Abstract:This study aimed to analyze the prevalence of work-related musculoskeletal disorders (WMSD) and their association with individual and contextual factors in the Brazilian population. This quantitative cross-sectional study used secondary data from the Brazilian National Health Survey from August 2013 to February 2014. The dependent variable included WMSD, and independent variables were analyzed as individual and contextual factors. WMSD was mostly prevalent in females, individuals aged 43 to 59 years, with chronic physical or mental disorders, reporting frequent sleep disorders, and performing integrative and complementary health practices, physical exercise or sports, and heavy physical activity or housework. Regarding contextual factors, high social classes and proportion of individuals with formal work were associated with a high prevalence of WMSD, whereas a high Gini index was associated with a low prevalence. Thus, a high prevalence of WMSD in the Brazilian population was associated with individual and contextual factors, which should be the target of health professionals for actions of promotion, prevention, and intervention at individual or collective care levels.

Abstract:Abstract This article aims to estimate the prevalence of musculoskeletal disorders (MD) on the adult population of Campinas, São Paulo, Brazil, verifying associated demographic and socioeconomic factors, and to analyze their impact on Health-Related Quality of Life (HRQoL) according to sex. A population-based study was conducted with 2,166 individuals using data from the ISACamp 2014/15. The Medical Outcomes Study SF-36-Item Short Form Health Survey (SF-36) was used to measure HRQoL according to MD. Prevalence ratios (PR) were estimated by Poisson regression. Musculoskeletal disorders had a prevalence of 8.5% (6.7% tendonitis and 2.7% work-related musculoskeletal disorders - WMSD). Results showed a higher prevalence of musculoskeletal disorders in women, active or on leave due to illness, and in individuals with higher education levels. Moreover, reduced HRQoL scores were observed in 6 of the 8 domains, due to MD. The mental component and physical component showed greater impairment respectively among women and men after self-reported WMSD. These findings point to substantial damage from musculoskeletal disorders on the population’s HRQoL. WMSD affect the HRQoL of men and women distinctly.

Abstract:This study aimed to verify the direct and indirect influence of risk factors on the symptoms of WMSDs in the thigh, leg, and foot among workers in the inner regions of Brazil . Research tested a structural equation model (SEM) that describes hypothetical relationships between risk factors and WMSDs. Questionnaires were used to collect data on sociodemographic, occupational, biomechanical, and psychosocial factors , and pain symptoms about 420 workers. The results showed that different factors directly affect the thigh, leg, and foot. Maintaining the lower limbs in uncomfortable positions increases the chances of developing WMSDs in the thigh (β = 0.219; p-value <0.001), leg (β = 0.223; p-value <0.001), and foot (β = 0.158; p-value <0.01). Most psychosocial factors attenuate psychosocial stress, which in turn directly alleviates WMSD symptoms in the thigh (β = −0.502; p-value <0.001), leg (β = −0.315; p-value <0.01), and foot (β = −0.246; p-value <0.01). Factors were also found to uniquely affect aspects such as "another job" (β = −0.246; p-value <0.01) in the leg and effort (β = 0.146; p-value <0.05) in the foot. It was concluded that multiple factors influence the development of WMSDs in the lower limbs. These findings contribute to the existing knowledge on the causes of WMSDs and can help improve working conditions for the population under study. Therefore, managers and health, ergonomics, and occupational safety professionals can use the findings to develop more precise strategies to reduce the impacts of WMSDs on workers.

Abstract:This study aimed to analyze the incidence of work-related musculoskeletal disorders (WMSD) in Brazil from 2007 to 2019, examining the spatial, temporal, and spatiotemporal patterns of their occurrence. An ecological time series study was conducted using spatial analysis techniques. WMSD morbidity data from 2007 to 2019 were collected from the Brazilian Information System for Notificable Diseases of the Brazilian Health Informatics Department. Incidence rates were standardized and smoothed using the local empirical Bayes' theorem. Time trends were analyzed by segmented linear regression. Spatial analysis was performed using Moran's univariate global (I) and local (LISA) indexes. The spatiotemporal scan statistic was used to identify high-risk spatiotemporal clusters for WMSD. A total of 93,387 cases of WMSD were recorded in Brazil. Temporal trends showed an increase in all regions except the Northeast, which remained stable. The incidence of WMSD showed a spatial dependence, with spatial and space-time clusters identified, especially in the Southeast region, overlapping the largest economic-industrial center of the country. The spatiotemporal clustering observed in one region suggests the highest level of industrial and economic development. Our findings highlight the need to implement intersectoral surveillance policies, inspect working conditions, and invest in the prevention and promotion of workers' health.

