Hydrogen Sulfide, a Potential Cardioprotective Gas Activating a Life Span Regulator.

Abstract:R eperfusion therapy is beneficial to prevent cardiomyocyte death and contractile dysfunction after myocardial infarction. However, numerous studies have shown that reperfusion itself may enhance the injury, resulting in the increase of infarct size after ischemia [ie, ischemiareperfusion (I/R) injury]. Although both ischemic preconditioning and pharmacological preconditioning mimetics strongly protect the heart against I/R injury, the requirement for pretreatment has greatly limited their clinical relevance. Meanwhile, it has been recently reported that brief intermittent ischemia applied after the onset of reperfusion, termed “postconditioning,” reduces myocardial injury to an extent comparable to the reduction by preconditioning. Various cardioprotective mechanisms of postconditioning have been proposed, and pharmacological postconditioning by the administration of agents that modulate these mechanisms seems to be beneficial in I/R injury. Hydrogen sulfide (H2S) is a colorless, flammable gas with a characteristic odor, which, until recently, had been known for its toxicity alone. However, H2S is now known to be a novel gaseous signaling molecule, similar to nitric oxide (NO) and carbon monoxide, and is endogenously produced in mammalian tissues. Notably, a growing body of evidence has shown that H2S is a critical regulator of cardiac functions and plays a protective role in the pathogenesis and development of heart diseases such as myocardial infarction, arrhythmia, hypertrophy, fibrosis, and I/R injury. Some mechanisms have been proposed by which H2S exerts cardioprotective effects, including reduction of apoptosis, preservation of mitochondrial function, anti-inflammatory responses, and angiogenic actions. However, the specific mechanism of cardioprotection mediated by H2S in I/R injury is still largely unknown.
