Abstract:Abstract Brain tumors are the leading cause of cancer death in children, and outcomes for patients with recurrent or aggressive disease remain poor. We have previously shown that locoregionally-delivered chimeric antigen receptor (CAR) T cells targeting the high-affinity IL13 receptor IL13Rα2 are safe and well-tolerated in adults, and that they can mediate a remarkable clinical response. This antigen is also expressed on roughly half of pediatric neuromalignancies. Here, we present the clinical experience from the first patients treated with IL13BBζ-CAR T cells on our trial. Patients received four doses of CAR T cells, delivered weekly through an indwelling CNS catheter; the first dose was 1e7 CAR T cells and subsequent doses were 5e7 cells. The first three patients received CAR T cells alone; the rest received lymphodepletion with cyclophosphamide and fludarabine before CAR T cell infusion. After follow-up imaging at the end of the dose-limiting toxicity (DLT) period, patients were given the option to continue therapy. Six patients have been treated without DLTs; one patient on the lymphodepletion arm did not complete the four infusions due to a Grade 3 catheter-related infection. Other patients experienced occasional Grade 3 AEs that were not attributed to CAR T therapy, with the exception of headache, muscle weakness, and ALT elevation. Otherwise, AEs were Grade 1-2. Five patients completed four infusions and were evaluable at the end of the DLT period. Of those, three experienced radiographic and/or clinical benefit, although none met protocol criteria for a partial response. Specific cytokines were increased in the cerebrospinal fluid (CSF) of patients during radiographic response periods. Additionally, single-cell transcriptomic analysis paired with TCR sequencing of CSF cells demonstrated characteristic immune signatures correlated with response. These preliminary results support combining lymphodepletion and repeated intraventricular CAR T cell delivery as a promising therapy in children with brain tumors.

Abstract:Locoregional delivery of chimeric antigen receptor (CAR) T cells has resulted in objective responses in adults with glioblastoma, but the feasibility and tolerability of this approach is yet to be evaluated for pediatric central nervous system (CNS) tumors. Here we show that engineering of a medium-length CAR spacer enhances the therapeutic efficacy of human erb-b2 receptor tyrosine kinase 2 (HER2)-specific CAR T cells in an orthotopic xenograft medulloblastoma model. We translated these findings into BrainChild-01 (<a href="https://clinicaltrials.gov/ct2/show/NCT03500991">NCT03500991</a>), an ongoing phase 1 clinical trial at Seattle Children's evaluating repetitive locoregional dosing of these HER2-specific CAR T cells to children and young adults with recurrent/refractory CNS tumors, including diffuse midline glioma. Primary objectives are assessing feasibility, safety and tolerability; secondary objectives include assessing CAR T cell distribution and disease response. In the outpatient setting, patients receive infusions via CNS catheter into either the tumor cavity or the ventricular system. The initial three patients experienced no dose-limiting toxicity and exhibited clinical, as well as correlative laboratory, evidence of local CNS immune activation, including high concentrations of CXCL10 and CCL2 in the cerebrospinal fluid. This interim report supports the feasibility of generating HER2-specific CAR T cells for repeated dosing regimens and suggests that their repeated intra-CNS delivery might be well tolerated and activate a localized immune response in pediatric and young adult patients.

Abstract:Abstract Chimeric antigen receptor T cell (CAR-T) therapy is an emerging strategy to improve treatment outcomes for recurrent high-grade glioma, a cancer that responds poorly to current therapies. Here we report a completed phase I trial evaluating IL-13Rα2-targeted CAR-T cells in 65 patients with recurrent high-grade glioma, the majority being recurrent glioblastoma (rGBM). Primary objectives were safety and feasibility, maximum tolerated dose/maximum feasible dose and a recommended phase 2 dose plan. Secondary objectives included overall survival, disease response, cytokine dynamics and tumor immune contexture biomarkers. This trial evolved to evaluate three routes of locoregional T cell administration (intratumoral (ICT), intraventricular (ICV) and dual ICT/ICV) and two manufacturing platforms, culminating in arm 5, which utilized dual ICT/ICV delivery and an optimized manufacturing process. Locoregional CAR-T cell administration was feasible and well tolerated, and as there were no dose-limiting toxicities across all arms, a maximum tolerated dose was not determined. Probable treatment-related grade 3+ toxicities were one grade 3 encephalopathy and one grade 3 ataxia. A clinical maximum feasible dose of 200 × 10 6 CAR-T cells per infusion cycle was achieved for arm 5; however, other arms either did not test or achieve this dose due to manufacturing feasibility. A recommended phase 2 dose will be refined in future studies based on data from this trial. Stable disease or better was achieved in 50% (29/58) of patients, with two partial responses, one complete response and a second complete response after additional CAR-T cycles off protocol. For rGBM, median overall survival for all patients was 7.7 months and for arm 5 was 10.2 months. Central nervous system increases in inflammatory cytokines, including IFNγ, CXCL9 and CXCL10, were associated with CAR-T cell administration and bioactivity. Pretreatment intratumoral CD3 T cell levels were positively associated with survival. These findings demonstrate that locoregional IL-13Rα2-targeted CAR-T therapy is safe with promising clinical activity in a subset of patients. ClinicalTrials.gov Identifier: NCT02208362 .

