Abstract:Background: Dual-energy computed tomography (DECT) scan is a non-invasive method for the in vivo identification of renal stone composition. However, DECT scanners have several demerits, including high cost, low accessibility, and high radiation dose to patients. Objectives: The present study aimed to investigate the efficacy of deep neural networks in the classification of renal stone types using single-energy CT imaging. The Taguchi method was used for the optimization of hyperparameters. Patients and Methods: A total of 146 pure renal stone samples were first surgically collected from the patients. The stones were then inserted into a Rando phantom and scanned using a DECT scanner. An ultra-low-dose CT scan was acquired to determine the stone position prior to the DECT scan. The result of chemical analysis was used as the gold standard for determining the stone composition throughout the study. Several neural networks, including ResNet-50, ResNet-18, GoogLeNet, and VGG-19, were used to classify the stone images into three composition groups, including uric acid, calcium oxalate, and cystine. Moreover, the Taguchi method was employed to optimize the network hyperparameters. The signal-to-noise ratio (SNR) was also analyzed to determine the optimal arrangement. Results: In this study, CT scans of 53 uric acid, 55 calcium oxalate, and 38 cystine stones, with diameters of 1 - 3 mm, were acquired. The deep learning findings showed that the ResNet-18 network had the highest accuracy for 120-kVp and 135-kVp CT scanning, while ResNet-50 performed better in 80-kVp CT scanning. The ResNet-50 network showed the best performance of all networks in predicting stone types in 80-kVp scanning, as indicated by its high sensitivity, specificity, and precision. Conclusion: The present results indicated that our deep learning approach could be used for the in vivo determination of renal stone types. Moreover, low-dose or ultra-low-dose single-energy CT scanning is more widely accessible and safer in terms of radiation exposure.

Abstract:Purpose: To compare image quality and detection accuracy of renal stones between deep learning image reconstruction (DLIR) and Adaptive Statistical Iterative Reconstruction-Veo (ASIR-V) reconstructed virtual non-contrast (VNC) images and true non-contrast (TNC) images in spectral CT Urography (CTU). Methods: A retrospective analysis was conducted on images of 70 patients who underwent abdominal-pelvic CTU in TNC phase using non-contrast scan and contrast-enhanced corticomedullary phase (CP) and excretory phase (EP) using spectral scan. The TNC scan was reconstructed using ASIR-V70 % (TNC-AR70), contrast-enhanced scans were reconstructed using AR70, DLIR medium-level (DM), and high-level (DH) to obtain CP-VNC-AR70/DM/DH and EP-VNC-AR70/DM/DH image groups, respectively. CT value, image quality and kidney stones quantification accuracy were measured and compared among groups. The subjective evaluation was independently assessed by two senior radiologists using the 5-point Likert scale for image quality and lesion visibility. Results: DH images were superior to AR70 and DM images in objective image quality evaluation. There was no statistical difference in the liver and spleen (both P > 0.05), or within 6HU in renal and fat in CT value between VNC and TNC images. EP-VNC-DH had the lowest image noise, highest SNR, and CNR, and VNC-AR70 images had better noise and SNR performance than TNC-AR70 images (all p < 0.05). EP-VNC-DH had the highest subjective image quality, and CP-VNC-DH performed the best in lesion visibility. In stone CT value and volume measurements, there was no statistical difference between VNC and TNC (P > 0.05). Conclusion: The DLIR-reconstructed VNC images in CTU provide better image quality than the ASIR-V reconstructed TNC images and similar quantification accuracy for kidney stones for potential dose savings.The study highlights that deep learning image reconstruction (DLIR)-reconstructed virtual non-contrast (VNC) images in spectral CT Urography (CTU) offer improved image quality compared to traditional true non-contrast (TNC) images, while maintaining similar accuracy in kidney stone detection, suggesting potential dose savings in clinical practice.
Generation of Brain Dual-Energy CT from Single-Energy CT Using Deep Learning

