Abstract:Abstract CDK4/6 inhibitors (CDK4/6i) is well established as the current standard of care for hormone positive (HR(+)) metastatic breast cancer (mBC), in combination with endocrine therapy. CDK4/6i extend a chemotherapy-free, progression free survival (PFS) that preserves quality of life in patients. The most common side effect of CDK4/6i is myelosuppression, with neutropenia the most prevalent adverse effect, especially for palbociclib and ribociclib. In PALOMA-2 and MONALEESA-2, where neutropenia was prevalent (any grade neutropenia 79.5%, grade 3/4 neutropenia 66.5% in PALOMA-2, and any grade neutropenia: 74.3%, grade 3/4 55.3% in MONALEESA-2), although febrile neutropenia seldom occurred (1.8%). Several studies have proposed different genetic factors predisposing to CDK4/6 inhibitor induced neutropenia, including Duffy antigen polymorphisms, ABCB1 and ERCC1 polymorphisms (ABCB1_rs1128503, ABCB1_rs1045642, ERCC1_rs11615, ERCC1_rs3212986), CDK6 polymorphisms, and others. Subgroup studies from the PALOMA trials have suggested that Asian patients receiving palbociclib have higher rates of neutropenia, although the exact explanation is unknown. We conducted a retrospective analysis of 102 Taiwanese patients who received palbociclib for HR(+) mBC at the Taipei Veterans General Hospital for the clinical features, incidence and time course of neutropenia were analyzed. Significant neutropenia incidence was observed (> 95% of all grade neutropenia, 74.5% grade 3/4 neutropenia). In addition, in our clinical cohort, one of the patients was a long term peritoneal dialysis patient newly diagnosed of HR(+) mBC and started on Palbociclib, initially 125mg, later decreased dosage to 100mg after neutropenia occurrence. By Liquid chromatography–mass spectrometry (LC–MS), we quantified the levels of palbociclib in her serum and peritoneal fluid at various time points, while on different dosage use. To our knowledge, this is the first pharmacokinetics analysis on a peritoneal dialysis patient receiving CDK4/6i. To investigate a possible genetic association for the high prevalence of neutropenia, we queried the Taiwan biobank, a nationwide biospecimen repository with 129,586 healthy individual genome wide DNA sequencing data deposited. We investigated the prevalence of the 4 SNPs previously reported to be related to neutropenia in the PALOMA studies. By comparing the Taiwan biobank data to SNP databases from different ethnicities, we observed interesting findings on ethnicity differences of SNP distribution and higher prevalence of neutropenia SNPs. Collectively, in this study we report real world data, biobank genome wide analysis, and a novel report of pharmacokinetic study in a peritoneal dialysis patient, providing novel insights into the real world use of palbociclib and CDKi. Safety and Patient-Reported Outcomes in Patients with Hormone Receptor-Positive, HER2-negative Advanced Breast Cancer Treated with CDK4/6 Inhibitors in China: A Cross-Sectional Study.

Abstract:1054 Background: It is reported that the adverse event (AE) profile may vary with different CDK4/6 inhibitors. With the prolongation of survival, AEs occurred during the treatment period may affect quality of life (QoL) for patients with advanced disease. Currently, there has been no study focusing on the safety and patient-reported outcomes in Chinese patients treated with different CDK4/6 inhibitors. Methods: This was a large cross-sectional study conducted at 10 sites in China. Randomly selected patients with hormone receptor-positive, HER2-negative advanced breast cancer and their physicians were asked to fill the self-designed questionnaires. Disease characteristics and AEs were collected from the perspectives of both patients and physicians, while the patient questionnaire additionally included feedback on the AE management and the European Organization for Research and Treatment of Cancer (EORTC) Quality-of-Life Questionnaire-breast cancer module (QLQ-BR23). Results: Between Jan 30, 2023 and Sep 15, 2023, a total of 1254 patients (median age, 55 years) were included; 212 (16.9%) patients were treated with palbociclib, 483 (38.5%) with abemaciclib, 550 (43.9%) with dalpiciclib, and 9 (0.7%) with ribociclib. Of them, 197 (15.7%) had switching history or dose adjustment of CDK4/6 inhibitor, with the main reason being AEs (51.8%). From the perspective of physicians, the most common AEs for patients were decreased white blood cell (63.3%), decreased neutrophil count (58.7%), and diarrhea (36.3%). The incidence of increased alanine aminotransaminase (ALT) and increased aspartate aminotransaminase (AST) was 9.3% (grade ≥3, 6.0%) and 8.4% (grade ≥3, 6.7%), respectively. Dalpiciclib showed the lowest incidence of increased ALT (3.5%; grade ≥3, none), increased AST (4.2%; grade ≥3, none), and diarrhea (10.5%) compared with other CDK4/6 inhibitors (P<0.001). From the perspective of patients, dalpiciclib showed the lowest incidence of fatigue, alopecia, hot flashes, insomnia, arthralgia, nausea, vomiting, diarrhea, and anorexia (P<0.05). Regarding QoL, the mean scores of upset by hair loss for different CDK4/6 inhibitors were higher than other symptom subscale scores. Dalpiciclib showed the lowest mean score of upset by hair loss compared with abemaciclib and palbociclib, whether in total (19.74 vs 27.23 vs 21.47, P=0.006) or in patients with >6 months of treatment (12.59 vs 27.78 vs 17.95, P<0.001). Conclusions: AEs are different among various CDK4/6 inhibitors. Dalpiciclib has the lowest risk of diarrhea and hepatotoxicity based on physicians' reports, and shows the best patient-perceived safety. Regarding QoL, CDK4/6 inhibitors can result in upset by hair loss, while patients have the least concern about this issue when using dalpiciclib, especially for those with long treatment duration (>6 months).
