Abstract:Carotid artery webs (CaW) are non-atherosclerotic projections into the vascular lumen and have been linked to up to one-third of cryptogenic strokes in younger patients. Determining how CaW affects local hemodynamics is essential for understanding clot formation and stroke risk. Computational fluid dynamics simulations were used to investigate patient-specific hemodynamics in carotid artery bifurcations with CaW, bifurcations with atherosclerotic lesions having a similar degree of lumen narrowing, and with healthy carotid bifurcations. Simulations were conducted using segmented computed tomography angiography geometries with inlet boundary conditions extracted from 2D phase contrast MRI scans. The study included carotid bifurcations with CaW (n = 13), mild atherosclerosis (n = 7), and healthy bifurcation geometries (n = 6). Hemodynamic parameters associated with vascular dysfunction and clot formation, including shear rate, oscillatory shear index (OSI), low velocity, and flow stasis were calculated and compared between the subject groups. Patients with CaW had significantly larger regions containing low shear rate, high OSI, low velocity, and flow stasis in comparison to subjects with mild atherosclerosis or normal bifurcations. These abnormal hemodynamic metrics in patients with CaW are associated with clot formation and vascular dysfunction and suggest that hemodynamic assessment may be a tool to assess stroke risk in these patients.

Abstract:Background and purpose A carotid web is a thin, shelf-like luminal protrusion in the internal carotid artery that might cause carotid stenosis and stroke by inducing disturbed flow patterns, thrombosis, and abnormal biomechanical stimulus to the endothelial cells. This study simulated and evaluated how the two main treatments (endarterectomy and stenting) influence hemodynamic environments in the carotid artery and distal carotid siphon arteries, aiming to provide more references for the selection of clinical treatment. Materials and methods The carotid web, endarterectomy, and stenting models were reconstructed based on CT images. The blood flow simulations were conducted, and critical parameters related to thrombosis formation and artery remodeling, including swirling strength, wall shear stress (WSS), vortex Q-criterion, and oscillating shear index (OSI), were analyzed. Results In the model of the carotid web, obvious recirculation formed distal to the web, accompanied by lower velocity, lower WSS, higher relative resident time (RRT), and higher Q value. While in both two treatment models, the velocity increased and the Q value and RRT decreased at the carotid bifurcation. In addition, both treatments provide more kinetic energy to the distal carotid siphon artery, especially the stenting model. Conclusion The carotid web can significantly influence the flow environments in the carotid artery. Both endarterectomy and stenting treatments could significantly diminish the side effects of the web and are feasible choices for web patients in terms of hemodynamics. Besides, the treatments for the carotid web would also influence the flow patterns at the distal carotid siphon, especially for the stenting treatment. But more innovational designs are needed to make the minimally invasive stenting treatment more beneficial.

Abstract:Internal carotid artery (CA) stenosis is a primary etiological factor for stroke and transient ischemic attack. The severity of arterial stenosis significantly impacts patient health and treatment decisions. Therefore, we conducted computational fluid dynamics analyses on five carotid arteries (CAs) of severe stenosis and compared them with five CAs in the control group. We improved the three-element Windkessel model method by pre-calculating the constant-pressure outlet simulation of the first cardiac cycle, which accelerated the stability of the model. The research results show that vortices were observed at the bifurcation of the CAs in the control group, whereas in the severe stenosis group, vortices predominantly occurred within the carotid sinus downstream of the stenotic segment. Notably, the vortex flow in the carotid aneurysm downstream of the stenotic segment arises due to the cross-sectional constriction induced by stenosis, which always flows in a clockwise direction and may contribute to the formation of aneurysms distal to the stenotic region. A high time-averaged wall shear stress value can effectively identify the stenosis site of CAs, while a high relative residence time value marks the protrusion near the stenosis segment. This study delved into the hemodynamic parameters between the CAs of the severe stenosis group and the control group and provided robust clinical evidence for carotid atherosclerotic disease.

