Abstract:Background. In this study, we introduced a novel surgical strategy to protect vagal nerve branches during radical thoracoscopic surgery in right lung cancer and explored the effects of vagal nerve branch preservation. Methods. We retrospectively studied 53 patients with right-sided lung cancer with clinically staged T1N0M0 between 2019 and 2020. All 53 patients were treated with total thoracoscopic lobectomy and mediastinal lymph node dissection in the same number of lymph node stations. Of these, 22 patients adopted a vagus nerve branch protection strategy during lymph node dissection. Another 31 patients were treated with traditional lymph node dissection as the control group. Results. The characteristics of the patients were similar between the two groups. The operation time and intraoperative bleeding in the protection group were longer than those in the control group. However, the protection group had a lower average postoperative pain score and average postoperative hospital stay. The above difference was not statistically significant. Three cases of arrhythmia occurred in the protection group, including 1 case of tachycardia and 2 cases of atrial fibrillation. In the control group, 13 cases of arrhythmia occurred after the operation, including 8 cases of tachycardia and 5 cases of atrial fibrillation. We also tracked changes in the patients’ heart rates throughout the treatment process (excluding patients with arrhythmias). An increased heart rate was observed postoperatively in both groups, but the increase of heart rate of the protection group was smaller than that of the control group; however, the difference was not statistically significant. Conclusions. A vagus nerve branch preservation-based approach to radical surgery is a safe and feasible strategy for right lung cancer treatment, which could significantly reduce the risk of postoperative arrhythmia in patients and may also have a potential role in reducing the length of hospital stay and maintaining heart rate stability in the postoperative period.

Abstract:Three‐dimensional illustration of the multi‐intersegmental vein V1+2d and their clinical significance. Background: To investigate the distribution pattern of V1+2d in the left superior pulmonary vein and its clinical significance. Methods: A retrospective analysis was conducted using three‐dimensional computed tomographic bronchography and angiography (3D‐CTBA) data from 500 lung cancer patients. Statistical analyses were performed to evaluate the incidence and drainage patterns of the three sub‐branches of V1+2d, namely V1+2d1, V1+2d2 and V1+2d3. Furthermore, clinical data from 10 patients' lesions involving V1+2d were reviewed to illustrate the impact of adjacency to V1+2d on the surgical approach. Results: The incidences of V1+2d1, V1+2d2 and V1+2d3 were 100%, 76.4% and 100% respectively. The relative interlobar distribution sizes of B3a and B1+2c and the left upper division (LUD) vein type influenced the incidence of V1+2d2 (p B3a patterns. V1+2d2 was entirely absent in the B3a > B1+2c pattern. V1+2d2 exhibited a higher incidence in both the central vein (CV) type and the noncentral vein (NCV) type when compared to the semi‐central vein (SCV) type (100% vs. 100% vs. 64.8%). The most prevalent venous drainage pattern was the three sub‐branches of V1+2d constituting a major trunk to drain (41.2%). All 10 cases with lesions involving V1+2d successfully underwent sublobar resection with no complications, and the surgical margin was ≥2 cm. Conclusion: The three sub‐branches of V1+2d exhibit a high incidence with diverse distribution patterns, yet a discernible pattern exists. For inter‐ or multi‐intersegmental nodules involving V1+2d, combined segmentectomy and subsegmentectomy or combined subsegmentectomy can ensure the safe margin.

