Abstract:Objective To study the clinical features of post-renal transplantation diabetes mellitus(PTDM) and improve its prevention and treatment.Methods Twenty-eight patients with PTDM were retrospectively reviewed.The general condition,blood glucose,glycosylated hemoglobin(HbA1c),blood cyclosporine A trough level(CsA Co),serum lipid,islet function and islet antibodies were detected and the results were compared with those of 30 patients with type 2 diabetes mellitus(T2DM) and matched DM duration.Results ① The renal transplantation history,median of PTDM history and median interval from transplantation to onset of PTDM were(5.5±4.7) years,0.8 years(2 d-7 years) and 2.3 years( years),respectively.Ten cases had the diabetic symptoms while 14 cases had family history of DM.② Compared with T2DM patients with matched diabetic duration,patients of PTDM had higher incidence of hypertension(82.1% vs.53.3%,P=0.019).PTDM patients had earlier onset than T2DM patients( years vs. years,P=0.001).The total cholesterol([5.57±1.53] vs.[4.45±1.18] mmo/L,P=0.003),triglyceride([2.51±1.55] vs.[1.82±0.68] mmo/L,P=0.037),apolipoprotein E([67.72±18.24] vs.[50.68±11.61] mg/L,P=0.001) and high-density lipoprotein cholesterol([1.33±0.46] vs.[0.93±0.28] mmo/L,P=0.02) in PTDM patients were significantly higher than those in the T2MD patients.Postprandial C-peptide was lower([1 394.96±835.81] vs.[2 399.71±1 513.56] pmol/L,P=0.004).More patients of PTDM had DM family history(50% vs.20%,P=0.016) and received insulin therapy(67.86% vs.36.67%,P=0.018).There were no significant difference in body mass index(BMI),apolipoprotein AI,apolipoprotein B,lipoprotein a,low-density lipoprotein cholesterol and fasting C-peptide between the 2 groups.③ Positive islet antibodies were found in 2 PTDM patients.④ There was no correlation between fasting blood glucose and CsA Co,either was HbA1c(both P>0.05).Conclusion PTDM is a serious complication secondary to post-renal transplantation immuno-suppression.Compared with T2DM,patients of PTDM are often asymptomatic and have earlier onset with impaired islet function.PTDM patients have severe dyslipidemia.Blood glucose and serum lipid should be carefully monitored after kidney transplantation.Earlier PTDM diagnosis and appropriate treatment are beneficial to reverse glucose and lipid disorder.
