Abstract:The rising number of the elderly incurs growing concern about healthcare, and in particular rehabilitation healthcare. Assistive technology and assistive robotics in particular may help to improve this process. We develop a robot coach capable of demonstrating rehabilitation exercises to patients, watch a patient carry out the exercises and give him feedback so as to improve his performance and encourage him. The HRI of the system is based on our study with a team of rehabilitation therapists and with the target population.The system relies on human motion analysis. We develop a method for learning a probabilistic representation of ideal movements from expert demonstrations. A Gaussian Mixture Model is employed from position and orientation features captured using a Microsoft Kinect v2. For assessing patients' movements, we propose a real-time multi-level analysis to both temporally and spatially identify and explain body part errors. This analysis combined with a classification algorithm allows the robot to provide coaching advice to make the patient improve his movements. The evaluation on three rehabilitation exercises shows the potential of the proposed approach for learning and assessing kinaesthetic movements.
Synthesize Personalized Training for Robot-Assisted Upper Limb Rehabilitation With Diversity Enhancement

Abstract:For upper limb rehabilitation, the robot-assisted technique in combination with serious games requires well-specified training plans. For the best quality of the rehabilitation process, customized game levels for each user are desired, while it is labor-intensive to design and adjust game levels for different individuals. We work on generating training content for a desktop end-effector rehabilitation robot and propose a method to automatically generate individualized training plans. By modeling the search of the training motions as finding optimal hand paths and trajectories, we introduce solving the design problem with a multi-objective optimization (MO) solver. We further improve the MO solver to enhance the diversity of the solutions. With the proposed approach, our system is capable of automatically generating various training plans considering the training intensity and dexterity of each joint in the upper limb. In addition, the enhanced diversity avoids repeated training plans, which helps motivate the user in the rehabilitation. We test our method with different requirements on the training plans and validate the solutions.

Abstract:Musculoskeletal disorders challenge significantly the performance of many daily life activities, thus impacting the quality of life. The efficiency of the traditional physical therapy programs is limited by ecological parameters such as intervention duration and frequency, number of caregivers, geographic accessibility, as well as by subjective factors such as patient's motivation and perseverance in training. The implementation of VR rehabilitation systems may address these limitations, but the technology still needs to be improved and clinically validated. Furthermore, current applications generally lack flexibility and personalization. A VR rehabilitation game simulation is developed, which focuses on the upper-limb movement of reaching, an essential movement involved in numerous daily life activities. Its novelty consists in the integration of a machine learning algorithm, enabling highly adaptive and patient-customized therapeutic intervention. An immersive VR system for the rehabilitation of reaching movement using a bubble popping game is proposed. In the virtual space, the patient is presented with bubbles appearing at different locations and is asked to reach the bubble with the injured limb and pop it. The implementation of a Q-learning algorithm enables the game to adjust the location of the next bubble according to the performance of the patient, represented by his kinematic characteristics. Two test cases simulate the performance of the patient during a training program of 10 days/sessions, in order to validate the effectiveness of the algorithm, demonstrated by the spatial and temporal distribution of the bubbles in each evolving scenario. The results show that the algorithm learns the patient's capabilities and successfully adapts to them, following the reward policy dictated by the therapist; moreover, the algorithm is highly responsive to kinematic features' variation, while demanding a reasonable number of iterations. A novel approach for upper limb rehabilitation is presented, making use of immersive VR and reinforcement learning. The simulation suggests that the algorithm offers adaptive capabilities and high flexibility, needed in the comprehensive personalization of a rehabilitation process. Future work will demonstrate the concept in clinical trials.
Socially Interactive Agents for Robotic Neurorehabilitation Training: Conceptualization and Proof-of-concept Study

