Abstract:PURPOSE To determine the characteristics of a new commercially available CT-compatible LDR Tandem and Ovoid (T&O) applicator using 3D dosimetry. METHODS We characterized source attenuation through the asymmetric gold shielding in the buckets by measuring dose with diode and 3D dosimetry and compared to an analytical line integral calculation. For 3D dosimetry, a cylindrical PRESAGE dosimeter (9.5cm diameter, 9.2cm height) with a central 6mm channel bored for source placement was scanned with the Duke Large field of view Optical CT-Scanner (DLOS) before and after delivering a nominal 7.7Gy at a distance of 1 cm using a Cs-137 source loaded in the bucket. The optical CT scan time lasted approximately 15 minutes during which 720 projections were acquired at 0.5° increments, anda 3D dose distribution was reconstructed with a 0.5mm3 isotropic voxel size. The 3D dose distribution was applied to a CT-based T&O implant to determine effect of ovoid shielding on the dose delivered to ICRU 38 Point A as well as D2cc of the bladder, rectum, bowel, and sigmoid. RESULTS Dose transmission through the gold shielding at a radial distance of 1-3cm from midplane of the source was 86.6%, 86.1, and 87.0% for analytical calculation, diode, and 3D dosimetry, respectively. For the gold shielding of the bucket, dose transmission calculated using the 3D dosimetrymeasurement was found to be lowest at oblique angles from the bucket witha minimum of ∼51%. For the patient case, attenuation from the buckets leadto a decrease in average Point A dose of ∼4% and decrease in D2cc to bladder, rectum, sigmoid, and bowel of 2%, 15%, 2%, and 7%, respectively. CONCLUSIONS The measured 3D dose distribution provided unique insight to the dosimetry and shielding characteristics of the investigated applicator, the technique for which can be applied to commissioning of other brachytherapy applicators. Analysis of 14,008 uroliths from dogs in the UK over a 10-year period

Abstract:Objectives: To identify breed-associated risk factors for urolithiasis in dogs from the UK. Methods: Records of all canine uroliths submitted to Hills Pet Nutrition UK for analysis at the University of Minnesota Urolith Centre over 10 years (1997 to 2006) were reviewed. The results, along with the request forms completed by the submitting veterinarian, were analysed. The most commonly affected breeds, age and gender of the dogs were identified for each of the most common types of uroliths (struvite, calcium oxalate, urate, cystine and mixed). Pearson's chi-squared tests were performed to assess whether certain breeds of dogs were over-represented relative to a reference population (from an insurance database). Results: The records of 14,008 urolith submissions were analysed. The relative frequency of struvite remained stable over time, whereas calcium oxalate decreased over the study period. Breeds found to be significantly over-represented for calcium oxalate uroliths included the Chihuahua, miniature poodle and Yorkshire terrier. Staffordshire bull terriers and English bulldogs were at increased risk for cystine uroliths. Clinical significance: Associations between breed, gender, age and urolith formation were similar to those reported elsewhere. However, temporal trends and novel breed predispositions were identified.
Cystine urolithiasis-free duration after first occurrence and treatment is longer for castrated dogs than for sexually intact male dogs

Abstract:Objective: To evaluate outcome in 60 dogs with cystine urolithiasis treated with surgical removal with and without castration and postoperative therapeutic diet to determine frequency of recurrence and urolith-free duration. Methods: Patient records were reviewed for dogs with documented cystine urolithiasis from September 2010 to December 2020. Medical records, client interviews, and referring veterinarians were contacted to document the absence of clinical signs associated with subsequent urolith formation and to evaluate risk factors for urolith reoccurrence. Results: 80 patients were identified with cystine uroliths, with 60 qualifying for inclusion in the study. Seven dogs were neutered prior to surgery, and 25 dogs were neutered at the time of the first surgery. Recurrence occurred in 20 dogs; 17 of those patients were intact (85%) at the time of recurrent urinary signs. Of the 20 dogs with recurrence, 50% (10 of 20) were being treated with dietary modifications. Conclusions: The risk of recurrence among neutered pets was 23% versus 47% for intact pets, but this difference was not statistically significant; however, neutered pets had a longer urolithiasis-free duration. There was no statistically significant difference in risk of recurrence and urolith-free duration between pets with and without therapeutic diet management, (30% vs 32.5%) respectively. Multivariant analysis showed no significant interaction between surgical intervention with therapeutic diet, with nonsignificant hazard ratios (HRs) for neuter status (HR = 0.503), diet (HR = 1.056), and their interaction (HR = 4.32 to 9). Clinical relevance: Sexually intact (vs castrated) male dogs should be monitored more closely for recurrence of surgical cystine urolithiasis.
Epidemiology of canine urolithiasis in the Czech Republic from 1997 to 2002