Abstract:Background: Iron-storage Dps proteins are expressed in prokaryotes to confer resistance to specific stressful conditions. Results: Dps from Porphyromonas gingivalis (PgDps) is able to bind heme through a conserved cysteine and protects DNA against H2O2-mediated degradation. Conclusion: The heme sequestration property of PgDps confers resistance to heme toxicity. Significance: This Dps protection strategy may be common among members of the order Bacteroidales. The widely expressed DNA-protective protein from starved-cells (Dps) family proteins are considered major contributors to prokaryotic resistance to stress. We show here that Porphyromonas gingivalis Dps (PgDps), previously described as an iron-storage and DNA-binding protein, also mediates heme sequestration. We determined that heme binds strongly to PgDps with an apparent Kd of 3.7 × 10−8 m and is coordinated by a single surface-located cysteine at the fifth axial ligand position. Heme and iron sequestered in separate sites by PgDps provide protection of DNA from H2O2-mediated free radical damage and were found to be important for growth of P. gingivalis under excess heme as the only iron source. Conservation of the heme-coordinating cysteine among Dps isoforms from the Bacteroidales order suggests that this function may be a common feature within these anaerobic bacteria.

Abstract:Magnesium is used implant material potentially for non-toxicity to the human body. Due to the excellent bio-compatibility, Mg alloys is applied to implants avoiding removal second surgery. However, it is found commercial magnesium alloys including aluminum has low corrosion resistance, resulting subcutaneous gas bubbles and consequently the approach as permanent bio-materials. Generally, Aluminum is known to pollution substance, and it raises toxicity to nervous system. Therefore especially Mg-35Zn-3Ca alloy is prepared for new biodegradable materials in this study. And the pulsed power is used in constant-current mode of DC power kinds of anodization. Based on the aforementioned study, it examines corrosion resistance and biocompatibility by effect of current and frequency variation. The surface properties and thickness were compared using scanning electronic microscopy. Corrosion resistance was assessed via potentiodynamic polarization and the effect of oxide layer on the body was assessed cell viability. Low back pain should be considered a health and research priority in Brazil: Lost productivity and healthcare costs between 2012 to 2016

Abstract:Background: Low Back Pain (LBP) is associated with an increase in disability-adjusted life years, and increased risk of disability retirement and greater absenteeism in Brazil. Hence, evidence on healthcare and lost productivity costs due to LBP is of utmost importance to inform decision-makers. Methods: Cost-of-illness study with top-down approach, and societal perspective. We extracted data from National databases, considering the period 2012-2016. Outpatient expenses included clinical, surgical, diagnosis, orthosis/prosthetics, and complementary actions. Inpatient care expenses included hospital and professional services, intensive care unit, and companion stay. For productivity losses, duration of work absence and associated information (work-related and non-work-related; value of the sickness absence benefit; age; gender; and economic activity) were analyzed. Lost productivity costs were calculated multiplying the absence from work (days) by the daily-benefit. Results: The societal costs amounted to US$ 2.2 billion, and productivity losses represented 79% of the costs. Total healthcare expenses were estimated to US$ 460 million. We found more than 880,000 diagnostic images. Individuals with LBP were in total 59 million days absent from work between 2012-2016. The mean lost days absent from work per person, for each year investigated was, respectively, 88; 84; 83; 87; and 100. Men were more days absent from work than women. In addition, rural workers presented greater absence from work compared to other professional activities. Conclusion: Healthcare expenses and lost productivity costs due to LBP were substantial, hence, there is a need for improvement of health services and policies to deal with this increasing burden of illness. We found an extensive use of diagnostic imaging, which is rather discouraged by clinical guidelines. We assume that men were experiencing high levels of back pain disability compared with women, as they presented greater absenteeism and higher lost productivity costs.

Abstract:BACKGROUND: Work-related musculoskeletal disorders (WRMSDs) are one of the main causes of morbidity among healthcare professionals. It has various secondary consequences on productivity by diminishing the quantity and quality of work completed by the affected personnel, in addition to having a primary impact on the individual with pain and discomfort. OBJECTIVE: The study aims to determine the overall prevalence rate of WRMSD among dentists, physiotherapists, and surgeons and also identify the commonly affected regions of the body about specific health care professions among each of the three professions, as recorded by the cross-sectional studies performed in various countries and regions of the world. METHODS: A systematic search strategy was framed following the PRISMA guidelines based on the present inclusion and exclusion criteria. A critical search of articles was conducted during June 2020 in CINAHL (DOAJ), PubMed, Google Scholar Scopus, PEDro databases and SAGE journals. RESULTS: Out of the 42 articles that met the eligibility criteria, there were 39 cross-sectional studies, 2 pilot cross-sectional surveys and 1 prospective cohort study with one-year follow-up. All studies included in this review used various survey tools for recording the demographic details and measuring the prevalence of WRMSDs and other outcome factors. CONCLUSION: We conclude that all three health care professionals (dentists, physiotherapists and surgeons) are highly prone to develop WRMSDs with surgeons and dentists being more vulnerable when compared to physiotherapists. The lower back and neck are identified as the two most commonly affected regions among all three professionals.