Abstract:Individuals with diverse motor abilities often benefit from intensive and specialized rehabilitation therapies aimed at enhancing their functional recovery. Nevertheless, the challenge lies in the restricted availability of neurorehabilitation professionals, hindering the effective delivery of the necessary level of care. Robotic devices hold great potential in reducing the dependence on medical personnel during therapy but, at the same time, they generally lack the crucial human interaction and motivation that traditional in-person sessions provide. To bridge this gap, we introduce an AI-based system aimed at delivering personalized, out-of-hospital assistance during neurorehabilitation training. This system includes a rehabilitation training device, affective signal classification models, training exercises, and a socially interactive agent as the user interface. With the assistance of a professional, the envisioned system is designed to be tailored to accommodate the unique rehabilitation requirements of an individual patient. Conceptually, after a preliminary setup and instruction phase, the patient is equipped to continue their rehabilitation regimen autonomously in the comfort of their home, facilitated by a socially interactive agent functioning as a virtual coaching assistant. Our approach involves the integration of an interactive socially-aware virtual agent into a neurorehabilitation robotic framework, with the primary objective of recreating the social aspects inherent to in-person rehabilitation sessions. We also conducted a feasibility study to test the framework with healthy patients. The results of our preliminary investigation indicate that participants demonstrated a propensity to adapt to the system. Notably, the presence of the interactive agent during the proposed exercises did not act as a source of distraction; instead, it positively impacted users' engagement.

Abstract:A typical application of upper-limb exoskeleton robots is deployment in rehabilitation training, helping patients to regain manipulative abilities. However, as the patient is not always capable of following the robot, safety issues may arise during the training. Due to the bias in different patients, an individualized scheme is also important to ensure that the robot suits the specific conditions (e.g., movement habits) of a patient, hence guaranteeing effectiveness. To fulfill this requirement, this paper proposes a new motion planning scheme for upper-limb exoskeleton robots, which drives the robot to provide customized, safe, and individualized assistance using both human demonstration and interactive learning. Specifically, the robot first learns from a group of healthy subjects to generate a reference motion trajectory via probabilistic movement primitives (ProMP). It then learns from the patient during the training process to further shape the trajectory inside a moving safe region. The interactive data is fed back into the ProMP iteratively to enhance the individualized features for as long as the training process continues. The robot tracks the individualized trajectory under a variable impedance model to realize the assistance. Finally, the experimental results are presented in this paper to validate the proposed control scheme.
THERAPIST: Towards an Autonomous Socially Interactive Robot for Motor and Neurorehabilitation Therapies for Children

Abstract:BACKGROUND: Neurorehabilitation therapies exploiting the use-dependent plasticity of our neuromuscular system are devised to help patients who suffer from injuries or diseases of this system. These therapies take advantage of the fact that the motor activity alters the properties of our neurons and muscles, including the pattern of their connectivity, and thus their functionality. Hence, a sensor-motor treatment where patients makes certain movements will help them (re)learn how to move the affected body parts. But these traditional rehabilitation processes are usually repetitive and lengthy, reducing motivation and adherence to the treatment, and thus limiting the benefits for the patients.OBJECTIVE: Our goal was to create innovative neurorehabilitation therapies based on THERAPIST, a socially assistive robot. THERAPIST is an autonomous robot that is able to find and execute plans and adapt them to new situations in real-time. The software architecture of THERAPIST monitors and determines the course of action, learns from previous experiences, and interacts with people using verbal and non-verbal channels. THERAPIST can increase the adherence of the patient to the sessions using serious games. Data are recorded and can be used to tailor patient sessions.METHODS: We hypothesized that pediatric patients would engage better in a therapeutic non-physical interaction with a robot, facilitating the design of new therapies to improve patient motivation. We propose RoboCog, a novel cognitive architecture. This architecture will enhance the effectiveness and time-of-response of complex multi-degree-of-freedom robots designed to collaborate with humans, combining two core elements: a deep and hybrid representation of the current state, own, and observed; and a set of task-dependent planners, working at different levels of abstraction but connected to this central representation through a common interface. Using RoboCog, THERAPIST engages the human partner in an active interactive process. But RoboCog also endows the robot with abilities for high-level planning, monitoring, and learning. Thus, THERAPIST engages the patient through different games or activities, and adapts the session to each individual.RESULTS: RoboCog successfully integrates a deliberative planner with a set of modules working at situational or sensorimotor levels. This architecture also allows THERAPIST to deliver responses at a human rate. The synchronization of the multiple interaction modalities results from a unique scene representation or model. THERAPIST is now a socially interactive robot that, instead of reproducing the phrases or gestures that the developers decide, maintains a dialogue and autonomously generate gestures or expressions. THERAPIST is able to play simple games with human partners, which requires humans to perform certain movements, and also to capture the human motion, for later analysis by clinic specialists.CONCLUSIONS: The initial hypothesis was validated by our experimental studies showing that interaction with the robot results in highly attentive and collaborative attitudes in pediatric patients. We also verified that RoboCog allows the robot to interact with patients at human rates. However, there remain many issues to overcome. The development of novel hands-off rehabilitation therapies will require the intersection of multiple challenging directions of research that we are currently exploring.