Abstract:T cell immunotherapy is emerging as a powerful strategy to treat cancer and may improve outcomes for patients with glioblastoma (GBM). We have developed a chimeric antigen receptor (CAR) T cell immunotherapy targeting IL-13 receptor α2 (IL13Rα2) for the treatment of GBM. Here, we describe the optimization of IL13Rα2-targeted CAR T cells, including the design of a 4-1BB (CD137) co-stimulatory CAR (IL13BBζ) and a manufacturing platform using enriched central memory T cells. Utilizing orthotopic human GBM models with patient-derived tumor sphere lines in NSG mice, we found that IL13BBζ-CAR T cells improved anti-tumor activity and T cell persistence as compared to first-generation IL13ζ-CAR CD8+ T cells that had shown evidence for bioactivity in patients. Investigating the impact of corticosteroids, given their frequent use in the clinical management of GBM, we demonstrate that low-dose dexamethasone does not diminish CAR T cell anti-tumor activity in vivo. Furthermore, we found that local intracranial delivery of CAR T cells elicits superior anti-tumor efficacy as compared to intravenous administration, with intraventricular infusions exhibiting possible benefit over intracranial tumor infusions in a multifocal disease model. Overall, these findings help define parameters for the clinical translation of CAR T cell therapy for the treatment of brain tumors.
Locoregional CAR T Cells for the Treatment of CNS Tumors in Children: Investigational Drug Service Pharmacy Activities

Abstract:Background: A major obstacle in translating the therapeutic potential of chimeric antigen receptor (CAR) T cells to children with central nervous system (CNS) tumors is the blood-brain barrier. To overcome this limitation, preclinical and clinical studies have supported the use of repeated, locoregional intracranial CAR T-cell delivery. However, there is limited literature available describing the process for the involvement of an investigational drug service (IDS) pharmacy, particularly in the setting of a children's hospital with outpatient dosing for CNS tumors. Objectives: To describe Seattle Children's Hospital's experience in clinically producing CAR T cells and the implementation of IDS pharmacy practices used to deliver more than 300 intracranial CAR T-cell doses to children, as well as to share how we refined the processing techniques from CAR T-cell generation to the thawing of fractionated doses for intracranial delivery. Methods: Autologous CD4+ and CD8+ T cells were collected and transduced to express HER2, EGFR, or B7-H3-specific CAR T cells. Cryopreserved CAR T cells were thawed by the IDS pharmacy before intracranial delivery to patients with recurrent/refractory CNS tumors or with diffuse intrinsic pontine glioma/diffuse midline glioma. Results: The use of a thaw-and-dilute procedure for cryopreserved individual CAR T-cell doses provides reliable viability and is more efficient than typical thaw-and-wash protocols. Cell viability with the thaw-and-dilute protocol was approximately 75% and was always within 10% of the viability assessed at cryopreservation. Cell viability was preserved through 6 hours after thawing, which exceeded the 1-hour time frame from thawing to infusion. Conclusion: As the field of adoptive immunotherapy grows and continues to bring hope to patients with fatal CNS malignancies, it is critical to focus on improving the preparatory steps for CAR T-cell delivery.

Abstract:Objective To investigate the indications,complications and prognosis of chimeric antigen receptor T cells (CAR-T)in the treatment of relapsed/refractory primary central nervous system lymphoma (PCNSL).Methods On May 21,2015,one patient with relapsed/refractory PNCSL admitted to the Department of Hematology,Xuzhou Medical University Affiliated Hospital was selected as the study object. The peripheral blood lymphocytes of the patient were collected by blood cell apheresis,and humanized CD19 and CD20 CAR-T were prepared.CAR is stably expressed in CD3+ T cells via lentiviral vector.On July 27, 2017,the patient received FC regimen of conditioning regimen by fludarabine 30 mg/(m2 ·d),d-5 to-3,and cyclophosphamide 750 mg/(m2 ·d),d-5.Humanized CD19 and CD20 CAR-T were transfused with dose of 1×106/kg each,on the first day after conditioning regimen.The clinical features and diagnosis of the patient were summarized by retrospective analysis.This study was in line with World Medical Association Declaration of Helsinki revised in 2013 and informed contents of clinical research and CAR-T immunotherapy were obtained from the subject.Results ① The patient was male and 45 years old.In February 2015,the patient appeared headache,dizziness,and blindness in the right eye without obvious incentives.In March 2015,the patient was diagnosed with diffuse large B-cell lymphoma based on his pathology and immunohistochemistry results.In January 2016,the patient achieved unconfirmed complete remission (CRu)after surgical removal of the corpus callosum,radiotherapy,and high-dose radiation therapy.In March 2017,the patient's primary disease recurred,but the condition was relieved after receiving temozolomide+high-dose methotrexate (HD-MTX)chemotherapy.② After 10 d of CAR-T infusion,the patient's headache symptoms were significantly reduced and the orientation was recovery. Grade 1 cytokine release syndrome(CRS)occurred during treatment.Patient's symptoms improved after he received symptomatic supportive treatment.After 29,69 and 116 d of CAR-T transfusion,the results of patient's head-enhanced MRI showed that the primary lesion continued to shrink.And the patient reached partial remission (PR).After 69 and 116 d of CAR-T transfusion,CAR-T were detected in cerebrospinal fluid flow cytometry of patient.After 154 d of CAR-T transfusion,the patient returned to hospital for treatment,due to "left limb weakness".Re-examination of head-enhanced MRI showed recurrence of primary disease.Then the patient received salvage treatment of high-dose chemotherapy.As of July 2019, the patient was still receiving temozolomide + HD-MTX chemotherapy for maintenance treatment. Conclusions As a new treatment regimen,CAR-T immunotherapy could be used as a salvage bridging treatment in the treatment of patients with relapsed/refractory PCNSL.And it is still necessary to maintain and consolidate the treatment.However,the above conclusions are based on one case study,and the maintenance treatment of patients with relapsed/refractory PCNSL after CAR-T immunotherapy,such as the time of maintenance treatment beginning ,as well as the maintenance treatment regimen,still need to be further explored.

