Abstract:Objectives. Congenital diaphragmatic hernia (CDH) is one of the most common and serious congenital disorders seen in the neonatal intensive care unit (NICU) and it is associated with a high mortality. In order to determine the risk factors and outcomes of CDH, we summarized data from a 10 year period.Methods. A retrospective study was conducted on 38 CDH patients. Clinical characteristics and risk factors were compared and non-conditional logistic regression analysis was performed to determine independent predictors for mortality. Results. Thirty patients, from a total of 38, underwent surgery for CDH. The total survival rate in patients with CDH was 63.2%(24/38) and the overall operative mortality was 20.0% (6/30). There was a significant difference between CDH patients who survived (n=24) and those who died (n=14) in the age on admission, 5-minute Apgar score, onset of respiratory distress, cardiac malformations and presence of persistent pulmonary hypertension of newborn (PPHN). Using logistic regression analysis, the following factors independently predicted mortality: the age on admission (OR: 8.15, 95%CI: 1.43 to 46.41) and cardiac malformations (OR: 18.54, 95%CI: 1.32 to 259.62). Moreover, when we compared CDH patients who survived after surgery (n=24) with those who died (n=6), there was a significant difference in the admission age, 1-minute Apgar score, presence of PPHN, lung hypoplasia, time of stabilization prior to surgery, and highest oxygenation index after surgery. Conclusions. Mortality was very high in CDH patients and was associated with care procedures. Risk factors for mortality in neonatal CDH were the age on admission and associated malformations.
Risk factors for increased mortality rate of newborns with congenital diaphragmatic hernia in the single medical center

Abstract:Objective:To study the risk factors increasing mortality rate in newborns with congenital diaphragmatic hernia (CDH) after surgery.Method:From March 2009 to March 2018, infants diagnosed with CDH and received surgery in the Department of Neonatology of our hospital were enrolled for retrospective analysis. According to the clinical outcome, they were assigned into the survival group and the death group. The clinical conditions and treatments of the two groups were compared and the risk factors of mortality were analyzed.Result:A total of 58 infants with CDH were enrolled, including 34(58.6%) male cases and 24(41.4%) female cases. 48 cases(82.8%) survived and 10 cases(17.2%) died. Compared with the death group, the survival group had less proportion of infants with pH<7.15 on admission (4/48 vs. 9/10), fewer pulmonary vasodilators usage (24/48 vs. 9/10), fewer comorbidity of pulmonary hypertension (2/48 vs. 7/10) and bigger age receiving surgery [4(2,6)d vs. 1(0,4)d]( P<0.05). Multivariate Logistic regression analysis showed that pH<7.15 on admission ( OR=0.009, 95% CI 0.000~0.310) and pulmonary hypertension ( OR=0.041, 95% CI 0.020~0.880) were both independent risk factors for death in infants with CDH ( P<0.05). Conclusion:The survival rate of CDH after surgery is high. pH<7.15 on admission and pulmonary hypertension are the independent risk factors for death in infants with CDH.

Abstract:Objective To evaluate the impact of permissive hypercapnea strategy on the survival of patients with congenital diaphragmatic hernia (CDH). Methods From January 1985 through May 2007, infants with CDH, symptomatic in the first 6 hours of life, were reviewed. Thirty-three patients were divided into two periods. In period 1, treatment consisted of paralysis, hyperventilation, and alkalization for pulmonary hypertension. However, in period 2, hyperventilation and alkalization were abandoned. Patients were lightly sedated and ventilated with the lowest pressure providing adequate chest movement, and the rate was set to patient comfort, with the goals of maintaining postductal PaCO2 at 40 to 60 mmHg, keeping preductal saturations remain≥80%, and the perfusion is satisfactory. High frequency ventilation and nitric oxide were reserved for the life- threatening situation. Results The disease severity and incidence of associated anomalies did not differ between groups. Overall, 16 of 19(84.2%) patients in study period 2 survived compared with 7 of 14(50%) in the hyperventilation period (P<0.05). Peak inspiratory pressure and arterial PH were lower and PaCO2 was higher (P<0. 05) in period 2 than in the period 1. Two patients died because of pneumothorax in period 1. There was no mortality in period 2. Delayed surgery was performed in several patients, but it showed no survival benefit. Although NO were used in 8 patients, this did not appeared to contribute to our improved results. Conclusions The majority of infants with life-threatening CDH treated with permissive hypercapnea and high frequency ventilation lead to significantly improved survival rates, and decreased incidence of barotraumas.

