Abstract:Introduction In every prosthetic treatment, the final purpose of the restoration is to restore function and esthetics for the patient, respecting biology and anatomical parameters. Regarding full coverage restorations, there are some factors to take into consideration: like the finish line (chamfer, shoulder, featheredge, etc.), the management of the provisional restoration and tissue healing, the material for the final restoration, and the impression technique. All these aspects will determine the treatment process and its possible results. In recent years, vertical preparation and more specifically biologically oriented preparation technique has generated great interest since its introduction in prosthodontics, changing the periodontal tissues, and providing long‐term stability. Materials and Methods With this technique, the objective is to change the position of the gingival margin, moving it more apically or coronally, depending on the modifications of the provisional. The relining of the provisional is a fundamental step of this technique: the intrasulcular part of the provisional is modified giving a new emergence profile and providing an augmentation of the tissue in the healing process. Discussion In vertical preparation, there is no predetermined margin. The dental technician should position the finish line more apically or coronally, depending on the depth of the sulcus and on the esthetic needs. Ceramic restorations should not invade the epithelial attachment, otherwise it may cause biological and mechanical problems, repetition of the impression, and of the crown restoration, which lead to increased clinical and laboratory time usage as well as expenses. One challenge is communicating the shape and contour of the temporary restoration to the dental lab to ensure accurate information for the final restoration. Conclusion Finally, the objective of this article is to describe a technique for transferring all necessary information of the provisional and obtaining a predictable result for the final restoration by using an intraoral scanner system and the fabrication of two 3D printed prototypes. One for the final volume of the crowns and the other for the finish line position. Clinical Significance The fabrication of 3D printed prototypes allows for a more predictable final restoration, reducing possible communication errors between clinician and technician when restoring with biologically oriented preparation technique. In any esthetic treatment where the restoration does not have a predetermined margin, it is possible with this additional clinical step to provide a more esthetic outcome.

Abstract:AIM To describe a digital workflow utilizing 3D printing technology to guide esthetic crown lengthening and control tooth preparation. CLINICAL CONSIDERATIONS After the initial intraoral and face scans, an esthetic treatment plan was performed digitally based on the patient's personality and face type using artificial intelligence-based 3D smile design software. A 3D-printed tray relined with silicone over a 3D-printed model was used for the mock-up. A 3D-printed guide was implemented to assist esthetic crown lengthening by incorporating, simultaneously, information about the desired free gingival line and the alveolar bone level. Based on the initial planning, a set of reduction guides was 3D printed to check and correct the tooth preparation. Prior to the start of construction of the final monolithic restorations, their design was verified using 3D-printed prototypes. A stabilization splint was digitally designed and 3D printed to protect and maintain the final result. CONCLUSIONS Technologic advances can improve the predictability of an interdisciplinary esthetic approach. Digital planning can be transferred to clinical reality using a digital workflow, utilizing a set of appropriate 3D-printed guides, which can help to control clinical procedures based on the initial planning. CLINICAL SIGNIFICANCE By following the proposed step-by-step workflow, clinicians can achieve predictable results through an interdisciplinary approach, guiding both the periodontal plastic surgery and the restorative treatment after an individualized CAD/CAD procedure for 3D-printed guides.

Digital Protocol to Record Occlusal Analysis in Prosthodontics: A Pilot Study

Abstract:Background: Digital technologies enable the accurate replication of occlusion, which is pivotal for stability in maximum intercuspation and dynamic occlusion. CAD softwares generates standardized occlusal morphologies requiring significant adjustments. The consideration of individual mandibular movements during restoration leads to better functional integration. This pilot study evaluates the efficacy of a novel, fully digital protocol for occlusal analysis recording in prosthodontics. Methods: Patients needing single or multiple metal-free restorations were included. Teeth underwent horizontal finish line preparation, while restorations on implants were either directly screwed or used multi-unit abutments. A digital impression (Trios 3 Intraoral Scanner) captured the mouth's elements. Dynamic occlusion was recorded via Patient Specific Motion (PSM). After the placement and functionalization of temporary restorations, subsequent scans included various elements, and CAD software (Dental system) was used for the restoration design. Restorations were milled in monolithic zirconia, pressed from CAD/CAM-milled wax, and sintered. Results: An evaluation of 52 restorations in 37 patients indicated high accuracy in restorations manufactured via the fully digital workflow. Monolithic zirconia was predominantly used. Subtractive (17.3%) and additive (7.7%) occlusal adjustments were mainly chairside. Conclusion: This study underscores the efficacy of meticulous verification measures and a centric contact system in reducing the need for clinical occlusal refinements in prosthetic restorations.
A new method of conclusively creating and transferring the emergence profile on single tooth immediate implant restorations: the E-merge protocol