Abstract:Background and Objectives : In clinical practice a large number of patients with ischemic stroke have internal carotid artery (ICA) stenosis accompanied by Circle of Willis (CoW) stenosis. In the presence of carotid artery stenosis, CoW atherosclerosis may cause cerebral blood flow decompensation and may promote the development of ischemic stroke. The reason for the concomitant stenosis at both sites is unknown. This study investigated the hemodynamic effects of ICA stenosis on the CoW. Methods : We developed a three-dimensional/zero-dimensional (3D/0D) closed-loop geometric multiscale model of the cerebral artery to quantify the hemodynamic indicators, including time-averaged wall shear stress (TAWSS) and oscillatory shear index (OSI). Mild ( 69%) ICA stenoses were established based on 3D models of cerebral arteries in two volunteers. Geometric multiscale computing models were numerically evaluated to obtain local hemodynamic changes in the CoW in order to assess the risk of stenosis in the CoW. Results : Model calculations showed that for all 3D models the A1 segment of the anterior cerebral artery (ACA) or the posterior communicating artery (PCA) within the CoW exhibited a hemodynamic environment with high OSI (>0.2) and low TAWSS (<1 Pa) when the ICA had a moderate stenosis. While in the case of mild and severe stenosis in ICA, there is no such phenomenon. The proportion of the surface area possessing high OSI and low TAWSS in the A1 segment of the ACA or in the PCA was mostly greater than 60%, which might potentially cause the formation and development of atherosclerosis in CoW and finally lead to CoW stenosis. Conclusions : Therefore, although moderate carotid artery stenosis may not cause ischemic stroke, it may cause hemodynamic changes in the CoW, which in turn may promote CoW stenosis and cause CoW decompensation. In clinical treatment attention should be paid not only to stenosis of the carotid arteries but also to changes in the hemodynamic environment within the CoW, in order to prevent the adverse effects of CoW stenosis.

Abstract:A carotid web (CaW) is an atypical form of fibromuscular dysplasia characterized by a fibrous, shelf-like intimal flap originating from the posterior wall of the internal carotid bulb projecting into the arterial lumen. CaWs disturb normal blood flow and create stasis between the intimal reflection and the carotid wall, thereby promoting thrombogenesis and increasing the risk of downstream embolic strokes. Observational data have suggested that CaWs are associated with strokes with otherwise unknown etiology, particularly in young patients without other stroke factors, and stroke recurrence rates of symptomatic CaWs have been reported to be as high as 20% over 2 years. Despite its clinical importance, there are currently no clear guidelines on the management of CaWs. In this narrative review, we discuss the epidemiology, pathogenesis, pathophysiology, diagnosis, and treatment options for this under-recognized entity.

Abstract:Objective: To validate the carotid web (CW) risk stratification assessment described in our previous work with a larger cohort of patients with incidentally found asymptomatic CWs. Methods: Retrospective analysis of our electronic medical record database identified all patients with a diagnosis of CW from 2017-2024 at our institution. We included symptomatic patients and those with asymptomatic carotid webs, meaning patients with incidentally found webs and no history of stroke or transient ischemic attack (TIA). Patient charts were reviewed for demographics, imaging, comorbidities, hospital course, and stroke incidence after diagnosis of an asymptomatic carotid web. All angles were measured as described in our previous work on a sagittal reconstruction of CTA imaging of the neck in which the common carotid artery (CCA), external carotid artery (ECA), and ICA could be well visualized, along with the CW itself. Standard logistic regression was performed to evaluate the association between the number of high-risk angles and stroke risk. Results: 26 asymptomatic and nine symptomatic patients were identified. Patients were categorized as having 0, 1, or 2+ high-risk angle values. Patients with more high-risk angles had a markedly higher risk of stroke (OR = 5.91, 95% CI: [4.25, 8.24]). The probability of stroke increased with the number of high-risk angles: patients with 2+ high-risk angles (83.4%) had the highest stroke probability compared to those with 0 (2.8%) or 1 (27.7%) high-risk angles. In the asymptomatic cohort, mean angles all fell below the high-risk threshold values. In the symptomatic cohort, mean angles were above the high-risk threshold values, whereas the mean CPT (53.6°) angle fell below the cut-off value for designation as a high-risk angle. Conclusions: Given the successful stratification of CWs into high and low-risk groups in this study, the utilization of geometric CW parameters may play a crucial role in improvement of patient selection for intervention in patients with an incidental diagnosis of CW.

