Abstract:Protecting patient privacy has become a top priority with the introduction of Healthcare 5.0 and the growth of the Internet of Things. This study provides a revolutionary strategy that makes use of blockchain technology, information fusion, and federated illness prediction and deep extreme machine learning to meet the difficulties with regard to healthcare privacy. The suggested framework integrates several innovative technologies to make healthcare systems safe and privacy-preserving. The framework leverages the blockchain system, a distributed and unchangeable ledger, to secure the integrity, traceability and openness of private medical information. Patient privacy is better protected as a result, and there is less chance of data breaches or unauthorized access. The system makes use of the Linear Discriminant Analysis (LDA), Decision Tree, Extra Tree Classifier, AdaBoost, and Federated Deep Extreme Machine Learning algorithms to increase the accuracy and efficacy of illness prediction. This method allows for collaborative learning across many healthcare organizations without disclosing raw data, protecting privacy. The system obtains a thorough awareness of patient health, allowing for the early diagnosis of diseases and the development of individualized treatment suggestions. To further detect and reduce possible security risks in the IoMT environment, the framework also includes intrusion detection methods. Protecting patient data and infrastructure, the system can quickly identify and react to unauthorized actions or threats. High accuracy and privacy protection are shown by the results, making it appropriate for Healthcare 5.0 applications. Abstract:Recently, the use of the Internet of Medical Things (IoMT) has gained popularity across various sections of the health sector. The historical security risks of IoMT devices themselves and the data flowing from them are major concerns. Deploying many devices, sensors, services, and networks that connect the IoMT systems is gaining popularity. This study focuses on identifying the use of blockchain in innovative healthcare units empowered by federated learning. A collective use of blockchain with intrusion detection management (IDM) is beneficial to detect and prevent malicious activity across the storage nodes. Data accumulated at a centralized storage node is analyzed with the help of machine learning algorithms to diagnose disease and allow appropriate medication to be prescribed by a medical healthcare professional. The model proposed in this study focuses on the effective use of such models for healthcare monitoring. The amalgamation of federated learning and the proposed model makes it possible to reach 93.89 percent accuracy for disease analysis and addiction. Further, intrusion detection ensures a success rate of 97.13 percent in this study.

Abstract:COVID-19 is a serious epidemic that not only endangers human health, but also wreaks havoc on the development of society. Recently, there has been research on using artificial intelligence (AI) techniques for COVID-19 detection. As AI has entered the era of big models, deep learning methods based on pre-trained models (PTMs) have become a focus of industrial applications. Federated learning (FL) enables the union of geographically isolated data, which can address the demands of big data for PTMs. However, the incompleteness of the healthcare system and the untrusted distribution of medical data make FL participants unreliable, and medical data also has strong privacy protection requirements. Our research aims to improve training efficiency and global model accuracy using PTMs for training in FL, reducing computation and communication. Meanwhile, we provide a secure aggregation rule using differential privacy and fully homomorphic encryption to achieve a privacy-preserving Byzantine robust federal learning scheme. In addition, we use blockchain to record the training process and we integrate a Byzantine fault tolerance consensus to further improve robustness. Finally, we conduct experiments on a publicly available dataset, and the experimental results show that our scheme is effective with privacy-preserving and robustness. The final trained models achieve better performance on the positive prediction and severe prediction tasks, with an accuracy of 85.00% and 85.06%, respectively. Thus, this indicates that our study is able to provide reliable results for COVID-19 detection.