Abstract:Purpose: The purpose of this article is to propose a new protocol for the accurate transfer of the peri-implant soft tissue emergence profile (EP) using a customized healing abutment or a provisional restoration whose shape is integrated into prosthetic design software. Report: The EP of the future restoration is created extraorally with an ideally shaped customized healing abutment or a provisional crown. Before insertion, the latter is screwed into a repositionable analog placed on a specially designed device and scanned with an intraoral scanner (IOS). This repositionable analog (Straumann RB Repositionable Implant Analog, Art. no. 065.0023), whose original purpose is to keep the correct orientation of the implant platform when placed in a printed model, is already available on the market. It is an exact replica of the implant with a corresponding external index. When scanned, it allows the digital transfer of the exact implant position and index. After healing, a full-mouth impression is made with an IOS without removing the customized healing abutment or provisional restoration. The STL files issued from both impressions are merged to produce a digital model used to design the final restoration, which can be inserted at the next appointment. Conclusion: The technique described is simple, predictable, and precise. It does not require a traditional implant level impression or the use of a digital accessory such as a scanbody to transfer the 3D implant position and the peri-implant gingival contour.

Abstract:Aim: The aim of the present study was to demonstrate that fully digital workflows can provide predictable esthetic and functional outcomes. Materials and methods: The present clinical case report provides a step-by-step documentation of a full-mouth rehabilitation planned and performed with a fully digital approach utilizing an adhesive and no-prep method. After assessing the patient's needs, a treatment plan was established that considered the patient's functional and esthetic requests. The overlap of 2D images and 3D models and facial scans of the patient allowed the digital previsualization of the esthetic result by implementing the "copy-paste" approach to restore the maxillary anterior sextant. Conclusion: The final outcome was satisfactory in terms of esthetics, function, and soft tissue health.

Abstract:The United Kingdom and Australia: New Titles The last few years have seen an upsurge of interest in medical ethics outside North America, where the subject is already well established. In 1987, a number of significant publications, reflected this change. In Britain, a Working Party from the Institute of Medical Ethics produced the "Pond Report" on undergraduate teaching of medical ethics in medical schools. [1] This authoritative document has been widely read and discussed and, although the standards of ethics courses very widely among medical schools, they are now at least accepted as a formal part of the curriculum. These and other initiatives, notably the development of three postgraduate masters degree programs, are described in a symposium on teaching medical ethics teatured in the autumn issue of the Journal of Medical Ethics. [2] In November the British government published a long-awaited white paper on "Human Fertilisation and Embryology: A Framework for Legislation," following the Warnock Report published in 1984. [3] This document, which willform the basis for a new bill following debate in Parliament, contains two main sets of proposals. The first concerns an independent Statutory Licensing Authority to be set up to control certain types of treatment for infertility where serious ethical issues arise, and human embryo research if it is decided that such research should be made lawful. The second concerns surrogacy; it is proposed that no surrogacy contract should be legally enforceable, although criminal sanctions will not apply to private surrogacy contracts. This document heralds the first wide-ranging piece of legislation in this area in Britain. The British Medical Association's recent report on euthanasia will be widely considered in Britain and abroad although it contains little that is new and fails to deal satisfactorily with many of the issues that trouble doctors and patients. [4] It considers active euthanasia wrong both morally and legally, but does not present the arguments coherently and dispassionately. Indeed, ethical reasoning seems to be introduced to support particular positions already held. Much of the problem may have arisen because the membership of the working party was composed entirely of medical professionals (apart from observers); this serves to warn even the most prestigious groups working in medical ethics against having too narrow a composition. We can hope that the failure of this report to provide a lead in this contentious area will prove a stimulus to others in tackling the questions that remain unresolved. In Australia, a major new international journal, Bioethics, was launched in 1987, edited by Helga Kuhse and Peter Singer. It was a year in which the moral issues related to AIDS reached a new height of prominence, so it is not surprising that Bioethics contained two articles on the subject in its first volume. One of them, a review of the ethical and practical issues, came from Finland and revealed the depth of terror the subject engenders. [5] In an otherwise sober and analytic account, as would be expected from philosophers, the authors conclude with what would be regarded as a hysterical statement in almost any other context: "Nuclear holocaust, the main source of fear among people today, will tomorrow look like the only peaceful way out of our misery if governments do not care to stop the triumphant march of AIDS now." Digital Planning to Enhance Diagnosis and Precision in Correcting Excessive Gingival Display in the Presence of Asymmetrical Maxillary Position: A Case Report