Abstract:Rehabilitation robots have been demonstrated to be an efficient tool in the field of rehabilitation training. Meanwhile, there are varieties of tasks designed for motion training. These tasks need to be transmitted to motion data for rehabilitation robots. In this paper, we designed a drinking task and captured the motion data as the ground truth, through sensors of an exoskeleton device named Neo-Arm. To verify the effectiveness of Neo-Arm, we used a Vicon system to capture the same motion task without Neo-Arm for comparison. Eight subjects participated in the experiment. The motion data of the drinking task, including the range of motion (ROM) and the velocity of each joint, are obtained. The result shows that the Neo-Arm can achieve the suitable precision and be fit for other kinds of upper limb motion tasks.

Abstract:We developed an exoskeleton for neurorehabilitation that covered all relevant degrees of freedom of the human arm while providing enough range of motion, speed, strength, and haptic-rendering function for therapy of severely affected (e.g., mobilization) and mildly affected patients (e.g., strength and speed). The ANYexo 2.0, uniting these capabilities, could be the vanguard for highly versatile therapeutic robotics applicable to a broad target group and an extensive range of exercises. Thereby, the practical adoption of these devices in clinics will be fostered. The unique kinematic structure of the robot and the bio-inspired controlled shoulder coupling allowed training for most activities of daily living. We demonstrated this capability with 15 sample activities, including interaction with real objects and the own body with the robot in transparent mode. The robot's joints can reach $200 \%$, $398 \%$, and $354 \%$ of the speed required during activities of daily living at the shoulder, elbow, and wrist, respectively. Further, the robot can provide isometric strength training. We present a detailed analysis of the kinematic properties and propose algorithms for intuitive control implementation.

Abstract:In today's world, many patients with cognitive impairments and motor dysfunction seek the attention of experts to perform specific conventional therapies to improve their situation. However, due to a lack of neurorehabilitation professionals, patients suffer from severe effects that worsen their condition. In this paper, we present a technological approach for a novel robotic neurorehabilitation training system. It relies on a combination of a rehabilitation device, signal classification methods, supervised machine learning models for training adaptation, training exercises, and socially interactive agents as a user interface. Together with a professional, the system can be trained towards the patient's specific needs. Furthermore, after a training phase, patients are enabled to train independently at home without the assistance of a physical therapist with a socially interactive agent in the role of a coaching assistant.