First-line CDK4/6 inhibitor-based combinations for HR+/HER2-advanced breast cancer: A Bayesian network meta-analysis

Abstract:BackgroundInternational guidelines recommend cyclin-dependent kinase 4/6 inhibitor (CDK4/6i)-based first-line therapy for hormone receptor-positive, human epidermal growth factor receptor 2-negative (HR+/HER2-) advanced breast cancer (ABC). However, direct drug comparisons are lacking. We aimed to identify the most effective and safe therapy through network meta-analysis (NMA).MethodsWe searched PubMed, Embase, Web of Science, Cochrane Central Register of Controlled Trials, and OpenGrey up to September 30, 2023. Eligible studies included randomized controlled trials (RCTs) assessing endocrine therapy alone or in combination with CDK4/6i as first-line endocrine treatment for HR+/HER2- ABC patients. The hazard ratios for progression-free survival (PFS) and overall survival (OS) and relative risks for objective response rate and adverse events (AEs) were available in selected trials. We performed a Bayesian NMA following PRISMA guidelines.ResultsThirteen RCTs, involving 10 treatments, were included. Most studies were at low risk of bias. Regarding PFS, ribociclib+fulvestrant ranked first with a surface under the cumulative ranking curve (SUCRA) of 85.0%, followed by dalpiciclib+nonsteroidal aromatase inhibitor (NSAI) (SUCRA = 78.9%). Considering OS, the top three ranked treatments were ribociclib+fulvestrant (SUCRA = 94.1%), abemaciclib+NSAI (SUCRA = 69.9%), and ribociclib+NSAI (SUCRA = 68.5%). Out of four CDK4/6is, ribociclib minimized the grade 3/4 AEs, while dalpiciclib demonstrated the worst safety. Publication bias could not be ignored in our analyses, and the certainty of evidence was downgraded primarily due to imprecision.ConclusionsRibociclib+fulvestrant probably represents the best option in a first-line setting. When combined with NSAI, dalpiciclib likely showed the best efficacy but the worst safety. Abemaciclib+NSAI and ribociclib+NSAI could also be promising treatments, while palbociclib presented inferiority. (PROSPERO Registration No. CRD42022370271)

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Cyclin-dependent kinase 4/6 (CDK4/6) inhibitors, including palbociclib, are approved to treat hormone receptor (HR)-positive/human epidermal growth factor receptor 2 (HER2)-negative advanced breast cancer (ABC) and are associated with hematologic toxicity. African American women, who are underrepresented in CDK4/6 inhibitor clinical trials, may experience worse neutropenia because of benign ethnic neutropenia. The authors specifically investigated the hematologic safety of palbociclib in African American women with HR-positive/HER2-negative ABC.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>PALINA was a single-arm, open-label, investigator-initiated study of palbociclib (125 mg daily; 21 days on and 7 days off) plus endocrine therapy (ET) in African American women who had HR-positive/HER2-negative ABC and a baseline absolute neutrophil count ≥1000/mm<sup>3</sup> (ClinicalTrials.gov identifier NCT02692755). The primary outcome was the proportion of patients who completed 12 months of therapy without experiencing febrile neutropenia or treatment discontinuation because of neutropenia. Single nucleotide polymorphism analysis was used to assess Duffy polymorphism status. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Thirty-five patients received ≥1 dose of palbociclib plus ET; 19 had a Duffy null polymorphism (cytosine/cytosine). There were no reports of febrile neutropenia or permanent study discontinuation because of neutropenia. Significantly more patients with the Duffy null versus the wild-type variant had grade 3 and 4 neutropenia (72.2% vs 23.1%; <i>P</i> = .029) and required a palbociclib dose reduction (55.6% vs 7.7%; <i>P</i> = .008). Patients with the Duffy null versus the wild-type variant had lower overall relative dose intensity (mean ± SD, 81.89% ± 15.87 and 95.67% ± 5.89, respectively; <i>P</i> = .0026) and a lower clinical benefit rate (66.7% and 84.6%, respectively). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>These findings suggest that palbociclib is well tolerated in African American women with HR-positive/HER2-negative ABC. Duffy null status may affect the incidence of grade 3 neutropenia, dose intensity, and possibly clinical benefit.</p></section>

Abstract:Abstract Background: PALB2 (Partner and Localizer of BRCA2) is a breast cancer predisposition gene, which plays a critical role in homologous recombination via interacting with BRCA1/2 and RAD51 when DNA break. However, the clinical relevance of PALB2 germline mutations in Chinese breast cancer patients remains unknown. We attempted to illustrate the full prevalence and spectrum of PALB2 germline mutations in Chinese breast cancer population, and to investigate the clinical outcomes of PALB2 germline mutations. Patients and methods: A total of 21216 unselected breast cancer patients were enrolled from multiple clinical centers in China. After evaluation of the DNA quality and clinical information, 16501 eligible BRCA1/2-negative breast cancer patients were analyzed. A group of 5890 Chinese women without cancer was enrolled as healthy controls. PALB2 screening was based on next-generation sequencing, by screening complete coding sequence and intron-exon boundaries of PALB2. Clinical information was synchronously collected. Results: The 79 PALB2 deleterious germline mutations included 25 nonsense mutations, 47 frameshift mutations, and 7 splicing mutations. The hotspot region for PALB2 mutations was exon4. A total of 41 novel