Abstract:Atherosclerosis is initiated by endothelial injury that is related to abnormal values of hemodynamic parameters such as wall shear stress (WSS), oscillatory shear index (OSI) and stress phase angle (SPA), which are more common in arterial bifurcations due to the complex structure. An experimental model of human carotid bifurcation with accurate geometrical and mechanical features was set up, and using realistic pulsatile flow rates, the inlet and outlet pressure pulses were measured for normal and stenosed models with 40% and 80% severities at common carotid (CCA), internal carotid (ICA) and external carotid (ECA) arteries. Based on the obtained experimental data, fluid–structure models were developed to obtain WSS, OSI, and SPA and evaluate pathological consequences at different locations. Mild severity had minor impact, however, inducing severe 80% stenosis in each branch led to considerable localized changes of hemodynamic parameters both in the stenosis site and other locations. This included sharp increases in WSS values accompanied by very low values close to zero before and after the peaks. Severe stenosis not only caused significant changes in the local artery, but also in other branches. OSI and SPA were less sensitive to stenosis, although high peaks were observed on bifurcation site for the stenosis at ECA. The interconnection of arteries at carotid bifurcation results in altered pressure/flow patterns in all branches when a stenosis is applied in any site. Such effect confirms pathological findings that atherosclerotic plaques are observed simultaneously in different carotid branches, although with different degrees of plaque growth and severity.

Hemodynamic Factors Affecting Carotid Sinus Atherosclerotic Stenosis

Abstract:Purpose: Carotid atherosclerotic plaque occurs predominantly at the outer wall of the carotid sinus, and computational fluid dynamics (CFD) plays an important role in explaining plaque formation. The present study investigated the hemodynamic factors affecting carotid atherosclerotic stenosis. Methods: Sixteen patients with normal carotid arteries and 16 with symptomatic stenotic carotid sinus underwent 3-dimensional angiographic imaging evaluations and were studied with CFD to simulate the complete 3-dimensional blood flow and hemodynamic parameter distribution in the carotid bifurcations. The hemodynamic parameters, including wall shear stress (WSS), dynamic and total pressure, total pressure gradient, strain rate, velocity, and velocity angle, were investigated. Results: The atherosclerosis-prone outer lateral walls of the carotid sinus and the external carotid artery at its start had significantly (P < 0.05) low dynamic pressure, WSS, strain rate, and total pressure gradient but high static pressure. The blood flow near these walls with flow separation had significantly (P < 0.05) decreased velocity and dynamic pressures but a high velocity angle. The carotid divider had significantly (P < 0.05) elevated dynamic and total pressure, WSS, strain rate, and total pressure gradient but reduced static pressure. Additional stenosis occurred at the downstream area of stenosis with significantly (P < 0.05) decreased dynamic pressure, WSS, strain rate, and total pressure gradient similar to the wall at the sinus and the start of the external carotid artery. Conclusion: Significantly decreased vascular WSS, dynamic pressure, strain rate, and total pressure gradient are key to atherosclerotic plaque formation at the carotid sinus.
Carotid webs associated with ischemic stroke. Updated general review and research directions

Abstract:Carotid web (CaW) is an intimal variant of fibromuscular dysplasia strongly associated with ipsilateral cerebral infarction. Although considered rare, it is a recent and increasing concern for physicians involved in stroke diagnosis and management. The present general review relies on a systematic literature analysis and aims to update readers on the latest knowledge in the field of symptomatic CaW (syCaW). CaW associated with ipsilateral cerebral infarction or transient ischemic attack has been identified in 189 patients. Ischemic strokes (IS) mostly occur in middle age (mean 46 years) and predominately in females (66%). The high frequency of African descendant patients among case reports and series (58%) suggests an ethnic susceptibility for CaW development. CaW features are characterised by a shelf-like intraluminal defect on contrast sagittal imaging, a linear defect that splits the lumen on axial section, a post-contrast stagnation rostral to the lesion and a frequent contralateral mirrored CaW (26.6%). An artery-to-artery embolism mechanism is widely accepted via CaW blood stasis, thrombus formation and clot fragmentation scattered by blood flow. Therefore, cerebral infarctions are often large related to a high proportion of proximal occlusion (62.5%). CaW confers a high rate of IS recurrence despite standard anti-platelet treatment that reaches 33.3% of patients prospectively followed with a median time to event of one year. Although no randomised therapeutic studies are available, surgery (n=39) or stenting (n=50) have been often proposed and seem to avoid recurrences. CaW clearly emerges as a cause of cryptogenic embolic stroke and should be systematically investigated in routine. A large number of points remain to be elucidated and CaW patients should be steadily included in registries and randomised therapeutic studies.

