Abstract:Laparoscopy is an electrosurgical medical operation often involving an application of high-frequency alternating current to remove undesired biological tissue from the insufflated abdomen accessible through inlet and outlets trocars. One of the main byproducts in this process are the gaseous particles, called surgical smoke, which is found hazardous for both the patient and the operating room staff. The elimination of this hazardous material is an area of active research in the medical community. Thus, understanding dynamics influenced by the underlying flow inside the abdomen is crucial. In this article, we propose a computational fluid dynamics model and analyse the velocity field in an insufflated abdomen shaped domain by identifying the Lagrangian Coherent Structures (LCS) that are responsible for the transportation, mixing and accumulation of the material particles in the flow. By calculating the mixing strength we show that the regions revealed by these material curves are dependent on the angle, positions and number of the outlets and inlets. Hence, a novel utility of LCS in medical surgery is presented that can detail the dynamics of surgical smoke informing better design of effective smoke removal technologies.

Quantitative Chemical Analysis of Surgical Smoke Generated During Laparoscopic Surgery With a Vessel-Sealing Device

Abstract:Background. Exposure to surgical smoke in the operation room has been a long-standing concern. Smoke generated by the interaction between lasers or electrocautery devices with biological tissue contains several toxic and carcinogenic substances, but only a few studies so far have provided quantitative data necessary for risk assessment. Methods. With laser and Fourier-transform infrared spectroscopy, we investigated the chemical composition of smoke produced with a vessel-sealing device in an anoxic environment during laparoscopic surgery. Results. Harmless concentrations of methane (<34 ppm), ethane (<2 ppm), and ethylene (<10 ppm) were detected. Traces of carbon monoxide (<3.2 ppm) and of the anesthetic sevoflurane (<450 ppm) were also found. Conclusions. Gas leaking or gas being released from the pneumoperitoneum could therefore increase pollution by waste anesthetic gas in the operating room. Most toxic compounds found in earlier studies remained undetected. Adverse health effects for operating room personnel due to some of those substances (eg, toluene, styrene, xylene) can be excluded, assuming no significant losses or changes in the chemical composition of the samples occurred between our sampling and measurements.

Abstract:A large amount of surgical smoke in electrosurgery seriously deteriorates the clean environment of the operating room and can potentially harm medical staff and patients. Exploring the distribution and removal of indoor particulate matter and selecting efficient ventilation patterns are effective ways to control harmful smoke. Therefore, in this study, we combined simulations and full-scale experiments to quantitatively explore the high-concentration spatial regions of particles and compared three ventilation patterns: vertical laminar airflow (VLAF), horizontal laminar airflow (HLAF), and hybrid ventilation, wherein unidirectional airflow (UDAF) was applied to the operating table along with peripheral mixing (UDAF + mixing). We found that simple laminar flow ventilation was significantly affected by the equipment layout and air change rate (air changes per hour; ACH), and the smoke particles were distributed in large amounts in the operating area and could not be removed completely. Conversely, hybrid ventilation can work effectively, and the optimal ACH is approximately 60, which can remove nearly 72% of smoke particles. The airflow distribution in the operating room is also an important factor affecting the distribution and removal of smoke particles. Therefore, medical staff should avoid prolonged exposure to areas with high particle concentrations and particle removal paths.

Abstract:Abstract Energy-based surgical instruments produce surgical smoke, which contains harmful byproducts, such as polycyclic aromatic hydrocarbons, volatile organic compounds, particulate matter, and viable microorganisms. The research setting has shifted from the laboratory to the operating room. However, significant heterogeneity in the methods of detection and placement of samplers, diversity in the tissue operated on, and types of surgeries tested has resulted in variability in detected levels and composition of surgical smoke. State regulation limiting surgical smoke exposure through local evacuators is expanding but has yet to reach the national regulatory level. However, most studies have not shown levels above standard established limits but relatively short bursts of high concentrations of these harmful by-products. This review highlights the limitations of the current research and unsupported conclusions while also suggesting further areas of interest that need more focus to improve Occupational Safety and Health Administration guidelines.

