Exploring the Utility and Fairness of a Multi-institutional Situational Judgment Test

Abstract:Objective: To pilot a situational judgment test (SJT) developed to assess empathy, integrity, and teamwork and explore differences in performance between first-year (P1) and fourth-year (P4) student pharmacists based on gender, race or ethnicity, and geographical region. Methods: An SJT was developed to assess empathy, integrity, and teamwork, using best practices. The SJT was piloted tested with P1 and P4 student pharmacists at 5 institutions. Scores were adjusted for elevation and scatter to minimize the subgroup differences in response styles. Results: The unadjusted mean SJT scores for P1 and P4 student pharmacists were 79.7 % and 80.7 %, respectively. The adjusted scores for P1 and P4 student pharmacists were 86.3 % and 87.2 %, respectively. Small differences were observed across institutions which can likely explained by multiple factors. The mean scores were higher among students who were older, female, identified as White, and spoke English as their first language. The subgroup differences persisted even after adjusting scores for elevation and scatter. Conclusion: Performance on an SJT designed to assess empathy, integrity, and teamwork was similar to others in the literature; however, differences in performance were observed across subgroups. Additional research is needed to determine whether these differences reflect test bias or other possible factors.

Abstract:Problem: Professionalism is a critical attribute of medical graduates. Its measurement is challenging. The authors sought to assess final-year medical students' knowledge of appropriate professional behavior across a broad range of workplace situations. Approach: Situational judgement tests (SJTs) are used widely in applicant selection to assess judgement or decision making in work-related settings as well as attributes such as empathy, integrity, and resilience. In 2014, the authors developed three 40-item SJTs with scenarios relevant to interns (first-year junior doctors) and delivered the tests to final-year medical students to assess aspects of professionalism. As preparation, students discussed SJT-style scenarios; after the tests they completed an evaluation. The authors applied the Angoff method for the standard-setting process, delivered electronic individualized feedback reports to students post test, and provided remediation for students failing to meet the cut score. Outcomes: Evaluation revealed that the tests positively affected students' learning and that students accepted them as an assessment tool. Validity and reliability were acceptable. Implementation costs were initially high but will be recouped over time. Next steps: Recent improvements include changes to pass requirements, question revision based on reliability testing, and provision of detailed item-level feedback. Work is currently under way to expand the item bank and to introduce tests earlier in the course. Future research will explore correlation of SJT performance with other measures of professionalism and focus on the impact of SJTs on professionalism and interns' ability to deal with challenging workplace situations.
Predicting undergraduate OSCE performance using traditional and construct-driven situational judgment tests at admission

Abstract:Situational Judgement Tests (SJTs) are popular to screen for social skills during undergraduate medical admission as they have been shown to predict relevant study outcomes. Two different types of SJTs can be distinguished: Traditional SJTs, which measure general effective behavior, and construct-driven SJTs which are designed to measure specific constructs. To date, there has been no comparison of the predictive validity of these two types of SJTs in medical admission. With the present research, we examine whether the HAM-SJT, a traditional SJT, and the CD-SJT, a construct-driven SJT with an agentic and a communal scale, administered during undergraduate medical admission can predict OSCE (i.e., objective structured clinical examination) performance in a low-stakes (nLS = 159) and a high-stakes (nHS = 160) sample of medical students. Results showed a moderate positive relation between the communal scale of the CD-SJT and performance in OSCE stations with trained patients in the high-stakes sample (r =.20, p =.009). This SJT had also an incremental value in predicting the OSCE performance above and beyond GPA (i.e., grade point average), a science test (i.e., HAM-Nat), and gender (ß = 0.18, 95% CI [0.03; 0.33], p =.020). That is, individuals who chose more communal behavioral responses in the SJT were rated more favorably in interactions with trained patients in the OSCE. A comparable correlation coefficient was observed for the HAM-SJT when controlling for range restriction due to admission (rraw = 0.14 vs. rcontrolled = 0.20). Our research provides a first indication for the predictive validity of construct-driven SJTs in high-stakes undergraduate medical admission.

