Abstract:17 The Infectious Diseases and Immunization Committee of the Canadian Paediatric Society (CPS) regularly reviews the recommendations of Health Canada’s National Advisory Committee on Immunization (NACI) for the routine immunization of infants, children and adolescents. It also reviews the provincial and territorial immunization protocols. Discrepancies between what the NACI recommends and what each province and territory makes available for children and youth has been highlighted previously by members of this committee and the CPS (1-5). This note provides an update for physicians and other health care professionals on changes related to NACI recommendations (Table 1) and to the provision of vaccines by each province and territory in Canada (Table 2) (6-12). The committee again makes a plea that all infants, children and adolescents in Canada have equal access to all NACI recommended vaccines for routine use, and that a harmonized national schedule be developed. Living in a ‘have not’ region should not dictate whether a child or youth has access to a ‘routine’ NACI recommended vaccine, nor should a move from one jurisdiction to another put a child or youth at increased risk for missing a vaccine because of regional variations in vaccine schedules. As of fall 2003, the four ‘newer’ NACI recommended vaccines (varicella [9], meningococcal conjugate [10], pneumococcal conjugate [11] and adolescent pertussis [12] vaccines) were added or will be added to the schedules in some provinces and territories, albeit in many instances only for high-risk children. Table 3 summarizes these ‘newer’ vaccine additions by province and territory. As can be seen, none of the provinces or territories has a program for all of the ‘newer’ vaccines. Alberta comes closest, but does not yet fund adolescent and adult pertussis vaccines. In one territory (Yukon) and one province (Manitoba), three of the four ‘newer’ vaccines (varicella, meningococcal conjugate and pneumococcal conjugate vaccines) have not been added for routine use, although consideration is being given in Manitoba to add these for those at high risk of disease or complications. Considering that the NACI recommended the varicella vaccine for routine use in infants and children in 1999 (9), it is disconcerting that by 2003 only five of the 13 provinces and territories had implemented a routine use program, particularly since the available refrigerator stable product simplifies storage and delivery issues (13). While the NACI recommendation for adolescent and adult pertussis vaccine is relatively new (September 2003 [12]), it is heartening to see that, as of November 2003, seven of the 13 provinces and territories have included this vaccine in the routine adolescent vaccination schedule. Abstract:Kinship caregivers (e.g., grandparents raising grandchildren) have been increasing over the last several decades. Approximately 3.5 million grandparents and other relatives are the primary caregivers for their related children, and African Americans are more likely to be kinship caregivers than persons from other groups. Kinship caregivers face unique challenges, such as parenting for uncertain periods of time and often with insufficient financial resources and support, placing them at significant risk of stress. Given the findings linking chronic stress to Alzheimer's disease (AD), there is a need for research to identify possible stressors and mitigate risks for outcomes such as AD among kin caregivers. Additionally, research indicates that African Americans (AAs) experience unusually high levels of stress due to factors often associated with structural racism, and they are disproportionately affected by cardiovascular disease (CVD), which is often a consequence of stress and another risk factor for AD. Regrettably, AA kin caregivers often incur a host of negative stress-related outcomes, including poor physical and mental health. Thus, there is an urgent need for research to identify modifiable risk factors for both stress and CVD to potentially mitigate the onset of AD in this population. The purpose of this paper is to provide a conceptual framework to examine the links between African Americans who commit to the unselfish act of providing kinship caregiving and their susceptibility to AD. Future research should investigate modifiable mechanisms to reduce the risks of AD in African American caregivers.

Abstract:Close attention should be given to the increased reliance on kinship care to provide out-of-home care for vulnerable children and youth because although these families have various strengths, they also frequently face financial instability and experience material hardship. Living in poverty and experiencing material hardship are linked to an array of negative outcomes, including physical and mental health problems, elevated parental stress, and children's academic difficulties and social and behavioral problems. This study examined African American families who are providing informal kinship care with the aim of developing a nuanced understanding of the financial characteristics, challenges, and coping strategies of these families. Data for this study were obtained from two sources: (1) an exploratory sequential mixed-method pilot study and (2) the National Financial Capability Study. It was found that most caregivers in the pilot reported their family resources were only seldom or sometimes adequate to cover their household's basic needs. Some caregivers reported being unaware of public benefits and community resources available for kinship care families, and they had difficulties navigating the system. Additionally, family members' love and support for each other helped them to cope with financial instability. Using a nationally representative dataset, predicted probabilities for three types of financial instability were higher among households with demographic characteristics of kinship care families including difficulty covering expenses, having income that exceeds expenses, and having emergency savings. Implications for practice, policy, and future research are discussed.
African American Family Caregivers' Experiences and Mental Health: A Convergent Mixed-Methods Study