Abstract:H3K27M-mutated diffuse midline gliomas (DMGs) are universally lethal cancers in children and young adults. Our team previously demonstrated efficacy of GD2-targeting chimeric antigen receptor T cells (GD2-CAR T-cells) in preclinical models of DMG of the pons (also called diffuse intrinsic pontine glioma (DIPG) and DMG of the spinal cord, and opened a Phase I clinical trial (NCT04196413) treating patients with first intravenous (IV) followed by repeated infusions of intracerebroventricular (ICV) GD2-CAR T-cells. We employed high-dimensional analyses to define immune states contributing to CAR-T activity in patients. Single cell RNA-sequencing (scRNAseq) was conducted on 555,406 single cells from 115 cerebrospinal fluid (CSF) samples of 13 patients before and after CAR-T treatment. This is the largest CSF CAR-T dataset in central nervous system (CNS) tumors and provides insights into the immune biology surrounding CAR-T treatment for CNS malignancies. Patient CSF samples were dominated by T cell and myeloid populations. After CAR-T infusion, patient CSF exhibited an increased fraction of regulatory T cells and myeloid populations from baseline. Myeloid cells in early timepoints after ICV administration demonstrated a unique pro-inflammatory signature, while CSF samples from IV and late ICV timepoints exhibited a suppressive signature. To further explore the immune biology of these myeloid contributors, we developed a patient-derived xenograft model of DMG relapse following low-dose ICV GD2 CAR-T treatment. Using a pharmacological CSF1R-inhibitor, we demonstrate that depletion of microglia/myeloid cells at a specific window following CAR-T administration enhances durability of tumor control. Together, these data display the power of in-depth correlative analyses to identify distinct immune populations that drive durability of response. Key findings from these data will allow for optimization of CAR-T therapies for H3K27M+ DMG patients, providing hope to shift the paradigm of this fatal disease.

Tisagenlecleucel Demonstrates Safety, Efficacy and CNS Trafficking in Primary CNS Lymphoma

Abstract:Abstract Background: Three CD19 directed CAR-T products have gained FDA approval for systemic large B-cell lymphoma. Due to heightened concerns of immune cell associated neurotoxicity syndrome (ICANS), patients with primary CNS lymphoma (PCNSL) were excluded from all pivotal CAR-T studies. Consequently, all three products carry a limitation of use in the PCNSL patient population per their FDA labels . Due to these exclusions, little is known about the treatment-related toxicities and therapeutic potential of the currently available CD19 directed CAR-T products in this challenging patient population with significant unmet need. Methods: Based on our prior experience of tisagenlecleucel in secondary CNS lymphoma (PMID: 31320380) we conducted a pilot study with expansion of tisagenlecleucel in adults with relapsed or refractory PCNSL (NCT04134117). Patients had to be 18 years of age or older and have had progression or relapse following methotrexate-based therapy. All patients needed a confirmed diagnosis of PCNSL without evidence of systemic disease. Patients received a standard regimen of fludarabine (25 mg/m 2) and cyclophosphamide (250 mg/m 2) daily on days -5, -4, and -3 of infusion and a dose of 0.6-6.0 x 10 8 tisagenlecleucel CAR+ T-cells. Patients who had progressed on prior BTKi were allowed to continue given its beneficial effect on CAR-T expansion and function with cessation by month 3. The primary endpoint of this study was tolerability and toxicity including the rate and grade of CRS and ICANS per the 2019 ASTCT (American Society of Transplantation and Cellular Therapy) consensus criteria. Secondary endpoints included overall response rate and complete response rate to tisagenlecleucel per the international PCNSL Collaborative Group (IPCG) criteria which included MRI and CSF assessments. Exploratory endpoints included long-term efficacy, expansion, persistence and phenotype of tisagenlecleucel, cytokine profiling of the blood and CSF and CNS trafficking of CAR-T cells. Results: As of April 1, 2021, 10 subjects (ages 35-70 years) were enrolled and 9 were infused with a median age of 67 years (range, 34-81). Of the 9 infused patients, the median time from leukapheresis to infusion was 30 days (range, 27-37). Patients were heavily pretreated prior to study enrollment and received a median of 4 prior lines of anti-neoplastic therapy. All patients had progressed or failed first line high-dose methotrexate (HD-MTX); two had a history of prior thiotepa based autologous stem cell transplant (ASCT). Eight out of 9 patients had progressed following a prior BTKi and/or an immunomodulatory drug (IMiD) as part of TEDDI-R (temozolomide, etoposide, doxil, dexamethasone, ibrutinib and rituximab, n = 3), ViPOR (venetoclax, ibrutinib, prednisone, obinutuzumab and Revlimid, n = 3), or as monotherapy (n = 5) and 2 patients had received prior stereotactic radiotherapy. All patients had measurable disease at time of lymphodepletion (pre-infusion). Grade 1 CRS was observed in 6 patients with a median onset of 4 days (range, 1-5) following tisagenlecleucel and no patients required intervention for CRS. ICANS developed in 5 out of the 9 patients, only a single case of grade 3 ICANS, with a median time of onset was 5 days (range, 3-11). With a median follow-up of 7.43 months for survivors, 6/9 patients were alive with 4/9 showing ongoing responses (Figure 1). Expansion of tisagenlecleucel was demonstrated in the peripheral blood and CSF. Nanostring and RNA pathway analysis of CSF infiltrates demonstrated higher degrees of CNS CAR-T penetration in responding patients and increased T-cell and macrophage gene signatures. Peripheral and CSF cytokines were assessed. Conclusion: Tisagenlecleucel in r/r PCNSL was safe and efficacious in a highly refractory group of patients with significant unmet need. The majority of patients demonstrated a response per IPCG, including responses beyond 12 months. Tisagenlecleucel was found to expand in the peripheral blood and CNS with CSF gene signatures suggestive of higher CAR-T cell infiltrates in responding patients. CAR T‐cell therapy induces a high rate of prolonged remission in relapsed primary CNS lymphoma: Real‐life results of the LOC network

