Abstract:Guipi Decoction is a famous Traditional Chinese Medicine formulae, which can be used to treat insomnia and depression. But its speci�c mechanism is still unclear. In this study, the active ingredients, targets and mechanisms of the Ziziphi Spinosae Semen-Poria cocos-Longan (ZPL) in Guipi Decocation was investigated by network pharmacology analysis and molecular docking. A total of 32 active ingredients, 344 intersection targets and 13 key targets were obtained. The result showed (S)-Coclaurine, coumestrol, n-trans-feruloyltyramine, ellagic acid, ellipticine and tartaric acid were the key compounds for depression. These compositions could reduce in�ammatory response, inhibit pro-inammatory cytokines, reduce neuronal apoptosis, and then modulate depression by the key targets of RAC-alpha serine/threonine-protein kinase (AKT1), tumor necrosis factor (TNF), interleukin (IL6), mitogen activated protein kinase 3 (MAPK3). Molecular docking results showed that the binding energy of n-trans-feruloyltyramine with PPARG was the lowest, -9.513 kcal/mol and the binding energy of (S)-Coclaurine and ESR1 was − 9.336. Upregulation of AKT1 gene inhibits apoptosis. Downregulation of TNF-α , MAPK and CTNNB1 genes reduces the expression of in�ammatory factors and decreases the in�ammatory response, which plays an important role in the treatment of depression. In conclusion, the active component of ZPL binds stably with AKT1, MAPK3, ESR1 and CTNNB1 and controlled the onset of depression by regulation of genes expression.

Abstract:Background: Depression is a common, chronic, and recurrent mood disorder that has become a worldwide health hazard. Fluoxetine hydrochloride, a common treatment method, can inhibit 5-hydroxytryptamine (5-HT) recycling in the presynaptic membrane; however, the efficacy of a single drug is inadequate. At present, mild-to-moderate depression can be treated with acupuncture of ghost caves, but the clinical curative effect of combined therapy with fluoxetine hydrochloride has not been sufficiently reported. Aim: To evaluate the clinical effect of acupuncture at ghost points combined with fluoxetine hydrochloride in the treatment of mild-to-moderate depression. Methods: This retrospective study included 160 patients with mild-to-moderate depression who were admitted to Shanghai Hospital of Integrated Traditional Chinese and Western Medicine, Affiliated to Shanghai University of Traditional Chinese Medicine, between January 2022 and June 2023. Patients were separated into a single-agent group (fluoxetine hydrochloride treatment, n = 80) and a coalition group (fluoxetine hydrochloride treatment combined with acupuncture at ghost points, n = 80). Pre-treatment symptoms were recorded, and the clinical curative effect and adverse reactions [Asberg Antidepressant Side Effects Scale (SERS)] were assessed. Depression before and after treatment [Hamilton Depression Scale (HAMD)-24], neurotransmitter levels [5-HT, norepinephrine (NE), dopamine (DA)], oxidative stress indicators [superoxide dismutase (SOD), malondialdehyde (MDA)], and sleep quality [Pittsburgh Sleep Quality Index (PSQI)] were compared. Results: The total efficacy rate was 97.50% in the coalition group and 86.25% in the single-agent group (P < 0.05). After 2, 4, 6, and 8 wk of treatment, the HAMD, self-rating depression scale, and SERS scores of the coalition and single-agent groups decreased compared with pre-treatment, and the decrease was more significant in the coalition group (P < 0.05). After 8 wk of treatment, the levels of NE, DA, 5-HT, and SOD in the coalition and single-agent groups increased, while the levels of MDA decreased; the increases and decrease in the coalition group were more significant (P < 0.05). The PSQI scores of the coalition and single-agent groups decreased, and the decrease was more significant in the coalition group (P < 0.05). Conclusion: Acupuncture at ghost points combined with paroxetine tablets can safely improve depressive symptoms and sleep disorders, regulate neurotransmitter levels, and reduce stress responses in patients with mild-to-moderate depression.
Modified Xiaochaihu Decoction Combined with Mirtazapine in the Treatment of Persistent Depression: A Pilot Randomized Controlled Trial

