Abstract:Objective To introduce the super minimally invasive transintercostal device closure of ventricular septal defect (VSD) and summarize it's therapeutic effect.Methods Nine patiens with VSD from August 2014 to December 2014 in our hospital were enrolled,which performed super minimally invasive transintercostal device closure of VSD.All patients were followed up prudently post-operation.Echocardiography was scheduled according to the protocol.Results All cases were successfully occluded.There was no death case in the peri-operation period and severe complications after operation.Conclusions The super minimally invasive transintercostal device closure of VSD is safe and reliable.
Transaxillary Minithoracotomy in Intrathoracic Surgery for 316 Infants and Children

Abstract:Objective To introduce the technique of intrathoracic surgery performed through vertical transaxillary minithoracotomy.Methods From March 1989 to March 2001, 316 patients underwent intrathoracic surgery through a vertical transaxillary minithoracotomy. 285 patients suffered from patent ductus arteriosus (PDA), 10 congenital esophageal atresia, 8 congenital pulmonary cysts, 6 congenital emphysema, 1 pulmonary sequestration, 5 mediastinal tumor, and 1 eventration of the diaphragm.Results All of the patients were successfully treated under satisfactory exposure. No operative mortality and severe postoperative complications occurred.Conclusions Intrathoracic surgery performed through a vertical transaxillary minithoracotomy appears to be less invasive, and is a simple, safe, cosmetically acceptable and efficient approach.

Hutchinson, Besnier, Schaumann, Darier and Pautrier have all made contributions, and in more recent years Leitner 1 and Coste 2 have helped to clarify the diverse collection of biologic and clinical findings which constitute sarcoidosis and have collected in their writings the best of the investigations and discussions anent the etiology, morphology and course of this disease. Abstract:ABSTRACT A positive youth development perspective focuses on recognizing psychosocial strengths and providing social environments that contribute to the development of these in children and adolescents. Bereavement camps can provide such an environment as they help children cope with the death of someone close. The purpose of this study was to observe bereavement camps through the lens of positive youth development to determine the applicability of the eight features of positive developmental settings for describing bereavement camps (safety, appropriate structure, supportive relationships, opportunities to belong, positive norms, support for efficacy, skill building opportunities, integration of family and community). Observational notes were recorded by researchers during on-site visits to three different weekend bereavement camps. Results identified how each element of positive developmental settings was exemplified in either typical camp activities or bereavement-focused activities. For example, assigning campers to cabin groups based on age and gender provided opportunities to belong, and giving campers a comfort object and a big buddy provided supportive relationships. Findings were used to create a positive developmental settings observation checklist for use by bereavement camp practitioners to assess the extent to which each camp provides the requisite elements for promoting positive youth development.

Abstract:Congenital heart disease (CHD), birth defect with the highest incidence rates worldwide, and is mainly characterized by the abnormal internal structure of the heart or/and the anatomical structure of great vessels. In the past few decades, CHD repair surgery through standard median sternotomy incision combined with cardiopulmonary bypass (CPB) technology has been considered the gold standard for surgical correction of heart and great vessels. With the promotion and clinical application of interventional catheterization technology, transcatheter closure of CHD under radioactive radiation has gradually been recognized and applied. However, its radiation exposure and potential complications related to arteriovenous vessels still face challenges. In recent years, an increasing number of surgeons have explored new surgical procedures, for the safe and effective treatment of CHD, as far as possible to reduce surgical trauma, avoid radiation exposure, and improve the cosmetic effect. Therefore, on the premise of satisfactory exposure or guidance, how to integrate ultrasound and percutaneous interventional technology remained the focus of the exploration. This mini-review highlights and summarizes the signs of progress of ultrasound intervention in the last decade that have proven the effectiveness and operability of a well-established procedure for percutaneous closure of congenital heart diseases under echocardiographic guidance only. We discuss potential diseases that will benefit from this emerging procedure based on this progress. Owing to the crucial advantages played by this strategy in the treatment of CHD, better understanding and promotion of this less exploited field may contribute to the development of therapeutics targeting CHD, improve medical utilization rate, promote the optimization of medical resources, and ultimately achieve precise and efficient medical treatment.
Correction of total anomalous pulmonary venous connection in infant less than one year old]