Abstract:Deep learning (DL) has shown great potential in conversions between various imaging modalities. Similarly, DL can be applied to synthesize a high-kV computed tomography (CT) image from its corresponding low-kV CT image. This indicates the feasibility of obtaining dual-energy CT (DECT) images without purchasing a DECT scanner. In this study, we investigated whether a low-to-high kV mapping was better than a high-to-low kV mapping. We used a U-Net model to perform conversions between different kV CT images. Moreover, we proposed a double U-Net model to improve the quality of original single-energy CT images. Ninety-eight patients who underwent brain DECT scans were used to train, validate, and test the proposed DL-based model. The results showed that the low-to-high kV conversion was better than the high-to-low kV conversion. In addition, the DL-based DECT images had better signal-to-noise ratios (SNRs) than the true (original) DECT images, but at the expense of a slight loss in spatial resolution. The mean CT number differences between the true and DL-based DECT images were within $\pm$ 1 HU. No statistically significant difference in CT number measurements was found between the true and DL-based DECT images (p > 0.05). The DL-based DECT images with improved SNR could produce low-noise virtual monoenergetic images. Our preliminary results indicate that DL has the potential to generate brain DECT images using single-energy brain CT images.

A deep learning system for automated kidney stone detection and volumetric segmentation on noncontrast CT scans

Abstract:Purpose: Early detection and size quantification of renal calculi are important for optimizing treatment and preventing severe kidney stone disease. Prior work has shown that volumetric measurements of kidney stones are more informative and reproducible than linear measurements. Deep learning-based systems that use abdominal noncontrast computed tomography (CT) scans may assist in detection and reduce workload by removing the need for manual stone volume measurement. Prior to this work, no such system had been developed for use on noisy low-dose CT or tested on a large-scale external dataset. Methods: We used a dataset of 91 CT colonography (CTC) scans with manually marked kidney stones combined with 89 CTC scans without kidney stones. To compare with a prior work half the data was used for training and half for testing. A set of CTC scans from 6185 patients from a separate institution with patient-level labels were used as an external validation set. A 3D U-Net model was employed to segment the kidneys, followed by gradient-based anisotropic denoising, thresholding, and region growing. A 13 layer convolutional neural network classifier was then applied to distinguish kidney stones from false positive regions. Results: The system achieved a sensitivity of 0.86 at 0.5 false positives per scan on a challenging test set of low-dose CT with many small stones, an improvement over an earlier work that obtained a sensitivity of 0.52. The stone volume measurements correlated well with manual measurements ( r 2 = 0.95 $r^2 = 0.95$ ). For patient-level classification, the system achieved an area under the receiver-operating characteristic of 0.95 on an external validation set (sensitivity = 0.88, specificity = 0.91 at the Youden point). A common cause of false positives were small atherosclerotic plaques in the renal sinus that simulated kidney stones. Conclusions: Our deep-learning-based system showed improvements over a previously developed system that did not use deep learning, with even higher performance on an external validation set.

Abstract:Objective:To develop and validate a cascaded deep learning algorithm for kidney stone detection on plain CT images.Methods:Plain CT images of the patients with kidney stones were retrospectively archived from January 2018 to July 2018 in Peking University First Hospital. The cases were divided into two datasets according to the date of the CT scanning: training dataset （ n=30) and held-out test dataset （ n=29). The development of the kidney stone detection method consisted of three steps. First, a U-Net model was trained on the training dataset for kidney segmentation, and the model′s performance was estimated with the dice coefficient. Second, the thresholding and region growing methods were used to detect the stones in the renal region predicted by the trained U-Net model. Third, the stones′ lengths （maximal, middle and minimal length) and CT attenuation were calculated and integrated into a structured report automatically. Using the radiologist′s labels and measurements (maximal, middle, minimal length and CT attenuation) as ground truth, the stone detection algorithm performance was evaluated with sensitivity, specificity and precision, and the stone measurement algorithm performance was evaluated with Bland-Altman plots. Results:The held-out test dataset consisted of 29 cases, containing 58 kidneys and 11 358 CT slices. The 38 kidneys containing 56 stones and 20 kidneys did not contain stones. The U-Net model showed good performance, with a mean dice coefficient of 0.96. And 10 945 of 11 358 CT slices had a dice coefficient no less than 0.90. The sensitivity, precision, and specificity of stone detection were 100% （38/38), 100% （38/38) and 100% （20/20) in the organ-level. The sensitivity and precision of stone detection were 100% （56/56) and 96.6% （56/58) in the lesion-level.Conclusion:A cascaded algorithm is constructed and can be used to detect kidney stones in plain CT images. The algorithm can improve efficiency with results automatically integrated into the structured report in clinical practice.