Abstract:Background. Western drugs effectively manage persistent depressive disorder (PDD) but are associated with side effects. Objective. To observe the efficacy and safety of modified Xiaochaihu Decoction combined with mirtazapine in treating PDD. Methods. Patients with PDD were enrolled at the Naval General Hospital (06/2018–02/2019) and randomized to modified Xiaochaihu Decoction and modified Xiaochaihu Decoction with mirtazapine. The self-rating depression scale (SDS) and traditional Chinese medicine (TCM) scale were assessed at baseline and after 12 weeks. The overall clinical efficacy (primary outcome) and adverse reactions were observed. Results. Sixty-four participants completed the trial in the combined and control groups (30 and 28), respectively. In controls, the total effective rate was 78.6%, compared with 96.7% in the combined group ( P = 0.035 ). The scores of the SDS and TCM syndrome scale in the two groups were lower after treatment ( P < 0.001 ) but without difference between groups ( P = 0.077 ). The combined group showed higher improvement rates regarding insomnia (96.4% vs. 44.0%, P < 0.001 ), bitter taste (90.5% vs. 52.6%, P = 0.007 ), languid (72.0% vs. 31.8%, P = 0.006 ), and belching/anorexia (100% vs. 52.6%, P < 0.001 ). The combined group showed a higher frequency of adverse events (73.3% vs. 3.6%) ( P < 0.001 ). Conclusion. Modified Xiaochaihu Decoction combined with mirtazapine effectively treats PDD, and its curative effect is better than that of TCM alone. Trial Registration. Abstract:Severe depression accounts for one-third of depressed patients. Increasing severity of depression usually hinders patients from achieving remission. This study evaluated the efficacy and safety of escitalopram in acute-phase treatment of severe major depressive disorder (MDD). A total of 225 participants with severe MDD (Diagnostic and Statistical Manual of Mental Disorders, 4th Edition criteria), with a current depressive episode and Montgomery-Asberg Depression Rating Scale (MADRS) score of ≥30 were enrolled. Participants received flexible dose escitalopram (10–20 mg/d) treatment for 8 weeks. Symptoms status was assessed by MADRS, Hamilton Depression Rating Scale (HAM-D-17), and Hamilton Anxiety Rating Scale (HAM-A). Quality of life was assessed by Short Form-12 (SF-12) and safety by adverse events, laboratory investigations, vital signs and physical findings. The remission (MADRS total score ≤ 10) rate in the intent-to-treat set (n = 207) was 72.9% at week 8. Significant improvement in symptoms compared to baseline, as evaluated by MADRS, HAMD-17 and HAMA scores at baseline, week 1, week 2, week 4, and week 8 (p < 0.0001 for all), was noted. Mean (SD) reduction from baseline in MADRS total score was 26.6 (11.38). Improvements in SF-12 score were significant (p = 0.000) and positively related to symptom improvement and negatively related to treatment-emergent adverse events (TEAEs). TEAEs were reported in 28.38% of participants. Most common TEAEs (>4%) were somnolence (9.0%), nausea (7.7%), hyperhidrosis (4.5%), dry mouth and dizziness (4.1% each). No serious TEAEs were reported. Escitalopram was effective and well-tolerated for acute-phase treatment of severe depression in Chinese population.

Abstract:This study aimed to evaluate the efficacy of escitalopram monotherapy in the treatment of major depressive disorder (MDD) on the basis of pooled data analysis of 4 Chinese clinical trials. A total of 649 outpatients with MDD score of >= 18 at the 17-item Hamilton Depression Rating Scale (HAMD(17)) were included across 4 eligible studies. Patients were treated with 10mg/day escitalopram for 2 weeks, and then 20mg/day escitalopram was administered if the clinical response was poor. The change in total HAMD(17) score was significantly greater in moderate MDD group than in other subgroups (P<.001), but the proportion of responders and remission rate in moderate MDD group were markedly lower than in mild MDD group. As compared to patients with concomitant anxiety, anxiety free patients showed significant improvement in total HAMD17 score at days 14 and 28 (P<.05). However, there was no significant difference in the change of total HAMD17 score at day 7 and the end of study. According to clinical global impression (CGI) score, the total response rate (very much improved and much improved) was 86.7%. There were 479 adverse events (AEs), but serious AEs were not observed. A total of 3.39% (22/649) of patients withdrew from these studies due to AEs. The most common (incidence >= 2.0%) AEs were nausea, dry mouth, somnolence, dizziness, fatigue, dyspepsia, liver dysfunction, and loss of appetite. Escitalopram monotherapy is effective and safe in the treatment of MDD in Chinese patients, and therapeutic efficacy is dependent on the severity of MDD. Further study is needed to identify better predictors of therapeutic responses.
Efficacy of fluoxetine combined with psychological intervention for treatment of depressive patients with insomnia