Abstract:Background: The optimal role of high frequency jet ventilation (HFJV) in lung protective stabilization of congenital diaphragmatic hernia (CDH) remains uncertain. We aimed to describe our center's experience with HFJV as both a rescue (following failed stabilization with CMV) and primary ventilation mode in the management of CDH. Methods: Liveborn CDH patients treated from 2013 to 2021 in a single institution were reviewed. We compared 3 groups based on their primary and last ventilation mode prior to surgery: CMV (Group 1); HFJV (Group 2); and CMV/HFJV (Group 3). Outcomes included a composite primary outcome (≥1 of mortality, need for ECMO or need for supplemental O2 at discharge), total invasive ventilation days and development of pneumothorax. A descriptive analysis including univariate group comparisons was performed. Multivariate logistic regression models investigating the relationship between mode of ventilation and the primary outcome adjusted by potentially confounding covariates were constructed. Results: 56 patients (32 Group 1, 18 Group 2, 6 Group 3) were analyzed. Group 2 and 3 patients had more severe disease based on liver position, SNAP-II score, pulmonary hypertension severity, need for inotropic support, CDHSG defect size and need for patch repair. There were no group differences in survival, need for ECMO, or pneumothorax occurrence, although infants receiving HFJV required longer invasive ventilation and had a greater need for O2 at discharge. Multivariate logistic regression revealed no associations between mode of ventilation and outcome. Conclusions: HFJV appears effective, both for CMV rescue and as a primary ventilation strategy in high risk CDH. Level of evidence: Level IV.
Early outcome of congenital diaphragmatic hernia in a Malaysian tertiary centre

Abstract:Introduction: This prospective observational study was conducted to determine the outcome of newborns with congenital diaphragmatic hernia (CDH). They were managed with a protocol of gentle ventilation to avoid barotraumas, and inhaled nitric oxide (iNO) or intravenous magnesium sulphate for treatment of persistent pulmonary hypertension of newborns (PPHN). Methods: All newborns with CDH admitted to neonatal intensive care unit of this hospital during the six-year study period were recruited. High frequency oscillatory ventilation was used when infants required peak inspiratory pressure of more than 25 mmHg. iNO at 20 ppm or intravenous magnesium sulphate was used when PPHN developed. Arterial blood pH was maintained between 7.35 and 7.45, and partial pressure of arterial carbon dioxide was kept above 35 mmHg. Surgery was performed when the infants' general condition and blood gases were stabilised for at least 24 hours. Results: Of 21 infants recruited (15 males and six females, median gestational age 39.0 weeks, median birth weight 2,800 grams), 52.4 percent had PPHN. 12 (57.1 percent) underwent surgery at a median age of 4.9 days. One died postoperatively due to PPHN. Out of the 21 subjects, 11 (52.4 percent) survived to discharge. There was no significant difference in the demographic characteristics, side and size of CDH defects, presence of PPHN, or type of treatment received, between infants who survived and died. However, infants who died had significantly lower mean Apgar scores at five minutes of life (p-value is 0.02), and higher mean oxygenation indexes (OI) (p-value is 0.01) than those of survivors. Two (18.2 percent) of the 11 survivors developed chronic lung disease. Conclusion: Low Apgar scores and high OI were associated with poor outcome in infants with CDH.