Abstract:Objectives: To compare data on the epidemiology of canine urolithiasis in the Czech Republic with that from other countries. Methods: The records from the Centre for Mineralogical Analysis from 1997 to 2002 were reviewed. The data were obtained from mineralogical analysis of 1366 canine uroliths obtained from patients in the Czech Republic. These included 396 females and 629 males. Results: Sixty-eight breeds plus crossbreeds were identified. Eight breeds plus the crossbreeds accounted for 71.3 per cent of all cases. Males were affected more frequently than females (61.4 per cent versus 38.6 per cent). Struvites significantly predominated in females, while in males calcium oxalates, brushites and cystines were the most common stones. Most of the uroliths (48.9 per cent) were 5 mm or less in dimension. By 2001, struvite was the most frequent (38.5 to 44.1 per cent) urolith, followed by calcium oxalate (26.5 to 32.0 per cent). In 2002, calcium oxalate became the most frequent calculus, followed by struvite, mixed calculi and others. Clinical significance: Comparison of these results with studies by other authors showed that for most of the monitored parameters there was agreement with respect to the proportions of different breeds within the populations of dogs in different geographical areas.
Canine and feline urolithiasis: examination of over 50 000 urolith submissions to the Canadian veterinary urolith centre from 1998 to 2008

Abstract:This study reports on trends in canine and feline urolithiasis in Canada during the past 10 years. Age, sex, breed of animals and mineral composition from 40 637 canine and 11 353 feline bladder uroliths submitted to the Canadian Veterinary Urolith Centre between 1998 and 2008 were recorded. Struvite and calcium oxalate uroliths comprised > 85% of all uroliths submitted. In dogs, the number of struvite submissions has declined and the number of calcium oxalate submissions has increased. Struvite uroliths were most common in female dogs and calcium oxalate uroliths in male dogs. The shih tzu, miniature schnauzer, bichon frisé, lhasa apso, and Yorkshire terrier were the breeds most commonly affected for both struvite and calcium oxalate uroliths. Urate uroliths were most common in male dalmatians. In cats, struvite submissions declined and calcium oxalate submissions remained constant. Struvite and calcium oxalate uroliths were common in domestic, Himalayan, Persian, and Siamese cats. Urate uroliths were over-represented in Egyptian maus.
Coalescence kinetics of bare and hydrogen-coated silicon nanoparticles : A molecular dynamics study

Abstract:One of the significant challenges in the use of nanoparticles is the control of primary particle size and extent of agglomeration when grown from the gas phase. In this paper we consider the role of surface passivation of the rate of nanoparticle coalescence. We have studied the coalescence of bare and H-coated silicon nanoparticles of sizes between 2\char21{}6 nm using molecular dynamics simulation at 1000 and 1500 K. We found that coalescence of coated particles consists of two steps, where reaction between particles and relocations of surface atoms near the reacting region, occur in the first step, which comprise an induction period. The second step consists of the nominal coalescence event, which depends on the surface tension and solid-state diffusion in the particle. The hydrogen passivation layer was found to remain on the surface of coalescing pair of the particles during the entire coalescence event. We also develop a mathematical model to describe the dynamics of coalescence of coated particles. The model is able to describe both the initial induction period and the coalescence period, and the role of the extent of surface coverage on the coalescence rate. In general, the entire coalescence time of coated particles is about 3\char21{}5 times that of bare particles, and the exothermicity from coalescence is about half that for the unpassivated particles.

