Abstract:Chronic subclinical inflammation represents a risk factor of type 2 diabetes and several diabetes complications, including neuropathy and atherosclerosis including macro-vasculopathy and micro-vasculopathy. However, the inflammatory response in the diabetic wound was shown to be remarkably hypocellular, unregulated and ineffective. Advanced glycation end products (AGEs) and one of its receptors, RAGE, were involved in inducing chronic immune imbalance in diabetic patients. Such interactions attracts immune cell into diffused glycated tissue and activates these cells to induce inflammatory damage, but disturbs the normal immune rhythm in diabetic wound. Traditional measurements of AGEs are high-performance liquid chromatography and immunohistochemistry staining, but their application faces the limitations including complexity, cost and lack of reproducibility. A new noninvasive method emerged in 2004, using skin autofluorescence as indicator for AGEs accumulation. It had been reported to be informative in evaluating the chronic risk of diabetic patients. Studies have indicated therapeutic potentials of anti-AGE recipes. These recipes can reduce AGE absorption/de novo formation, block AGE-RAGE interaction and arrest downstream signaling after RAGE activation. Copyright (c) 2014 John Wiley & Sons, Ltd.
Nodular Glomerular Lesion: A Later Stage of Diabetic Nephropathy?

Abstract:Background: Two typical pathological changes can develop in the type 2 diabetic nephropathy: one is diffuse glomerulosclerosis (DIF) and the other is nodular glomerular lesion (NOD). The aim of this study is to retrospectively investigate these two types of diabetic nephropathy and compare the characteristics of clinical findings and prognosis.Methods: One hundred and twenty-four patients with biopsy-proven diabetic nephropathy, including 58 DIF and 66 NOD were investigated in this study. Clinical data were registered for each patient: age, sex, diabetes duration, hypertension duration, body mass index (BMI), and laboratory tests included: glycosylated hemoglobin, plasma albumin, plasma cholesterol and triglycerides, serum creatinine, proteinuria, urine osmole, urine N-acetyl-beta-D-glucosaminidase (NAG) enzyme and creatinine clearance rate (Ccr). Furthermore, the 1- and 5-year renal survival rates were analysed and compared.Results: (1) Compared with DIF patients, NOD patients had a longer duration of diabetic mellitus, and higher levels of proteinuria and NAG enzyme, along with lower BMI, Ccr and plasma albumin levels (p < 0.01). (2) In the NOD cases, there were 90% presented diabetic retinopathy; while in the DIF cases, there were 14% presented diabetic retinopathy. (3) In the DIF patients, the 1- and 5-year renal survival rates were 96 and 78%, respectively, while in the NOD patients, the rates were 86 and 18%, respectively (p < 0.01).Conclusion: The NOD patients had longer diabetic durations, more severe renal damage, and exhibit a poorer renal prognosis, Furthermore, the NOD patients revealed more frequently evidence of diabetic retinopathy. Abstract:Key Points: People with glomerular disease (GD) and comorbid diabetes have similar baseline characteristics irrespective of superimposed diabetic lesions.Immunosuppression for GD with comorbid diabetes is the same regardless of superimposed diabetic glomerular lesions.ESKD or death is more rapid in GD and comorbid diabetes only in the presence of moderate-severe diabetic glomerular lesions. Background: We aimed to evaluate whether concomitant diabetic glomerulosclerosis (DGS) and its severity affect the treatment and outcomes of primary glomerular diseases (GDs) with comorbid diabetes. Methods: We conducted a retrospective review of people with diabetes and GD. We searched the GD Collaborative Network for biopsies from 2008 to 2015 among persons with diabetes and any of the following diagnoses: FSGS, IgA nephropathy, minimal change disease, membranous nephropathy, or antineutrophil cytoplasmic autoantibody GN. Data were abstracted from health records and histologic diabetic glomerular class scored. The primary composite end point was ESKD or death. Multivariable Cox regression models tested whether any or the severity of diabetes histopathology affected the primary end point. Results: Data from 134 cases were available for analysis (78 DGS+GD and 56 GD alone). Diabetes duration and glycemic control were similar between the two groups (P = 0.2; P = 0.09, respectively). Use of immunosuppression did not differ between the two groups (P = 0.3). The composite end point was significantly higher in DGS+GD (22.5 cases per 100 person-years [95% confidence interval (CI), 16.6 to 30.5]) versus GD alone (10.2 cases per 100 person-years [95% CI, 6.4 to 16.2]). Regression analyses demonstrated that compared with the GD-alone group, the risk for the composite end point was similar in the group with mild DGS+GD (DGS class 1, 2a) (hazard ratio, 1.15 [95% CI, 0.54 to 2.43]) while the group with severe DGS+GD (DGS class 2b, 3, 4) had a greater risk (hazard ratio, 3.60 [1.79 to 7.22]). Conclusions: Among people with diabetes and GD, mild diabetic glomerular lesions on biopsy do not affect outcomes, but moderate-severe lesions increase the risk for ESKD and death. Whether use of immunosuppression, particularly glucocorticoids, is less successful in inducing GD remission in people with moderate-severe diabetic lesions will be a focus of future study in a larger population.

