Abstract:The day before yesterday: the physician-philologist. The times of the first appearance of printed books and of the re-discovery of ancient texts are also the times of syphilis. The physician, more than a philosopher as Galen suggested, is also a philologist. A very interesting example is fournished by Antonio Benivieni (+1502) and by his book De causis abditis (1506). Yesterday: the philologist-physician. Charles Daremberg (1816-1872) is our example. In 1841, he wrote his medical thesis Exposition des connaissances de Galien sur l'anatomie, la physiologie et la pathologie du systeme nerveux (faculte de medecine de Paris, n 222). Today: a necessary couple, the physician and the philologist. Greek and Latin are no more subjects of teaching in school, and the philologists seem to be strange, dusty and dirty people. Is the physician-philologist still alive or not? To be useful to the daily practice of medicine, history of medicine will be also a philological matter, as philology is the complete study of ancient, modern and contemporary texts. It teaches to read well, to understand all that has been written and, as a consequence, all that the patients tell today to their physicians. The problem is not teaching something more (new facts, new notions...) but teaching to know well; in a word, teaching the famous continuous learning, which is still a difficult reality.

Abstract:The doctor has been from de Antiquity a victim of the people's affective ambivalence, who loves and hates him accord his success or failures. Some rant against the medical guild, but are proud to be patients of one or another doctor, more o less celebrated, referring familiarly to him as Tony or Jim. A certain envy hurts the doctor's image, countered by gratitude, but finally, as times goes by, the continuous progress of medicine has been improving the public perception about the medical work. A quick review of the Modern Age literature, personal, whimsical and may be imperfect, from Michel de Montaigne, in the sixteenth century until. A. J. Cronin in the twentieth, shows the evolution of the doctor as a literary character, first as a tricky ignorant, after like a clown, later fighting epidemics and ending as a self-sacrificing medical researcher.

Abstract:This article explores the links between chymical medicine, charity, and vocation in the writings and careers of Henri Rousseau de Montbazon and Nicolas Aignan, known as "the Louvre Capuchins" (les capucins du Louvre) because they operated a royally sponsored medical laboratory at the Louvre from 1678 to 1679. It shows that Rousseau and Aignan's hybrid persona as chymical physicians and mendicant friars allowed them to leverage courtly values surrounding charitable poor relief into lucrative patronage under Louis XIV. Aignan in particular developed a detailed theological and natural philosophical defence of this identity, framing it as a recovery of the authentic vocation of the Christian priest-physician, a healer of bodies as well as souls. By the end of the seventeenth century, however, I argue that Rousseau and Aignan's attempts to reconcile their identities as both friars and healers into a single persona were increasingly challenged by the medical establishment and their own order.

Abstract:Much has been written about the dilemma of the physician-scientist, mostly lamenting their disappearance owing to the long training requirements, student debt, family responsibilities, and reduced federal funding, not to mention the challenge of wearing both hats, and wearing them well. Here, I offer a personal perspective, not an analysis, which has been ably done by others, and not a solution really, except perhaps that my experience might provide encouragement for some considering, or perhaps already on, this difficult but very rewarding career path.

Abstract:This essay examines the medical debates over hereditary disease and moral hygiene in France between 1748 and 1790. During this time, which was marked by two formal academic exchanges about pathological inheritance, doctors critically studied the existence of hereditary diseases--including syphilis, arthritis, phthisis, scrofula, rickets, gout, stones, epilepsy, and insanity--and the problems that heredity might pose for curing and preventing these diseases. Amid public debate, doctors first treated heredity with formal skepticism and then embraced the idea. Their changing attitudes stemmed less from epistemological or cognitive reasons than from new cultural beliefs about gender, domesticity, and demographic policy. Fearing moral degeneracy and demographic decline, they argued that a number of social pathologies were truly hereditary and that these diseases spread within the family itself. These beliefs were seemingly confirmed by new clinical studies on tuberculosis. Though doctors conceded that hereditary diseases might limit Enlightenment hopes to perfect society, they also suggested that sexual hygiene and physical education could cure hereditary degeneracy and transcend genealogy and descent. Consequently, they stressed that physical regeneration was a dynamic process, one that stretched from the conjugal bed to weaning and beyond. Rather than accepting the accidents of birth, physicians believed that their patients could self-consciously overcome inherited defects and thus regenerate themselves and even all of society itself. Heredity thus gave doctors an idiom with which to diagnose a felt social crisis and to prescribe appropriate hygienic responses.

Abstract:The practice of medicine as we have known it is rapidly becoming a "dinosaur" giving way to more complex business and legal relationships among physicians. Quality medical care, however, does not have to become obsolete. As physicians begin to collaborate with each other to survive and thrive in an increasingly competitive environment, their collective influence over payors and other guardians of the cost of health care should focus on ensuring the preservation of quality medical care. By minimizing the adverse consequences of administrative burdens on individual practices, physicians should be able to maximize their practice efficiency and earning potential while preserving their goal of providing quality medical care.

