Abstract:Abstract Dramatic alterations in brain morphology, such as cortical thickness and sulcal folding, occur during the 3rd trimester of gestation which overlaps with the period of premature births. Here, we investigated the ability of the graph convolutional network (GCN) to predict brain age for preterm neonates by accounting for morphometrics measured on the cortical surface and the surface mesh topology as a sparse graph. Our findings demonstrate that GCN-based age prediction of preterm neonates (n=170; mean absolute error [MAE]: 1.06 weeks) outperformed conventional machine learning algorithms and deep learning methods that did not use topological information. We further evaluated how predicted brain age (PBA) emerges as a biologically meaningful index that characterizes the current status of brain development at the time of imaging. We hypothesized that the relative brain age (RBA; PBA minus chronological age) at scan reflects a combination of perinatal clinical factors, including preterm birth, birthweight, perinatal brain injuries, exposure to postnatal steroids, etc. We also hypothesized that RBA of neonatal scans may be associated with brain functional development in the future. To validate these hypotheses, we used general linear models. Furthermore, we established structural equation models (SEM) to determine the structural relationship between preterm birth (as a latent variable of birthweight and birth age), perinatal injuries (as a latent variable of three leading brain injuries), postnatal factors (as a latent variable of six clinical conditions), RBA at scan, and neurodevelopmental scores at 30 months. Our results suggest that low birthweight, chronic lung disease, and exposure to postnatal steroids impair cortical growth, as low RBA was significantly associated with these risks. Furthermore, RBA was associated with cognitive and language scores at 30 months. SEM analysis indicated that RBA mediated the influences of preterm birth and postnatal clinical factors, but not perinatal brain injuries, toward brain functional development at 30 months. The left middle cingulate cortex showed the most accurate prediction of brain age (MAE: 1.19 weeks), followed by left posterior and right middle cingulate cortices (1.21 weeks). These cingulate regions presented faster growth than others. RBAs of several frontal cortices significantly correlated with cognitive abilities at 30 months of age (n=50). Whereas, RBA of left Broca’s area, which is important for language production and comprehension, was associated with language functional scores. Overall, our results demonstrate the potential of the GCN in both predicting brain age and localizing regional growth that relates to postnatal factors and future neurodevelopmental outcome.

Abstract:Structural and functional connectomes undergo rapid changes during the third trimester and the first month of postnatal life. Despite progress, our understanding of the developmental trajectories of the connectome in the perinatal period remains incomplete. Brain age prediction uses machine learning to estimate the brain’s maturity relative to normative data. The difference between the individual’s predicted and chronological age—or brain age gap (BAG)—represents the deviation from these normative trajectories. Here, we assess brain age prediction and BAGs using structural and functional connectomes for infants in the first month of life. We used resting-state fMRI and DTI data from 611 infants (174 preterm; 437 term) from the Developing Human Connectome Project (dHCP) and connectome-based predictive modeling to predict postmenstrual age (PMA). Structural and functional connectomes accurately predicted PMA for term and preterm infants. Predicted ages from each modality were correlated. At the network level, nearly all canonical brain networks—even putatively later developing ones—generated accurate PMA prediction. Additionally, BAGs were associated with perinatal exposures and toddler behavioral outcomes. Overall, our results underscore the importance of normative modeling and deviations from these models during the perinatal period.

Deep Learning to Predict Neonatal and Infant Brain Age from Myelination on Brain MRI Scans

