Abstract:Machine Learning (ML) models have been applied to solve problems in various fields, which necessarily involves proper evaluation of models to ensure performance. Once deployed, ML models are subject to performance issues, such as those related to changes in data (drift). This type of issue has prompted efforts in model analysis and maintenance, as well as in continual learning, which seeks the ability to continuously learn from a (continuous) stream of data. Therefore, it's important to understand and develop methodologies that can be used to evaluate ML models, making their use in real-world environments feasible. Amongst current areas of application for ML, one that stands out, in particular, is Machine Learning for Healthcare, especially in conjunction with Software for Decision Support of Medical Applications, which presents specific challenges for the evaluation and monitoring of models, particularly given that incorrect prediction or classification can lead to life-threatening situations. This paper presents a systematic literature review that aims at identifying state-of-the-art techniques for evaluating and maintaining ML models for healthcare in effective use in the real world.
Systematic Review of Supervised Machine Learning Models in Prediction of Medical Conditions

Abstract:ABSTRACT Machine learning (ML) models for analyzing medical data are critical for both accelerating development of novel diagnostic and treatment strategies and improving the accuracy of medical care delivery. Our objective was to comprehensively review supervised ML models for diagnosis or treatment prediction. Publications indexed in PubMed were reviewed to identify articles utilizing supervised predictive ML models in medicine. Articles published between 01/01/2020–01/01/2022 were included in this review. Initially, PubMed was searched using MeSH major terms, and if more extensive search results were needed, a broader search was applied (titles/abstracts). PubMed indexed 21,268 published articles (MeSH Major topic) describing ML methods implemented in medicine. Of those, 11,726 articles were published within the last 2 years. Most of the published ML models in medicine in the last two years were different types of deep learning models (about 75%). Fifty articles were included in this review. Almost all categories of disease were subjects of ML predictions. Positive and negative factors in each of the scenarios need to be evaluated before the most optimal ML model is selected. Domain knowledge and collaborations between physicians and ML experts can improve the selection and prediction performance of ML models in medicine and facilitate implementation in clinical practice. Predictive ML models could provide recommendations to recruit suitable patients for clinical trials. Prediction ML models may contribute to development of more effective diagnostic and therapeutic choices, founded on evidence-based medicine. A broad range of methodological approaches have been taken toward this goal, and those approaches are presented here with their various advantages and disadvantages. AUTHOR SUMMARY Over the last decade, there has been rapid development of Machine learning (ML) methods to analyze Big Data in medicine. ML is aimed to make the computer learn from past experiences and make predictions by recognizing patterns in medical data. We performed a comprehensive systematic literature review of recent publications (last two years), indexed in PubMed/MEDLINE that have described either traditional or deep supervised prediction ML models in medicine. We identified 21,268 articles describing ML implementation in medicine. 11,726 articles were published in the last 2 years. We presented the number of publications describing each of the most often ML methods to show current trends in development of these models. Most of the recently published ML models in medicine were deep learning models. We found that the understanding of disease is likely to lead to more accurate prediction. An important dilemma is the selection of optimal ML models for a specific task, considering amount and type of available data. Domain knowledge and collaborations between physicians and ML experts can improve the prediction performance of ML models, which could help clinicians to select the most effective diagnostic and therapeutic choices available and decrease medical errors.

Abstract:Objectives-Geriatric clinical care is a multidisciplinary assessment designed to evaluate older patients (age 65 years and above) functional ability, physical health, and cognitive wellbeing. The majority of these patients suffer from multiple chronic conditions and require special attention. Recently, hospitals utilize various artificial intelligence (AI) systems to improve care for elderly patients. The purpose of this systematic literature review is to understand the current use of AI systems, particularly machine learning (ML), in geriatric clinical care for chronic diseases. Materials and Methods-We restricted our search to eight databases, namely PubMed, WorldCat, MEDLINE, ProQuest, ScienceDirect, SpringerLink, Wiley, and ERIC, to analyze research articles published in English between January 2010 and June 2019. We focused on studies that used ML algorithms in the care of geriatrics patients with chronic conditions. Results-We identified 35 eligible studies and classified in three groups-psychological disorder (n=22), eye diseases (n=6), and others (n=7). This review identified the lack of standardized ML evaluation metrics and the need for data governance specific to health care applications. Conclusion- More studies and ML standardization tailored to health care applications are required to confirm whether ML could aid in improving geriatric clinical care.
Machine Learning First Response to COVID-19: A Systematic Literature Review of Clinical Decision Assistance Approaches during Pandemic Years from 2020 to 2022