Abstract:The prognosis of relapsed primary central nervous system lymphoma (PCNSL) remains dismal. CAR T‐cells are a major contributor to systemic lymphomas, but their use in PCNSL is limited. From the LOC network database, we retrospectively selected PCNSL who had leukapheresis for CAR‐T cells from the third line of treatment, and, as controls, PCNSL treated with any treatment, at least in the third line and considered not eligible for ASCT. Twenty‐seven patients (median age: 68, median of three previous lines, including ASCT in 14/27) had leukapheresis, of whom 25 received CAR T‐cells (tisa‐cel: N = 16, axi‐cel: N = 9) between 2020 and 2023. All but one received a bridging therapy. The median follow‐up after leukapheresis was 20.8 months. The best response after CAR‐T cells was complete response in 16 patients (64%). One‐year progression‐free survival from leukapheresis was 43% with a plateau afterward. One‐year relapse‐free survival was 79% for patients in complete or partial response at CAR T‐cell infusion. The median overall survival was 21.2 months. Twenty‐three patients experienced a cytokine release syndrome and 17/25 patients (68%) a neurotoxicity (five grade ≥3). Abstract:Recurrent glioblastoma (rGBM) remains a major unmet medical need, with a median overall survival of less than 1 year. Here we report the first six patients with rGBM treated in a phase 1 trial of intrathecally delivered bivalent chimeric antigen receptor (CAR) T cells targeting epidermal growth factor receptor (EGFR) and interleukin-13 receptor alpha 2 (IL13Rα2). The study's primary endpoints were safety and determination of the maximum tolerated dose. Secondary endpoints reported in this interim analysis include the frequency of manufacturing failures and objective radiographic response (ORR) according to modified Response Assessment in Neuro-Oncology criteria. All six patients had progressive, multifocal disease at the time of treatment. In both dose level 1 (1 ×10 7 cells; n = 3) and dose level 2 (2.5 × 10 7 cells; n = 3), administration of CART-EGFR-IL13Rα2 cells was associated with early-onset neurotoxicity, most consistent with immune effector cell-associated neurotoxicity syndrome (ICANS), and managed with high-dose dexamethasone and anakinra (anti-IL1R). One patient in dose level 2 experienced a dose-limiting toxicity (grade 3 anorexia, generalized muscle weakness and fatigue). Reductions in enhancement and tumor size at early magnetic resonance imaging timepoints were observed in all six patients; however, none met criteria for ORR. In exploratory endpoint analyses, substantial CAR T cell abundance and cytokine release in the cerebrospinal fluid were detected in all six patients. Taken together, these first-in-human data demonstrate the preliminary safety and bioactivity of CART-EGFR-IL13Rα2 cells in rGBM. An encouraging early efficacy signal was also detected and requires confirmation with additional patients and longer follow-up time. ClinicalTrials.gov identifier: NCT05168423.

Abstract:Abstract INTRODUCTION The phase 1/2 clinical trials of CD19 CAR T-cell that led to US Food and Drug Administration approval excluded patients with central nervous system (CNS) involvement, due to concerns for CAR T-cell related neurotoxicity. The clinical trial (ChiCTR1800014457) examined efficacy and safety of sequential different B cell antigen-targeted CAR T-cell therapy for pediatric refractory/relapsed (r/r) Burkitt Lymphoma with secondary CNS involvement. METHODS Twenty-three patients up to 18 years of age with r/r Burkitt Lymphoma were enrolled between January 17, 2018 and November 8, 2019 including 10 patients with secondary CNS involvement. All enrolled patients received the first mCD19 CAR T-cell infusion. The patients who achieved an ongoing complete response (CR) did not receive further therapy; for patients who retained a partial response (PR) or no response (NR) until mCD19 CAR T cells became undetectable in PB by flow cytometry (FCM), the second hCD22 CAR T-cell infusion was initiated so that it did not prematurely interfere with the antitumor effect of mCD19 CAR T cells, and could prevent disease progression in the setting of continuous mCD19 CAR T-cell contraction; and for patients who had progressive disease (PD) or relapsed disease (RD) after responding to mCD19 CAR T cells, considering that the tumor had already produced immune escape from mCD19 CAR T cells, regardless of whether there were mCD19 CAR T cells detectable in PB by FCM, the second hCD22 CAR T-cell infusion was immediately initiated. After the second hCD22 CAR T-cell infusion, the patients who retained a PR until hCD22 CAR T cells became undetectable in PB by FCM received the third hCD20 CAR T-cell infusion; for patients who suffered relapses after achieving the first CR (CR1) following the first infusion and then attained the second CR (CR2) by the second infusion, the third hCD20 CAR T-cell infusion was given as consolidation therapy after loss of hCD22 CAR T cells detectable in PB by FCM, and the patients who had PD after having obtained a PR immediately received the third hCD20 CAR T-cell infusion even if hCD22 T cells were still detectable in PB by FCM. After the third hCD20 CAR T-cell infusion, the patients who retained a PR until hCD20 CAR T cells became undetectable in PB by FCM were given the fourth hCD19 CAR T-cell infusion. RESULTS The median time from the last infusion to cutoff date was 17 months (range, 15 to 23). At 18 months after the last infusion, all patients had an estimated PFS rate of 78% (95% CI, 55 to 90) and OS rate of 83% (95% CI, 60 to 93). Through mCD19 CAR T-cell expansion in the cerebrospinal fluids (CSF) of ten patients with CNS involvement, 8 patients achieved a CR, and 2 PR. Among the 8 patients who achieved a CR, 4 patients (P5, P15, P16, and P19) maintained an ongoing CR, whereas 4 patients (P7, P11, P17, and P21 ) developed a RD at m6, m2, m3, and m1.5 after infusion. P17 died of rapid intracerebral mass (ICM) progression, which resulted in elevated intracranial pressure (ICP), a contraindication to further CAR T-cell therapy. Two patients (P7 and P21) with CNS RD, P11 with bone marrow RD, and 2 patients (P13 and P20) with PR received the second