Abstract:Background: Base excision repair (BER) plays an important role in the maintenance of genome integrity and anticancer drug resistance. This study aimed to explore the role of BER gene polymorphisms in response to chemotherapy for advanced non-small cell lung cancer (NSCLC) patients treated with platinum-based chemotherapy. Methods: During the period from November 2009 to January 2016, a total of 152 patients diagnosed with NSCLC Stage IIIB and IV in the First Hospital of Jilin University were admitted into this study. The XRCC1 G28152A, MUTYH G972C, HOGG1 C1245G, and PARP1 T2444C polymorphisms of all the patients were detected by mass spectrometry. The logistic regression was used for statictical analysis. All tests were bilateral test, and a P < 0.05 was considered statistically significant. Results: The logistic regression model showed that the response rate of chemotherapy of the PARP1 T2444C polymorphisms, CC genotype (odds ratio [OR]: 5.216, 95% confidence interval [CI]: 1.568–17.352, P = 0.007), TC genotype (OR: 2.692, 95% CI: 1.007–7.198, P = 0.048), as well as the genotype of TC together with CC (OR: 3.178, 95% CI: 1.229–8.219, P = 0.017) were significantly higher than those of TT wild type. There was no relationship between the MUTYH G972C, XRCC1 G28152A, and HOGG1 C1245G gene polymorphisms and chemosensitivity. Conclusions: The PARP1 2444 mutation allele C might be associated with the decreased sensitivity to platinum-based chemotherapy in advanced NSCLC. These findings may be helpful in designing individualized cancer treatment.
GSTP1 c.313A > G mutation is an independent risk factor for neutropenia hematotoxicity induced by anthracycline-/paclitaxel-based chemotherapy in breast cancer patients

Abstract:Background The link between glutathione S-transferase P1 (GSTP1) c.313A > G polymorphism and chemotherapy-related adverse events remains controversial. The goal of this study was to assess how this variant affected the toxicity of anthracycline-/paclitaxel-based chemotherapy in patients with breast cancer. Methods This study retrospectively investigated pharmacogenetic associations of GSTP1 c.313A > G with chemotherapy-related adverse events in 142 breast cancer patients who received anthracycline and/or paclitaxel chemotherapy. Results There were 61 (43.0%), 81 (57.0%), 43 (30.3%), and 99 (69.7%) patients in the T0-T2, T3-T4, N0-N1, and N2-N3 stages, respectively. There were 108 (76.1%) patients in clinical stages I–III and 34 (23.9%) patients in clinical stage IV. The numbers of patients with luminal A, luminal B, HER2 + , and triple-negative breast cancer (TNBC) were 10 (7.0%), 77 (54.2%), 33 (23.2%), and 22 (15.5%), respectively. The numbers of patients who carried GSTP1 c.313A > G A/A, A/G, and G/G genotypes were 94 (66.2%), 45 (31.7%), and 3 (2.1%), respectively. There were no statistically significant differences in the proportion of certain toxicities in patients with A/G, G/G, and A/G + G/G genotypes, except for neutropenia, in which the proportion of patients with A/G + G/G ( χ 2 = 6.586, P = 0.035) genotypes was significantly higher than that with the AA genotype. The logistic regression analysis indicated that GSTP1 c.313A > G mutation (A/G + G/G vs. A/A genotype) (adjusted OR 4.273, 95% CI 1.141–16.000, P = 0.031) was an independent variable associated with neutropenia. Conclusions The findings of this study indicate that the GSTP1 c.313A > G mutation is an independent risk factor for neutropenia hematotoxicity in breast cancer patients induced by anthracycline-/paclitaxel-based chemotherapy.
The Association of Genetic Variations in DNA Repair Pathways with Severe Toxicities in NSCLC Patients Undergoing Platinum-Based Chemotherapy.