Abstract:This case report addresses the treatment of excessive gingival display (EGD) in the context of maxillary asymmetry, zenith irregularities, and occlusal plane inclination. Digital planning was pivotal in formulating a precise treatment strategy by incorporating facial photographs, digital models, and cone-beam computed tomography data. Parameters, including occlusal plane inclination, teeth position, and lip-to-gingival margin relationships, were considered to ensure treatment alignment with the patient's facial characteristics. Notably, during the planning phase, it was evident that the conventional approach using the cementoenamel junction as the apical limit for incisions would result in asymmetry. Consequently, the gingival margin position was defined in accordance with facial and lip features. The chosen treatment, flapless crown lengthening, was tailored to the patient's thin phenotype and guided by measurements derived from digital planning. Postsurgery, the patient experienced a swift and painless recovery. A harmonious smile with a stable gingival margin position was achieved at the 1-year follow-up, seamlessly complementing the patient's facial attributes. This case underscores the importance of personalized EGD treatment and the value of digital planning in enhancing diagnostic accuracy and precise treatment planning, ultimately facilitating optimal treatment strategies.
Computer-assisted prosthetic planning and implant design with integrated digital bite registration: a treatment protocol

Abstract:The aim of this clinical study is to present an integrated digital project through the description of a clinical case, made entirely in digitized form, taking advantage of the opportunity offered by instrumental diagnostic software. A case report participant is a 65-year-old female patient presents with loss of diffuse bone support, caused by periodontal disease. After a sign of an informed consent and an explication of a plan of treatment, technical intraoral and extraoral pictures and intraoral digital impressions were taken. The digital images improved from the 2D Smile Lynx Software and the scanner stereolithographic (STL) file was matched into the CAD Lynx to obtain a virtual previsualization of teeth and smile design, and to mill the provisional and the definitive crowns. The digital prosthetic design allows the evaluation of the dental parameters in relation to the parameters of the patient's face for the new prosthetic project and the radiological examination using CBCT guides the insertion of the fixtures for the rehabilitation phase. The surgical and prosthetic design are subsequently integrated. The evaluation of the bone bases is carried out with a radiological diagnostic software for CT (Real Guide 5.0-3Diemme, Cantù-Italy) which can virtually design the implant insertion. The functional examination of the patient is carried out through an occlusion-postural examination that uses digital electromyographic assessments. The integrated digital protocol proposal inserts in the rehabilitation path the digital recording of the free mandibular movement, as well as the scan of the patient's face, data that will be integrated into the CAD software for the design of temporary and definitive prosthetic artifacts, made using the CAM method. This study showed guided implant placement and the application of fixed implant-supported prosthetic restorations carried out with a fully digital workflow, dependent on the functional digital evaluation of the patient's occlusion. The proposed protocol described the correct use of digitalization of clinical, surgical, and prosthetic procedures, and the matching of the data into a computerized environment, to improve team communication and to take advantage of the combination of collected data to not lose information using classic manual steps.