Abstract:Background: Carotid webs are a potential cause of ischemic stroke, particularly in younger patients with few traditional risk factors. Despite advances in imaging technology, the management of carotid webs remains poorly defined due to its rarity, absence of evidence-based guidelines, and the unique challenges presented by these lesions. This narrative review evaluates current knowledge on carotid web management, emphasizing diagnostic features, pathophysiological considerations, and treatment strategies. Methods: A literature search was conducted using PubMed and Google Scholar databases with search terms "carotid web management," "carotid web treatment," and "carotid web," focusing on studies published between 2014 and 2024. Results: Carotid webs are a variant of fibromuscular dysplasia and represent a potential source of embolic strokes. Despite the low overall prevalence, they account for a significant proportion of strokes in younger patients, with a high risk of recurrence when managed medically. Diagnosis often requires advanced imaging, such as computed tomography angiography or magnetic resonance angiography. Treatment options include medical management, carotid endarterectomy, and stenting, though no consensus guidelines exist. Medical management alone has a high recurrent stroke risk. Conclusions: Carotid webs should be included in the differential diagnosis of patients with cryptogenic stroke, particularly in younger individuals. Given the high recurrence rates with medical management, both endarterectomy and stenting are recognized as safe and effective treatment options. Future prospective studies are needed to determine the optimal management strategy, including the role of preoperative anticoagulation and comparative outcomes of different treatment modalities.
Imaging and Clinical Features of Cervical Artery Web: Report of 41 Cases and Literature Review

Abstract:This study investigated the imaging and clinical features of cervical artery web to improve disease diagnosis. Imaging and clinical data of 41 patients with cervical artery web were retrospectively analyzed and summarized in the context of the literature. Of the 8653 patients who underwent carotid computed tomography angiography (CTA) in the past 3 years at our hospital, 41 (0.47%) were diagnosed with cervical artery web. Among the 41 patients, there were 38 web structures in typical position, including 26 web structures located in the internal carotid artery and 12 in the proximal bifurcation of the common carotid artery. And the other three web structures were located outside carotid artery, including two in the subclavian artery and one in the vertebral artery. There were 47 web structures in 41 cases of cervical artery web; 35/41 (85.3%) were unilateral and 6/41 (14.7%) were bilateral. Among 41 patients, there were 20 patients who had cerebral infarction, 14 who experienced transient ischemic attack, 3 patients with cerebral hemorrhage; and 4 with other manifestations. Cervical artery web showed typical imaging features in the carotid CTA. Cervical artery web was most common in the internal carotid artery and at the bifurcation of the common carotid artery. It was also occasionally found in other rare artery such as vertebral and subclavian artery. Based on the various locations of web structure, we think it is necessary to rename carotid web as cervical artery web. Most of the attachment sites of the web structures were located in the posterior wall of the lumen, followed by the lateral wall; sites on the anterior wall were rare. Cervical artery web may be an underappreciated risk factor for stroke. And hypertension can increase the incidence of ischemic stroke in patients with cervical artery web.

Abstract:Vascular Medicine, Ahead of Print. A carotid web (CaWeb), otherwise known as a carotid bulb diaphragm, is a spur of intimal fibrous tissue extending into the carotid bulb. It is a rare, underdiagnosed cause of ischemic strokes in young people. The purpose of this narrative review was to provide an update on CaWebs, highlighting recent evolutions in their management. We undertook a comprehensive literature search on main electronic databases – MEDLINE/PubMed, Cochrane Library, Web of Science, and EMBASE – using a dedicated equation to include studies up to February 13, 2024. We also searched for the most recent guidelines about carotid disease or stroke including CaWeb management. A CaWeb is found in up to 10% of young patients, particularly young women, with severe anterior stroke due to an arterial–arterial embolism from the intra-nidus thrombus. Most patients with a CaWeb have less than 50% stenosis on duplex ultrasound, and diagnosis is mostly obtained by computed tomography angiography. When applying traditional stenosis criteria for symptomatic disease (> 50% stenosis), this highly morbid condition is easily overlooked, leading to recurrent strokes. Antithrombotic treatment is associated with a high recurrence rate of stroke after the index event. The first-line treatment of symptomatic CaWebs is increasingly based on endarterectomy or stenting. The lack of recommendations before 2021 and recent discordant guidelines make CaWeb management complex. No guidelines are available to manage patients with asymptomatic CaWebs. Results from ongoing multicenter registries will be useful in guiding management decisions.

