Abstract:Background: Transcranial direct current stimulation (tDCS) is used to modulate neuronal activity, but the exact mechanism of action (MOA) is unclear. This study investigates tDCS-induced modulation of the corticospinal excitability and the underlying MOA. By anesthetizing the scalp before applying tDCS and by stimulating the cheeks, we investigated whether stimulation of peripheral and/or cranial nerves contributes to the effects of tDCS on corticospinal excitability. Materials and methods: In a randomized cross-over study, four experimental conditions with anodal direct current stimulation were compared in 19 healthy volunteers: 1) tDCS over the motor cortex (tDCS-MI), 2) tDCS over the motor cortex with a locally applied topical anesthetic (TA) on the scalp (tDCS-MI + TA), 3) DCS over the cheek region (DCS-C), and 4) sham tDCS over the motor cortex(sham). tDCS was applied for 20 minutes at 1 mA. Motor evoked potentials (MEPs) were measured before tDCS and immediately, 15, 30, 45, and 60 minutes after tDCS. A questionnaire was used to assess the tolerability of tDCS. Results: A significant MEP amplitude increase compared with baseline was found 30 minutes after tDCS-MI, an effect still observed 60 minutes later; no time∗condition interaction effect was detected. In the other three conditions (tDCS-MI + TA, DCS-C, sham), no significant MEP modulation was found. The questionnaire indicated that side effects are significantly lower when the local anesthetic was applied before stimulation than in the other three conditions. Conclusions: The significant MEP amplitude increase observed from 30 minutes on after tDCS-MI supports the modulatory effect of tDCS on corticospinal neurotransmission. This effect lasted one hour after stimulation. The absence of a significant modulation when a local anesthetic was applied suggests that effects of tDCS are not solely established through direct cortical stimulation but that stimulation of peripheral and/or cranial nerves also might contribute to tDCS-induced modulation.
State dependent motor cortex stimulation reveals distinct mechanisms for corticospinal excitability and cortical responses

Abstract:Transcranial magnetic stimulation (TMS) is a non-invasive brain stimulation method which can modulate brain activity by inducing electric fields in the brain. It is a popular tool to study causal brain-behavior relationships. However, brain states vary over time and affect the response of TMS. Neural oscillations can track the current brain state and are a promising marker to guide stimulation timing. Real-time, state-dependent brain stimulation has shown that neural oscillation phase modulates corticospinal excitability reflecting the connection from the primary motor cortex to a target muscle. However, such motor-evoked potentials (MEPs) only indirectly reflect motor cortex activation and are unavailable at other brain regions of interest. The direct and secondary cortical effects of phase-dependent brain stimulation remain an open question. In this study, we recorded the cortical responses during single-pulse transcranial magnetic stimulation (TMS) using electroencephalography (EEG) concurrently with the MEP measurements. TMS was delivered at peak, rising, trough, and falling phases of mu (8-13 Hz) and beta (14-30 Hz) oscillations in the motor cortex. The cortical responses were quantified through TMS-evoked potential components N15, P50, and N100 as peak-to-peak amplitudes (P50-N15 and P50-N100). We further analyzed whether the pre-stimulus frequency band power was predictive of the motor cortical responses. We found a significant main effect of neural oscillation phase on early evoked component (P50-N15). Furthermore, we found an interaction effect of oscillation phase and frequency on both early and late (P50-N100) components. Next, we compared the direct EEG response to the corticospinal excitability reflected by MEP amplitude. Interestingly, the preferred phase of the mu rhythm showed a 900 phase shift between the early TEP components and MEPs. The late component showed the same phase preference between EEG and MEPs. However, such a well-defined relationship did not exist for either of the components during beta phase specific stimulation. In addition, pre-TMS mu oscillatory power and phase significantly predicted both early and late cortical EEG responses when mu rhythm was targeted, indicating the independent causal effects of phase and power. However, only pre-TMS beta power significantly predicted the early and late TEP components when beta rhythm was targeted. Further analysis indicated that both pre-TMS mu and beta power jointly affect early cortical responses. In contrast, the late cortical responses were only influenced by pre-TMS mu power. These findings provide insight to mechanistic understanding of neural oscillation states in cortical and corticospinal activation in humans.

