Abstract:Objective: Intestinal obstruction occurs due to the failure of passage of contents through intestine. It can be because of a mechanical or functional pathology. Main causes of intestinal obstruction may vary from country to country or among zones within a specific country. The aim of this study was to evaluate the etiology, presentation, management strategies, and patients with acute mechanical intestinal obstruction and to find incidence of abdominal tuberculosis at a tertiary care hospital in Punjab, North India. Methods: This prospective study was conducted in the Department of Surgery, Rajindra Medical College Hospital, Patiala, Punjab, India, between November 2020 and September 2022. A total of 100 patients were enrolled in our study who were diagnosed to have intestinal obstruction. Observation and Results: The most common age group affected was 21–50 years (63%). Intestinal obstruction was diagnosed with males preponderance (66%). Abdominal pain, constipation, and vomiting were the most common presenting symptoms and abdominal distension was the most common physical finding on the clinical examination. The most common radiological finding was multiple air fluid levels seen on X rays. The main cause of obstruction was Adhesions and Bands followed by Ileocecal tuberculosis. About 13% patients diagnosed with abdominal tuberculosis. Patients showed good response to anti-tubercular therapy. About 6% mortality rate was reported. Conclusion: Adhesions and bands were the main causes of acute intestinal obstruction in our region. An algorithmic diagnostic approach based on different radio investigations, biopsy, and appropriate management with a rational combination of anti-tubercular therapy can reduce the morbidity of this easily curable yet potentially lethal disease.

Abstract:Background: Acute intestinal obstruction is one of common abdominal emergency and is associated with significant morbidity and mortality especially if it progresses to bowel ischemia. The diagnosis and management of the patient with intestinal obstruction is one of the more challenging emergencies that a general surgeon can come across. Although the mortality due to acute intestinal obstruction is decreasing with better understanding of pathophysiology and improvement in diagnostic techniques, fluid and electrolyte correction, much potent anti-microbials but still mortality ranges from 10-15% and more so in developing countries.Methods: The study will be conducted in the department of general surgery with symptoms and signs of acute intestinal obstruction admitted in the surgical ward. Fifty cases of intestinal obstruction have been studied. Patients belonged to all the age groups except infants are included in present study. The criteria for selection of cases was based on clinical history, physical findings, radiological and haematological investigations. The study was divided into clinical study, investigations and treatment. The results are tabulated stressing on following points age, sex, symptoms examination findings, investigations, abnormalities, probable causative factors, operative findings and operative procedure adopted and complications if any.Results: The commonest cause of intestinal obstruction in the adults in this study series was postoperative adhesions (36%) followed by obstructed/strangulated hernia (26%). Malignancy of the large bowel was seen in 9 cases constituting 18% of cases where as incidence of Volvulus of bowel was 4% in this series. Although pulmonary tuberculosis is more prevalent in India due to advent in use of antitubercular drugs incidence of abdominal tuberculosis is becoming less. In present study incidence of ileocaecal tuberculosis was 10%. One case of mesenteric ischaemia was present in our study.Conclusions: Acute intestinal obstruction remains an important surgical emergency in the surgical field. Success in the treatment of acute intestinal obstruction depends largely upon its early diagnosis, skilful management and treating the pathological effects of the obstruction just as much as the cause itself.