Abstract:In this study, the presence and types of urine crystals in cats and dogs with urolithiasis/crystaluria and the effects of some demographic factors (such as type, breed, age, sex, sterilization status) on them were evaluated. In the study, physical, chemical and sediment examinations of urine samples of 67 cats and dogs were performed. It was determined that 83.58% of the animals with crystals in their urine were male. It was determined that 63.64% of the female animals and 44.64% of the male animals with crystals detected in the urinary tract were sterilized. It was determined that urinary crystals were more common in cats between 1-3 years of age (AO) and in dogs older than 3 AO. It was determined that calcium oxalate crystal (64.41%) in cats and struvite crystal (50%) in dogs were the most common crystals. It was determined that urine crystals were found mostly in cross breeds in cats and dogs. It was determined that most of the cats and dogs with urine crystals were fed with home meals and non-branded foods. It was determined that urinary tract infections had an effect on the formation of urine crystals. As a result, it was concluded that it is important in terms of Veterinary Medicine that such studies should be routinely performed, especially in the United States of North America (such as the Minnesota Urolith Center), since urine crystals cause significant lower urinary system diseases.

Prevalence of cystine and urate uroliths in bulldogs and urate uroliths in dalmatians

Abstract:Data were evaluated from all dogs admitted to the University of Minnesota Veterinary Teaching Hospital (UMVTH) between June 1, 1981 and Dec 31, 1991. During this period, uroliths were retrieved and analyzed from 452 of 37,574 dogs admitted. The odds that uroliths from Bulldogs were composed of cystine were 32.3 times greater than for other breeds evaluated. The odds that a Bulldog admitted was affected with cystine uroliths were 154.1 times greater than for other breeds. Cystine uroliths were retrieved only from male Bulldogs. The odds that uroliths from Bulldogs were composed of urate were 7.9 times greater than for other breeds. The odds that a Bulldog admitted was affected with urate uroliths were 43.0 times greater than for other breeds. Male Bulldogs were 14.3 times more likely to be affected with urate uroliths than were females. The odds that uroliths from Dalmatians were composed of urate were 228.9 times greater than for other breeds. The odds that a Dalmation admitted was affected with urate uroliths were 122.4 times greater than for other breeds. Male Dalmatians were 16.4 times more likely to be affected with urate uroliths than were females. Data also were evaluated from files of canine uroliths submitted to the Minnesota Urolith Center for quantitative mineral analysis between June 1, 1981 and Dec 31, 1991. During this period, 94 of 11,188 uroliths analyzed were obtained from Bulldogs and 387 were obtained from Dalmatians. The odds that uroliths retrieved from Bulldogs were composed of cystine were 40.7 times greater than for other breeds. Abstract:Background: Medical dissolution of struvite uroliths in dogs is commonly recommended, but data on success rates and complications are limited. Objectives: To evaluate the efficacy of medical dissolution for suspected struvite cystoliths in dogs. Animals: Fifty client-owned dogs fed a therapeutic dissolution diet, with or without administration of antimicrobials, for treatment of suspected struvite cystoliths. Methods: Single institution, retrospective case series. Medical records were reviewed for dogs with at least 1 follow-up visit. Dissolution success, complications, and possible predictors of success were evaluated. Results: Full dissolution of cystoliths was achieved in 58% (29/50) of dogs within a median of 35 days (range, 13-167). Of 21 dogs without success, 7 each had partial dissolution, no dissolution, or undetermined outcome. Uroliths containing >10% nonstruvite mineral were common in the nonsuccess group (11/16 analyzed). Maximum urolith diameter, number of uroliths, and baseline urine pH did not differ significantly between dogs with and without success. Dissolution was more likely in dogs receiving antimicrobial therapy (OR = 16.3, 95% confidence interval 1.9-787.4, P = .002). Adverse events occurred in 9 dogs (18%); urethral obstructions were the most common, but 3 of 4 dogs with this complication were obstructed on presentation, before trial initiation. Conclusions and clinical importance: Results support a medical dissolution trial for dogs with suspected struvite cystoliths. If no reduction in urolith size or number occurs by 1 month, a nonstruvite composition is likely, and alternative interventions should be considered. Dogs presenting with urethral obstructions should not be considered candidates for medical dissolution.

