Abstract:PURPOSE:To observe the effect of excimer laser photorefractive keratectomy (PRK) correcting myopia and astigmatism, and to study the related factors.METHOD:PRK was performed with the aim to correct myopia errors from 1.00 to 16.00 diopters on 529 myopia eyes (306 patients). All eyes were divided into two groups: Group I: -1.00-6.00 D(294 eyes); group II: -6.25-16.00 D(235 eyes). All the patients were followed up for 3 to 12 months.RESULTS:At postoperative 12 months the rates of uncorrected visual acuity more than or equal to 10/20 or 20/20 were 97.5%, 77.5% in group I, and 80.3%, 32.8% in group II respectively; 92.5% of eyes were within +/- 1.00 diopter of the desired emmetropia in group I, but 42.6% in group II; The influential factors of changes of postoperative diopters was the severity of corneal haze, methods of removing corneal epithelium, intraocular pressure, the central corneal thickness.CONCLUSION:The excimer laser PRK is regarded to be a very predictable, safe, stable, and effective method to correct myopia up to -6.00 D. Prior to operation, the plan would be disigned according to diopters, ages, corneal thickness intraocular pressure. During operation, the corneal epithelium bad better to be removed by laser.
Characteristics and Correlative Factors of Posterior Corneal Surface Changes after Laser in Situ Keratomileusis

Abstract:OBJECTIVE:To assess the changes of the posterior corneal surface and the factors influencing these changes after laser in situ keratomileusis (LASIK). These factors included the preoperative refraction, preoperative intraocular pressure (IOP), preoperative corneal thickness, spherical equivalent correction and one-week postoperative corneal thickness.METHODS:The posterior corneal radius of curvature for the posterior best fit sphere (PBFS), irregularity of posterior surface, and anterior chamber depth (ACD) were obtained with the scanning slit topography system before and 1 week, 1 month and 3 months after LASIK in 64 eyes of 34 patients with myopic refractive errors of -2.25 to -12.5 diopters. Multiple regression analysis was used to assess the factors that affecting these changes of posterior corneal surface. The difference of radius PBFS and posterior elevation difference (PED) was used as an indicator for the forward shift of posterior corneal surface.RESULTS:The ACD before the surgery and 1 week, 1 month and 3 months after the surgery was (3.2448 +/- 0.2806), (3.2770 +/- 0.2810), (3.2579 +/- 0.2932) and (3.2651 +/- 0.2875) mm, respectively. The PBFS at that time was (6.5095 +/- 0.2177), (6.3731 +/- 0.2127), (6.4257 +/- 0.2358) and (6.4354 +/- 0.2266) mm, respectively. The posterior corneal irregularity at 5 diameters zone was 0.622 +/- 0.142, 0.978 +/- 0.244, 0.884 +/- 0.207 and 0.881 +/- 0.174, respectively The preoperative posterior corneal radius of curvature for PBFS, the irregularity of the posterior surface and the ACD were significantly different from that of postoperative data at each time point (P < 0.01). There was no significant difference in these indexes between 1 month and 3 months postoperatively. Multiple regression analysis showed that factors relevant to the changes of posterior corneal surface were the corrected preoperative refraction (regression coefficient r = 0.0173, P = 0.005) and 1 week postoperative corneal thickness (mm) (regression coefficient r = -1.495, P = 0.001).CONCLUSIONS:The posterior corneal surface after LASIK become steeper and more irregular and remains stable in three months after the surgery. Eyes with thinner corneas and higher myopia requiring more ablation are more predisposed to the changes of posterior corneal surface. The results of the present study should not be used in extremely high degree myopia. A study using a larger group of patients with a longer observation period is required.
Agreement Between Predicted and Measured Ablation Depth After Femtosecond Laser-Assisted LASIK for Myopia