Abstract:In the third generation mobile networks (UMTS) which base on IP protocols, real time services like speech and moving pictures are important applications. For ensuring an appropriate quality of service for such services, there is an admission control necessary, rejecting service requests of users if their acceptance probably would imply an overload congestion for the network. Several models for packet streams, data handling systems and admission control strategies have been investigated. In particular, the superposition of many on-off sources, leading to bursty packet streams, is often modeled and approximated by certain stochastic processes, in particular by Markovmodulated rate processes (fluid approximation), cf. e.g. [AMS], [SE], [K], [BB], [EM], [R] and the references therein. Admission control strategies often base on monitoring the actual packet arrival rate at the system: if too ”many” packets arrive, then a control mechanism regulates the arrival rate. Token bucket algorithms are used for preventing overload for the processors handling the packets. Various kinds of stochastic processes and queueing models including fluid models are extensively used for modeling and analyzing packet handling mechanisms, cf. e.g. [IKKM], [ADRS], [AR], [ARK], [AS], [BBS], [EM], [LP], [PVL] and the references therein. Abstract:Congenital diaphragmatic hernia (CDH) is characterized by failure of diaphragmatic development with lung hypoplasia and persistent pulmonary hypertension of the newborn (PPHN). If conventional treatment with gentle ventilation and optimized vasoactive medication fails, extracorporeal membrane oxygenation (ECMO) may be considered. The benefits of ECMO in CDH are still controversial, since there are only few randomized trials demonstrating the advantages of this therapeutic option. At present, there is no precise prenatal and/or early postnatal prognostication parameter to predict reversibility of PPHN in CDH patients. Indications for initiating ECMO include either respiratory or circulatory parameters, which are also undergoing continuous refinement. Centers with higher case numbers and the availability of ECMO published promising survival rates, but data on long-term results, including morbidity and quality of life, are rare. Survival might be influenced by the timing of ECMO initiation and the timing of surgical repair. In this regard a trend toward early initiation of ECMO and early surgery on ECMO exists. The results concerning the cannulation modes are similar and a consensus on time limit for ECMO runs does not exist. The use of ECMO in CDH will continue to be evaluated, and prospective randomized trials and registry network are necessary to help answering the addressed questions of patient selection and management.
Conventional mechanical ventilation versus high-frequency oscillatory ventilation in congenital diaphragmatic hernia of neonates: a systematic review

Abstract:Background: Neonates with congenital diaphragmatic hernia (CDH) present with respiratory distress and circulatory insufficiency, requiring immediate intubation and mechanical ventilation. Studies in the literature present contradictory results regarding the optimal ventilation mode for neonates with congenital diaphragmatic hernia. We present a systematic review of the selected literature regarding high-frequency oscillatory ventilation (HFOV) compared to conventional mechanical ventilation (CMV) in congenital diaphragmatic hernia. Methodology: PubMed, SCOPUS, EBSCOhost, and ProQuest databases were used to identify literature regarding HFOV compared to CMV in a CDH. The methodological quality of the included studies was assessed using the Newcastle-Ottawa Scale (NOS) for cohort studies and the Joanna Briggs Institute critical appraisal tool for randomized clinical trials. Results: Four studies were identified and considered eligible for the study. One study was a randomized clinical trial, and the other three cohort studies. Patients in the high-frequency oscillatory (HFO) group presented with a higher length of ventilation and hospital stay. There was a lack of evidence regarding any significant difference in the mortality rate. Conclusion: We cannot make an evidence-based recommendation regarding the superiority of either CMV or HFOV as the optimal ventilation method in neonates with CDH. However, almost all studies observed a lengthened period of ventilation and time required before surgical repair in the HFOV group. A Case of Henoch-Schönlein Purpura in Disseminated Tuberculosis

Abstract:Tuberculosis is still a common disease, even in some parts of developing countries. Although its major impact is pulmonary, the tuberculosis is actually a disseminated disease. An unusual form of renal involvement of tuberculosis is glomerulonephritis, as a part of systemic vasculitis, Henoch-Schönlein Purpura (HSP). A 41-year-old man, being treated with antituberculous agents for pulmonary tuberculosis, was transferred to our hospital because of newly developed generalized purpura and pretibial edema. Renal manifestations were proteinuria and hematuria. Renal biopsy disclosed interstitial chronic granulomatous inflammation with caseous necrosis and strong nodular mesangial Ig A deposit, along with trace granular Ig G deposition and perivascular C3 deposit. Skin lesions were nonthrombocytopenic palpable purpurae, proved leukocy toclastic vasculitis by skin biopsy. All clinical symptoms and signs were relieved by antituberculous medication. We concluded that disseminated tuberculosis might be a cause of HSP, an immune complex mediated disease.
A novel tumour marker RCAS1 in a case of extramammary Paget's disease

Abstract:A 66‐year‐old male, whose primary skin lesion in extramammary Paget's disease had been surgically resected 4 years previously, was hospitalized with liver metastases. Hepatic arterial infusion chemotherapy was carried out and the tumours clearly reduced in size. Serum levels of some common tumour markers were not elevated, even prior to therapy. We measured serum levels of a novel tumour‐associated antigen, RCAS1, because its expression was detected in the tumour cells. The patient's serum RCAS1 level was elevated (22.0 U/mL) before therapy and fell during (10.5 U/mL) and after (5.0 U/mL) therapy. Therefore, serum RCAS1 levels may be valuable as a potential biomarker for monitoring therapeutic efficacy against Paget's disease.
Optimal early surgery timing for congenital diaphragmatic hernia: A systematic review