Abstract:Background Assessment of appropriate brain myelination on T1- and T2-weighted MRI scans is based on gestationally corrected age (GCA) and requires subjective visual inspection of the brain with knowledge of normal myelination milestones. Purpose To develop a convolutional neural network (CNN) capable of estimating neonatal and infant GCA based on brain myelination on MRI scans. Materials and methods In this retrospective study from one academic medical center, brain MRI scans of patients aged 0-25 months with reported normal myelination were consecutively collected between January 1995 and June 2019. The GCA at MRI was manually calculated. After exclusion criteria were applied, T1- and T2-weighted MRI scans were preprocessed with skull stripping, linear registration, z scoring for normalization, and downsampling. A three-dimensional regression CNN was trained to predict GCA using mean absolute error (MAE) as its loss function. Attention maps were calculated using layer-wise relevance propagation. Models were validated on an external test set from the National Institutes of Health (NIH). Model MAEs were compared using Kruskal-Wallis and Mann-Whitney tests. Results A total of 518 neonates and infants (mean GCA, 67 weeks ± 33 [SD], 56% male) was included, comprising 469 T1-, 438 T2-, and 389 T1- and T2-weighted studies. Across 10 runs, MAEs of T1-, T2-, and T1- and T2-weighted networks were 9.8 ± 2.3, 9.1 ± 1.9, and 7.7 ± 1.7 weeks, respectively. Attention map analysis demonstrated increased network attention to the cerebellum, posterior white matter, and basal ganglia signal in neonates with GCA of less than 40 weeks and the anterior white matter signal in infants with GCA of more than 120 weeks, corresponding to the known progression of myelination. The T1- and T2-weighted network tested on the external NIH test set had an MAE of 9.1 weeks, which was reduced to 5.9 weeks with further training using half the external test set (P < .001). Conclusion A three-dimensional convolutional neural network can predict the gestationally corrected age of neonates and infants aged 0-25 months based on brain myelination patterns on T1- and T2-weighted MRI scans. © RSNA, 2022 Online supplemental material is available for this article.

Abstract:Infancy is a critical period for the human brain development, and brain age is one of the indices for the brain development status associated with neuroimaging data. The difference between the predicted age based on neuroimaging and the chronological age can provide an important early indicator of deviation from the normal developmental trajectory. In this study, we utilize the Graph Convolutional Network (GCN) to predict the infant brain age based on resting-state fMRI data. The brain connectivity obtained from rs-fMRI can be represented as a graph with brain regions as nodes and functional connections as edges. However, since the brain connectivity is a fully connected graph with features on edges, current GCN cannot be directly used for it is a node-based method for sparse graphs. Hence, we propose an edge-based Graph Path Convolution (GPC) method, which aggregates the information from different paths and can be naturally applied on dense graphs. We refer the whole model as Brain Connectivity Graph Convolutional Networks (BC-GCN). Further, two upgraded network structures are proposed by including the residual and attention modules, referred as BC-GCN-Res and BC-GCN-SE to emphasize the information of the original data and enhance influential channels. Moreover, we design a two-stage coarse-to-fine framework, which determines the age group first and then predicts the age using group-specific BC-GCN-SE models. To avoid accumulated errors from the first stage, a cross-group training strategy is adopted for the second stage regression models. We conduct experiments on infant fMRI scans from 6 to 811 days of age. The coarse-to-fine framework shows significant improvements when being applied to several models (reducing error over 10 days). Comparing with state-of-the-art methods, our proposed model BC-GCN-SE with coarse-to-fine framework reduces the mean absolute error of the prediction from >70 days to 49.9 days. Abstract:Purpose of reviewRecent progress in elucidating the physiopathology of X-linked lymphoproliferative syndrome (XLP) has raised novel and important issues regarding the biology of natural killer T cells. Here I will review this information and discuss the issues involved. Recent findingsXLP is a rare inherited immunodeficiency characterized by a high susceptibility to severe infection by the Epstein–Barr virus. Mutations in the gene SH2D1A (or alternatively SAP) underlie 80% of familial XLP (XLP-1) cases. Recently the remaining 20% of familial XLP (XLP-2) cases were shown to harbor mutations in the gene XIAP (X-linked inhibitor of apoptosis protein). Both SAP and XIAP deficiencies are associated with a defect in the development and/or homeostasis of natural killer T cells. SummaryIt can be hypothesized that the susceptibility to Epstein–Barr virus in XLP might result from the defect of natural killer T cells. The role of these cells in viral infection is unclear, but several herpes viruses have developed strategies to escape natural killer T cells. The discovery that SAP and XIAP deficiency leads to a defect in natural killer T cells has also shed light on novel signaling pathways required for natural killer T cell development and/or homeostasis.
Predicting neurodevelopmental outcomes in extremely preterm neonates with low-grade germinal matrix-intraventricular hemorrhage using synthetic MRI

