Abstract:Objective: Personalized dose monitoring and risk management are of increasing significance with the growing number of computer tomography (CT) examinations. These require high-quality Monte Carlo (MC) simulations that are of the utmost importance for the new developments in personalized CT dosimetry. This work aims to extend the MC framework EGSnrc source code with a new particle source. This, in turn, allows CT-scanner-specific dose and image calculations for any CT scanner. The novel method can be used with all modern EGSnrc user codes, particularly for the simulation of the effective dose based on DICOM images and the calculation of CT images. Approach: The new particle source can be used with input data derived by the user. The input data can be generated by the user based on a previously developed method for the experimental characterization of any CT scanner (doi.org/10.1016/j.ejmp.2015.09.006). Furthermore, the new particle source was benchmarked by air kerma measurements in an ionization chamber at a clinical CT scanner. For this, the simulated angular distribution and attenuation characteristics were compared to measurements to verify the source output free in air. In a second validation step, simulations of air kerma in a homogenous cylindrical and an anthropomorphic thorax phantom were performed and validated against experimentally determined results. A detailed uncertainty evaluation of the simulated air kerma values was developed. Main results: We successfully implemented a new particle source class for the simulation of realistic CT scans. This method can be adapted to any CT scanner. For the attenuation characteristics, there was a maximal deviation of 6.86 % between the measurement and the simulation. The mean deviation for all tube voltages was 2.36 % (σ=1.6 %). For the phantom measurements and simulations, all the values agreed within 5.0 %. The uncertainty evaluation resulted in an uncertainty of 5.5 % (k=1).

Integer Optimization of CT Trajectories using a Discrete Data Completeness Formulation

Abstract:X-ray computed tomography (CT) plays a key role in digitizing three-dimensional structures for a wide range of medical and industrial applications. Traditional CT systems often rely on standard circular and helical scan trajectories, which may not be optimal for challenging scenarios involving large objects, complex structures, or resource constraints. In response to these challenges, we are exploring the potential of twin robotic CT systems, which offer the flexibility to acquire projections from arbitrary views around the object of interest. Ensuring complete and mathematically sound reconstructions becomes critical in such systems. In this work, we present an integer programming-based CT trajectory optimization method. Utilizing discrete data completeness conditions, we formulate an optimization problem to select an optimized set of projections. This approach enforces data completeness and considers absorption-based metrics for reliability evaluation. We compare our method with an equidistant circular CT trajectory and a greedy approach. While greedy already performs well in some cases, we provide a way to improve greedy-based projection selection using an integer optimization approach. Our approach improves CT trajectories and quantifies the optimality of the solution in terms of an optimality gap.

Abstract:Among the well-known methods to approximate derivatives of expectancies computed by Monte-Carlo simulations, averages of pathwise derivatives are often the easiest one to apply. Computing them via algorithmic differentiation typically does not require major manual analysis and rewriting of the code, even for very complex programs like simulations of particle-detector interactions in high-energy physics. However, the pathwise derivative estimator can be biased if there are discontinuities in the program, which may diminish its value for applications.

Abstract:Quantitative kinetic parameters derived from dynamic contrast-enhanced (DCE) data are dependent on signal measurement quality and choice of pharmacokinetic model. However, the fundamental optimization analysis method is equally important and its impact on pharmacokinetic parameters has been mostly overlooked. We examine the effects of those choices on accuracy and performance of parameter estimation using both computer processing unit and graphical processing unit (GPU) numerical optimization implementations and evaluate the improvements offered by a novel optimization approach. A test framework was developed where experimentally derived population-average arterial input function and randomly sampled parameter sets {Ktrans, Kep, Vb, τ} were used to generate known tissue curves. Five numerical optimization algorithms were evaluated: sequential quadratic programming, downhill simplex (Nelder–Mead), pattern search, simulated annealing, and differential evolution. This was combined with various objective function implementation details: delay approximation, discretization and varying sampling rates. Then, impact of noise and CPU/GPU implementation was tested for speed and accuracy. Finally, the optimal method was compared to conventional implementation as applied to clinical DCE computed tomography. Nelder–Mead, differential evolution and sequential quadratic programming produced good results on clean and noisy input data outperforming simulated annealing and pattern search in terms of speed and accuracy in the respective order of 10−8%, 10−7%, and ×10−6%). A novel approach for DCE numerical optimization (infinite impulse response with fractional delay approximation) was implemented on GPU for speed increase of at least 2 orders of magnitude. Applied to clinical data, the magnitude of overall parameter error was <10%.