Abstract:Objective: The aim of the current study was to investigate the clinical effects of fluoxetine combined with psychological intervention on depressive patients with insomnia, examining influences on sleep quality and quality of life. Methods: One hundred and seventy-eight patients with depression and insomnia were selected as research subjects. A total of 98 patients were treated with fluoxetine combined with psychological intervention (group A). A total of 80 patients were treated with fluoxetine only (group B). Clinical therapeutic effects of the two groups were observed. Depression degrees of the patients were evaluated via Hamilton depression rating scale (HAMD) scores. Sleep quality was assessed using Pittsburgh Sleep Quality Index (PSQI) scores. Quality of life, after treatment, was evaluated by World Health Organization Quality of Life-Brief (WHOQOL-BREF) scores. Incidence of adverse reactions in the two groups was observed. Results: There were no significant differences in HAMD scores and PSQI scores between groups A and B before treatment (both P>0.05). Scores in both groups decreased significantly after treatment (both P<0.001). After treatment, HAMD scores and PSQI scores in group A were significantly lower than those in group B (both P<0.001). Reduction rates in group A were significantly higher than those in group B (both P<0.001). There were no significant differences in incidence of adverse reactions between groups A and B (P>0.05). After treatment, scores of mental health, physiological health, surrounding environment, and social relations via the WHOQOL-BREF scale in group A were significantly higher than those in group B (all P<0.001). Conclusion: Fluoxetine combined with psychological intervention demonstrated good clinical effects on depressive patients with insomnia, significantly reducing depression, improving sleep quality, and improving quality of life. Therefore, it is worthy of clinical application and promotion.

Efficacy and safety of Chinese patent medicines combined with antidepressants for treatment of depression in adults: A multiple-treatment meta-analysis

Abstract:Background: Combinations of Chinese patent medicines (CPM) with antidepressants (including selective serotonin reuptake inhibitors (SSRI), selective serotonin-norepinephrine reuptake inhibitors (SNRI), tricyclic antidepressants (TCA), and noradrenergic and specific serotonergic antidepressants (NaSSA)) are frequently utilized for treating depression in adults. However, the efficacy and safety of these combination treatments remain to be established. Methods: Systematic search was conducted in seven electronic databases, regulatory websites and international registers of trials from 1994 to 2023 that included adult patients with depressive disorders who received CPM combined with antidepressants. The Multiple-Treatment Meta-Analysis (MTMA) was conducted using a random effects model with Stata/MP17 and R4.3.5 software. Primary outcomes were total efficacy rate, Hamilton Depression Scale (HAMD) score, and Treatment Emergency Symptom Scale (TESS) score. Secondary outcomes included brain-derived neurotrophic factor (BDNF) levels. Results: A total of 146 randomized controlled trials (13,754 participants: 6929 in intervention and 6825 in control groups) were included. For total effective rate, Multiple-Treatment Meta-Analysis results showed that the overall effect of combined intervention was better compared with antidepressants alone, where Jieyuanshenkeli (JYASKL) presented the optimal option for improving total efficacy (OR = 5.39, 95% CI [2.60, 11.18], SUCRA = 84.50%). In reduding the HAMD, Shuganjieyujiaonang (SGJYJN) was most likely to reduce the HAMD score (SMD = -2.20, 95% CI [-3.06, -1.33], SUCRA = 86.10%), Jieyuanshenkeli (JYASKL),Tianewangbuxindan (TWBXD), Shuyukeli (SYKL), Anshenbuxinwan (ASBXW) combination intervention did not appear to be statistically superior to antidepressants alone. In theTreatment Emergency Symptom Scale (TESS), Wulinjiaonang induced the most significant reduction in TESS score (SMD = -1.98, 95% CI [-3.59, -0.36], SUCRA = 90.40%). Tianmengjiaonang (TMJN) + Antidepressants(AD) (SUCRA = 88.30%) displayed the highest scores in increasing the levels of BDNF, although not statistically significant compared to Antidepressants(AD) alone (SMD = 1.23, 95% CI [0.90, 1.55]). Conclusion: Combinations of CPM and antidepressants showed superior efficacy over antidepressants alone. The optimal combinations were determined as Shuganjieyu Jiaonang (SGJYJN)/SSRIs and Jieyuanshenkeli (JYASKL)/SSRIs. In terms of safety, results showed that combination therapy did not show better TESS efficacy than antidepressants alone.Although some of the combination interventions were not superior than antidepressants alone in reducing HAMD scores,our findings provide a potentially significant alternative option for clinical complementary therapy. However, these results require further validation through larger sample sizes, multicenter randomized controlled trials, and real-world data.

