Abstract:Objective To investigate the diagnostic values of prenatal two-dimensional ultrasound and four-dimensional ultrasound for fetal spina bifida occulta.Methods Ninety pregnant woman with suspected diagnosis of fetal spina bifida occulta in Zhejiang Xiaoshan Hospital from June 2012 to June 2018 were included,75 of them were confirmed as spina bifida according to neonatal outcomes and 15 cases were healthy babies.Prenatal records of two-and four-dimensional ultrasound were retrospectively analyzed.Results Among 75 cases of fetal spina bifida occulta,45 fetuses were diagnosed by two-dimensional ultrasound and 69 fetuses were diagnosed by four-dimensional ultrasound.Among the 15 misdiagnosed cases,two-dimensional ultrasound suggested negative results in 11 fetuses and four-dimensional ultrasound indicated 13 ones.Two-dimensional ultrasound showed poor diagnostic consistency (Kappa =0.378) and four-dimensional ultrasound showed good diagnostic consistency (Kappa =0.912).Sensitivity of four-dimensional ultrasound was 92.0％ (69/75),specificity was 86.7％ (13/15),false positive rate was 13.3％ (2/15),false negative rate was 8.0％ (6/75),accuracy was 91.1％ (82/90).Sensitivity of two-dimensional ultrasound was 60.0％ (45/75),specificity was 73.3％ (11/15),false positive rate was 26.7％ (4/15),false negative rate was 40.0％ (30/75),accuracy was 62.2％ (56/90).Four-dimensional ultrasound showed a better diagnostic value than two-dimensional ultrasound.Conclusion Four-dimensional ultrasound provides clear,audio-visual,continuous and dynamic images and has higher diagnostic accuracy than two-dimensional ultrasonogram for fetal spina bifida occulta.

Abstract:Objective To explore the clinical significance of prenatal ultrasonography for detecting birth defects.Methods A total of 8089 women during 13-40 gestational weeks underwent real-time color Doppler ultrasonography to find out fetal congenital anomalies.All the cases were tracked within seven days after delivery.Results In all 8089 pregnant women,164 cases with fetal defects were found and 104 cases were diagnosed fetal congenital anomalies on ultrasonography of 100% accordance with clinical diagnosis.Another 60 cases who had micro-malformation were misdiagnosed on color Doppler ultrasonography.Conclusions A systemic prenatal ultrasonographic examination in the second-trimester pregnancy is important for prenatal diagnosis of congenital malformations,which plays a very significant role in decreasing fetal birth defects and improving new born population quality.

Abstract:Objectives To assess diagnostic accuracy of 2D ultrasound at 11‐14 weeks gestation as a screening test for individual fetal anomalies and identify screening factors impacting detection. Methods Systematic review and meta‐analysis, developed and registered with PROSPERO (CRD42018111781). MEDLINE, EMBASE, Web of Science Core Collection and The Cochrane Library) were searched for studies evaluating the diagnostic accuracy of screening for 16 pre‐specified, non‐cardiac, congenital anomalies considered to be of interest to the early anomaly scan. We included prospective and retrospective studies from any healthcare setting and low risk, mixed risk and unselected populations. The reference standard was the detection of an anomaly on postnatal or post‐mortem examination. Data were extracted to populate 2 x 2 tables and meta‐analysis (random‐effects model) undertaken to determine the diagnostic accuracy of screening for the pre‐specified anomalies (individually and as a composite). Secondary analyses were performed to determine the impact of (1) imaging protocol (2) ultrasound modality (3) publication year and (4) index of sonographer suspicion at time of scan. Post‐hoc secondary analysis was conducted to assess performance for studies from 2010. Risk of bias and quality assessment was undertaken for included studies using the Quality Assessment of Diagnostic Accuracy Studies (QUADAS‐2). Results From 5684 citations, 202 papers were identified as eligible and reviewed, resulting in the inclusion of 526,322 fetuses (52 studies) of which 2,399 were affected by one or more of the 16 anomalies. Individual anomalies were not equally amenable to detection on first trimester ultrasound ranging from high (>80%) detection rates for severe conditions including acrania (98%), gastroschisis (96%) and exomphalos (95%) and holoprosencephaly (88%); they were lower for open spina bifida (69%), lower urinary tract obstruction (66%) lethal skeletal dysplasias (57%) and limb reduction defects (50%) and below 50% for facial clefts (43%), polydactyly (40%) and congenital diaphragmatic hernia (38). Conditions with low ( 99% for all anomalies. Secondary analysis showed improvement of detection with publication year, and that the use of imaging protocols had a statistically significant impact on screening performance (p<0.0001). Conclusions Accurate detection of congenital anomalies using first trimester ultrasound is feasible. In this study we have determined screening characteristics for individual anomalies and have shown that detection rates and false positive rates are dependent on the type of anomaly. The use of a standardised protocol allows diagnostic performance to be maximised, and this particularly enhances screening performance for the detection of spina bifida, facial clefts and limb reduction defects. Highlighting the types of anomalies amenable to diagnosis and determining favourable screening test factors can support the development of first‐trimester anomaly screening programs. Value Analysis of Four-Dimensional Color Ultrasound Combined with Maternal Serological Index in Prenatal Screening for Fetal Anomalies