Abstract:The aromatic L-amino acid decarboxylase (AAAD) (EC 4.1.1.28) enzyme converts L-DOPA to dopamine and is critical in dopamine replacement therapy in Parkinson’s disease (PD). Following stimulated release, the action of synaptic dopamine is terminated by neuronal reuptake and deamination by monoamine oxidase (MAO). While MAO inhibition is known to downregulate tyrosine hydroxylase, the rate-limiting enzyme in dopamine synthesis (Lamensdorf and Fin-berg, 1997), the effect of MAO inhibition on AAAD is unclear. There are two MAO subtypes based on substrate and inhibitor speciﬁcity, MAO-A and MAO-B (Kinemuchi et al., 1984). Dopamine is effectively de-aminated by both subtypes, but MAO-B has higher activity than MAO-A in the primate striatum, suggesting that MAO-B plays a greater role than MAO-A in striatal dopamine metabolism in primates (Paterson et al., 1995). Abstract:Introduction Abdominal tuberculosis presents in a variety of ways. Different testing modalities must be applied in addition to having a high clinical suspicion to diagnose and initiate therapy. Medications have a good response; however, morbidity has been seen following surgical management of complicated presentations like intestinal obstruction and perforation. There is a paucity of studies in the pediatric age group which evaluate response to the different treatment regimen and identify factors associated with poorer outcomes in children with abdominal tuberculosis. Methods Patient records of 75 children with abdominal tuberculosis at a single center were evaluated using a questionnaire, covering a 14-year period from 2007 to 2021. Demographic features, presenting signs and symptoms, investigations and treatment details were studied. In- person or telephonic follow-up was conducted to identify treatment outcomes. Results Incidence of abdominal TB was 7%, of all TB children with a mean age of 10.1 years. Mesenteric lymph nodes were involved in 67% and small intestine in 33% cases. Surgery was required in 22 children. 85% children completed treatment. Small intestine involvement had higher probability of undergoing surgery. Of the 70 children with complete follow up, 64 were well and 6 children succumbed to the disease. Older age, small intestine involvement and surgery were independently associated with higher mortality. Conclusion Intestinal involvement is associated with greater need for surgical intervention and greater mortality. Adolescents have poorer outcomes. Further studies are required focusing on these individual subgroups to understand the patterns of presentation, causes for mortality and prevention. Level Of Evidence Level 5

STUDY OF SURGICAL MANAGEMENT OF INTESTINAL OBSTRUCTION FROM CENTRAL INDIA: A CROSS SECTIONAL STUDY

Abstract:Intestinal obstruction is a common surgical emergency with high mortality and morbidity rates. About 15% of all emergency room visits for acute abdominal pain are due to intestinal obstruction. Acute intestinal obstruction may manifest in a variety of ways, from a relatively normal appearance with only minor abdominal pain and distension to a condition of hypovolemic or septic shock (or both) that necessitates an emergency procedure. In present study we aimed to evaluate surgical management of intestinal obstruction at our tertiary hospital. The current research included patients aged 21 to 80 who had an acute intestinal obstruction that was surgically treated. After initial resuscitation, patients with strong signs and symptoms of acute obstruction were treated with an appropriate surgical procedure. All of the information was recorded in a preformat and analysed using descriptive statistics. 144 patients had surgery for acute intestinal obstruction during the study period. 51 to 60 years old was the most common age group (26 %), followed by 41 to 50 years old (21 %). In a ratio of 2.8:1, male patients were more frequently affected than female patients. Previous abdominal surgery (56 %), diabetes (31 %), smoking (25 %), hypertension (24 %), and bronchial asthma/ COPD (15 %) were all found to be common comorbidities in this research. Postoperative adhesions (47 %), malignancy (15 %), obstructed hernia (11 %), and Koch's abdomen were the most common causes of intestinal obstruction in this report (8 %). Adhesiolysis (47 %), resection and anastomosis (22 %), diversion colostomy (13 %), and hernioplasty were the most common surgical procedures in this study (11 %). The most common post-operative complications were fever (15%) and wound infection (11%) respectively. The post-operative mortality rate was 13%. (9 patients). Complications such as septicaemia, peritonitis, and respiratory infection account for the majority of deaths. Intestinal obstruction is often caused by postoperative adhesions. Early operative procedures, in combination with clinical diagnosis and radiological findings, may enhance the outcome of acute intestinal obstruction.