Abstract:Introduction: Congenital diaphragmatic hernia (CDH) is a failure of closure of the pleuro-peritoneal canal due to faulty embryogenesis caused herniation of intra-abdominal contents into the chest. There needs to be more clarity about the optimal surgical timing for CDH. The aim of this study is to determine the optimal surgical timing for CDH using a systematic review analysis. Materials and methods: Our study used the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020. The literature search approach used publications between 2013 and 2023 using Pubmed and SagePub databases. Studies were included if they contained reports of the best timing for emergency surgery for CHD repair. We did not include review articles and unpublished data. Results: Five articles met the criteria. The overall result, the first pre-operative 24-hour oxygenation index mean, was temporally reliable and representative (intraclass correlation coefficient = 0.70, 95% CI = 0.61-0.77). Within any severity level, there were no differences in 90-day survival or mortality rate between delayed repair and early repair (p = 0.002). As a result, there is no optimal timing for surgery in severe cases of CDH. A delay in repair did not predict an increased risk of death, nor did it suggest an increased need for post-operative extracorporeal membrane oxygen therapy. Conclusion: Regardless of the severity of the illness, the timing of CDH repair does not affect the mortality rate.Surgery is done after the physiology index achievement.
Congenital diaphragmatic hernia: an evaluation of risk factors for failure of thoracoscopic primary repair in neonates

Abstract:Purpose: Publications aiming to prove the feasibility and safety of thoracoscopic CDH-repair in neonates grow in numbers. Some teams use selection criteria, but none have proven statistical evidence. The aim of this study is to detect risk factors for failure of thoracoscopic primary closure of CDH in neonates. Methods: In 8 centers performing minimal access surgery (MAS), complete prenatal, postnatal, and operative data were evaluated for a retrospective study concerning patients with thoracoscopic congenital diaphragmatic hernia (CDH) repair. Most of the selection criteria and risk factors mentioned in the literature were analyzed. Two groups were defined: Group A - neonates who tolerated thoracoscopic primary repair, and Group B - neonates who required conversion or presented with major complications after thoracoscopic repair. Univariate and multivariate logistic regressions were used to compare these two groups. Results: From 2006 to 2010, thoracoscopy was performed in 40 neonates: Group A consisting of 28 neonates, and Group B 9 patients. Three patients were excluded because of insufficient data or major associated malformations. Significant statistical differences were found in Group B for postnatal PaCO2 >60 mmHg, need of iNO during postnatal stabilization, intrathoracic position of the stomach, pulmonary hypertension signs on the postnatal cardiac ultrasound, and preoperative OI >3.0. On multivariate analysis, only an OI >3.0 was significantly associated with conversion or major post-operative complication of thoracoscopic primary repair. Conclusion: CDH can be safely repaired in the neonatal period by thoracoscopy. The limiting factor for thoracoscopic CDH repair is PPHN. The best preoperative indicator for PPHN is OI. Prospective studies are nonetheless necessary to prove the effectiveness of using these risk factors as selection criteria to help design surgical management protocols for neonates presenting CDH.
Comparison of early versus delayed strategies for repair of congenital diaphragmatic hernia on extracorporeal membrane oxygenation

Abstract:Purpose: For the last seven years, our institution has repaired infants with CDH that require ECMO early after cannulation. Prior to that, we attempted to decannulate before repair, but repaired on ECMO if we were unable to wean after two weeks. This study compares those strategies. Methods: From 2002 to 2016, 65 infants with CDH required ECMO. 67.7% were repaired on ECMO, and 27.7% were repaired after decannulation. Data were compared between patients repaired ≤5days after cannulation ("early protocol", n=30) and >5days after cannulation or after de-cannulation ("late protocol", n=35). We used Cox regression to assess differences in outcomes between groups. Results: Survival for the early and late protocol groups was 43.3% and 68.8%, respectively (p=0.0485). For patients that were successfully decannulated before repair, survival was 94.4%. Moreover, the early repair protocol was associated with prolongation of ECMO (16.8±7.4 vs. 12.6±6.8days, p=0.0216). After multivariate regression, the early repair protocol was an independent predictor of both mortality (HR=3.48, 95% CI=1.28-9.45, p=0.015) and days on ECMO (IRR=1.39, 95% CI=1.07-1.79, p=0.012). All bleeding occurred in patients repaired on ECMO (29.5%, 13/44). Conclusions: Our data suggest that protocolized CDH repair early after ECMO cannulation may be associated with increased mortality and prolongation of ECMO. However, early repair is not necessarily harmful for those patients who would otherwise be unable to wean from ECMO before repair. Further work is needed to better move towards individualized patient care. Type of study: Treatment Study. Level of evidence: Level III.