Abstract:Purpose: To investigate agreement between the predicted ablation depth calculated by the EX500 excimer laser (Wavelight Laser Technologie AG, Erlangen, Germany) and the measured ablation depth in eyes that have undergone femtosecond laser-assisted LASIK (FS-LASIK) for myopia. Methods: Corneal thickness was measured with a rotating Scheimpflug camera preoperatively and 3 months postoperatively and the difference between these values was defined as the measured ablation depth. The difference between the predicted and the measured ablation depth was defined as the difference in ablation depth (ΔAD). Results: In 85 eyes of 85 patients, no statistically significant difference was detected between the mean predicted ablation depth (66.33 ± 24.15 µm) and the measured ablation depth at the thinnest corneal location (67.04 ± 30.94 µm), the corneal apex (67.52 ± 31.22 µm), or the pupil center (67.73 ± 31.48 µm). Bland-Altman plots revealed moderate agreement for measurements at the thinnest point (95% limits of agreement [LoA]: -25.13 to 23.70 µm), corneal apex (95% LoA: -24.70 to 22.33 µm), and pupil center (95% LoA: -25.30 to 22.51 µm), with a proportional bias between the average ablation depth and ΔAD. The predicted ablation depth was overestimated in eyes with lower correction and underestimated in eyes with higher correction. Conclusions: Moderate agreement between the predicted and measured ablation depth warrants caution when planning myopic FS-LASIK and calculating the residual bed thickness and percent tissue altered. When higher amounts of correction are planned, the laser software may underestimate the predicted ablation depth.
Comparison of High Order Aberration after Conventional and Customized Ablation in Myopic LASIK in Different Eyes of the Same Patient.

Abstract:Purpose: To verify whether there exists any difference in higher order aberrations after undergoing myopic LASIK (laser in situ keratomileusis) with conventional ablation and customized ablation in different eyes of the same patient. Methods: This was a prospective randomized study of 54 myopic eyes (27 patients) that underwent LASIK using the Nidek EC-5000 excimer laser system (Nidek, Gamagori, Japan). Topography-guided customized aspheric treatment zone (CATz) was used in the first eye of the patient (study group) and the other eye of the same patient was operated on with conventional ablation (control group). Higher order aberrations [root-mean-square (RMS) in the 5-mm zone] of both groups were observed with the Nidek OPD-Scan aberrometer before and 3 months after LASIK. Preoperative mean refractive error was similar between two eyes of the same patient (t=−0.577, P>0.05). Results: Preoperatively, higher order aberrations (RMS in the 5-mm zone) in the CATz ablation and conventional groups were (0.3600±0.0341) μm and (0.2680±0.1421) μm, respectively. This difference was not statistically significant (t=1.292, P>0.05). Three months after LASIK, higher order aberrations (RMS in 5-mm zone) in the CATz ablation and conventional groups were (0.3627±0.1510) μm and (0.3991±0.1582) μm, respectively. No statistically significant difference was noted between pre-and postoperative higher order aberrations in the CATz group (t=−0.047, P>0.05). However, a statistically significant increase in higher order aberrations was observed after conventional ablation (t=−5.261, P<0.05). A statistically significant difference was noted in the increase of higher order aberrations after LASIK between groups (t=−2.050, P=0.045). Conclusion: LASIK with conventional ablation and topography-guided CATz ablation resulted in the same BSCVA (best spectacle-corrected visual acuity) 3 month after LASIK. Higher order aberrations were increased, but the increase of higher order aberrations after customized ablation treatment was less than that after conventional ablation.
[A clinical study of actual corneal ablation depth in laser in situ keratomileusis]

Abstract:Purpose: To assess the predictability of corneal ablation depth in LASIK using NIDEK EC5000 excimer laser. Methods: Standard LASIK surgery was performed in 79 myopic patients with or without astigmatism with the NIDEK EC5000 excimer laser system.Ultrasonic corneal pachymetry was performed immediately after flap creation and after laser ablation during LASIK procedure,by which the actual corneal ablation depth was calculated.The values of actual and predicted ablation depth were compared. Results: The actual ablation depth was (92.32+/-29.86)μm, the predicted ablation depth was (74.16+/-25.95)μm.The differences between them(18.16+/-14.71)μm were statistically significant(p<0.001).Linear regression suggested that the actual ablation depth correlated closely with the predicted ablation depth(r=0.87,p<0.001).The regression model was Y=18.06+1.001X.The differences remained statistically significant and were independent of the levels of preoperative corneal keratometry,absolute preoperative spherical equivalent and the preoperative central cornea thickness. Conclusions: The actual ablation depth was about(18.16+/-14.71)μm thicker than the predicted ablation depth in the NIDEK EC5000 excimer laser system. We may have to take into account this deviation in order to ensure sufficient thickness of residual stromal bed.