Abstract:Background: Abdominal tuberculosis (TB) remains an issue as it masquerades as many malignant or benign abdominal conditions. Objective: To analyze the clinical and laboratory features of abdominal TB retrospectively and discuss its management. Methods: The data of patients with a histopathologic diagnosis of abdominal TB seen from January 1, 2008, to February 1, 2019 were collected in The First Affiliated Hospital of Nanjing Medical University. Nodal, visceral, peritoneal, and mixed TB cases were included while excluding other forms of extra-pulmonary TB (EPTB). Results: A total of 21 patients presented having a median age of 49 years (interquartile range 33-57 years) with 12 females and 9 males. Ten presented with abdominal pain, whereas four had abdominal pain and distention. Weight loss was present in five and type 2 diabetes mellitus (DM) in three. Every patient received contrast-enhanced computed tomography (CE-CT) with positive results in all the cases. Seven patients received endoscopic ultrasound-guided fine-needle aspiration cytology examination (EUS-FNAC) and five had results positive for TB. Pre-operative diagnosis of abdominal TB was possible in seven; however, the majority (n = 14) underwent exploratory laparotomy, and all obtained a definitive diagnosis of TB. No deaths occurred. Conclusions: Both CE-CT and EUS-FNAC can aid in the timely diagnosis. Laparotomy is an invasive but efficient tool for the final diagnosis of abdominal TB.
Demographic, Clinical, Radiological, and Surgical Outcome of Patients with Intestinal Tuberculosis: A Single-Center Retrospective Study

Abstract:Background: Intestinal tuberculosis (iTB) represents a potentially underrecognized clinical entity with limited clinical and radiological differentiating features. This study aims to assess the patterns of iTB clinical and radiological findings, along with the treatment approaches and the overall outcome. Methods: This retrospective cross-sectional study included patients with histopathologically confirmed iTB who presented with acute abdomen and were surgically managed between September 2005 and October 2023. Clinical and sociodemographic variables, imaging features, surgical treatments, and overall outcomes were retrospectively analyzed. Results: 96 patients with iTB were included, with a mean age of 36.1 ± 11.5 years and a relatively proportionate gender distribution. Abdominal pain was the most common presenting symptom (45.8%). The radiological features varied by the modality. Plain imaging showed non-specific findings, while ultrasonography showed loculated ascites (25%), and lymphadenopathy (22%). In computed tomography scans, multi-segmental symmetric intestinal thickening (53.1%) was the most prevalent finding. The most commonly performed surgical procedure was adhesiolysis (29.2%), with the ileocecal junction being the most commonly involved structure (39.6%). Histopathological examination of all the tissue biopsies revealed epithelioid granulomas. Postoperative complications occurred in 19 patients (19.8%), with surgical site infection being the most common complication (10.4%). Conclusion: Intestinal obstruction is an underrecognized manifestation of tuberculosis, particularly in endemic regions. The non-specific clinical presentation, coupled with the limited utility of laboratory and radiological tests, often leads to delayed recognition and treatment. Maintaining a high index of suspicion is essential, especially in younger patients, inhabitants of endemic areas, or those with laboratory findings indicative of chronic inflammation. Prompt recognition is crucial to ensure the timely initiation of anti-tuberculosis therapy and to optimize patient outcomes through appropriate follow-up.

Abstract:Background: Abdominal tuberculosis comprises all forms of tuberculosis that involve the gastrointestinal tract. Controversies exist regarding the surgical approach and timing and type of intervention for complicated forms of abdominal tuberculosis. The aim of this systematic review is to define the rate of surgical treatment, the type of surgical procedures performed and the role of minimally invasive surgery in the management of abdominal tuberculosis. Methods: The literature in MEDLINE, Scopus and Google Scholar and forward and backward citations for studies published between database inception and July 2022 were searched without language restrictions. All prospective and retrospective studies were included. The electronic database search yielded 2440 records. Additionally, eight records were identified through snowball searching. Following duplicate removal (45 duplicates found), 2403 records were screened for titles and abstracts. After screening for titles and abstracts and exclusion criteria, 38 reports were included for systematic review, 27 retrospective studies and 11 prospective studies. Data extracted included the general and demographic characteristics of the studies, diagnostic methods used, clinical presentation, site of involvement and details on surgical treatment. Results: In total, 2870 patients with a diagnosis of abdominal tuberculosis were included, and 1803 (63%) underwent a surgical procedure. The majority of patients underwent an open surgical procedure (95%). The most commonly performed procedures were adhesiolysis (21%) and small bowel resection with primary anastomosis (21%). Conclusions: The results of this review suggest that whenever surgery is required, there is a tendency to perform open surgical procedures in patients with complicated abdominal tuberculosis, both in emergency and elective settings, despite advances in minimally invasive surgery. Abstract:Background:Acute Abdomen is a term used to encompass a spectrum of surgical, medical and gynecological conditions ranging from trivial to life threatening conditions, which require hospital admission, investigations and treatment. The purpose of this study was to identify the epidemiological pattern and to determine the spectrum of disease causing “non-traumatic acute abdomen in central India”.Methods: This is a prospective study of 98 patients of non-traumatic acute abdominal cases conducted in the Department of Surgery, Sri Aurobindo Medical College and PG Institute, Indore, Madhya Pradesh, India. In this study, preoperative detailed history and thorough physical examination was done for all acute abdominal emergencies, to arrive at pre-operative diagnosis.Results:Amongst the study of 98 patients, males have higher incidence of acute abdomen with the young age group (21-30 years) most commonly affected. Perforation peritonitis constituted the most common cause of acute abdomen (39.7%), followed by acute appendicitis (37.7%), followed by intestinal obstruction (14.2%).Conclusions: This study was conducted to evaluate the epidemiology, etiology and differential diagnosis of non-traumatic acute abdomen. At the end of the study, we had a better insight of the spectrum of the condition and we concluded that there is more scope for further work in the same field for better understanding of this topic.