Abstract:Genetic variations in genes involved in repairing platinum‐induced DNA lesions may contribute to the toxicity of platinum‐based chemotherapy. The role of single‐nucleotide polymorphisms (SNPs) within DNA repair pathways in the occurrence of severe toxicity is not yet understood. Current studies prefer to do original works rather than analyze previously published data. Our study aimed to replicate associations between previously investigated SNPs and toxicities and to identify new genetic makers. We systematically examined the relevance of 97 SNPs in 54 candidate genes responsible for repairing DNA interstrand and intrastrand cross‐links to severe toxicity in a discovery cohort of 437 NSCLC patients receiving platinum‐based chemotherapy. Statistically significant SNPs were then assessed for replication in an independent validation cohort of 781 NSCLC patients. We found that 7 SNPs were significant at p < 0.01 (RRM1 rs12806698, XPC rs2228000, XPF rs1799801, hMLH1 rs1800734, PMS2 rs1062372, REV3L rs462779 and FANCC rs4647554) in the discovery cohort. Among them, two SNPs (RRM1 rs12806698 and hMLH1 rs1800734) remained significant after Bonferroni correction. XPC rs2228000 showed a significant relationship with severe gastrointestinal toxicity in the validation cohort. When the two cohorts were combined, XPC rs2228000 presented better tolerance of severe hematologic toxicity, gastrointestinal toxicity and leukopenia (OR = 0.677, 95% CI: 0.510–0.899, p = 0.007; OR = 0.565, 95% CI: 0.368–0.869, p = 0.009; OR = 0.628, 95% CI: 0.439–0.899, p = 0.011, respectively). Our findings can offer comprehensive pharmacogenetic information for platinum‐induced toxicities.
Genetic polymorphisms and platinum-induced hematological toxicity: a systematic review

Abstract:Background: Platinum-based chemotherapy bring severe hematological toxicity that can lead to dose reduction or discontinuation of therapy. Genetic variations have been reported to influence the risk and extent of hematological toxicity; however, the results are controversial and a comprehensive overview is lacking. This systematic review aimed to identify genetic biomarkers of platinum-induced hematological toxicity. Method: Pubmed, Embase and Web of science database were systematically reviewed for studies that evaluated the association of genetic variants and platinum-related hematological toxicity in tumor patients with no prior history of chemotherapy or radiation, published from inception to the 28th of January 2022. The studies should have specific toxicity scoring system as well as defined toxicity end-point. The quality of reporting was assessed using the Strengthening the Reporting of Genetic Association Studies (STREGA) checklist. Results were summarized using narrative synthesis. Results: 83 studies were eligible with over 682 single-nucleotide polymorphisms across 110 genes. The results are inconsistent and diverse with methodological issues including insufficient sample size, population stratification, various treatment schedule and toxicity end-point, and inappropriate statistics. 11 SNPs from 10 genes (ABCB1 rs1128503, GSTP1 rs1695, GSTM1 gene deletion, ERCC1 rs11615, ERCC1 rs3212986, ERCC2 rs238406, XPC rs2228001, XPCC1 rs25487, MTHFR rs1801133, MDM2 rs2279744, TP53 rs1042522) had consistent results in more than two independent populations. Among them, GSTP1 rs1695, ERCC1 rs11615, ERCC1 rs3212986, and XRCC1 rs25487 present the most promising results. Conclusion: Even though the results are inconsistent and several methodological concerns exist, this systematic review identified several genetic variations that deserve validation in well-defined studies with larger sample size and robust methodology. Abstract:Abstract Purpose Docetaxel-based combination treatment has known to be effective for breast cancer. Genotyping of ABCC2 and SLCO1B3 were demonstrated determining the risk of docetaxel-induced leucopenia in other ethnic. We designed this phase II trial to evaluate single nucleotide polymorphisms (SNPs) of the two genes association with docetaxel haematological toxicity in chemonaive breast cancer patients of chinese population. Patients and Methods We selected 45 breast cancer patients who received docetaxel combination chemotherapy, Genomic DNA was isolated from peripheral blood and detected polymorphisms in ABCC2 and SLCO1B3. Results All patients were evaluated for toxicity. 28 patients developed with grade 3 or 4 leukopenia, and 17 with grade 1 or 2 in nadir leukocyte from cycle 1 baseline. The combined analysis indicated a significant association of 9383C/G in ABCC2 (P = 0.002) and 5676A/G in SLCO1B3 (P = 0.005) with docetaxel induced ***leukopenia. Patients homozygous for the variant allele (GG) of SLCO1B3 5676A/G had significantly higher leukopenia compared to patients with AA/AG ***genotypes(P <0.05). Similar trend was observed for ABCC2 (9383C/G) with heterozygotes (CG) having the highest percentage decrease compared to those with CC/GG genotypes(P = 0.06). Conclusion The study indicating that SNPs in ABCC2 and SLCO1B3 may predict the risk of leukopenia induced by docetaxel chemotherapy in china breast cancer patients. Single Nucleotide Polymorphism of Sod2 to Predict Survival for Chinese Gastric Cancer Patients Received Platinum/Fluorouracil-Based Adjuvant Chemotherapy