Abstract:The emergence profile in implant restoration has an important contribution to the final esthetic result. Moreover, a properly shaped emergence profile enables well-performed dental hygiene and protects the implant-restoration complex from peri-implant infections. This report describes a clinical case that combines digital workflow and designing with a custom implant healing abutment system (Cervico). This system is utilized to customize the provisional restoration's emergence profile with the conventional processing along with the digitally designed and milled provisional restoration to deliver an interim prosthesis after implant placement. A simplified digital workflow for the prosthetic finishing of implant rehabilitations: a case report

Abstract:The aim of this article is to describe how, during the provisional and definitive prosthetic phases, using new digital technologies, it is possible to improve the ergonomics of the prosthetist's work and reduce the discomfort of patients, subjecting them to the fewest possible appointments at the dentist. The proposal of a full digital protocol, described by the following case report, for the realization of a definitive prosthetic rehabilitation supported by a reduced number of implants, in fact, allows to considerably reduce the number of appointments and reduce any bias. A 67-year-old male patient presents for the first visit to the Department of Dentistry of the San Raffaele Hospital, wearing a removable upper prosthesis and with the request to heal the aesthetic and functional situation through prosthetics fixed. An initial panoramic radiograph was performed, intra and extra oral photos were taken and also intraoral impressions. A stereo-lithographic models are obtained from intraoral scans, and two total prostheses, upper and lower, were packaged for the provisional post-surgical phase was performed. In accordance with the All-on-4 method 8 implant fixtures were placed. For the final prosthetic phase, the patient underwent only two operative sessions. In the first session, scans were taken with the provisionals in situ, of the patient's mucous membranes and with the Scan-abutments in place. In the second session using specific CADSoftware the matching of the STL files of the three scans were created, the opposing arches of the patient were related on a digital articulator, and the milled titanium bars were immediately constructed and finished with the resin. Finally, the definitive prostheses were delivered to the patient without any other test. Digital technology has allowed a clear reduction in working times and costs and has allowed the reduction of stress for patients who undergo invasive and extensive treatments to recover aesthetics and function, and for clinicians who must manage complex cases with fewer appointments possible.

Abstract:Aim: To report on a novel digital superimposition workflow that enables measuring the supra-crestal peri-implant soft tissue dimensions all along implant treatment and afterwards. Materials and methods: A preoperative CBCT and intra-oral scans (IOS) are successively taken before surgery, at the end of the healing period, at prosthesis delivery, and over time; they are digitally superposed on a dedicated software. Then, the stereolithography files (STL) of the healing abutment, of the prosthetic abutment and the crown are successively merged into the superposition set of IOSs. Result: The workflow protocol of merging successively the STL of each item into the superposition set of IOSs enables capturing the dimensions of the height and width of the supra-crestal soft tissues, at every level of the healing abutment, the prosthetic abutment and the crown. In addition, it allows measuring the vertical distance that the crown exerts pressure on the gingiva and the thickness of the papillae at every level of the abutment. Conclusion: This novel digital superimposition workflow provides a straightforward method of measuring the vertical and horizontal dimensions of the supra-crestal peri-implant soft tissues, including the papillae, at each stage of the implant treatment process. It allows investigating a certain number of soft tissue variables that were previously inaccessible to clinical research. It should help enhancing our comprehension of the peri-implant soft tissue dynamics.

Abstract:This report describes a digital workflow for the fabrication of a complete maxillary denture opposing an implant-retained overdenture. This procedure provides a predictable and accurate 3-visit technique to digitally scan and articulate edentulous ridges, avoiding the discomfort associated with analog impression making. Wax rims were applied to intraorally relined plastic edentulous stock trays. The occlusal relationship was registered at the appropriate vertical dimension of occlusion by using these tray and rim assemblies. The digital laboratory scanning of the relined tray and rim assemblies resulted in accurate virtual articulation of the digital edentulous intraoral scans. The removable restorations were digitally designed, milled, and delivered, and the clinical and laboratory steps are described.