Long-term changes in functional connectivity predict responses to intracranial stimulation of the human brain

Abstract:Targeted electrical stimulation of the brain perturbs neural networks and modulates their rhythmic activity both at the site of stimulation and at remote brain regions. Understanding, or even predicting, this neuromodulatory effect is crucial for any therapeutic use of brain stimulation. To this end, we analyzed the stimulation responses in 131 stimulation sessions across 66 patients with focal epilepsy recorded through intracranial EEG (iEEG). We considered functional and structural connectivity features as predictors of the response at every iEEG contact. Taking advantage of multiple recordings over days, we also investigated how slow changes in interictal functional connectivity (FC) ahead of the stimulation relate to stimulation responses. The results reveal that, indeed, this long-term variability of FC exhibits strong association with the stimulation-induced increases in delta and theta band power. Furthermore, we show through cross-validation that long-term variability of FC improves prediction of responses above the performance of spatial predictors alone. These findings can enhance the patient-specific design of effective neuromodulatory protocols for therapeutic interventions.

Abstract:State-dependent non-invasive brain stimulation (NIBS) informed by electroencephalography (EEG) has contributed to the understanding of NIBS inter-subject and inter-session variability. While these approaches focused on local EEG characteristics, it is acknowledged that the brain exhibits an intrinsic long-range dynamic organization in networks. This proof-of-concept study explores whether EEG connectivity of the primary motor cortex (M1) in the pre-stimulation period aligns with the motor network (MN) and how MN state affects responses to transcranial magnetic stimulation (TMS) of M1. One thousand suprathreshold TMS pulses were delivered to left M1 in 8 subjects at rest, with simultaneous EEG. Motor evoked potentials (MEPs) were measured from the right hand. Source-space functional connectivity of left M1 to the whole-brain was assessed using the imaginary part of the Phase Locking Value at the frequency of the sensorimotor µ-rhythm in a 1-second window before the pulse. Group-level connectivity revealed functional links between left M1, left supplementary motor area, and right M1. Also, pulses delivered at high MN connectivity states result in a greater MEP amplitude compared to low connectivity states. At single-subject level, this relation is more expressed in subjects that feature an overall high cortico-spinal excitability. In conclusion, this study paves the way for MN connectivity based NIBS.

Abstract:Transcranial magnetic stimulation (TMS) has a wide range of clinical applications, and there is growing interest in neural oscillations and corticospinal excitability determined by TMS. Previous studies have shown that corticospinal excitability is influenced by fluctuations of brain oscillations in the sensorimotor region, but it is unclear whether brain network activity modulates corticospinal excitability. Here, we addressed this question by recording electroencephalography (EEG) and TMS measurements in 32 healthy individuals. The resting motor threshold (RMT) and active motor threshold (AMT) were determined as markers of corticospinal excitability. The least absolute shrinkage and selection operator (LASSO) was used to identify significant EEG metrics and then correlation analysis was performed. The analysis revealed that alpha2 power in the sensorimotor region was inversely correlated with RMT and AMT. Innovatively, graph theory was used to construct a brain network, and the relationship between the brain network and corticospinal excitability was explored. It was found that the global efficiency in the theta band was positively correlated with RMT. Additionally, the global efficiency in the alpha2 band was negatively correlated with RMT and AMT. These findings indicated that corticospinal excitability can be modulated by the power spectrum in sensorimotor regions and the global efficiency of functional networks. EEG network analysis can provide a useful supplement for studying the association between EEG oscillations and corticospinal excitability.