Abstract:Abstract Background To compare the clinical outcomes of transepithelial photorefractive keratectomy (TPRK) with femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for correction of high myopia. Methods In this prospective, non-randomised, cohort study, 85 eyes of 46 patients treated with TPRK and 80 eyes of 42 patients treated with FS-LASIK were included. All eyes were highly myopic (spherical equivalent refraction <− 6.00 diopters). Both TPRK and FS-LASIK were performed by Schwind Amaris 750S excimer laser. Visual acuity, refraction, corneal high order aberration (HOA) and other variables were analyzed before and at 1, 3, 6, 12 months after surgery. Results At 12 months after surgery, uncorrected logMAR distance visual acuity (UDVA) in the TPRK and FS-LASIK groups was − 0.04 ± 0.04 and − 0.01 ± 0.08, respectively ( P = 0.039). Corrected logMAR distance visual acuity (CDVA) was − 0.06 ± 0.05 and − 0.04 ± 0.05 in both groups ( P = 0.621). For UDVA, 86% of eyes in the TPRK group and 80% in the FS-LASIK group remained unchanged or improved one or more logMAR lines ( P = 0.314), compared to preoperative CDVA. For CDVA, 97% of eyes in the TPRK group and 90% in the FS-LASIK group remained unchanged or improved one or more lines ( P = 0.096), compared to preoperative CDVA. Spherical equivalent refraction was − 0.05 ± 0.39 and − 0.26 ± 0.47 in both groups ( P = 0.030). 87% of eyes in the TPRK group and 73% in the FS-LASIK group achieved ±0.50 D target refraction ( P = 0.019). All 85 eyes (100%) in the TPRK group and 75 eyes (92%) in the FS-LASIK group were within ±1.00 D of target ( P = 0.003). Root mean square (RMS) of corneal total HOA and vertical coma in the TPRK group were lower compared with the FS-LASIK group ( P < 0.001 for both variables). Conclusions TPRK and FS-LASIK showed good safety, efficacy and predictability for correction of high myopia. Clinical outcomes of TPRK were slightly better than FS-LASIK.

Preoperative Refraction, Age and Optical Zone As Predictors of Optical and Visual Quality after Advanced Surface Ablation in Patients with High Myopia: a Cross-Sectional Study.

Abstract:Objective To investigate the factors associated with optical and visual quality of advanced surface ablation in high myopia. Design A cross-sectional study of high myopic eyes treated with laser epithelial keratomileusis (LASEK)/ epipolis laser in situ keratomileusis (Epi-LASIK). Setting6 Eye and ENT Hospital of Fudan University in Shanghai. Methods One hundred and thirty-eight high myopic eyes (138 patients) (myopia -6 D or more) were examined more than 12 months after LASEK or Epi-LASIK with advanced surface ablation on the MEL 80 excimer laser (Zeiss AG, Jena, Germany). Refraction, higher order aberrations (HOAs) and contrast sensitivity before and after surgery were evaluated. Factors including preoperative refraction, age, gender, central corneal thickness, pupil size, optical diameter, ablation depth and flap creation method were analysed for association with postoperative high-order aberration, contrast and glare sensitivities, and different analytic diameters. Results HOAs increased significantly postoperatively (p<0.05), with the most significant change found in Z(spherical aberration). At a 5mm analysis diameter, increased coma was associated with age; increased spherical aberration difference was associated with age, optical zone diameter and method of epithelial flap creation. At a 3 mm analysis diameter, none of the factors contributed to changes in HOAs. Higher preoperative refractive error was associated with decreased contrast and glare sensitivity at each spatial frequency. Conclusion A larger optical zone diameter design is recommended to achieve better visual quality in advanced surface ablation for high myopia correction. Age and preoperative refraction may help predict postoperative visual quality.
Comparison on biomechanical stability between femtosecond laser assisted LASIK and LASEK with ocular response analyzer