Abstract:To evaluate the effects of early correction for total anomalous pulmonary venous connection (TAPVC) in infant under one.Twenty-eight infants less than one year old with TAPVC were admitted and prepared to be treated surgically from July 1995 to August 2001. Total correction was performed on 24 cases, 20 males and 4 females, aged 6.04 +/- 3.34 months, with the mean body weight of 5.74 +/- 1.22 kg, of which 10 (41.67%) being of supracardic type, 11 (45.83%) being of intracardiac type, 1 (4.17%) being of infracardiac type, and 2 (8.33%) being of mixed type. Four patients died of hypoxic attack during anteoperative preparation and failed to be operated upon.Three infants (12.5%) died within 30 days after operation. One case of intracardiac type died of circulatory failure because of severe postoperative bleeding and late exploratory thoracotomy. One case of supracardiac type died of cardiac sufficiency because of bleeding at anastomotic stoma during operation. One case of mixed type died of circulatory failure because of high pressure of left cardiac atrium. All of the 4 cases who underwent emergent operation survived. The most common complication was arrhythmia (41.67%).The early effect of early correction for TAPVC is satisfactory and its long-term effect is to be observed. A wide anastomosis between the posterior wall of the left atrium and common pulmonary vein is more important for the supracardic type. The diameters of left atrium, left ventricle and atrial septum defect have little effect on operative results. Arrhythmia after operation has no significant increase in supracardic type through. With rather high incidence rate of arrhythmia (5/8, 62.5%), the combined left atrium incision and right atrium incision approach assures a sufficiently large incision for the supracardiac type. However, this approach is statistically different in the incidence rate of arrhythmia only between supracardiac type and intracardiac type.
Single-stage Correction of Coarctation of Aorta with Intracardiac Anomalies and Pulmonary Arterial Hypertension in Infants

Abstract:Objective To explore the clinical experiences of surgical treatments for coarctation of aorta (COA)with intracardiac anomalies and pulmonary arterial hypertension (PAH)in infants. Methods A retro-spective case-control study was carried out in 36 infants operated in our hospital during February 2012 to February 2017. There were 16 cases with COA and PAH in group A,and 20 cases with ventricular septal defect(VSD)and PAH in group B. The perioperative clinical data and follow-up data of the two groups were compared. Results All the patients were operated by both arch repair and intracardiac anomalies repair completed via sternotomy un-der extracorporeal circulation in single-stage. There was no significant difference of the mean pulmonary arterial pressure preoperatively between the two groups(P>0. 05). By the end of extracorporeal circulation,the mean pulmonary arterial pressure of group A was (28. 4 ± 8. 8)mmHg(1mmHg=0. 133 kPa),which was higher than (21. 2 ± 5. 0 )mmHg of group B(P<0. 05). It was longer in extracorporeal circulation time,aortic block time and postoperative ventilator aeration time of group A(P<0. 05). There was no mortality,nor paraplegia after operation. The postoperative arterial pressure gradient between the upper and the lower extremities was less than 10 mmHg,and no PAH was found under ultrasound detection 3 months after operation. Conclusions It would turn a rapider deteriorate in infants of COA and left to right shunt combined with PAH. It may get satis-factory effect if correction of the heart malformation and relief of COA could achieve in time.

Abstract:Objective To study the surgical treatment for Ebstein's anomaly in children.Methods From August 1992 to August 2010,there were 31 cases of Ebstein's anomaly in children underwent surgical repair,including thirteen males and eighteen females,aged from 6 months to fifteen years[mean(6.9±1.6) years] and the weight was from 5.5 to 53.5 kg[mean(19.6±4.7)kg].Eight patients underwent Danielson repair of tricuspid valve,among these 2 patients were added bidirectional Glenn operation;23 patients underwent Carpentier repair of tricuspid valve,among these 5 patients were added bidirectional Glenn operation.The results of 31 cases of Ebstein's anomaly in children were retrospectively analyzed.Results There was no postoperative death with Carpentier repair of tricuspid valve.All survivals were in New York Heart Association class Ⅰ.Postoperative tricuspid regurgitation was markedly improved,echocardiography showed disappearance of tricuspid incompetence in 9 patients,mild incompetence in 13 and moderate incompetence in 1.There was one postoperative death with Danielson repair of tricuspid valve,and main causes of death were tachyarrhythmia and continuance hypoxia.All survivals were in New York Heart Association class Ⅰ.Postoperative echocardiaography showed disappearance of tricuspid incompetence in 3 patients,mild competence in 3 and moderate mild competence in 2. No reoperation for residual or recurrent tricuspid incompetence was required in all survivals. Conclusions In spite of the variable degree of Ebstein's anomaly present,the majority of patients can undergo a surgical repair.Tricuspid reconstruction should be the first choice of surgical procedures whenever possible in children.Carpentier repair of tricuspid valve is the best procedure at present.The bidirectional Glenn could be adopted according to the function of tricuspid valve and right ventricle.
Experience of Surgical Repair for Cor Triatriatum Sinister in Chirdren