Abstract:The use of devices for tissue dissection and hemostasis during surgery is almost unavoidable. Electrically powered devices such as electrocautery, ultrasonic and laser units produce surgical smoke containing more than a thousand different products of combustion. These include large amounts of carcinogenic, mutagenic and potentially teratogenic noxae. The smoke contains particles that range widely in size, even as small as 0.007 µm. Most of the particles (90%) in electrocautery smoke are ≤6.27 µm in size, but surgical masks cannot filter particles smaller than 5 µm. In this situation, 95% of the smoke particles which pass through the mask reach deep into the respiratory tract and frequently cause various symptoms, such as headache, dizziness, nausea, eye and respiratory tract irritation, weakness, and abdominal pain in the acute period. The smoke can transport bacteria and viruses that are mostly between 0.02 µm and 3 µm in size and there is a risk of contamination. Among these viruses, SARS-CoV-2, influenza virus, HIV, HPV, HBV must be considered. The smoke may also carry malignant cells. The long-term effects of the surgical smoke are always ignored, because causality can hardly be clarified in individual cases. The quantity of the smoke changes with the technique of the surgeon, the room ventilation system, the characteristics of the power device used, the energy level at which it is set, and the characteristics of the tissue processed. The surgical team is highly exposed to the smoke, with the surgeon experiencing the highest exposure. However, the severity of exposure differs according to certain factors, e.g., ventilation by laminar or turbulent mixed airflow or smoke evacuation system. In any case, the surgical smoke must be removed from the operation area. The most effective method is to collect the smoke from the source through an aspiration system and to evacuate it outside. Awareness and legal regulations in terms of hygiene, toxicology, as well as occupational health and safety should increase.

Abstract:The indoor environment of the operating room has a significant impact on the health of personnel. Compared with the pollutants released from occupants, the amount of pollutants generated by surgical equipment is greater, which easily affects the health of patients and doctors. In this study, local exhaust ventilation (LEV) is applied to an operating room and LEV is mounted near the operation zone to remove the surgical smoke from the surgical equipment. CFD technique is used and the surgical smoke concentration reduction potential under different inlet air volumes and different inlet areas of LEV is studied. The results show that compared with the traditional method, when the inlet area of each LEV is set to 0.030 m2, the surgical smoke concentration can be reduced by 10.8%-39.2% as the inlet air volume increases from 0.003 to 0.027 m3/s; when the inlet air volume of each LEV is set to 0.015 m3/s, the surgical smoke concentration can be reduced by 24.7%-31.6% as the inlet area varies from 0.015 to 0.060 m2. This study shows that the LEV has a good potential in reducing surgical smoke concentration in the operating room. Abstract:Air quality in the microenvironment of operating rooms (ORs) has attracted much attention as surgical smoke may pose health risks. We investigated air pollution in the operating room with a laminar flow system by examining the number and size distribution of airborne particles, the chemical composition and morphology of single particles, and polycyclic aromatic hydrocarbons (PAHs). In addition, environmentally persistent free radicals (EPFRs) in ORs are reported for the first time. The results showed that there were high levels of fine particles, EPFRs, and PAHs in laminar flow operating rooms during surgical procedures. PM2.5 is the dominant particle in ORs (accounting for >90%), consisting mainly of calcareous and metal-related particles based on the morphology and chemical analysis of single particles. In addition, anesthetic gas-related particles were found in the fine particles, and their toxicology requires more attention. EPFRs in the ORs were mainly carbon-centered radicals that may be reactive to cells. The concentrations of EPFRs and PAHs in ORs were higher than in the outside environment and present a potential health risk to surgeons and anesthetists. Hence, effective filtration and evacuation of surgical smoke are necessary.