Abstract:AIM: To compare the difference of biomechanical stability after a femtosecond laser-assisted in situ keratomileusis(FS-LASIK)and laser-assisted subepithelial keratomileusis(LASEK)by ocular response analyzer(ORA). METHODS: This prospective study was conducted at the First Affiliated Hospital of Harbin Medical University, and myopic patients with the equivalent sphere between -2.00 and -5.00 diopters from January 2016 and December 2017 were enrolled. All the subjects were divided into FS-LASIK and LASEK group respectively according to different surgical methods. There were 64 patients(100 eyes)participated in the FS-LASIK group while 53 patients(100 eyes)in the LASEK surgery group. Corneal hysteresis value(CH)and corneal resistance factor value(CRF)were measured pre-, 1mo and 3mo postoperation of all the subjects. And the variance analysis of two groups was repeated to compare the overall difference between the two modes of operation from pre-operation to post operation. RESULTS: The CH and CRF value of the two groups were different before and after operation(P<0.05). The comparison results of repeated measurements showed that the CH value and CRF value of the surgical methods were reduced in 1 and 3mo after operation, respectively, and there was significance(P<0.05). The CH and CRF of FS-LASIK group was lower than LASEK group only in 1mo after the operation(P<0.05). CONCLUSION: Both surgeries could affect the biomechanical stability of cornea. From the point of view of biomechanical stability, LASEK operation is safer than FS-LASIK operation and reduces the possibility of postoperative refractive regression.
The Influence of Corneal Ablation Patterns on Prediction Error after Cataract Surgery in Post-Myopic-lasik Eyes

Abstract:Abstract Purpose To evaluate the influence of corneal ablation patterns on the prediction error after cataract surgery in post-myopic-LASIK eyes. Methods Eighty-three post-myopic-LASIK eyes of 83 patients that underwent uneventful cataract surgery were retrospectively included. Predicted postoperative spherical equivalence (SE) was calculated for the implanted lens using the Haigis-L and Barrett True-K formula. Prediction error at one month postsurgery was calculated as actual SE minus predicted SE. For each eye, area and decentration of the ablation zone was measured using the tangential curvature map. The associations between prediction errors and corneal ablation patterns were investigated. Results The mean prediction error was − 0.83 ± 1.00 D with the Haigis-L formula and − 1.00 ± 0.99 D with the Barrett True-K formula. Prediction error was positively correlated with keratometry (K) value and negatively correlated with ablation zone area using either formula, and negatively correlated with decentration of the ablation zone using the Barrett True-K formula (all P < 0.05). In the K < 37.08 D group, prediction error was negatively correlated with decentration of the ablation zone with both formulas (all P < 0.05). Multivariate analysis showed that with the Haigis-L formula, prediction error was associated with axial length (AL), K value and decentration, and with the Barrett True-K formula, prediction error was associated with AL and decentration (all P < 0.05). Conclusion A flatter cornea, larger corneal ablation zone and greater decentration will lead to more myopic prediction error after cataract surgery in post-myopic-LASIK eyes.
Evaluation of the Difference Between Predicted and Measured Central Corneal Thickness Reduction after SMILE and Femtosecond Laser-assisted LASIK for Myopia.

