretrained CNN models with XGBoost algorithm where the learned features from the three models are combined and fed as input to the XGBoost classifier. It is employed as an ensemble strategy method to learn the inherent structure of the combined features from each pretrained model and provide the final classification. Furthermore, Bayesian optimization is utilized to unlock the ultimate feature representation of each CNN model by searching for the optimal structural and learning-based hyperparameters, including the number of initial layers that should remain frozen to avoid loss of the acquired generic features as well as modifying the structure of the classification part together with the last activation and pooling layers responsible for delivering the necessary features for the XGBoost classifier. The obtained results demonstrate that the proposed methodology, defined by the modified versions of different CNN models with XGBoost achieved promising performance in comparison with state-of-the art methods with a correct classification rate of 99.15%, 99.53% for precision, 99.30% for sensitivity and AUC equals 0.9972%.

Abstract:Lung diseases such as COVID-19, tuberculosis (TB), and pneumonia continue to be serious global health concerns that affect millions of people worldwide. In medical practice, chest X-ray examinations have emerged as the norm for diagnosing diseases, particularly chest infections such as COVID-19. Paramedics and scientists are working intensively to create a reliable and precise approach for early-stage COVID-19 diagnosis in order to save lives. But with a variety of symptoms, medical diagnosis of these disorders poses special difficulties. It is essential to address their identification and timely diagnosis in order to successfully treat and prevent these illnesses. In this research, a multiclass classification approach using state-of-the-art methods for deep learning and image processing is proposed. This method takes into account the robustness and efficiency of the system in order to increase diagnostic precision of chest diseases. A comparison between a brand-new convolution neural network (CNN) and several transfer learning pre-trained models including VGG19, ResNet, DenseNet, EfficientNet, and InceptionNet is recommended. Publicly available and widely used research datasets like Shenzen, Montogomery, the multiclass Kaggle dataset and the NIH dataset were used to rigorously test the model. Recall, precision, F1-score, and Area Under Curve (AUC) score are used to evaluate and compare the performance of the proposed model. An AUC value of 0.95 for COVID-19, 0.99 for TB, and 0.98 for pneumonia is obtained using the proposed network. Recall and precision ratings of 0.95, 0.98, and 0.97, respectively, likewise met high standards.

Abstract:Pneumonia has long been a significant concern in global public health. With the advancement of convolutional neural networks (CNNs), new technological methods have emerged to address this challenge. However, the application of CNNs to pneumonia diagnosis still faces several critical issues. First, the datasets used for training models often suffer from insufficient sample sizes and imbalanced class distributions, leading to reduced classification performance. Second, although CNNs can automatically extract features and make decisions from complex image data, their interpretability is relatively poor, limiting their widespread use in clinical diagnosis to some extent. To address these issues, a novel weighted cross-entropy loss function is proposed, which calculates weights via an inverse proportion exponential function to handle data imbalance more efficiently. Additionally, we employ a transfer learning approach that combines pretrained CNN model parameter fine-tuning to improve classification performance. Finally, we introduce the gradient-weighted class activation mapping method to enhance the interpretability of the model's decisions by visualizing the image regions of focus. The experimental results indicate that our proposed approach significantly enhances CNN performance in pneumonia diagnosis tasks. Among the four selected models, the accuracy rates improved to over 90%, and visualized results were provided.

Abstract:Pneumonia is a common lung disease that is the leading cause of death worldwide. It primarily affects children, accounting for 18% of all deaths in children under the age of five, the elderly, and patients with other diseases. There is a variety of imaging diagnosis techniques available today. While many of them are becoming more accurate, chest radiographs are still the most common method for detecting pulmonary infections due to cost and speed. A convolutional neural network (CNN) model has been developed to classify chest X-rays in JPEG format into normal, bacterial pneumonia, and viral pneumonia. The model was trained using data from an open Kaggle database. The data augmentation technique was used to improve the model's performance. A web application built with NextJS and hosted on AWS has also been designed. The model that was optimized using the data augmentation technique had slightly better precision than the original model. This model was used to create a web application that can process an image and provide a prediction to the user. A classification model was developed that generates a prediction with 78 percent accuracy. The precision of this calculation could be improved by increasing the epoch, among other subjects. With the help of artificial intelligence, this research study was aimed at demonstrating to the general public that deep-learning models can be created to assist health professionals in the early detection of pneumonia.