Abstract:e15035 Background: The antioxidant enzymes SOD2 and GSTP1are involved in oxidative stress and drug detoxification, which may interfere with the efficacy of anticancer drugs. Here, we investigated the clinical associations of two functional single nucleotide polymorphisms (SNPs) of SOD2 and GSTP1in gastric cancer patients receiving curative surgery and adjuvant chemotherapy. Methods: A total of 740 gastric cancer patients were genotyped for SOD2 rs4880 and GSTP1 rs1695. Two hundred and sixty-two of them received platinum/fluorouracil-based adjuvant chemotherapy. Pearson’s chi-square test, Kaplan-Meier survival and Cox proportional hazards regression analyses were used to examine the relationships between SNPs and clinical outcomes. Results: SOD2 rs4880 CT/CC significantly associated with high grades of invaded depth of tumor and regional lymph nodes metastasis. SOD2 rs4880 CT/CC and GSTP1 rs1695 GA/GG were identified as “risk” genotypes associated with inferior survival. Combined “risk” genotypes predicted the worst clinical outcome, especially for the patients receiving adjuvant chemotherapy (HR, 2.694; 95%CI, 1.405-5.165, P=0.003). Stratification analysis showed the effect of SOD2 rs4880 CT/CC on survival affected significantly for patients with stage III disease (HR, 1.421; 95%CI, 1.077-1.875, P=0.013) or patients receiving adjuvant chemotherapy (HR, 1.903; 95%CI, 1.294-2.799, P=0.001), whereas GSTP1 rs1695 GA/GG strongly affected patients with proximal gastric cancer (HR, 1.449, 95%CI: 1.015-2.068, P=0.041), tumor size larger than 5cm (HR, 1.450; 95%CI: 1.051-2.001, P=0.024), or patients without adjuvant chemotherapy (HR, 1.317; 95%CI: 1.012-1.714, P=0.038). Conclusions: The two functional polymorphisms involved in oxidative stress would affect the survival of gastric cancer patients. SOD2rs4880 could be used as a predictive marker for patients receiving platinum/fluorouracil-based adjuvant chemotherapy.