Abstract:Purpose: To evaluate the agreement between the predicted and measured central corneal thickness (CCT) reduction after the small incision lenticule extraction (SMILE) surgery and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) surgery. Methods: A total 165 patients were enrolled in this prospective study. Eighty patients with a mean spherical equivalent (SE) of -4.72 +/- 1.80 Diopters (D) were treated with the FS-LASIK procedure and Eighty-five patients with a mean SE of -4.78 +/- 1.63 D were treated with SMILE procedure. One eye for each patient was randomly selected and included for statistical analysis. Ultrasound pachymetry measurement and Scheimpflug camera corneal topography were performed preoperatively and 3 months postoperatively. The measured CCT reduction was calculated by comparing the preoperative examinations with postoperative examinations. Comparative statistics and linear regression analyses were performed. Results: The mean predicted CCT reduction was 95.02 +/- 21.39 mu m in FS-LASIK group and 103.49 +/- 22.87 mu m in SMILE group (P = .015). The prediction of laser platform was found to overestimate the measured CCT reduction for both FS-LASIK group (ultrasound 13.20 +/- 9.34 mu m) and SMILE group (ultrasound 13.12 +/- 8.68 mu m). The prediction of laser platform was found to systematically overestimate the measured CCT reduction in FS-LASIK group. In SMILE group, the difference between predicted and measured CCT reduction were found significantly related with the predicted CCT reduction (P P = .004 for Pentacam). Conclusion: A systematic overestimation of measured CCT reduction in FS-LASIK group did not influence the refractive precision of FS-LASIK. Due to the different biomechanical distributions in post-SMILE cornea, the measured CCT reduction was influenced as the changes in refractive correction. Nomogram adjustment for high myopic correction needs further research.
Comparison of the Effects among Three Different Corneal Refractive Surgeries for Low and Moderate Myopia

Abstract:Objective To compare postoperative uncorrected visual acuity (UCVA) and residual diopters among three different corneal refractive surgeries for low and moderate myopia.Methods This is a retrospective study on non-randomized clinical trial without control group.The following three groups were involved in this trial:LASEK group,261 patients (521 eyes);microkeratome-assisted flap creation LASIK (MM-LASIK) group,1513 patients (3024 eyes) and the femtosecond laser-assisted flap creation LASIK (FS-LASIK) group,1082 patients (2164 eyes).Single factor analysis of variance was used to compare the postoperative UCVA,residual spherical equivalent (S E),root mean aquare of high aberration for pupil diameter of 3mm and 5mm (HOA RMS3 and HOA RMS5) groups.Results After the follow-up of 12 months,the difference in UCVA between MM-LASIK and LASEK group was not statistically significant (P =0.705).The UCVA in MM-LASIK and LASEK group were better than that in FS-LASIK group and the differences were statistically significant (F =17.08,P =0.00).The differences in postoperative residual S E among the three groups were statistically significant (F =4.36,P =0.01).The residual SE in MMLASIK and FS-LASIK group were lower than that in LASEK group.The descending order of HOA RMS3 was FS-LASIK group,MM-LASIK and LASEK group.There was no statistically significant difference in HOA RMS5 among three groups.Conclusion Three corneal refractive surgeries for correcting low to moderate myopia brought good results.Smaller postoperative residual SE and lower HOA RMS3 are beneficial to the recovery of postoperative UCVA.LASEK and MM-LASIK produced better visual acuity and less corneal aberrations compared to FS-LASIK.The value of HOA RMS3 in LASEK group was the smallest and the postoperative residual SE in LASEK group was the biggest.
[The Effect of the Flap Thickness on Forward Shift of Posterior Corneal Surface in Excimer Laser Surgery Correcting High Myopia].

Abstract:OBJECTIVE:To investigate the influence of flap thickness of laser in situ keratomileusis (LASIK) and subepithelial keratomileusis on the forward shift of the posterior corneal surface in high myopia METHODS:It was a case-control study. In this study, 151 right high myopic eyes of patients [spherical equivalent range - 6.00 to - 10.00 diopter, 46 subepithelial keratomileusis eyes (epithelial sheet group), 64 LASIK eyes with 110 microm flap thickness (thin flap group) and 41 LASIK eyes with 130 microm flap thickness (normal flap group)] were evaluated preoperatively and postoperatively with the Pentacam to determine the shape change in the posterior corneal surface. Changes in posterior shape were calculated by comparing refraction and elevation of posterior corneal surface in the part corresponding to the pupil center preoperatively and postoperatively. RESULTS:The mean estimated residual bed thickness (RBT) of epithelial sheet eyes, thin flap eyes and normal flap eyes was (362 +/- 23), (320 +/- 25) and (300 +/- 21) microm, respectively. The RBT difference between epithelial sheet group and thin or normal flap group was statically significant (F = 85.685, P <0.001) and the difference between 2 LASIK groups was also significant (P < 0.05). There was no significant difference between three groups in ablation depth (P > 0.05). The mean posterior corneal refraction and elevation change of epithelial sheet eyes, thin flap eyes and normal flap eyes in the pupil center was (-0.12 +/- 0.14)D and (0.9 +/-1.8) microm, ( -0.20 +/- 0.22)D and (1.3 +/- 1.6) microm, (-0.21 +/- 0.18)D and (1.5 +/- 1.5) microm, respectively. A significant correlation between posterior elevation and refraction change was present in all 3 groups. The amount of posterior corneal elevation change correlated significantly with the ablation depth in thin flap group and normal flap group (r = 0.265, 0.311; P <0.05). CONCLUSION:In treating high myopia, both subepithelial ablation and LASIK surgery induce tiny forward shift of posterior corneal surface when the RBT was thick enough. Deeper ablation or thicker corneal flap may increase the trend of forward shift of posterior corneal surface.