Abstract:Kidney stones are currently considered a very common disease and recent studies have shown a tendency for the incidence of this disease to increase in recent years. The disease is recognized as a serious threat to the population's health because it is associated with other serious illnesses that can greatly compromise people's quality of life. The development of technologies and strategies aimed at aiding the diagnosis and treatment of this disease has the potential to improve the quality and effectiveness of services provided by health professionals. Diagnosis based on medical images has been one of the main tools for diagnosing kidney stones and Deep Learning techniques have been widely proposed to perform this task. This study proposes a Deep Learning model for detecting kidney stones in computed tomography images. The model was trained with a dataset composed of images obtained from individuals who underwent examinations to analyze diseases in the urinary system. The model achieved an accuracy rate of 96.20% in its predictions and proved to be a suitable tool for treating the problem in question. The results obtained in this study demonstrate the potential of Deep Learning techniques as tools to help improve healthcare procedures related to imaging diagnosis.
Dose independent characterization of renal stones by means of dual energy computed tomography and machine learning: an ex-vivo study

Abstract:Objectives: To predict the main component of pure and mixed kidney stones using dual-energy computed tomography and machine learning. Methods: 200 kidney stones with a known composition as determined by infrared spectroscopy were examined using a non-anthropomorphic phantom on a spectral detector computed tomography scanner. Stones were of either pure (monocrystalline, n = 116) or compound (dicrystalline, n = 84) composition. Image acquisition was repeated twice using both, normal and low-dose protocols, respectively (ND/LD). Conventional images and low and high keV virtual monoenergetic images were reconstructed. Stones were semi-automatically segmented. A shallow neural network was trained using data from ND1 acquisition split into training (70%), testing (15%) and validation-datasets (15%). Performance for ND2 and both LD acquisitions was tested. Accuracy on a per-voxel and a per-stone basis was calculated. Results: Main components were: Whewellite (n = 80), weddellite (n = 21), Ca-phosphate (n = 39), cysteine (n = 20), struvite (n = 13), uric acid (n = 18) and xanthine stones (n = 9). Stone size ranged from 3 to 18 mm. Overall accuracy for predicting the main component on a per-voxel basis attained by ND testing dataset was 91.1%. On independently tested acquisitions, accuracy was 87.1-90.4%. Conclusions: Even in compound stones, the main component can be reliably determined using dual energy CT and machine learning, irrespective of dose protocol. Automatic Detection and Scoring of Kidney Stones on Noncontrast CT Images Using S.T.O.N.E. Nephrolithometry: Combined Deep Learning and Thresholding Methods

Abstract:To develop and validate a deep learning and thresholding-based model for automatic kidney stone detection and scoring according to S.T.O.N.E. nephrolithometry. Abdominal noncontrast computed tomography (NCCT) images were retrospectively archived from February 2018 to April 2019 for three parts: a segmentation dataset (n = 167), a hydronephrosis classification dataset (n = 282), and test dataset (n = 117). The model consisted of four steps. First, the 3D U-Nets for kidney and renal sinus segmentation were developed. Second, the deep 3D dual-path networks for hydronephrosis grading were developed. Third, the thresholding methods were used to detect and segment stones in the renal sinus region. The stone size, CT attenuation, and tract length were calculated from the segmented stone region. Fourth, the stone’s location was determined. The stone detection performance was estimated with sensitivity and positive predictive value (PPV). The hydronephrosis grading and stone size, tract length, number of involved calyces, and essence grading were estimated with the area under the curve (AUC) method and linear-weighted κ statistics, respectively. The stone detection algorithm reached a sensitivity of 95.9 % (236/246) and a PPV of 98.7 % (236/239). The hydronephrosis classification algorithm achieved an AUC of 0.97. The scoring model results showed good agreement with radiologist results for the stone size, tract length, number of involved calyces, and essence grading (κ = 0.95, 95 % confidence interval [CI]: 0.92, 0.98; κ = 0.97, 95 % CI: 0.95, 1.00; κ = 0.95, 95 % CI: 0.92, 0.98; and κ = 0.97, 95 % CI: 0.94, 1.00), respectively. The scoring model was constructed that can automatically detect and score stones in NCCT images.
An Efficient and Robust Approach Using Inductive Transfer-Based Ensemble Deep Neural Networks for Kidney Stone Detection