Abstract:A 47-year-old man presented with gross hematuria,proteinuria and progressive renal dysfunction for more than 1 month.His serum anti-GBM antibody was positive.Renal biopsy showed glomerular crescent formation with linear distribution of IgG along GBM.In addition,there were glomerular nodular lesions like Kimmelstiel-Wilson nodules,diffuse thickness of tubular basement membrane and arterial hyalinosis.Ulrastructral examination indicated uniform thickness of GBM.Abnormal OGTT suggested diabetes mellitus.The final diagnosis of anti-GBM glomerulonephritis associated with diabetic nephropathy was made.

Abstract:Diabetic nephropathy (DN) develops in a subset of diabetic patients, on average about 15 years after onset of metabolic abnormalities. The earliest lesions consist of thickened glomerular basement membranes (GBM), mild mesangial expansion, and arteriolar accumulation of hyaline. Mesangiolysis and exuberant mesangial repair then develop, ultimately resulting in marked increase in mesangial matrix. Established nephropathy is characterized by mesangial expansion which may be nodular, so-called Kimmelstiel-Wilson nodules, hyaline in both afferent and efferent arterioles, and markedly thickened GBM by electron microscopy. Podocyte loss may be a crucial contributor to this progressive sclerosis. There is a distinct predilection of segmental sclerosis to occur at the glomerulotubular junction in type 1 diabetic patients, which can lead to disruption of outflow from the glomerulus, resulting in so-called a tubular glomeruli. A recent classification of DN may be useful for categorizing stages of development of the diabetic lesions. Diabetic injury also affects the tubulointerstitium. Tubular basement membranes thicken in parallel to GBM. Early interstitial inflammation with predominantly mononuclear cells is followed by later increased interstitial fibrosis and tubular atrophy. Lesions in type 1 and type 2 diabetic patients share many similarities, with more severe vascular disease and heterogeneous lesions, perhaps reflecting older age and comorbid conditions such as hypertension, in the latter. Diabetic patients typically undergo diagnostic renal biopsies only when the clinical course is not typical for DN, and not surprisingly, a range of other lesions may then be present in addition to DN. DN also recurs or occurs de novo in the transplant, developing more quickly than in the native kidney. Efforts to develop rodent animal models that more completely capture these key features of human DN will allow advances to be made in understanding pathogenesis and targeting novel treatment.

Short-Term Outcomes of Infection-Related Glomerulonephritis in Diabetes Mellitus

Abstract:Background and objective Infection-related glomerulonephritis (IRGN) in adults, particularly the diabetic population, has a grave prognosis with many patients progressing to dialysis-dependent renal failure. Indian data on this entity are very scarce. This study attempts to correlate the clinicopathological factors related to diabetic IRGN and its short-term outcomes. Subjects and methods A retrospective analysis of all diabetic patients with biopsy-proven IRGN between January 2017 and August 2021 was conducted. Factors affecting outcomes such as clinical characteristics, urine examination, complete blood count, serum biochemistry, renal biopsy, and follow-up data were obtained and analyzed to determine the risk of progression to chronic kidney disease (CKD)/end-stage renal disease (ESRD). Univariate/multivariate analysis and receiver operating characteristic (ROC) curve were performed to identify independent risk factors affecting outcomes. Results A total of 40 diabetic patients with IRGN was included in the study, with a mean age of 53.08 ± 10 years, comprising predominantly males (60%). Infective foci were occult in majority (37.5%). Isolated low C3 levels were documented in the majority, while three patients (7.5%) had normal complement levels. Complete renal recovery was noted in 15 patients (37.5%), while 12 patients (30%) progressed to ESRD. Anuria or uremia at presentation, glomerulosclerosis >28.6%, interstitial fibrosis with tubular atrophy (IFTA) >17.5%, and diabetic nephropathy correlated to poor renal recovery. No correlation was observed between endocapillary proliferation, the pattern of deposits, the prevalence of crescents, and complement levels with the outcome. Conclusion IRGN is a common immune-mediated clinical entity among diabetics and often requires renal replacement therapy. Anuria or uremia at presentation, diabetic nephropathy, elevated glomerulosclerosis, and IFTA were associated with poor renal recovery. Complement levels and crescents had no impact on the outcome.
Clinicopathological Characteristics and Disease Chronicity in Glomerular Diseases: A Decade-Long Study at Romania's Largest Kidney Biopsy Reference Center