Abstract:Objectives: This study aims to assess the predictive capability of synthetic MRI in assessing neurodevelopmental outcomes for extremely preterm neonates with low-grade Germinal Matrix-Intraventricular Hemorrhage (GMH-IVH). The study also investigates the potential enhancement of predictive performance by combining relaxation times from different brain regions. Materials and methods: In this prospective study, 80 extremely preterm neonates with GMH-IVH underwent synthetic MRI around 38 weeks, between January 2020 and June 2022. Neurodevelopmental assessments at 18 months of corrected age categorized the infants into two groups: those without disability (n = 40) and those with disability (n = 40), with cognitive and motor outcome scores recorded. T1, T2 relaxation times, and Proton Density (PD) values were measured in different brain regions. Logistic regression analysis was utilized to correlate MRI values with neurodevelopmental outcome scores. Synthetic MRI metrics linked to disability were identified, and combined models with independent predictors were established. The predictability of synthetic MRI metrics in different brain regions and their combinations were evaluated and compared with internal validation using bootstrap resampling. Results: Elevated T1 and T2 relaxation times in the frontal white matter (FWM) and caudate were significantly associated with disability (p < 0.05). The T1-FWM, T1-Caudate, T2-FWM, and T2-Caudate models exhibited overall predictive performance with AUC values of 0.751, 0.695, 0.856, and 0.872, respectively. Combining these models into T1-FWM + T1-Caudate + T2-FWM + T2-Caudate resulted in an improved AUC of 0.955, surpassing individual models (p < 0.05). Bootstrap resampling confirmed the validity of the models. Conclusion: Synthetic MRI proves effective in early predicting adverse outcomes in extremely preterm infants with GMH-IVH. The combination of T1-FWM + T1-Caudate + T2-FWM + T2-Caudate further enhances predictive accuracy, offering valuable insights for early intervention strategies.

Abstract:The ability to determine if the brain is developing normally is a key component of pediatric neuroradiology and neurology. Brain magnetic resonance imaging (MRI) of infants demonstrates a specific pattern of development beyond simply myelination. While radiologists have used myelination patterns, brain morphology and size characteristics in determining if brain maturity matches the chronological age of the patient, this requires years of experience with pediatric neuroradiology. Due to the lack of standardized criteria, estimation of brain maturity before age three remains fraught with interobserver and intraobserver variability. An objective measure of brain developmental age estimation (BDAE) could be a useful tool in helping physicians identify developmental delay as well as other neurological diseases. We investigated a three-dimensional convolutional neural network (3D CNN) to rapidly classify brain developmental age using common MRI sequences. MRI datasets from normal newborns were obtained from the National Institute of Mental Health Data Archive from birth to 3 years. We developed a BDAE method using T1-weighted, as well as a fusion of T1-weighted, T2-weighted, and proton density (PD) sequences from 112 individual subjects using 3D CNN. We achieved a precision of 94.8% and a recall of 93.5% in utilizing multiple MRI sequences in determining BDAE.

Abstract:Early identification and intervention of abnormal brain development individual subjects are of great significance, especially during the earliest and most active stage of brain development in children aged under 3. Neuroimage-based brain's biological age has been associated with health, ability, and remaining life. However, the existing brain age prediction models based on neuroimage are predominantly adult-oriented. Here, we collected 658 T1-weighted MRI scans from 0 to 3 years old healthy controls and developed an accurate brain age prediction model for young children using deep learning techniques with high accuracy in capturing age-related changes. The performance of the deep learning-based model is comparable to that of the SVR-based model, showcasing remarkable precision and yielding a noteworthy correlation of 91% between the predicted brain age and the chronological age. Our results demonstrate the accuracy of convolutional neural network (CNN) brain-predicted age using raw T1-weighted MRI data with minimum preprocessing necessary. We also applied our model to children with low birth weight, premature delivery history, autism, and ADHD, and discovered that the brain age was delayed in children with extremely low birth weight (less than 1000 g) while ADHD may cause accelerated aging of the brain. Our child-specific brain age prediction model can be a valuable quantitative tool to detect abnormal brain development and can be helpful in the early identification and intervention of age-related brain disorders.

Abstract:The neurodevelopmental outcomes of preterm infants can be stratified based on the level of prematurity. We explored brain structural networks in extremely preterm (EP; < 28 weeks of gestation) and very-to-late (V-LP; ≥ 28 and < 37 weeks of gestation) preterm infants at term-equivalent age to predict 2-year neurodevelopmental outcomes. Using MRI and diffusion MRI on 62 EP and 131 V-LP infants, we built a multimodal feature set for volumetric and structural network analysis. We employed linear and nonlinear machine learning models to predict the Bayley Scales of Infant and Toddler Development, Third Edition (BSID-III) scores, assessing predictive accuracy and feature importance. Our findings revealed that models incorporating local connectivity features demonstrated high predictive performance for BSID-III subsets in preterm infants. Specifically, for cognitive scores in preterm (variance explained, 17%) and V-LP infants (variance explained, 17%), and for motor scores in EP infants (variance explained, 15%), models with local connectivity features outperformed others. Additionally, a model using only local connectivity features effectively predicted language scores in preterm infants (variance explained, 15%). This study underscores the value of multimodal feature sets, particularly local connectivity, in predicting neurodevelopmental outcomes, highlighting the utility of machine learning in understanding microstructural changes and their implications for early intervention.