Abstract:Objective To share the experience of surgical repair for cor triatriatum in chidren.Methods Between July 2008 and June 2013,24 children with cor triatriatum underwent surgical correction in Cardiac Center of Children's Hospital of Fudan University.The minimum age at the time of operation was 1 month,median age was 5 month;the minimum body weight at the time of operation was 3.5 kg,median body weight was 7.4 kg.Retrospectively analyzed their cardiac anatomy,clinical data,surgical procedures and follow-up data.Results Of all 24 patients,there were 4 patients diagnosed semplice cor triatriatum,and 20 patients diagnosed cor triatriatum associated with other malformation includes cardiac and non-cardiac issues.Procedures were performed on 22 patients to correct all cardiac defects in one stage.1 patient underwent surgical repair for cor triatriatum and Glenn procedure as well.1 patient,diagnosed as cor triatriatum and functional single ventricle S/P Glenn,underwent surgical repair for cor tiratriatum only.2 patients recovered from pulmonary artery hypertension crisis after operation.1 patient dead 17 days post procedure caused by respiratory failure.23 patients discharged from hospital.The longest follow-up was 73 months,at least 12 months after discharge.All alive patients were in New York Heart Association's function class Ⅰ-Ⅱ.Echo assessment revealed normal size of left atriums of almost all alive patients one year after discharge.Till April 2015,no postoperation intervention,no blood flow obstruction at level of pulmonary veins,left atrium or mitral valve.Conclusion Pediatric patients diagnosed cor tiratriatum sinister need intervention,no matter hemodynamic change occurs or not.Right time of surgery is important for successful treatment.
Modified Edge-to-Edge Technique for Correction of Congenital Mitral Regurgitation in Infants and Children

Abstract:Since 2008, 28 patients with congenital mitral regurgitation have undergone mitral valve repair with a modified edge-to-edge technique at our institution. The regurgitant mitral leaflet was sutured with a pledget-reinforced, horizontal mattress suture with No. 4-0 polypropylene on the ventricle side and a pledget-reinforced mattress suture with Gore-Tex sutures (W.L. Gore & Associates, Flagstaff, AZ) and Dacron pledgets (Chest, Shanghai) placed on the anterior and posterior annulus corresponding to the edge-to-edge suturing site. Early results are encouraging, but a longer follow-up is needed.

Abstract:The purpose of this article is to provide a comprehensive review of recent advances in the diagnosis and treatment of congenital heart disease (CHD). CHD, which affects about 1% of live births worldwide, is still a major problem in pediatric cardiology. The most prevalent congenital cardiac defects—Ventricular Septal Defect (VSD), Tetralogy of Fallot (TOF), Coronary Artery Fistula, Coarctation of the Aorta (CoA), Aortic and Pulmonary Stenosis, Valve Insufficiency, Double Outlet Right Ventricle (DORV), Ebstein Anomaly, Transposition of the Great Arteries (TGA), Anomalous Venous Return, and Hypoplastic Left Heart Syndrome (HLHS) are all thoroughly examined in this review. The review examines current diagnostic and treatment strategies, highlighting the benefits, drawbacks, sensitivity, and specificity of several imaging modalities, including as CT, MRI, and echocardiography. It also discusses the historical evolution of treatment outcomes, emphasizing the tremendous advances in patient care and surgical procedures that have significantly mproved survival rates and quality of life for CHD patients.

Abstract:Background: The treatment of preterm and low birth weight (LBW) neonates born with congenital heart disease (CHD) requiring early cardiac intervention remains challenging. We aimed to analyze morbidity and mortality in this combined high-risk patient group. Methods: A retrospective cohort study was conducted of preterm [<37 weeks gestational age (GA)] and/or LBW neonates (<2,500 g) born with a diagnosis of CHD, which requires invasive cardiac intervention (surgery or catheter) within their first year of life. Patients born between 2016 and 2020 and treated in three European pediatric heart centers were included. Results: A total of 308 neonates (51% male) with CHD were included. Of those, 237 (77%) were born preterm, 259 (84%) were LBW, and 188 (61%) were both. The median GA was 35.4 weeks (interquartile range 33.3–36.9) and the mean birth weight was 2,016 ± 580 g. CHD was categorized as simple (12%), moderate (64%), or severe (24%). The overall complication rate was 45% and was highest in patients with severe CHD ( p = 0.002). One-year mortality (19%) was associated with severe CHD, low relative birth weight in patients with genetic diagnoses, and low GA at birth, whereas GA at birth significantly impacted survival only after 3 months of life. Conclusions: The high morbidity and mortality in preterm and LBW neonates with CHD reflect their complexity and consequent limited treatment feasibility.