Abstract:Objectives: Situational Judgement Tests (SJT) are a cost-efficient method for the assessment of personal characteristics (e.g., empathy, professionalism, ethical thinking) in medical school admission. Recently, complex open-ended response format SJTs have become more feasible to conduct. However, research on their applicability to a German context is missing. This pilot study tests the acceptability, reliability, subgroup differences, and validity of an online SJT with open-ended response format developed in Canada ("Casper"). Methods: German medical school applicants and students from Hamburg were invited to take Casper in 2020 and 2021. The test consisted of 12 video- and text-based scenarios, each followed by three open-ended questions. Participants subsequently evaluated their test experience in an online survey. Data on sociodemographic characteristics, other admission criteria (Abitur, TMS, HAM-Nat, HAM-SJT) and study success (OSCE) was available in a central research database (stav). Results: The full sample consisted of 582 participants. Test-takers' global perception of Casper was positive. Internal consistency was satisfactory in both years (α=0.73; 0.82) while interrater agreement was moderate (ICC(1,2)=0.54). Participants who were female (d=0.37) or did not have a migration background (d=0.40) received higher scores. Casper scores correlated with HAM-SJT (r=.18) but not with OSCE communication stations performance. The test was also related to Abitur grades (r=-.15), the TMS (r=.18), and HAM-Nat logical reasoning scores (r=.23). Conclusion: This study provides positive evidence for the acceptability, internal consistency, and convergent validity of Casper. The selection and training of raters as well as the scenario content require further observation and adjustments to a German context to improve interrater reliability and predictive validity.
Use of situational judgment tests for assessing non-cognitive attributes of final year dental students

Abstract:Objective: Situational judgment tests (SJT) have been previously used in the admission process of medical and dental undergraduate programs, but their utilization as an assessment tool is fairly new. The objective of this study was to explore essential non-cognitive attributes of dental graduates. We also aimed to assess the attributes of final year dental students utilizing SJT and later collected participants' reactions on the test. Methods: A mixed-method exploratory sequential study was conducted. One-on-one and group discussions were done with subject matter experts (SMEs) to explore the essential attributes of fresh dental graduates. Based on the thematic analysis, 10 SJT items per domain were constructed by one group of SMEs and expert validated by a different group of SMEs and vice versa. The final dental-SJT was piloted online on final year dental students and their perceptions were collected using a five-point Likert scale questionnaire. Descriptive statistics and Cronbach's alpha were calculated for the test. Results: The consultation with SMEs generated three main themes, namely, Conventional skills, Tribal skills, and Occupational skills. The 70-item dental-SJT piloted on 150 final year dental students revealed data were normally distributed. The internal reliability of test was good (0.738). Students scored highest in "management skills" (>70%) and lowest in "professional ethics" (56%) and "diverse experiences" (58%). The overall student perception about the test was good. Conclusion: SJT is an effective tool to assess non-cognitive attributes of dental students. It has an overall positive perception about perceived predictive validity, perceived fairness, face and content validity.
Measurement of Non-academic Attributes in the Situational Judgment Test as Part of School Teacher Selection: Systematic Literature Review

Abstract:The Situational Judgment Test (SJT) has become an increasingly well-known measurement method that is frequently used in various fields, especially for personnel selection, promotion, and professional development. The SJT also has a greater potential in relation to the selection of teachers in the education sector. Teacher selection aspects such as non-academic attributes, specifically the interpersonal and intrapersonal, are the focus of this study. This study uses a Systematic Literature Review (SLR) consisting of a review protocol to determine the topic that has been researched and the method used for making revisions. The SLR approach has four phases: searching, screening, analysing, and the results with numerous criteria established. The review consists of seven studies on teacher selection using the SJT instrument over the past ten years during the period January 2012 to December 2021. Eight interpersonal attributes were obtained based on six previous studies, namely organisation, planning, empathy, communication, teaching, relationships with colleagues, counselling, and contingency. Meanwhile seven intrapersonal attributes were obtained, namely conscientiousness, mindset, emotion regulation, adaptability, enthusiasm and motivation, resilience, and professional ethics. The attributes found can be utilised as they are fundamental for teacher selection criteria. The previous studies employed the same attributes. However, the attributes should only be employed according to the suitability of the selection context being performed.
A generalizability study of the medical judgment vignettes interview to assess students' noncognitive attributes for medical school