Abstract:Introduction Major depressive disorder (MDD), the second leading cause of disability globally, is considered to be associated with a consequent deterioration in the quality of life and can lead to a major economic burden on medical service and suicide-related costs. Previous research has shown that acupuncture may be beneficial for treating MDD. However, there is a lack of rigorous evidence from previous studies comparing acupuncture with antidepressant medications. This study aims to assess the therapeutic potential of acupuncture in the management of depressive disorders. Methods and analysis A multicentre, randomised, participant-blind, sham-controlled, 2x2 factorial clinical trial, Acupuncture and Escitalopram for Treating Major Depression Clinical Study, aims to compare the efficacy of acupuncture versus escitalopram in treating depression. This study will be conducted at three hospitals in China, enrolling 260 patients with moderate-to-severe major depression, as defined by DSM-5 criteria and Hamilton Depression Rating Scale (HDRS-17) Scores above 17. Participants will be randomly assigned in equal proportions to one of four groups (acupuncture/escitalopram, sham acupuncture/escitalopram, acupuncture/placebo and sham acupuncture/placebo) and undergo 30 sessions across 10 weeks. The primary outcome is change in HDRS-17 Score and secondary outcomes include BDI, Clinical Global Impression, Generalised Anxiety Disorder-7 and Mini-Mental State Examination Scores, alongside potential biological markers. Ethics and dissemination Ethical approval for the study was granted by the Ethics Committees of the Seventh People's Hospital of Shanghai University of Traditional Chinese Medicine (2023-7th-HIRB-020), Shanghai Mental Health Centre (2022–86) and Shanghai Pudong New Area Hospital of Traditional Chinese Medicine (2023–003). Abstract:Major depressive disorder (MDD) is a mental disease characterized by depressed mood, anhedonia, and lack of energy. Modified electroconvulsive therapy (MECT) can effectively alleviate depression. However, it has been reported that MECT may cause cognitive impairments. Therefore, it is imperative to explore a new treatment approach with high efficacy in alleviating depressive symptoms and ensuring safety. In this study, we investigate the clinical efficacy of Chaihu Guizhi Ganjiang Decoction (CGGD) combined with cognitive behavioral intervention in patients with MDD after MECT and elucidate the therapeutic mechanism of CGGD in treating MDD. A total of 90 patients with MDD who underwent MECT at our hospital from 2021.01 to 2023.01 were included and randomly divided into 2 groups: a regular group received cognitive behavioral intervention and a coalition group received cognitive behavioral intervention combined with CGGD. A comparative analysis was performed on the clinical efficacy, cognitive function, and depressive state between the two groups. We observed that the coalition group exhibited a significantly higher total effective rate (93.33%) compared to the regular group (73.3%). After the intervention, the HAMD scores in the coalition group were lower than those in the regular group. Furthermore, our results demonstrated that compared to the regular group, the coalition group showed higher numbers of total tests and correct responses and lower numbers of wrong responses and persistent errors. In conclusion, these findings suggest that the combined treatment can effectively enhance antidepressant effects and improve cognitive function in patients with MDD after MECT.

Abstract:Objective: To observe the curative effect of Shugan Tiaoshen (soothing liver and regulating mind) acupuncture combined with repetitive transcranial magnetic stimulation (rTMS) in the treatment of post-stroke depression (PSD), and to explore its mechanism. Methods: Ninety patients of PSD were randomly divided into an acupuncture+rTMS combination group (30 cases), a rTMS combination group (30 cases, 1 case dropped off) and a western medication group (30 cases, 1 case dropped off). The western medication group was treated with escitalopram oxalate tablets, 10 mg orally each time, once a day; on the basis of the treatment in the western medication group, the rTMS combination group was additionally given rTMS, and the frequency was 20 Hz, 20 min each time, once a day, 5 times a week; on the basis of the treatment in the rTMS combination group, the acupuncture+rTMS combination group was additionally treated with Shugan Tiaoshen acupuncture at Baihui (GV 20), Sishencong (EX-HN 1), Yintang (GV 24+), Shenting (GV 24), etc. for 40 min each time, once a day, weekly 5 times, and each group was treated for 4 weeks. Before and after treatment, the scores of Hamilton depression scale-17 (HAMD-17), Montreal cognitive assessment scale (MoCA), Pittsburgh sleep quality index (PSQI) were observed, and serum levels of 5-hydroxytryptamine (5-HT) and brain-derived neurotrophic factor (BDNF) were detected in each group. Results: After treatment, the HAMD-17 scores in the three groups were lower than those before treatment (P<0.01), and the PSQI scores in the acupuncture+rTMS combination group and the rTMS combination group were lower than those before treatment (P<0.01); the above indexes in the acupuncture+rTMS combination group and the rTMS combination group were lower than those in the western medication group (P<0.05), and the PSQI score in the acupuncture+rTMS combination group was lower than that in the rTMS combination group (P<0.05). After treatment, the MoCA scores and serum BDNF levels in the acupuncture+rTMS combination group and the rTMS combination group were higher than those before treatment (P<0.01), and the serum 5-HT levels in the three groups were higher than those before treatment (P<0.01); and the above indexes in the acupuncture+rTMS combination group and the rTMS combination group were higher than those in the western medication group (P<0.05), and which in the acupuncture+rTMS combination group were higher than those in the rTMS combination group (P<0.05). Conclusion: On the basis of western medication escitalopram oxalate, the addition of Shugan Tiaoshen acupuncture combined with rTMS therapy can effectively improve cognitive function and sleep quality in patients with PSD, and the effect is better than that of western medication alone or rTMS combined with western medication. Its mechanism of action may be related to the increase of peripheral serum 5-HT and BDNF levels.