mutations were identified in PALB2. Among the 16501 BRCA1/2-negative breast cancer patients and 5890 healthy women, PALB2 deleterious mutation carriers account for 0.97% (n=160) and 0.19% (n=11) separately. PALB2 mutations were significantly associated with increased breast cancer risk (OR: 5.23, 95% CI 2.89-9.49; P<0.0001), and especially in women aged 30 or younger (OR: 11.97, 95% CI 3.76-34.39; P<0.0001). PALB2 mutation carriers were significantly more likely to be triple negative breast cancers and have bigger tumors, positive axillary lymph nodes, bilateral breast cancers, family history of breast and/or ovarian cancer and family history of any other cancer, than noncarriers. Conclusions: The PALB2 mutations confer a moderately increased breast cancer risk in women, but relatively highly increased breast cancer risk in those aged 30 or younger. PALB2 mutation status is associated with distinct clinical characteristics in breast cancer. Starting Dose Selection of Palbociclib in Chinese Patients with Breast Cancer Based on Population Kinetic–pharmacodynamic Model of Neutropenia

Abstract:Neutropenia is the most common adverse event (AE) of palbociclib, an oral CDK4/6 inhibitor for breast cancer. Neutropenia increases the risk of infection and is even life threatening. Asian patients generally suffer more severe neutropenia from palbociclib treatment, but the label does not recommend a reduction in the starting dose for Asian patients. Therefore, the study aimed to explore the exposure–response (E–R) relationship in Chinese patients and preliminarily generate a scale for starting dose selection of palbociclib in Chinese patients. After comparing the kinetic–pharmacodynamic (K–PD) and the pharmacokinetic/pharmacodynamic (PK/PD) model, a semi-mechanistic K–PD model was selected and developed on the basis of real-world data from 28 patients with breast cancer to describe the time course of longitudinal absolute neutrophil counts (ANC). The longitudinal ANC data were well described by the population K–PD model with reasonable parameters: mean transit time (MTT) of 198 h, feedback parameter (γ) of 0.317, baseline ANC level (Circ0) of 3.36 × 109 L−1, drug effect coefficient (kd) of 0.0349, and drug effect power (β) of 0.383. No covariate was included in the final model. The model showed that palbociclib dose-dependently reduced ANC levels in a Chinese population, and lower baseline ANC level was associated with more severe neutropenia. The dose selection scale suggested that palbociclib 125 mg daily was appropriate for Chinese patients with Circ0 higher than 3.75 × 109 L−1. In summary, the K–PD model of palbociclib well described the longitudinal ANC in Chinese patients. Besides, the starting dose selection scale may provide reference for clinicians during individualized therapy.

Abstract:e13039 Background: Palbociclib has been approved as the first CDK4/6 inhibitor in China since 2018. This study aimed to collect real-world data for retrospective analysis of the clinical outcomes and potential clinical and genetic risk factors for Palbociclib plus ET. Methods: Data of 194 HR+HER2- ABC patients who received Palbociclib plus ET between May 2018 and Dec 2020 were collected from the electronic medical record system of seven cancer centers across China. Cox regression models and bioinformatic analysis were conducted to investigate the risk factors. Results: In total, 194 patients were included in the study. Among them, 138 patients discontinued Palbociclib plus ET due to disease progression (130 patients), financial inability (5 patients), or intolerable adverse reactions (3 patients) as of the observation endpoint. With a median follow-up period of 22.4 months, the median progression-free survival (mPFS) was 12.3 months, the median time to failure (mTTF) was 11.8 months, and the median overall survival (mOS) was 35.5 months. Moreover, the 1-year PFS, TTF and OS rates were 50.8%, 49.0% and 87.6%, respectively. Of the 155 with measurable disease, the objective response rate (ORR) was 27.1%, the disease control rate (DCR) was 71.6% and the clinical benefit rate (CBR) was 81.3%. KM curves showed that first-line users had the mPFS of 19.0 months, which was significantly longer than the second-line users and the third (or higher) line users (mPFS = 10.7 and 7.2 months, p<0.0001). Multivariate Cox regression analysis revealed several clinical features as risk factors, whereas liver metastasis was a significant risk factor for poor PFS and OS (HR = 1.993, [95%CI, 1.243-3.195], p = 0.004; HR = 2.345, [95%CI, 1.089-5.049, p = 0.029). Central nervous system (CNS) metastasis was an independent risk factor for poor OS (HR = 2.658, [95%CI, 1.152-6.136], p = 0.022). Response to CR/PR was a significant protective factor for PFS (HR = 0.289, [95%CI, 1.161-0.519], p<0.001) but not for OS. Postmenopausal patients had a higher PFS risk than premenopausal patients (HR = 1.549, [95%CI, 1.007-2.382], p = 0.046). Seven patients received circulating tumor DNA (ctDNA) testing at the baseline and after disease progression, which showed that the mutation frequencies of FGFR1 , ARID1A , ATRX , CHD4 , FAT1 , and PTEN increased significantly after disease progression. Conclusions: Treatment with Palbociclib plus ET exhibited favorable efficacy for HR+HER2– ABC in Chinese real-world practices, especially for non-liver, non-CNS metastasis or premenopausal patients. Early-line treatment yielded more durable benefits, and response to CR/PR indicated a better PFS prognosis. Mutations in FGFR1 , ARID1A , ATRX , CHD4 , FAT1 , and PTEN might be associated with resistance to the combination therapy.