Abstract:Abdominal tuberculosis (TB) remains a significant health concern globally, particularly in regions with high endemicity such as North Africa and Morocco. Despite advances in diagnostic modalities, the nonspecific presentation of abdominal TB poses challenges for timely diagnosis and management. Here, we report a case of abdominal TB in a critically state of a young man from Morocco, presenting with acute abdominal pain and signs of sepsis. Radiological investigations revealed features suggestive of intestinal perforation complicating peritoneal TB. Urgent laparotomy confirmed the diagnosis, yet the patient succumbed to advanced sepsis postoperatively. This case underscores the complexity of abdominal TB diagnosis and management, necessitating a high index of suspicion and multidisciplinary collaboration. With evolving surgical techniques and ongoing research efforts, optimizing strategies for early detection and treatment of abdominal TB remains imperative, particularly in endemic regions.
Peritoneal tuberculosis with an atypical presentation. Surgical case report

Abstract:Introduction and importance: 1-3% of tuberculosis cases are extrapulmonary, which 11-16% are abdominal. In recent years, a progressive increase in the number of peritoneal tuberculosis cases has been observed. Peritoneal tuberculosis accounts for only 1-2% of all cases of peritonitis. Historically it is known as "the great mimicker" since it can resemble a neoplastic, infectious or inflammatory intestinal disease. The most common triad of presentation is fever, weight loss and abdominal pain; ascites is also usually present. For diagnosis, computed tomography is the most sensitive technique and is complemented by elevation of CA 125 and Adenosine deaminase (ADA) but is confirmed by pathology study. Treatment is the same as the pulmonary presentation. Case presentation: A 33-year-old man presented with acute complication appendicitis. A limited right hemicolectomy was performed due to the nature of the patient advanced disease. Multiple small tumor resembling peritoneal implant and a granuloma were identified has sampled during the patient's surgical procedure. Pathologic examination revealed peritoneal tuberculosis and medical therapy was initiated. Laboratory assays including CA 125 and adenosine deaminase (ADA) can be useful serum markers to follow during treatment if they are positive. Discussion: The most common presentation of peritoneal tuberculosis is fever, weight loss and abdominal pain. Our patient did not have any of these symptoms, we founded granulomas in surgery of intestinal restitution. Methods: The work has been reported in line with the SCARE criteria. Conclusions: This pathology should always be kept in mind as a differential diagnosis when faced with unexpected findings in surgery.
Clinico-Epidemiological Analysis of Small Bowel Obstruction in Adults at a Tertiary Care Center in India