Abstract:BackgroundThe role of video-assisted thoracoscopic surgery (VATS) in mediastinal lymph node dissection (MLND) for non-small cell lung cancer (NSCLC) following neoadjuvant therapy remains controversial. The aim of this study was to demonstrate the sufficiency of VATS by evaluating perioperative and long-term outcomes. MethodsPatients with locally advanced NSCLC and treated with radical surgery after neoadjuvant therapy were identified in our database. The thoroughness of MLND was compared by approach. Multivariable logistic regression analysis was used to evaluate predictors of sufficient MLND. Propensity score matching was performed. Kaplan-Meier and Cox proportional hazard analyses were used to assess long-term survival. ResultsOf the 127 enrolled patients, 56 underwent attempted VATS and 71 underwent thoracotomy. Multivariable logistic regression analysis revealed that approach was not a predictor of sufficient MLND (odds ratio 0.81, 95% confidence interval [CI] 0.364-1.803; P =0.606). After matching, 28 pairs of patients were selected from the two groups. There was no significant difference between the numbers of dissected lymph nodes (15 vs. 20; P =0.191) and nodal stations (7 vs. 7; P =0.315). Recurrence-free (log-rank P =0.613) and overall survival (log-rank P =0.379) was similar in both groups. Multivariable Cox proportional hazards model analysis indicated that VATS was not an independent predictor of recurrence-free (hazard ratio 0.955, 95% CI 0.415-2.198; P =0.913) or overall survival (hazard ratio 0.841, 95% CI 0.338-2.093; P =0.709). ConclusionCompared to thoracotomy, VATS is a sufficient approach for MLND to treat locally advanced NSCLC following neoadjuvant therapy without compromising long-term survival.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.
A comparative study for three methods of thoracoscopic resection of lung cancer

Abstract:Objective This comparative study was carried out to define the advantages and disadvantages of the three methods of video-assisted lobectomy and systematic dissection of the mediastinal nodes,close distance two ports VATS,normal two ports VATS and three ports VATS.Methods Between Sep.2012 and Sep.2014 in Peking University First Hospital and Beijing Hospital,319 patients who were diagnosed primary lung cancer and were going to be performed VATS lobectomy and systematic lymph node dissection,were selected,265 patients were included in this study finally.113 cases were involved in the group of close distance two ports VATS,79 cases in normal two ports VATS and 73 cases in three ports VATS.The clinical data were gathered and statistically analyzed.Results No severe postoperative complications or death occurred in the three groups.No significant differences existed among the three groups in intraoperative blood loss,duration and volume of chest tube drainage,hospital stay after surgery,hospitalization expenses and complications (P ＞ 0.05).The data about operating time,number of dissected lymph nodes,postoperative pain score was significantly different [(241.5 ± 56.8) min vs (112.5 ±0.8)min vs (119.1 ±12.6) min、(14.5 ±2.9) gold vs (22.4 ±6.6)gold vs (21.3 ±3.7)gold、(3.2 ±6.0) branch vs (7.8 ±7.1) branch vs (10.3 ±6.5) branch,P＜0.05].Conclusions The method of close distance two ports video-assistant lobectomy and systematic dissection of the mediastinal nodes is safe and practicable with definite therapeutic effect.Compared with the other two methods,there is obviously advantage in postoperative pain.
Clinical Analysis of Single Utility Port Video-Assisted Thoracic Surgery in Surgical Treatment of Non-Small Cell Lung Cancer

Abstract:Objective To explore the feasibility and clinical application value of single utility port video-assisted thoracic surgery(VATS) pulmonary lobectomy and systematic mediastinal lymphadenectomy in surgical treatment of non-small cell lung cancer(NSCLC). Methods We reviewed the clinical data of 42patients with NSCLC received single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy from March 2011 to March 2013. The utility port for observation was about 1.5 cm long and located at the 7th and 8th rib in the middle axillary line. The port for operation was about 4.0-5.0 cm long and located at the 3rd and 4th rib in the outer edge of pectoralis major. Results All of the 42 patients were successfully subjected to single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy, none of them needed converse to transit-assisted incision or thoracotomy. Operative duration was 90-200 min; intraoperrative blood loss was 50-400 ml;lymph node dissection number was 9-16. After the surgery, the patients received chest drainage for 5-8 days. They were discharged from hospital in 6-10 days after the surgery without severe complications. Conclusions Based on the conventional three utility ports VATS, the single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy caused less injury in surgical treatment of non-small cell lung cancer(NSCLC). It has certain clinical advantages and was a minimally invasive surgical treatment for selected patients with NSCLC.
Pulmonary Function Changes after Sublobar Resection in Patients with Peripheral Non-Subpleural Nodules