Abstract:Objective: This study aimed to assess the efficacy of combining four-dimensional (4D) color ultrasound with maternal serological index testing in prenatal screening for fetal anomalies. Methods: A retrospective analysis was conducted on data from 864 pregnant women who underwent prenatal checkups at the hospital between January 2020 and January 2021. During the mid-pregnancy period, serological tests were performed to determine levels of alpha-fetoprotein (AFP), free β-subunit of human chorionic gonadotropin (Free-HCG β), pregnancy-associated plasma protein A (PAPP-A), and vitamin B12 (VitB12). Additionally, 4D color ultrasound examinations were conducted. The gold standard for evaluation was the results of delivery or labor induction. AFP, Free-HCG β, PAPP-A, and VitB12 levels were compared between the anomaly group and the normal group. The diagnostic efficacy of single and combined detection of serological indexes and 4D color ultrasound was analyzed, with the calculation of the areas under the curve (AUC) for different detection methods. Results: Among the 864 pregnant women, 44 cases (5.09%) exhibited fetal anomalies, while 820 cases (94.91%) did not. The anomaly group showed significantly higher multiples of the median (MOM) values for AFP and Free-HCG β (P < .001) and significantly lower PAPP-A MOM and VitB12 levels (P < .001) compared to the normal group. The sensitivity of single detections for AFP MOM, Free-HCG β MOM, PAPP-A MOM, VitB12, 4D color ultrasound, and combined detection were 63.64%, 68.18%, 65.91%, 54.55%, 77.27%, and 97.93%, respectively. The corresponding AUC values were 0.805, 0.829, 0.818, 0.761, 0.885, and 0.974. Conclusions: The combination of 4D color ultrasound with maternal serological index testing demonstrated high sensitivity in prenatal screening for fetal anomalies.
Investigating Groundwater Flow Between Edwards and Trinity Aquifers in Central Texas

Abstract:Understanding the nature of communication between aquifers can be challenging when using traditional physical and geochemical groundwater sampling approaches. This study uses two multiport wells completed within Edwards and Trinity aquifers in central Texas to determine the degree of groundwater inter‐flow between adjacent aquifers. Potentiometric surfaces, hydraulic conductivities, and groundwater major ion concentrations and Sr isotope values were measured from multiple zones within three hydrostratigraphic units (Edwards and Upper and Middle Trinity aquifers). Physical and geochemical data from the multiport wells were combined with historical measurements of groundwater levels and geochemical compositions from the region to characterize groundwater flow and identify controls on the geochemical compositions of the Edwards and Trinity aquifers. Our results suggest that vertical groundwater flow between Edwards and Middle Trinity aquifers is likely limited by low permeability, evaporite‐rich units within the Upper and Middle Trinity. Potentiometric surface levels in both aquifers vary with changes in wet vs. dry conditions, indicating that recharge to both aquifers occurs through distinct recharge areas. Geochemical compositions in the Edwards, Upper, and Middle Trinity aquifers are distinct and likely reflect groundwater interaction with different lithologies (e.g., carbonates, evaporites, and siliceous sediments) as opposed to mixing of groundwater between the aquifers. These results have implications for the management of these aquifers as they indicate that, under current conditions, pumping of either aquifer will likely not induce vertical cross‐formational flow between the aquifers. Inter‐flow between the Trinity and the Edwards aquifers, however, should be reevaluated as pumping patterns and hydrogeologic conditions change.
Importance of a detailed anomaly scan after a cfDNA test indicating fetal trisomy 21, 18 or 13