Abstract:The full optimization of the design and operation of instruments whose functioning relies on the interaction of radiation with matter is a super-human task, given the large dimensionality of the space of possible choices for geometry, detection technology, materials, data-acquisition, and information-extraction techniques, and the interdependence of the related parameters. On the other hand, massive potential gains in performance over standard, "experience-driven" layouts are in principle within our reach if an objective function fully aligned with the final goals of the instrument is maximized by means of a systematic search of the configuration space. The stochastic nature of the involved quantum processes make the modeling of these systems an intractable problem from a classical statistics point of view, yet the construction of a fully differentiable pipeline and the use of deep learning techniques may allow the simultaneous optimization of all design parameters. In this document we lay down our plans for the design of a modular and versatile modeling tool for the end-to-end optimization of complex instruments for particle physics experiments as well as industrial and medical applications that share the detection of radiation as their basic ingredient. We consider a selected set of use cases to highlight the specific needs of different applications.
Robust optimization strategies for contour uncertainties in online adaptive radiation therapy

Abstract:Objective: Online adaptive radiation therapy requires fast and automated contouring of daily scans for treatment plan re-optimization. However, automated contouring is imperfect and introduces contour uncertainties. This work aims at developing and comparing robust optimization strategies accounting for such uncertainties. Approach: A deep-learning method was used to predict the uncertainty of deformable image registration, and to generate a finite set of daily contour samples. Ten optimization strategies were compared: two baseline methods, five methods that convert contour samples into voxel-wise probabilities, and three methods accounting explicitly for contour samples as scenarios in robust optimization. Target coverage and organ-at-risk (OAR) sparing were evaluated robustly for simplified proton therapy plans for five head-and-neck cancer patients. Results: We found that explicitly including target contour uncertainty in robust optimization provides robust target coverage with better OAR sparing than the baseline methods, without increasing the optimization time. Although OAR doses first increased when increasing target robustness, this effect could be prevented by additionally including robustness to OAR contour uncertainty. Compared to the probability-based methods, the scenario-based methods spared the OARs more, but increased integral dose and required more computation time. Significance: This work proposed efficient and beneficial strategies to mitigate contour uncertainty in treatment plan optimization. This facilitates the adoption of automatic contouring in online adaptive radiation therapy and, more generally, enables mitigation also of other sources of contour uncertainty in treatment planning.

Abstract:Proton computed tomography (pCT) is an image modality that will improve treatment planning for patients receiving proton radiation therapy compared with the current treatment techniques, which are based on X-ray CT. Reconstruction of a pCT image requires solving a large and sparse system of linear equations, which should be accomplished within a few minutes in order to be clinically practical. Analyzing the efficiency of potentially clinical reconstruction implementations requires multiple quality pCT data sets. The purpose of this paper is to describe the simulator that was developed to generate realistic pCT data sets to be used in testing the efficiency of reconstruction algorithms, in particular string-averaging and block-iterative projection algorithms using sparse matrix formats on General Purpose Graphics Processing Units (GPGPU)s.

Task-based Generation of Optimized Projection Sets using Differentiable Ranking

Abstract:We present a method for selecting valuable projections in computed tomography (CT) scans to enhance image reconstruction and diagnosis. The approach integrates two important factors, projection-based detectability and data completeness, into a single feed-forward neural network. The network evaluates the value of projections, processes them through a differentiable ranking function and makes the final selection using a straight-through estimator. Data completeness is ensured through the label provided during training. The approach eliminates the need for heuristically enforcing data completeness, which may exclude valuable projections. The method is evaluated on simulated data in a non-destructive testing scenario, where the aim is to maximize the reconstruction quality within a specified region of interest. We achieve comparable results to previous methods, laying the foundation for using reconstruction-based loss functions to learn the selection of projections.