Abstract:Background: The declaration of the COVID-19 pandemic triggered global efforts to control and manage the virus impact. Scientists and researchers have been strongly involved in developing effective strategies that can help policy makers and healthcare systems both to monitor the spread and to mitigate the impact of the COVID-19 pandemic. Machine Learning (ML) and Artificial Intelligence (AI) have been applied in several fronts of the fight. Foremost is diagnostic assistance, encompassing patient triage, prediction of ICU admission and mortality, identification of mortality risk factors, and discovering treatment drugs and vaccines. Objective: This systematic review aims to identify original research studies involving actual patient data to construct ML- and AI-based models for clinical decision support for early response during the pandemic years. Methods: Following the PRISMA methodology, two large academic research publication indexing databases were searched to investigate the use of ML-based technologies and their applications in healthcare to combat the COVID-19 pandemic. Results: The literature search returned more than 1000 papers; 220 were selected according to specific criteria. The selected studies illustrate the usefulness of ML with respect to supporting healthcare professionals for (1) triage of patients depending on disease severity, (2) predicting admission to hospital or Intensive Care Units (ICUs), (3) search for new or repurposed treatments and (4) the identification of mortality risk factors. Conclusion: The ML/AI research community was able to propose and develop a wide variety of solutions for predicting mortality, hospitalizations and treatment recommendations for patients with COVID-19 diagnostic, opening the door for further integration of ML in clinical practices fighting this and forecoming pandemics. However, the translation to the clinical practice is impeded by the heterogeneity of both the datasets and the methodological and computational approaches. The literature lacks robust model validations supporting this desired translation.

Abstract:Artificial Intelligence (AI) has recently altered the landscape of cancer research and medical oncology using traditional Machine Learning (ML) algorithms and cutting-edge Deep Learning (DL) architectures. In this review article we focus on the ML aspect of AI applications in cancer research and present the most indicative studies with respect to the ML algorithms and data used. The PubMed and dblp databases were considered to obtain the most relevant research works of the last five years. Based on a comparison of the proposed studies and their research clinical outcomes concerning the medical ML application in cancer research, three main clinical scenarios were identified. We give an overview of the well-known DL and Reinforcement Learning (RL) methodologies, as well as their application in clinical practice, and we briefly discuss Systems Biology in cancer research. We also provide a thorough examination of the clinical scenarios with respect to disease diagnosis, patient classification and cancer prognosis and survival. The most relevant studies identified in the preceding year are presented along with their primary findings. Furthermore, we examine the effective implementation and the main points that need to be addressed in the direction of robustness, explainability and transparency of predictive models. Finally, we summarize the most recent advances in the field of AI/ML applications in cancer research and medical oncology, as well as some of the challenges and open issues that need to be addressed before data-driven models can be implemented in healthcare systems to assist physicians in their daily practice.

Abstract:According to the World Health Organization, chronic illnesses account for over 70% of deaths globally, underscoring the need for effective health risk assessment (HRA). While machine learning (ML) has shown potential in enhancing HRA, no systematic review has explored its application in general health risk assessments. Existing reviews typically focus on specific conditions. This paper reviews published articles that utilize ML for HRA, and it aims to identify the model development methods. A systematic review following Tranfield et al.'s three-stage approach was conducted, and it adhered to the PRISMA protocol. The literature was sourced from five databases, including PubMed. Of the included articles, 42% (11/26) addressed general health risks. Secondary data sources were most common (14/26, 53.85%), while primary data were used in eleven studies, with nine (81.81%) using data from a specific population. Random forest was the most popular algorithm, which was used in nine studies (34.62%). Notably, twelve studies implemented multiple algorithms, while seven studies incorporated model interpretability techniques. Although these studies have shown promise in addressing digital health inequities, more research is needed to include diverse sample populations, particularly from underserved communities, to enhance the generalizability of existing models. Furthermore, model interpretability should be prioritized to ensure transparent, trustworthy, and broadly applicable healthcare solutions.