hCD22 CAR T cells infusion. Among 5 patients experiencing 2 cycles of CAR T-cell infusion, 4 patients (P7, P13, P20, and P21) had hCD22 CAR T-cell expansion in CSF. P13 achieved an ongoing CR1; P20 and P21 died of ICM progression, who did not proceed with the third hCD20 CAR T-cell infusion due to elevated ICP and loss of CD20 in tumor cells, respectively; P7 and P11 achieved a CR2, who underwent the third hCD20 CAR T cells infusion, and then had hCD20 CAR T cells expansion in CSF. P11 achieved an ongoing CR2; P7 had a CNS RD again at m3 after obtaining a CR2 and finally died of rapid disease progression. Six patients achieved ongoing CR after the last infusion. The estimated 18-month PFS rate of 60% (95% CI, 25 to 83) in the patients with CNS involvement was not significantly lower than 92% (95% CI, 56 to 99) in the patients without CNS involvement (p=0.28 ). Grade 3 neurotoxicity occurred in 30% (3/10) of patients with CNS involvement. All adverse events were reversible. Statistically significant difference was not observed in the severity of neurotoxicity between patients with and without CNS involvement (p= 0.62). CONCLUSION Sequential CAR T-cell therapy may result in a durable response and is safe in pediatric r/r Burkitt lymphoma. Abstract:Background: Single-input anti-CD19 CAR T-cells have demonstrated clinical efficacy for relapsed or refractory (R/R) non-Hodgkin B-cell lymphoma (NHL) and chronic lymphocytic leukemia (CLL). Despite excellent response rates, over 50% of CD19 CAR T-cell recipients relapse. Preclinical data show engineering of bispecific anti-CD19/CD20 CAR T-cells via lentiviral transduction effectively targets tumor cells and overcomes antigen escape (Zah E et al., Cancer Immunol Res, 2016). Based on these promising preclinical results and the limitations of single-input anti-CD19 CARs, we investigated the bispecific anti-CD19/CD20 CAR naïve/memory T-cells in a phase I dose-escalation clinical trial for patients with R/R NHL/CLL (NCT04007029). Methods: This trial includes patients who have measurable disease after 2 lines of therapy for diffuse large B-cell lymphoma (DLBCL) and primary mediastinal B-cell lymphoma (PMBCL), and after 3 lines of therapy for mantle cell lymphoma (MCL), follicular lymphoma (FL), CLL and small lymphocytic leukemia (SLL). Eligible participants received lymphodepleting chemotherapy with fludarabine 30 mg/m2 and cyclophosphamide 500 mg/m2 for three days, followed by anti-CD19/CD20 CAR T-cell infusion. The CAR T-cell infusion will be given with standard "3+3" dose escalation to determine the maximum tolerated dose (MTD), with a dose range of 5 x 107 to 6 x 108 CAR-positive cells per patient. Results: To date, three patients received treatment on cohort 1 with 5 x 107 CD19/CD20 CAR T-cells for R/R MCL, FL and PMBCL, with an average age of 49.3 (range, 29-60) and a mean of 3.7 prior regimens (range, 3-4). All 3 patients' lymphomas were CD19+/CD20+ on tissue biopsy prior to CAR infusion and all 3 received bridging chemotherapy. The infusion was well tolerated and no major infusion reactions occurred. Peak expansion was noted on day 14. No dose limiting toxicities were identified. The maximum grade CRS was 1 and there was no ICANS. At the 6.0-month cutoff date, 2 of the 3 patients remain in ongoing complete remission. Unfortunately, one patient developed progressive disease 0.5 months after CAR infusion, yet remains alive after treatment with immunotherapy. Both of the responders continue to demonstrate ongoing CAR T-cell persistence and B-cell aplasia by 3.0 and 6.0-month follow up, respectively. Conclusions: Here we demonstrate impressive responses in 2 of 3 patients at the 5 x 107 CD19/CD20 CAR T-cell dosages. Bispecific CD19/CD20 CAR T-cell therapy appears to be safe and effective in patients with R/R NHL and CLL and obviates the challenges with the single antigen directed CARs by decreasing risk of target antigen loss and expression downregulation. A longer follow up period is required to determine the impact of modifying naïve/memory T cells and the durability of response. The trial continues to enroll patients and additional clinical and translational data are being collected on the initial patient cohort. Abstract:PURPOSE T cells modified with chimeric antigen receptors (CARTs) have demonstrated efficacy for hematologic malignancies; however, benefit for patients with CNS tumors has been limited. To enhance T cell activity against GD2+ CNS malignancies, we modified GD2-directed CART cells (GD2.CARTs) with a constitutively active interleukin (IL)-7 receptor (C7R-GD2.CARTs). METHODS Patients age 1-21 years with H3K27-altered diffuse midline glioma (DMG) or other recurrent GD2-expressing CNS tumors were eligible for this phase I trial (ClinicalTrials.gov identifier: NCT04099797 ). All subjects received standard-of-care adjuvant radiation therapy or chemotherapy before study enrollment. The first treatment cohort received GD2.CARTs alone (1 × 10 7 cells/m 2 ), and subsequent cohorts received C7R-GD2.CARTs at two dose levels (1 × 10 7 cells/m 2 ; 3 × 10 7 cells/m 2 ). Standard lymphodepletion with cyclophosphamide and fludarabine was included at all dose levels. RESULTS Eleven patients (age 4-18 years) received therapy without dose-limiting toxicity. The GD2.CART cohort did not experience toxicity, but had disease progression after brief improvement of residual neurologic deficits (≤3 weeks). The C7R-GD2.CART cohort developed grade 1 tumor inflammation–associated neurotoxicity in seven of eight (88%) cases, controllable with anakinra. Cytokine release syndrome was observed in six of eight (75%, grade 1 in all but one patient) and associated with increased circulating IL-6 and IP-10 ( P 12 months), and seven of eight (88%) remained eligible for additional treatment cycles (range 2-4 cycles). Partial responses by iRANO criteria were observed in two of seven (29%) patients with DMG treated by C7R-GD2.CARTs. CONCLUSION Intravenous GD2.CARTs with and without C7R were well tolerated. Patients treated with C7R-GD2.CARTs exhibited transient improvement of neurologic deficits and increased circulating cytokines/chemokines. Treatment with C7R-GD2.CARTs represents a novel approach warranting further investigation for children with these incurable CNS cancers.