Since its establishment in 1847, the American medical association (AMA) defined the duty of physicians in times of crisis as follows: ' when an epidemic prevails, a physician must continue his labors without regard to the risk to his own health ' [ Citation 3 ]. In other words, physicians should commit themselves even at the risk of their own lives. Nevertheless, does a physician have the right to withdraw? Does he legally have the freedom not to die?

Abstract:Our aim is to determine whether the disulfide bonds of alpha-lactalbumin account for the lack of cooperative folding behavior reported for some molten globule variants, in contrast to the highly cooperative folding reported for the pH 4 molten globule of apomyoglobin. Two different alpha-lactalbumin genetic constructs are studied: [28-111], which has a single disulfide bond connecting two segments of the alpha-helix domain, and [all-Ala], which has no disulfide bonds. The superposition test used earlier to probe for cooperative folding of the apomyoglobin molten globule is used to determine whether there is an important difference in folding cooperativity between the molten globules of [28-111] and [all-Ala]. The [all-Ala] construct behaves in the same manner as the apomyoglobin molten globule: its folding satisfies the superposition test in the three sets of anion conditions studied, and anions stabilize it against urea unfolding. The [28-111] construct behaves differently in both respects: the folding of its molten globule does not satisfy the superposition test in two of the three sets of anion conditions, and anions barely affect its stability. The 28-111 disulfide bond stabilizes the molten globule substantially, as expected from earlier work. Comparison of the unfolding transition curves monitored by circular dichroism also demonstrates that [28-111] folds in a less cooperative manner than [all-Ala]: the unfolding curve of [28-111] is significantly broader. Moreover, the unfolding curves indicate that [28-111] has a lower helix content than [all-Ala]. Consequently, the 28-111 bond constrains the folding behavior of the molten globule and weakens its cooperativity of folding.

Abstract:Abstract Context The shortage of general practitioners (GPs) is a growing concern in Europe, especially in France. This problem is likely to continue until the end of the 2020s. Objectives To study the GPs’ perceptions of access to care in medically underserved areas (i.e. with low physician density), its consequences on their working conditions, and how they cope with the resulting difficulties. Methods Semi-structured individual interviews were conducted between May and August 2021 of 29 GPs practising in areas of southeastern France with a low physician density or at risk of a doctor shortage. Purposive sampling was used to include profiles of diverse physicians and diverse rural and urban areas. The interviews, conducted with an interview guide, were transcribed and analysed thematically. Results The participants described a serious degradation of access to care in their areas. These issues also concerned urban areas, where they were, according to the participants, underrecognized. The participants’ workloads were rising, at a rate often perceived as unsustainable: many participants, including the youngest group, reported they were exhausted. Their principal source of dissatisfaction was their impression that they could not do their work correctly. Participants reported that these difficulties required them to improvise and adapt without any official or formal method to keep their practice manageable. Conclusion These GPs were worried about the future of their profession and their patients. They expected strong measures by public policymakers and officials, but paradoxically seemed to have little interest in the solutions these officials are promoting.

Abstract:This paper considers the design of TCP-like congestion control protocols for deployment in high-speed communication networks. A basic problem in this area is to design congestion control strategies that probe more aggressively than standard TCP, but which co-exist with each other and result in globally stable and equitable network behaviour. Under the assumption of player synchronisation, conditions for global network stability are derived in the form of growth conditions on the local nonlinear probing functions.

Abstract:Background. General practitioners (GPs) face quantitative and qualitative changes in patient demand and doctor shortages. Objectives. To investigate how GPs cope with doctor shortage issues. Materials and methods. Two cross-sectional surveys of a representative panel of 1530 GPs in 2019 and 2022 about their perceptions of physician shortages, working hours worked (WHW), and adaptive behaviors. Hierarchical clustering enabled identification of profiles with different adaptation patterns. Multiple Poisson or logistic regression models studied associations between GPs' profiles and professional characteristics. Results 87.4% of GPs applied at least one adaptation to control patients' healthcare demand. 24% adopted task-shifting while their average WHW decreased by 3.6 hours between 2019 and 2022. Four GP profiles were identified. "Low adapters/low workload" and "Low adapters/high workload" (25% of the sample each) reported 2.4 adaptive measures: 75.5% refused to be new patients' preferred doctor in the former group (vs 5.1%). "High adapters/unchanged consultations" (30.7%) and "High adapters/shortened consultations" (18.9%) reported 4.8 and 6.1 adaptations, respectively. They were more likely to practice in medically underserved areas. Conclusion These results call into question GPs' gatekeeper role in the French healthcare system. Moreover, the marked reduction in WHW in underserved areas is likely to exacerbate their uneven distribution nationwide. Encouraging vertical integration between HCPs while enhancing cooperation and task-shifting is probably a pathway toward improving the relative GP shortage.