Abstract:This study proposes a framework to enhance privacy in Blockchain-based Internet of Things (BIoT) systems used in the healthcare sector. The framework addresses the challenge of leveraging health data for analytics while protecting patient privacy. To achieve this, the study integrates Differential Privacy (DP) with Federated Learning (FL) to protect sensitive health data collected by IoT nodes. The proposed framework utilizes dynamic personalization and adaptive noise distribution strategies to balance privacy and data utility. Additionally, blockchain technology ensures secure and transparent aggregation and storage of model updates. Experimental results on the SVHN dataset demonstrate that the proposed framework achieves strong privacy guarantees against various attack scenarios while maintaining high accuracy in health analytics tasks. Abstract:This research combines two emerging technologies, the IoT and blockchain, and investigates their potential and use in the healthcare sector. In healthcare, IoT technology can be utilized for purposes such as remotely monitoring patients' health. This paper details ongoing research towards individualized health monitoring using wearable gadgets. The goal of improving healthcare facilities and improvement of the quality of life of citizens naturally brings up Internet of Things (IoT) technologies for consideration. Health observation is exceptionally critical in terms of avoidance, especially since the early determination of illnesses can minimize trouble and treatment costs. The cornerstones of intelligent, integrated, and individualized healthcare are continuous monitoring of physical signs and evaluation of medical data. To build a more reliable and robust IoMT model, the study will monitor the application of blockchain technology in federated learning (FL). A viable way to address the heterogeneity problem in federated learning is to design the system, data, and model tiers to lessen heterogeneity and produce a high-quality, tailored model for each endpoint. Blockchain-based federated learning allows for smarter simulations, lower latency, and lower power consumption while maintaining privacy at the same time. Abstract:The rapid adoption of Internet of Things (IoT) devices in healthcare has introduced new challenges in preserving data privacy, security and patient safety. Traditional approaches need to ensure security and privacy while maintaining computational efficiency, particularly for resource-constrained IoT devices. This paper proposes a novel hybrid approach combining federated learning and blockchain technology to provide a secure and privacy-preserved solution for IoT-enabled healthcare applications. Our approach leverages a public-key cryptosystem that provides semantic security for local model updates, while blockchain technology ensures the integrity of these updates and enforces access control and accountability. The federated learning process enables a secure model aggregation without sharing sensitive patient data. We implement and evaluate our proposed framework using EMNIST datasets, demonstrating its effectiveness in preserving data privacy and security while maintaining computational efficiency. The results suggest that our hybrid approach can significantly enhance the development of secure and privacy-preserved IoT-enabled healthcare applications, offering a promising direction for future research in this field.

Abstract:The advancement of Internet and Communication Technologies (ICTs) has led to the era of Industry 4.0. This shift is followed by healthcare industries creating the term Healthcare 4.0. In Healthcare 4.0, the use of IoT-enabled medical imaging devices for early disease detection has enabled medical practitioners to increase healthcare institutions' quality of service. However, Healthcare 4.0 is still lagging in Artificial Intelligence and big data compared to other Industry 4.0 due to data privacy concerns. In addition, institutions' diverse storage and computing capabilities restrict institutions from incorporating the same training model structure. This paper presents a secure multi-party computation-based ensemble federated learning with blockchain that enables heterogeneous models to collaboratively learn from healthcare institutions' data without violating users' privacy. Blockchain properties also allow the party to enjoy data integrity without trust in a centralized server while also providing each healthcare institution with auditability and version control capability.

Abstract:The Internet of Things (IoT) compromises multiple devices connected via a network to perform numerous activities. The large amounts of raw user data handled by IoT operations have driven researchers and developers to provide guards against any malicious threats. Blockchain is a technology that can give connected nodes means of security, transparency, and distribution. IoT devices could guarantee data centralization and availability with shared ledger technology. Federated learning (FL) is a new type of decentralized machine learning (DML) where clients collaborate to train a model and share it privately with an aggregator node. The integration of Blockchain and FL enabled researchers to apply numerous techniques to hide the shared training parameters and protect their privacy. This study explores the application of this integration in different IoT environments, collectively referred to as the Internet of X (IoX). In this paper, we present a state-of-the-art review of federated learning and Blockchain and how they have been used in collaboration in the IoT ecosystem. We also review the existing security and privacy challenges that face the integration of federated learning and Blockchain in the distributed IoT environment. Furthermore, we discuss existing solutions for security and privacy by categorizing them based on the nature of the privacy-preservation mechanism. We believe that our paper will serve as a key reference for researchers interested in improving solutions based on mixing Blockchain and federated learning in the IoT environment while preserving privacy.