Abstract:Objective This clinical case describes a digital workflow using face scans and CAD/CAM technology for a full‐mouth rehabilitation with increased vertical dimension of occlusion (VDO) with adhesive lithium disilicate restorations after 9 years. Clinical Considerations A healthy, 46‐year‐old man displaying severe tooth wear, underwent an extensive full‐mouth rehabilitation involving an increase of the VDO through laminate veneers and adhesive partial coverage lithium disilicate restorations. Anatomical landmarks of the face and reference planes were captured using a digital face scanning system. The anatomical position of the maxilla was registered with a transfer device. Digital data sets of the intraoral situation combined with a facial scan enabled precise virtual planning, guiding minimally invasive preparations. Long‐term provisional restorations, milled from high‐performance polycarbonate, were used to test the novel VDO before the final lithium disilicate restorations were fabricated. This approach provided a time‐ and cost‐efficient treatment solution. No failures were observed at the 9‐year follow‐up. Conclusions Utilizing face scans and design software enable a virtual visualization and comprehensive quality control for patients with dental wear, resulting in significant time savings and increased predictability for the dental technician, dentist and patient. Clinical Significance The described digital workflow optimizes the planning and implementation of a comprehensive adhesive full‐arch rehabilitation with an increase of the VDO. The clinical long‐term follow up result of CAD/CAM assisted minimally invasive lithium disilicate restorations after 9 years demonstrates to clinicians a reliable treatment concept for patients with dental wear.

Full digital workflow for implant-prosthetic rehabilitations: a case report

Abstract:Objectives: The aim of the present study was to describe a proof of concept digital workflow for the implant-prosthetic treatment in partially edentate patients and evaluate its clinical performance. Methods: A 55-year-old woman with a Kennedy class III bilateral edentulism in the mandible has been selected for a computer guided implantology according with the Smart Fusion® protocol (NobelBiocare, Kloten, Switzerland). After the template-guided implant placement, an immediate loading of the implants has been performed. QuickTemp conical temporary abutments were selected to deliver immediately prefabricated cement-retained provisionals. Two months after an impression was taken with an intraoral digital scanner (3Shape Trios3, Copenhagen, Denmark). The provisional restoration optical scanning was used to transfer the functionalized prosthetic contour to be duplicated into the definitive porcelain fused to zirconia restoration cemented on customized titanium abutments. Results: A satisfying esthetic and functional result has been achieved. No biological and mechanical complications were recorded. Conclusions: The investigated fully digital implant-prosthetic protocol provided a smooth, complication free and time effective treatment alternative to the conventional workflow. Besides the fully digital workflow allowed the surgical and prosthetic decision making and the communication within the dental team and with the patient. Further improvements heading to a direct match between the intraoral scan and the CBCT are strongly advised in order to create the so-called virtual patient.
Digital planning protocol for functional and esthetic prosthetic treatment

Abstract:Aim: The aim of the present study was to present the different stages of prosthetic treatment planning involved in the design of an esthetic smile and the improvement of masticatory function using CAD/CAM technology. Materials and methods: The patient underwent the following tests and procedures: CBCT (CS 9300; Carestream), intraoral scans and occlusal detection (CS 3600; Carestream), a photographic portrait session (Nikon D610; Nikon), a face scan (Bellus 3D FaceApp; iPhone XS, Apple), and registration of individual temporomandibular joint (TMJ) angles and mandibular movements with a Zebris for Ceramill device (Amman Girrbach). All the data were transferred to Ceramill Mind software (Amman Girrbach) where they were integrated. The face scan and photographs were superimposed on the CBCT scans. Scans of the dental arches were combined with the CBCT scans. On this CBCT base, the position of the condyles in the articular fossae was determined. A Virtual Artex CR virtual articulator (Amann Girrbach) was attached to the 3D object. Individual TMJ angles and mandibular movements were then introduced. Results: A virtual patient was created in the Ceramill Mind software. The optimal shape and position of each tooth were designed into the program. The wax-up was printed using a 3D printer and a temporary mock-up, and final restorations were made for the patient. At the same time, the esthetics of the smile was improved, and a harmonious central occlusion and articulation were obtained on virtual models and in the patient's oral cavity. Conclusion: The presented digital planning protocol allows the working out of an optimal solution in complicated patient cases from both the functional and esthetic points of view. Advanced digital planning approach using a 3D-printed mock-up. A case report