Abstract:Violence against women is an important public health problem in Brazil and in the world. The objective of the present study was to describe the profile of mortality due to aggression against women, and analyze whether the victims of reported violence are more likely to die from aggression than the general female population. This is a descriptive study of mortality due to aggression against women, from database linkage. The databases used for linkage were SINAN Brazil's Notifiable Diseases Information System (reports of violence against women) (2011 - 2015), and SIM, the Mortality Information System (women dying as a result of aggression) (2011 - 2016). The risk of death due to aggression among women previously reporting violence is higher than in the general female population, thus revealing a situation of vulnerability. Black women with lower schooling are the main victims of violence and homicides. The large number of women killed by aggression and repeated violence reveal the fragility of the care and protection networks in providing comprehensive, qualified and timely care for victims.

Abstract:Background The prevalence of obesity has increased substantially in the last decades in the elderly population and, consequently, it has been considered one of the main risk factors for non-communicable diseases in the world. Compared to general obesity, central obesity, which is characterized by excessive accumulation of fat in the abdominal region, has shown a greater association with diabetes and all-cause mortality, in addition to being considered a risk factor for disability in the elderly, regardless of BMI. Some studies suggest that the relationship between central obesity and disability differs according to gender, however, the results are still conflicting. Objectives To investigate the association between central obesity and mobility limitation in elderly Brazilians and to assess whether this association was modified by gender. Methods This is a cross-sectional, analytical, and quantitative study that used data from the PNS. This is a household-based survey proposed by the Ministry of Health in partnership with the Brazilian Institute of Geography and Statistics (IBGE). For the present study, those aged 60 years or older were considered (n=11,177). Central obesity was defined when waist circumference (WC) was greater than 84 cm in women and 88 cm in men. Participants reported the degree of difficulty to move, being classified as no difficulty versus some difficulty. Binary logistic regression was used to investigate the association between central obesity and mobility limitation adjusted for sex, age, exercise, chronic diseases, self-reported health, and literacy. P<0.05 and a 95% confidence interval were considered. Results Women were the majority, representing 55.7% of the sample and the mean age was 66.9 ± 8.07 years. Central obesity was identified in 76.7% of the participants, being more prevalent among older women (60.4%) and 25.9% reported some difficulty with mobility, with a higher prevalence of females (64.8%). Elderly people with central obesity were 1.2 times more likely to have some difficulty with mobility (OR=1.21; 95%CI: 1.08-1.36) compared to elderly people who did not have any difficulty, even after adjusting the covariates. In the analysis stratified by gender, the association between central obesity and mobility limitation was significant among women (OR=1.48; 95%CI: 1.27-1.7; however, it, lost significance among men (OR =0.93;95%CI: 0.78-1.10). Conclusion The results suggest that elderly women with central obesity are more likely to have some difficulty with mobility compared to men. Implications The high prevalence of central obesity in the Brazilian elderly population, especially in the female public, represents a serious public health problem. Our findings identified a subgroup of the elderly population that is more vulnerable to limited mobility associated with abdominal obesity. Therefore, implementing health actions to prevent or reduce abdominal obesity should be strongly encouraged among elderly women.

Abstract:Introduction: Many health changes are related to how individuals respond to work events that are influenced by organizational, physical, and human factors. Objectives: We aim to evaluate the mental workload, the manifestation of musculoskeletal disorders, the associated factors, and their relationships among international transport truck drivers. Methods: This is a descriptive, cross-sectional diagnostic study of 70 truck drivers who entered the intermodal terminal of the city of Uruguaiana, state of Rio Grande do Sul, Brazil, in January 2021. Participants completed the NASA Task Load Index questionnaire and the Nordic Musculoskeletal Symptom Questionnaire, along with questions about work characteristics and health care. Results: The results showed that most truck drivers have a high mental workload (90.0%) and have experienced musculoskeletal symptoms in the past 12 months (72.9%). In addition, this occupational group has characteristics that are predictive of health problems, as most work over 10 hours a day (61.5%). The results also show a correlation between mental workload, hours worked, and lower back injuries (p < 0.05). There is also an association between hours worked and the occurrence of musculoskeletal disorders in the neck, shoulders, and lower back in the past 12 months and in the shoulders and lower back in the past 7 days, especially for those who work over 12 hours a day. Conclusions: The population studied had a high mental workload and a high prevalence of musculoskeletal disorders as well as occupational characteristics predictive of harm to both physical and mental health.
Prevalence and risk factors for anxiety and depression disorders in workers with work-related musculoskeletal strain or sprain in British Columbia, Canada: a comparison of men and women using administrative health data