Distributed and Scalable Optimization for Robust Proton Treatment Planning

Abstract:Purpose: The importance of robust proton treatment planning to mitigate the impact of uncertainty is well understood. However, its computational cost grows with the number of uncertainty scenarios, prolonging the treatment planning process. We developed a fast and scalable distributed optimization platform that parallelizes this computation over the scenarios. Methods: We modeled the robust proton treatment planning problem as a weighted least-squares problem. To solve it, we employed an optimization technique called the Alternating Direction Method of Multipliers with Barzilai-Borwein step size (ADMM-BB). We reformulated the problem in such a way as to split the main problem into smaller subproblems, one for each proton therapy uncertainty scenario. The subproblems can be solved in parallel, allowing the computational load to be distributed across multiple processors (e.g., CPU threads/cores). We evaluated ADMM-BB on four head-and-neck proton therapy patients, each with 13 scenarios accounting for 3 mm setup and 3:5% range uncertainties. We then compared the performance of ADMM-BB with projected gradient descent (PGD) applied to the same problem. Results: For each patient, ADMM-BB generated a robust proton treatment plan that satisfied all clinical criteria with comparable or better dosimetric quality than the plan generated by PGD. However, ADMM-BB's total runtime averaged about 6 to 7 times faster. This speedup increased with the number of scenarios. Conclusion: ADMM-BB is a powerful distributed optimization method that leverages parallel processing platforms, such as multi-core CPUs, GPUs, and cloud servers, to accelerate the computationally intensive work of robust proton treatment planning. This results in 1) a shorter treatment planning process and 2) the ability to consider more uncertainty scenarios, which improves plan quality.

Abstract:Cone-beam computed tomography (CBCT) systems, with their flexibility, present a promising avenue for direct point-of-care medical imaging, particularly in critical scenarios such as acute stroke assessment. However, the integration of CBCT into clinical workflows faces challenges, primarily linked to long scan duration resulting in patient motion during scanning and leading to image quality degradation in the reconstructed volumes. This paper introduces a novel approach to CBCT motion estimation using a gradient-based optimization algorithm, which leverages generalized derivatives of the backprojection operator for cone-beam CT geometries. Building on that, a fully differentiable target function is formulated which grades the quality of the current motion estimate in reconstruction space. We drastically accelerate motion estimation yielding a 19-fold speed-up compared to existing methods. Additionally, we investigate the architecture of networks used for quality metric regression and propose predicting voxel-wise quality maps, favoring autoencoder-like architectures over contracting ones. This modification improves gradient flow, leading to more accurate motion estimation. The presented method is evaluated through realistic experiments on head anatomy. It achieves a reduction in reprojection error from an initial average of 3mm to 0.61mm after motion compensation and consistently demonstrates superior performance compared to existing approaches. The analytic Jacobian for the backprojection operation, which is at the core of the proposed method, is made publicly available. In summary, this paper contributes to the advancement of CBCT integration into clinical workflows by proposing a robust motion estimation approach that enhances efficiency and accuracy, addressing critical challenges in time-sensitive scenarios.

Efficient inversion strategies for estimating optical properties with Monte Carlo radiative transport models

Abstract:Indirect imaging problems in biomedical optics generally require repeated evaluation of forward models of radiative transport, for which Monte Carlo is accurate yet computationally costly. We develop a novel approach to reduce this bottleneck which has significant implications for quantitative tomographic imaging in a variety of medical and industrial applications. Using Monte Carlo we compute a fully stochastic gradient of an objective function for a given imaging problem. Leveraging techniques from the machine learning community we then adaptively control the accuracy of this gradient throughout the iterative inversion scheme, in order to substantially reduce computational resources at each step. For example problems of Quantitative Photoacoustic Tomography and Ultrasound Modulated Optical Tomography, we demonstrate that solutions are attainable using a total computational expense that is comparable to (or less than) that which is required for a single high accuracy forward run of the same Monte Carlo model. This approach demonstrates significant computational savings when approaching the full non-linear inverse problem of optical property estimation using stochastic methods.