We aimed to evaluate the pharmacokinetics, safety, and efficacy of palbociclib combined with letrozole as first-line treatment for ER-positive and HER2-negative advanced breast cancer. Methods This is a phase 1, open-label, single-arm study in Chinese women with ER-positive and HER2-negative advanced breast cancer. From cycle 1 day 1, patients received letrozole 2·5 mg orally once daily continuously plus palbociclib 125 mg orally once daily for 3 weeks followed by 1 week off treatment. Blood samples for pharmacokinetic evaluation of palbociclib were collected up to 120 h after a single dose of palbociclib in a lead-in phase and after palbociclib dose on cycle 1 day 21 for multiple dose pharmacokinetic evaluation. Predose blood samples for pharmacokinetic evaluation of palbociclib and letrozole were collected on days 19–21 of cycle 1 and cycle 2 day 1 (letrozole only). Disease assessments were done every 12 weeks from cycle 1 day 1. Safety was assessed per Common Terminology Criteria for Adverse Events, version 4.0. This study is registered with ClinicalTrials, number NCT02499146, and is ongoing but not recruiting. Findings As of March 16, 2016, the cutoff date, the study completed enrolment with 26 patients. All patients completed single dose pharmacokinetic evaluation and 13 completed multiple dose pharmacokinetic evaluation. After multiple doses, the steady-state palbociclib geometric mean area under the concentration–time curve from 0 to 24 h was 2005 ng × h/mL and maximum observed concentration was 112·3 ng/mL, similar to those obtained in Caucasian patients following the same dosing regimen. Comparisons of single dose and multiple dose pharmacokinetic data indicated linear pharmacokinetics of palbociclib. The geometric mean accumulation ratio of palbociclib exposure after multiple dose compared with single dose was 2·1, consistent with a terminal half-life of 23·4–27·5 h. Trough concentrations of letrozole were similar to concentrations obtained in Caucasian patients. No deaths or permanent discontinuations due to adverse events were reported. One serious adverse event occurred but was not considered to be treatment-related. The most common clustered adverse events were neutropaenia (n=20, 77%), leukopaenia (n=19, 73%), and anemia (n=11, 42%), with neutropaenia (n=15, 58%) and leukopaenia (n=8, 31%) the most common grade 3–4 adverse events. Interpretation No dose adjustment for palbociclib is needed based on the Chinese population. The toxicity of palbociclib in combination with letrozole treatment was tolerable and manageable. Abstract:Background The cyclin-dependent kinase (CDK) 4/6 inhibitor PAL is approved for treatment of estrogen receptor–positive (ER+)/human epidermal growth factor receptor 2–negative (HER2–) ABC in combination with endocrine therapy. This ongoing, phase 1, open-label, single-arm study (NCT02499146) is evaluating the pharmacokinetics, safety, and efficacy of PAL+L as first-line treatment for ER+/HER2– ABC in Chinese women. The longitudinal alterations of multiple pharmacodynamic biomarkers related to PAL mechanism of action and cell cycle modulation were also characterized in this study. Methods From Cycle (C) 1, Day (D) 1, patients received PAL 125 mg/d 3 wk on followed by 1 wk off plus L 2.5 mg/d continuously. Skin biopsies (SBs) and blood samples for biomarker evaluation were collected after a single dose (SD) of PAL in a lead-in phase and after PAL dose on C1D21 (at steady state). SB as a tumor tissue surrogate for Ki67 and pRb expression were collected predose and on 2 randomly assigned schedules: (1) 24 h after SD and on days 22, 24 and 26 of C1; (2) 10 h after SD and on days 21, 23 and 25 of C1. Serum samples for thymidine kinase (TK) evaluation as a marker for cell cycle modulation were collected predose and up to 120 h after SD, on D 21–26 of C1, and D1 of C2. Results Out of 26 patients, 25 had an evaluable baseline and ≥1 postdose pRb and Ki67 value; 26 blood samples from baseline, 156 from the lead-in phase, and 222 after multiple-doses were analyzed for TK activity, including 4, 54, and 153 samples, respectively, below the limit of quantification. Skin expression levels of pRb were reduced instantly from baseline after a SD of PAL+L and remained low at steady-state levels of PAL following multiple doses, then rebounded back to or higher than baseline level during the off treatment period. A similar pattern was observed for skin Ki67 levels and serum TK activity, but both had a delayed reduction and rebounding during treatment and the off treatment period, respectively. Conclusions The on-target effect of PAL measured by longitudinal alterations of Ki67, pRb, and TK activity consistently demonstrated CDK4/6 inhibition and subsequent prohibition of cell cycle progression by PAL. Abstract:This phase 1, open-label, single-arm clinical trial evaluated pharmacokinetics, safety, and biomarker activity of palbociclib-letrozole as first-line treatment for estrogen receptor–positive/human epidermal growth factor receptor 2–negative advanced breast cancer (ABC) in postmenopausal Chinese women to support palbociclib approval in China. Patients received palbociclib 125 mg once daily (3/1 schedule) plus letrozole 2.5 mg once daily. Blood samples were collected predose and ≤ 120 h after single and multiple doses of palbociclib. The incidence and