Abstract:ABSTRACT Proteins located in the tegument layer of herpesvirus particles play important roles in the replicative cycle at both early and late times after infection. As major constituents of the virion, they execute important functions in particular during formation of progeny virions. These functions have mostly been elucidated by construction and analysis of mutant viruses deleted in single or multiple tegument protein-encoding genes (reviewed in the work of T. C. Mettenleiter, Virus Res. 106:167-180, 2004). However, since tegument proteins have been shown to be involved in numerous protein-protein interactions, the impact of single protein deletions on the composition of the virus particle is unknown, but they could impair correct interpretation of the results. To analyze how the absence of single virion constituents influences virion composition, we established a procedure to assay relative amounts of virion structural proteins in deletion mutants of the alphaherpesvirus Pseudorabies virus (PrV) in comparison to wild-type particles. The assay is based on the mass spectrometric quantitation of virion protein-derived peptides carrying stable isotope mass tags. After deletion of the US3, UL47, UL49, or glycoprotein E gene, relative amounts of a capsid protein (UL38), a capsid-associated protein (UL25), several tegument proteins (UL36 and UL47, if present), and glycoprotein H were unaffected, whereas the content of other tegument proteins (UL46, UL48, and UL49, if present) varied significantly. In the case of the UL48 gene product, a specific increase in incorporation of a smaller isoform was observed after deletion of the UL47 or UL49 gene, whereas a larger isoform remained unaffected. The cellular protein actin was enriched in virions of mutants deficient in any of the tegument proteins UL47, UL49, or US3. By two-dimensional gel electrophoresis multiple isoforms of host cell-derived heat shock protein 70 and annexins A1 and A2 were also identified as structural components of PrV virions.
Ray-tracing-Guided or Q-Value-Adjusted FS-LASIK for Correction of Myopia and Myopic Astigmatism: A Comparative Contralateral Eye Study.

Abstract:PURPOSE:To compare the safety and efficacy of individualized ray-tracing-guided femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for correction of myopia and myopic astigmatism. METHODS:This prospective, randomized, double-blind, contralateral eye study included 68 eyes of 34 patients with myopia or myopic astigmatism requiring FS-LASIK treatment. For each patient, one eye was randomly assigned to receive the ray-tracing-guided treatment, whereas the contralateral eye underwent Q-value-adjusted ablation. Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction spherical equivalent (MRSE), sphere, cylinder, effective optical zone (EOZ), and 6-mm corneal aberrations were measured and analyzed before operation and after a 3-month postoperative follow-up. RESULTS:At 3 months postoperatively, the UDVA of 20/16 or better was measured in 94% of eyes in the ray-tracing group and 85% of eyes in the Custom-Q group (P = .064). Forty-seven percent in the ray-tracing group and 32% in the Custom-Q group gained one or more Snellen lines of CDVA (P = .043). The MRSE, refractive astigmatism, surgically induced astigmatism, and difference vector were better in the Custom-Q group (P < .05). The postoperative corneal HOAs and optical path difference were significantly better in the ray-tracing group (P = .008). The EOZs of the ray-tracing and Custom-Q groups were 5.77 and 5.43 mm (P < .001), and the average ablation depths of the ray-tracing and Custom-Q groups were 100.97 and 85.24 µm (P < .001), respectively. CONCLUSIONS:Despite the overcorrection and excessive ablation of corneal tissue, ray-tracing-guided FS-LASIK in clinical practice was found to be safe and effective for myopic correction both with and without astigmatism, particularly in achieving UDVA and inducing fewer corneal HOAs and less OPD. A comparative analysis of the effectiveness of laser correction with a mechanical and laser microkeratome for myopia