Abstract:Glomerular diseases (GDs), significant causes of end-stage kidney disease, are better understood through epidemiological studies based on kidney biopsies (KBs), which provide important insights into their prevalence and characteristics. This study aims to analyze the clinicopathological features of GDs diagnosed from 2008 to 2017 at Romania's largest reference center. In this decade-long study, 1254 adult patients diagnosed with GDs were included. The local previously validated renal histopathological prognostic score was calculated for each KB using four histopathologic lesions: global glomerulosclerosis, tubular atrophy, interstitial fibrosis and fibrocellular/fibrous crescents. The mean patient age was 50 years, with a male predominance (57%). The primary referral reasons were nephrotic syndrome (46%), nephritic syndrome (37%), chronic kidney disease (12%), asymptomatic urinary abnormalities (4%), and acute kidney injury (1%). Immunoglobulin A nephropathy (IgAN) was the most frequently diagnosed GD (20%), aligning with frequencies reported in European registries. Diabetic glomerular nephropathy was the most common secondary GD (10%). It also presented the highest median renal histopathological prognostic score (2), indicating a poorer prognosis. Lower eGFR and higher proteinuria were independently associated with higher scores. This decade-long study highlights IgAN as the most frequent GD diagnosed by KB. Diabetic glomerular nephropathy was identified as the most common secondary GD. The renal histopathological prognostic score, notably high in diabetic glomerular nephropathy patients, was correlated with lower eGFR and higher proteinuria, underlining its clinical relevance.

Abstract:We present the case of a 58-year-old male diabetic patient admitted to our department for a slight decrease in kidney function, with nephrotic range proteinuria, hematuria (16,000/ml) and positive anti-glomerular basement membrane antibodies. Kidney biopsy revealed diabetic nephropathy with no evidence of crescent formation or linear immunoglobulin deposits along the basement membrane. We discuss the various clinical settings involving positive anti-glomerular basement membrane in the absence of crescentic glomerulonephritis.

Abstract:Glomerulonephritis (GN) encompasses a heterogeneous group of disease processes. It accounts for approximately 20% of chronic kidney disease and is the second most common cause of kidney failure worldwide. A study of a cohort of Medicare patients found that approximately 1.2% were affected. GN should be suspected in patients with unexplained hematuria, particularly with persistent hematuria with red blood cell casts and/or acanthocytes, and proteinuria. Other presenting features include purpura (in children) and hypertension. When GN is suspected based on test results, patients should be referred to a nephrologist for further evaluation and consideration of kidney biopsy, which is the gold standard diagnostic test. GN is categorized as acute (sudden onset of hematuria and proteinuria) or chronic (with irreversible scarring on biopsy). Acute GN is more likely to be reversible. Initial management consists of supportive and protective measures, including blood pressure control, drugs to block the renin-angiotensin system, and lifestyle modifications to minimize cardiovascular risk. The underlying cause should be treated when possible. Subsequent management depends on the specific type of GN and might include antimicrobial therapy and/or immunosuppressive therapy when appropriate.

Abstract:Diabetes mellitus (DM) and chronic kidney disease (CKD) are common in middle aged and older adult individuals. DM may accelerate the aging process, and the age-related declines in the estimated glomerular filtration rate (eGFR) can pose a challenge to diagnosing diabetic kidney disease (DKD) using standard diagnostic criteria especially with the absence of severe albuminuria among older adults. In the presence of CKD and DM, older adult patients may need multidisciplinary care due to susceptibility to various health issues, e.g., cognitive decline, auditory or visual impairment, various comorbidities, complex medical regimens, and increased sensitivity to medication adverse effects. As a result, it can be challenging to apply recent therapeutic advancements for the general population to older adults. We review the evidence that the benefits from these newer therapies apply equally to older and younger patients with CKD and diabetes type 2 and propose a comprehensive management. This framework will address nonpharmacological measures and pharmacological management with renin angiotensin system inhibitors (RASi), sodium glucose co-transporter 2 inhibitors (SGLT2i), non-steroidal mineralocorticoids receptor antagonists (MRAs), and glucagon like peptide 1 receptor agonists (GLP1-RAs).