Abstract:To enable safe and efficient human-robot collaboration in shared workspaces it is important for the robot to predict how a human will move when performing a task. While predicting human motion for tasks not known a priori is very challenging, we argue that single-arm reaching motions for known tasks in collaborative settings (which are especially relevant for manufacturing) are indeed predictable. Two hypotheses underlie our approach for predicting such motions: First, that the trajectory the human performs is optimal with respect to an unknown cost function, and second, that human adaptation to their partner's motion can be captured well through iterative re-planning with the above cost function. The key to our approach is thus to learn a cost function which "explains" the motion of the human. To do this, we gather example trajectories from pairs of participants performing a collaborative assembly task using motion capture. We then use Inverse Optimal Control to learn a cost function from these trajectories. Finally, we predict reaching motions from the human's current configuration to a task-space goal region by iteratively re-planning a trajectory using the learned cost function. Our planning algorithm is based on the trajectory optimizer STOMP, it plans for a 23 DoF human kinematic model and accounts for the presence of a moving collaborator and obstacles in the environment. Our results suggest that in most cases, our method outperforms baseline methods when predicting motions. We also show that our method outperforms baselines for predicting human motion when a human and a robot share the workspace.

Abstract:The use of social assistive robots for interactive stimulation has strong potential in neurorehabilitation therapies. It is of particular interest in the case of pediatric patients to promote children's motivation and adherence, specially when those robots are able of guide gamified activities, as it is the case of NAOTherapist. NAOTherapist is a Social Assistive Robotics (SAR) platform for hands-off rehabilitation based on upper-limb activities, that was originally designed for pediatric patients with Cerebral Palsy (CP) or Obstetric Braxial Plexus Palsy (OBPP). Formerly, it endowed the therapists with tools to perform rehabilitation exercises. This paper proposes the gamification of NAOTherapist in order to incorporate additional characteristics which allow its intensive use in new rehabilitation procedures, such as the Hand-Arm Bimanual Intensive Therapy (HABIT). This intensive therapy setting involves daily activities in several consecutive days, which require a strong engagement of the patients with the therapeutic methods and the acceptation of the NAOTherapist as a rehabilitation system. The gamified system shows very accurate results considering the different aspects defined in the USUS methodology; namely Usability, Social acceptance, User experience and Societal impact.

Abstract:A retrospective examination was made to determine parasitemia and episodes of fever in 97 patients, previously infected with Plasmodium malariae, P. ovale, and/or P. vivax, who were reinfected with P. falciparum for treatment of neurosyphilis, the standard treatment at the time. Data were collected at the National Institutes of Health laboratories in Columbia, South Carolina and Milledgeville, Georgia during the period 1940 to 1963. Results were compared with observations recorded for patients following primary infection with P. falciparum. The mean daily percentage of patients with fever > or = 101 degrees F during the first 20 days of primary infection with P. falciparum was 42.4; the percentage with fever > or = 104 degrees F was 19.9%. Those previously infected with P. ovale, P. vivax, and P. malariae had mean daily percentages of fever > or = 101 degrees F and > or = 104 degrees F of 39.1% and 14.8%, 39.1% and 19.4%, and 28.4%, and 11.3%, respectively. Previous infection with P. ovale or P. vivax had little, if any, effect on subsequent clinical malaria due to P. falciparum, whereas infection with P. malariae resulted in reduced frequencies of fever. A similar comparison was made for parasite counts > or = 1,000/microl and > 10,000/microl. The percentages for 268 patients during the first 20 days of primary infection with P. falciparum parasite counts > or = 1,000/microl and > or = 10,000/microl were 58.2% and 29.9%, respectively. Those previously infected with P. ovale, P. vivax, and P. malariae had mean daily percentages of parasitemia > or = 1,000/microl and > or = 10,000/microl of 58.0% and 24.3%, 57.3% and 31.1%, and 45.9% and 19.0%, respectively. Previous infection with P. malariae resulted in a reduction in the frequency of high-density parasitemia (> or = 10,000/microl) as well as an asexual parasite count > or = 1,000/microl. These results suggest that P. falciparum and P. malariae share common antigens that are able to induce parasitologic and clinical protection when infection with P. falciparum follows that with P. malariae. The results did not suggest that protection to P. falciparum is provided by previous infection with P. ovale or P. vivax.