Abstract:Objective To examine the prevalence and risk factors for medically treated anxiety and depression disorders among men and women with musculoskeletal strain or sprain work injury in British Columbia, Canada. Methods A retrospective population-based cohort of accepted workers’ compensation lost-time claims from 2000 to 2013 was constructed using linked administrative health data. Anxiety and depression disorders were identified using diagnoses from physician, hospital and pharmaceutical records. The 1-year period prevalence was estimated for the year before and the year after injury. Sociodemographic, clinical and work-related risk factors for prevalent and new onset anxiety and depression disorders were examined using multinomial regression. Results 13.2% of men and 29.8% of women had medically treated anxiety, depression or both in the year before injury. Only a slight increase (~2%) in the prevalence of these disorders was observed in the year after injury. Somatic and mental comorbidities were both strong risk factors for pre-existing and new onset anxiety and depression for both men and women, but these relationships were stronger for men. Conclusion Anxiety and depression disorders including those from prior to injury are common in workers with musculoskeletal strain or sprain and are associated with a complicated clinical profile. Gender-sensitive and sex-sensitive mental healthcare is an important consideration for work disability management.

The burden of neck pain in Brazil: estimates from the global burden of disease study 2019

Abstract:Abstract Background This study analyzed neck pain estimates in Brazil and its states between 2000 and 2019, in view of the country’s lacking epidemiological data. Methods An analysis was performed of the GBD 2019 estimates by location, sex, and age, per 100,000 population, with uncertainty intervals (95% UI). Brazilian estimates were compared to global, Mexican, English, and American rates. Results Global, Brazilian, and Mexican prevalence numbers were statistically homogeneous and stable in the period. Throughout the period analyzed in the study, Brazilian neck pain prevalence (2241.9; 95%UI 1770.5–2870.6) did not show statistical differences when compared to global (2696.5; 95%UI 2177.0–3375.2) or Mexican (1595.9; 95%UI 1258.9–2058.8) estimates. Estimates observed in the USA (5123.29; 95%UI 4268.35–6170.35) and England (4612.5; 95%UI 3668.8–5830.3) were significantly higher. In 2019, when compared to the USA and England, age-standardized prevalences were lower globally, in Brazil, and in Mexico. Prevalences in Brazilian states were similar, being that Roraima (1915.9; 95%UI 1506.5–2443.1) and the Federal District (1932.05; 95%UI 1515.1–2462.7) presented the lowest and highest values respectively. The exception was the state of São Paulo (3326.5; 95%UI 2609.6–4275.5). There was no statistical difference by sex, but the prevalence tended to increase with aging. In 2019, the Brazilian prevalence was 2478.6 (95% UI 1791.0–3503.8), 5017.2 (95%UI 3257.26–7483.8), and 4293.4 (95% UI 2898,8–6343.9), for those aged 15 to 49, 50 to 69, and 70+ years. There was no statistical difference among the YLDs in all locations and times. Conclusions Brazil is going through a fast-paced process of populational aging; a higher prevalence of neck pain in middle-aged individuals and the elderly highlights the need for lifelong prevention initiatives. The higher rates observed among higher-income populations and the homogeneity of the Brazilian estimates suggest a lack of robust epidemiological data in lower-income countries.

Abstract:OBJECTIVE: To investigate the prevalence of low back pain in truck drivers and check for possible risk factors related to the presence of low back pain.METHODS: A questionnaire was developed including personal factors such as age, practice of physical and sports activities, general health habits, occupation-related issues, and factors associated with low back pain. The body mass index (ratio of body weight to the square of height, kg/m2) was calculated. Of 489 male truck drivers assessed, 410 were enrolled into the study. The statistical analysis was divided into 2 stages: descriptive analysis of data, and binary logistical regression, where the response (dependent) variable (presence of low back pain) was associated with each independent variable to check for correlation with low back pain.RESULTS: Of the 410 truck drivers evaluated, 242 (59%) presented with low back pain, while 168 (41%) did not have low back pain. The only factor correlated with the presence of low back pain was the number of working hours; the other factors had no correlation with low back pain.CONCLUSION: The prevalence of low back pain in truck drivers was 59%. The number of working hours was the only variable associated with occurrence of low back pain, with an average of about 1 hour longer work time for drivers with low back pain compared to those without it and with an odds ratio of 1.07 (P = 0.026).