Abstract:Objective: To investigate the effect of the presence or absence of fetal anomalies and soft markers diagnosed by ultrasound on positive predictive value (PPV) 21, 18 and 13 in pregnancies with a high-risk cfDNA result. Methods: Retrospective study including singleton pregnancies with high-risk NIPT results for common trisomies followed by invasive testing. The cases were grouped by gestational age at the time of invasive testing and by the presence or absence of fetal abnormalities or soft markers. The ultrasound was considered abnormal if at least one major defect or a soft marker was detected. Results: A total of 173 women were included. Median maternal and gestational age was 37.7 years and 14.0 weeks, respectively. CfDNA test result showed high-risk for trisomy 21 and trisomy 18 or 13 in 119 and 54 cases, respectively. The "pre-ultrasound" PPV for trisomy 21 and for trisomy 18 or 13 were 98.3% and 68.4%, respectively. In case of a high-risk result for trisomy 21 and no fetal anomalies, the PPV was 86.7% while it was 100% if there were anomalies or markers present. In the case of a high-risk result for trisomy 18 or 13, the PPV was 9.5% if the ultrasound examination was normal and 100% if the ultrasound examination was abnormal. Conclusion: This study suggests that a detailed ultrasound examination performed after a cfDNA result that is high-risk for one of the common autosomal trisomies adds significantly to establishing an individualized risk assessment. This is particularly true in cases with a high-risk result for trisomies 18 or 13.

Abstract:Objective To determine the live born prenatal detection rate of significant congenital heart disease (CHD) in a large, integrated, multi‐center community‐based health system using a strategy of referral only of patients with significant cardiac abnormalities on obstetrical screening ultrasound for fetal echocardiography. Detection rates were assessed for screening in both radiology and maternal fetal medicine (MFM). The impact on fetal echocardiography utilization was also assessed. Methods This was a retrospective cohort study using an electronic health record, outside claims databases and chart review to determine all live births between 2016 and 2020 with postnatally confirmed sCHD that were prenatally detectable and resulted in cardiac surgery, intervention, or death within 1 year. Results There were 214,486 pregnancies resulting in live births. Prenatally detectable significant CHD was confirmed in 294 infants. Of those 183 were detected for an overall live‐born detection rate of 62%. Detection rates in MFM were 75% and in radiology were 52%. The number of fetal echocardiograms needed to detect (NND) sCHD was 7. Conclusions A focus on quality and standardization of obstetrical screening ultrasound with referral to fetal echocardiography for cardiac abnormalities alone achieves benchmark targets for live‐born detection of significant CHD requiring fewer fetal echocardiograms.

The Correlation for Prenatal Ultrasound Screening of Fetal Malformations and Fetal Chromosomal Abnormality

Abstract:Objective:To assess the effectiveness of systematic ultrasound in second or third trimester to detect fetal malformations for instructing the necesity of further invasive prenatal diagnosis.Methods:This retrospective study included pregnancy women from January 2008 to June 2009 in the Affiliated Drum Tower hospital of Nanjing University Medical College.Those women had systematic ultrasound and echocardiogram examination within the second or third trimester of pregnancy.When fetal malformations were found,the invasive prenatal diagnosis(puncture of amniotic fluid or umbilical vein) was suggested to check the chromosomal abnormalities and analyze their relationship.Results:105 pregnant women who were found fetal abnormalities were recruited and had the invasive prenatal diagnosis,26 cases of chromosomal abnormalities were found in 77 obvious ultrasound abnormalities.There was significant difference in chromosomal abnormalities between obvious and minor ultrasound abnormalities(χ2=12.566,P0.001),especially for fetal congenital heart disease(CHD) combined with extracardiac malformations,their chromosomal abnormalities could reach 55%(11/20).Conclusions: Prenatal systematic ultrasound and echocardiogram could find most of the fetal malformations,especially for CHD,and offer valuable evidence to accept the invasive prenatal diagnosis.
The value of detailed first-trimester ultrasound in the era of noninvasive prenatal testing