Abstract:Background: The diagnosis and treatment of depression in patients with chronic heart failure (CHF) is challenging, with no ideal treatment at present. Aim: To analyze the clinical intervention effect of Xuefu Zhuyu decoction (XFZYD) on CHF complicated with depression. Methods: The study cohort comprised 116 patients with CHF complicated with depression who received treatment from July 2020 to July 2023, of which 55 received Western medicine (control group) and 61 received XFZYD (research group). Data on clinical effectiveness, traditional Chinese medicine (TCM) syndrome score, cardiac function, negative emotions, and serum inflammatory factors, were collected for comparative analyses. Results: Compared with the control group, the research group had an evidently higher total effective rate. Furthermore, there were marked reductions in TCM symptom score, left ventricular end-diastolic diameter, left ventricular end-systolic diameter, Self-Rating Depression Scale, Hamilton Depression Scale, high-sensitivity C-reactive protein, monocyte chemoattractant protein-1, and matrix metalloproteinase-9 in the research group after treatment, and these were lower than the corresponding values in the control group. Left ventricular ejection fraction was increased and higher in the research group compared with the control group after treatment. Conclusion: Our findings conclusively proved that XFZYD was considerably superior to Western medicine for treating CHF complicated with depression because it significantly alleviated patients' symptoms, improved cardiac function, relieved negative emotions, and reduced the levels of serum inflammatory factors.

Aim of the study: This study aimed to investigate the antidepressant-like effects of GPT and explore its underlying molecular mechanisms. Materials and methods: Male Sprague-Dawley rats were subjected to a chronic unpredictable mild stress (CUMS) procedure and treated with various doses of GPT, with fluoxetine treatment as a positive control. Behavioural tests (including sucrose preference test, novelty-suppressed feeding test, open-field test and forced swim test), terminal deoxynucleotidyl transferase dUTP nick end labeling and enzyme-linked immunosorbent assay were conducted. The levels of Bax, Bcl-2, cleaved caspase-3, PI3K, p-PI3K, AKT, p-AKT, BDNF, TrkB and CREB or p-CREB were assessed at the protein level using western blotting or immunofluorescence. Results: GPT consists of mainly known drugs, such as liquiritin and ginsenosides. It reversed depressive behaviours and decreased cell apoptosis in the hippocampi of CUMS rats. It significantly upregulated the protein level of Bax, p-Akt, p-PI3K, BDNF, TrkB and p-CREB and downregulated the level of cleaved caspase-3 and Bcl-2. Conclusions: GPT had anti-depressive activity as indicated by the amelioration of depression-like behaviour and the inhibition of hippocampal neuronal apoptosis in CUMS rats. This inhibition was mediated partly by modulating the PI3K/Akt and/or BDNF/TrkB/CREB pathway, in which, glycosides, the main components of GPT, might be involved.
Chinese herbal medicines for the treatment of depression: a systematic review and network meta-analysis