Abstract:We describe a software package, TomOpt, developed to optimise the geometrical layout and specifications of detectors designed for tomography by scattering of cosmic-ray muons. The software exploits differentiable programming for the modeling of muon interactions with detectors and scanned volumes, the inference of volume properties, and the optimisation cycle performing the loss minimisation. In doing so, we provide the first demonstration of end-to-end-differentiable and inference-aware optimisation of particle physics instruments. We study the performance of the software on a relevant benchmark scenarios and discuss its potential applications.

Abstract:Dynamic Contrast Enhanced (DCE) Magnetic Resonance Imaging (MRI) is an important diagnostic technique that can quantify the structure and function of microvasculature processes, using T1 relaxation times and tracer kinetic maps. However, a series of methodological limitations affect both the accuracy and standardisation of the quantified maps, and consequently their diagnostic ability. The main methodological challenge in the quantification of tracer kinetics is a multi-parameter optimization, with correlated parameters that have different scales, which results in local minima particularly when measurements are highly undersampled. This work suggests a novel data driven optimization scheme, based on a variation of the Stochastic Gradient Langevin dynamics (SGLD) Markov chain Monte Carlo algorithm, which combines stochastic gradient descent and Langevin dynamics. The proposed SGDL algorithm avoided local minima and accurately quantified proton density, T1 relaxation times and tracer kinetics. Joint direct parameterization significantly benefited the quantification of proton density, T1 relaxation times, and the selection of a suitable tracer kinetic model per tissue type. Model based arterial and portal vein input functions were automatically determined during the joint direct parameterization. Observations made on simulated fully and highly undersampled liver DCE MRI data were confirmed on acquired clinical data.

Abstract:PURPOSE: The risk of inducing cancer to patients undergoing CT examinations has motivated efforts for CT dose estimation, monitoring, and reduction, especially among pediatric population. The method investigated in this study is Acuros CTD (Varian Medical Systems, Palo Alto, CA), a deterministic linear Boltzmann transport equation (LBTE) solver aimed at generating rapid and reliable dose maps of CT exams. By applying organ contours, organ doses can also be obtained, thus patient-specific organ dose estimates can be provided. This study experimentally validated Acuros against measurements performed on a clinical CT system using a range of physical pediatric anthropomorphic phantoms and acquisition protocols.METHODS: The study consisted of (1) the acquisition of dose measurements on a clinical CT scanner through thermoluminescent dosimeters (TLDs), and (2) the modeling in the Acuros platform of the measurement set up, which includes the modeling of the CT scanner and of the anthropomorphic phantoms. For the measurements, 1-year-old, 5-year-old, and 10-year-old anthropomorphic phantoms of the CIRS ATOM family were used. TLDs were placed in selected organ locations such as stomach, liver, lungs, and heart. The pediatric phantoms were scanned helically with the GE Discovery 750 HD clinical scanner for several examination protocols. For the simulations in Acuros, scanner-specific input, such as bowtie filters, overrange collimation, and tube current modulation schemes, were modeled. These scanner complexities were implemented by defining discretized X-ray beams whose spectral distribution, defined in Acuros by only six energy bins, varied across fan angle, cone angle, and slice position. The images generated during the CT acquisitions were used to create the geometrical models, by applying thresholding algorithms and assigning materials to the HU values. The TLDs were contoured in the phantom models as sensitive cylindrical volumes at the locations selected for dosimeters placement, to provide dose estimates, in terms of dose per unit photon. To compare measured doses with dose estimates, a calibration factor was derived from the CTDI<sub>vol</sub> displayed by the scanner, to account for the number of photons emitted by the X-ray tube during the procedure.RESULTS: The differences of the measured and estimated doses, in terms of absolute % errors, were within 13% for 153 TLD locations, with an error of 17% at the stomach for one study with the 10-year-old phantom. Root-mean-squared-errors (RMSE) across all TLD locations for all configurations were in the range of 3%-8%, with Acuros providing dose estimates in a time range of a few seconds up to 2 min.CONCLUSIONS: An overall good agreement between measurements and simulations was achieved, with average RMSE of 6% across all cases. The results demonstrate that Acuros can model a specific clinical scanner despite the required discretization in spatial and energy domains. The proposed deterministic tool has the potential to be part of a near real-time individualized dosimetry monitoring system for CT applications, providing patient-specific organ dose estimates.