Associations Between Carotid Plaques and White Matter Hyperintensities in Cerebral Small Vessel Disease

Abstract:Background: Cerebral small vessel disease (CSVD) can lead to stroke and cognitive impairment. Small vessels cannot be visualized by neuroimaging directly, and CSVD can only be evaluated by cerebral parenchymal changes in MRI. Therefore, a convenient screening method for identifying high-risk and susceptible patients is needed. Recently, some studies found that CSVD was related to large atherosclerosis, and atherosclerosis was an essential pathological feature of CSVD. Therefore, we aimed to investigate the association between carotid plaque size characteristics and white matter hyperintensities (WMHs) in patients with CSVD. Methods: We continuously enrolled patients with CSVD. Carotid plaque features were evaluated using carotid ultrasound, and WMHs were evaluated using brain magnetic resonance imaging. Plaque characteristics were compared between patients with no/mild WMHs and those with severe WMHs. Associations between the plaque characteristics and WMH severity were analyzed using logistic regression. Results: In total, 180 patients were recruited, of whom 92 had severe WMHs. The severe WMHs group had a higher sum of the bilateral maximum intima-media thickness (4.15 mm vs. 3.30 mm), longer maximum plaque length (17.20 mm vs. 13.90 mm), thicker plaques (2.70 mm vs. 2.30 mm), and more plaques (3 vs. 2) than the no/mild WMHs group. Adjusted logistic regression analyses revealed that maximum plaque length and thickness were associated with WMHs. Conclusions: Carotid atherosclerotic plaque features, such as plaque length and thickness, were associated with the severity of WMHs, which suggested that carotid atherosclerotic plaque characteristics measured using ultrasound might be helpful indicators for identifying high-risk patients with CSVD.

Abstract:Background: Histopathological studies have shown significant differences in plaque components and surface conditions between upstream and downstream of the stenosis. It can be deduced that the flow status near the plaques is different from the flow status at the upstream side, stenosis center, or downstream side of the plaque. Purpose: To study the hemodynamic parameter distribution in different locations near atherosclerotic plaques in the carotid arteries with different stenosis degrees. Material and methods: Eleven patients were recruited in this study. CE-MRA was performed to obtain the carotid three-dimensional surface data and the stenosis degrees were calculated. The hemodynamic parameters including wall shear stress (WSS), pressure, and velocity near the plaques were obtained by computational fluid dynamic (CFD) method. Local hemodynamics parameters were analyzed and compared between different stenosis degree groups, and between upstream, stenosis center, and downstream sides of plaques. Relative ratio of velocity, WSS, and pressure values in different locations was calculated and compared. Results: Fourteen carotid arteries (with 4 mild, 6 moderate, and 4 severe stenosis) were analyzed. Significant differences were found in Pressure max (P = 0.025), Pressure mean (P = 0.020), and Pressure min (P = 0.026) between three stenosis groups. It showed significant differences in Vmin (P < 0.001) and WSSmin (P < 0.001) between three different locations. It showed upstream to downstream ratio of WSSmax (P = 0.034) and WSSmean value (P = 0.042) was significantly different between mild and moderate/severe groups. Significant differences were found in upstream to stenosis center ratio of Pressure max value (P = 0.018), Pressure mean value (P = 0.029), and Pressure min value (P = 0.026), as well as in stenosis center to downstream ratio of Pressure min value (P = 0.042). Conclusion: Velocity, WSS, pressure, and relative ratio of these parameters have certain trends in distribution around the plaques in the carotid arteries.
In vivo bone strain in the mandible of Galago crassicaudatus.

Abstract:Single element foil strain gages were bonded to mandibular cortical bone in eight specimens of Galago crassicaudatus. The gage was bonded below the Pm4 or M2 adjacent to the lower border of the mandible. The bonded strain gage was connected to form one arm of a Wheatstone bridge. Following recovery from the general anesthetic, the restrained Galago bit either a piece of wood, a food object, or a bite force transducer. During these biting episodes, mandibular bone strain deformed the strain gage and the resulting change in electrical resistance of the gage caused voltage changes across the Wheatstone bridge. These changes, directly proportional to the amount of bone strain along the gage site, were recovered by a strip chart recorder. Bone strain was measured on both the working and balancing sides of the jaws. Maximum values of bone strain and bite force were 435 microstrain (compression) and 8.2 kilograms respectively. During bending of the mandible, the correlation between bone strain (tension or compression) and bite force ranged from -0.893 (tension) to 0.997 (compression). The experiments reported here demonstrate that only a small percentage of the Galago bite force is due to balancing side muscle force during isometric unilateral molar biting. In addition, these experiments demonstrate that the Galago mandible is bent in a predictable manner during biting. The amount of apparent mandibular bone strain is dependent on (1) the magnitude of the bite force and (2) the position of the bite point.
Numerical investigation of the effect of carotid bifurcation stenosis degree on pulsatility characteristics