Abstract:The use of machine learning (ML) models have become a crucial factor in the growing field of healthcare, ushering in a new era of medical research and diagnosis. This study rigorously reviews research publications published in reputable journals during the last five years. The pace and dynamic nature of machine learning in the healthcare domains demonstrated by the arduous criteria, which are used to sort through these articles. Disease-centric analysis uncovered a wide range of deep learning and machine learning models which are designed to address particular medical problems. Convolutional neural networks (CNNs), one of the most complex deep learning architectures, coexist with more conventional statistical models like logistic regression and support vector machines. CNNs are particularly prominent when it comes to disorders that need picture processing, which highlights the significant influence of deep learning in deciphering complex medical patterns. The popularity of ensemble methods, such as Random Forest, Gradient Boosting, and AdaBoost, indicates that their ability to combine predictive capability and strengthen model resilience is well acknowledged. Hybrid techniques, which integrate the advantages of many models, provide novel approaches to tackle distinct healthcare problems. This research also sheds light on a nuanced approach for model selection, wherein deep learning models performs well with huge datasets and image analysis, while statistical and ensemble models provides better results with numerical and categorical data. The adaptability needed in healthcare analytics is shown by hybrid models, which frequently combine standard models for classification with deep learning for feature extraction. The present review can endow problems related to ML in healthcare domain, possible solutions, potential directions and some knowledge to the researchers working in this field.

Abstract:Abstract: The Industrial Revolution 4.0 has flooded the virtual world with data, which includes Internet of Things (IoT) data, mobile data, cybersecurity data, business data, social networks, including health data. To analyse this data efficiently and create related efficient and streamlined applications, expertise in artificial intelligence specifically machine learning (ML), is required. This field makes use of a variety of machine learning methods, including supervised, unsupervised, semi-supervised, and reinforcement. Additionally, deep learning, which is a subset of a larger range of machine learning techniques, is capable of effectively analysing vast amounts of data. Machine learning is a broad term that encompasses a number of methods used to extract information from data. These methods may allow the rapid translation of massive real-world information into applications that assist patients and providers in making decisions. Abstract:Abstract Background/Introduction Artificial intelligence (AI) in the healthcare sector is receiving attention from researchers and health professionals. Few previous studies have investigated this topic from a multi-disciplinary perspective, including accounting, business and management, decision sciences and health professions. Methods The structured literature review with its reliable and replicable research protocol allowed the researchers to extract 288 peer-reviewed papers from Scopus. The authors used qualitative and quantitative variables to analyse authors, journals, keywords, and collaboration networks among researchers. Additionally, the paper benefited from the Bibliometrix R software package. Results The investigation showed that the literature in this field is emerging. It focuses on health services management, predictive medicine, patient data and diagnostics, and clinical decision-making. The United States, China, and the United Kingdom contributed the highest number of studies. Keyword analysis revealed that AI can support physicians in making a diagnosis, predicting the spread of diseases and customising treatment paths. Conclusions The literature reveals several AI applications for health services and a stream of research that has not fully been covered. For instance, AI projects require skills and data quality awareness for data-intensive analysis and knowledge-based management. Insights can help researchers and health professionals understand and address future research on AI in the healthcare field.