Abstract:Objectives: Amidst rising global burden of depression and the associated challenges with conventional antidepressant therapies, there is a growing interest in exploring the efficacy and safety of alternative treatments. This study uses a Bayesian network meta-analysis to rigorously evaluate the therapeutic potential of Chinese herbal medicines in the treatment of depression, focusing on their comparative efficacy and safety against standard pharmacological interventions. Methods: Five databases (PubMed, Wanfang Data, EMBASE, CNKI, and the Cochrane Library) and grey literature were searched from inception to end of July 2023 to identify studies that assessed the efficacy and safety of Chinese herbal medicines in treating depression. The response rate, Hamilton Depression Scale (HAMD) scores, and rates of adverse events were assessed through both direct and indirect comparisons. Data extraction and risk of bias assessment were meticulously performed. Statistical analysis used Markov chain Monte Carlo methods, with effect size estimates provided as odd ratios and their 95% confidence intervals. Results: A total of 198 RCTs involving 8,923 patients were analyzed, assessing 17 Chinese herbal medicines. Surface Under the Cumulative Ranking results indicated that the top three treatments with the best response rate were possibly Guipiwan , Ease Pill , and Chaihu Jia Longgu Muli Decoction ; the top three treatments on the reduction of HAMD scores were Chai Hu Shu Gan San , Xingnao Jieyu Decoction , and Xiaoyao Powder ; and the top three treatments with the lowest adverse effects rates were Xiaoyao Powder , Alprazolam , and Xingnao Jieyu Decoction . Interestingly, commonly used synthetic drugs such as Fluoxetine , Escitalopram , Amitriptyline , Sertraline , Flupentixol and Melitracen , and Venlafaxine , not only appeared to be less effective than specific Chinese herbal medicines ( Gan Mai Da Zao Decoction , Chaihu Jia Longgu Muli Decoction , Chai Hu Shu Gan San , Danzhi-Xiaoyao-San , and Xingnao Jieyu Decoction ), but they were also related to substantially higher risk of adverse events. Conclusion: Our findings elucidate the promising therapeutic potential of Chinese herbal medicines as viable alternatives in the treatment of depression, with certain herbs demonstrating enhanced efficacy and safety profiles. The outcomes of this study advocate for the integration of these alternative modalities into contemporary depression management paradigms. However, it underscores the necessity for larger, methodologically robust trials to further validate and refine these preliminary findings. Hybrid modulation for asymmetrical cascaded multilevel converters under fault conditions

Abstract:This paper proposes a simple hybrid modulation strategy for asymmetrical cascaded multilevel converters (ACMCs) with fault conditions. The algorithm is described by means of an example, considering a converter with one 1pu cell and one 2pu cell per phase. The modulation integrates the benefits of both the geometrical and space vector approaches. The last one is used to define the voltages to be synthesized by the high-voltage-low-frequency (HVLF) cells, avoiding the overmodulation of the low-voltage-high-frequency LVHF cells. The carrier-based geometrical approach is used for the LVHF cells to simplify the implementation of the algorithm. As a result, this new modulation is considerably simplified when compared to the space vector. Finally, experimental results are shown to validate the proposed modulation strategy.
[Tiaoqi Jieyu acupuncture for treatment-resistant depression: a randomized controlled trial].

Abstract:OBJECTIVE To observe the effect of Tiaoqi Jieyu (regulating qi and relieving depression) acupuncture on the clinical symptoms of treatment-resistant depression (TRD), and to explore the relationship between the acupuncture pain sensitivity and symptom's improvement. METHODS A total of 78 patients with TRD were randomly divided into an observation group (39 cases, 3 cases dropped off) and a control group (39 cases, 4 cases dropped off). The patients in the control group were treated with medications according to the treatment plan of psychiatrists (at least one medication was 5-hydroxytryptamine reuptake inhibitor). On the basis of the control group, the patients in the observation group were treated with Tiaoqi Jieyu acupuncture, and Baihui (GV 20), Yintang (GV 24+), Yanglingquan (GB 34), Taichong (LR 3), Hegu (LI 4), Neiguan (PC 6), Yinlingquan (SP 9) and Zusanli (ST 36), etc. were selected. The acupuncture was given three times a week. Both groups were treated for 8 weeks. After 8-week treatment, the response rate of Hamilton depression scale-24 (HAMD-24) score after was evaluated in the two groups. The scores of HAMD-24 and Hamilton anxiety scale (HAMA) were compared between the two groups before treatment, after 4, 8-week treatment and 12 weeks after treatment (follow-up). After the first treatment and 8-week treatment, the visual analogue scale (VAS) score in the observation group was evaluated, and the correlation between VAS score after the first treatment and HAMD-24 score before treatment, between VAS score after the first treatment and the course of disease in the observation group was analyzed, and the correlation between difference of VAS after 8-week treatment and after the first treatment and difference of HAMD-24 score before treatment and after 8-week treatment was analyzed. RESULTS After 8-week treatment, the response rate of HAMD-24 score in the observation group was 52.8% (19/36), higher than 17.1% (6/35) in the control group (P<0.001). Compared before treatment, the scores of HAMD-24 and HAMA in the two groups were decreased after 4-week treatment, 8-week treatment and in follow-up (P<0.05), and those in the observation group were superior to the control group (P<0.05). After 8-week treatment, the acupuncture pain VAS score in the observation group was (5.28±2.13) points, which was higher than (3.33±1.62) points after the first treatment (P<0.001). There was a negative correlation between VAS score after the first treatment and HAMD-24 score before treatment in the observation group (r =-0.486, P=0.003); there was no correlation between acupuncture pain VAS score after the first treatment and the course of disease in the observation group (P>0.05). After 8-week treatment, there was a positive correlation between the difference of VAS score and the difference of HAMD-24 score in the observation group (r =0.514, P=0.001). CONCLUSION Tiaoqi Jieyu acupuncture could improve the depression and anxiety in patients with TRD, and the symptom's improvement is related to the recovery of acupuncture pain sensitivity.