Abstract:Background: Diabetic kidney disease (DKD) is characterized by reduced estimated glomerular filtration rate (eGFR) and albuminuria, which play a pivotal role in both diagnosing and determining the disease’s progression. This study aimed to assess the trajectory of these markers concerning age in individuals with DKD and identify predictive factors for the decline in eGFR decline, variation in albuminuria, mortality, and progression to renal replacement therapy (RRT). Design: This retrospective cohort encompassed patients with type 2 diabetes (T2D), divided into two age categories: <75 and ⩾75 years old. Methods: Over a 3-year span, the study evaluated eGFR (CKD-EPI) and 24-h albuminuria. Univariate and multivariate analyses were employed to pinpoint factors associated with deteriorating renal function and mortality. Significance was set at p < 0.05, and Kaplan–Meier survival curves were constructed to illustrate renal and overall survival. Results: The analysis comprised 304 patients. Comparable eGFR declines were evident in both age groups during the transition from the first to the second year and from the second to the third year. Nonetheless, a more pronounced rise in albuminuria was evident in the ⩾75 years group during the first to the second year. Multivariate analysis unveiled that systolic blood pressure (SBP) measurements in the first year positively forecasted eGFR decline. Age was associated with heightened albuminuria and mortality, while hospitalizations linked to cardiovascular causes robustly predicted mortality. Hospitalizations due to sepsis and cardiovascular reasons, coupled with first-year SBP measurements, served as predictive indicators for progression to RRT. Conclusion: Both age groups experienced similar declines in eGFR, though the ⩾75 years group displayed a more significant increase in albuminuria during the first to the second year. Age, hospitalizations, and higher blood pressure levels were correlated with exacerbated renal function deterioration and/or elevated mortality in DKD. Timely intervention and tailored management strategies stand as critical components for enhancing outcomes among DKD patients.

Abstract:Abstract Diabetic nephropathy (DN) is the leading cause of kidney failure worldwide. High levels of advanced glycation end products (AGEs) derived from chronic hyperglycemia occur in diabetes. As AGEs are highly antigenic, it promotes us to hypothesize that AGEs may exert an autoimmune response in DN development. First, we demonstrated that podocytes have the ability to present antigens under inflammatory condition. Second, we confirmed the presence of AGEs-recognizing autoantibodies in the serum of DN patients, not nondiabetic individuals or diabetic patients without nephropathy. Third, we showed that the autoantobodies in sera of DN patients specifically identify kidney biopsies of DN patients, not nondiabetic individuals or diabetic patients without nephropathy. In particular, the autoantobodies recognize signal specificity in podocytes from DN patients. Finally, employing the kidney tissue sections from DN patients as antigen to screen serum samples from diabetic or DN patients, we found that significant DN patients (48%) but not diabetic patients without nephropathy (<5%) exhibited podocyte labeling with autoantibodies. These results suggest that podocyte-targeting autoimmunity may plays an important role in promoting immune-mediated DN progression.
Alterations of gut microbiota in biopsy-proven diabetic nephropathy and a long history of diabetes without kidney damage

Abstract:The gut microbiota is closely related to parenteral noncommunicable diseases through intestinal immunity and plays an important role in the occurrence of diabetes and diabetic nephropathy. The aim of the study was to understand the gut–kidney axis by an analysis of gut microbiota composition among patients with biopsy-proven diabetic nephropathy (DN), patients with type 2 diabetes for more than 10 years without kidney damage (DM), and healthy controls (NC). Thirty-five DN patients, 40 DM patients and 40 healthy subjects matched by age and sex were enrolled between January 2022 and December 2022. Baseline information and clinical parameters were collected. 16S rDNA sequencing was performed to characterize the gut microbiome and identify gut microbes that were differentially abundant between patients and healthy controls. The relationship between the relative abundance of specific bacterial taxa in the gut and clinical phenotype and pathological indicators was evaluated. Substantial differences were found in the richness of the gut microbiota and the variation in the bacterial population among DN patients, DM patients and healthy controls. DM patients could be accurately distinguished from age- and sex-matched healthy controls by variations in g_Clostridium-XVIII (AUC = 0.929), and DN patients could be accurately distinguished from age- and sex-matched healthy controls by variations in g_Gemmiger (AUC = 0.842). DN patients could be accurately distinguished from age- and sex-matched DM patients by variations in g_Flavonifractor or g_Eisenbergiella (AUC = 0.909 and 0.886, respectively). The gut microbiota was also closely related to clinical phenotypes and pathological indicators. The study of gut microbiota composition was explored to determine its relationship to the occurrence of DN and a long history of diabetes without kidney damage. The renal pathological progression of DN may be delayed by regulating changes in the gut microbiota.