Abstract:Background: Caregiving in the African American community is informed by strong cultural expectations but may be associated with negative experiences and poor mental health outcomes. Objective: The purpose of this convergent mixed-methods study is to understand the relationship between caregiving experiences and mental health and explore stress management strategies in African American family caregivers of adults with chronic or disabling conditions. Methods: African American family caregivers (N = 100) were recruited using community-engaged methods and completed a sociodemographic questionnaire, the Caregiver Reaction Assessment scale, and the Center for Epidemiologic Studies Depression (CES-D-10) Scale. A subsample (n = 24) participated in semi-structured interviews. Data were analyzed using linear regression and content analysis. A matrix was developed to integrate quantitative and qualitative results. Results: Participants were on average 59 years old. Most were women and provided care to a parent. Lack of family support (B = 1.37, P = .03) and impact of caregiving on caregivers' finances (B = 1.74, P = .004), schedule (B = 2.92, P < .001), and health (B = 3.26, P < .001) were associated with depressive symptoms and were reported as stressful experiences. Negative interactions with the care recipient and caring for multiple people emerged as other sources of stress. Participants used independent and interpersonal coping strategies, as well as strategies to facilitate their caregiving role to reduce their stress. Values of reciprocity, limited use of community-based resources, and mental health stigma emerged as important cultural considerations. Conclusions: Our findings suggest the need for culturally-sensitive interventions to improve communication and care coordination within African American family caregiving networks and educational programs about mental health and caregiving resources endorsed by trusted community sources.

Abstract:Abstract With the rapid growth of aging populations, the number of older adults with dementia is increasing worldwide. While there is a significant amount of research on dementia caregivers, we know very little about Chinese American (the largest subgroup of the Asian American population in America) caregivers. Therefore, the aims of this study are to 1) conduct a scoping review by identifying existing studies on Chinese American dementia caregivers, 2) present the current state of the science on Chinese American dementia caregiving, and 3) provide direction for future research. Twenty-one studies were included in the final review with 3 main themes synthesized (care experience, utilization of programs/services, and recruitment for caregivers). Care experience included illness perception towards Alzheimer’s Disease and related dementia (ADRD) such as stigma and normalization of the disease process. Filial piety was another important cultural belief underpinning care experience. An underutilization of supportive programs/services among this population was identified. Additionally, the few existing programs/services for Chinese American caregivers as well as the barriers encountered when seeking these programs/services were seen in the literature. The strategies and barriers of the included research articles for recruitment of Chinese American caregivers are also discussed in this study. These findings provide an overview of the current knowledge about Chinese American caregivers and serve as a stepping stone for future studies on similar populations in promoting caregiver’s health and developing culturally sensitive caregiver support services.

Abstract:Children with intellectual and developmental disabilities are often over-represented in child protection and out-of-home care services. Despite this fact, little is known or understood about their experiences with these services. This scoping review seeks to identify the published literature about children with intellectual and developmental disabilities who are engaged with child protection and out-of-home care services. Findings from the 18 studies included in this review show that these children are at increased risk of entering the child protection and out-of-home care system, with outcomes that do not fare as favourably as their nondisabled peers. This is largely due to a lack of support, training, and resources for the children, their families, and carers. Placement failure was a common theme with residential care more likely for children with greater complexity and support needs. These children are understudied, with more research needed to better understand the factors that contribute to their experiences. Greater insights into how a child protection and out-of-home care pathway can be avoided, via the provision of adequate services and supports, are also urgently needed so that fewer children are placed at risk in a system where poor outcomes are common.