Abstract:Objective: To observe the effect of Shugan Tiaoshen acupuncture (acupuncture for soothing the liver and regulating the mentality) combined with western medication on depression and sleep quality in the patients with depression-insomnia comorbidity due to COVID-19 quarantine, and investigate the potential mechanism from the perspective of cortical excitability. Methods: Sixty patients with depression-insomnia comorbidity due to COVID-19 quarantine were randomly divided into an acupuncture group and a sham-acupuncture group, 30 cases in each one. The patients of both groups were treated with oral administration of sertraline hydrochloride tablets. In the acupuncture group, Shugan Tiaoshen acupuncture was supplemented. Body acupuncture was applied to Yintang (GV 24+), Baihui (GV 20), Hegu (LI 4), Zhaohai (KI 6), Qihai (CV 6), etc. The intradermal needling was used at Xin (CO15), Gan (CO12) and Shen (CO10). In the sham-acupuncture group, the sham-acupuncture was given at the same points as the acupuncture group. The compensatory treatment was provided at the end of follow-up for the patients in the sham-acupuncture group. In both groups, the treatment was given once every two days, 3 times a week, for consecutive 8 weeks. The self-rating depression scale (SDS) and insomnia severity index (ISI) scores were compared between the two groups before and after treatment and 1 month after the end of treatment (follow-up) separately. The cortical excitability indexes (resting motor threshold [rMT], motor evoked potential amplitude [MEP-A], cortical resting period [CSP]) and the level of serum 5-hydroxytryptamine (5-HT) were measured before and after treatment in the two groups. Results: After treatment and in follow-up, SDS and ISI scores were decreased in both groups compared with those before treatment (P<0.05), and the scores in the acupuncture group were lower than those in the sham-acupuncture group (P<0.05), and the decrease range in the acupuncture group after treatment was larger than that in the sham-acupuncture group (P<0.05). After treatment, rMT was reduced (P<0.05), while MEP-A and CSP were increased (P<0.05) in the acupuncture group compared with that before treatment. The levels of serum 5-HT in both groups were increased compared with those before treatment (P<0.05). The rMT in the acupuncture group was lower than that in the sham-acupuncture group, while MEP-A and CSP, as well as the level of serum 5-HT were higher in the acupuncture group in comparison with the sham-acupuncture group (P<0.05). Conclusion: Shugan Tiaoshen acupuncture combined with western medication can relieve depression and improve sleep quality in the patients with depression-insomnia comorbidity due to COVID-19 quarantine, which is probably related to rectifying the imbalanced excitatory and inhibitory neuronal functions.