Abstract:It is widely known that diabetes mellitus negatively impacts both the innate immunity (the inflammatory response) and the acquired immunity (the humoral and cellular immune responses). Many patients with diabetes go on to develop chronic kidney disease, which will necessitate hemodialysis. In turn, long-term chronic hemodialysis generates an additional chronic inflammatory response and impairs acquired immunity. The purpose of this paper is to outline and compare the mechanisms that are the basis of the constant aggression towards self-components that affects patients with diabetes on hemodialysis, in order to find possible new therapeutic ways to improve the functionality of the immune system. Our study will take a detailed look at the mechanisms of endothelial alteration in diabetes and hemodialysis, at the mechanisms of inflammatory generation and signaling at different levels and also at the mechanisms of inflammation-induced insulin resistance. Severe emphysematous pyelonephritis as a complication of klebsiella pneumonia and uncontrolled diabetes mellitus

Abstract:A female in her 40s, with poorly controlled Type I diabetes mellitus, was brought to our tertiary hospital by ambulance after being found drowsy. Six days prior, she had self-discharged from the Intensive Care Unit whilst being treated for a Klebsiella pneumonia. At re-admission, she had an acute kidney injury with abdominal pain and clinical features of sepsis. Her presentation was attributed to ongoing Klebsiella pneumoniae infection; however, a chest radiograph showed marked improvement of pulmonary consolidations and an unusual subdiaphragmatic gas pattern. A CT scan demonstrated severe bilateral emphysematous pyelonephritis. The patient was unfit for bilateral nephrectomy and was medically managed in the Intensive Care Unit for 41 days, before transfer to a specialist renal unit for life-long haemodialysis. This case highlights the importance of considering emphysematous pyelonephritis in patients presenting with uncontrolled diabetes mellitus and acute kidney injury and/or infection, the role of imaging in its diagnosis, and the challenges of complex social circumstances in health management.

Abstract:Background: The incidence of diabetes mellitus (DM) in China is increasing yearly and has become a major problem plaguing national public health. The diagnosis of diabetic kidney disease (DKD) is based primarily on clinical criteria, and most patients do not receive a formal evaluation by renal biopsy; thus, misdiagnosis and underdiagnosis are common. The incidence of non-diabetic kidney disease (NDKD) is also higher in those with DM. To date, many cases of IgA nephropathy (IgAN) among those with DKD have been reported, while cases of IgAN in patients with long-duration DM who did not develop DKD are less commonly reported. Case description: A 70-year-old male patient with a diabetes duration of 26 years had proteinuria for one year. The clinical manifestations of nephrotic syndrome and IgAN were confirmed by renal biopsy. The patient received targeted treatment for three years with partial alleviation of proteinuria. Conclusion: Renal biopsy might aid in the definitive diagnosis of DKD, NDKD, and NDKD combined with DKD. Precise therapy based on renal pathology might help to improve outcomes in the kidney.

Abstract:Diabetic nephropathy (DN) is one of the most feared diabetic chronic microvascular complications and the major cause of end-stage renal disease (ESRD). The classical presentation of DN is characterized by hyperfiltration and albuminuria in the early phases which is then followed by a progressive renal function decline. The presentation of diabetic kidney disease (DKD) can vary especially in patients with T2DM where concomitant presence of other glomerular/tubular pathologies and severe peripheral vascular disease can become important confounders. All-cause mortality in individuals with DKD is approximately 30 times higher than that in diabetic patients without nephropathy and a great majority of patients with DKD will die from cardiovascular disease before they reach ESRD. The management of metabolic and hemodynamic perturbations for the prevention and for the delay of progression of DKD is very important. DKD is a global challenge and a significant social and economic burden; research should aim at developing new ideas to tackle this devastating condition.