Abstract:Background: Acute small bowel obstruction (SBO) is a common surgical emergency. The study aims to provide a comprehensive clinical-epidemiological description of SBO in adults at a tertiary care center in western India. Methods: This hospital-based cross-sectional study was conducted from July 2020 to June 2022 and enrolled 88 SBO patients requiring surgical intervention. After adequately resuscitating the patients, various surgical procedures were performed based on the intraoperative conditions of the bowel. Patients were assessed postoperatively for the duration of their hospital stay, postoperative complications, and surgical recovery. Results: There was a male preponderance (n=55), with a median age of 50 (18-90) years. Abdominal discomfort was the most frequent symptom, necessitating a hospital visit (97.9%, n= 86), followed by nausea (85.2%, n= 75), constipation (78.1%, n=69), and abdominal distension (51.1%, n=45). Ileal strictures (18.2%, n=16) were the most common etiology, followed by postoperative adhesions (14.8%, n=13) and bands (13.6%, n=12), of which 76.4% (n=9) had past surgical history. Resection and anastomosis were the most frequently performed surgical interventions in this study (36.4%, n=32), followed by stoma creation (27.3%, n=24) and adhesiolysis (17%, n=15). The postoperative 30-day mortality of 11.36% (n=10) was noted, which could be ascribed to the elderly population with comorbidity, postoperative complications, and who required extended stay in the critical care unit. Conclusion: Benign ileal stricture was the most common cause of acute SBO in the emergency. Prompt and timely diagnosis combined with a multidisciplinary approach and effective management can improve outcomes and reduce morbidity and mortality in adult patients with SBO.

Methods: There were 50 cases of intestinal obstruction studied from October 2020 to October 2022 at MVJMC&RH (Rural Bangalore) and relevant investigations were sent and operative procedures were performed and data was collected. Results: In this study, intestinal obstruction is more common in the age group of 30-60 year. Male and female are nearly in equal ratio. Small bowel obstruction is more common. Pain abdomen and abdominal distension was the most common presentation. Most common etiological factor is postoperative adhesions. Malignant obstruction is more common in large bowel. Most common operation performed was resection-anastomosis. The mortality in intestinal obstruction is high in individuals who developed strangulated/perforated bowel, those present beyond 72 hours and in those are having pre-existing associated diseases and elderly people. Conclusions: Intestinal obstruction remains still a common and important surgical emergency. Obstruction due to adhesions increasing in incidence due to increased abdominal & pelvic surgeries. The obstruction due to external hernias decreasing due to early elective surgeries. The morbidity and mortality depends on the age of the patient, etiology of obstruction, site of obstruction, state of hydration, viability of the bowel, delay in diagnosis and surgical intervention and associated medical illness.

Abstract:Background: Intestinal obstruction is one the major cause that patients underwent emergency operations and which might lead to complications that could be turn fatal. Prompt evaluation and treatment prevents mortality and morbidity of post-operative patients. Methods: Prospective study conducted between 01 April 2020 to 31 March 2022 in 50 patients of intestinal obstruction operated in emergency according to inclusion criteria. Pre-operative, intraoperative, and post-operative assessment and recording of data done. Results: In 50 cases of intestinal obstruction, commonest age group is 41 to 60 years, pain and obstipation remains major presenting symptoms and tenderness, guarding and rigidity were most common signs. The commonest cause of intestinal obstruction in the adults in this study was postoperative adhesions (44%) followed by Koch’s abdomen (30%). Malignancy of the large bowel was seen in 6 cases constituting 12% of cases, obstructed hernia in 5 cases (10%), intussusception in 3 cases (6%) and gut malrotation in 1 case (2%). Comorbidity like hypertension and diabetes were main causes of post-operative complications. Conclusions: Maximum incidence of intestinal obstructions were reported in Age group 41-60 years with male predominance. Abdominal pain was the consistent symptoms followed by obstipation. Post-operative adhesion were the most common cause of the intestinal obstruction. Malignancy and multiple comorbidity remains the major risk factor for the post-operative complications and mortality.
Role of Diagnostic Laparoscopy in Case of Koch’s Abdomen with Intestinal Obstruction: An Observational Study