Abstract:Pneumonia is a life-threatening disease, which occurs in the lungs caused by either bacterial or viral infection. It can be life-endangering if not acted upon at the right time and thus the early diagnosis of pneumonia is vital. The paper aims to automatically detect bacterial and viral pneumonia using digital x-ray images. It provides a detailed report on advances in accurate detection of pneumonia and then presents the methodology adopted by the authors. Four different pre-trained deep Convolutional Neural Network (CNN): AlexNet, ResNet18, DenseNet201, and SqueezeNet were used for transfer learning. A total of 5247 chest X-ray images consisting of bacterial, viral, and normal chest x-rays images were preprocessed and trained for the transfer learning-based classification task. In this study, the authors have reported three schemes of classifications: normal vs. pneumonia, bacterial vs. viral pneumonia, and normal, bacterial, and viral pneumonia. The classification accuracy of normal and pneumonia images, bacterial and viral pneumonia images, and normal, bacterial, and viral pneumonia were 98%, 95%, and 93.3%, respectively. This is the highest accuracy, in any scheme, of the accuracies reported in the literature. Therefore, the proposed study can be useful in more quickly diagnosing pneumonia by the radiologist and can help in the fast airport screening of pneumonia patients.

Efficacy and Safety of the Oral Neuraminidase Inhibitor Oseltamivir in Treating Acute Influenza

Abstract:ContextPrevious studies have shown oseltamivir, a neuraminidase inhibitor, to be effective in preventing influenza and treating experimental influenza.ObjectiveTo evaluate the efficacy and safety of oseltamivir in the treatment of naturally acquired influenza infection.DesignRandomized, placebo-controlled, double-blind study conducted January through March 1998.SettingSixty primary care and university health centers throughout the United States.ParticipantsA total of 629 healthy nonimmunized adults aged 18 to 65 years with febrile respiratory illness of no more than 36 hours' duration with temperature of 38°C or more plus at least 1 respiratory symptom and 1 constitutional symptom.InterventionsIndividuals were randomized to 1 of 3 treatment groups with identical appearing pills: oral oseltamivir phosphate, 75 mg twice daily (n = 211) or 150 mg (n = 209) twice daily, or placebo (n = 209).Main Outcome MeasuresDuration and severity of illness in individuals infected with influenza.ResultsTwo individuals withdrew before receiving medication and were excluded from further analyses. A total of 374 individuals (59.6%) were infected with influenza. Their duration of illness was reduced by more than 30% with both oseltamivir, 75 mg twice daily (median, 71.5 hours; P<.001), and oseltamivir, 150 mg twice daily (median, 69.9 hours; P = .006), compared with placebo (median, 103.3 hours). Severity of illness was reduced by 38% (median score, 597 score-hours; P<.001) with oseltamivir, 75 mg twice daily, and by 35% (median score, 626 score-hours; P<.001) with oseltamivir, 150 mg twice daily, vs placebo (median score, 963 score-hours). Oseltamivir treatment reduced the duration of fever and oseltamivir recipients returned to usual activities 2 to 3 days earlier than placebo recipients (P≤.05). Secondary complications such as bronchitis and sinusitis occurred in 15% of placebo recipients compared with 7% of combined oseltamivir recipients (P = .03). Among all 629 subjects, oseltamivir reduced illness duration (76.3 hours and 74.3 hours for 75 mg and 150 mg, respectively, vs 97.0 hours for placebo; P = .004 for both comparisons) and illness severity (686 score-hours and 629 score-hours for 75 mg and 150 mg, respectively, vs 887 score-hours for placebo; P<.001 for both comparisons). Nausea and vomiting occurred more frequently in both oseltamivir groups (combined, 18.0% and 14.1%, respectively; P = .002) than in the placebo group (7.4% and 3.4%; P<.001).ConclusionsOur data suggest that oral oseltamivir treatment reduces the duration and severity of acute influenza in healthy adults and may decrease the incidence of secondary complications.