Abstract:Smoke generated by surgical instruments during laparoscopic surgery can obscure the visual field, impairing surgeons' ability to perform operations accurately and safely. Thus, smoke removal task for laparoscopic images is highly desirable. Despite laparoscopic image desmoking has attracted the attention of researchers in recent years and several algorithms have emerged, the lack of publicly available high-quality benchmark datasets is the main bottleneck to hamper the development progress of this task. To advance this field, we construct a new high-quality dataset for Laparoscopic Surgery image Desmoking, named LSD3K, consisting of 3,000 paired synthetic non-homogeneous smoke images. In this paper, we provide a dataset generation pipeline, which includes modeling smoke shape using Blender, collecting ground-truth images from the Cholec80 dataset, random sampling of smoke masks and etc. Based on the proposed benchmark, we further conducted a comprehensive evaluation of the existing representative desmoking algorithms. A Systems Approach to Assess Transport and Diffusion of Hazardous Airborne Particles in a Large Surgical Suite: Potential Impacts on Viral Airborne Transmission

Abstract:Airborne transmission of viruses, such as the coronavirus 2 (SARS-CoV-2), in hospital systems are under debate: it has been shown that transmission of SARS-CoV-2 virus goes beyond droplet dynamics that is limited to 1 to 2 m, but it is unclear if the airborne viral load is significant enough to ensure transmission of the disease. Surgical smoke can act as a carrier for tissue particles, viruses, and bacteria. To quantify airborne transmission from a physical point of view, we consider surgical smoke produced by thermal destruction of tissue during the use of electrosurgical instruments as a marker of airborne particle diffusion-transportation. Surgical smoke plumes are also known to be dangerous for human health, especially to surgical staff who receive long-term exposure over the years. There are limited quantified metrics reported on long-term effects of surgical smoke on staff's health. The purpose of this paper is to provide a mathematical framework and experimental protocol to assess the transport and diffusion of hazardous airborne particles in every large operating room suite. Measurements from a network of air quality sensors gathered during a clinical study provide validation for the main part of the model. Overall, the model estimates staff exposure to airborne contamination from surgical smoke and biological material. To address the clinical implication over a long period of time, the systems approach is built upon previous work on multi-scale modeling of surgical flow in a large operating room suite and takes into account human behavior factors.
Analysis of the airflow features and ventilation efficiency of an Ultra-Clean-Air operating theatre by qDNS simulations and experimental validation

Abstract:Ultra-Clean-Air (UCA) operating theatres aim to minimise surgical instrument contamination and wound infection through high flow rates of ultra-clean air, reducing the presence of Microbe Carrying Particles (MCPs). This study investigates the airflow patterns and ventilation characteristics of a UCA operating theatre (OT) under standard ventilation system operating conditions, considering both empty and partially occupied scenarios. Utilising a precise computational model, quasi-Direct Numerical Simulations (qDNS) were conducted to delineate flow velocity profiles, energy spectra, distributions of turbulent kinetic energy, energy dissipation rate, local Kolmogorov scales, and pressure-based coherent structures. These results were also complemented by a tracer gas decay analysis following ASHRAE standard guidelines. Simulations showed that contrary to the intended laminar regime, the OT's geometry inherently fosters a predominantly turbulent airflow, sustained until evacuation through the exhaust vents, and facilitating recirculation zones irrespective of occupancy level. Notably, the occupied scenario demonstrated superior ventilation efficiency, a phenomenon attributed to enhanced kinetic energy induced by the additional obstructions. The findings underscore the critical role of UCA-OT design in mitigating MCP dissemination, highlighting the potential to augment the design to optimise airflow across a broader theatre spectrum, thereby diminishing recirculation zones and consequently reducing the propensity for Surgical Site Infections (SSIs). The study advocates for design refinements to harness the turbulent dynamics beneficially, steering towards a safer surgical environment.
Patient-Specific Simulation of Pneumoperitoneum for Laparoscopic Surgical Planning