Abstract:Arterial bifurcations are regions that are susceptible to hemodynamic effects and thrombus formation. In the current study, the hemodynamic effects of a simplified 3D model of an arterial bifurcation were simulated using the commercial computational fluid dynamics software FLUENT. The non-Newtonian properties of blood were modeled using the Carreau model, and the pulsation dynamics and heat transfer characteristics of blood at different degrees of stenosis in the arterial bifurcation were analyzed. The results indicate that arterial stenosis caused by a thrombus when the pulsation velocity reaches its peak has an essential impact on blood transport. The stenosis of the bifurcation increases the peak pulsatile flow pressure drop, and each 0.5 mm stenosis of the arterial bifurcation increases the mean wall shear stress of the bifurcated segment by approximately 0.25 Pa. From the heat transfer perspective, arterial stenosis has little effect on the heat transfer coefficient. The heat transfer coefficient measured inside the bifurcation is much larger than that measured outside the bifurcation. The stenosis of the arterial bifurcation causes an increase in the mean velocity of the arterial cross-section, and the volume-averaged absolute vorticity is introduced to quantify the secondary flow effect during the pulsation cycle, where the arterial stenosis causes an increase in the mean absolute vorticity at pulsation velocity and accelerates the decay of the vorticity at uniform velocity. In this paper, the hemodynamics of carotid bifurcation pulsation is analyzed in conjunction with flow field properties to reveal the flow field dynamics factors and heat transfer characteristics of local stenosis of the carotid bifurcation and to conduct an exploratory study for the diagnosis and treatment of carotid bifurcation thrombosis.

Abstract:Carotid artery diseases, such as atherosclerosis, are a major cause of death in the United States. Wall shear stresses are known to prompt plaque formation, but there is limited understanding of the complex flow structures underlying these stresses and how they differ in a pre-disposed high-risk patient cohort. A 'healthy' and a novel 'pre-disposed' carotid artery bifurcation model was determined based on patient-averaged clinical data, where the 'pre-disposed' model represents a pathological anatomy. Computational fluid dynamic simulations were performed using a physiological flow based on healthy human subjects. A main hairpin vortical structure in the internal carotid artery sinus was observed, which locally increased instantaneous wall shear stress. In the pre-disposed geometry, this vortical structure starts at an earlier instance in the cardiac flow cycle and persists over a much shorter period, where the second half of the cardiac cycle is dominated by perturbed secondary flow structures and vortices. This coincides with weaker favorable axial pressure gradient peaks over the sinus for the 'pre-disposed' geometry. The findings reveal a strong correlation between vortical structures and wall shear stress and imply that an intact internal carotid artery sinus hairpin vortical structure has a physiologically beneficial role by increasing local wall shear stresses. The deterioration of this beneficial vortical structure is expected to play a significant role in atherosclerotic plaque formation.
Morphometric and hemodynamic analysis of atherosclerotic progression in human carotid artery bifurcations

Abstract:Although atherosclerosis has been widely investigated at carotid artery bifurcation, there is a lack of morphometric and hemodynamic data at different stages of the disease. The purpose of this study was to determine the lesion difference in patients with carotid artery disease compared with healthy control subjects. The three-dimensional (3D) geometry of carotid artery bifurcation was reconstructed from computed tomography angiography (CTA) images of Chinese control subjects ( n = 30) and patients with carotid artery disease ( n = 30). We defined two novel vector angles (i.e., angles 1 and 2) that were tangential to the reconstructed contour of the 3D vessel. The best-fit diameter was computed along the internal carotid artery (ICA) center line. Hemodynamic analysis was performed at various bifurcations. Patients with stenotic vessels have larger angles 1 and 2 (151 ± 11° and 42 ± 20°) and smaller diameters of the external carotid artery (ECA) (4.6 ± 0.85 mm) compared with control subjects (144 ± 13° and 36 ± 16°, 5.2 ± 0.57 mm) although there is no significant difference in the common carotid artery (CCA) (7.1 ± 1.2 vs. 7.5 ± 1.0 mm, P = 0.18). In particular, all patients with carotid artery disease have a stenosis at the proximal ICA (including both sinus and carina regions), while 20% of patients have stenosis at the middle ICA and 20% have stenosis expansion to the entire cervical ICA. Morphometric and hemodynamic analyses suggest that atherosclerotic plaques initiate at both sinus and carina regions of ICA and progress downstream.