Abstract:Abstract Background Patients with relapsed or refractory B-cell non-Hodgkin lymphoma have poor outcomes with the available strategies. Based on the promising results seen in patients treated with anti-CD19 CAR T-cell therapy in relapsed or refractory B-cell non-Hodgkin lymphoma, we initiated a phase 1 study to evaluate the safety and efficacy in patients. Autologous T cells were genetically modified to express a chimeric antigen receptor consisting of an anti-CD19-scFv domain with CD3ζ and 4–1BB signalling domains. Methods We did this phase 1 study in patients with relapsed or refractory B-cell non-Hodgkin lymphoma in Peking University Cancer Hospital, Beijing, China. Patients received a CAR T-cell infusion after a conditioning regimen of cyclophosphamide (250 mg/m 2 ) and fludarabine (25 mg/m 2 ) daily for 3 days. Three CAR T cell doses were tested: 5 × 10 4 CAR T cells/kg, 1 × 10 5 CAR T cells/kg, and 1 × 10 6 CAR T cells/kg. Primary endpoints included safety and pharmacokinetics of CAR T cells. Secondary endpoints include complete response, overall response, and duration of response per International Working Group Criteria. This trial is registered at ClinicalTrials, number NCT02842138. Findings As of March 2017, 14 patients were enrolled and ten patients, with a median age of 37 years (range 24–68) underwent response evaluation. Of the ten evaluable patients, four were women, six were men, three had follicular lymphoma, and seven had diffuse large B-cell lymphoma. The median number of previous therapies was 2·5 (range 2–7). The first group of three patients (two with follicular lymphoma and one with diffuse large B-cell lymphoma) received 5 × 10 4 CAR T cells/kg. Two patients with follicular lymphoma achieved partial remission on day 28, and one of these patients achieved complete remission 3 months later. The second group of three patients (one with follicular lymphoma and two with diffuse large B-cell lymphoma) received 1 × 10 5 CAR T cells/kg. The patient with follicular lymphoma achieved partial remission on day 28. The last group of four patients (all were diagnosed with diffuse large B-cell lymphoma) received 1 × 10 6 CAR T cells/kg. Three patients (75%) achieved partial remission on day 28. One patient attained progressive disease on day 28, and died 10 days later due to disease progression. A significant CAR T-cell expansion was detected in this case. On day 26, 26·8% of the patientu0027s peripheral blood mononuclear cells were CAR T cells. PD1 expression was significant in her expanded CAR T cells. 78·3% of CD8-positive CAR T cells and 71·4% of CD4-positive CAR T cells expressed PD1 on day 26, as compared with 22% and 42% on day 13, respectively in the same patient. Increased PD1-expressing CAR T cells were also found in other patientsu0027 peripheral blood. No serious adverse event was observed in any patient. Two patients had mild fever (grade 1–2). In-vivo expansion of CAR T cells was detected in all patients between day 14–28. All responding patients were still in remission at the last follow-up (range 2·5–9·0 months). Interpretation This study demonstrated early promising activity of anti-CD19 CAR T cells in patients with relapsed or refractory B-cell non-Hodgkin lymphoma. The toxicities of cytokine release syndrome were mild and manageable. Our study also provided the first clinical evidence that expanded CAR T cells could express PD1. Abstract:Pediatric CNS tumors are responsible for the majority of cancer-related deaths in children and have poor prognoses, despite advancements in chemotherapy and radiotherapy. As many tumors lack efficacious treatments, there is a crucial need to develop more promising therapeutic options, such as immunotherapies; the use of chimeric antigen receptor (CAR) T cell therapy directed against CNS tumors is of particular interest. Cell surface targets such as B7-H3, IL13RA2, and the disialoganglioside GD2 are highly expressed on the surface of several pediatric and adult CNS tumors, raising the opportunity to use CAR T cell therapy against these and other surface targets. To evaluate the repeated locoregional delivery of CAR T cells in preclinical murine models, an indwelling catheter system that recapitulates indwelling catheters currently being used in human clinical trials was established. Unlike stereotactic delivery, the indwelling catheter system allows for repeated dosing without the use of multiple surgeries. This protocol describes the intratumoral placement of a fixed guide cannula that has been used to successfully test serial CAR T cell infusions in orthotopic murine models of pediatric brain tumors. Following orthotopic injection and engraftment of the tumor cells in mice, intratumoral placement of a fixed guide cannula is completed on a stereotactic apparatus and secured with screws and acrylic resin. Treatment cannulas are then inserted through the fixed guide cannula for repeated CAR T cell delivery. Stereotactic placement of the guide cannula can be adjusted to deliver CAR T cells directly into the lateral ventricle or other locations in the brain. This platform offers a reliable mechanism for the preclinical testing of repeated intracranial infusions of CAR T cells and other novel therapeutics for these devastating pediatric tumors.