Abstract:Abstract Magnetic resonance imaging offers unrivaled visualization of the fetal brain, forming the basis for establishing age-specific morphologic milestones. However, gauging age-appropriate neural development remains a difficult task due to the constantly changing appearance of the fetal brain, variable image quality, and frequent motion artifacts. Here we present an end-to-end, attention-guided deep learning model that predicts gestational age with R 2 score of 0.945, mean absolute error of 6.7 days, and concordance correlation coefficient of 0.970. The convolutional neural network was trained on a heterogeneous dataset of 741 developmentally normal fetal brain images ranging from 19 to 39 weeks in gestational age. We also demonstrate model performance and generalizability using independent datasets from four academic institutions across the U.S. and Turkey with R 2 scores of 0.81–0.90 after minimal fine-tuning. The proposed regression algorithm provides an automated machine-enabled tool with the potential to better characterize in utero neurodevelopment and guide real-time gestational age estimation after the first trimester.

Abstract:Background: Deep-learning-based brain age estimation using magnetic resonance imaging data has been proposed to identify abnormalities in brain development and the risk of adverse developmental outcomes in the fetal brain. Although saliency and attention activation maps have been used to understand the contribution of different brain regions in determining brain age, there has been no attempt to explain the influence of shape-related cortical structural features on the variance of predicted fetal brain age. Methods: We examined the association between the predicted brain age difference (PAD: predicted brain age–chronological age) from our convolution neural networks-based model and global and regional cortical structural measures, such as cortical volume, surface area, curvature, gyrification index, and folding depth, using regression analysis. Results: Our results showed that global brain volume and surface area were positively correlated with PAD. Additionally, higher cortical surface curvature and folding depth led to a significant increase in PAD in specific regions, including the perisylvian areas, where dramatic agerelated changes in folding structures were observed in the late second trimester. Furthermore, PAD decreased with disorganized sulcal area patterns, suggesting that the interrelated arrangement and areal patterning of the sulcal folds also significantly affected the prediction of fetal brain age. Conclusion: These results allow us to better understand the variance in deep learning-based fetal brain age and provide insight into the mechanism of the fetal brain age prediction model.

Abstract:Predicting brain age of children accurately and quantitatively can give help in brain development analysis and brain disease diagnosis. Traditional methods to estimate brain age based on 3D magnetic resonance (MR), T1 weighted imaging (T1WI), and diffusion tensor imaging (DTI) need complex preprocessing and extra scanning time, decreasing clinical practice, especially in children. This research aims at proposing an end-to-end AI system based on deep learning to predict the brain age based on routine brain MR imaging. We spent over 5 years enrolling 220 stacked 2D routine clinical brain MR T1-weighted images of healthy children aged 0 to 5 years old and randomly divided those images into training data including 176 subjects and test data including 44 subjects. Data augmentation technology, which includes scaling, image rotation, translation, and gamma correction, was employed to extend the training data. A 10-layer 3D convolutional neural network (CNN) was designed for predicting the brain age of children and it achieved reliable and accurate results on test data with a mean absolute deviation (MAE) of 67.6 days, a root mean squared error (RMSE) of 96.1 days, a mean relative error (MRE) of 8.2%, a correlation coefficient (R) of 0.985, and a coefficient of determination (R2) of 0.971. Specially, the performance on predicting the age of children under 2 years old with a MAE of 28.9 days, a RMSE of 37.0 days, a MRE of 7.8%, a R of 0.983, and a R2 of 0.967 is much better than that over 2 with a MAE of 110.0 days, a RMSE of 133.5 days, a MRE of 8.2%, a R of 0.883, and a R2 of 0.780.