Abstract:Background: Fluoropyrimidine plus platinum chemotherapy remains the standard first line treatment for gastric cancer (GC). Guidelines exist for the clinical interpretation of four DPYD genotypes related to severe fluoropyrimidine toxicity within European populations. However, the frequency of these single nucleotide polymorphisms (SNPs) in the Latin American population is low (< 0.7%). No guidelines have been development for platinum. Herein, we present association between clinical factors and common SNPs in the development of grade 3-4 toxicity. Methods: Retrospectively, 224 clinical records of GC patient were screened, of which 93 patients were incorporated into the study. Eleven SNPs with minor allelic frequency above 5% in GSTP1, ERCC2, ERCC1, TP53, UMPS, SHMT1, MTHFR, ABCC2 and DPYD were assessed. Association between patient clinical characteristics and toxicity was estimated using logistic regression models and classification algorithms. Results: Reported grade ≤ 2 and 3-4 toxicities were 64.6% (61/93) and 34.4% (32/93) respectively. Selected DPYD SNPs were associated with higher toxicity (rs1801265; OR = 4.20; 95% CI = 1.70-10.95, p = 0.002), while others displayed a trend towards lower toxicity (rs1801159; OR = 0.45; 95% CI = 0.19-1.08; p = 0.071). Combination of paired SNPs demonstrated significant associations in DPYD (rs1801265), UMPS (rs1801019), ABCC2 (rs717620) and SHMT1 (rs1979277). Using multivariate logistic regression that combined age, sex, peri-operative chemotherapy, 5-FU regimen, the binary combination of the SNPs DPYD (rs1801265) + ABCC2 (rs717620), and DPYD (rs1801159) displayed the best predictive performance. A nomogram was constructed to assess the risk of developing overall toxicity. Conclusion: Pending further validation, this model could predict chemotherapy associated toxicity and improve GC patient quality of life.
The impact of EGFR gene polymorphisms on the response and toxicity derived from neoadjuvant chemotherapy for breast cancer.

Abstract:Background: Neoadjuvant chemotherapy is usually used for treating locally advanced breast cancer. However, not all patients achieve pathologic complete response (pCR). In this study, we selected two epidermal growth factor receptor (EGFR) single nucleotide polymorphism (SNP) sites, rs1468727 and rs845552, to investigate the association between the genotypes and the response and toxicity derived from neoadjuvant chemotherapy for breast cancer. Methods: All participants took part in clinical trial SHPD001 and SHPD002. For univariate analyses, the association between SNP and pCR or toxicity was analyzed by Chi-square or Fisher's exact test. For multivariate analyses, logistic regression was used instead. Results: In all, one hundred and eighteen patients were enrolled. We found that the frequency of AA genotype in rs845552 was higher than that of other genotypes in I IER2-positive breast cancer (AA vs. AG, P=0.039; AA vs. GG, P=0.005; AA vs. AG+GG, P=0.009). Multivariate logistic regression analyses showed that pCR was more difficult to be achieved in patients with a cr genotype in rs1468727 compared to those with a CC+TT genotype (OR =0.288, 95% CI: 0.109-0.762, P=0.012) or a CC genotype (OR =0.254, 95% CI: 0.076-0.849, P=0.026). Moreover, we demonstrated that both rs1468727 and rs845552 were associated with toxicity that results in complications such as increased total bilirubin, skin rash, peripheral neuropathy, and alopecia (P<0.05). Conclusions: Our study reported for the first time, that in treating breast cancer with neoadjuvant chemotherapy, EGFR SNP rs1468727 is associated with treatment response, and that both rs1468727 and rs845552 are related to treatment-derived toxicity. In addition, we also found that rs845552 may be related to the status of HER2 in breast cancer.

Abstract:Purpose: Platinum agents cause DNA cross-linking and adducts. Xeroderma pigmentosum group D (XPD) plays a key role in the nucleotide excision repair pathway of DNA repair. Genetic polymorphisms of XPD may affect the capacity to remove the deleterious DNA lesions in normal tissues and lead to greater treatment-related toxicity. This study aimed to investigate the association of three polymorphisms of XPD at codons 156, 312, and 711, with the occurrence of grade 3 or 4 toxicity in advanced non-small cell lung cancer patients. Experimental Design: We used matrix-assisted laser desorption/ionization time-of-flight mass spectrometry to genotype the three polymorphisms in 209 stage III and IV non-small cell lung cancer patients treated with platinum-based chemotherapy. Results: The variant homozygotes of XPD p.Arg(156)Arg (rs238406) polymorphism were associated with a significantly increased risk of grade 3 or 4 hematologic toxicity (adjusted odds ratios, 3.24; 95% confidence interval, 1.35-7.78; P for trend = 0.009), and, more specifically, severe leukopenia toxicity (P for trend = 0.005). No statistically significant association was found for the three polymorphisms and grade 3 or 4 gastrointestinal toxicity. Consistent with these results of single-locus analysis, both the haplotype and the diplotype analyses revealed a protective effect of the haplotype "CG" (in the order of p.Arg(156)Arg-p.Asp(312)Asn) on the risk of grade 3 or 4 hematologic toxicity. Conclusions: This investigation, for the first time, provides suggestive evidence of an effect of XPD p.Arg(156)Arg polymorphism on severe toxicity variability among platinum-treated non-small cell lung cancer patients.