Abstract:The type of care setting a child is placed in when they enter the child welfare system has long-lasting effects. Although numerous studies have examined factors associated with placement decisions and placement moves, few studies have paid attention to factors involved in both placing children into and removing children from kinship care. This study focused on kinship care as the top of the hierarchy of out-of-home and utilized data obtained from a U.S. mid-Atlantic State Automated Child Welfare Information System. The study followed children who placed in out-of-home care over a three-year period. Descriptive, bivariate, and regression model analyses were conducted to identify: (1) Among children experiencing a first out-of-home care placement (N = 3838), what factors are associated with initial placement into kinship care versus non-kin foster care and group homes/residential treatment centers (RTC); and (2) among children experiencing a second out-of-home placement during this period (n = 1893; 51% of the original sample), what factors are associated with being moved from a kinship care to a less-preferred care setting and vice versa. Two factors increased the odds of initially being placed in kinship care: race of the child (African American children are more likely to be placed in kinship care) and presence of parental substance abuse; in contrast, the presence of child behavioral problems and disabilities, as well as more advanced age, decreased the odds of kinship placement. For children experiencing a second move from kinship care into non-kin foster care, the associated factors were: Older children, behavioral problems, and parental incarceration. Finally, for children placed initially in non-kin foster care and moved to kinship care, moves were primarily associated with parental characteristics, including housing issues, substance abuse, and difficulties coping with the child. Implications for policy, research, and practice are discussed.

Abstract:Information on maternal age and maternal serum alpha‐fetoprotein, unconjugated oestriol (uE3), and human chorionic gonadotrophin (hCG) levels was used to investigate retrospectively the effect of estimating Edward's syndrome risk in women having multi‐marker screening for Down's syndrome. The screened population comprised 15 pregnancies affected by Edward's syndrome, 15 with Down's syndrome and 5472 unaffected pregnancies. The use of all three markers to estimate Edward's syndrome risk would have led to the detection of 10–12 (67–80 per cent) cases with a false‐positive rate of 0.3–0.6 per cent depending on the risk cut‐off. A further case would have been detected as a result of screening for Down's syndrome alone. Similar results were obtained when the Edward's syndrome risk was based on uE3 and hCG only. These data suggest that extending Down's syndrome screening to include Edward's syndrome risk will yield a high detection rate with only a small increase in the false‐positive rate.
Methodological Challenges to the Evaluation of Interventions for Foster/Kinship Carers and Children: A Systematic Review

Abstract:Caregivers of children in alternative care face a complex range of challenges that can result in placement disruption and poor long-term outcomes. Interventions designed to help carers meet these challenges report positive outcomes. Nevertheless, several reviewers have reported these positive results may be mitigated by limitations in trial methodology. This review aims to systematically review these methodological challenges and limitations, to provide an analysis of the current state of the evidence base for these interventions. A systematic review was conducted into the methods used to evaluate the effectiveness of psychosocial interventions for foster and kinship carers. Limitations relating to internal validity, external validity and clinical heterogeneity were identified and synthesised. Seventeen studies met inclusion criteria. The quality of methods used in the included studies is mixed, with high and unknown levels of bias in the majority of trials. Heterogeneity in participant characteristics, intervention aims and outcome measures across interventions reflect the diversity of carer and child needs and make it difficult to generalise results or compare and synthesise the efficacy of different interventions. These factors limit the application of trial results to evidence-based clinical practice. The diverse and complex needs of this population present significant challenges to robustly evaluating interventions for foster/kinship families. Participant needs, theoretical approaches, intervention aims and outcome measures need to be better coordinated, both within trials and across the field. Exploratory research should be used to generate focussed and concrete hypotheses that can be robustly tested in high-quality randomised controlled trials. Abstract:Trauma, Violence, &Abuse, Ahead of Print. Professional quality of life (ProQOL) refers to workers' subjective feelings associated with work involved in helping others who have experienced trauma. It consists of positive and negative aspects, that is, subscales of compassion satisfaction, and burnout and secondary traumatic stress. Foster and kinship caring inherently involves risks associated with exposure to the trauma responses of children in their care. This exposure can lead to poor ProQOL, carer attrition, and placement instability. While limited studies specifically explore ProQOL of carers, many studies have examined factors and interventions related to ProQOL. However, there is a lack of synthesis of these studies. To fill such a research gap, we undertook a scoping review of 70 empirical studies from Australia, the United Kingdom, and the United States, published from 2012 to 2022 reporting on ProQOL, and its related factors and concepts. We applied a multilevel ecosocial construct to examine complex interrelationships between private and governance settings to better understand factors related to ProQOL of carers and interventions aimed to improve it in these dynamic systems. In our review, some studies showed positive outcomes for carers, such as reduced stress or burnout associated with training. However, there was insufficient attention to factors associated with ProQOL at relational and sociopolitical levels. It is crucial to improve carers' ProQOL or well-being to ensure their retention and placement stability. Long-term systemic improvements require interventions across different levels of the system.