Abstract:Background: In 2020, the American College of Obstetricians and Gynecologists recommended noninvasive prenatal testing be offered to all patients. However, current societal guidelines in the United States do not universally recommend a detailed first-trimester ultrasound. Objective: This study aimed to determine the additional findings identified through first-trimester ultrasound that would have otherwise been missed if noninvasive prenatal testing was used alone as a first-trimester screening method. Study design: This was a retrospective cohort study involving 2158 pregnant patients and 2216 fetuses that were seen at a single medical center between January 1, 2020, and December 31, 2022. All those included underwent both noninvasive prenatal testing and detailed first-trimester ultrasound between 11.0 and 13.6 weeks of gestation. Noninvasive prenatal testing results were categorized as low risk or high risk, and first-trimester ultrasound results were categorized as normal or abnormal. Abnormal first-trimester ultrasounds were further classified as first-trimester screening markers (increased nuchal translucency, absent nasal bone, tricuspid regurgitation, and ductus venosus reverse a-wave) or structural defects (the cranium, neck, heart, thorax, abdominal wall, stomach, kidneys, bladder, spine, and extremities). Descriptive statistics were used to report our findings. Results: Of 2216 fetuses, 65 (3.0%) had a high-risk noninvasive prenatal testing result, whereas 2151 (97.0%) had a low-risk noninvasive prenatal testing result. Of those with a low-risk noninvasive prenatal testing result, 2035 (94.6%) had a normal first-trimester ultrasound, whereas 116 (5.4%) had at least 1 abnormal finding on first-trimester ultrasound. The most common screening marker detected within the low-risk noninvasive prenatal testing group was absent nasal bone (52/2151 [2.4%]), followed by reversed a-wave of the ductus venosus (30/2151 [1.4%]). The most common structural defect in this group was cardiac abnormality (15/2151 [0.7%]). Overall, 181 fetuses were identified as having "abnormal screening" through either a high-risk noninvasive prenatal testing result (n=65) or through a low-risk noninvasive prenatal testing result but abnormal first-trimester ultrasound (n=116). In summary, the incorporation of first-trimester ultrasound screening identified 116 additional fetuses (5.4%) that required further follow-up and surveillance than noninvasive prenatal testing alone would have identified. Conclusion: Detailed first-trimester ultrasound identified more fetuses with a potential abnormality than noninvasive prenatal testing alone. Therefore, first-trimester ultrasound remains a valuable screening method that should be used in combination with noninvasive prenatal testing.
Accuracy of Antenatal Ultrasound in Predicting Large for Gestational Age Babies: Population Based Cohort Study

Abstract:Background Pregnancies with large for gestational age fetuses are at increased risk of adverse maternal and neonatal outcomes. There is uncertainty about how to manage birth in such pregnancies. Current guidelines recommend a discussion with women of the pros and cons of options including expectant management, induction of labour and caesarean birth. For women to be able to make an informed decision about birth, antenatal detection of large for gestational age is essential. Objective To investigate the ability of antenatal ultrasound scans to predict large for gestational age at birth. Study Design In this retrospective cohort study, we analysed data from a routinely collected database from the West Midlands, United Kingdom. We included pregnancies that had an antenatal ultrasound estimated fetal weight between 35+0 and 38+0 weeks gestation for any indication, as well as a subgroup where the reason for scan was that the fetus was suspected to be big. Large for gestational age was defined as >90 th customized GROW centile, for estimated fetal weight as well as for neonatal weight. We also tested performance of an uncustomized standard, with Hadlock fetal weight >90 th centile and neonatal weight >4kg. We calculated diagnostic characteristics for the whole population as well as groups with different maternal body mass indices. Results The study cohort consisted of 26,527 pregnancies which on average had a scan at 256 days gestation and delivered 20 days later at a median of 276 days (interquartile range 15). In total 2241 (8.4%) of neonates were large for gestational age by customized centiles, of which 1459 (65.1%) had a scan estimated fetal weight >90 th centile, with a false positive rate of 8.6% and a positive predictive value of 41.0%. In the subgroup of 912 (3.4%) pregnancies scanned for a suspected large fetus, 293 (32.1%) babies were large for gestational age at birth, giving a positive predictive value of 50.3%, with sensitivity of 77.1% and false positive rate 36.0%. When comparing subgroups from low (<18.5kg/m 2 ) to high body mass index (>30kg/m 2 ), sensitivity increased from 55.6 to 67.8%, false positive rate from 5.2 to 11.5% and positive predictive value from 32.1 to 42.3%. A total of 2585 (9.7%) babies were macrosomic (birthweight >4kg), and of these 1058 (40.9%) were large for gestational age (>90 th centile) antenatally by Hadlock's growth standard, with a false positive rate of 4.9% and a positive predictive value 41.0%. Analysis within subgroups showed better performance by customized compared to uncustomized standards for low body mass index ( 30; diagnostic odds ration 16.2 vs 8.8). Conclusion Late third trimester ultrasound estimation of fetal weight for any indication has good ability to identify and predict large for gestational age at birth, and improves with the use of a customised standard. Detection rate is better when ultrasound is performed for a suspected large fetus, however at the risk of higher false positive diagnosis. Our results provide information for women and clinicians to aid antenatal decision making about birth of a fetus suspected of being large for gestational age.