Assessment of Uncertainties Associated with Monte Carlo-based Personalized Dosimetry in Clinical CT Examinations

Abstract:The clinical value of x-ray computed tomography (CT) has skyrocketed in the last decade while at the same time being the main source of medical exposure to the population. Concerns regarding the potential health hazards associated with the use of ionizing radiation were raised and an appropriate estimation of absorbed dose to patients is highly desired. In this work, we aim to validate our developed Monte Carlo CT simulator using in-phantom dose measurements and further assess the impact of personalized scan-related parameters on dosimetric calculations. We developed a Monte Carlo-based CT simulator for personalized organ level dose calculations, in which the CT source model, patient-specific computational model and personalized scanning protocol were integrated. The CT simulator was benchmarked using an ionization chamber and standard CT Dose Index phantom while the dosimetry methodology was validated through experimental measurements using thermoluminescent dosimeters (TLDs) embedded within an anthropomorphic phantom. Patient-specific scan protocols extracted from CT raw data and DICOM image metadata, respectively, were fed as input into the CT simulator to calculate individualized dose profiles. Thereby, the dosimetric uncertainties associated with using different protocol-related parameters were investigated. The absolute absorbed dose difference between measurements and simulations using the ionization chamber was less than 3%. In the case of the anthropomorphic phantom, the absolute absorbed dose difference between simulations and TLD measurements ranged from -8.3% to 22%, with a mean absolute difference of 14% while the uncertainties of protocol-related input parameters introduced an extra absolute error of 15% to the simulated results compared with TLD measurements. The developed methodology can be employed for accurate estimation of organ level dose from clinical CT examinations. The validated methodology can be further developed to produce an accurate MC simulation model with a reduced computational burden.

Abstract:Purpose: 4DCT is an important technique for motion management in radiotherapy, but involves a significant amount of dose to patients. Low-dose helical 4DCT scans can be done by (a) lowering tube current and pulse duration, (b) increasing helical pitch, and (c) reducing data sampling. Conventional FDK methods may fail for these low-dose scan protocols due to noisy or incomplete data. We aim to develop an accurate and fast image reconstruction solver that enables dose reduction for helical 4DCT. Methods: For improved accuracy, the developed solver is based on a novel 4D regularization method for CT image reconstruction, which promotes both the smoothness in spatial variables for each phase and the similarity in temporal variables between phases, in contrast with the 3D regularization method with spatial regularization only. Both 3D and 4D methods here are based on total variation. The formulated optimization problem is solved by alternating direction method of multipliers (ADMM), which is chosen for its effectiveness for solving the convex problem, and more importantly its suitability for distributed computing.For improved speed, the developed solver utilizes an efficient parallel algorithm for X-ray transform and its adjoint with optimized computational cost per parallel thread O (1) that takes flying focal spots into account, and multi-GPU platforms for solving such a massive parallel computing problem in helical 4DCT. Results: The solver was validated using both simulations (4D NCAT phantoms) and experimental sinogram data (Siemens, Somatom 64-slices) with low-dose, high-pitch and sparse-view helical 4DCT scans. Both the reconstructed images and quantitative results demonstrate that the 4D method improved image quality compared with the 3D method, which is in turn better than FDK. Conclusion: We have developed a fast, accurate solver that synergizes state-of-art iterative reconstruction methods and parallel computing techniques that can potentially enable low-dose, high-pitch and sparse-view helical 4DCT scans. Abstract:The most mature image reconstruction algorithms in multislice helical computed tomography are based on analytical and iterative methods. Over the past decades, several methods have been developed for iterative reconstructions that improve image quality by reducing noise and artifacts. In the regularization step of iterative reconstruction, noise can be significantly reduced, thereby making low-dose CT. The quality of the reconstructed image can be further improved by using model-based reconstructions. In these reconstructions, the main focus is on modeling the data acquisition process, including the behavior of the photon beams, the geometry of the system, etc. In this article, we propose two model-based reconstruction algorithms using a virtual detector for multislice helical CT. The aim of this study is to compare the effect of using a virtual detector on image quality for the two proposed algorithms with a model-based iterative reconstruction using the original detector model. Since the algorithms are implemented using multiple GPUs, the merging of separately reconstructed volumes can significantly affect image quality. This issue is often referred to as the "long object" problem, for which we also present a solution that plays an important role in the proposed reconstruction processes. The algorithms were evaluated using mathematical and physical phantoms, as well as patient cases. The SSIM, MS-SSIM and L1 metrics were utilized to evaluate the image quality of the mathematical phantom case. To demonstrate the effectiveness of the algorithms, we used the CatPhan 600 phantom. Additionally, anonymized patient scans were used to showcase the improvements in image quality on real scan data.