Abstract:The swift advancement of the Internet of Things (IoT), coupled with the growing application of healthcare software in this area, has given rise to significant worries about the protection and confidentiality of critical health data. To address these challenges, blockchain technology has emerged as a promising solution, providing decentralized and immutable data storage and transparent transaction records. However, traditional blockchain systems still face limitations in terms of preserving data privacy. This paper proposes a novel approach to enhancing privacy preservation in IoT-based healthcare applications using homomorphic encryption techniques combined with blockchain technology. Homomorphic encryption facilitates the performance of calculations on encrypted data without requiring decryption, thus safeguarding the data's privacy throughout the computational process. The encrypted data can be processed and analyzed by authorized parties without revealing the actual contents, thereby protecting patient privacy. Furthermore, our approach incorporates smart contracts within the blockchain network to enforce access control and to define data-sharing policies. These smart contracts provide fine-grained permission settings, which ensure that only authorized entities can access and utilize the encrypted data. These settings protect the data from being viewed by unauthorized parties. In addition, our system generates an audit record of all data transactions, which improves both accountability and transparency. We have provided a comparative evaluation with the standard models, taking into account factors such as communication expense, transaction volume, and security. The findings of our experiments suggest that our strategy protects the confidentiality of the data while at the same time enabling effective data processing and analysis. In conclusion, the combination of homomorphic encryption and blockchain technology presents a solution that is both resilient and protective of users' privacy for healthcare applications integrated with IoT. This strategy offers a safe and open setting for the management and exchange of sensitive patient medical data, while simultaneously preserving the confidentiality of the patients involved.

Abstract:Over the years, the popularity and usage of wearable Internet of Things (IoT) devices in several healthcare services are increased. Among the services that benefit from the usage of such devices is predictive analysis, which can improve early diagnosis in e-health. However, due to the limitations of wearable IoT devices, challenges in data privacy, service integrity, and network structure adaptability arose. To address these concerns, we propose a platform using federated learning and private blockchain technology within a fog-IoT network. These technologies have privacy-preserving features securing data within the network. We utilized the fog-IoT network's distributive structure to create an adaptive network for wearable IoT devices. We designed a testbed to examine the proposed platform's ability to preserve the integrity of a classifier. According to experimental results, the introduced implementation can effectively preserve a patient's privacy and a predictive service's integrity. We further investigated the contributions of other technologies to the security and adaptability of the IoT network. Overall, we proved the feasibility of our platform in addressing significant security and privacy challenges of wearable IoT devices in predictive healthcare through analysis, simulation, and experimentation.

Abstract:In recent years, the global Internet of Medical Things (IoMT) industry has evolved at a tremendous speed. Security and privacy are key concerns on the IoMT, owing to the huge scale and deployment of IoMT networks. Machine learning (ML) and blockchain (BC) technologies have significantly enhanced the capabilities and facilities of healthcare 5.0, spawning a new area known as “Smart Healthcare.” By identifying concerns early, a smart healthcare system can help avoid long-term damage. This will enhance the quality of life for patients while reducing their stress and healthcare costs. The IoMT enables a range of functionalities in the field of information technology, one of which is smart and interactive health care. However, combining medical data into a single storage location to train a powerful machine learning model raises concerns about privacy, ownership, and compliance with greater concentration. Federated learning (FL) overcomes the preceding difficulties by utilizing a centralized aggregate server to disseminate a global learning model. Simultaneously, the local participant keeps control of patient information, assuring data confidentiality and security. This article conducts a comprehensive analysis of the findings on blockchain technology entangled with federated learning in healthcare. 5.0. The purpose of this study is to construct a secure health monitoring system in healthcare 5.0 by utilizing a blockchain technology and Intrusion Detection System (IDS) to detect any malicious activity in a healthcare network and enables physicians to monitor patients through medical sensors and take necessary measures periodically by predicting diseases. The proposed system demonstrates that the approach is optimized effectively for healthcare monitoring. In contrast, the proposed healthcare 5.0 system entangled with FL Approach achieves 93.22% accuracy for disease prediction, and the proposed RTS-DELM-based secure healthcare 5.0 system achieves 96.18% accuracy for the estimation of intrusion detection.