The Clinical Significance of Color Doppler Ultrasound in Prenatal Heart Screening in Non-High-Risk Fetus

Abstract:Purpose To explore the clinical significance of color Doppler ultrasound in prenatal heart screening in non-high-risk fetus. Materials and Methods A total of 6014 fetuses were included, 5317 in non-high-risk group and 697 in high-risk group. The fetal heart was scanned in four-chamber, left and right ventricular outflow tract views. All fetuses were followed to confirm the diagnosis. The incidences of cardiac anomalies in the two groups were compared. Diagnostic test method was used to calculate the sensitivity, specificity, positive and negative predictive value, prenatal detection rate and diagnostic accuracy of ultrasound to screen cardiac anomalies. Results 41 cardiac anomalies cases were included, of which 38 cases were detected by prenatal ultrasound(32 in non-high-risk group and 6 in high-risk group respectively). Echocardiography after delivery confirmed that 2 cases of ventricular septal defect and 1 case of tetralogy of Fallot were missed in non-high-risk group; 1 double outlet was misdiagnosed as tetralogy of Fallot prenatally in high-risk group. The incidence of cardiac anomalies in all fetuses, non-high-risk group, high-risk group were 6.8‰, 6.5‰, 8.6‰ respectively. There was no statistical difference in the incidence of cardiac anomalies between the two groups(P>0.05). The sensitivity, specificity, positive predictive value, negative predictive value of untrasound to screen cardiac anomalies were 90.2%, 99.9%, 97.3% and 99.9% respectively. Prenatal detection rate and diagnostic accuracy were 92.6%, 90.2% respectively. Conclusion It is important to perform prenatal ultrasound heart screening in non-high-risk group. The four-chamber, left and right ventricular outflow tract views have high specificity and sensitivity in fetal heart anomalies. It should be gradually introduced into clinical routine.
The predictive value of thromboelastography, routine blood indices, ultrasound parameters, and placental thickness in determining fetal outcome

Abstract:Objective: To evaluate the predictive value of thromboelastography, routine blood indices, ultrasound measurements, and placental thickness for fetal outcome. Methods: A retrospective analysis of 218 expectant mothers at our hospital from April 2020 to June 2022 was conducted. Mothers were classified into favorable (n=164) and adverse (n=54) fetal outcome groups. We compared thromboelastography, blood counts, and ultrasound parameters, including placental thickness, between the two groups. Predictive models using lasso regression were developed for individual assessment type and their combinations. Model efficacies were evaluated by ROC curves and Delong's test. Results: Thromboelastography indicated significantly higher values of R (P=0.004), Angle (P<0.001), and MA (P=0.002) while notably lower K (P<0.001) in the adverse outcome group compared to the favorable outcome group. Peripheral blood analysis showed elevated levels of WBC (P<0.001), CRP (P=0.001), and PLR (P<0.001) in the adverse outcome group. Ultrasound assessments revealed significant increases in S/D (P<0.001), PI (P=0.016), RI (P<0.001), and placental thickness (P<0.001) in the adverse outcome group. The areas under the curve (AUCs) for the thromboelastography (4 features), peripheral blood indices (3 features), ultrasound parameters (4 features), and combined index model (11 features) were 0.774, 0.779, 0.961, and 0.978, respectively. Delong's test indicated that the combined model's AUC did not significantly differ from that of the ultrasound parameters (P>0.05) but was superior to the models based on thromboelastography, peripheral blood indices, and placental thickness alone (P<0.001). Conclusion: This study underscores the unparalleled predictive value of ultrasound metrics in identifying the risk of adverse pregnancy outcomes, highlighting their critical role in prenatal risk assessment and monitoring frameworks.