Abstract:Since the appearance of COVID-19, the accurate diagnosis of pneumonia-type lung diseases by chest radiographs has been a challenging task for experts, mainly due to the similarity of patterns between COVID-19 and viral or bacterial pneumonia. To address this challenge, a model for the computer-aided diagnosis of chest X-Rays has been developed in this research. This model might contribute to substantially increasing the accuracy of the diagnosis. This approach is based on supervised learning using neural networks, where the quality of the result depends on the quality of the dataset used during training. Image data augmentation techniques, hyperparameter adjustments and dropout layer contributed to achieve high performance values on test data in multi-class classification. The experiments conducted to evaluate the model yielded that it detects and classifies domain classes with an accuracy of 99.45% on training data, 99.27% on validation data and 99.06% on selected test data. The main contribution of this paper is a new Deep Convolutional Neural Network model using Deep Transfer Learning through the Xception architecture for the assisted diagnosis of COVID-19, pneumonia or healthy patients, trained on COVID-19 Chest X-Ray Database and evaluated through two external databases, which give the model novelty within the lack of external validation in all the literature reviewed.

Abstract:AbstractPneumonia is one of the most chronic diseases, and therefore, its timely diagnosis is of utmost importance. Traditionally, clinical decisions have been considered as a gold standard for diagnosis, but it is not a practical option in all scenarios. Therefore, several methods have been explored to make the process of diagnosis faster, efficient and as accurate as clinical decisions. In this paper, we have described and proposed a Convolutional Neural Network (CNN) based deep learning technique for the classification of chest X-ray images for the diagnosis of Pneumonia. The proposed model is trained on 4099 images and tested on 1757 images resulting in an accuracy of 96.18%. The evaluation and training are conducted on ‘Labeled Optical Coherence Tomography (OCT) and Chest X-Ray Images for Classification’ dataset which is one of the largest labeled datasets which is publicly available. Also, a comparison of the proposed model with various other popular models is discussed. The results indicate that our model despite having simpler architecture and without any pre-training outperforms many of the popular models on several different performance parameters.

Abstract:Pneumonia is a dangerous disease that kills millions of children and elderly patients worldwide every year. The detection of pneumonia from a chest x-ray is perpetrated by expert radiologists. The chest x-ray is cheaper and is most often used to diagnose pneumonia. However, chest x-ray-based diagnosis requires expert radiologists which is time-consuming and laborious. Moreover, COVID-19 and pneumonia have similar symptoms which leads to false positives. Machine learning-based solutions have been proposed for the automatic prediction of pneumonia from chest X-rays, however, such approaches lack robustness and high accuracy due to data imbalance and generalization errors. This study focuses on elevating the performance of machine learning models by dealing with data imbalanced problems using data augmentation. Contrary to traditional machine learning models that required hand-crafted features, this study uses transfer learning for automatic feature extraction using Xception and VGG-16 to train classifiers like support vector machine, logistic regression, K nearest neighbor, stochastic gradient descent, extra tree classifier, and gradient boosting machine. Experiments involve the use of hand-crafted features, as well as, transfer learning-based feature extraction for pneumonia detection. Performance comparison using Xception and VGG-16 features suggest that transfer learning-based features tend to show better performance than hand-crafted features and an accuracy of 99.23% can be obtained for pneumonia using chest X-rays.