Abstract:Background: Abnormal brain development is common in children with cerebral palsy (CP), but there are no recent reports on the actual brain age of children with CP. Objective: Our objective is to use the brain age prediction model to explore the law of brain development in children with CP. Methods: A two-dimensional convolutional neural networks brain age prediction model was designed without segmenting the white and gray matter. Training and testing brain age prediction model using magnetic resonance images of healthy people in a public database. The brain age of children with CP aged 5–27 years old was predicted. Results: The training dataset mean absolute error (MAE) = 1.85, r = 0.99; test dataset MAE = 3.98, r = 0.95. The brain age gap estimation (BrainAGE) of the 5- to 27-year-old patients with CP was generally higher than that of healthy peers ( p < 0.0001). The BrainAGE of male patients with CP was higher than that of female patients ( p < 0.05). The BrainAGE of patients with bilateral spastic CP was higher than those with unilateral spastic CP ( p < 0.05). Conclusion: A two-dimensional convolutional neural networks brain age prediction model allows for brain age prediction using routine hospital T1-weighted head MRI without segmenting the white and gray matter of the brain. At the same time, these findings suggest that brain aging occurs in patients with CP after brain damage. Female patients with CP are more likely to return to their original brain development trajectory than male patients after brain injury. In patients with spastic CP, brain aging is more serious in those with bilateral cerebral hemisphere injury than in those with unilateral cerebral hemisphere injury.

Abstract:Preterm birth adversely affects postnatal brain development. In order to investigate the critical gestational age at birth (GAB) that alters the developmental trajectory of gray and white matter structures in the brain, we investigated diffusion tensor and quantitative T2 mapping data in 43 term-born and 43 preterm-born infants. A novel multivariate linear model the change point model, was applied to detect change points in fractional anisotropy, mean diffusivity, and T2 relaxation time. Change points captured the "critical" GAB value associated with a change in the linear relation between GAB and MRI measures. The analysis was performed in 126 regions across the whole brain using an atlas-based image quantification approach to investigate the spatial pattern of the critical GAB. Our results demonstrate that the critical GABs are region- and modality-specific, generally following a central-to-peripheral and bottom-to-top order of structural development. This study may offer unique insights into the postnatal neurological development associated with differential degrees of preterm birth.

Abstract:The brain development during the perinatal period is characterized by rapid changes in both structure and function, which have significant impact on the cognitive and behavioral abilities later in life. Accurate assessment of brain age is a crucial indicator for brain development maturity and can help predict the risk of neonatal pathology. However, evaluating neonatal brains using magnetic resonance imaging (MRI) is challenging due to its complexity, multi-dimension, and noise with subtle alterations. In this paper, we propose a multi-modal deep learning framework based on transformers for precise post-menstrual age (PMA) estimation and brain development analysis using T2-weighted structural MRI (T2-sMRI) and diffusion MRI (dMRI) data. First, we build a two-stream dense network to learn modality-specific features from T2-sMRI and dMRI of brain individually. Then, a transformer module based on self-attention mechanism integrates these features for PMA prediction and preterm/term classification. Finally, saliency maps on brain templates are used to enhance the interpretability of results. Our method is evaluated on the multi-modal MRI dataset of the developing Human Connectome Project (dHCP), which contains 592 neonates, including 478 term-born and 114 preterm-born subjects. The results demonstrate that our method achieves a 0.5-week mean absolute error (MAE) in PMA estimation for term-born subjects. Notably, preterm-born subjects exhibit delayed brain development, worsening with increasing prematurity. Our method also achieves 95% accuracy in classification of term-born and preterm-born subjects, revealing significant group differences.

Abstract:Objective: Our objective was to determine the association of early brain maturation with neurodevelopmental outcome in premature neonates. Methods: Neonates born between 24 and 32 weeks' gestation (April 2006 to August 2010) were prospectively studied with MRI early in life and again at term-equivalent age. Using diffusion tensor imaging and magnetic resonance spectroscopic imaging, fractional anisotropy (FA) (microstructure) and N-acetylaspartate (NAA)/choline (metabolism) were measured from the basal nuclei, white matter tracts, and superior white matter. Brain maturation is characterized by increasing FA and NAA/choline from early in life to term-equivalent age. In premature neonates, systemic illness and critical care therapies have been linked to abnormalities of these measures. Of the 177 neonates in this cohort, 5 died and 157 (91% of survivors) were assessed at 18 months' corrected age (adjusted for prematurity) using the Bayley Scales of Infant and Toddler Development III motor, cognitive, and language composite scores (mean = 100, SD = 15). Results: Among these 157 infants, white matter injury was seen in 48 (30%). Severe white matter injury, in 10 neonates (6%), was associated with a decrease in motor (-18 points; p < 0.001) and cognitive (-8 points; p = 0.085) scores. With greater severity of adverse neurodevelopmental outcomes, slower increases in FA and NAA/choline were observed in the basal nuclei and brain white matter regions as neonates matured to term-equivalent age, independent of the presence of white matter injury. Conclusions: In the preterm neonate, abnormal brain maturation evolves through the period of neonatal intensive care and is associated with adverse neurodevelopmental outcomes.