Abstract:Objective The corticospinal responses of the motor network to transcranial magnetic stimulation (TMS) are highly variable. While often regarded as noise, this variability provides a way of probing dynamic brain states related to excitability. We aimed to uncover spontaneously occurring cortical states that alter corticospinal excitability. Approach Electroencephalography (EEG) recorded during TMS registers fast neural dynamics -- unfortunately, at the cost of anatomical precision. We employed analytic Common Spatial Patterns (aCSP) technique to derive excitability-related cortical activity from pre-TMS EEG signals while overcoming spatial specificity issues. Main results High corticospinal excitability was predicted by alpha-band activity, localized adjacent to the stimulated left motor cortex, and suggesting a travelling wave-like phenomenon towards frontal regions. Low excitability was predicted by alpha-band activity localized in the medial parietal--occipital and frontal cortical regions. Significance We established a data-driven approach for uncovering network-level neural activity that modulates TMS effects. It requires no prior anatomical assumptions, while being physiologically interpretable, and can be employed in both exploratory investigation and brain state-dependent stimulation.

Abstract:Transcranial direct current stimulation (tDCS) can noninvasively modulate behavior, cognition, and physiologic brain functions depending on polarity and dose of stimulation as well as montage of electrodes. Concurrent tDCS-fMRI presents a novel way to explore the parameter space of non-invasive brain stimulation and to inform the experimenter as well as the participant if a targeted brain region or a network of spatially separate brain regions has been engaged and modulated. We compared a multi-electrode (ME) with a single electrode (SE) montage and both active conditions with a no-stimulation (NS) control condition to assess the engagement of a brain network and the ability of different electrode montages to modulate network activity. The multi-electrode montage targeted nodal regions of the right Arcuate Fasciculus Network (AFN) with anodal electrodes placed over the skull position of the posterior superior temporal/middle temporal gyrus (STG/MTG), supramarginal gyrus (SMG), posterior inferior frontal gyrus (IFG) and a return cathodal electrode over the left supraorbital region. In comparison, the single electrode montage used only one anodal electrode over a nodal brain region of the AFN, but varied the location between STG/MTG, SMG, and posterior IFG for different participants. Whole-brain rs-fMRI was obtained approximately every three seconds. The tDCS-stimulator was turned on at 3 minutes after the scanning started. A 4D rs-fMRI data set was converted to dynamic functional connectivity (DFC) matrices using a set of ROI pairs belonging to the AFN as well as other unrelated brain networks. In this study, we evaluated the performance of five algorithms to classify the DFC matrices from the three conditions (ME, SE, NS) into three different categories. The highest accuracy of 0.92 was obtained for the classification of the ME condition using the K Nearest Neighbor (KNN) algorithm. In other words, applying the classification algorithm allowed us to identify the engagement of the AFN and the ME condition was the best montage to achieve such an engagement. The top 5 ROI pairs that made a major contribution to the classification of participant's rs-fMRI data were identified using model performance parameters; ROI pairs were mainly located within the right AFN. This proof-of-concept study using a classification algorithm approach can be expanded to create a near real-time feedback system at a participant level to detect the engagement and modulation of a brain network that spans multiple brain lobes.

Abstract:Background Cortical excitability measures neural reactivity to stimuli, usually delivered via Transcranial Magnetic Stimulation (TMS). Excitation/inhibition balance (E/I) is the ongoing equilibrium between excitatory and inhibitory activity of neural circuits. According to some studies, E/I could be estimated in-vivo and non-invasively through the modeling of electroencephalography (EEG) signals and termed 'intrinsic excitability' measures. Several measures have been proposed (phase consistency in the gamma band, sample entropy, exponent of the power spectral density 1/f curve, E/I index extracted from detrend fluctuation analysis, and alpha power). Intermittent theta burst stimulation (iTBS) of the primary motor cortex (M1) is a non-invasive neuromodulation technique allowing controlled and focal enhancement of TMS cortical excitability and E/I of the stimulated hemisphere. Objective Investigating to what extent E/I estimates scale with TMS excitability and how they relate to each other. Methods M1 excitability (TMS) and several E/I estimates extracted from resting state EEG recordings were assessed before and after iTBS in a cohort of healthy subjects. Results Enhancement of TMS M1 excitability, as measured through motor-evoked potentials (MEPs), and phase consistency of the cortex in high gamma band correlated with each other. Other measures of E/I showed some expected results, but no correlation with TMS excitability measures or strong consistency with each other. Conclusions EEG E/I estimates offer an intriguing opportunity to map cortical excitability non-invasively, with high spatio-temporal resolution and with a stimulus independent approach. While different EEG E/I estimates may reflect the activity of diverse excitatory-inhibitory circuits, spatial phase synchrony in the gamma band is the measure that best captures excitability changes in the primary motor cortex.