Abstract:Assistive technology allows for intensive practice and kinematic measurements during rehabilitation exercises. More recent approaches attach a gravity-compensating multi-joint exoskeleton to the upper extremity to facilitate task-oriented training in three-dimensional space with virtual reality feedback. The movement quality, however, is mostly captured through end-point measures that lack information on proximal inter-joint coordination. This limits the differentiation between compensation strategies and genuine restoration both during the exercise and in the course of rehabilitation. We extended in this proof-of-concept study a commercially available seven degree-of-freedom arm exoskeleton by using the real-time sensor data to display a three-dimensional multi-joint visualization of the user's arm. Ten healthy subjects and three severely affected chronic stroke patients performed reach-to-grasp exercises resembling activities of daily living assisted by the attached exoskeleton and received closed-loop online feedback of the three-dimensional movement in virtual reality. Patients in this pilot study differed significantly with regard to motor performance (accuracy, temporal efficiency, range of motion) and movement quality (proximal inter-joint coordination) from the healthy control group. In the course of 20 training and feedback sessions over 4 weeks, these pathological measures improved significantly toward the reference parameters of healthy participants. It was moreover feasible to capture the evolution of movement pattern kinematics of the shoulder and elbow and to quantify the individual degree of natural movement restoration for each patient. The virtual reality visualization and closed-loop feedback of joint-specific movement kinematics makes it possible to detect compensation strategies and may provide a tool to achieve the rehabilitation goals in accordance with the individual capacity for genuine functional restoration; a proposal that warrants further investigation in controlled studies with a larger cohort of stroke patients.

Individualized Three-Dimensional Gait Pattern Generator for Lower Limbs Rehabilitation Robots

Abstract:In the field of robotic gait rehabilitation, controlling robotic devices to follow specific human-like trajectories is often required. In recent years, various gait generator models have been proposed, providing customized gait patterns adjustable to a range of heights and gait speeds. However, these models were developed with a focus on gait rehabilitation devices designed to control the angular trajectories of the subject's joints, e.g. exoskeletons. Similar devices, e.g. end-effector robots, control the orientation and also the 3D position of the subject's joints and cannot easily implement these models. In this study, it is proposed a new individualized three-dimensional gait pattern generator for gait rehabilitation robots. The generator employs multi-variable regression models to predict the joint angular trajectories of the pelvis, hip, and ankle along the gait cycle. The 3D joints positions are then reconstructed by applying the predicted angular trajectories over a human model inspired on the inverted pendulum analogy using inverse kinematics. The generator's performance was statistically evaluated against real gait patterns from 42 participants walking at 8 different velocities. The predicted trajectories matched the measured ones with an average Root Mean Squared Error of 25.73 mm for all joints at all Cartesian axes, with better results between 3.3 - 5.4 km/h. Suggesting to be a good solution to be applied in end-effector gait robotic rehabilitation devices.

Abstract:Rehabilitation training for patients with motor disabilities usually requires specialized devices in rehabilitation centers. Home-based multi-purpose training would significantly increase treatment accessibility and reduce medical costs. While it is unlikely to equip a set of rehabilitation robots at home, we investigate the feasibility to use the general-purpose collaborative robot for rehabilitation therapies. In this work, we developed a new system for multi-purpose upper-limb rehabilitation training using a generic robot arm with human motor feedback and preference. We integrated surface electromyography, force/torque sensors, RGB-D cameras, and robot controllers with the Robot Operating System to enable sensing, communication, and control of the system. Imitation learning methods were adopted to imitate expert-provided training trajectories which could adapt to subject capabilities to facilitate in-home training. Our rehabilitation system is able to perform gross motor function and fine motor skill training with a gripper-based end-effector. We simulated system control in Gazebo and training effects (muscle activation level) in OpenSim and evaluated its real performance with human subjects. For all the subjects enrolled, our system achieved better training outcomes compared to specialist-assisted rehabilitation under the same conditions. Our work demonstrates the potential of utilizing collaborative robots for in-home motor rehabilitation training.