Abstract:The combined effect of race and place of residence on caregivers’ perceptions of children’s mental health services is underresearched. Differences in caregiver strain, barriers-to-care endorsement, and provider satisfaction are examined among 175 rural and urban African American caregivers from one Southern state whose children received Medicaid sponsored mental health services. Children exhibited clinical levels of externalizing and internalizing mental health symptoms and high levels of co-occurring physical health problems. Almost one quarter were cared for by someone other than biological parents. Almost all caregivers reported strain associated with children’s mental health problems, compromised personal health status, and high levels of depression. Family perception barriers (such as concern about potential negative reactions of significant others) were most endorsed. Most caregivers were satisfied with their children’s mental health services, with lowest levels on the provider sensitivity subscale. Initial rural—urban differences in caregiver strain did not remain significant in multiple regression analyses after other covariates were included.

Abstract:BACKGROUND/AIMS It is useful to decide whether lymphatic involvement or lymph node metastasis exists before polypectomy or operation in submucosal colorectal cancer. Whether vascular endothelial growth factor-C (VEGF-C) or VEGF-D could predict lymph node metastasis and lymphatic involvement is uncertain. METHODOLOGY Expression of the VEGF-C and VEGF-D in human submucosal colorectal cancers was investigated in paraffin-embedded stepwise sections by means of immunohistochemistry, and the correlation between immunohistochemical expression pattern and clinicopathological features was also evaluated. RESULTS The results showed that VEGF-C overexpression correlated with lymphatic involvement (P = 0.01) and lymph node metastasis (P = 0.02), but VEGF-D overexpression did not correlate significantly. In multivariate analysis lymphatic invasion was the predictive factor (P = 0.0129), but VEGF-C positivity was not predictive (P = 0.3437). CONCLUSIONS These results may suggest that VEGF-C is a more specific risk factor for lymph node metastasis than VEGF-D in submucosal colorectal cancer.

Abstract:Several studies indicate that management styles and knowledge of organizational structures have an impact on the potential for staff innovation. This exploratory study was conducted to investigate the implications of such variables in relation to nursing managers in a public sector health organization in Victoria, Australia. It reviewed factors that appeared to impact and encourage innovative behaviour amongst nurse managers within a decentralized management structure. To evaluate nurse managers' ability to be innovative, a 55-item survey consisting of both open questions and Likert-type scale questions was developed and distributed to 140 nurse managers working in a public sector health organization. The response rate was 58% yielding 82 usable responses. The study findings revealed that factors of management structures and management styles, in conjunction with the nurse unit managers' perceptions and communication skills, had an impact on nurse manager's ability to exhibit innovative behaviour in the 21st century workplace.

Abstract:We aim to explore the experiences of fathers from inner-city families caring for children affected by chronic health conditions or disabilities. A systematic scoping review was conducted using the Arskey and O’Malley framework. Fourteen of the 5114 articles were included in the full review and were qualitatively evaluated in terms of stressors, resources, perception, coping, and adaptation according to the Double ABCX model. Stressors included financial strain and health care access barriers. Resources ranging from immediate to extended family members depended on ethnicity. Fathers’ perceptions of their primary caregiver roles depended on ethnicity in the context of cultural gender norms. While inner-city fathers desired information about their children’s health, some were uncomfortable asking physicians. They had a higher risk for coping difficulties and maladaptation, including depression. We highlight a need for pediatricians to advocate for additional resources to provide comprehensive care for inner-city fathers caring for their children with chronic health conditions or disabilities.