Abstract:The modular organization of the cortex refers to subsets of highly interconnected nodes, sharing specific cytoarchitectural and dynamical properties. These properties condition the level of excitability of local pools of neurons. In this study, we described TMS evoked potentials (TEP) input–output properties to provide new insights into regional cortical excitability. We combined robotized TMS with EEG to disentangle region‐specific TEP from threshold to saturation and describe their oscillatory contents. Twenty‐two young healthy participants received robotized TMS pulses over the right primary motor cortex (M1), the right dorsolateral prefrontal cortex (DLPFC) and the right superior occipital lobe (SOL) at five stimulation intensities (40, 60, 80, 100, and 120% resting motor threshold) and one short‐interval intracortical inhibition condition during EEG recordings. Ten additional subjects underwent the same experiment with a realistic sham TMS procedure. The results revealed interregional differences in the TEPs input–output functions as well as in the responses to paired‐pulse conditioning protocols, when considering early local components (<80 ms). Each intensity in the three regions was associated with complex patterns of oscillatory activities. The quality of the regression of TEPs over stimulation intensity was used to derive a new readout for cortical excitability and dynamical properties, revealing lower excitability in the DLPFC, followed by SOL and M1. The realistic sham experiment confirmed that these early local components were not contaminated by multisensory stimulations. This study provides an entirely new analytic framework to characterize input–output relations throughout the cortex, paving the way to a more accurate definition of local cortical excitability.

Abstract:Over the last decade, Transcranial Direct Current Stimulation (tDCS) has been applied not only to modulate local cortical activation, but also to address communication between functionally-related brain areas. Stimulation protocols based on simple two-electrode placements are being replaced by multi-electrode montages to target intra- and inter-hemispheric neural networks using multichannel/high definition paradigms. Objective: This study aims to investigate the characteristics of electric field (EF) patterns originated by tDCS experiments addressing changes in functional brain connectivity. Methods: A previous selection of tDCS experimental studies aiming to modulate motor-related connectivity in health and disease was conducted. Simulations of the EF induced in the cortex were then performed for each protocol selected. The EF magnitude and orientation are determined and analysed in motor-related cortical regions for five different head models to account for inter-subject variability. Functional connectivity outcomes obtained are qualitatively analysed at the light of the simulated EF and protocol characteristics, such as electrode position, number and stimulation dosing. Main findings: The EF magnitude and orientation predicted by computational models can be related with the ability of tDCS to modulate brain functional connectivity. Regional differences in EF distributions across subjects can inform electrode placements more susceptible to inter-subject variability in terms of brain connectivity-related outcomes. Significance: Neuronal facilitation/inhibition induced by tDCS fields may indirectly influence intra and inter-hemispheric connectivity by modulating neural components of motor-related networks. Optimization of tDCS using computational models is essential for adequate dosing delivery in specific networks related to clinically relevant connectivity outcomes.