Abstract:Background & Aims: The diagnosis of abdominal tuberculosis has always been a challenge to the physician. The clinical presentation is subtle with many vague symptoms and nonspecific signs. We did this study to find the various diagnostic findings in a case of chronic abdominal pain and find out the efficacy of laparoscopy in diagnosing Koch’s abdomen. Materials and Methods: A prospective observational study was conducted on patients attending surgery department from Nov-2020 to Nov-2021 having clinical and radiological diagnosis of abdominal tuberculosis. Total 59 patients of suspected abdominal tuberculosis underwent diagnostic laparoscopy and started on anti-tuberculosis treatment. Result: The most common presenting symptom was abdominal pain present in 35 patients (59.32%). In CT scan, 31 of them were suggestive of abdominal tuberculosis and seven were inconclusive. Only 37 of them had positive (62.71%) histology for tuberculosis and 22 were negative (37.28%). The PPV and NPV of CT scan was 77.42% (95% CI=60.19-88.61%) and 85.71%% respectively. Out of 27, 20 mesenteric lymph nodes had positive histology for tuberculosis. 29 patients had caseating granuloma and 12 had non-caseating granuloma. All 18 histology negative patients had nonspecific chronic inflammation with reactive lymph nodes. Peritoneal fluid was aspirated and sent for CBNAAT from 30 patients (50.84%). 10 were positive (33.33%) for tuberculosis gene and 20 (66.33%) were negative. Conclusions: Performing laparoscopy in the majority of patients with suspected abdominal tuberculosis is a clinically rewarding idea. It has a high yield to establish the diagnosis of abdominal tuberculosis (65.78%) by sampling macroscopically pathological tissues. Keywords: Koch’s abdomen, Diagnostic laparoscopy, Koch’s abdomen
Tuberculosis of gastrointestinal tract revealed by acute abdomen due to ileocecal perforation in an HIV-infected patient: A case report

Abstract:Introduction: Gastrointestinal tuberculosis (TB) is a rare condition, but it poses significant diagnostic and management challenges, especially in immunocompromised individuals. This case report discusses the diagnostic complexities and therapeutic strategies for managing gastrointestinal TB in an HIV-positive patient, emphasizing the importance of considering TB in differential diagnoses. Case presentation: A 33-year-old male with no significant medical history presented with a five-day history of severe abdominal pain, initially epigastric and later localized to the right lower quadrant (RLQ). Physical examination revealed RLQ tenderness, and elevated inflammatory markers were observed in laboratory tests. Imaging studies suggested splenomegaly and signs consistent with acute appendicitis. Laparotomy uncovered a perforated cecal mass and diffuse white lesions across the small intestine. Histopathological analysis confirmed necrotizing granulomatous colitis, and PCR identified Mycobacterium tuberculosis (MTB). During hospitalization, the patient was diagnosed with HIV. Clinical discussion: This case underscores the diagnostic challenge of abdominal TB, particularly in HIV-infected patients where clinical presentation can mimic other conditions like Crohn's disease or appendicitis. Effective management requires timely surgical intervention, followed by appropriate anti-tuberculous and antiretroviral therapies. The multidisciplinary approach ensures comprehensive care and better patient outcomes. Conclusion: Effective recognition and diagnosis of gastrointestinal TB in HIV-positive patients are critical for successful treatment. This report highlights the necessity for heightened clinical suspicion and a collaborative approach in managing such complex cases, ultimately improving patient prognosis and care.
Abdominal Tuberculosis Simulating Advanced Ovarian Cancer: A Diagnostic Dilemma