Abstract:The JNK and P38α pathways play a crucial role in tissue homeostasis, apoptosis and autophagy under genotoxic stresses, but it is unclear whether single nucleotide polymorphisms (SNPs) of genes in these pathways play a role in platinum-based chemotherapy-induced toxicities in patients with advanced non-small cell lung cancer (NSCLC). We genotyped 11 selected, independent, potentially functional SNPs of nine genes in the JNK and P38α pathways in 689 patients with advanced NSCLC treated with platinum-combination chemotherapy regimens. Associations between these SNPs and chemotherapy toxicities were tested in a discovery group of 345 patients and then validated in a replication group of 344 patients. In both discovery and validation groups as well as their pooled analysis, carriers of GADD45B rs2024144T variant allele had a significantly higher risk for severe hematologic toxicity and carriers of MAPK14 rs3804451A variant allele had a significantly higher risk for both overall toxicity and gastrointestinal toxicity. In addition, carriers of GADD45A rs581000C had a lower risk of anemia, while carriers of GADD45B rs2024144T had a significantly higher risk for leukocytopenia or agranulocytosis. The present study provides evidence that genetic variants in genes involved in the JNK and P38α pathways may predict platinum-based chemotherapy toxicity outcomes in patients with advanced NSCLC. Larger studies of other patient populations are needed to validate our findings.
XRCC1 Polymorphisms and Severe Toxicity in Lung Cancer Patients Treated with Cisplatin-Based Chemotherapy in Chinese Population

Abstract:Cisplatin kills tumor cells through DNA cross linking. Alterations in the function of DNA repair genes may affect DNA repair proficiency and influence cancer patients' response to cisplatin. The predictability of DNA repair XRCC1 (X-ray repair cross-complementing group 1 protein) single nucleotide polymorphisms (SNPs) for cisplatin-based grades 3 and 4 chemotherapy-related toxicity in patients with newly diagnosed advanced lung cancer was evaluated. The genotypes of XRCC1 at the Arg194Trp, and Arg399Gln sites were determined by PCR-based restriction fragment length polymorphism (RFLP) methods. There was no statistically significant association between either the Arg194Trp or the Arg399Gln polymorphisms and hematologic grade 3 or 4 toxicity. However, carrying at least one variant XRCC1 Arg399Gln allele (399Arg/Gln or 399Gln/Gln) was associated with a significantly increased risk of overall grade 3 or 4 toxicity (odds ratio, 2.05; 95% confidence interval, 1.02-4.10; p=0.04); and grade 3 or 4 gastrointestinal toxicity (odds ratio, 2.53; 95% confidence interval, 1.06-6.03; p=0.03). Our results suggested that patients carrying at least one variant XRCC1 Arg399Gln allele have a 2.5-fold increased risk of grade 3 or 4 gastrointestinal toxicity when treated with first-line cisplatin-based chemotherapy.