Abstract:TO THE EDITOR: We appreciate Dr. Sekhon’s comments on our recent article evaluating high-dose ursodeoxycholic acid (UDCA) as a therapy for patients with primary sclerosing cholangitis (PSC) (1). The Mayo risk score is a numerical factor that can be extrapolated into survival probability within 1–4 yr of its calculation for an individual patient with PSC (2). It is the difference in risk score and the corresponding survival probability between high-dose UDCA and the placebo that was statistically compared and reported in the original manuscript (1). Our study (1) was a small, open label, 1-yr duration pilot study. Although improvement in liver biochemistries, at least in the short term, is expected to occur in most patients with PSC under UDCA therapy, 1 yr of treatment is not enough to determine the effect of treatment on clinically relevant endpoints such as disease progression and long term actual survival. Hence, the Mayo risk score and its corresponding survival probability (2) may be a more accurate surrogate marker of treatment effect after this relatively short treatment period. Nevertheless, as we concluded in our study (1), these results are not unequivocal evidence of a therapeutic benefit of high-dose UDCA in PSC, but such results are, in our view, enough to prompt evaluation of high-dose UDCA in a placebo-controlled trial that must possess enough power, have adequate duration of follow-up, and include clinically relevant endpoints. Such a study, which has already been designed, will allow us to determine the effect of high-dose UDCA on actual survival and disease progression, thus determining if the predicted benefit in survival estimated by changes in the Mayo risk score occurring during therapy is appropriately matched by actual survival of enrolled patients. Dr. Sekhon’s concerns regarding the construction of the original multicenter Mayo model (3) and the revised model (2) are well taken; however, whether or not these models were constructed using statistically inferior methodology is out of the scope of our study.
Cortical oscillatory activity and the induction of plasticity in the human motor cortex

Abstract:Repetitive transcranial magnetic stimulation paradigms such as continuous theta burst stimulation (cTBS) induce long-term potentiation- and long-term depression-like plasticity in the human motor cortex. However, responses to cTBS are highly variable and may depend on the activity of the cortex at the time of stimulation. We investigated whether power in different electroencephalogram (EEG) frequency bands predicted the response to subsequent cTBS, and conversely whether cTBS had after-effects on the EEG. cTBS may utilize similar mechanisms of plasticity to motor learning; thus, we conducted a parallel set of experiments to test whether ongoing electroencephalography could predict performance of a visuomotor training task, and whether training itself had effects on the EEG. Motor evoked potentials (MEPs) provided an index of cortical excitability pre- and post-intervention. The EEG was recorded over the motor cortex pre- and post-intervention, and power spectra were computed. cTBS reduced MEP amplitudes; however, baseline power in the delta, theta, alpha or beta frequencies did not predict responses to cTBS or learning of the visuomotor training task. cTBS had no effect on delta, theta, alpha or beta power. In contrast, there was an increase in alpha power following visuomotor training that was positively correlated with changes in MEP amplitude post-training. The results suggest that the EEG is not a useful state-marker for predicting responses to plasticity-inducing paradigms. The correlation between alpha power and changes in corticospinal excitability following visuomotor training requires further investigation, but may be related to disengagement of the somatosensory system important for motor memory consolidation.
Motor cortical plasticity in extrinsic hand muscles is determined by the resting thresholds of overlapping representations

Abstract:Knowledge of the properties that govern the effectiveness of transcranial magnetic stimulation (TMS) interventions is critical to clinical application. Extrapolation to clinical populations has been limited by high inter-subject variability and a focus on intrinsic muscles of the hand in healthy populations. Therefore, the current study assessed variability of continuous theta burst stimulation (cTBS), a patterned TMS protocol, across an agonist-antagonist pair of extrinsic muscles of the hand. Secondarily, we assessed whether concurrent agonist contraction could enhance the efficacy of cTBS. Motor evoked potentials (MEP) were simultaneously recorded from the agonist flexor (FCR) and antagonist extensor (ECR) carpi radialis before and after cTBS over the FCR hotspot. cTBS was delivered with the FCR relaxed (cTBS-Relax) or during isometric wrist flexion (cTBS-Contract). cTBS-Relax suppressed FCR MEPs evoked from the FCR hotspot. However, the extent of FCR MEP suppression was strongly correlated with the relative difference between FCR and ECR resting motor thresholds. cTBS-Contract decreased FCR suppression but increased suppression of ECR MEPs elicited from the FCR hotspot. The magnitude of ECR MEP suppression following cTBS-Contract was independent of the threshold-amplitude relationships observed with cTBS-Relax. Contraction alone had no effect confirming the effect of cTBS-Contract was driven by the interaction between neuromuscular activity and cTBS. Interactions across muscle representations should be taken into account when predicting cTBS outcomes in healthy and clinical populations. Contraction during cTBS may be a useful means of focusing aftereffects when differences in baseline excitability across overlapping agonist-antagonist cortical representations may mitigate the inhibitory effect of cTBS.