Abstract:Pneumonia is a dangerous lung disease that has affected millions of people worldwide. Several people have died as a result of incorrect pneumonia diagnosis and treatment. This has necessitated the urgent need for quick detection and classification methods of pneumonia detection for efficient treatment and quick recovery of affected persons. However, the causes of pneumonia are not accurately diagnosed using outmoded methods which use chest X-rays. This paper therefore presents a method for identifying and classifying chest X-ray images of normal and pneumonia-infected persons. The designed deep learning model first preprocesses the X-ray images to extract useful features, then segments them using a threshold segmentation technique, detects normal and pneumonia infected persons from X-ray images using the YOLOv3 detector, and classifies them as normal and with pneumonia using Support vector machine (SVM) and softmax. The suggested model was trained and evaluated using a dataset of chest X-ray images. The results show that the overall accuracy, precision, recall, and F1-score are all 99%. The findings show that deep features produced accurate and consistent characteristics for pneumonia detection. Using the presented approach, radiologists can assess pneumonia patients and deliver a rapid diagnosis.

Abstract:Pneumonia is a life-threatening lung infection resulting from several different viral infections. Identifying and treating pneumonia on chest X-ray images can be difficult due to its similarity to other pulmonary diseases. Thus, the existing methods for predicting pneumonia cannot attain substantial levels of accuracy. Therefore, this paper presents a computer-aided classification of pneumonia, coined as Ensemble Learning (EL), to simplify the diagnosis process on chest X-ray images. Our proposal is based on Convolutional Neural Network (CNN) models, which are pre-trained CNN models that have been recently employed to enhance the performance of many medical tasks instead of training CNN models from scratch. We propose to use three well-known CNN pre-trained (DenseNet169, MobileNetV2 and Vision Transformer) using the ImageNet database. Then, these models are trained on the chest X-ray data set using fine-tuning. Finally, the results are obtained by combining the extracted features from these three models during the experimental phase. The proposed EL approach outperforms other existing state-of-the-art methods, and it obtains an accuracy of 93.91% and a F1-Score of 93.88% on the testing phase.

Abstract:Summary Pneumonia disease progress rapidly leading to missed diagnosis and diagnosis error. However, it is significant to be diagnosed accurately as well as adopted timely treatments, particularly for children and elderly people over 65. In the first place, we propose a novel method for data augmentation to expand non‐case samples in order to solve the problem of imbalance of the raw data. Then we have designed a neural network based on separable convolution layers, using deep learning to classify between Pneumonia cases and normal cases from chest X‐ray images. Finally, well‐known performance measures are utilized to evaluate our model, obtaining the following results: Accuracy (0.97), AUC (0.99), Sensitivity (0.98), Specificity (0.98), Precision (0.95), F1‐Score (0.97), and Youden Index (0.90). Simultaneously, we highlighted the proposed neural network recognition area via Class Activation Method for the purpose of providing an explainable diagnosis. Compared with the‐state‐of‐art models, our model performed better, and it may be regarded as an alternative in countries or regions lack of the relevant equipment and professional radiologists.

Abstract:The identification and characterization of lung diseases is one of the most interesting research topics in recent years. They require accurate and rapid diagnosis. Although lung imaging techniques have many advantages for disease diagnosis, the interpretation of medial lung images has always been a major problem for physicians and radiologists due to diagnostic errors. This has encouraged the use of modern artificial intelligence techniques such as deep learning. In this paper, a deep learning architecture based on EfficientNetB7, known as the most advanced architecture among convolutional networks, has been constructed for classification of medical X-ray and CT images of lungs into three classes namely: common pneumonia, coronavirus pneumonia and normal cases. In terms of accuracy, the proposed model is compared with recent pneumonia detection techniques. The results provided robust and consistent features to this system for pneumonia detection with predictive accuracy according to the three classes mentioned above for both imaging modalities: radiography at 99.81% and CT at 99.88%. This work implements an accurate computer-aided system for the analysis of radiographic and CT medical images. The results of the classification are promising and will certainly improve the diagnosis and decision making of lung diseases that keep appearing over time.