Abstract:Purpose: Myopia, or nearsightedness, is one of the most common eye conditions worldwide. However, a comparison of the effectiveness of different laser-assisted interventions is lacking. Thus, we aimed to compare the efficacy and safety of LASIK and IntraLASIK in addressing myopia. Methods: The study was conducted in two ophthalmology clinics in Beijing, China, in 2022. A total of 84 patients (152 eyes) with different degrees of myopia were examined and underwent LASIK (n=46, 80 eyes) or IntraLASIK (n=38, 72 eyes). Keratometry, corneal topography, pachymetry, visual acuity evaluation, and corneal biomechanical analysis were performed before and after the intervention. Results: IntraLASIK produced more precise flaps than LASIK, with deviations of <8 mm and 0.1 mm from the intended thickness and diameter, respectively. LASIK resulted in nonuniform flaps, with thickness deviations of 5-86 mm. IntraLASIK demonstrated a superior efficacy for patients with severe myopia and thin corneas, with a mean spherical equivalent of 0.9 D at 6 months compared to the 1.4 D for LASIK. Approximately 91% and 83% of the patients with mild to moderate and severe myopia, respectively, achieved results within ± 0.49 D from the refractive target with IntraLASIK. Conclusions: Corneal hysteresis and corneal resistance factor decreased with an increase in laser intensity, and they decreased faster with thinner corneas. Thus, IntraLASIK is more useful than LASIK in patients with thin corneas and severe myopia.

Abstract:PURPOSE:To assess the influence of the origin of astigmatism on the correction of myopic astigmatism by laser-assisted subepithelial keratectomy (LASEK).SETTING:Ophthalmology Department, Eye and ENT Hospital, Shanghai, China.DESIGN:Prospective study.METHODS:Patients having LASEK to correct myopia or myopic astigmatism were divided into 2 groups according to their ocular residual astigmatism (ORA). Patients were examined preoperatively and 1 and 3 months postoperatively. The efficacy of LASEK was compared between those with and those without a significant amount of intraocular astigmatism.RESULTS:The study comprised 54 eyes of 54 patients. The mean index of success (ratio of magnitude of remaining uncorrected astigmatism to that of initial preoperative astigmatism) in the high ORA group (n=21) and low ORA group (n=33) was 0.85 and 0.48, respectively, 1 month after surgery (t=2.17, P=.04) and 0.88 and 0.32, respectively, 3 months after surgery (t=2.18, P=.04). The Zernike coefficient of horizontal coma, Z(3,+1), increased more after surgery in the high ORA group than in the low ORA group (1 month versus preoperative, t=2.32, P=.024; 3 months versus preoperative, t=2.07, P=.048).CONCLUSIONS:Nine percent and 2% of the eyes had minimal corneal haze at 1 month and 3 months, respectively. Laser-assisted subepithelial keratectomy was less effective in correcting myopic astigmatism when astigmatism was mainly located at the internal optics. Differences in the early biomechanical effects of hyperopic and myopic laser in situ keratomileusis