Abstract:This study aimed to investigate the clinical, laboratory, radiography and ultrasonography findings for diagnosis of urolithiasis in cats and dogs. This study was consisted of totally 17 animals, of those, 7 cats and 11 dogs were different ages, breeds and sex. Urolithiasis diagnosis was made through urine analysis, direct and indirect radiography and ultrasonographic findings. Radiopaque stones were detected in the bladder (n=13), only in the urethra (n=2), and both in the urethra and bladder (n=2). Urethrotomy was performed in two animals with urolith in the urethra, and cystotomy was performed in two cases by pushing the stones in the urethra into the urinary bladder with uroretropropulsion. In other cases, uroliths were removed from the bladder by cystotomy. The uroliths were analyzed by biochemical method. The results of the present study revealed that clinical, hematological, biochemical, urinalysis, radiographic, and ultrasonographic examinations are essential in dogs and cats for the accurate diagnosis of urolithiasis.

Abstract:Objective: To report the presence of urolithiasis in dogs long-term after gradual attenuation of congenital extrahepatic portosystemic shunts (cEHPSS). Animals: 25 client-owned dogs that underwent gradual attenuation of a cEHPSS, of which 19 had a closed cEHPSS and 6 developed multiple acquired portosystemic shunts (MAPSS) following surgery. Procedures: A retrospective study with prospective follow-up was performed. Dogs that underwent cEHPSS surgery and had their postoperative cEHPSS status determined by transsplenic portal scintigraphy or CT angiography 3 months postoperatively were prospectively contacted and invited for a long-term follow-up visit (a minimum of 6 months postoperatively). Retrospective data were collected, and during the prospective follow-up visit a thorough history, blood tests and urinalysis, and ultrasonography of the urinary tract were performed to assess the presence of urinary signs and urolithiasis. Results: Of 25 included dogs, 1 of 19 (5%) dogs with closed cEHPSS and 4 of 6 (67%) dogs with MAPSS had urolithiasis at long-term follow-up. Three (50%) dogs with MAPSS developed new uroliths. Long-term, dogs with closed cEHPSS that initially presented with and without urolithiasis had significantly less urolithiasis compared to dogs with MAPSS (P = .013 and P = .010, respectively). In the 4 dogs with closed cEHPSS that initially presented with nephrolithiasis, nephroliths became smaller or were no longer visible at the long-term follow-up visit. Clinical relevance: Dogs that developed MAPSS following cEHPSS surgery are at greater risk of urolithiasis compared to those with closed cEHPSS. Furthermore, ammonium urate uroliths might dissolve if portosystemic shunting ceases to exist.

Abstract:The prevalence of type 2 diabetes is increasing, which is alarming because of its serious complications. Anti‐diabetic treatment aims to control glucose homeostasis as tightly as possible in order to reduce these complications. Dipeptidyl peptidase‐4 (DPP‐4) inhibitors are a recent addition to the anti‐diabetic treatment modalities, and have become widely accepted because of their good efficacy, their benign side‐effect profile and their low hypoglycaemia risk. The actions of DPP‐4 inhibitors are not direct, but rather are mediated indirectly through preservation of the substrates they protect from degradation. The two incretin hormones, glucagon‐like peptide‐1 and glucose‐dependent insulinotropic polypeptide, are known substrates, but other incretin‐independent mechanisms may also be involved. It seems likely therefore that the mechanisms of action of DPP‐4 inhibitors are more complex than originally thought, and may involve several substrates and encompass local paracrine, systemic endocrine and neural pathways, which are discussed here.