Abstract:Background In the treatment of peripheral early-staged lung cancer and benign lesions, segmentectomy and wedge resection are both reliable treatment methods. It is debatable that how much pulmonary function will be lost after different sublobar resection in the treatment of early-staged deep-located peripheral NSCLC (non-small cell lung cancer). The purpose of this study was to explore postoperative pulmonary function changes of sublobar resection in enrolled patients with non-subpleural peripheral nodules. Methods We collected clinical data of patients undergoing VATS (video-assisted thoracoscopic surgery) segmentectomy or wedge resection for single nodule. These nodules were confirmed as peripheral non-subpleural nodules by preoperative 3D imaging. Patients were divided into two groups according to the operation procedure. Demographic characteristics, pulmonary function, postoperative outcomes, and others were collected. All data was gathered at the First Affiliated Hospital of Soochow University. Outcomes after wedge resection were compared with those after segmentectomy resection. Results A total of 88 patients were included in this study, including 46 patients with VATS wedge resection and 42 patients with VATS segmentectomy. No difference was detected when comparing FEV1 (forced expiratory volume in 1 s) loss between these two groups (17.6 +/- 2.1%, wedge resection vs. 19.4 +/- 5.4%, segmentectomy, P = 0.176). FVC (forced vital capacity) loss (8.7 +/- 2.3%, wedge resection vs. 17.1 +/- 2.2%, segmentectomy, P < 0.001) and MVV (maximum ventilatory volume) loss (11.5 +/- 3.1%, wedge resection vs. 20.6 +/- 7.8%, segmentectomy, P < 0.001) in segmentectomy group was significantly higher than those in wedge resection group. Discrepancies were investigated when comparing duration of surgery (70 +/- 22 min, wedge resection vs. 111 +/- 52 min, segmentectomy, P = 0.0002), postoperative drainage (85 +/- 45 mL, wedge resection vs. 287 +/- 672 mL, segmentectomy, P = 0.0123), and treatment hospitalization expenses [35148 +/- 889CNY, wedge resection vs. 52,502 (38,276-57,772) CNY, segmentectomy, P < 0.0002]. No significant difference was found between air leak time (1.7 +/- 0.7 days, wedge resection vs. 2.5 +/- 1.7 days, segmentectomy, P = 0.062) and hospitalization time (2.7 +/- 0.7 days, wedge resection vs. 3.5 +/- 1.7 days, segmentectomy, P = 0.051). Conclusions For patients with peripheral non-subpleural nodules, we observed that patients who underwent wedge resection had less lung function loss than those who underwent segmentectomy when their lung function was reviewed at the 6th month after surgery. Patients undergoing wedge resection had partial advantages over patients with segmental resection in terms of hospitalization cost, operation time and postoperative drainage, etc. Wedge resection, as a treatment for peripheral non-subpleural pulmonary nodules, seemed to have more advantages in preserving patients' pulmonary function.
Retrospective Study on Video-Assisted Vs. Open Mediastinal Lymphadenectomy for Non-Small Cell Lung Cancer: a Propensity-Matched Analysis