Abstract:Objective Assess the capacity of brain state of the newborn (BSN) to predict neurodevelopment outcomes in neonatal encephalopathy. Methods Trends of BSN, a deep learning‐based measure translating EEG background to a continuous trend, were studied from a three‐channel montage long‐term EEG monitoring from a prospective cohort of 92 infants with neonatal encephalopathy and neurodevelopmental outcomes assessed by Bayley Scales of Infant Development, 3rd edition (Bayley‐III) at 18 months. Outcome prediction used categories "Severe impairment" (Bayley‐III composite score ≤70 or death) or "Any impairment" (score ≤85 or death). Results "Severe impairment" was predicted best for motor outcomes (24 h area under the curve (AUC) = 0.97), followed by cognitive (36 h AUC = 0.90), overall (24 h AUC = 0.84), and language (24 h AUC = 0.82). "Any impairment" was best predicted for motor outcomes (12 h AUC = 0.95), followed by cognitive (24 h AUC = 0.85), overall (12 h AUC = 0.75), and language (12 and 24 h AUC = 0.68). Optimal BSN cutoffs for outcome predictions evolved with the postnatal age. Low BSN scores reached a 100% positive prediction of poor outcomes at 24 h of age. Interpretation BSN is an excellent predictor of adverse neurodevelopmental outcomes in survivors of neonatal encephalopathy after therapeutic hypothermia, even at 24 h of life. The trend provides a fully automated, objective, quantified, and reliable interpretation of EEG background. The high temporal resolution supports continuous bedside brain assessment and early prognostication during the initial dynamic recovery phase.

Abstract:Background and Objectives Children with neonatal encephalopathy (NE) are at risk for basal ganglia/thalamus (BG/T) and watershed (WS) patterns of brain injury. Children with BG/T injury are at high risk for motor impairment in infancy but the predictive validity of a published rating scale for outcome at age four years is not known. We examined a cohort of children with NE and MRI to examine the relationship between BG/T injury and severity of cerebral palsy (CP) in childhood. Methods Term neonates at risk for brain injury due to NE were enrolled at UCSF Benioff Children's Hospital from 1993-2014 and received MRI within two weeks of birth. Brain injury was scored by a pediatric neuroradiologist. The Gross Motor Function Classification System (GMFCS) level was determined at four years. The relationship between BG/T injury and dichotomized GMFCS (no CP or GMFCS I-II=none/mild vs. III-V=moderate/severe CP) was evaluated with logistic regression and predictive performance was assessed by cross-validated area under the ROC curve (AUROC). Results Among 174 children, higher BG/T scores were associated with more severe GMFCS level. Clinical predictors had a low AUROC (0.599), compared with that of MRI (0.895). Risk of moderate-severe CP was low (<20%) in all patterns of brain injury except BG/T=4, which carried a 67% probability (95% CI 36-98%) of moderate-severe CP. Hypothermia therapy did not affect model performance. Conclusions The BG/T injury score can be used to predict risk and severity of CP at age four years and thereby inform early developmental interventions.