A systematic literature review of machine learning in online personal health data

Abstract:Objective: User-generated content (UGC) in online environments provides opportunities to learn an individual's health status outside of clinical settings. However, the nature of UGC brings challenges in both data collecting and processing. The purpose of this study is to systematically review the effectiveness of applying machine learning (ML) methodologies to UGC for personal health investigations. Materials and methods: We searched PubMed, Web of Science, IEEE Library, ACM library, AAAI library, and the ACL anthology. We focused on research articles that were published in English and in peer-reviewed journals or conference proceedings between 2010 and 2018. Publications that applied ML to UGC with a focus on personal health were identified for further systematic review. Results: We identified 103 eligible studies which we summarized with respect to 5 research categories, 3 data collection strategies, 3 gold standard dataset creation methods, and 4 types of features applied in ML models. Popular off-the-shelf ML models were logistic regression (n = 22), support vector machines (n = 18), naive Bayes (n = 17), ensemble learning (n = 12), and deep learning (n = 11). The most investigated problems were mental health (n = 39) and cancer (n = 15). Common health-related aspects extracted from UGC were treatment experience, sentiments and emotions, coping strategies, and social support. Conclusions: The systematic review indicated that ML can be effectively applied to UGC in facilitating the description and inference of personal health. Future research needs to focus on mitigating bias introduced when building study cohorts, creating features from free text, improving clinical creditability of UGC, and model interpretability.

Abstract:Background: Healthcare institutions focus on improving the quality of life for end-users, with key performance indicators like access to essential medicines reflecting the effectiveness of management. Effective healthcare management involves planning, organizing, and controlling institutions built on human resources, data systems, service delivery, access to medicines, finance, and leadership. According to the World Health Organization, these elements must be balanced for an optimal healthcare system. Big data generated from healthcare institutions, including health records and genomic data, is crucial for smart staffing, decision-making, risk management, and patient engagement. Properly organizing and analysing this data is essential, and machine learning, a sub-field of artificial intelligence, can optimize these processes, leading to better overall healthcare management. Objectives: This review examines the major applications of machine learning in healthcare management, the algorithms frequently used in data analysis, their limitations, and the evidence-based benefits of machine learning in healthcare. Methods: Following PRISMA guidelines, databases such as IEEE Xplore, ScienceDirect, ACM Digital Library, and SCOPUS were searched for eligible articles published between 2011 and 2021. Articles had to be in English, peer-reviewed, and include relevant keywords like healthcare, management, and machine learning. Results: Out of 51 relevant articles, 6 met the inclusion criteria. Identified algorithms include topic modelling, dynamic clustering, neural networks, decision trees, and ensemble classifiers, applied in areas such as electronic health records, chatbots, and multi-disease prediction. Conclusion: Machine learning supports healthcare management by aiding decision-making, processing big data, and providing insights for system improvements.
Decoding machine learning in nursing research: A scoping review of effective algorithms

Abstract:Introduction The rapid evolution of artificial intelligence (AI) technology has revolutionized healthcare, particularly through the integration of AI into health information systems. This transformation has significantly impacted the roles of nurses and nurse practitioners, prompting extensive research to assess the effectiveness of AI‐integrated systems. This scoping review focuses on machine learning (ML) used in nursing, specifically investigating ML algorithms, model evaluation methods, areas of focus related to nursing, and the most effective ML algorithms. Design The scoping review followed the Preferred Reporting Items for Systematic Review and Meta‐Analysis Extension for Scoping Reviews (PRISMA‐ScR) guidelines. Methods A structured search was performed across seven databases according to PRISMA‐ScR: PubMed, EMBASE, CINAHL, Web of Science, OVID, PsycINFO, and ProQuest. The quality of the final reviewed studies was assessed using the Medical Education Research Study Quality Instrument (MERSQI). Results Twenty‐six articles published between 2019 and 2023 met the inclusion and exclusion criteria, and 46% of studies were conducted in the US. The average MERSQI score was 12.2, indicative of moderate‐ to high‐quality studies. The most used ML algorithm was Random Forest. The four second‐most used were logistic regression, least absolute shrinkage and selection operator, decision tree, and support vector machine. Most ML models were evaluated by calculating sensitivity (recall)/specificity, accuracy, receiver operating characteristic (ROC), area under the ROC (AUROC), and positive/negative prediction value (precision). Half of the studies focused on nursing staff or students and hospital readmission or emergency department visits. Only 11 articles reported the most effective ML algorithm(s). Conclusion The scoping review provides insights into the current status of ML research in nursing and recognition of its significance in nursing research, confirming the benefits of ML in healthcare. Recommendations include incorporating experimental designs in research studies to optimize the use of ML models across various nursing domains. Clinical Relevance The scoping review demonstrates substantial clinical relevance of ML applications for nurses, nurse practitioners, administrators, and researchers. The integration of ML into healthcare systems and its impact on nursing practices have important implications for patient care, resource management, and the evolution of nursing research.