Abstract:Background: This retrospective study is designed to compare the surgical outcomes of mediastinal lymph node (MLN) dissection between the two groups of non-small cell lung cancer (NSCLC) patients underwent lung cancer surgery through video-assisted thoracoscopic surgery (VATS) or thoracotomy, as well as classify the differences and identify the safer method between two groups. Methods: A total of 497 patients underwent pulmonary lobectomy and systemic MLN dissection were enrolled in this study. They were divided into VATS group (n=242) and traditional thoracotomy (TT) group (n=255). The VATS group received three-port VATS, while the thoracotomy group underwent posterior-lateral or muscle sparing thoracotomy. The range of MLN dissection included groups 2R, 4R, 7, 8, 9 in right-sided lung cancer surgery and groups 4L, 5, 6, 7, 8, 9 in left-sided. The two groups were matched at a ratio of 1: 1 using the propensity score matching (PSM) method. Results: Out of 497 patients, 376 cases were matched for PSM, with 188 cases in each group. Baseline date did not get any significant difference between two groups. The average group number of MLN dissection in VATS group and TT group was 4.14 and 3.97, respectively. While the average amount of dissected lymph nodes in both groups were 16.63 and 17.32, respectively. Compare with the lymph node numbers among all the groups in TT group, only the number of group 7 though left approach (7L) in VATS group was statically significant. Besides, the operation time and postoperative length of hospital stay in VATS group were superior to those in TT group (P<0.05). No significant difference had been found in intraoperative blood loss, accessory surgical injury and postoperative complication between two groups. Conclusion: Compared with thoracotomy, systemic MLN dissection and pulmonary lobectomy via VATS has quicker recovery, less postoperative complication and length of hospital stay with similar surgical outcomes. Moreover, our findings also suggest thoracotomy has an advantage on the dissection of MLN in group 7L.

Abstract:Background: Thoracoscopic surgery is a primary treatment for lung cancer, with lobectomy and mediastinal lymph node dissection being the predominant surgical approaches for invasive lung cancer. While many thoracic surgeons can proficiently perform lobectomy, thorough and standardized lymph node dissection remains challenging. This study aimed to explore a safer and more efficient surgical method for mediastinal lymph node dissection in lung cancer. Methods: A prospective randomized controlled study was conducted, involving 100 patients with right lung cancer who were admitted to our hospital from January 2021 to April 2024 and met the inclusion criteria. These patients were randomly divided into an observation group (tissue pneumoperitoneum technique around lymph nodes group) and a control group (conventional surgery group). Thoracoscopic lobectomy and mediastinal lymph node dissection were performed. Intraoperative and postoperative related indicators were observed to validate the effectiveness and safety of the tissue pneumoperitoneum technique around lymph nodes. Results: The observation group showed a significantly shorter lymph node dissection surgery time compared to the control group, with a statistically significant difference (p < 0.05). The number of lymph nodes dissected in the observation group was significantly higher than that in the control group, with a statistically significant difference (p 0.05). The total drainage volume within three days postoperatively was comparable between the two groups, with no statistically significant difference (p > 0.05). The observation group had shorter chest tube indwelling time and postoperative hospital stay than the control group, with statistically significant differences (p < 0.05). The incidence of surgical complications was similar between the two groups, and there were no perioperative deaths. Conclusion: The tissue pneumoperitoneum technique around lymph nodes is a more efficient method for mediastinal lymph node dissection in lung cancer, demonstrating safety and feasibility, and is worthy of promotion.

Abstract:In recent years, the occurrence rate of non-small cell lung cancer (NSCLC) has grown rapidly. [ 1 ] At the same time, with the computerized tomographic scanning of low doses applied extensively, the screen-positive rate of early NSCLC has been accelerating observably. [ 2 ] Video-assisted thoracoscopic surgery (VATS), which results in less trauma, complications, and pain than traditional thoracotomy surgery for patients, is more conducive for patients with NSCLC. [ 3 ] The major structures need to be ligated in VATS lobectomy and standard lymphadenectomy (including systematic lymph node sampling and complete lymphadenectomy), especially in the branches of the pulmonary vein, pulmonary artery, and bronchus. Since the 1940s, for lobectomy and standard lymphadenectomy, the basic method has not varied especially in the operation of hilar. [ 4 ]

Transient Blocking of Both Pulmonary Artery and Veins for Surgical Treatment of Stage III Central Lung Cancer