Abstract:Central nervous system tumors are the most common solid tumors and the leading cause of cancer-related deaths in children. Outcomes remain dismal for a significant proportion of patients despite aggressive multimodal therapy and survivors suffer from treatment-related morbidity and neurological deficits. Despite their remarkable success in adult cancers, immunotherapies are not approved for pediatric brain tumors. Several factors like the relatively smaller patient population, a tumor location that makes them difficult to access through biopsies and poorly-studied tumor immune microenvironment (TME) have posed challenges in advancing immunotherapeutic options for these patients. Immune checkpoint blockers (ICB) tested in unstratified cohorts of children with brain tumors have not shown promising results; therefore, the role of immunotherapy remains unclear. Because the magnitude and nature of pre-existing T cell responses in the tumor has been shown to correlate with response to ICB, we asked if pediatric brain tumors show evidence of such favorable T cell responses. In the current study, we address this question using an unbiased approach to analyze the clonal relationship and transcriptional status of T cells present in tumor tissue of children with brain tumors using paired single-cell RNA-sequencing and TCR-sequencing. Our results demonstrate that a large fraction of T cells in the tumor tissue are clonally expanded with potential to recognize tumor antigens. Such clonally-expanded T cells display states linked to anti-tumor immunity, express higher levels of transcripts encoding for molecules linked to T cell activation, effector functions, immune cell recruitment, tissue-residency, immune checkpoints, and importantly, show significant enrichment of signatures linked to neoantigen-specific T cells and immunotherapy response. Notably, we identify several neoantigens in pediatric brain tumors, and show that neoantigen-specific T cell gene signatures are linked to better survival outcomes in high-grade glioma. We further show that PD1-expressing CD8+ T cells in pediatric brain tumors are indeed functional as evidenced by their capacity for cytotoxicity, cytokine production and proliferation. Among the patients in our cohort, we observe substantial heterogeneity in the degree of clonal expansion and expression of transcripts encoding immune checkpoints in tumor-infiltrating T cells. Overall, our findings suggest that accurate characterization of intra-tumoral T cell responses for features linked to immune check point therapy (ICB) response may enable selection of patients where immunotherapy is likely to be beneficial, an approach that requires prospective validation in future clinical trials. Abstract:Background/Purpose: Chimeric antigen receptor (CAR) T cell therapy has shown remarkable responses in hematologic malignancies. However, suboptimal clinical outcomes have been associated with decreased expansion and persistence of adoptive transferred CAR-T cells, antigen-negative relapses, and impairment by an immunosuppressive tumor microenvironment. We previously developed CD19CAR-T cells secreting PD-1Ab21 fusion protein composed of anti-PD-1 single chain antibody and IL-21(PD-1Ab21-CD19CAR-T). This investigator-initiated clinical trial is designed to assess the safety and efficacy of the novel CAR-T cells in advanced relapsed/refractory acute Lymphoblastic leukemia (ALL). Methods: This study recruited 10 patients with advanced relapsed/refractory B-ALL and half of them relapsed after allogeneic hematopoietic stem cell transplantation (allo-HSCT). All patients received fludarabine and cyclophosphamide before a single infusion of PD-1Ab21-CD19CAR-T cells at dose of 0.3, 1 or 3 x 106 cells/kg of body weight. The patients were monitored for adverse events, clinical response, as well as expansion of CAR-T and lymphocytes. Results: All five non-post-transplant patients achieved complete remission (CR), including two active CNS diseases, and three of them pursued consolidative allo-HSCT. Among the five post-transplant patients, one patient, who had undergone autologous, allogeneic CAR-T cell therapies and allo-HSCT, achieved CR and remained leukemia-free for 5 months, which was longer than the duration of remission (< 4 monthes) after allo-HSCT. The last treated patient achieved CR. Three other post-transplant patients, including two active CNS diseases, eventually died of the infection. One of the patients achieved minimal residual disease negative CR. The other two patients did not undergo laboratory evaluation, but their CNS symptoms disappeared. Once the expansion of CAR-T cells was detected, the proportion of CD8+T cell in CD3+T cells increased dramatically, rapidly reaching 77.8~91.2% and maintaining above 50% for a long time in all treated patients. Except for the three patients who died from infection, other patients experienced no or mild cytokine release syndrome (CRS) (grade ≤2). None of the patients had neurotoxic side effects. Conclusion: This study demonstrates that PD-1Ab21-secreting CAR-T cells are safe and effective therapeutic platform, even in cancer patients with CNS invasion. Our data also suggest that PD-1Ab21-secreting CAR-T therapy can activate endogenous anti-tumor CD8+ T cell responses. Abstract:Abstract Diffuse intrinsic pontine glioma (DIPG) and other H3K27M-mutated diffuse midline gliomas (DMGs) are universally lethal paediatric tumours of the central nervous system 1 . We have previously shown that the disialoganglioside GD2 is highly expressed on H3K27M-mutated glioma cells and have demonstrated promising preclinical efficacy of GD2-directed chimeric antigen receptor (CAR) T cells 2 , providing the rationale for a first-in-human phase I clinical trial (NCT04196413). Because CAR T cell-induced brainstem inflammation can result in obstructive hydrocephalus, increased intracranial pressure and dangerous tissue shifts, neurocritical care precautions were incorporated. Here we present the clinical experience from the first four patients with H3K27M-mutated DIPG or spinal cord DMG treated with GD2-CAR T cells at dose level 1 (1 × 10 6 GD2-CAR T cells per kg administered intravenously). Patients who exhibited clinical benefit were eligible for subsequent GD2-CAR T cell infusions administered intracerebroventricularly 3 . Toxicity was largely related to the location of the tumour and was reversible with intensive supportive care. On-target, off-tumour toxicity was not observed. Three of four patients exhibited clinical and radiographic improvement. Pro-inflammatory cytokine levels were increased in the plasma and cerebrospinal fluid. Transcriptomic analyses of 65,598 single cells from CAR T cell products and cerebrospinal fluid elucidate heterogeneity in response between participants and administration routes. These early results underscore the promise of this therapeutic approach for patients with H3K27M-mutated DIPG or spinal cord DMG.