Abstract:Objective: A diagnostic mock-up is a key tool that allows a preview of the outcome of an esthetic restoration. With recent developments in CAD/CAM technology, it is important to understand the pros and cons of chairside digital dentistry and the restorative materials used. The aim of the present case report is to describe in detail the use of a 3D-printed mock-up fabricated from a polymer-based material for an esthetic treatment plan within a fully digital workflow. Case report: A 45-year-old female patient presented at the clinic concerned about her esthetic appearance and the color of her anterior incisors. After a conclusive diagnosis, a restoration was planned using ceramic veneers from maxillary premolar to premolar. For a preview visualization of the outcome, an intraoral scanner was used to obtain 3D images and to allow the design of a digital smile. The template STL file was exported to a 3D printer and a 0.6-mm mock-up in A3-shade 3D resin was produced after 25 min. The mock-up was tested through a try-in and approved by the patient. As a result, the printed mock-up was considered predictable and reliable for the final restoration. Conclusions: The ease, speed, and reduced costs derived from the digital workflow, in conjunction with the accuracy of the mock-up, made the procedure highly efficient and recommendable.
Digital Interactive Design and Robot-Assisted Preparation Experiment of Tooth Veneer Preparation: An in Vitro Proof-of-Concept

Abstract:In the fixed prosthesis process dental prosthesis treatment, the dentist needs to conceive the morphology of the tooth preparation and make numerous micro-adjustments by manual operation. This study aims to establish a digital design system for accurate designing of veneer preparation and study the trajectory planning of the robot-assisted preparation. According to the standard operation requirements, the projection line segmentation method based on point drawing is proposed for interactive selection to ensure the margin smoothness of the preparation area. The preparation depth is parameterized, and the foldback point correction method based on secondary segmentation is used for repairing the offset preparation area to ensure surface smoothness. The fenestration preparation model was obtained in the digital interactive design software. The model is divided into two feature areas which are interpolated by the B-spline curve and NURBS surface, respectively. The isoparametric curve method is used to plan the preparation trajectory. The experimental and theoretical values of the target feature points from the X, Y and Z coordinate directions were also calculated and analyzed. The experiment is carried out by the physical prototype of the tooth preparation robot. The maximum error of the feature points on the preparation are 0.180 mm, 0.104 mm and 0.142 mm in X, Y and Z directions, respectively. The confidence interval of the system error is stable within 0.30 mm. The error of the feature points verifies the feasibility of the digital interactive design system and the correctness of the trajectory planning of tooth veneer preparation. The system could have the potential to assist dentists in the precise design of the veneer preparation, improve the processing efficiency and ensure the effect of tooth veneer prosthesis.

Abstract:Computer-aided design and computer-aided manufacturing (CAD/CAM) dentistry have significantly changed workflows in recent years. Restorations and devices can now be digitally designed and 3D-printed for dental care purposes. This clinical case report provides straightforward protocols for the digital design and 3D manufacture of gingivectomy and tooth preparation guides. These types of guides improved the gingival architecture of the anterior teeth and provided controllable tooth preparations prior to labial ceramic veneers. Thoughtful clinical evaluation started with listening to the patient's chief complaint and extra- and intra-oral evaluations. Then a digital wax-up was performed, followed by an intra-oral mock-up, to evaluate the shape of the proposed restorations. After patient acceptance, the clinical procedure started with the gingivectomy and tooth preparation. Hand-crafted porcelain veneers were bonded under rubber dam isolation to avoid any contamination and maximize the bonding protocol. The esthetic and functional demands were fully satisfied. Predictable outcomes can be obtained whenever a meticulous evaluation and execution of all the steps are performed. Three dimensional printing technology allows the fabrication of devices such as gingivectomy and tooth reduction guides that help accomplish the desired results.

Abstract:This article describes a case with a full digital procedure to prefabricate an implant-supported interim restoration based on the preoperative digital implant planning. A fully guided surgical template is designed and printed, and then an interim restoration is fabricated based on the planned implant position through a dental computer-aided design (CAD) software. Once the implant was placed at the predetermined position through the fully guided surgical guide, the prefabricated interim restoration could be inserted immediately after the surgery, which can guide the healing of the soft tissue and enhance the esthetic outcomes. This novel technique eliminates the conventional impression making to insert an implant-supported interim restoration immediately after the implant placement surgery, which can guide the healing of the soft tissue, minimize the chairside time and optimize the clinical workflow.