Abstract:Objective To summarize the prenatal ultrasonography characteristics of fetal urinary system abnormalities. And to investigate the clinical value of prenatal ultrasound diagnosis for these malformations. Methods The ultrasound images of 492 cases of fetal urinary system abnormalities detected by prenatal ultrasound were retrospectively analyzed, including associated malformations. Results There were total 548 abnormalities in 492 cases. Some malformations were detected in most cases (84.3%, 415/492), including 232 cases of hydronephrosis (47.2%, 232/492), 100 cases of renal agenesis or atrophy (20.3%, 100/492) and 83 cases of polycystic kidney disease (16.9%, 83/492), while other kinds of malformations were detected in less cases (27.0%, 133/492), including renal dysplasia, ectopic kidney, confluent kidney, renal duplication, renal cyst, megalocystis, ureterocele, urachal diverticulum, OEIS and prune belly syndrome. Only five cases of megacystis were detected in the first trimester and the others were found in the second or third trimester. Three hundred and sixty-six cases (74.4%, 366/492) were isolated urinary system abnormalities, and the other 126 cases (25.6%, 126/492) were combined with one or more other malformations, totally 214 associated abnormalities. Eighty-eight cases (69.8%, 88/126) with other structural malformations were as followings, 22 cardiac abnormalities, 19 digestive system abnormalities, 17 cranial malformations, 12 limbs abnormalities, nine facial malformations and nine spinal abnormalities, still another 34 cases (27.0%, 34/126) with oligohydramnios. During follow-up, five cases were found to be misdiagnosed by prenatal ultrasonography. Two normal cases were misdiagnosed as renal duplication, one adrenal cyst was misdiagnosed as renal cyst, and two cases of ectopic kidney were misdiagnosed as renal agenesis. Conclusion Ultrasonography is a choice in prenatal diagnosis of fetal urinary system abnormalities. It plays an important role in the early detection of these abnormalities and has great significance for clinical diagnosis and treatment. Abstract:OBJECTIVE:To assess prenatal heart disease screening program by ultrasound.METHODS:A total of 11,544 second-trimester screening scans were performed before 24 weeks' gestation on 11,410 women between February 2004 and May 2007 in Obstetrics and Gynecology Hospital of Fudan University. Fetal heart screening was based on four-chamber and outflow tract views (left ventricular outflow + three vessel view). The sensitivity and specificity of different views were evaluated. Follow-up data of newborns was obtained.RESULTS:(1) Among 11,544 cases,48 cases of congenital heart disease (CHD) were diagnosed in utero. Six cases were false negative, and 2 cases were false positive. The incidence of CHD was 0.47% (54/11 544). (2) Thirty-three CHDs were detected based on the four-chamber view, including 18 ventricular septal defect (9 with conotruncal anomalies), 6 anomalous atrioventricle valve, 9 disproportion of left/right ventricle. The sensitivity of the four-chamber view alone was 61.11% (33/54), and the specificity was 99.98% (11 488/11 490). Fifteen CHDs were detected based on the left ventricular outflow and three vessel views, including 1 pulmonary atresia, 3 pulmonary valve stenosis, 2 transposition of the great arteries (TGA), 1 pulmonary stenosis with TGA, 6 tetralogy of Fallot, and 2 pulmonary stenosis. The sensitivity of the combination of the four-chamber and outflow tract views was 88.89% (48/54), and the specificity was 99.98% (11 488/11 490). (3) Of 48 CHDs, 11 cases were accompanied by other malformations. Eleven cases were performed amniocentesis, among whom 5 cases were trisomy 21.CONCLUSION:The screening program based on four-chamber and outflow tract views shows good sensitivity and excellent specificity. Our prenatal heart screening program is clinically feasible.

Abstract:Background Selective fetal restriction growth (sFGR) is one of the common diseases of monochorionic diamniotic (MCDA) twin pregnancies, resulting in many adverse outcomes. At present, second trimester ultrasonography is widely used in the prenatal diagnosis of sFGR, but the diagnostic effectiveness is still uncertain. The aim of this study is to assess the diagnostic accuracy of second trimester Doppler ultrasound measurements for sFGR. Methods A retrospective study included 280 pregnant women (118 with and 162 without sFGR) with MCDA pregnancies was conducted in the fetal medicine center from Leiden University Medical Center from January 2008 to December 2013. The women participating had already undergone an ultrasound examination in the second trimester. The postnatal criteria of sFGR was a single birth weight (BW) < 3 rd percentile in a twin, or birth weight discordance (BWD)≥25% between two twins, while the BW of the smaller twin < 10th percentile. Early prenatal diagnosis of sFGR was defined as a single EFW < 3 rd percentile in a twin, or at least 2 of the following 4 parameters must be met (fetal weight of one fetus < 10th percentile, AC of one fetus 95th percentile). According to the diagnosis of sFGR after birth, we evaluate diagnostic effectiveness of Doppler ultrasound in the second trimester for sFGR. Results Of these 280 participants, the mean age was 32.06 ± 4.76 years. About 43.9% of pregnant women were primiparas. The ability of second trimester Doppler ultrasound to accurately diagnosed sFGR is 75.4%, missed diagnosis rate and the misdiagnosis rate were 24.6% and 10.5% respectively. The ROC curve indicated that the combination of AC discordance, EFW discordance, and small fetal UA blood flow was the best diagnostic indicator of sFGR in MCDA pregnancy with the AUC was 0.882 (95%CI, 0.839–0.926). Conclusions Second trimester Doppler and ultrasound measurements is an effective method for early prenatal diagnosis of sFGR. The combined indicator of AC discordance, EFW discordance, and the small fetal UA blood flow reaches highest diagnostic value.