Abstract:Gas insufflation in laparoscopy deforms the abdomen and stretches the overlying skin. This limits the use of surgical image-guidance technologies and challenges the appropriate placement of trocars, which influences the operative ease and potential quality of laparoscopic surgery. This work describes the development of a platform that simulates pneumoperitoneum in a patient-specific manner, using preoperative CT scans as input data. This aims to provide a more realistic representation of the intraoperative scenario and guide trocar positioning to optimize the ergonomics of laparoscopic instrumentation. The simulation was developed by generating 3D reconstructions of insufflated and deflated porcine CT scans and simulating an artificial pneumoperitoneum on the deflated model. Simulation parameters were optimized by minimizing the discrepancy between the simulated pneumoperitoneum and the ground truth model extracted from insufflated porcine scans. Insufflation modeling in humans was investigated by correlating the simulation's output to real post-insufflation measurements obtained from patients in theatre. The simulation returned an average error of 7.26 mm and 10.5 mm in the most and least accurate datasets respectively. In context of the initial discrepancy without simulation (23.8 mm and 19.6 mm), the methods proposed here provide a significantly improved picture of the intraoperative scenario. The framework was also demonstrated capable of simulating pneumoperitoneum in humans. This study proposes a method for realistically simulating pneumoperitoneum to achieve optimal ergonomics during laparoscopy. Although further studies to validate the simulation in humans are needed, there is the opportunity to provide a more realistic, interactive simulation platform for future image-guided minimally invasive surgery.

Abstract:INTRODUCTION: Electrocautery is a widely used tool to coagulate and separate tissue during neurosurgical procedures. Surgical smoke generated by electrocautery has harmful properties if inhaled over long-term. To our knowledge, no efforts have been made within the neurosurgery field to investigate the air quality of operating rooms (OR) where neurosurgical procedures are being performed. METHODS: Four freshly post-mortem porcine models were used to perform 4 laminectomy exposures measuring 20cm using electrocautery in a simulated neurosurgical OR. Air sampling media and pumps were connected to a 3D-printed human face model. In 2 models the face was covered with a standard surgical mask, and in 2 models no masks were placed. The models were positioned at 40 cm distance from the surgical field. Air samples were analyzed by an independent laboratory. Analyses of total amount of respirable dust and 31 VOCs were conducted. The mean total amount between each 2 samples of the masked vs non-masked models were compared. RESULTS: On all 2 masked and 2 non-masked models, all 31 VOCs were below the levels of quantitation. On the 2 non-masked models, the total amount of respirable dust were 8.2 times higher than in the 2 masked models. CONCLUSIONS: Total amount of VOCs appears to be low at the distance measured regardless of mask use, however further studies are needed to determine if even small amounts can still be harmful over time. Standard surgical masks seem to be efficient in protecting against the high amounts of respirable dust generated by surgical smoke.

Abstract:Electrocauterization causes foul smells and surgical smoke that affect operating room (OR) air quality. This study analyzed the concentration distribution of 87 volatile organic compounds (VOCs) in surgical smoke generated during head and neck surgeries and evaluated their size and concentration using pig skin and pork tissues. Surgical smoke from 22 head and neck surgeries were collected; VOC concentrations were analyzed using a gas chromatography–mass spectrometry. In vitro experiments utilized an electrosurgical model to burn pig skin and pork tissues; thereafter, the particle size and concentration of surgical smoke in different sampling areas were analyzed. High methanol and carcinogen levels were observed in the surgical smoke generated by head and neck surgeries. The concentrations of 87 identified VOCs in surgical smoke were affected by human tissues and electrosurgical units. Moreover, the median concentrations of particulate matter from the electrical burning of pig skin measured in the breathing area of simulated surgical personnel were significantly higher than those in pork tissue. High VOC levels in surgical smoke from head and neck surgeries were affected by human tissues and electrosurgical units. Ultrafine aerosols primarily existed in surgical smoke from the smoke generation area. Human oral mucosa tissues treated with carbon dioxide (CO 2 ) lasers produced higher VOC concentrations than craniofacial adipose tissues that were treated using conventional electrosurgical knives. Medical staff in ORs should be informed of the hazards of exposure to VOCs and ultrafine aerosols in surgical smoke.