Abstract:Kinship care and child fostering are a common practice in certain parts of Africa. This narrative review seeks to explore and thematically code the contextual circumstances in which children leave their natal homes and are fostered across Africa and the corresponding outcomes for these children. It draws on case studies from Cameroon, the Democratic Republic of the Congo, Ethiopia, Kenya, and Namibia and other countries across the region to illustrate the diversity and cultural importance of kinship care and child fostering. These countries were selected based on their inclusion in the Thrive by Five International Program, a child rearing program promoting social, emotional, and cognitive development of children from birth to age five. The importance of kinship care and child fostering is also reinforced by outcomes from codesign workshops conducted for the Thrive by Five International Program in which parents, caregivers, and subject matter experts discussed their perspectives on child rearing practices and the challenges faced by caregivers in their home countries. The paper highlights the need to support kinship and foster caregivers in Africa with culturally adapted and place‐based child rearing programs to help ensure fostered children reach their full developmental potential.

Abstract:OBJECTIVE: As numbers of sandwiched caregivers in the United States grow, it is essential to document the literature on the impacts of dual care demands on well-being and health.METHOD: Guided by Arksey and O'Malley's (2005) framework, this scoping review examined the literature on sandwiched caregivers' psychological well-being and physical health, identified gaps in the literature, and provided future directions to advance the family caregiving literature.RESULTS: Findings showed that there were inconsistencies in the conceptualization of sandwiched caregivers. Additionally, an examination of the studies showed that sandwiched caregivers exhibited lower levels of psychological well-being and poorer health behaviors compared to other types of family caregivers and noncaregivers. Furthermore, the quality of sandwiched caregivers' psychological well-being was shaped by family and work contexts.CONCLUSIONS: Future research should direct more attention to the examination of sandwiched caregivers' physical health and those of minority status given that multigenerational care occurs at greater rates in these populations. Abstract:Purpose: The COVID-19 pandemic magnified pre-existing socioeconomic, operational, and structural challenges in long-term care across the world. In Canada, the long-term care sector's dependence on caregivers as a supplement to care workers became apparent once restrictive visitation policies were employed. We conducted a scoping review to better understand the associations between caregiving and resident, formal and informal caregiver health in long-term care before and during the COVID-19 pandemic. Methods: A literature search was performed using MEDLINE, AgeLine, Google Advanced, ArXiv, PROSPERO, and OSF. Pairs of independent reviewers screened titles and abstracts followed by a review of full texts. Studies were included if they reported biological, psychological, or social health outcomes associated with caregiving (or lack thereof). Results: After screening and reviewing 252 records identified by the search strategy, a total of 20 full-text records were eligible and included in this review. According to our results, research on caregiving increased during the pandemic, and researchers noted restrictive visitation policies had an adverse impact on health outcomes for residents and formal and informal caregivers. In comparison, caregiving in long-term care prior to the pandemic, and once visitation policies became less restrictive, led to mostly beneficial health outcomes. Conclusion: Caregiver interventions, for the most part, appear to promote better health outcomes for long-term care residents and formal and informal caregivers. Suggestions to better support caregiving in long-term care settings are offered.
Program Components of Psychosocial Interventions in Foster and Kinship Care: A Systematic Review

Abstract:Foster children frequently experience early trauma that significantly impacts their neurobiological, psychological and social development. This systematic review examines the comparative effectiveness of foster and kinship care interventions. It examines the components within each intervention, exploring their potential to benefit child and carer well-being, particularly focussing on child behaviour problems, and relational functioning. Systematic searches of electronic databases included PsycINFO, MEDLINE, Web of Science Core Collection, the Cochrane Collaborations Register of Controlled Trials (CENTRAL) and Scopus to identify randomised or quasi-randomised trials of psychosocial foster/kinship care interventions, published between 1990 and 2016. Seventeen studies describing 14 interventions were included. Eleven studies reported comparative benefit compared to control. Overall, effective interventions had clearly defined aims, targeted specific domains and developmental stages, provided coaching or role play, and were developed to ameliorate the effects of maltreatment and relationship disruption. Interventions effective in reducing behaviour problems included consistent discipline and positive reinforcement components, trauma psychoeducation, problem-solving and parent-related components. Interventions effective in improving parent-child relationships included components focussed on developing empathic, sensitive and attuned parental responses to children's needs. Given the prevalence of both behaviour problems and relational difficulties in foster families, targeting these needs is essential. However, interventions have tended to measure outcomes in either behavioural or relational terms. A more coordinated and collaborative research approach would provide a better understanding of the association between parent-child relationships and child behaviour problems.