Abstract:The United States is in the midst of the second revolution in American health care to occur during this century, as Kenneth Ludmerer makes clear in his book Time to Heal: American Medical Education from the Turn of the Century to the Era of Managed Care. The "Flexnerian revolution" eventually led to the closing of a third of the medical schools. Although such closures are not likely this time, familiar arrangements are collapsing, without a clear picture of the shape of things to come. Whatever the outcome of the current revolution, well-trained physicians will be needed to care for the sick. Academic medical centers truly are at risk and increasingly require public support to flourish or even to survive, but medical schools and their teaching hospitals must demonstrate that they deserve this support. These institutions have responded by focusing on the business aspects of medicine, perhaps to the detriment of medical education. Lost in this focus is teaching time, and perhaps even more important, the time for mentoring. Often lacking too is a clear vision of the preparation needed by the student to practice medicine successfully in the future: different specialty mixes, interdisciplinary group practice; vastly increased use of information technologies, and overwhelming amounts of relevant and interrelated information. Yet the answer is the same as it was 75 years ago when Yale introduced the first radical medical curricular reform--the "liberal arts physician," trained in science, the values of medicine, and particularly for uncertainly and with the capacity to adapt.

Abstract:This article examines the place that has been reserved for medicine in the historiography of the sciences. More precisely, it focuses on the motifs that have lead historians of science to grant only a minor role to medicine within the movement commonly designated by the notion of the "scientific revolution". Among those motifs, the persistent and late application of teleological schemas in the thinking of the biological and the difficulties in "mathematizing" anatomy are often invoked. Starting with an overview of the critical literature on the topic, this bibliographical essay shows how the situation has changed over the last decades. The opposition between, on the one hand, the physical sciences founded on a model of mechanistic explanation of nature and, on the other hand, the life sciences that remained guided by a finalist mode of thinking are today much put into question. What we find today is more open reflection on the diversity of "models" for understanding the living, and on how to integrate them into more complex schemas than those that simply oppose mechanism and teleology. The essay is finally based on discussions and debates among medical doctors and philosophers in the modern period, and insists on the importance of studying this "medico-philosophical" tradition in order to avoid reconstructing a posteriori a mythical history that trends to consecrate a single model of rationality.

We are versatile and broad in both professional gaze and skill set, celebrated in Yonder by Ahmed Rashid, who is approaching his 10th anniversary writing the column! Yet, this issue's Life & Times articles describe the failure of society and policymakers to value general practice. I use value in two distinct ways: to value is to understand and appreciate both the beauty and appropriate uses of a thing; it also has a sense of quantitate weighing: to value is to attribute worth, usually in the form of money! Our articles also highlight that the care that is espoused in the RCGP motto, Cum Scientia Caritas . Simon Morgan relates the experience of new GP registrars in Australia to the survivalist gameshow 'Alone'. New GP registrars face a range of similar challenges as their TV survivalist counterparts: isolation, uncertainty, a steep learning curve, and genuine fear. Tim Senior takes the survival challenge further. Not only is general practice already complex, fraught with professional peril, underappreciated, undersupported, and underfunded, but it has to cope with society's failure to adequately resource and support other hospital and community ...

[The beginnings of the nursing profession : the complementary relationship between secular caregivers and hospital nuns in France in the 17th and 18th centuries]

Abstract:The words used for designating the caregivers are ambiguous. Little by little, the word "nurse" becomes widely used, mainly in the feminine form due to the need of specialized staff. Health care structures are developing in the 17th and 18 centuries, the remains of which you can find in today hospitals (Salpêtrière hospital, Hôtel-Dieu hospital in Paris). The government of Louis XIV cares for the poor sick people, the vagabonds and the beggars. It opens new general hospitals as it will be the case later in all Europe. In the 17th century, the staff of the general hospital in Paris is entirely secular. The Paris general hospital is headed by the magistrates of Paris Parliament. The healthcare institutions employ both secular and religious staff for example the Hotel Dieu in Paris and the one in Marseilles. In the 17th century, there are 2000 secular caregivers in France. The order of the "Filles de la Charité" (grey sisters) is not submitted to the rule of enclosure. They renew their vows every year. For their founders Vincent de Paul and Louise de Marcillac, their monastery should be the cells of the sick, their cloister should be the rooms of the hospitals or the streets of the town. The secular or religious caregivers are excellent in the apothecary and they open a network of small dispensaries. It improves the health of the French population and allows fighting against the epidemics. This activity allowed some women to have a rewarding activity and a social status of which they were apparently satisfied.

Abstract:Searching for one word to describe the state of mind of the physician in the United States today, we might choose beleaguered. Threats appear from all sides--from payers, would-be managers of care, the growth of technology, and even patients. The rhetoric is one of siege and battle, and the dynamic seems to be a clash of values from which only one winner can emerge. But scientific and health services research suggest otherwise. Science suggests that health care could, indeed, perform a great deal better than it does today and that a shared aim of improving health outcomes for patients at a cost that society can afford is sensible and within reach. However, achievement of these improvements will require of physicians not handwringing and resistance to change but concerted, positive, capable leadership. The goal of this series in Annals is to describe a new knowledge base that will help physicians participate effectively in the redesign of the health care system. The series is intended to raise the curiosity of physicians about the skills they will need to become more active and influential citizens of the health care community in accomplishing improvements. These skills will help physicians better deploy their clinical expertise and professional purpose in a debate that has heretofore been informed primarily by economics.