Abstract:BACKGROUND: Although many dermatologic surgeons are aware of the risks of surgical smoke, many do not use hazard reduction strategies.OBJECTIVE: To identify the infectious, inhalational, chemical, and mutagenic risks of surgical smoke in dermatologic procedures and suggest evidence-based hazard reduction strategies.METHODS: A review of articles indexed for MEDLINE on PubMed using the keywords surgical smoke, dermatology, surgical mask, respirator, smoke evacuator, and guidelines in 13 combinations was performed by using Preferred Reporting Items for Systematic Reviews and Meta-Analyses protocols. The review included data from 45 articles from the dermatology, surgery, infectious disease, obstetrics, and cancer biology literature.RESULTS: There are risks associated with surgical smoke, and although some dermatologists are aware of these risks, many are not using hazard reduction strategies such as smoke evacuators and surgical masks.LIMITATIONS: Most of the data regarding the hazards of surgical smoke and methods for smoke safety are derived from in vitro and in vivo studies in nonhumans, as well as from resources outside of the dermatology literature.CONCLUSION: Standardized guidelines for surgical smoke safety should be implemented in the dermatology community and residency curriculum.

Measurement and Control of Surgical Smoke to Enhance Surgical Team Safety

Abstract:Amid the coronavirus disease 2019 era, concern about the safety of surgical teams related to surgical smoke (SS) is rising. As simple ventilation improvement methods (SVIMs), we replaced 4 of the 8 supply diffusers with a direction-adjustable louver-type, closed 2 of the 4 exhaust grills, and strengthened the sealing of the doorway. Dynamic changes in the concentration of particulate matter (PM) with sizes of < 1.0 μm (PM1.0) were measured using low-cost PM meters (LCPMs) at eight locations in the operating room (OR). SS concentration up to 4 minutes at the location of the surgeon, first assistant, and scrub nurse before and after SVIMs application decreased from 65.4, 38.2, 35.7 µg/m3 to 9.5, 0.1 and 0.7 µg/m3 respectively. A similar decrease was observed in the other 5 locations. SVIMs could effectively control SS and the LCPM was also effective in measuring SS in the OR or other spaces of the hospital.
Applying an air curtain to reduce surgical smoke concentration

Abstract:Operation room personnel are exposed to high concentrations of surgical smoke during electrosurgery and laser treatment. Surgical smoke contains viral aerosol, particulate matter, volatile organic compounds, and microorganisms. Current local exhaust ventilation control methods can be noisy, bulky, and expensive. In this study, we are the first to build a cost-effective air curtain device to remove surgical smoke. Experiments were conducted in an operating room by cutting porcine samples with electrosurgical units. An air curtain system was installed below the surgical light. We measured the particle number and mass concentrations in the breathing zone. The concentrations were recorded under four scenarios: no control, commercial smoke evacuation pencil, low-velocity air curtain, and high-velocity air curtain. Results indicate that the air curtain reduces the concentration of particulate matter and produces less noise than commercial smoke evacuation pencils. The particle number removal efficiencies for the smoke evacuation pencil, low-velocity air curtain, and high-velocity air curtain were 88.52%, 70.79%, and 91.29%, respectively. The respective PM 2.5 removal efficiencies were 90.92%, 85.38%, and 97.99%. Thus, installing an air curtains under surgical lights is a promising method for reducing surgical smoke and protecting medical personnel.

Abstract:Surgical Site Infections (SSIs) is one of the common postoperative complications in hospitals. SSIs risk is positively correlated to the air contamination level in operating room. In order to explore the influence of different moving paths and different moving speeds of personnel on the ventilation efficiency under different air supply velocities in operating room, computational fluid dynamics simulation was performed with the dynamic mesh model. The air cleanliness level inside the surgical critical area was the best when the unidirectional air supply velocity was 0.35 m/s and when medical staff was standing still in the operating area. When medical staff was located in the peripheral area, particles generated from medical staff can be efficiently removed by the unidirectional airflow without transported into the surgical critical area. With the air supply velocity 0.35 m/s and the occupant's walking speed 1.0 m/s, the ventilation efficiency reached 53.8 %. It is found that the impact on the indoor airflow is the most significant when medical staff walks from the peripheral area to the surgical area. The numerical simulation method could be applied as an effective way to predict the performance of the air distribution in operating room under operational condition and to reduce the pollutant concentration in the critical surgical region.