Abstract:Several upper-limb exoskeleton robots have been developed for stroke rehabilitation, but their rather low level of individualized assistance typically limits their effectiveness and practicability. Individualized assistance involves an upper-limb exoskeleton robot continuously assessing feedback from a stroke patient and then meticulously adjusting interaction forces to suit specific conditions and online changes. This paper describes the development of a new upper-limb exoskeleton robot with a novel online generative capability that allows it to provide individualized assistance to support the rehabilitation training of stroke patients. Specifically, the upper-limb exoskeleton robot exploits generative models to customize the fine and fit trajectory for the patient, as medical conditions, responses, and comfort feedback during training generally differ between patients. This generative capability is integrated into the two working modes of the upper-limb exoskeleton robot: an active mirroring mode for patients who retain motor abilities on one side of the body and a passive following mode for patients who lack motor ability on both sides of the body. The performance of the upper-limb exoskeleton robot was illustrated in experiments involving healthy subjects and stroke patients.
Closed-Loop Task Difficulty Adaptation during Virtual Reality Reach-to-Grasp Training Assisted with an Exoskeleton for Stroke Rehabilitation

Abstract:Stroke patients with severe motor deficits of the upper extremity may practice rehabilitation exercises with the assistance of a multi-joint exoskeleton. Although this technology enables intensive task-oriented training, it may also lead to slacking when the assistance is too supportive. Preserving the engagement of the patients while providing "assistance-as-needed" during the exercises, therefore remains an ongoing challenge. We applied a commercially available seven degree-of-freedom arm exoskeleton to provide passive gravity compensation during task-oriented training in a virtual environment. During this 4-week pilot study, five severely affected chronic stroke patients performed reach-to-grasp exercises resembling activities of daily living. The subjects received virtual reality feedback from their three-dimensional movements. The level of difficulty for the exercise was adjusted by a performance-dependent real-time adaptation algorithm. The goal of this algorithm was the automated improvement of the range of motion. In the course of 20 training and feedback sessions, this unsupervised adaptive training concept led to a progressive increase of the virtual training space (p < 0.001) in accordance with the subjects' abilities. This learning curve was paralleled by a concurrent improvement of real world kinematic parameters, i.e., range of motion (p = 0.008), accuracy of movement (p = 0.01), and movement velocity (p < 0.001). Notably, these kinematic gains were paralleled by motor improvements such as increased elbow movement (p = 0.001), grip force (p < 0.001), and upper extremity Fugl-Meyer-Assessment score from 14.3 ± 5 to 16.9 ± 6.1 (p = 0.026). Combining gravity-compensating assistance with adaptive closed-loop feedback in virtual reality provides customized rehabilitation environments for severely affected stroke patients. This approach may facilitate motor learning by progressively challenging the subject in accordance with the individual capacity for functional restoration. It might be necessary to apply concurrent restorative interventions to translate these improvements into relevant functional gains of severely motor impaired patients in activities of daily living.

Abstract:The motor synergy pattern is an intrinsic characteristic found in natural human movements, particularly in the upper limb. It is essential to improve the multijoint coordination ability for stroke patients by integrating the synergy pattern into rehabilitation tasks and trajectory design. However, current robot-assisted rehabilitation systems tend to overlook the incorporation of a multijoint synergy model. This article proposes postural synergistic kernelized movement primitives (PSKMP) method for the human-like trajectory planning of robot-assisted upper limb rehabilitation. First, the demonstrated trajectory obtained from the motion capture system is subject to principal component analysis to extract postural synergies. Then, the PSKMP is proposed by kernelizing the postural synergistic subspaces with the kernel treatment to preserve human natural movement characteristics. Finally, the rehabilitation trajectory accord with human motion habits can be generated based on generalized postural synergistic subspaces. This approach has undergone practical validation on an upper limb rehabilitation robot, and the experimental results show that the proposed method enables the generation of human-like trajectories adapted to new task points, in accordance with the natural movement style of human. This method holds great significance in promoting the recovery of coordination ability of stroke patients.