Abstract:Abstract Background The prevalence and burden of musculoskeletal (MSK) conditions are growing around the world, and low back pain (LBP) is the most significant of the five defined MSK disorders in the Global Burden of Disease (GBD) study. LBP has been the leading cause of non-fatal health loss for the last three decades. The objective of this study is to describe the current status and trends of the burden due to LBP in Brazil based on information drawn from the GBD 2017 study. Methods We estimated prevalence and years lived with disability (YLDs) for LBP by Brazilian federative units, sex, age group, and age-standardized between 1990 and 2017 and conducted a decomposition analysis of changes in age- and sex-specific YLD rates attributable to total population growth and population ageing for the purpose of understanding the drivers of changes in LBP YLDs rates in Brazil. Furthermore, we analyzed the changes in disability-adjusted life years (DALYs) rankings for this disease over the period. Results The results show high prevalence and burden of LBP in Brazil. LBP prevalence increased 26.83% (95% UI 23.08 to 30.41) from 1990 to 2017. This MSK condition represents the most important cause of YLDs in Brazil, where the increase in burden is mainly related to increase in population size and ageing. The LBP age-standardized YLDs rate are similar among Brazilian federative units. LBP ranks in the top three causes of DALYs in Brazil, even though it does not contribute to mortality. Conclusions Findings from this study show LBP to be the most important cause of YLDs and the 3rd leading cause of DALYs in Brazil. The Brazilian population is ageing, and the country has been experiencing a rapid epidemiological transition, which generates an increasing number of people who need chronic care. In this scenario, more attention should be paid to the burden of non-fatal health conditions.

Risk of WMSDs in monofunctional and multifunctional workers in a Brazilian footwear company

Abstract:This study aimed to analyze the risk of musculoskeletal disorders in monofunctional and multifunctional workers in a footwear company. The sample comprised 114 workers in the shoe production sector. The method Occupational Repetitive Actions was used to assess the risk of work-related musculoskeletal disorders (WMSDs). Proportional odds models were constructed, relating the risk of WMSDs to the type of work and the worker’s level of multifunctionality. For monofunctional workers, exposure to the higher risk was related to cycle time and the technical actions within their activities, whereas for multifunctional workers, it was related to the range of motion, use of gloves and precision needed in activities. For monofunctional workers, greater risks were associated with a short activity cycle, whereas for multifunctional workers, they were associated with complementary and organizational factors. Moreover, workers whose intracellular activities were less than 30% of the total appeared to be less exposed to the risk of WMSDs.
Prevalence of musculoskeletal disorders among perioperative nurses: a systematic review and META-analysis

Abstract:Abstract Background To evaluate the prevalence of work-related musculoskeletal disorders (WRMSDs) in perioperative nurses and to explore their association with personal characteristics. Methods Medline, Cumulative Index to Nursing and Allied Health Literature, Scopus, Web of Science, Cochrane Library and Joanna Briggs Institute Database were systematically searched. A meta-analysis calculating event rates, and relative 95% Confidence Intervals (CI) was performed for each musculoskeletal body region. The contribution of perioperative nurses’ sex, age, and BMI was assessed through a meta-regression. Results Twenty-two studies, considering 3590 perioperative nurses, were included in the systematic review. The highest prevalence of WRMSDs was found for the lower-back (62%; 95% CI 0.54–0.70), followed by knee (47%; 95% CI 0.36–0.59), shoulder (44%; 95% CI 0.37–0.51), waist (42%; 95% CI 0.31–0.53), neck (39%; 95% CI 0.29–0.51), ankle-feet (35%; 95% CI 0.22–0.51), upper-back (34%; 95% CI 0.25–0.44), hand-wrist (29%; 95% CI 0.20–0.40), and elbow (18%; 95% CI 0.12–0.26). Meta-regression showed that sex, age, and BMI were not significant predictors of low-back disorders ( p = 0.69; R 2 = 0). Conclusions WRMSDs represent a high prevalence issue among perioperative nurses. Perioperative nurses, in general, are steadily exposed to both physical and temporal risk factors. Further studies should be addressed to identify specific interventions aimed at reducing the burden of WRMSDs including ergonomic education and physical rehabilitation. Our data could be used in future studies as a reference to assess the risk of WRMSDs in other health-care professionals’ population.