Abstract:Background: The rate of preoperative misdiagnosis between abdominal tuberculosis and ovarian cancer is high because the symptoms are non-specific, clinical findings can be misleading, and biomarkers commonly linked with other diseases also make appropriate diagnosis difficult. Objective: To evaluate the clinical and diagnostic features in women with abdominal tuberculosis that simulated advanced ovarian malignancy. Materials and Methods: A retrospective review of 11 cases of abdominal tuberculosis managed at the Gynaecological Oncology unit of the University of Port Harcourt Teaching Hospital with suspected advanced ovarian cancer between January 1, 2016, and December 31, 2021. Clinical characteristics, laboratory results, including serum tumour markers, radiological findings, and ascitic fluid evaluation were extracted from theatre records and case notes and entered into a proforma. SPSS 25 was used for data analysis, and the results are displayed as frequency tables. Results: More than half 6 (55%) of the cases of abdominal TB were found in patients aged 20 to 40 years, with many patients 8 (72.7%) being nulliparous. Abdominal distension, Abdominal pain, and weight loss were the most common presenting symptoms. All patients had clinical evidence of ascites, along with thickening or ill-defined nodularities in the pouch of Douglas and/or in the adnexal areas on pelvic examination. Serum CA-125 levels were elevated in all 11 patients (100%), with a mean value of 327.8 U/mL. Pleural effusion and increased infiltration were seen in 5 (45.5%) patients. Ultrasound scan and computed tomography studies showed ascites in all 11 (100%) patients. Debritic ascites with peritoneal thickening, mesenteric and omental stranding were observed in all 11 patients (100%), and all 11 (100%) patients had laparotomy. All patients received standard antitubercular therapy. Conclusion: In relatively young females with nonspecific symptoms, a diagnosis of abdominal TB should be considered. Ultrasound-guided biopsy, laparoscopy, or a mini-laparotomy may be the most direct and least-invasive approach for a diagnosis, avoiding unnecessary extended surgeries in these patients.

Abstract:Objectives: The intestinal obstruction is a widespread emergency problem in our surgical practice day by day. It is a significant cause of morbidity and mortality in our health system. It is caused by adhesions, hernias, malignancies, endometriosis, inflammatory bowel disease stenosis, intussusception, and others. Our research aims to identify the etiology, epidemiology, and clinical presentations of the pathology so that we can manage the disease as early as possible for better outcomes in our peripheral institution. Methods: The study was performed on 100 cases of the age group 12–76 years admitted in the surgical ward of Midnapore Medical College and Hospital, Paschim Medinipur (W.B.) from December 2016 to June 2018. Cases of acute intestinal obstruction who underwent operation were included in our research to establish the pathology of intestinal obstruction. Results: In our study, the incidence of acute intestinal obstruction is 1.6% of total surgical cases. Maximum patients (25%) were in the age group 51– 60 years. The most common cause of intestinal obstruction was obstructed/strangulated hernia (40%). The most common symptoms were pain abdomen (88%), abdominal distension (84%), and vomiting (78%). An X-ray revealed presence of air-fluid levels in 75% of patients. The overall mortality rate was 10%. Conclusion: The morbidity and mortality of intestinal obstruction are dependent on early diagnosis, prompt resuscitation, and proper management. The critical determinants of the morbidity are age of the patient, duration of the disease, existing comorbidity, and delay in treatment. X-ray in erect view is an essential mode of diagnostic method and obstructed hernia is the most common etiology of obstruction in our research study.
Abdominal tuberculosis in a tertiary care centre in Saudi Arabia

Abstract:Objectives: Abdominal tuberculosis (ATB) is the second most common type of extra-pulmonary tuberculosis. Though it does not usually pose a significant risk of infectivity, ATB can go unidentified and progress to disseminated infection. The aim of this study is to highlight the incidence and outcome of this infection in a tertiary care centre in the Kingdom of Saudi Arabia (KSA). Methods: In this retrospective study, we included all ATB patients admitted to our centre between January 1 st, 2010 and December 31, 2018. A total of 42 patients with a median age of 49 (range 18-83 years, 78.6% males) were identified. Results: The most common presentation was abdominal pain, weight loss, and abdominal distension. All the patients were HIV negative; however, 50% had a comorbid condition, mainly diabetes mellitus, chronic renal failure, and liver cirrhosis. Tuberculous peritonitis was the predominant type of ATB. Suspicious and potentially malignant abdominal masses appeared on the abdominal CT scans of six patients. This suggest that TB should be excluded in patients from endemic area presenting with abdominal masses. All patients received standard anti-tuberculous medication for an average duration of 7.4 months. The outcome was excellent with 88%% achieving complete response. Adjunctive corticosteroids were not used, and none of the patients had a surgical complication. Conclusion: The diagnosis of ATB is challenging. It can mimic inflammatory bowel disease in young populations and malignancy in middle-aged and elderly population. For this reason, a high index of suspicion with prompt treatment is required to improve the prognosis and prevent complications.