Abstract:Background/Objectives: Truncus arteriosus communis (TAC) is a rare congenital heart defect characterized by a single arterial trunk that supplies systemic, pulmonary, and coronary circulations. This defect, constituting approximately 1–4% of congenital heart diseases, poses significant challenges in prenatal diagnosis, management, and postnatal outcomes. Methods: A retrospective analysis was conducted at the local tertiary referral center on cases of TAC diagnosed prenatally between 2019 and 2024. Additionally, a systematic literature review was performed to evaluate the accuracy of prenatal diagnostics and the presence of associated anomalies in fetuses with TAC and compare already published data with the local results. The review included studies that especially described the use of fetal echocardiography, the course and outcome of affected pregnancies, and subsequent management strategies. Results: The analysis of local prenatal diagnoses revealed 14 cases. Of the 11 neonates who survived to birth, the TAC diagnosis was confirmed in 7 instances. With all seven neonates undergoing surgery, the intention-to-treat survival rate was 86%, and the overall survival rate was 55%. By reviewing published case series, a total of 823 TAC cases were included in the analysis, of which 576 were diagnosed prenatally and 247 postnatally. The presence of associated cardiac and extracardiac manifestations as well as genetic anomalies was common, with a 22q11 microdeletion identified in 27% of tested cases. Conclusions: Advances in prenatal imaging and early diagnosis have enhanced the management of TAC, allowing for the detailed planning of delivery and immediate postnatal care in specialized centers. The frequent association with genetic syndromes underscores the importance of genetic counseling in managing TAC. An early surgical intervention remains crucial for improving long-term outcomes, although the condition is still associated with significant risks. Long-term follow-up studies are essential to monitor potential complications and guide future management strategies. Overall, a coordinated multidisciplinary approach from prenatal diagnosis to postnatal care is essential for improving outcomes for individuals with TAC.

Techniques and Experience Which Improve the Surgical Outcomes of Critical and Complex Congenital Heart Disease in Neonates

Abstract:Objective To discuss the experience of early surgical intervention to critical and complex congenital heart diseases (CHD) in neonates.Methods A retrospective study of clinical records of patients with congenital heart diseases underwent surgical intervention in one single pediatric cardiac center was performed.From January 2009 to December 2017,841 critical and complex CHD neonates were identified at Department of Cardiovascular Surgery,Children's Hospital of Fudan University,of which 6.5％ were premature.There were 557 male and 284 female patients.The age was (11±9) days,ranging from 0 to 28 days,M(QR):8 (17) days.The weight was (3.26±0.57) kg,ranging from 1.9 to 5.0 kg,M(QR):3.3(0.7) kg.There were 189 patients associated with other malformation besides CHD.Before surgery,13.6％ (114/841) patients were on ventilation,and severe acidosis was addressed in 136 patients.All patients underwent surgical interventions,including surgical procedures and hybrid procedures.Results Emergency surgeries were performed in 35 patients.One hundred patients had palliative procedures,other 633 patients had complete repair,while the rest 108 patients underwent hybrid procedures.The in-hospital mortality was 4.0％ (34/841).The ventilation time was 1.5 to 1 130.0 hours,M(QR):94 (116) hours,with the rate of reintubation 3.3％ (27/807).The ICU stay time was 1 to 195 days,M(QR):14 (15) days.Conclusion The improvement on early screening and diagnosing,referral system,multidisciplinary cooperation and hybrid invention skills together contributed to better outcomes of critical and complex congenital heart diseases in neonates.

Abstract:INTRODUCTION: Anterior mini-thoracotomy approach is a good alternative to median sternotomy in Atrial Septal Defect (ASD) repair. Our purpose is to explain the details of our technique and peresent the results.METHODS: Seventy five patients with ASD (52 female and 23 male) were operated with anterior mini-thoracotomy approach in our tertiary center between March 2012 and March 2014. The mean age was 14±10 ranged from 2 to 42 years. Outcomes were defined according to cardiopulmonary and aortic cross-clump time, intensive care unit stay time, morbidity, mortality, the size of incision, the amount of post-operative bleeding, need for blood transfusion and reoperation.RESULTS: Mean Cardiopulmonary bypass time was 49.62 minutes (26 to 105 minutes) and mean aortic cross clamp time was 22.29±6.77 minutes (11 to 47 minutes). The mean amount of blood transfusion was 47.49± 62.22 mm (0 to 200 cc) and the mean chest tube drainage after surgery was 80.17 ±121.06 mm (0 to 600 cc). One patient re-operated for dehiscence of ASD surgical sutures and there was no reoperation for surgical bleeding or tamponade drainage in these patients. In 74 cases the defect was secundum type, in 2 patients it was sinus venosus type and in one with associated partial Anomalous repair.CONCLUSION: Anterior thoracotomy approach is safe and may be the surgical technique of choice for secundum ASD repair in all age groups and we can utilize this technique also for more complicated kinds of surgery for instance, sinus venosus type ASD with or without Partial Anomalous Defect.
Open-surgery repair of congenital malformation of the chest: indications, technical tips and outcomes