Abstract:Cerebral cortical folding becomes dramatically more complex in the fetal brain during the 3rd trimester of gestation; the process continues in a similar fashion in children who are born prematurely. To quantify this morphological development, it is necessary to extract the interface between gray matter and white matter, which is particularly challenging due to changing tissue contrast during brain maturation. We employed the well-established CIVET pipeline to extract this cortical surface, with point correspondence across subjects, using a surface-based spherical registration. We then developed a variant of the pipeline, called NEOCIVET, that quantified cortical folding using mean curvature and sulcal depth while addressing the well-known problems of poor and temporally-varying gray/white contrast as well as motion artifact in neonatal MRI. NEOCIVET includes: i) a tissue classification technique that analyzed multi-atlas texture patches using the nonlocal mean estimator and subsequently applied a label fusion approach based on a joint probability between templates, ii) neonatal template construction based on age-specific sub-groups, and iii) masking of non-interesting structures using label-fusion approaches. These techniques replaced modules that might be suboptimal for regional analysis of poor-contrast neonatal cortex. The proposed segmentation method showed more accurate results in subjects with various ages and with various degrees of motion compared to state-of-the-art methods. In the analysis of 158 preterm-born neonates, many with multiple scans (n=231; 26-40weeks postmenstrual age at scan), NEOCIVET identified increases in cortical folding over time in numerous cortical regions (mean curvature: +0.003/week; sulcal depth: +0.04mm/week) while folding did not change in major sulci that are known to develop early (corrected p<0.05). The proposed pipeline successfully mapped cortical structural development, supporting current models of cerebral morphogenesis, and furthermore, revealed impairment of cortical folding in extremely preterm newborns relative to relatively late preterm newborns, demonstrating its potential to provide biomarkers of prematurity-related developmental outcome.

Abstract:Importance: Congenital heart disease (CHD), especially the complex forms- such as hypoplastic left heart syndrome (HLHS) and transposition of the great arteries (TGA)- have been linked to neurodevelopmental deficits including impairments in gross cognitive functions, language abilities, and visuo-motor skills. The prognostic value of early infant brain trajectories and cumulative impact of demographic factors in relation to childhood neurodevelopmental outcomes is unknown. Objective: To determine whether early structural brain trajectories predict early childhood neurodevelopmental deficits in complex CHD patients and to assess relative cumulative risk profiles of clinical, genetic, and demographic risk factors across early development. Design, Setting, and Participants: We studied a prospective cohort study of term neonates with complex CHD (TGA and HLHS) were recruited at Texas Childrens Hospital between 2005-2011. Participants underwent structural MRI scans at three time points (one preoperative scan, one postoperative scan within 7 days of surgery, and one follow-up postoperative scan at 4 months). Participants also received three neurodevelopmental assessments at 1, 3, and 5 years of age. Main Outcomes and Measures: Brain region volumes (macrostructure) and white matter tract (microstructure) fractional anisotropy (FA) and radial diffusivity (RD) were measured from the MRI scans acquired in the three neonatal time points. Three imaging trajectories- changes in volume, FA and RD, over time- corresponding to periods of brain changes were determined: perioperative (preoperative to postoperative #1), post-surgical (postoperative #1 to postoperative #2), and overall (preoperative #1 to postoperative #2). Gross cognitive, language, and visuo-motor outcomes were assessed with the Bayley Scales of Infant and Toddler Development, Third Edition (Bayley-III) at 1 and 3 years, and with the Wechsler Preschool and Primary Scale of Intelligence, Third Edition (WPPSI-III Full-Scale IQ and Verbal IQ, and Beery-Buktenica Developmental Test of Visual-Motor Integration (VMI)., 6th Edition at 5 years. The analysis included development of predictive multi-variable models incorporating other known risk factors (i.e., heart lesion type, microdeletion-related genetic abnormality, and maternal IQ) of poor neurodevelopmental outcomes in CHD. Results: A total of 95 term (38.5+/-1.3 weeks gestational age) neonates with complex CHD (49 [51.6%] HLHS, 46 [48.4%] TGA; 42 [44.2%] girls) were analyzed. Reduced overall period trajectories predicted poor language outcomes: brainstem (p=0.0022) and white matter (p=0.0397) predicted poor 5-year verbal IQ; brainstem (p=0.0134), deep grey (p=0.0258), and FA of superior longitudinal fasciculus (SLF) (p=0.0256) predicted poor 3-year language; whole brain volume predicted poor performance on measures of language at 1 year. Maternal IQ was the strongest contributor to language outcome variance that increased from 37% at 1-year, up to 62% at 3-year, and up to 81% at 5-year testing. Genetic abnormality contribution to variance in these same models decreased from 41% in 1-year to about 25% at 3-year, and then to not significant in the 5-year assessments. Heart lesion type was found to be not significant in predicting outcomes in these models. Conclusion and Relevance: A dysmaturation pattern of reduced postnatal trajectories of subcortical-cerebral white matter MRI metrics predicted poor early childhood neurodevelopmental outcomes, despite the high relative contribution of maternal IQ. Maternal IQ was cumulative over time, exceeding the influence of known innate cardiac and genetic factors in complex CHD, underscoring the importance of both heritable factors and parent-based environmental factors.