Abstract:Chronic kidney disorder is a global health problem involving the repercussions of impaired kidney function and kidney failure. A kidney stone is a kidney scenario that impairs kidney function. Because this disease is usually asymptomatic, early and quick detection of kidney problems is essential to avoid significant consequences. This study presents an automated detection of Computed Tomography (CT) kidney stone images using an inductive transfer-based ensemble Deep Neural Network (DNN). Three datasets are created for feature extraction from kidney CT images using pre-trained DNN models. After assembling several pre-trained DNNs, such as DarkNet19, InceptionV3, and ResNet101, the ensemble deep feature vector is created using feature concatenation. The Iterative ReliefF feature selection method is used to choose the most informative ensemble deep feature vectors, which are then fed into the K Nearest Neighbor classifier tuned using a Bayesian optimizer with a 10-fold cross-validation approach to detect kidney stones. The proposed strategy achieves 99.8% and 96.7% accuracy using the quality and noisy image datasets, which are superior to other DNN-based and traditional image detection approaches. This proposed automated approach can help urologists confirm their physical inspection of kidney stones, reducing the possibility of human mistakes.

Diagnostic performance and feasibility of dual-layer detector dual-energy CT for characterization of urinary stones in patients of different sizes

Abstract:Background: Urinary stones are frequently encountered in urology and are typically identified using non-contrast CT scans. Dual-energy CT (DECT) is a valuable imaging technique that produces material-specific images and allows for precise assessment of stone composition by estimating the effective atomic number (Zeff), a capability not achievable with the conventional single-energy CT's attenuation measurement method. Purpose: To investigate the diagnostic performance and image quality of dual-layer detector DECT (dlDECT) in characterizing urinary stones in patients of different sizes. Methods: All consecutive dlDECT examinations with stone protocol and presence of urinary stones between July 2018 and November 2019 were retrospectively evaluated. Two radiologists independently reviewed 120 kVp and color-overlay Zeff images to determine stone composition (reference standard = crystallography) and image quality. The objective analysis included image noise and Zeff values measurement. Results: A total of 739 urinary stones (median size 3.7 mm, range 1-35 mm) were identified on 177 CT examinations from 155 adults (mean age, 57 ± 15 years, 80 men, median weight 82.6 kg, range 42.6-186.9 kg). Using color-overlay Zeff images, the radiologists could subjectively interpret the composition in all stones ≥ 3 mm (n = 491). For stones with available reference standards (n = 74), dlDECT yielded a sensitivity of 80% (95%CI 44-98%) and a specificity of 98% (95%CI 92-100%) in visually discriminating uric acid from non-uric acid stones. Patients weighing > 90 kg and ≤ 90 kg had similar stone characterizability (p = 0.20), with 86% of stones characterized in the > 90 kg group and 87% in the ≤ 90 kg group. All examinations throughout various patients' weights revealed acceptable image quality. A Zeff cutoff of 7.66 accurately distinguished uric acid from non-uric acid stones (AUC = 1.00). Zeff analysis revealed AUCs of 0.78 and 0.91 for differentiating calcium-based stones from other non-uric stones and all stone types, respectively. Conclusion: dlDECT allowed accurate differentiation of uric acid and non-uric acid stones among patients with different body sizes with acceptable image quality. Clinical impact: The ability to accurately differentiate uric acid stones from non-uric acid stones using color-overlay Zeff images allows for better tailored treatment strategies, helping to choose appropriate interventions and prevent potential complications related to urinary stones in patient care.