Abstract:Background and Purpose Transcranial direct current stimulation (tDCS) is an emerging non-invasive neuromodulation technique for focal epilepsy. Because epilepsy is a disease affecting the brain network, our study was aimed to evaluate and predict the treatment outcome of cathodal tDCS (ctDCS) by analyzing the ctDCS-induced functional network alterations. Methods Either the active 5-day, −1.0 mA, 20-min ctDCS or sham ctDCS targeting at the most active interictal epileptiform discharge regions was applied to 27 subjects suffering from focal epilepsy. The functional networks before and after ctDCS were compared employing graph theoretical analysis based on the functional magnetic resonance imaging (fMRI) data. A support vector machine (SVM) prediction model was built to predict the treatment outcome of ctDCS using the graph theoretical measures as markers. Results Our results revealed that the mean clustering coefficient and the global efficiency decreased significantly, as well as the characteristic path length and the mean shortest path length at the stimulation sites in the fMRI functional networks increased significantly after ctDCS only for the patients with response to the active ctDCS (at least 20% reduction rate of seizure frequency). Our prediction model achieved the mean prediction accuracy of 68.3% (mean sensitivity: 70.0%; mean specificity: 67.5%) after the nested cross validation. The mean area under the receiver operating curve was 0.75, which showed good prediction performance. Conclusion The study demonstrated that the response to ctDCS was related to the topological alterations in the functional networks of epilepsy patients detected by fMRI. The graph theoretical measures were promising for clinical prediction of ctDCS treatment outcome.

Abstract:Objective: The literature investigating the effects of alpha oscillations on corticospinal excitability is divergent. We believe inconsistency in the findings may arise, among others, from the EEG processing for brain-state determination. Here, we provide further insights in the effects of the brain-state on cortical and corticospinal excitability and quantify the impact of different EEG processing. Approach: Corticospinal excitability was measured using motor evoked potential (MEP) peak-to-peak amplitudes elicited with transcranial magnetic stimulation (TMS); cortical responses were studied through TMS-evoked potentials' (TEPs) features. A TMS-EEG-EMG dataset of 18 young healthy subjects who received 180 single-pulse and 180 paired pulses to determine short-intracortical inhibition was investigated. To study the effect of different EEG processing, we compared the brain-state estimation deriving from three published methods. The influence of presence of neural oscillations was also investigated. To evaluate the effect of the brain-state on MEP and TEP features variability, we defined the brain-state based on specific EEG phase and power combinations, only in trials where neural oscillations were present. The relationship between TEPs and MEPs was further evaluated. Main results: The presence of neural oscillations resulted in more consistent results regardless of the EEG processing approach. Nonetheless, the latter still critically affected the outcomes, making conclusive claims complex. With our approach, the MEP amplitude was positively modulated by the alpha power and phase, with stronger responses during the trough phase and high power. Power and phase also affected TEP features. Importantly, similar effects were observed in both TMS conditions. Significance: These findings support the view that the brain state of alpha oscillations is associated with the variability observed in cortical and corticospinal responses to TMS, with a tight correlation between the two. The results further highlight the importance of closed-loop stimulation approaches while underlining that care is needed in designing experiments and choosing the analytical approaches, which should be based on knowledge from offline studies to control for the heterogeneity originating from different EEG processing strategies.