severity of adverse events were reported. Skin biopsy tissues and blood samples were collected for biomarker assessments. By 31 July 2018, 26 patients were enrolled. After single and multiple dosing, palbociclib maximum plasma concentration was 82.14 and 139.7 ng/mL, apparent clearance was 52.40 and 49.97 L/h, AUCτ was 1217 and 2501 ng∙h/mL, and t½ was 23.46 and 27.26 h, respectively. Levels of Ki67, retinoblastoma protein, and thymidine kinase decreased after palbociclib treatment. A similar safety profile as previously reported was observed. Pharmacokinetic and pharmacodynamic effects of palbociclib were well characterized in Chinese patients with ABC. Despite higher exposure, pharmacokinetic parameters were similar to those of a previously studied non-Asian population. No palbociclib dose adjustment based on Chinese ethnicity is needed. Abstract:Background: The clinical selection of three CDK4/6 inhibitors presents a challenging issue, owing to the absence of distinct clinical case characteristics, biomarkers, and their comparable clinical benefits in progression-free survival and overall survival To inform clinical treatment decisions, we conducted a comprehensive evaluation of the adverse events associated with CDK4/6 inhibitors in combination with endocrine therapy for hazard ratio+/HER2-breast cancer. Methods: We searched Cochrane, PubMed, Embase, and Web of Science databases from their inception until 1 August 2022. The results were summarized narratively, and we assessed the methodological quality, reporting quality, and evidence quality of AEs by AMSTAR-2, PRISMA, and GRADE. Results: Our analysis included 24 meta-analyses systematic reviews that evaluated the quality of AEs in 13 cases of early breast cancer (EBC) and 158 cases of advanced breast cancer The addition of CDK4/6 inhibitors was found to significantly increase AEs of any grade and AEs of grade 3 or higher in early breast cancer, along with a significant increase in the risk of treatment discontinuation. In advanced breast cancer, high and moderate-quality evidence indicated that CDK4/6 inhibitors significantly increased AEs across all grades, including grade 3/4 AEs, leucopenia, grade 3/4 leucopenia, neutropenia, grade 3/4 neutropenia, anemia, grade 3/4 anemia, nausea, grade 3/4 constipation, fatigue, pyrexia, venous thromboembolism abdominal pain, and cough. However, they did not significantly elevate the incidence of grade 3/4 diarrhea. Subgroup analysis revealed that palbociclib primarily increased hematologic toxicity, particularly grade 3/4 neutropenia, anemia, and thrombocytopenia. Ribociclib was mainly associated with grade 3/4 neutropenia, prolonged QT interval, and alopecia. Abemaciclib was closely linked with diarrhea and elevated blood creatinine levels. Conclusion: The AEs associated with CDK4/6 inhibitors vary, necessitating individualized and precise clinical selection for optimal management. This approach should be based on the patient's medical history and the distinct characteristics of different CDK4/6 inhibitors to improve the patient's quality of life. Real-world experience with CDK4/6 inhibitors in hormone receptor-positive metastatic and recurrent breast cancer: findings from an Asian population

Abstract:Purpose: Cyclin-dependent kinase 4/6 inhibitors (CDK4/6i) combined with endocrine therapy have demonstrated significant clinical benefits in progression-free and overall survival. This study investigates the outcomes associated with two kinds of CDK4/6i in patients with hormone receptor (HR)-positive metastatic and relapsed breast cancer to inform real-world evidence of treatment strategies. Methods: This retrospective study included 340 Taiwanese patients with HR-positive advanced breast cancer from the Taipei Veterans General Hospital, between 2018 and 2023. We analyzed patient characteristics, treatment strategies and outcomes associated with two CDK4/6i. The efficacy of patients who experienced economic burden and interrupted CDK4/6i treatment after 2 years of National Health Insurance (NHI) reimbursement was also investigated. Results: Patients receiving ribociclib and palbociclib showed no significant differences in age, histology, body mass index(BMI), or pathologic status. The distribution of disease status and endocrine therapy partners was comparable between the two groups. Dose reduction was similar, while patients with palbociclib tended to discontinue CDK4/6i usage, and those with ribociclib tended to switch to the other CDK4/6i or endocrine partners. There was no significant difference in progression-free survival (PFS) between the two CDK4/6i in the first-line setting. Adverse prognostic factors were increasing HER2 IHC score, higher Ki-67 levels, visceral and liver metastasis, prior chemotherapy, and endocrine therapy resistance, while higher BMI, bone-only metastasis, and letrozole treatment were associated with a lower risk of progression. The limited follow-up time in our study was insufficient to assess the outcomes of patients treated with interrupted CDK4/6i for up to two years under the NHI reimbursement policy. Conclusion: Treatment outcomes between the two types of CDK4/6i did not differ significantly, indicating the safety and efficacy of CDK4/6i for the Asian population. Ribociclib and palbociclib showed similar efficacy in PFS in the real-world setting.