Abstract:In this study, a novel methodology is proposed to combine deep learning techniques and metaheuristic optimization to enable early diagnosis of patients with kidney stone using CT images. This approach focuses on the utilization of the InceptionV4 model, a state-of-the-art deep learning architecture known for its outstanding performance in image classification tasks. To further enhance the performance of InceptionV4, a modified metaheuristic algorithm called Combined Dwarf Mongoose Optimization (CDMO) algorithm is introduced. The CDMO algorithm optimizes the arrangement and selection of hyperparameters for the InceptionV4 model. By fine-tuning the model's arrangement, the aim is to improve its accuracy and efficiency in detecting kidney stones from CT images. This optimization process ensures that the InceptionV4 model is tailored specifically to the task of kidney stone diagnosis, maximizing its diagnostic capabilities. To evaluate the performance of the proposed methodology, it is applied to the CT Kidney Dataset, which contains a diverse range of CT images with and without kidney stones. The accuracy of the methodology is compared with existing methodologies, including Kronecker product-based Convolution Neural Network (K-CNN), Combined Discrete First Chebyshev Wavelets Transform and Convolutional Neural Network (DFCWTCNN), Enhanced Deep Neural Network (EDNN), hybrid Convolutional Neural Network and Extreme Learning Machine (CNN-ELM), and Probabilistic Neural Network (PNN) and Watershed Algorithm (PNN/WA). The results demonstrate the superior performance of the proposed method compared with other methods in terms of accuracy, F1-score, and Jaccard Index. These results provide evidence of the system's dependability, efficacy, and potential for use in medical picture classification tasks. The classification of CT scans as positive or negative for kidney stones demonstrates a notable level of accuracy, reaching 98.14%. This high accuracy is attributed to the model's sensitivity of 97.74% and specificity of 96.33%. Additionally, the model has a precision rate of 86.66%, demonstrating its ability to accurately forecast positive instances. Moreover, it maintains a balanced trade-off between sensitivity and accuracy, as shown by its F1-score of 92.51%. By enabling early diagnosis of kidney stones, the proposed methodology has the potential to improve patient's outcomes and reduce healthcare costs associated with delayed diagnosis and complications.

Abstract:In a standard computed tomography (CT) image, pixels having the same Hounsfield Units (HU) can correspond to different materials and it is, therefore, challenging to differentiate and quantify materials. Dual-energy CT (DECT) is desirable to differentiate multiple materials, but DECT scanners are not widely available as single-energy CT (SECT) scanners. Here we develop a deep learning approach to perform DECT imaging by using standard SECT data. A deep learning model to map low-energy image to high-energy image using a two-stage convolutional neural network (CNN) is developed. The model was evaluated using patients who received contrast-enhanced abdomen DECT scan with a popular DE application: virtual non-contrast (VNC) imaging and contrast quantification. The HU differences between the predicted and original high-energy CT images are 3.47, 2.95, 2.38 and 2.40 HU for ROIs on the spine, aorta, liver, and stomach, respectively. The HU differences between VNC images obtained from original DECT and deep learning DECT are 4.10, 3.75, 2.33 and 2.92 HU for the 4 ROIs, respectively. The aorta iodine quantification difference between iodine maps obtained from original DECT and deep learning DECT images is 0.9\%, suggesting high consistency between the predicted and the original high-energy CT images. This study demonstrates that highly accurate DECT imaging with single low-energy data is achievable by using a deep learning approach. The proposed method can significantly simplify the DECT system design, reducing the scanning dose and imaging cost.

Abstract:Background Dual-energy CT (DECT) is a non-invasive way to determine the presence of monosodium urate (MSU) crystals in the workup of gout. Color-coding distinguishes MSU from calcium following material decomposition and post-processing. Most software labels MSU as green and calcium as blue. There are limitations in the current image processing methods of segmenting green-encoded pixels. Additionally, identifying green foci is tedious, and automated detection would improve workflow. This study aimed to determine the optimal deep learning (DL) algorithm for segmenting green-encoded pixels of MSU crystals on DECTs. Methods DECT images of positive and negative gout cases were retrospectively collected. The dataset was split into train (N = 28) and held-out test (N = 30) sets. To perform cross-validation, the train set was split into seven folds. The images were presented to two musculoskeletal radiologists, who independently identified green-encoded voxels. Two 3D Unet-based DL models, Segresnet and SwinUNETR, were trained, and the Dice similarity coefficient (DSC), sensitivity, and specificity were reported as the segmentation metrics. Results Segresnet showed superior performance, achieving a DSC of 0.9999 for the background pixels, 0.7868 for the green pixels, and an average DSC of 0.8934 for both types of pixels, respectively. According to the post-processed results, the Segresnet reached voxel-level sensitivity and specificity of 98.72 % and 99.98 %, respectively. Conclusion In this study, we compared two DL-based segmentation approaches for detecting MSU deposits in a DECT dataset. The Segresnet resulted in superior performance metrics. The developed algorithm provides a potential fast, consistent, highly sensitive and specific computer-aided diagnosis tool. Ultimately, such an algorithm could be used by radiologists to streamline DECT workflow and improve accuracy in the detection of gout.
A deep learning approach for virtual monochromatic spectral CT imaging with a standard single energy CT scanner