Abstract:Developing countries have had numerous obstacles in diagnosing the COVID-19 worldwide pandemic since its emergence. One of the most important ways to control the spread of this disease begins with early detection, which allows that isolation and treatment could perhaps be started. According to recent results, chest X-ray scans provide important information about the onset of the infection, and this information may be evaluated so that diagnosis and treatment can begin sooner. This is where artificial intelligence collides with skilled clinicians' diagnostic abilities. The suggested study's goal is to make a contribution to battling the worldwide epidemic by using a simple convolutional neural network (CNN) model to construct an automated image analysis framework for recognizing COVID-19 afflicted chest X-ray data. To improve classification accuracy, fully connected layers of simple CNN were replaced by the efficient extreme gradient boosting (XGBoost) classifier, which is used to categorize extracted features by the convolutional layers. Additionally, a hybrid version of the arithmetic optimization algorithm (AOA), which is also developed to facilitate proposed research, is used to tune XGBoost hyperparameters for COVID-19 chest X-ray images. Reported experimental data showed that this approach outperforms other state-of-the-art methods, including other cutting-edge metaheuristics algorithms, that were tested in the same framework. For validation purposes, a balanced X-ray images dataset with 12,000 observations, belonging to normal, COVID-19 and viral pneumonia classes, was used. The proposed method, where XGBoost was tuned by introduced hybrid AOA, showed superior performance, achieving a classification accuracy of approximately 99.39% and weighted average precision, recall and F1-score of 0.993889, 0.993887 and 0.993887, respectively.

Abstract:As the rapid spread of coronavirus disease (COVID-19) worldwide, chest X-ray radiography has also been used to detect COVID-19 infected pneumonia and assess its severity or monitor its prognosis in the hospitals due to its low cost, low radiation dose, and wide accessibility. However, how to more accurately and efficiently detect COVID-19 infected pneumonia and distinguish it from other community-acquired pneumonia remains a challenge. In order to address this challenge, we in this study develop and test a new computer-aided diagnosis (CAD) scheme. It includes several image pre-processing algorithms to remove diaphragms, normalize image contrast-to-noise ratio, and generate three input images, then links to a transfer learning based convolutional neural network (a VGG16 based CNN model) to classify chest X-ray images into three classes of COVID-19 infected pneumonia, other community-acquired pneumonia and normal (non-pneumonia) cases. To this purpose, a publicly available dataset of 8,474 chest X-ray images is used, which includes 415 confirmed COVID-19 infected pneumonia, 5,179 community-acquired pneumonia, and 2,880 non-pneumonia cases. The dataset is divided into two subsets with 90% and 10% of images in each subset to train and test the CNN-based CAD scheme. The testing results achieve 94.0% of overall accuracy in classifying three classes and 98.6% accuracy in detecting Covid-19 infected cases. Thus, the study demonstrates the feasibility of developing a CAD scheme of chest X-ray images and providing radiologists useful decision-making supporting tools in detecting and diagnosis of COVID-19 infected pneumonia.

Abstract:Pneumonia is a serious inflammatory disease that causes lung ulcers, and it is one of the leading reasons for pediatric death in the world. Chest X-rays are perhaps the most commonly utilized modalities to recognize pneumonia. Generally, the illness could be analyzed by a specialist radiologist. But for some reason, the diagnosis may be subjective. Thus, the physicians must be guided by computer-aided diagnosis frameworks in this challenging task. In this study, we propose a combined deep learning architecture to identify pneumonia in chest radiography images. We first, use Adaptive Median Filter for images enhancement, then we employ a regularized Convolutional Neural Network for features extraction, and then we use Long Short Term Memory as a classifier. Finally, the attention mechanism is used to direct the network attention to relevant features. The suggested approach was tested on two publicly available pneumonia X-ray datasets provided by Kermany and the Radiological Society of North America. On the Kermany and RSNA datasets, the suggested technique attained accuracy rates of 99.91% and 88.86%, respectively. In the last stage of our experiments, we employed a Grad-CAM-based color visualization technique to precisely interpret the detection of pneumonia in radiological images. The results outperformed those of state-of-the-art approaches.