Clinical performance of automated machine learning: A systematic review

Abstract:Introduction: Automated machine learning (autoML) removes technical and technological barriers to building artificial intelligence models. We aimed to summarise the clinical applications of autoML, assess the capabilities of utilised platforms, evaluate the quality of the evidence trialling autoML, and gauge the performance of autoML platforms relative to conventionally developed models, as well as each other. Method: This review adhered to a prospectively registered protocol (PROSPERO identifier CRD42022344427). The Cochrane Library, Embase, MEDLINE and Scopus were searched from inception to 11 July 2022. Two researchers screened abstracts and full texts, extracted data and conducted quality assessment. Disagreement was resolved through discussion and if required, arbitration by a third researcher. Results: There were 26 distinct autoML platforms featured in 82 studies. Brain and lung disease were the most common fields of study of 22 specialties. AutoML exhibited variable performance: area under the receiver operator characteristic curve (AUCROC) 0.35-1.00, F1-score 0.16-0.99, area under the precision-recall curve (AUPRC) 0.51-1.00. AutoML exhibited the highest AUCROC in 75.6% trials; the highest F1-score in 42.3% trials; and the highest AUPRC in 83.3% trials. In autoML platform comparisons, AutoPrognosis and Amazon Rekognition performed strongest with unstructured and structured data, respectively. Quality of reporting was poor, with a median DECIDE-AI score of 14 of 27. Conclusion: A myriad of autoML platforms have been applied in a variety of clinical contexts. The performance of autoML compares well to bespoke computational and clinical benchmarks. Further work is required to improve the quality of validation studies. AutoML may facilitate a transition to data-centric development, and integration with large language models may enable AI to build itself to fulfil user-defined goals.

Abstract:Abstract Artificial intelligence (AI) refers to the performance of tasks by machines ordinarily associated with human intelligence. Machine learning (ML) is a subtype of AI; it refers to the ability of computers to draw conclusions (ie, learn) from data without being directly programmed. ML builds from traditional statistical methods and has drawn significant interest in healthcare epidemiology due to its potential for improving disease prediction and patient care. This review provides an overview of ML in healthcare epidemiology and practical examples of ML tools used to support healthcare decision making at 4 stages of hospital-based care: triage, diagnosis, treatment, and discharge. Examples include model-building efforts to assist emergency department triage, predicting time before septic shock onset, detecting community-acquired pneumonia, and classifying COVID-19 disposition risk level. Increasing availability and quality of electronic health record (EHR) data as well as computing power provides opportunities for ML to increase patient safety, improve the efficiency of clinical management, and reduce healthcare costs.

Abstract:Background Cancer is progressively becoming the most prevalent disease worldwide, accompanied by significantly increasing investments in research to improve its prevention, early detection, diagnosis, prognosis and treatment. Predictive analytics are showing promising performance when applied to these tasks, with recent reporting guidelines supporting unbiased data analytics whose outcomes demonstrate a clinical benefit. Methods A systematic review has been conducted to analyse statistical- and ML-based prediction model studies on cancer research from 2010 to 2020. The PRISMA and PROBAST methodologies have been adopted. Findings Statistical analysis (46.4 %) and linear ML-based methods (36.4 %) predominate over non-linear ML-based methods (17.2 %) among the examined studies. Only 11 % of the studies are associated with a low risk of bias (ROB), whereas the majority of studies (69 %) has been judged as unclear ROB, an aftereffect of the incompleteness (non-transparency) in their reporting. Lastly, 81.6 % of the investigated studies do not report any data quality assessment procedure. A qualitative analysis of the studies from 2021 to 2023 shows a shift to combining data-driven and systems biology computational approaches. Interpretation The alignment with systematic procedures for reporting and assessing prediction model studies is a prerequisite towards responsible research. These procedures will enable ML-based interventions in the field of cancer research, demonstrating the clinical value of their findings.