Abstract:Purpose/Objectives: To develop and assess a strategy of using deep learning (DL) to generate virtual monochromatic CT (VMCT) images from a single-energy CT (SECT) scan. Materials/Methods: The proposed data-driven VMCT imaging consists of two steps: (i) using a supervised DL model trained with a large number of 100 kV and 140 kV dual-energy CT (DECT) image pairs to produce the corresponding high-energy CT image from a low-energy image; and (ii) reconstructing VMCT images with energy ranging from 40 to 150 keV. To evaluate the performance of the method, we retrospectively studied 6,767 abdominal DECT images. The VMCT images reconstructed using both DL-derived DECT (DL-DECT) images and the images from DECT scanner were compared quantitatively. Paired-sample t-tests were used for statistical analysis to show the consistency and precision of calculated HU values. Results: Excellent agreement was found between the DL-DECT and the ground truth DECT images (p values ranged from 0.50 to 0.95). Noise reduction up to 68% (from 163 HU to 51 HU) was achieved for DL-based VMCT imaging as compared to that obtained by using the standard DECT. For the DL-based VMCT, the maximum iodine contrast-to-noise ratio (CNR) for each patient (ranging from 15.1 to 16.6) was achieved at 40 keV. In addition to the enormous benefit of VMCT acquisition with merely a SECT image, an improvement of CNR as high as 55% (from 10.7 to 16.6) was attained with the proposed approach. Conclusions: This study demonstrates that high-quality VMCT images can be obtained with only a SECT scan.

Value of deep learning reconstruction at ultra-low-dose CT for evaluation of urolithiasis

Abstract:ObjectivesTo determine the diagnostic accuracy and image quality of ultra-low-dose computed tomography (ULDCT) with deep learning reconstruction (DLR) to evaluate patients with suspected urolithiasis, compared with ULDCT with hybrid iterative reconstruction (HIR) by using low-dose CT (LDCT) with HIR as the reference standard.MethodsPatients with suspected urolithiasis were prospectively enrolled and underwent abdominopelvic LDCT, followed by ULDCT if any urinary stone was observed. Radiation exposure, stone characteristics, image noise, signal-to-noise ratio (SNR), and subjective image quality on a 5-point Likert scale were evaluated and compared.ResultsThe average effective radiation dose of ULDCT was significantly lower than that of LDCT (1.28 ± 0.34 vs. 5.49 ± 1.00 mSv, p < 0.001). According to the reference standard (LDCT-HIR), 148 urinary stones were observed in 85.0% (51/60) of patients. ULDCT-DLR detected 143 stones with a rate of 96.6%, and ULDCT-HIR detected 142 stones with a rate of 95.9%. The urinary stones that were not observed with ULDCT-DLR or ULDCT-HIR were renal calculi smaller than 3 mm. There were no significant differences in the detection of clinically significant calculi (≥ 3 mm) or stone size estimation among ULDCT-DLR, ULDCT-HIR, and LDCT-HIR. The image quality of ULDCT-DLR was better than that of ULDCT-HIR and LDCT-HIR with lower image noise, higher SNR, and higher average subjective score.ConclusionsULDCT-DLR performed comparably to LDCT-HIR in urinary stone detection and size estimation with better image quality and decreased radiation exposure. ULDCT-DLR may have potential to be considered the first-line choice to evaluate urolithiasis in practice.Key Points• Ultra-low-dose computed tomography (ULDCT) has been investigated for diagnosis of urolithiasis, but stone evaluation may be adversely impacted by compromised image quality.• This study evaluated the value of novel deep learning reconstruction (DLR) at ULDCT by comparing the stone evaluation and image quality of ULDCT-DLR to the reference standard of low-dose CT (LDCT) with hybrid iterative reconstruction (HIR).• ULDCT-DLR performed comparably to LDCT-HIR in urinary stone detection and size estimation with better image quality and reduced radiation exposure.

