Third Year Medical Student Reflections on their First Virtual Post‐Acute Care Teaching Experience and Witnessing a Real‐Time Advance Care Planning Discussion TopicRepresentative ReflectionsHearing a real‐time spontaneous goals of care discussionThis was an emotional experience for me. Although we had lectures to prepare us for these hard conversations, it is a different experience listening in real time. This real‐time experience showed me an example on how to talk to the family members about these difficult decisions in an unbiased and professional manner.I really valued this experience and hope that I can experience more end‐of‐life discussions in real‐time before I am tasked with conducting this conversation myself as a physician.This was a very emotional and educational experience for me. The timing of this only a few weeks after a simulation center event where we learned and practiced discussing end of life issues was a good progression. While the simulation center with a standardized patient was educational and helped me to understand and develop a framework for these challenging conversations, it is difficult to simulate the emotions of a real end of life clinical experience.One truly exceptional learning experience involved a spontaneous real‐time family meeting in which a decision was made to pursue comfort care rather than repeated hospitalizations.This was a rare opportunity observe a real‐life but difficult conversation.I acknowledge it is rare to be present during conversations like these‐ so I wanted to make the most of it by absorbing as much as I could. I feel incredibly fortunate to have witnessed, as a student, the implementation of advance care planning and advance directives in a stressful situation as this was.My training adequately prepared me for what advanced care planning and advanced directives entail; however, it was beneficial experiencing an end‐of‐life discussion in real time. It is one thing to run this difficult scenario in my head, but it is a completely different experience seeing it done in real time by an expert.While it was true that the son agreed that transitioning to comfort care was the best choice, it was still saddening to hear him ask questions and discuss the decision with his sister, who was also on the phone call.Effectiveness of the communication about the care plan and key learnings about advance care planning at the end of lifeI was able to sit at home and listen to the attending have a conversation with the patient's children about how their mother's health was rapidly deteriorating. The conversation continued with a discussion about the process of providing her with comfort care at the end of her life. This allowed me to learn first‐hand how such challenging conversations can be effectively handled.Often times, students are anxious about having difficult conversations with patients and their families. Having the opportunity to witness a challenging conversation happen in real‐time helped me to understand how these conversations can be conducted in a professional and successful manner. It also relieved some of my apprehension about having these difficult conversations in the future.The spontaneous family meeting also put into perspective the importance of advanced care planning as well as advance directives. While the spontaneous family meeting allowed family members to be on the same page about the patient's care, had an earlier family meeting taken place, the decisions about the patient's care could have been ascertained before there was a crisis.I especially appreciated that both the son and daughter were on the phone call as we were able to hear the deliberations they had amongst themselves during the discussion as a whole.It was a testament to the importance of deeply understanding your patient‐ from their state of health prior to entering the hospital to now, their hospital course, their likely future hospital course, and the state of their family's concerns, knowledge, and expectations. This all needs to be synthesized for the conversation with family members, and decisions guided by weighing wishes and beliefs against sound medical advice and experience.My most important take‐away from those experiences was that -Abstract Truncated-