Abstract:Background: Congenital malformations of the chest may influence significantly the life of the patients, because of functional or psychological impairment. The interest for these anomalies has dramatically increased during the last 20 years. In this period, most of the concepts and surgical techniques that had dominated the surgical approaches over the last century have been abandoned or have evolved, leaving more space to non-surgical or minimally invasive treatments, in particular in pediatric patients. In growing children, we prefer to avoid open surgical repair if other minimally or non-invasive methods are available. Nevertheless, in some congenital anomalies the open treatment is still indicated in the pediatric age, as it is the only effective option available. In this review, we will present the open surgical treatment of these less common congenital anomalies of the chest wall: Currarino-Silverman (CS) deformity, Poland syndrome (PS), sternal cleft and Jeune syndrome. Methods: Based on the large experience of two national referral centers of pediatric and thoracic surgery, we present the surgical treatment of the aforementioned thoracic anomalies, including the indications, technical aspects and the outcomes. Results: While pectus excavatum (PE) is usually repaired through a minimally invasive approach and pectus carinatum (PC) is treated with orthotic measures, CS anomaly requires an open sternocondroplasty. Cartilages are removed on both sides of the sternum and the sternal angle is resected. The correction of the lower half of the sternum is achieved either by inserting a retrosternal support or by filling the depression with cartilage tissue. The correction of the thoracic defect in PS is tailored according to the single patient. Removal of prominent cartilages, sternal osteotomies, bridging the costal agenesis with metallic bars or custom-made prosthesis may be necessary in severe cases. Sternal clefts are better repaired early with a primary closure or later on with prosthetic materials. Jeune syndrome in the asphyxiating fatal variant requires lateral thoracic expansion (LTE). Conclusions: In conclusion, chest wall anomalies encompass a large spectrum of deformities of varying severity and functional or cosmetically important impact.
Spontaneously Corrected Hypoplastic Left Heart: A Case Report and Exceptional Opportunity to Discuss Etiology with Novel Therapeutic Vision

Abstract:Hypoplastic left heart syndrome (HLHS) is a relatively prevalent fetal echocardiography finding in complex congenital heart diseases. Current studies indicate that intrinsic and extrinsic mechanisms could be involved in the development of left heart hypoplasia. Left ventricular inflow or outflow disorders may cause left heart hypoplasia. Prenatal aortic valvuloplasty has become more common as a therapeutic strategy. Our case presentation provides an opportunity for a new vision toward the etiology, prevention, and treatment of HLHS. In our patient, prenatal progressive left heart hypoplasia associated with restrictive foramen oval (FO) suggested the likelihood of a flow-mediated mechanism. Additionally, postnatal improvement of the hypoplastic left heart in the presence of a functional perimembranous ventricular septal defect (PM-VSD) reinforced the suspicion of an extrinsic mechanism. Pre- or postnatal interventional creation of an atrial septal defect (ASD) or VSD is our proposed method for HLHS in selected patients.
[Ectopia cordis and Cantrell's pentalogy: personal experience and considerations on the surgical treatment]

Abstract:Clefts of the sternum have always attracted attention whether for pathological and physiological features or for research of surgical correction. Two cases of sternal cleft, one with partial ectopia cordis, the other with Cantrell's pentalogy, are presented. Embryology, strategies and several surgical techniques are discussed on the grounds of personal experience. The pediatric surgeon can make a choice among a lot of surgical techniques, because the ectopia cordis and Cantrell's pentalogy are very uncommon and the surgical treatment has a difficult codification. The knowledge of several methods of surgical correction is necessary to reduce high mortality of ectopia cordis and Cantrell's pentalogy. Primary repair in the neonatal period is the best type of management for these rare conditions, because simple closure of the sternal defect during the first month of life avoids the more complex reconstruction necessary in older children.