Abstract:Selenium is an essential trace element in humans and animals. Its only established function in humans is the antioxidant activity of glutathione peroxidase, a selenoenzyme. Severe prolonged deficiency may cause a fatal cardiomyopathy. Iatrogenic causes of selenium deficiency include parenteral and enteral nutrition. Low plasma selenium is also found in malabsorption, cystic fibrosis, rheumatoid arthritis, neoplasia, and other varied clinical disorders. Death has resulted from a single massive ingestion of selenium, while chronic excessive intake causes skin, nail, and hair pathology. Extreme geographical variation in population blood and urine selenium levels and a marked age-specific variation in population reference intervals are important factors in understanding selenium nutrition. Nutritional requirements, biological availability, and metabolism are discussed in relation to geographical, age, and method variability. Sampling, processing procedures, and methods for selenium quantitation are reviewed. Selenium content in different biological matrices and reference values for pediatric, adult, and obstetric populations are provided.

Abstract:Federated learning-based medical data privacy sharing can promote the development of medical industry intelligence, but limited by its own security and privacy deficiencies, federated learning still suffers from a single point of failure and privacy leakage of intermediate parameters. To address these problems, this paper proposes a privacy protection framework for medical data based on blockchain and cross-silo federated learning, using cross-silo federated learning to establish a collaborative training platform for multiple medical institutions to enhance the privacy of medical data, introducing blockchain and smart contracts to realize decentralized federated learning to enhance trust between distrustful medical institutions and solve the problem of a single point of failure. In addition, a secure aggregation scheme is designed using threshold homomorphic encryption to prevent the privacy leakage problem during parameter transmission. The experimental and analytical results show that the accuracy of this paper's scheme is consistent with the original federated learning scheme, effectively deals with the problems of single-point failure and inference attacks of federated learning, improves system robustness, and is suitable for medical scenarios with more stringent requirements on security and accuracy.

Abstract:Medical health care centers are envisioned as a promising paradigm to handle the massive volume of data of COVID-19 patients using artificial intelligence (AI). Traditionally, AI techniques often require centralized data collection and training the model in a single organization, which is most common weakness due to the privacy and security of raw data communication. To solve this challenging task, we propose a blockchain-based federated learning framework that provides collaborative data training solutions by coordinating multiple hospitals to train and share encrypted federated models without leakage of data privacy. The blockchain ledger technology provides the decentralization of federated learning model without any central server. The proposed homomorphic encryption scheme encrypts and decrypts the gradients of model to preserve the privacy. More precisely, the proposed framework: i) train the local model by a novel capsule network to segmentation and classify COVID-19 images, ii) then use the homomorphic encryption scheme to secure the local model that encrypts and decrypts the gradients, and finally the model is shared over a decentralized platform through proposed blockchain-based federated learning algorithm. The integration of blockchain and federated learning leads to a new paradigm for medical image data sharing in the decentralized network. The conducted experimental resultsdemonstrate the performance of the proposed scheme.

The Internet of Medical Things (IoMT) refers to interconnected medical systems and devices that gather and transfer healthcare information for several medical applications. Smart healthcare leverages IoMT technology to improve patient diagnosis, monitoring, and treatment, providing efficient and personalized healthcare services. Privacy-preserving Federated Learning (PPFL) is a privacy-enhancing method that allows collaborative method training through distributed data sources while ensuring privacy protection and keeping the data decentralized. In the field of smart healthcare, PPFL enables healthcare professionals to train machine learning algorithms jointly on their corresponding datasets without sharing sensitive data, thereby maintaining confidentiality. Within this framework, anomaly detection includes detecting unusual events or patterns in healthcare data like unexpected changes or irregular vital signs in patient behaviors that can represent security breaches or potential health issues in the IoMT system. Smart healthcare systems could enhance patient care while protecting data confidentiality and individual privacy by incorporating PPFL with anomaly detection techniques. Therefore, this study develops a Privacy-preserving Federated Learning with Blockchain-based Smart Healthcare System (PPFL-BCSHS) technique in the IoMT environment. The purpose of the PPFL-BCSHS technique is to secure the IoMT devices via the detection of abnormal activities and FL concepts. Besides, BC technology can be applied for the secure transmission of medical data among the IoMT devices. The PPFL-BCSHS technique employs the FL for training the model for the identification of abnormal patterns. For anomaly detection, the PPFL-BCSHS technique follows three major processes, namely Mountain Gazelle Optimization (MGO)-based feature selection, Bidirectional Gated Recurrent Unit (BiGRU), and Sandcat Swarm Optimization (SCSO)-based hyperparameter tuning. A series of simulations were implemented to examine the performance of the PPFL-BCSHS method. The empirical analysis highlighted that the PPFL-BCSHS method obtains improved security over other approaches under various measures.