Abstract:Abstract Background: The cyclin-dependent kinase 4/6 inhibitor palbociclib (PAL) is approved for the treatment of HR+/HER2– advanced breast cancer (ABC) in combination with an aromatase inhibitor (AI) as initial endocrine-based therapy or with fulvestrant (FUL) in patients whose disease progressed on prior endocrine therapy (ET). In clinical trials, the most common any-grade adverse event (AE) associated with PAL treatment was neutropenia. Here we evaluated the incidence of and risk factors for neutropenia by treatment cycle and line of therapy (LOT), as well as incidence of dose modification due to neutropenia, in patients with HR+/HER2– ABC receiving PAL in a real-world setting. Methods: POLARIS is a prospective, noninterventional, multicenter, real-world study of patients with HR+/HER2‒ ABC receiving PAL in routine clinical practice in the United States and Canada. Per protocol, sites were instructed to report absolute neutrophil count at baseline, anytime during the first 3 cycles, then at Day 1 (or before the start of a new cycle) of subsequent cycles per standard of care or as needed up to 6 months. The safety analysis set included all patients enrolled in the study who received at least one dose of study medication.Results: As of March 30, 2022, 1242 patients had enrolled in the study (median age, 64.0 years; 98.8% female; 94.8% stage IV; 39.1% visceral disease). Of these, 902 (72.6%) patients received PAL as first LOT and 340 (27.4%) as second LOT or greater. Among all patients, 722 received PAL + AI, 487 PAL + FUL, and 33 PAL + none or other ET (other than letrozole, anastrozole, fulvestrant, or exemestane); 90% of all patients started with 125 mg of PAL in the first cycle. Incidence of neutropenia among patients receiving PAL by cycle and LOT is shown in the Table. Percentages of patients experiencing on-treatment neutropenia were similar regardless of LOT. Over the course of treatment grade ≥3 neutropenia and febrile neutropenia were reported in 16.1% and 0.8% patients, respectively, and 19.2% of patients experienced a decreased neutrophil count. A total of 337 (27.1%) had a dose modification due to neutropenia. Of these 337 patients, 190 (56.4%) had a dose reduction associated with neutropenia. Conclusions: In this real-world study, incidences of treatment-related neutropenia associated with PAL were consistent with those reported in randomized clinical trials and were manageable by dose modification. Febrile neutropenia was uncommon. Incidences of neutropenia reported within the first 6 months were comparable among patients regardless of LOT. Abstract:Palbociclib has been shown to be a highly effective therapy in hormone receptor positive metastatic breast cancer when used in combination with letrozole or fulvestrant. Grade 3/4 neutropenia is a common side effect although febrile neutropenia is relatively uncommon. Insufficient data exist to describe the hematological safety of palbociclib in African American women (AAW) known to have a high incidence of benign ethnic neutropenia (BEN). PALOMA 1, 2 and 3, the initial phase II/III studies that led to the U.S. Food and Drug Administration (FDA) approval of palbociclib in metastatic breast cancer, only included participants with baseline absolute neutrophil count (ANC) of 1500/mm3 or higher. African American women (AAW) were underrepresented in the PALOMA trials and this may be partially explained by strict requirements for minimal ANC ≥1500/mm3. The ANC of 1500/mm3 for initiation of treatment in those with BEN has been previously challenged. In this study, we propose to lower the ANC cutoff for enrollment to 1000/mm3. PALINA (NCT02692755) is a phase II, single arm, multicenter clinical trial that will enroll 35 patients. The primary endpoint is to assess the proportion of patients who complete therapy without the development of febrile neutropenia or treatment discontinuation due to neutropenia. The secondary endpoints include number of patients who required dose delays or dose reductions in palbociclib attributed to neutropenia, rate of grade 3/4 neutropenia, clinical benefit rate at 24 weeks, the association between metabolite and exosomal signature with disease response and the association between baseline ANC prior to cancer diagnosis and the Duffy Null polymorphism (SNP rs2814778) with hematological safety. PALINA will provide important information about the hematologic safety of palbociclib in AAW with advanced breast cancer.