Abstract:In an age of advancing endoscopic and lithotripsy technologies, the management of urolithiasis poses a unique opportunity to advance compassionate veterinary care, not only for patients with urolithiasis but for those with other urinary diseases as well. The following are consensus-derived, research and experience-supported, patient-centered recommendations for the treatment and prevention of uroliths in dogs and cats utilizing contemporary strategies. Ultimately, we hope that these recommendations will serve as a foundation for ongoing and future clinical research and inspiration for innovative problem solving.

Why is childhood urolithiasis increasing? Etiology, diagnosis and management: a single-center experience

Abstract:Background: Globally, urolithiasis is becoming more and more common among children. We aimed to determine the etiology, and the diagnostic and therapeutic approaches in patients with urolithiasis. Methods: This was a retrospective study which included all patients (aged 1 month-18 years) admitted to the pediatric nephrology clinic in Elazığ Fethi Sekin City Hospital with urolithiasis between November 2019 and 2021. Only patients whose diagnosis of urolithiasis was confirmed by urinary ultrasonography were included in the study, while patients with chronic diseases (neurological diseases such as epilepsy, cerebral palsy, chronic bowel diseases, etc.) predisposing to kidney stone formation were not. Demographic characteristics, serum and urine biochemical parameters, urine metabolic and kidney stone metabolic and chemical analyses, urinary tract ultrasonography findings and treatment modalities were collected. Results: One hundred ninety-seven patients (91 female and 106 male) were included in the study. Hypervitaminosis D was detected in 4 (2%) patients, suppressed parathyroid hormone in 12 (6%) and hypercalcemia in 27 (14%) patients. Metabolic screening showed hypercalciuria in 69 (35%) patients, hypocitraturia in 39 (20%), hyperoxaluria in 15 (8%) and cystinuria in 6 (3%) patients. Eighty three (42%) patients had a positive family history for kidney stones. One hundred eighteen (60%) patients received potassium citrate treatment, 71 (36%) were given hydration and diet recommendations without medical treatment, 6 (3%) received tiopronin treatment, and 2 (1%) patients were treated surgically. Conclusions: Our study suggests that Vitamin D supplementation at doses higher than 400 IU/day may be a risk factor for kidney stones in children. We observed that mothers tend not to give water to infants who are breastfed or formula-fed in the first year of life. K-citrate treatment can be a good option for prevention and dissolution of stones by alkalinization.

We hope that the information presented here will prompt physicians to identify system weaknesses in their office and hospital practices, thereby reducing the risk of harm to patients.
A multicenter study of antimicrobial prescriptions for cats diagnosed with bacterial urinary tract disease

Abstract:Objectives: The aim of this study was to evaluate initial antimicrobial therapy in cats diagnosed with upper or lower bacterial urinary tract infections at veterinary practices in the USA and Canada. Methods: Electronic medical records from a veterinary practice corporation with clinics in the USA and Canada were queried between 2 January 2016 and 3 December 2018. Feline patient visits with a diagnosis field entry of urinary tract infection, cystitis and pyelonephritis, as well as variation of those names and more colloquial diagnoses such as kidney and bladder infection, and where an antimicrobial was prescribed, were retrieved. Results: Prescription data for 5724 visits were identified. Sporadic cystitis was the most common diagnosis (n = 5051 [88%]), with 491 (8.6%) cats diagnosed with pyelonephritis and 182 (3.2%) with chronic or recurrent cystitis. Cefovecin was the most commonly prescribed antimicrobial for all conditions, followed by amoxicillin-clavulanic acid. Significant differences in antimicrobial drug class prescribing were noted between practice types and countries, and over the 3-year study period. For sporadic cystitis, prescription of amoxicillin-clavulanic acid increased significantly and cefovecin decreased between 2016 and 2018, and 2017 and 2018, while fluoroquinolone use increased between 2017 and 2018. Conclusions and relevance: The results indicate targets for intervention and some encouraging trends. Understanding how antimicrobials are used is a key component of antimicrobial stewardship and is required to establish benchmarks, identify areas for improvement, aid in the development of interventions and evaluate the impact of interventions or other changes.