Abstract:Purpose: To compare changes in corneal hysteresis (CH) and the corneal resistance factor (CRF) in myopic and hyperopic laser in situ keratomileusis (LASIK) and evaluate their relationship to the number of photoablative pulses delivered, a surrogate for ablation volume. Setting: Cleveland Clinic Cole Eye Institute, Cleveland, Ohio, USA. Methods: Preoperative and 1-week postoperative Ocular Response Analyzer measurements in eyes that had femtosecond-assisted LASIK were studied retrospectively. Changes in CH and CRF were compared and tested for correlation with the number of excimer laser pulses. Results: Thirteen myopic eyes and 11 hyperopic eyes were evaluated. Preoperative corneal thickness, CH, CRF, programmed correction magnitude, flap thickness, and total number of fixed spot-size photoablative pulses were similar in the 2 groups (P>.1). Decreases in CH and CRF were greater after myopic LASIK than after hyperopic LASIK (P<.005), and changes in CRF were correlated with the number of excimer laser pulses in the myopic group only (r = -0.63, P = .02). Regardless of ablation profile, changes in CH were more strongly correlated with preoperative CH values than with attempted ablation volume. Conclusions: With comparable flap thickness and attempted ablation volumes, myopic photoablation profiles were associated with greater decreases in CRF and CH than hyperopic profiles. Results indicate that preoperative corneal biomechanical status, ablation volume, and the spatial distribution of ablation are important factors that affect corneal resistance and viscous dissipative properties differently. Preferential tissue removal in the natively thicker paracentral cornea in hyperopia may partially account for the rarity of ectasia after hyperopic LASIK.

Abstract:Background: To compare the changes in posterior corneal elevation after small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) in correcting myopia over -9 diopters (D). Methods: In this prospective comparative study, 82 eyes of 82 patients scheduled for refractive correction were recruited. Eyes were randomly assigned to the SMILE group (45 eyes, -10.43 +/- 0.92 D) or FS-LASIK group (37 eyes, -10.97 +/- 1.37 D). The posterior corneal surface was measured using a Scheimpflug camera (Pentacam, Oculus, Germany) preoperatively and at 1 day, 1 month, and 6 months after surgery. Posterior corneal elevation in the central point and central 4-mm area, and in various optical zones above the best-fit sphere, was analyzed. A P value of less than 0.05 was considered statistically significant. Results: All surgeries were completed successfully. The safety index and efficacy index were 1.20 and 1.00, respectively, in the SMILE group, and was 1.10 and 0.90, respectively, in the FS-LASIK group. No significant difference existed in all analyzed data before and at 6 months after surgery in both the SMILE group and the FS-LASIK group. Changes in posterior corneal elevation after FS-LASIK were greater than after SMILE, with no statistical significance (P >= 0.07). In the SMILE group, residual bed thickness was found to be moderately negatively correlated with changes in the elevation in the central area (P <= 0.045); whereas it was positively correlated in the peripheral area (P=0.002). Conclusions: SMILE and FS-LASIK presented stable posterior corneal surface in correction of myopia over -9.0 D at the follow-up visit of 6 months.

Abstract:Purpose: To study correlation among corneal asphericity, higher-order aberrations and visual performance for eyes of virgin myopia and postoperative laser in situ keratomileusis (LASIK). Methods: There were 320 candidates 590 eyes for LASlK treatment included in this study. The mean preoperative spherical equivalence was -4.35 +/- 1.51 D (-1.25 to -9.75), with astigmatism less than 2.5 D. Corneal topography maps and contrast sensitivity were measured and analyzed for every eye before and one year after LASIK for the analysis of corneal asphericity and wavefront aberrations. Results: Preoperatively, only 4th and 6th order aberration had significant correlation with corneal asphericity and apical radius of curvature (p < 0.001). Postoperatively, all 3th to 6th order aberrations had statistically significant correlation with corneal asphericity (p < 0.01), but only 4th and 6th order aberration had significant correlation with apical radius of curvature (p < 0.05). The asymmetrical aberration like coma had significant correlation with vertical offset of pupil center (p < 0.01). Preoperatively, corneal aberrations had no significant correlation with visual acuity and area under the log contrast sensitivity (AULCSF) (P > 0.05). Postoperatively, corneal aberrations still didn't have significant correlation with visual acuity (P > 0.05), but had significantly negative correlation with AULCSF (P < 0.01). Corneal asphericity had no significant correlation with AULCSF before and after the treatment (P > 0.05). Conclusions: Corneal aberrations had different correlation with corneal profile and visual performance for eyes of virgin myopia and postoperative LASIK, which may be due to changed corneal profile and limitation of metrics of corneal aberrations.