Abstract:Cyclin-dependent kinase 4 and 6 inhibitors (CDK4/6i) are the standard agents for treating patients with estrogen receptor-positive and human epidermal growth factor receptor 2-negative advanced breast cancer (ER + HER2 − ABC). However, markers predicting the outcomes of CDK4/6i treatment have yet to be identified. This study was a single-center retrospective cohort study. We retrospectively evaluated 101 patients with ER + HER2 − ABC receiving CDK4/6i in combination with endocrine therapy at Fukuyama City Hospital between November 2017 and July 2021. We investigated the clinical outcomes and the safety of CDK4/6i treatment, and the absolute lymphocyte count (ALC) and neutrophil-to-lymphocyte ratio (NLR) as predictive markers for CDK4/6i. We defined the cut-off values as 1000/μL for ALC and 3 for NLR, and divided into "low" and "high" groups, respectively. We evaluated 43 and 58 patients who received abemaciclib and palbociclib, respectively. Patients with high ALC and low NLR had significantly longer overall survival than those with low ALC and high NLR (high vs. low; ALC: HR 0.29; 95% CI 0.12–0.70; NLR: HR 2.94; 95% CI 1.21–7.13). There was no significant difference in efficacy between abemaciclib and palbociclib and both had good safety profiles. We demonstrated that ALC and NLR might predict the outcomes of CDK4/6i treatment in patients with ER + HER2 − ABC.

Mesenchymal stem cells: An alternative source of hepatocytes?

Abstract:In the beginning, there was the hepatocyte! If we limit the definition of a stem cell to its ability to self renew and to reconstitute a given tissue in vivo, hepatocytes fulfill both criteria. Serial transplantation experiments performed in the fumarylacetoacetate hydrolase (FAH)– deficient mice have underlined not only the extraordinary replication capacity of these differentiated cells but also the absence of a specific stem cell requirement to repopulate a rodent liver.1 Unfortunately, hepatocyte transplantation has very rarely produced therapeutic effects in human clinical trials mostly because hepatocytes are difficult to expand ex vivo and because their numbers are too low to achieve a biological effect.2,3 A somatic human stem cell that could be propagated in large quantities while retaining its ability to differentiate into different cell types could serve as a highly valuable resource for the development of cellular therapy in liver diseases. But does such a cell type exist? In certain pathological conditions, mainly when hepatocyte replication is blocked, bipotent oval cells proliferate and participate in liver regeneration by giving rise to hepatocytes and bile duct cells. However, the fact that oval cells have been shown to generate hepatocellular carcinoma or cholangiocarcinoma in rodents and that they are difficult to characterize to homogeneity jeopardizes their use in therapeutic approaches. (For review, see Fausto and Campbell.4) Then came the hematopoietic stem cells. Numerous recent studies including indirect a posteriori studies in humans suggested that hematopoietic stem cells (HSCs) have a broader differentiation potential than previously expected and could generate hepatocytes. However, except when a very strong selective pressure occurs to expand this phenomenon, such as in FAH-deficient mice,5 it seems that bone marrow cells have a very low capacity to generate hepatocytes in vivo even in injured livers, and that differentiation is probably dependent on the type of injury.6–11 In contrast, hematopoietic stem cells generate numerous nonparenchymal cells, such as Küpffer cells or endothelial cells that could play a role in liver regeneration or repair. Altogether, these observations prompted N. Fausto to conclude in a recent review: “as paradoxical as it may sound to stem cell biologists, the hepatocyte seemed, until now, the most highly efficient stem cell for the liver”12 Where have we gone from there? One very recent and intriguing paper has demonstrated the repopulation of preimmune sheep fetal liver with human hematopoietic stem cell-derived hepatocytes.13 The fetal sheep model was developed more than 10 years ago by Zanjani’s group.14 It takes advantage of recipient sheep fetuses’ immune permissivity, allowing to obtain a long-term hematopoietic chimerism after in utero injection of human HSCs into the fetal peritoneal cavity at 48–54 days of gestation. In the recent study, a direct correlation between hepatocyte formation and hematopoietic cell engraftment was found. One of the sheep exhibited up to 17% of donor human hepatocytes at 11 months posttransplant, and bone marrow of a first recipient maintained the ability to produce human hepatocytes in secondary recipients. However, these results do not rule out the hypothesis of the presence of liver-specific progenitor cells within the sorted hematopoietic stem cell population as it has been recently proposed.15 Reexamination of some bone marrow transplantation experiments has led to the conclusion that donor cells had simply fused with resident hepatocytes. Such a fusion event has been demonstrated not only in the very specific FAH-deficient mouse model, where it has been shown to occur between myelomonocytic cells and resident hepatocytes,16,17 but also in normal mice, using an elegant Cre-lox strategy.18 However, using the same approach, Harris et al.19 have recently brought some controversy to the subject, by demonstrating that epithelial