Abstract:Objective To investigate the feasibility of transient blocking of both pulmonary artery and veins for surgical treat- ment of central lung cancer with stage Ⅲ to preserve the normal pulmonary.Methods Firstly,the relation of the pulmonary artery, the lung neoplasm and the enlarged mediastinal lymph nodes was investigated.If the hilum of lung remained frozen,the pericardium was opened and the pulmonary artery,the upper and lower lobe pulmonary veins were dissected.Then those three vessels were blocked.When the pulmonary cireulation was stopped,bloodless lobectomy and pulmonary artery angioplasty and/or anastomosis were performed.Then the blockers were released,and pulmonary circulation was restored.The time of blocking was(35±15)minutes (16～66 minutes).Results All 20 patients suffer from stage Ⅲ central lung cancer,which' s the hila of lung remained frozen,re- ceived complete resection of the tumor.The normal functioning pulmonary tissue in the 20 patients was preserved instead of pneumone- ctomy.The average amount of bleeding was 256 ml(180～420 ml)during operation.All 20 patients recovered well.Conclusion Transient blocking of both pttlmonary artery and veins for surgical treatment of stage Ⅲ central lung cancer is and innovation in surgical technique,which makes the operation safe and easy.This technique may provide a chance to patients,with poor cardio-pulmonary function.In addition,this technique widens the surgical indications for patients suffering from lung cancer.
A clinical study of lung and esophageal cancer radical surgery with minimally invasive incision

Abstract:Objective:To study the application of small thoracic incision for lung cancer and esophageal cancer radical surgery with comparabale to that of conventional incison.Methods:A total of 310 patients with lung cancer or esophageal cancer radical surgery during the period of from January 2006 to January 2007 were retrospective analyzed.Small incison group had 143 patients and conventional incision had 167 patients,the incision length,operation time,intra-operation bleeding and post-operation drainage volume,upper limb function recovery time,lymph node scavenge number,postoperative pain,postoperative complications of two groups were compared.Results:The operation time,lymph node scavenge number,postoperative complications of two groups have no difference obviously.But the incision length,intra-operation bleeding and post-operation drainage volume,upper limb function recovery time,postoperative pain of small incison groups were much better than that of conventional groups.Conclusion:Muscle-sparing thoracic small incision can achieve excellent clinical result and has such advantages as less invasiveness,reduced bleeding volume,pain and better postoperative appearance.This procedure meets the requirements of esophageal and lung cancer radical surgery,and also provides an alternative for lung cancer and esophageal cancer surgery.
Previously Blocking Both Pulmonary Artery and Veins for Surgical Treatment of Advanced Non-Small Cell Lung Cancer

Abstract:Objective To investigate the feasibility of previously blocking both pulmonary artery and veins for surgical treatment of advanced non-small cell lung cancer(NSCLC) to preserve the normal function of pulmonary tissue.Methods Firstly,the relation of the pulmonary artery,the lung neoplasm and the enlarged mediastinal lymph nodes was observed.If the hilum of lung remained freezing stably,the pericardium was opened and the pulmonary artery,the upper and lower lobe pulmonary veins were divided.Then those three vessels were blocked to prevent the blood flow from the pulmonary artery or refluence of the left atrium.When the pulmonary circulation was stopped,bloodless lobectomy and pulmonary artery angioplasty and/or anastomosis were performed.Then the blockers were removed,and pulmonary circulation was recovered.Twenty hybrid swine were divided randomly into two groups:10 hybrid swine were assigned to the group of the lung circulation blocked and another 10 hybrid swine were assigned to the group of the pulmonary artery blocked.The experimental method was blocking the unilateral lung circulation or the pulmonary artery for 60 minutes,and then reperfusing for 240 minutes,detected the lung tissues into the level of reperfusion injury by light microscope and electron microscope,and then breeded the swine for 7 days,and detected the lung tissue into the level of reperfusion injury by light microscope and electron microscope again.Results All 30 patients suffer from advanced NSCLC,whose the hila of lung remained freezing stably,received complete resection of the tumor.The normal function of pulmonary tissue in the 30 patients was preserved,and the pneumonectomy did not need to perform.The time of blocking pulmonary artery was(48±7.5)minutes(18～72 minutes),vessels was 33±6.8 minutes(16～66 minutes).The average amount of bleeding was 248±34 ml(172～412 ml) during operation.All 30 patients have recovered well.The results of experiments indicated that the group of the lung circulation blocked and the group of the pulmonary artery blocked both have reperfusion injury,and the reperfusion injury is reversible in 60 minutes.Conclusions Previously blocking both pulmonary artery and veins for surgical treatment of advanced NSCLC is an innovation in surgical technique,which makes the operation safely and easily.This technique provides a chance to some patients,whose cardio-pulmonary function is too poor to tolerate pneumonectomy,to receive surgical treatments.Also,this technique widens the surgical indications for patients suffering from lung cancer.
Safety and Feasibility of Preferential Manual Bronchoplasty in 2-3 Cm Single-Port Video-Assisted Thoracoscopic Lobectomy.