Risk Factors Associated With Renal and Urinary Tract Anomalies Delineated by an Ultrasound Screening Program in Infants

Abstract:ObjectiveTo evaluate the value of ultrasound screening for congenital anomalies of the kidney and urinary tract (CAKUT) during the early postnatal period. MethodsThis is a prospective study that enrolled all neonates born from August 2019 to July 2020 at one medical center. Postnatal ultrasound screening was conducted in all neonates at 1, 3, and 6 months old, respectively. Information on antenatal detection and pregnancy was collected. We performed logistic regression analyses and established a predictive model to assess the potential risk factors of abnormal ultrasound screening results. ResultsPostnatal ultrasound scanning in 4,877 infants identified 268 cases (5.5%) of anomalies of kidney and urinary tract by primary screening and 92 cases (1.9%) by tertiary screening. A specific diagnosis was identified in 47 cases within the 6-month screening and follow-up program. Logistic regression revealed that preterm birth, oligohydramnios, antenatal ultrasound screening anomalies, and gestational hypothyroidism were independent risk factors for the early detection of CAKUT by postnatal ultrasound screening. The above factors were adopted to develop a predictive model that showed good calibration in predicting ultrasound findings of CAKUT. Decision curve analysis demonstrated good clinical utility. ConclusionsPostnatal ultrasound screening should be conducted in infants with risk factors associated with CAKUT. Further study on prenatal and fetal factors could help establish the predictive model for the early detection of CAKUT.

Abstract:ABSTRACT Background Congenital anomalies of the kidney and urinary tract (CAKUT), often discovered in utero, cover a wide spectrum of outcomes ranging from normal postnatal kidney function to foetal death. The current ultrasound workup does not allow for an accurate assessment of the outcome. The present study aimed to significantly improve the ultrasound-based prediction of postnatal kidney survival in CAKUT. Methods Histological analysis of kidneys of 15 CAKUT foetuses was performed to better standardize the ultrasound interpretation of dysplasia and cysts. Ultrasound images of 140 CAKUT foetuses with 2-year postnatal follow-up were annotated for amniotic fluid volume and kidney number, size, dysplasia and/or cysts using a standardized ultrasound readout. Association of ultrasound features and clinical data (sex and age at diagnosis) with postnatal kidney function was studied using logistic regression. Amniotic fluid proteome related to kidney dysplasia or cysts was characterized by mass spectrometry. Results Histologically, poor ultrasound corticomedullary differentiation was associated with dysplastic lesions and ultrasound hyperechogenicity was associated with the presence of microcysts. Of all ultrasound and clinical parameters, reduced amniotic volume, dysplasia and cysts were the best predictors of poor outcome (odds ratio 57 [95% confidence interval (CI) 11–481], 20 [3–225] and 7 [1–100], respectively). Their combination into an algorithm improved prediction of postnatal kidney function compared with amniotic volume alone (area under the receiver operating characteristics curve 0.92 [95% CI 0.86–0.98] in a 10-fold cross-validation). Dysplasia and cysts were correlated (Cramer's V coefficient = 0.44, P < .0001), but amniotic fluid proteome analysis revealed that they had a distinct molecular origin (extracellular matrix and cell contacts versus cellular death, respectively), probably explaining the additivity of their predictive performances. Conclusion Antenatal clinical advice for CAKUT pregnancies can be improved by a more standardized and combined interpretation of ultrasound data.