cells can develop from bone marrow without cell fusion. Most of these experiments attempting to demonstrate the plasticity or the fusion of bone marrow derived cells have been performed in vivo. Only rare publications have shown the hepatocyte differentiation potentiality of human peripheral blood monocyte-derived cells20 or hematopoietic stem cells21 directly in vitro. Abstract:Firstly, the adverse events of palbociclib among Chinese patients are different from those reported in clinical trials, as a lower rate of grade 3/4 neutropenia and a higher rate of anemia and thrombocytopenia are observed. Secondly, palbociclib is more effective for front lines and endocrine sensitive patients, but less effective for patients who are primary endocrine resistant and have received chemotherapy for metastatic disease. Thirdly, palbociclib plus aromatase inhibitor and palbociclib plus fulvestrant show similar efficacy irrespective of the treatment lines or previously received aromatase inhibitor.BackgroundPalbociclib was the only available cyclin‐dependent kinase 4/6 inhibitor in China until very recently, and its effect has not been systemically evaluated among Chinese patients. This study aims to assess the efficacy, safety and patient‐reported outcomes (PROs) of palbociclib plus endocrine therapy (ET) in real‐world China.MethodsAn ambispective cohort study was conducted on patients with advanced HR+HER2− breast cancer who received palbociclib between July 2018, and November 2020 and were enrolled from 12 hospitals. Treatment patterns, survival outcomes, and safety events were documented, and PROs (European Organization for Research and Treatment of Cancer Quality of Life Questionnaire‐Core 30 items [EORTC QLQ‐C30] and EuroQoL 5 dimensions [EQ‐5D]) were analyzed. The Kaplan–Meier method was used to visualize and estimate the median progression‐free survival (mPFS). Log‐rank tests, Cox regressions, t tests, and chi‐square tests were performed for comparison.ResultsA total of 190 patients (median follow‐up of 18.0 months) were enrolled. Palbociclib was mostly combined with aromatase inhibitors (66.3%), fulvestrant (32.6%), and tamoxifen (1.1%). The mPFS values were 21.0, 14.0, and 7.0 months with palbociclib administered in first‐ (n = 83), second‐ (n = 41) and subsequent‐line settings (n = 66), respectively. Abstract:Background/Objectives: Palbociclib, an oral CDK 4/6 inhibitor, was evaluated in a Pediatric Brain Tumor Consortium (PBTC) phase 1 (NCT02255461; PBTC-042) study to treat children and young adults with recurrent, progressive, or refractory brain tumors. The objectives of this study were to characterize the palbociclib population pharmacokinetics in children enrolled on PBTC-042, to conduct a population pharmacodynamic analysis in this patient population, and to perform a simulation study to assess the role of palbociclib exposure on neutropenia and thrombocytopenia. Methods: The palbociclib population pharmacokinetics and pharmacodynamics were characterized in this patient population (n = 34 patients; 4.9–21.6 years old). Population pharmacokinetics were modeled using a one-compartment model with first-order absorption and elimination. Covariate analysis was performed, evaluating demographics, laboratory values, and concomitant medications. A pharmacodynamic model was used to describe the relation between palbociclib plasma exposure and changes in the ANC and platelet counts. Results: The population estimates for the apparent oral volume, apparent oral clearance, and absorption rate constant were 664.5 L/m2, 36.8 L/h/m2, and 0.48 h−1, respectively. The palbociclib apparent oral clearance was decreased in patients with higher AST values (p = 0.0066). The ANC and platelet pharmacodynamic models estimated that the median (5th–95th percentile) time individuals had grade 3 or greater neutropenia was 4 (0, 21) days. Simulations showed that given 75 mg/m2 palbociclib, 49% of the individuals were expected to have grade 3 or greater neutropenia. Conclusions: Palbociclib pharmacokinetics and pharmacodynamics were adequately characterized in this patient population, no unexpected adverse reactions were noted, and the drug was well tolerated.

Abstract:Background: This study aimed to reveal the treatment patterns and clinical outcomes of diverse palbociclib-based regimens in Han patients with estrogen receptor-positive (ER+) metastatic breast cancer in routine clinical practice. Methods: The clinical data of patients with ER+ metastatic breast cancer treated with palbociclib were collected from the National Cancer Center database. The efficacy profile of palbociclib in this Han population was evaluated, especially for various combination regimens. The efficacy of palbociclib-based therapy in patients with prior everolimus treatment was also assessed. Results: A total of 186 patients from 89 cities in 18 provinces in China were enrolled. The median progression-free survival (PFS) was similar among different palbociclib-combined groups (P = 0.566): 10.0 months (95% confidence interval [CI] 3.8–16.1) in the +exemestane group, 9.7 months (95% CI 6.3–13.1) in the +letrozole group, 7.8 months (95% CI 5.5–10.2) in the +fulvestrant group, 7.2 months (95% CI 3.2–11.3) in the +toremifene group, and 6.1 months (95% CI 1.2–11.0) in the +anastrozole group. Thirty-four patients (18.3%) had received everolimus for their metastatic disease before the prescription of palbociclib.