Abstract:OBJECTIVE:To explore the predictive values of platinum-related genes in gastric cancer patients on oxaliplatin-based adjuvant chemotherapy.METHODS:A total of 126 gastric cancer patients received at least 6 cycles of modified FOLFOX4 adjuvant chemotherapy. Single nuclear polymorphisms (SNPs) in ERCC1 Asp118Asp, XRCC1 Arg399Gln, XPD Lys751Gln and GSTP1 Ile105Val were assessed with 5' nuclease allelic discrimination assay (TaqMan) by real-time polymerase chain reaction and direct sequencing. The genotypes were tested for an association with survivals in gastric cancer patients on an oxaliplatin-based adjuvant chemotherapy regimen.RESULTS:The genotypic analysis of all patients indicated the frequencies for the homozygous wild-type allele, heterozygous and homozygous polymorphic variant: 64.29%, 28.57% and 7.14% for ERCC1-118; 56.35%, 38.89% and 4.76% for XRCC1-399; 84.92%, 15.08% and 0 for XPD-751; and 68.25%, 30.60% and 3.97% for GSTP1-105. Univariate analysis indicated that the ERCC1-118, the XRCC1-399 and the GSTP1-105 SNPs showed the predictive values for RFS (relapse-free survival) (P<0.001, P=0.001 and P<0.001 respectively) and OS (overall survival) (P<0.001, P=0.001 and P=0.019 respectively). A multivariable analysis of Cox proportional hazard regression model suggested that ERCC1-118 had a significant predictive value for RFS (P<0.001, HR=2.362; 95%CI: 1.458-3.827) and OS (P=0.001; HR=2.388; 95%CI: 1.448-3.937) and XRCC1-399 had only a significant predictive value for RFS. And XRCC1-399 (A/A+A/G) genotype could significantly decrease the recurrence risk of patients (P<0.001, HR=0.569; 95%CI: 0.341-0.949).CONCLUSION:Gastric cancer patients with ERCC1-118 C/C genotype and XRCC1-399A/G or A/A genotype may benefit from an oxaliplatin-based adjuvant chemotherapy.

Abstract:Platinum-based chemotherapy is the standard first-line treatment for most lung cancer patients. However, the toxicity induced by platinum-based chemotherapy greatly impedes its clinical use. Previous studies showed that long non-coding RNAs (lncRNAs) with over 200 nucleotides in length affect drug response and toxicity. In the present study, we investigated the association of well-characterized lung cancer lncRNA polymorphisms with platinum-based chemotherapy toxicity in Chinese patients with lung cancer. A total of 467 lung cancer patients treated with platinum-based chemotherapy for at least two cycles were recruited. We primarily focused on gastrointestinal and hematological toxicities. A total of 14 potentially functional polymorphisms within 8 lncRNAs (HOTTIP, HOTAIT, H19, ANRIL, CCAT2, MALAT1, MEG3, and POLR2E) were genotyped. Unconditional logistical regression analysis was conducted to assess the associations. Gene-gene and gene-environment interactions were identified using the software generalized multifactor dimensionality reduction (GMDR). ANRIL rs1333049 was associated with severe overall toxicity in an additive model (adjusted OR=0.723, 95% CI=0.541–0.965, P=0.028). ANRIL rs1333049 was also associated with severe gastrointestinal toxicity in both the additive (adjusted OR=0.690, 95% CI=0.489–0.974, P=0.035) and dominant (adjusted OR=0.558, 95% CI=0.335–0.931, P=0.025) models. MEG3 rs116907618 was associated with severe gastrointestinal toxicity in an additive model (adjusted OR=1.717, 95% CI=1.007–2.927, P=0.047). GMDR identified the three-factor interaction model of POLR2E rs3787016-HOTTIP rs3807598-chemotherapy regimen as the best predictive model for hematological toxicity. In conclusion, ANRIL and MEG3 genetic polymorphisms are associated with severe platinum toxicity and could be considered as biomarkers for pretreatment evaluation in Chinese patients with lung cancer.