Abstract:Introduction Various methods are employed to evaluate the well-being of the fetus in high-risk pregnancies which consists of a biophysical profile (BPP), a non-stress test (NST), and daily fetal movements. Detection of aberrant blood flow in fetoplacental beds has been revolutionized by recent developments in ultrasound technology, such as color Doppler flow velocimetry. The cornerstone of maternal and fetal care is lowering maternal and perinatal mortality and morbidity is antepartum fetal surveillance. Doppler ultrasound is a non-invasive way of obtaining a qualitative and quantitative evaluation of maternal and fetal circulation and is utilized to investigate complications like fetal growth restriction (FGR) and fetal distress. Thus, it is useful in making distinctions between fetuses that are truly growth restricted and small for gestational age and healthy fetuses. The aim of the current study was to determine the role of Doppler indices in high-risk pregnancies and their accuracy in predicting fetal outcomes. Material and methods This prospective cohort study included 90 high-risk pregnancies in the III trimester (after 28 weeks of gestation) on whom ultrasonography and Doppler were performed. Ultrasonography was performed using PHILIPS EPIQ 5, a curvilinear probe of frequency 2-5MHz. Gestational age was determined with a biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC), and femoral length (FL). Placental grading and position were noted. Estimated fetal weight and the amniotic fluid index were calculated. BPP scoring was done. Doppler study was conducted and the findings of Doppler indices that is pulsatility index (PI) and resistive index (RI) of the middle cerebral artery (MCA), umbilical artery (UA), and uterine artery (UTA), and cerebroplacental (CP) ratio in these high-risk pregnancies were documented and compared with standard. The flow patterns in MCA, UA, and UTA were also assessed. These findings were correlated with fetal outcomes. Results Among 90 cases, the common high-risk factor in pregnancy was preeclampsia without severe features (30%). Growth lag was present in 43 (47.8%) participants. Among the study population, HC/AC ratio was increased in 19 (21.1%) participants which indicates asymmetrical intrauterine growth restriction. Adverse fetal outcomes were seen in 59 (65.6%) of the subjects. CP ratio and UA PI had better sensitivity (83.05% and 79.66%, respectively) and positive predictive value (PPV) (87.50% and 90.38%, respectively) in identifying the adverse fetal outcomes. Diagnostic accuracy of CP ratio and UA PI (Accuracy=81.11%) was highest in predicting adverse outcomes than all the other parameters. Conclusion CP ratio and UA PI had better sensitivity, PPV, and diagnostic accuracy in identifying adverse fetal outcomes than other parameters. The study's findings support that the use of color Doppler imaging in high-risk pregnancies will help in the early identification of adverse fetal outcomes and aid in early intervention. This study is non-invasive, simple, safe, and reproducible. This study can also be performed bedside in high risk and unstable patients. This study is required to accurately assess fetal well-being in all high-risk pregnancies in order to improve fetal outcomes and to incorporate this procedure as a part of the protocol for the assessment of fetal well-being in these patients.

Abstract:Objectives To evaluate the association of standardized prenatal imaging parameters and immediate neonatal variables with mortality prior to discharge in infants with isolated left congenital diaphragmatic hernia (LCDH), and to compare the performance of ultrasound‐ and magnetic resonance imaging (MRI)‐based severity grading for the prediction of neonatal mortality. Methods This was a retrospective study of infants with prenatally diagnosed isolated LCDH referred to a single tertiary center between 2008 and 2020. Fetuses with right or bilateral congenital diaphragmatic hernia, additional major structural anomaly or known genetic condition, as well as cases that underwent fetal intervention or declined postnatal intervention, were excluded. Ultrasound and MRI images were reviewed retrospectively. Univariable and multivariable analyses were performed, incorporating prenatal and immediate neonatal factors to analyze the association with neonatal mortality prior to discharge, and a prediction calculator was generated. The performance of ultrasound and that of MRI for the prediction of neonatal mortality were compared. Results Of 253 pregnancies with fetal CDH, 104 met the inclusion criteria, of whom 77 (74%) neonates survived to discharge. Seventy‐five fetuses underwent both prenatal ultrasound and MRI. On multivariable analysis, observed/expected (o/e) lung‐to‐head ratio and o/e total fetal lung volume were associated independently with neonatal death (adjusted odds ratio, 0.89 (95% CI, 0.83–0.95) and 0.90 (95% CI, 0.84–0.97), respectively), whereas liver position was not. There was no significant difference in predictive performance between using ultrasound and MRI together (area under the receiver‐operating‐characteristics curve (AUC), 0.85 (95% CI, 0.76–0.93)) compared with using ultrasound alone (AUC, 0.81 (95% CI, 0.72–0.90); P = 0.19). The addition of neonatal parameters (gestational age at birth and small‐for‐gestational age) did not improve model performance (AUC, 0.87 (95% CI, 0.80–0.95)) compared with the combined ultrasound and MRI model (P = 0.22). There was poor agreement between severity assessment on ultrasound and MRI (Cohen's κ, 0.19). Most discrepancies were seen among cases deemed to be non‐severe on ultrasound and severe on MRI, and outcomes were more consistent with MRI‐based prognostication. Conclusions In fetuses with prenatally diagnosed isolated LCDH, mortality prediction using standardized ultrasound and MRI measurements performed reasonably well. In cases classified as non‐severe on ultrasound, MRI is recommended, as it may provide more accurate prognostication and assist in the determination of candidacy for fetal intervention. © 2024 The Author(s).