Abstract:Objective To observe the clinical efficacy and safety of bajitian oligosaccharide capsules in the treatment of depression,and its effects on serum biochemical indexes.Methods A total of 86 patients with depression were randomly divided into control group and treatment group with 43 cases per group.Control group was given the escitalopram oxalate tablets 10 mg qd,after 1 week the dosage increased to 20 mg qd.Treatment group was given oral bajitian oligosaccharide capsule every 0.3 g bid.Two groups were treated for 6 weeks.The clinical efficacy,Hamilton depression scale (HAMD-17) score,Hamilton anxiety scale (HAMA) score,and traditional Chinese medical (TCM) kidney scale score,serum biochemical indexes,and adverse drug reactions were compared between two groups.Results After treatment,the total effective rates of treatment and control groups were 95.35％ (41/43 cases) and 81.40％ (35/43 cases) with significant difference (P ＜ 0.05).After treatment,the main indexes in treatment and control groups were compared:HAMD-17 scores were (9.45 ± 1.12),(14.25 ± 1.25) points;HAMA scores were (7.68 ± 1.04),(12.42 ± 1.45) points;TCM kidney deficiency scores were (15.32 ± 2.12),(18.32 ± 2.16) points;serum nerve growth factor were (5.48 ± 0.65),(6.37 ± 1.28) ng · L-1;cortisol were (387.62 ± 152.41),(472.38 ± 161.29) nmol · L-1;5-hydroxytryptamine were (3.68 ± 1.50),(3.02 ± 1.37) μmol · L-1;brain derived neurotropic factor were (47.99 ± 13.62),(42.03 ± 12.15) ng · L-1,the differences were statistically significant (P ＜ 0.05).The adverse drug reactions in two groups were based on nausea,dry mouth,insomnia,loss of appetite,fatigue.And the incidences of adverse drug reactions were 16.28％ and 23.26％ without significant difference (P ＞ 0.05).Conclusion Bajitian oligosaccharide capsule helps to improve the clinical symptoms of depression patients and the therapeutic effect,which may be related to the decrease of nerve growth factor and serum corfisol levels and the increase of 5-HT levels of serotonin and brain-derived neurotropic factor expression.

Abstract:Results of randomized, placebo-controlled trials with the selective serotonin reuptake inhibitor citalopram suggest that this agent is safe and effective for the treatment of depressive disorders. We investigated the safety and efficacy of citalopram in the treatment of Chinese patients with depressive disorders. An 8-week, open-label, multicenter study evaluated the safety and efficacy of citalopram in the treatment of patients with an ICD-10 diagnosis of depressive disorder or depressive episode of bipolar disorder. Efficacy measures included the Hamilton Rating Scale for Depression (HAMD) and the Clinical Global Impression (CGI). A total of 6080 patients (2553 men, 3527 women) (mean age 40.9 +/- 15.6 years, range 18 - 92) participated in the study. Mean HAMD scores decreased significantly (p < 0.001) after 2 weeks of treatment and at all subsequent study visits. Endpoint analyses showed that 89.9% of the patients demonstrated a clinical response, defined as a >= 50% reduction in HAMD scores. The mean daily dose of citalopram at endpoint was 23.65 mg. Nausea (7.6%), headache (3.7%) and dry mouth (2.9%) were the most frequently reported adverse events. Patients with bipolar depression and comorbid obsessive compulsive disorder (OCD) received a higher mean daily dose (26.4 and 27.7 mg, respectively), while patients with comorbid physical disorders received a lower dose (21.9 mg) than patients with simple depressive disorder. The results of this 8-week open-label study suggest that citalopram is safe and effective in the treatment of depression in Chinese patients. Limitations is an open-label and uncontrolled.
Comparison of Escitalopram with Citalopram in Treatment of Depression: a Randomized, Double-Blind, Multicenter Study

Abstract:AIM To evaluate the efficacy and safety of escitalopram in treating depression.METHODS In this randomized,double-blind,double-mimicry,citalopram parallel controlled multicenter study,the patients were randomized in two groups who were treated with escitalopram oxalate tablet 10-20 mg·d~(-1) or citalopram hydrobromide tablet 20 - 40 mg ? d~(-1) respectively.The primary efficacy variable was Hamilton Depression Scale 17-item(HAMD-17) while Clinical Global Impression(CGI) and Hamilton Anxiety Scale (HAMA) were used as secondary variables.Adverse events observation,lab examination and vital signs check were taken to assess the safety.RESULTS In FAS(235 patients),the amount of patients belonged to escitalopram group or citalopram group was 119 vs.116;in PPS(218 patients),110 vs.108.At the end of wk 1,2,4 and 6 in treatment,there were statistically significant reductions in HAMD-17 or HAMA total scores in both groups compared with their baseline(P0.01),however,there was no significant difference between the two groups(P0.05).After 6 weeks treatment,the reduction in HAMD-17 total score of escitalopram group was(13.28±4.30),while the reduction of citalopram group was(13.67±5.29) by FAS analysis. For escitalopram group and citalopram group,the response rate was 79.8%and 81.9%,the remission rate was 66.4%and 62.9%,respectively,with no significant difference between the two groups(both P0.05). Similar findings were found in CGI and HAMA(P0.05).Safety assessment showed there was no statistically significant difference between the symptoms and incidence of adverse reactions(P0.05). CONCLUSION Escitalopram is effective,safe and comparable to citalopram in treatment of patients with depression.