An ERCC4 Regulatory Variant Predicts Grade-3 Or-4 Toxicities in Patients with Advanced Non-Small Cell Lung Cancer Treated by Platinum-Based Therapy

Abstract:Platinum‐based chemotherapy (PBC) in combination with the 3rd generation drugs is the first‐line treatment for patients with advanced non‐small cell lung cancer (NSCLC); however, the efficacy is severely hampered by grade 3–4 toxicities. Nucleotide excision repair (NER) pathway is the main mechanism of removing platinum‐induced DNA adducts that contribute to the toxicity and outcome of PBC. We analyzed data from 710 Chinese NSCLC patients treated with PBC and assessed the associations of 25 potentially functional single nucleotide polymorphisms (SNPs) in nine NER core genes with overall, gastrointestinal and hematologic toxicities. Through a two‐phase study, we found that ERCC4 rs1799798 was significantly associated with overall and gastrointestinal toxicities [all patients: GA/AA vs. GG, odds ratio (OR)adj=1.61 and 2.35, 95% confidence interval (CI)=1.11–2.33 and 1.25–4.41, and Padj=0.012 and 0.008, respectively]. Our prediction model for the overall toxicity incorporating rs1799798 demonstrated a significant increase in the area under the curve (AUC) value, compared to that for clinical factors only (all patients: AUC = 0.61 vs. 0.59, 95% CI = 0.57–0.65 vs. 0.55–0.63, P = 0.010). Furthermore, the ERCC4 rs1799798 A allele was associated with lower ERCC4 mRNA expression levels according to the expression quantitative trait loci (eQTL) analysis. Our study provided some new clue in future development of biomarkers for assessing toxicity and outcomes of platinum drugs in lung cancer treatment.

Abstract:Lung cancer is the leading cause of tumor-derived death. Although target therapy is proven very efficient, traditionally platinum-based chemotherapies are still primary treatment for most patients. Platinum can suppress the tumor growth and impair normal cells together. The primary aim of the present study was to study the potential role of translesion synthesis (TLS) that might play in platinum-chemotherapy tolerance and side-effect. In present study, a total of 663 patients who were newly histologically diagnosed with advanced NSCLC (aNSCLC) were enrolled. Treatment response was classified into four categories: complete response, partial response, stable disease, and progressive disease. Incidence of gastrointestinal and hematological toxicities was assessed twice a week during the whole first-line treatment. Eleven SNPs of POLK were genotyped. The associations between SNPs and treatment response or toxicity were analyzed with logistic regression model. Cox regression was used for survival analysis between SNPs and progression-free survival or overall survival. We identified that rs3213801 and rs5744533 showed complete linkage in the present study, and they were significantly associated with treatment response (adjusted P = 0.044), together with rs5744655 (adjusted P = 0.039). rs1018119 was correlated with gastrointestinal toxicity in smokers specially (adjusted P = 0.041). Besides, rs3756558 was associated with hematological toxicity and overall toxicity in smokers and combined cohort with additive model. We also identified the significant association between two SNPs, rs10077427 and rs5744545, and PFS. The polymorphism of POLK, an important gene in TLS, participates in platinum-chemotherapy tolerance and side-effect.

Abstract:Objective:To investigate the relationship between genetic polymorphism in 105 codons of the glutathione S-transferase P1(GSTP1) gene and patient sensitivity to platinum-based chemotherapy.Method:A total of 50 cancer patients were enrolled in our study,which were confirmed by pathologic diagnosis and CT scanning to have measurable lesions and then subjected to platinum -based chemotherapy.Peripheral blood was obtained before the therapy,and direct sequencing was performed to detect single nucleotide polymorphisms(SNPs) in the 105 codons of GSTP1 in peripheral blood.The clinical responses of these patients after 2 to 4 cycles of chemotherapy were observed.The relationship between SNPs of the GSTPl gene and clinical responses was analyzed.Results: No correlation was found between the distribution of GSTP1 genotypes and different cancers(P>0.05).The response rates to chemotherapy in patients with the Ile/Ile,Ile/Val,and Val/Val genotypes in the 105 GSTP1 codons were 20.0%,36.4%and 87.5%,respectively. The response rate was higher in the patients with the homozygous Val/Val mutant genotype than in those with the Ile/Ile or Ile/Val genotypes(P<0.05).Conclusion:Single nucleotide polymorphisms of the GSTPl gene may be associated with sensitivity to platinum-based chemotherapy.
Predictive Role of MMP-2 Genetic Variants on Severe Hematologic Toxicity of NSCLC Patients Treated with First-Line, Platinum-Based Chemotherapy