Abstract:Dual-energy computed tomography (DECT) is of great significance for clinical practice due to its huge potential to provide material-specific information. However, DECT scanners are usually more expensive than standard single-energy CT (SECT) scanners and thus are less accessible to undeveloped regions. In this paper, we show that the energy-domain correlation and anatomical consistency between standard DECT images can be harnessed by a deep learning model to provide high-performance DECT imaging from fully-sampled low-energy data together with single-view high-energy data. We demonstrate the feasibility of the approach with two independent cohorts (the first cohort including contrast-enhanced DECT scans of 5,753 image slices from 22 patients and the second cohort including spectral CT scans without contrast injection of 2463 image slices from other 22 patients) and show its superior performance on DECT applications. The deep-learning-based approach could be useful to further significantly reduce the radiation dose of current premium DECT scanners and has the potential to simplify the hardware of DECT imaging systems and to enable DECT imaging using standard SECT scanners.

Abstract:Abstract Objectives To assess the accuracy of low-dose dual-energy computed tomography (DECT) to differentiate uric acid from non-uric acid kidney stones in two generations of dual-source DECT with stone composition analysis as the reference standard. Methods Patients who received a low-dose unenhanced DECT for the detection or follow-up of urolithiasis and stone extraction with stone composition analysis between January 2020 and January 2022 were retrospectively included. Collected stones were characterized using X-ray diffraction. Size, volume, CT attenuation, and stone characterization were assessed using DECT post-processing software. Characterization as uric acid or non-uric acid stones was compared to stone composition analysis as the reference standard. Sensitivity, specificity, and accuracy of stone classification were computed. Dose length product (DLP) and effective dose served as radiation dose estimates. Results A total of 227 stones in 203 patients were analyzed. Stone composition analysis identified 15 uric acid and 212 non-uric acid stones. Mean size and volume were 4.7 mm × 2.8 mm and 114 mm 3 , respectively. CT attenuation of uric acid stones was significantly lower as compared to non-uric acid stones ( p < 0.001). Two hundred twenty-five of 227 kidney stones were correctly classified by DECT. Pooled sensitivity, specificity, and accuracy were 1.0 (95%CI: 0.97, 1.00), 0.93 (95%CI: 0.68, 1.00), and 0.99 (95%CI: 0.97, 1.00), respectively. Eighty-two of 84 stones with a diameter of ≤ 3 mm were correctly classified. Mean DLP was 162 ± 57 mGy*cm and effective dose was 2.43 ± 0.86 mSv. Conclusions Low-dose dual-source DECT demonstrated high accuracy to discriminate uric acid from non-uric acid stones even at small stone sizes. Abstract:Computed tomographic (CT) is a fundamental imaging modality to generate cross-sectional views of internal anatomy in a living subject or interrogate material composition of an object, and it has been routinely used in clinical applications and nondestructive testing. In a standard CT image, pixels having the same Hounsfield Units (HU) can correspond to different materials, and it is therefore challenging to differentiate and quantify materials. Dual-energy CT (DECT) is desirable to differentiate multiple materials, but the costly DECT scanners are not widely available as single-energy CT (SECT) scanners. Recent advancement in deep learning provides an enabling tool to map images between different modalities with incorporated prior knowledge. Here we develop a deep learning approach to perform DECT imaging by using the standard SECT data. The end point of the approach is a model capable of providing the high-energy CT image for a given input low-energy CT image. The feasibility of the deep learning-based DECT imaging method using a SECT data is demonstrated using contrast-enhanced DECT images and evaluated using clinical relevant indexes. This work opens new opportunities for numerous DECT clinical applications with a standard SECT data and may enable significantly simplified hardware design, scanning dose and image cost reduction for future DECT systems.