Automated computation of topological derivatives with application to nonlinear elasticity and reaction-diffusion problems

Abstract:While topological derivatives have proven useful in applications of topology optimisation and inverse problems, their mathematically rigorous derivation remains an ongoing research topic, in particular in the context of nonlinear partial differential equation (PDE) constraints. We present a systematic yet formal approach for the computation of topological derivatives of a large class of PDE-constrained topology optimization problems with respect to arbitrary inclusion shapes. Scalar and vector-valued as well as linear and nonlinear elliptic PDE constraints are considered in two and three space dimensions including a nonlinear elasticity model and nonlinear reaction-diffusion problems. The systematic procedure follows a Lagrangian approach for computing topological derivatives. For problems where the exact formula is known, the numerically computed values show good coincidence. Moreover, by inserting the computed values into the topological asymptotic expansion, we verify that the obtained values satisfy the expected behaviour also for other, previously unknown problems, indicating the correctness of the procedure. We present a systematic approach for the computation of topological derivatives that is applicable to a large class of problems. Most notably, our approach covers the topological derivative for a nonlinear elasticity problem, which has not been reported in the literature.

Abstract:The utilization of Monte Carlo simulation is a highly effective method in advancing research within the field of nuclear medicine. An important software tool for simulating and modeling cutting-edge systems for Positron Emission Tomography (PET) is the Geant4 Application for Tomographic Emission (GATE). This research specifically concentrates on two Siemens PET scanners: the Siemens Biograph True point (True V) and Siemens Biograph mcT 20 excel. Our primary objective was to validate the GATE V8.2 simulation in accordance with the NEMA (National Electrical Manufacturers Association) NU 2-2018 protocol, using a parallel computing platform on a local cluster of computers. This cluster was managed by the open-source package TORQUE version 6.1.0, which is based on the original wrapper PBS. The results of this validation process for the Siemens Biograph True point (True V) scanner were highly promising. The scatter fraction (SF), peak noise equivalent count rate (NECR), and sensitivity all demonstrated a remarkable level of agreement, with deviations within 10.02%, 5.4%, and 1.57%, respectively. Additionally, the spatial resolution was found to deviate by less than 0.59 mm. Image evaluations demonstrate high-quality results for both Siemens Biograph True point (True V) and Siemens Biograph mcT 20 excel scanners, validating the GATE simulation. The study on acquisition time provides insights into optimizing computing simulation time. This research demonstrates the effectiveness of Monte Carlo simulation, facilitated by GATE, in accurately modeling advanced PET systems. The validation results affirm the reliability of GATE V8.2 in accordance with NEMA standards for the Siemens Biograph True point (True V) scanner. The high-quality images obtained from GATE-validated scanners underscore the potential of this simulation approach in advancing nuclear medicine research. Additionally, the study on acquisition time provides valuable considerations for optimizing simulation efficiency.