Abstract:Purpose: MMP-2 plays an important role in hematopoietic recovery after chemotherapy-induced myelosuppression. In this study, we investigated the association between MMP-2 SNPs and the incidence of adverse hemtologic events in advanced NSCLC patients treated with platinum-based chemotherapy. Methods: A retrospective pharmacogenetic association study was performed in 1004 Chinese patients with advanced NSCLC receiving platinum-based regimens (including NP, GP, TC, TP). Information about grade 3 or 4 hematologic toxicity (neutropenia, anemia, thrombocytopenia) was available. 16 tag SNPs of MMP-2 were assessed. Results: There were 6 SNPs significantly associated with risk of grade 3 or 4 hematologic toxicity in platinum-based treatment. The variant homozygotes of rs12934241 exhibited the most significant influence on the occurrence of severe neutropenia ( P =9.3×10 -5 ). In stratification analysis by chemotherapy regiments, we found the greates correlations in patients receiving cisplatin-gemcitabine: 7 polymorphisms were found be associated with severe neutropenia, especially rs12934241 (2.9% for C/C v 66.7% for T/T; P =4.9×10 -5 ). Consistent results were found in MMP-2 haplotype analyses. In patients receiving cisplatin-navelbine, we also observed 6 SNPs significantly associated with grade 3 or 4 hemtologic toxicity. However, in cisplatin/carboplatin-paclitaxel treatment groups, no correlation with such toxicity was found. Conclusions: Our study, for the first time, provides evidence for the predictive role of MMP-2 polymorphisms on severe chemotherapy-related hematologic toxicity variability among platinum-treated advanced NSCLC Chinese patients.

Abstract:PURPOSE: The Bcl2 family members have dual functions in apoptosis. Platinum agents cause genetic lesions and cell death that may be modulated by the normal pathway of apoptosis regulated by the Bcl2 family genes. We hypothesized that genetic polymorphisms of these genes have an impact on the outcome of platinum-based chemotherapy. The aim of our research was to assess whether single nucleotide polymorphism (SNPs) of BAD, BAX, PUMA, BID, and BCL2 in a pathway of regulating apoptosis affect the response to and toxicity in patients with NSCLC treated with platinum-based chemotherapy. METHODS AND METERIALS: We genotyped 64 SNPS of 5 genes by Illumina Custom Designed Chip in 1004 patients with advanced NSCLC treated with platinum-based chemotherapy. We investigated the response and the toxicity of patients after platinum-based chemotherapy. The genotyping data were analyzed by the Sofeware of SPSS12.0, PHASE2.1 and Haploview. RESULTES: A SNP of BID (rs5746474) was significantly associated with the hematological toxicity. Compared with the wild-type GG genotype, the variant AA genotype was associated with significantly increased risk to hematological toxicity (adjusted OR: 2.4344, 95%CI=1.4084–4.2077, p=0.0014) in a recessive model (adjusted OR: 2.5241, 95% CI = 1.4839–4.2933, P=0.0006). In the allelic analysis, a SNP of BCL2 (rs8099575) showed an effect on gastrointestinal toxicity (adjusted OR: 0.3041, 95% CI=0.1524–0.6.68, P=0.0007). Several haplotypes of these genes were associated with the response and toxicity; however, after Bonfernoni correction, no association between haplotypes and clinical outcome remained. CONCLUSION: Genetic polymorphisms in the BID and BCL2 genes may have an impact on the response to and toxicity of platinum-based chemotherapy in patients with NSCLC, which may be prognostic biomarkers of chemotherapy in NSCLC patients, if validated in larger studies.