Abstract:Congenital diaphragmatic hernia (CDH) is typified morphologically by failure of diaphragmatic development with accompanying lung hypoplasia and persistent pulmonary hypertension of the newborn (PPHN). Patients who have labile physiology and low preductal saturations despite optimal ventilatory and inotropic support may be considered for extracorporeal membrane oxygenation (ECMO). Systematic reviews into the benefits of ECMO in CDH concluded that any benefit is unclear. Few randomised trials exist to demonstrate clear benefit and guide management. However, ECMO may have its uses in those that have reversibility of their respiratory disease. A few centres and networks have demonstrated an increase in survival rate by post hoc analysis (based on a difference in referral patterns with the availability of ECMO) in their series. One issue may be that of careful patient selection with regard to reversibility of pathophysiology. At present, there is no single test or prognostication that predicts reversibility of PPHN and criteria for referral for ECMO is undergoing continued refinement. Overall survival is similar between cannulation modes. There is no consensus on the time limit for ECMO runs. The optimal timing of surgery for patients on ECMO is difficult to definitively establish, but it seems that repair at an early stage (with careful perioperative management) is becoming less of a taboo, and may improve outcome and help with either coming off ECMO or decisions on withdrawal later. The provision of ECMO will continue to be evaluated, and prospective randomised trial are needed to help answer question of patient selection and management.
The role of extracorporeal life support and timing of repair in infants with congenital diaphragmatic hernia

Abstract:Extracorporeal life support (ECLS) serves as a rescue therapy for patients with congenital diaphragmatic hernia (CDH) and severe cardiopulmonary failure, and only half of these patients survive to discharge. This costly intervention has a significant complication risk and is reserved for patients with the most severe disease physiology refractory to maximal cardiopulmonary support. Some contraindications to ECLS do exist such as coagulopathy, lethal chromosomal or congenital anomaly, very preterm birth, or very low birth weight, but many of these limits are being evaluated through further research. Consensus guidelines from the past decade vary in recommendations for ECLS use in patients with CDH but this therapy appears to have a survival benefit in the most severe subset of patients. Improved outcomes have been observed for patients treated at high-volume centers. This review details the evolving literature surrounding management paradigms for timing of CDH repair for patients receiving preoperative ECLS. Most recent data support early repair following cannulation to avoid non-repair which is uniformly fatal in this population. Longer ECLS runs are associated with decreased survival, and patient physiology should guide ECLS weaning and eventual decannulation rather than limiting patients to arbitrary run lengths. Standardization of care across centers is a major focus to limit unnecessary costs and improve short-term and long-term outcomes for these complex patients.

Abstract:Objective To explore the changes of clinical characteristics,treatments and prognosis of congenital diaphragmatic hernia (CDH).Methods There were 31 patients during the first period of January 1985 to December 2004 while another 117 patients during the second period of January 2005 to December 2015.Gender,age,birth weight,Apgar score,prenatal diagnosis,gestational age,combined deformities, blood gas analysis,delayed surgery,perioperative nitric oxide inhalation,operation methods,postoperative complications and prognosis were reviewed.And analysis of variance,chi -square and regression analysis were performed.Results During the first period,there were 20 severe and 11 mild cases (19 boys and 12 girls,28 left CDH,3 right CDH).Three cases were diagnosed during prenatal examinations.Six cases had combined simple deformity.Twenty -five cases survived while 6 cases died.During the second period,there were 64 se-vere and 53 mild cases (75 boys,42 girls,102 left CDH,15 right CDH).Twenty -one cases were diagnosed prenatally.Twenty -four cases had combined deformity.And 91 cases survived and 26 cases died.During the first period,length of hospital stay,ventilator support time and length of intensive care unit (ICU)stay were 17.2 ±11.5,3.28 ±2.30 and 11.7 ±14.5 days respectively.During the second period,length of hospital stay,ventilator support time and length of ICU stay were 17.2 ±11.5,28 ±2.30 and 11.7 ±14.5 days.Dur-ing the second period,there were postoperative sepsis (n =3),pericardial effusion (n =2)and gastroesopha-geal reflux (n =1).And among 7 recurrent cases,there were 4 thoracoscopic cases. Conclusions Over the last decade,severe CDH has become more complex and severe.And 1 /5 -min Apgar score and admission pH value are correlated with prognosis.
Prevalence and early surgical outcome of congenital diaphragmatic hernia in the Netherlands: a population-based cohort study from the European Pediatric Surgical Audit