Abstract:Medical imaging treatment is one of the best-known computer science disciplines. It can be used to detect the presence of several diseases such as skin cancer and brain tumors, and since the arrival of the coronavirus (COVID-19), this technique has been used to alleviate the heavy burden placed on all health institutions and personnel, given the high rate of spread of this virus in the population. One of the problems encountered in diagnosing people suspected of having contracted COVID-19 is the difficulty of distinguishing symptoms due to this virus from those of other diseases such as influenza, as they are similar. This paper proposes a new approach to distinguishing between lung diseases and COVID-19 by analyzing chest x-ray images using a convolutional neural network (CNN) architecture. To achieve this, pre-processing was carried out on the dataset using histogram equalization, and then we trained two sub-datasets from the dataset using the Train et Test, the first to be used in the training phase and the second to be used in the model validation phase. Then a CNN architecture composed of several convolution layers and fully connected layers was deployed to train our model. Finally, we evaluated our model using two different metrics: the confusion matrix and the receiver operating characteristic. The simulation results recorded are satisfactory, with an accuracy rate of 96.27%.

Abstract:Pneumonia is a lung infection that causes 15% of childhood mortality, over 800,000 children under five every year, all over the world. This pathology is mainly caused by viruses or bacteria. X-rays imaging analysis is one of the most used methods for pneumonia diagnosis. These clinical images can be analyzed using machine learning methods such as convolutional neural networks (CNN), which learn to extract critical features for the classification. However, the usability of these systems is limited in medicine due to the lack of interpretability, because of these models cannot be used to generate an understandable explanation (from a human-based perspective), about how they have reached those results. Another problem that difficults the impact of this technology is the limited amount of labeled data in many medicine domains. The main contributions of this work are two fold: the first one is the design of a new explainable artificial intelligence (XAI) technique based on combining the individual heatmaps obtained from each model in the ensemble. This allows to overcome the explainability and interpretability problems of the CNN "black boxes", highlighting those areas of the image which are more relevant to generate the classification. The second one is the development of new ensemble deep learning models to classify chest X-rays that allow highly competitive results using small datasets for training. We tested our ensemble model using a small dataset of pediatric X-rays (950 samples) with low quality and anatomical variability (which represents one of the biggest challenges). We also tested other strategies such as single CNNs trained from scratch and transfer learning using CheXNet. Our results show that our ensemble model outperforms these strategies obtaining highly competitive results. Finally, we confirmed the robustness of our approach using another pneumonia diagnosis dataset [1].

Abstract:AbstractThe confrontation of COVID-19 pandemic has become one of the promising challenges of the world healthcare. Accurate and fast diagnosis of COVID-19 cases is essential for correct medical treatment to control this pandemic. Compared with the reverse-transcription polymerase chain reaction (RT-PCR) method, chest radiography imaging techniques are shown to be more effective to detect coronavirus. For the limitation of available medical images, transfer learning is better suited to classify patterns in medical images. This paper presents a combined architecture of convolutional neural network (CNN) and recurrent neural network (RNN) to diagnose COVID-19 from chest X-rays. The deep transfer techniques used in this experiment are VGG19, DenseNet121, InceptionV3, and Inception-ResNetV2. CNN is used to extract complex features from samples and classified them using RNN. The VGG19-RNN architecture achieved the best performance among all the networks in terms of accuracy in our experiments. Finally, Gradient-weighted Class Activation Mapping (Grad-CAM) was used to visualize class-specific regions of images that are responsible to make decision. The system achieved promising results compared to other existing systems and might be validated in the future when more samples would be available. The experiment demonstrated a good alternative method to diagnose COVID-19 for medical staff.