Abstract:BackgroundThis retrospective study aimed to compare preferential manual bronchoplasty (PMB) and mechanical stapler closure (MSC) of the bronchial stump after 2-3 cm single-port (SP) video-assisted thoracoscopic surgery (VATS) lobectomy in patients with pathological T1 (pT1) stage lung cancer.MethodsBetween January 2019 and March 2022, patients with pulmonary neoplasms who underwent 2-3 cm SP VATS lobectomy were retrospectively screened. After propensity-matched analysis, we compared perioperative outcomes and analyzed the safety and feasibility of PMB and MSC of the bronchial stump while performing VATS lobectomy.ResultsIn this study, 280 and 832 patients were enrolled in the PMB and MSC groups, respectively. Propensity score matching produced 280 pairs. The operation time was shorter in the PMB group, whereas the average number of lymph nodes dissected was higher in the PMB group. The conversion rate was significantly lower in the PMB group. The following were similar between the PMB and MSC groups, respectively: average blood loss volume, postoperative hospital stay, and chest tube removal time. Postoperatively, the incidence of atelectasis was significantly higher in the MSC group. As per subgroup analyses, PMB was associated with a shorter operation time in left and right upper lobectomies. Particularly in left upper lobectomy, PMB had more lymph node dissections and less conversion to open and postoperative atelectasis.ConclusionsIn comparison with MSC of the bronchial stump, PMB showed better safety and feasibility in 2-3 cm SP VATS left and right superior lobectomies in patients with pT1 stage lung cancer.