Abstract:TPS2662 Background: Glioblastoma (GBM) is the most common and most aggressive primary brain tumor. Around 294,900 new cases are diagnosed globally with 241,000 deaths each year. The 5-year survival is only 5%. Median overall survival from first recurrence is only 5-8 months. There is no established standard of care for recurrent GBM. City of Hope (COH) has developed and optimized a CAR T cell therapy utilizing the chlorotoxin peptide (CLTX) as the CAR’s tumor recognition domain against GBM. CLTX-CAR T cells specifically and broadly target GBM through recognition of a receptor complex including membrane-bound matrix metalloprotease 2 (MMP-2). CLTX-CAR T cells do not exhibit off-tumor recognition of normal human or murine cells and tissues in preclinical models. In in vitro studies, COH evaluated patient-derived brain tumor (PBT) cell lines for CLTX binding and expression of IL13Rα2, HER2 and EGFR, three targets of CAR T cell trials for GBMs. Strong CLTX binding to tumor cells was observed in of the majority of primary GBM lines, independent of these other antigens. In preclinical studies using in vivo mouse models, a single intratumoral (ICT) injection of CLTX-CAR T cells (1×10 6 CAR+ T cells) exhibited robust anti-tumor activity against ffLuc+ PBT106 tumors orthotopically-engrafted in NSG mice. Overall, when compared to mice treated with mock-transduced Tn/mem (no CAR) T cells, the CLTX(EQ)28ζ/CD19t+ T cells reduced tumor burden and significantly increased survival. Taken together, these preclinical findings support the potential safety and efficacy of CLTX-CAR T cells, and provide the rationale for clinical testing of this therapy. As cellular heterogeneity intrinsic to GBM likely contributes to resistance to therapy and limited response rates, CLTX-CAR T cells may provide greater tumor eradication in a higher proportion of patients with GBM. Methods: This study is a phase 1, single center, safety and maximum tolerated dose (MTD) finding study of CLTX-CAR T cells for subjects with MMP2+ recurrent or progressive GBM. A safety lead-in of 3−6 participants receiving CLTX-CAR T cells by ICT delivery will be completed first. Subsequently, subjects would receive cells administered through both ICT and intraventricular (ICV catheters) (i.e. dual delivery) in two dose schedules. Subjects will be evaluated for safety and tolerability, and may continue to receive treatment until disease progression. Time to progression, overall survival, and disease response by Response Assessment in Neuro-Oncology (RANO) criteria, will be evaluated and descriptively compared to historical data. Abstract:Introduction: Patients with relapsed or refractory (R/R) B-cell non-Hodgkin lymphoma (NHL) have poor outcomes with currently available strategies. Based on the promising results seen in patients treated with anti-CD19 CAR T-cell therapy in R/R B-NHL, we initiated the phase I study evaluating the safety and efficacy in patients with R/R B-NHL (NCT02842138). Autologous T cells were genetically modified to express a chimeric antigen receptor consisting of an anti-CD19-scFv domain with CD3ζ and 4-1BB signaling domains. Methods: Patients received CAR T-cell infusion after a conditioning regimen of cyclophosphamide (250 mg/m2) and fludarabine (25 mg/m2) daily for 3 days. Three CAR T cell doses were tested: 5 × 104, 1 × 105, and 1 × 106 CAR T cells/kg, respectively. Primary endpoints include safety and pharmacokinetics (PK) of CAR T cells. Secondary endpoints include complete and overall response (CR, OR) rates and duration of response (DOR) per International Working Group Criteria (Cheson, J ClinOncol2007). Results: As of March 2017, 14 patients were enrolled, 10 patients underwent response evaluation. Median age of the 10 evaluable patients was 37 years (range 24–68), 4 females and 6 males, 3 follicular lymphoma (FL) and 7 diffuse large B-cell lymphoma (DLBCL). Median number of prior therapies was 2.5 (range 2–7). The first group of three patients (2 FL and 1 DLBCL) received 5 × 104 CAR T cells/kg. Two FL patients achieved partial remission (PR) on day 28, and one of the patients achieved complete remission (CR) 3 months later. The second group of three patients (1 FL and 2 DLBCL) received 1 × 105 CAR T cells/kg. The FL patient achieved PR on day 28. The last group of four patients (all were diagnosed with DLBCL) received 1 × 106 CAR T cells/kg. Three patients achieved PR on day 28 (75%). One patient attained PD on day 28 and died 10 days later due to disease progression. A significant CAR T-cell expansion was detected in this case. On day 26, 26.8% of her PBMC were CAR T cells. Interestingly, PD1 expression was significant in her expanded CAR T cells. Approximately 78.3% of CD8+ CAR T cells and 71.4% of CD4+ CAR T cells expressed PD1 on day 26, as compared to 22% and 42% on day 13, respectively. Increased PD1-expressing CAR T cells were also found in other patients' peripheral blood. No serious adverse event (sAE) was observed in all patients. Two patients experienced mild fever (grade 1–2). In vivo expansion of CAR T cells was detected in all patients between day14 and 28. All responding patients are still in remission at the last follow-up (Range: 2.5–9 months). Conclusions: This study demonstrated early promising activity of anti-CD19 CAR T cells in patients with R/R B-NHL. The toxicities of CRS were mild and manageable. Our study also provided the first clinical evidence that expanded CAR T cells could express PD1. Blocking PD1 pathway may enhance the effector function of CAR T cells and improve the clinical outcomes of CAR T-cell therapy. Keywords: B-cell lymphoma; CD19; immune system.