The aggregated data collected from multiple sources in the healthcare domain is sufficient for training advanced deep learning models. However, these sources are frequently hesitant to share such data due to privacy considerations. To address this challenge, researchers have proposed the integration of blockchain and federated learning to develop a system that facilitates the secure sharing of medical records. This work provides a succinct review of the current state of the art in the use of blockchain and federated learning in the decentralized healthcare domain.

Abstract:The rapid growth of the Internet of Medical Things (IoMT) has introduced significant security and privacy challenges in managing and protecting medical data. This paper proposes a secure federated cloud storage system designed to address these challenges using a hybrid heuristic attribute-based encryption (ABE) scheme integrated with a permissioned Blockchain. The proposed system enhances data confidentiality and integrity by first collecting medical information and then encrypting it with ABE using an optimal key generated by the Hybrid Mexican Axolotl with Energy Valley Optimizer (HMO-EVO). The encrypted data is securely stored in a permissioned blockchain, ensuring robust access control and protection against data breaches. For effective healthcare monitoring, the system employs federated learning with a Multi-scale Bi-Long Short-Term Memory and Gated Recurrent Unit (MBiLSTM-GRU) to predict diseases accurately. This federated approach allows for decentralized training of deep learning models, preserving patient data privacy while leveraging collective learning. Experimental results show that the proposed system outperforms conventional methods in terms of security, efficiency, and predictive accuracy. This research offers a comprehensive framework for secure medical data management, combining the strengths of federated learning and blockchain technology to address the critical issues of data ownership, regulatory compliance, and privacy in IoMT networks.

Abstract:Healthcare stands out as a critical domain profoundly impacted by Internet of Things (IoT) technology, generating vast data from sensing devices as IoT applications expand. Addressing security challenges is paramount for a successful IoT healthcare framework, with blockchain technology offering a decentralized structure for robust data protection and secure data exchange within multi-node IoT networks. The research introduces a secure IoT healthcare diagnostic model empowered by deep neural networks, emphasizing encryption, safe transactions, and healthcare diagnostics as key components. Notably, the model incorporates innovative techniques like the orthogonal particle swarm optimization algorithm for sharing medical images and a neighborhood indexing sequence method for hash value encryption. The development of an optimized deep neural network-based classification model for illnesses, validated through extensive trials, demonstrates superior performance metrics compared to existing decision-making techniques, with significant improvements in f -Measure (96.25%), sensitivity (93.26%), specificity (94.26%), and accuracy (93.26%). This study's scientific contribution lies in its innovative approach to securing IoT-healthcare diagnosis models, validated performance enhancements using real-world datasets, and insightful recommendations for future research directions, fostering advancements in healthcare technology for enhanced patient care and system efficiency.

Abstract:Diabetes is a global epidemic with severe consequences for individuals and healthcare systems. While early and personalized prediction can significantly improve outcomes, traditional centralized prediction models suffer from privacy risks and limited data diversity. This paper introduces a novel framework that integrates blockchain and federated learning to address these challenges. Blockchain provides a secure, decentralized foundation for data management, access control, and auditability. Federated learning enables model training on distributed datasets without compromising patient privacy. This collaborative approach facilitates the development of more robust and personalized diabetes prediction models, leveraging the combined data resources of multiple healthcare institutions. We have performed extensive evaluation experiments and security analyses. The results demonstrate good performance while significantly enhancing privacy and security compared to centralized approaches. Our framework offers a promising solution for the ethical and effective use of healthcare data in diabetes prediction.