Abstract:Today, artificial intelligence (AI) and machine learning (ML) have dramatically advanced in various industries, especially medicine. AI describes computational programs that mimic and simulate human intelligence, for example, a person’s behavior in solving problems or his ability for learning. Furthermore, ML is a subset of artificial intelligence. It extracts patterns from raw data automatically. The purpose of this paper is to help researchers gain a proper understanding of machine learning and its applications in healthcare. In this paper, we first present a classification of machine learning-based schemes in healthcare. According to our proposed taxonomy, machine learning-based schemes in healthcare are categorized based on data pre-processing methods (data cleaning methods, data reduction methods), learning methods (unsupervised learning, supervised learning, semi-supervised learning, and reinforcement learning), evaluation methods (simulation-based evaluation and practical implementation-based evaluation in real environment) and applications (diagnosis, treatment). According to our proposed classification, we review some studies presented in machine learning applications for healthcare. We believe that this review paper helps researchers to familiarize themselves with the newest research on ML applications in medicine, recognize their challenges and limitations in this area, and identify future research directions.

Abstract:Abstract Machine learning (ML) is a rapidly advancing field with increasing utility in health care. We conducted a systematic review and critical appraisal of ML applications in vascular surgery. MEDLINE, Embase, and Cochrane CENTRAL were searched from inception to March 1, 2021. Study screening, data extraction, and quality assessment were performed by two independent reviewers, with a third author resolving discrepancies. All original studies reporting ML applications in vascular surgery were included. Publication trends, disease conditions, methodologies, and outcomes were summarized. Critical appraisal was conducted using the PROBAST risk-of-bias and TRIPOD reporting adherence tools. We included 212 studies from a pool of 2235 unique articles. ML techniques were used for diagnosis, prognosis, and image segmentation in carotid stenosis, aortic aneurysm/dissection, peripheral artery disease, diabetic foot ulcer, venous disease, and renal artery stenosis. The number of publications on ML in vascular surgery increased from 1 (1991–1996) to 118 (2016–2021). Most studies were retrospective and single center, with no randomized controlled trials. The median area under the receiver operating characteristic curve (AUROC) was 0.88 (range 0.61–1.00), with 79.5% [62/78] studies reporting AUROC ≥ 0.80. Out of 22 studies comparing ML techniques to existing prediction tools, clinicians, or traditional regression models, 20 performed better and 2 performed similarly. Overall, 94.8% (201/212) studies had high risk-of-bias and adherence to reporting standards was poor with a rate of 41.4%. Despite improvements over time, study quality and reporting remain inadequate. Future studies should consider standardized tools such as PROBAST and TRIPOD to improve study quality and clinical applicability.

Abstract:This review aims to explore the growing impact of machine learning and deep learning algorithms in the medical field, with a specific focus on the critical issues of explainability and interpretability associated with black-box algorithms. While machine learning algorithms are increasingly employed for medical analysis and diagnosis, their complexity underscores the importance of understanding how these algorithms explain and interpret data to take informed decisions. This review comprehensively analyzes challenges and solutions presented in the literature, offering an overview of the most recent techniques utilized in this field. It also provides precise definitions of interpretability and explainability, aiming to clarify the distinctions between these concepts and their implications for the decision-making process. Our analysis, based on 448 articles and addressing seven research questions, reveals an exponential growth in this field over the last decade. The psychological dimensions of public perception underscore the necessity for effective communication regarding the capabilities and limitations of artificial intelligence. Researchers are actively developing techniques to enhance interpretability, employing visualization methods and reducing model complexity. However, the persistent challenge lies in finding the delicate balance between achieving high performance and maintaining interpretability. Acknowledging the growing significance of artificial intelligence in aiding medical diagnosis and therapy, and the creation of interpretable artificial intelligence models is considered essential. In this dynamic context, an unwavering commitment to transparency, ethical considerations, and interdisciplinary collaboration is imperative to ensure the responsible use of artificial intelligence. This collective commitment is vital for establishing enduring trust between clinicians and patients, addressing emerging challenges, and facilitating the informed adoption of these advanced technologies in medicine.