Abstract:Background Congenital diaphragmatic hernia (CDH) is a rare birth defect with substantial mortality. This study aims to generate a population-based overview of CDH care and outcomes in the Netherlands. Moreover, it assesses interhospital variations between the two Dutch CDH expert centres. Methods This study uses data from the Dutch branch of the European Pediatric Surgical Audit, a prospective clinical audit for congenital anomalies. Data of all patients with CDH treated between 2014 and 2021 were included for epidemiological analysis. For comparative analyses, patients presenting after the neonatal period or not treated in the two CDH expert centres were excluded. Identified interhospital variations were assessed using regression analysis. Results In the study period, 283 children with CDH were born, resulting in a national prevalence rate of 2.06/10 000 live births. The patient population, treatment and outcomes at 1 year were comparable between the hospitals, except for length of hospital stay. Regression analysis identified the treating hospital as the strongest significant predictor thereof. Other factors associated with longer length of stay include the presence of other malformations, intrathoracic liver position on prenatal ultrasound, extracorporeal membrane oxygenation treatment, patch repair, complicated postoperative course and discharge to home rather than to another care facility. Conclusion Outcomes of CDH care throughout the Netherlands are comparable. However, the length of stay differed between the two hospitals, also when adjusting for other covariates. Further qualitative analysis to explain this interhospital variation is indicated. Our findings underscore the potential of clinical auditing as a quality measurement tool in rare conditions.

Abstract:Congenital Diaphragmatic hernia (CDH) is a condition characterized by a defect in the diaphragm leading to protrusion of abdominal contents into the thoracic cavity interfering with normal development of the lungs. The defect may range from a small aperture in the posterior muscle rim to complete absence of diaphragm. The pathophysiology of CDH is a combination of lung hypoplasia and immaturity associated with persistent pulmonary hypertension of newborn (PPHN) and cardiac dysfunction. Prenatal assessment of lung to head ratio (LHR) and position of the liver by ultrasound are used to diagnose and predict outcomes. Delivery of infants with CDH is recommended close to term gestation. Immediate management at birth includes bowel decompression, avoidance of mask ventilation and endotracheal tube placement if required. The main focus of management includes gentle ventilation, hemodynamic monitoring and treatment of pulmonary hypertension followed by surgery. Although inhaled nitric oxide is not approved by FDA for the treatment of PPHN induced by CDH, it is commonly used. Extracorporeal membrane oxygenation (ECMO) is typically considered after failure of conventional medical management for infants ≥ 34 weeks' gestation or with weight >2 kg with CDH and no associated major lethal anomalies. Multiple factors such as prematurity, associated abnormalities, severity of PPHN, type of repair and need for ECMO can affect the survival of an infant with CDH. With advances in the management of CDH, the overall survival has improved and has been reported to be 70-90% in non-ECMO infants and up to 50% in infants who undergo ECMO.
Predictors of Mortality among Neonates with Congenital Diaphragmatic Hernia: Experience from an Inborn Unselected Cohort in India

Abstract:Objective: To evaluate the clinical profile and predictors of mortality in neonates with congenital diaphragmatic hernia (CDH). Methods: Demographic and clinical parameters of neonates with congenital diaphragmatic heria (n=37) between January 2014 and October, 2017 were reviewed, and compared among those who survived or expired in hospital. Results: Median (range) gestation and birthweight were 38 (37-39) weeks and 2496 (2044-2889) g, respectively. Persistent pulmonary hypertension (PPHN) was documented in 19 (51%) neonates and 10 (27%) had associated malformations. Surgery could be performed in 18 (49%), overall mortality was 60%. On univariate analysis, low Apgar scores, presence of malformations, PPHN, need for higher initial peak inspiratory pressure/high frequency ventilation, and requirement of a patch for closure were associated with increased mortality. On multivariate analysis, PPHN remained the only significant risk factor [adjusted RR 3.74 (95% CI 1.45-9.68)]. Conclusions: The survival of infants with CDH is low, and PPHN is an important predictor of mortality.