Abstract:Dynamics within and between functional resting‐state networks have a crucial role in determining both healthy and pathological brain functioning in humans. The possibility to noninvasively interact and selectively modulate the activity of networks would open to relevant applications in neuroscience. Here we tested a novel approach for multichannel, network‐targeted transcranial direct current stimulation (net‐tDCS), optimized to increase excitability of the sensorimotor network (SMN) while inducing cathodal inhibitory modulation over prefrontal and parietal brain regions negatively correlated with the SMN. Using an MRI‐compatible multichannel transcranial electrical stimulation (tES) device, 20 healthy participants underwent real and sham tDCS while at rest in the MRI scanner. Changes in functional connectivity (FC) during and after stimulation were evaluated, looking at the intrinsic FC of the SMN and the strength of the negative connectivity between SMN and the rest of the brain. Standard, bifocal tDCS targeting left motor cortex (electrode ~C3) and right frontopolar (~Fp2) regions was tested as a control condition in a separate sample of healthy subjects to investigate network specificity of multichannel stimulation effects. Net‐tDCS induced greater FC increase over the SMN compared to bifocal tDCS, during and after stimulation. Moreover, exploratory analysis of the impact of net‐tDCS on negatively correlated networks showed an increase in the negative connectivity between SMN and prefrontal/parietal areas targeted by cathodal stimulation both during and after real net‐tDCS. Results suggest preliminary evidence of the possibility of manipulating distributed network connectivity patterns through net‐tDCS, with potential relevance for the development of cognitive enhancement and therapeutic tES solutions.

Abstract:Higher-order cognitive and affective functions are supported by large-scale networks in the brain. Dysfunction in different networks is proposed to associate with distinct symptoms in neuropsychiatric disorders. However, the specific networks targeted by current clinical transcranial magnetic stimulation (TMS) approaches are unclear. While standard-of-care TMS relies on scalp-based landmarks, recent FDA-approved TMS protocols use individualized functional connectivity with the subgenual anterior cingulate cortex (sgACC) to optimize TMS targeting. Leveraging previous work on precision network estimation and recent advances in network-level TMS targeting, we demonstrate that clinical TMS approaches target different functional networks between individuals. Homotopic scalp positions (left F3 and right F4) target different networks within and across individuals, and right F4 generally favors a right-lateralized control network. We also modeled the impact of targeting the dorsolateral prefrontal cortex (dlPFC) zone anticorrelated with the sgACC and found that the individual-specific anticorrelated region variably targets a network coupled to reward circuitry. Combining individualized, precision network mapping and electric field (E-field) modeling, we further illustrate how modeling can be deployed to prospectively target distinct closely localized association networks in the dlPFC with meaningful spatial selectivity and E-field intensity and retrospectively assess network engagement. Critically, we demonstrate the feasibility and reliability of this approach in an independent cohort of participants (including those with Major Depressive Disorder) who underwent repeated sessions of TMS to distinct networks, with precise targeting derived from a low-burden single session of data. Lastly, our findings emphasize differences between selectivity and maximal intensity, highlighting the need to consider both metrics in precision TMS efforts.

Abstract:Abstract Corticospinal volleys evoked by transcranial magnetic stimulation (TMS) over the primary motor cortex (M1) consist of high-frequency bursts (≈667 and ≈333 Hz). However, intracortical circuits producing such corticospinal high-frequency bursts are unknown. We here investigated whether neurons activated by single TMS pulses over M1 are resonant to high-frequency oscillations, using a combined transcranial alternating current stimulation (tACS)-TMS approach. We applied 667, 333 Hz or sham-tACS and, concurrently, we delivered six single-pulse TMS protocols using monophasic or biphasic pulses, different stimulation intensities, muscular states, types and orientations of coils. We recorded motor evoked potentials (MEPs) before, during and after tACS. 333 Hz tACS facilitated MEPs evoked by biphasic TMS through a figure-of-eight coil at active motor threshold (AMT), and by monophasic TMS with anterior-to-posterior-induced current in the brain. 333 Hz tACS also facilitated MEPs evoked by monophasic TMS through a circular coil at AMT, an effect that weakly persisted after the stimulation. 667 Hz tACS had no effects. 333 Hz, but not 667 Hz, tACS may have reinforced the synchronization of specific neurons to high-frequency oscillations enhancing this activity, and facilitating MEPs. Our findings suggest that different bursting modes of corticospinal neurons are produced by separate circuits with different oscillatory properties.

Abstract:Putative neurosecretory cells were observed with the electron microscope in the nerve cords of the neck region of Hymenolepis microstoma. The cells show evidence of glandular activity by the large numbers of dense-core vesicles produced by the Golgi apparatus. The axons contain synaptoidlike structures with surrounding clouds of vesicles; features analogous to known neurosecretory release sites.