Abstract:Background. It has been technically challenging to perform simultaneous triple plasty on the superior vena cava (SVC), pulmonary artery (PA), and main bronchus for central-type lung cancers. In the present study, the authors describe a corresponding technique and clinical outcomes of this surgical manipulation. Methods. Clinical data from 4 patients with non-small cell lung cancer (NSCLC) who underwent triple plastic resections and reconstructions were retrospectively reviewed. Three patients received neoadjuvant chemotherapy for pathologically proven locally advanced disease. For pulmonary arteries, sleeve resection with end-to-end anastomosis was performed in 2 patients; tangential resection was used in the other 2 patients. SVC resection with ringed polytetrafluoroethylene (PTFE) graft interposition was performed in 1 patient; the other 3 patients underwent tangential SVC resection. Sleeve resection of the bronchus was performed in all 4 patients. Systemic lymphadenectomy was accomplished in all patients. Results. There was histologic confirmation of large cell carcinoma, adenocarcinoma, squamous cancer, and adeno-squamous cancer, respectively, in these 4 patients. Stage pT4N2M0-IIIB was confirmed in 2 patients, and stage T4N1M0-IIIA and stage T2aN2M0-IIIA were confirmed in the other 2 patients. There were no perioperative deaths. Postoperative atrial fibrillation and prolonged air leakage occurred in 2 patients, respectively. Four patients underwent postoperative chemotherapy and 2 patients were administered radiotherapy. Patients were followed for 21 to 38 months: Two patients had disease-free survival at their 32-month and 38-month follow-ups, and the other 2 patients died 21 and 22 months, respectively, after operation because of remote metastasis. Conclusions. Triple plasty of bronchus, PA, and SVC is both practical and safe for patients with locally advanced NSCLC. For patients with strict indications, the long-term survival is favorable. Abstract:Lung cancer, a leading cause of cancer-related death, often requires surgical resection for early-stage cases, with recent data supporting less invasive resections for tumors smaller than 2 cm. Central to resection is lymph node assessment, an area of controversy worldwide, compounded by advances in minimally invasive techniques. The review aims to assess current standards for lymph node assessment, recent data from the surgical era, and the immunobiological basis of how lymph node metastases impact patient outcomes. The British Thoracic Society guidelines recommend systematic nodal dissection during lung cancer resection, without specifying node removal or sampling. Historical data on mediastinal lymph node dissection (MLND) survival benefits are inconclusive, although proponents argue for lower recurrence rates. Recent trials such as ACOSOG Z0030 found no survival difference between MLND and nodal sampling, reinforcing the need for robust staging. While lobe-specific dissection strategies have been proposed, they currently lack consensus. JCOG1413 aims to compare the clinical benefits of lobe-specific and systematic dissection. TNM-9 staging revisions emphasize the prognostic significance of single-station N2 involvement. Robotic surgery shows promise, with trials such as RAVAL, which reported comparable outcomes to video-assisted thoracic surgery (VATS) and improved lymph node sampling. Immunobiological insights suggest preserving key immunological sites during lymphadenectomy, especially for patients receiving adjuvant immunotherapy. In conclusion, the standard lymph node resection strategy remains unsettled. The debate between systematic and selective dissection continues, with implications for staging accuracy and patient outcomes. As minimally invasive techniques evolve, robotic surgery emerges as an effective and low-risk approach to delivering optimal lymph node assessment.

Abstract:Objective To summarize the clinical experience of the operation mode of single hole thoracoscopic lung resection,lobectomy and lymph node dissection of experience in operation,summarize and analyze the clinical effect.Methods 495 cases of patients with lung cancer,including 320 cases of adenocarcinoma,147 cases of squamous cell carcinoma,24 cases of adenosquamous carcinoma,other 4 cases,adopt Uniportal VATS anatomic lobectomy concurrent lymph node dissection,record the operation time,bleeding volume,lymph nodes and lymph node dissection group number,length of the incision the occurrence of complications,extubation time,hospitalization time and postoperative.Observe the clinical results.Results 495 cases of lung cancer patients were successfully completed surgery,including 10 cases with lymph node interference or vascular variation caused by hemorrhage in open chest surgery.495 cases of patients with operation time was (141.8 ± 30.4) min,the amount of bleeding (80.2 ± 20.8) ml,lymph nodes (12.6 ± 2.8),lymph node (5.2 ± 1.6) group,were found in patients with lymph node metastasis in 84 cases (17.0％),of which the lymph node positive lymph node number (2.4 ± 1.4),incision length (3.2 ± 1.2) cm,extubation time (3.8 ± 0.6) d,hospitalization time (5.2 ± 2.2) d.Postoperative patients with good prognosis,no pulmonary embolism,pulmonary infection,hemorrhage,atelectasis and other serious complications,the other on the 30 day after operation,no patient died.Conclusions It is a safe and effective method to eliminate the lung cancer by single operation,and the visual effect is more beautiful than other methods,and it can reduce the pain of patients after operation.In addition,lobectomy and lymph node dissection is generally considered in clinical complex operation,but also has its corresponding processing method.