Abstract:BackgroundAlthough the reliability of admission interviews has been improved through the use of objective and structured approaches, there still remains the issue of identifying and measuring relevant attributes or noncognitive domains of interest. In this present study, we use generalizability theory to determine the estimated variance associated with participants, judges and stations from a semi-structured, Medical Judgment Vignettes interview used as part of an initiative to improve the reliability and content validity of the interview process used in the selection of students for medical school.MethodsA three station, Medical Judgment Vignettes interview was conducted with 29 participants and scored independently by two judges on a well-defined 5-point rubric. Generalizability Theory provides a method for estimating the variability of a number of facets. In the present study each judge (j) rated each participant (p) on all three Medical Judgment Vignette stations (s). A two-facet crossed designed generalizability study was used to determine the optimal number of stations and judges to achieve a 0.80 reliability coefficient.ResultsThe results of the generalizability analysis showed that a three station, two judge Medical Judgment Vignettes interview results in a G coefficient of 0.70. As shown by the adjusted Eρ2 scores, since interviewer variability is negligible, increasing the number of judges from two to three does not improve the generalizability coefficient. Increasing the number of stations, however, does have a substantial influence on the overall dependability of this measurement. In a decision study analysis, increasing the number of stations to six with a single judge at each station results in a G coefficient of 0.81.ConclusionThe Medical Judgment Vignettes interview provides a reliable approach to the assessment of candidates' noncognitive attributes for medical school. The high inter-rater reliability is attributed to the greater objectivity achieved through the used of the semi-structured interview format and clearly defined scoring rubric created for each of the judgment vignettes. Despite the relatively high generalizability coefficient obtained for only three stations, future research should further explore the reliability, and equally importantly, the validity of the vignettes with a large group of candidates applying for medical school.

Abstract:Introduction: There is a paramount need for moral development for pharmacists and pharmacy students to practice the patient-centered profession. We aimed to explore the current situational judgment utilizing ethical reasoning among undergraduate pharmacy students. Methods: A set of ten ethical dilemmas, representing potential real-life situations that the students come across in the university and may face in the future as a pharmacist were developed by a team of students, academic staff, and stakeholders. These ethical dilemmas were validated, checked for accuracy, and piloted. An online questionnaire was created consisting of these ten scenarios as open-ended questions and administered to fourth year and fifth year PharmD students in a public university located at the city of Mecca, Saudi Arabia, asking them how they would react in that situation. Responses of the participants were analyzed using thematic analysis independently by four researchers and inter-rater agreement were achieved through consensus. Results: Out of 205, 186 students completed the questionnaire with a response rate of 90.7%. Analysis and resulted in the generation of 32 codes, which were then categorized into seven overarching themes: student engagement, social and professional responsibility, academic integrity, legal obligation, moral obligation, signposting, and moral engagement and patient safety. Conclusions: Undergraduate pharmacy students experience complex state of mind in connection with ethical reasoning. The participants' situational judgment were driven by cultural norm, authority, and responsibility. Student engagement is also affected by the state of mind and feelings of mutual trust, perceived cultural influence and peer pressure. The students were prone to seek help from university administrators or teachers when faced with situations in which they were helpless.
The Relationship of Epinephrine Oxidation to Rat Parotid Metabolism and Secretion in Vitro 1

Abstract:Summary Epinephrine conversion to ad-renochrome by rat parotid in vitro was verified by the appearance of a pink color in the medium and an increase in UV absorbance. Preincubation with epinephrine to increase adrenochrome concentration during incubation produced no alteration in the magnitude of catecholamine stimulation of glucose-6-14C oxidation. Inhibition of epinephrine conversion to adrenochrome or of degradation by monamine oxidase also had no effect on the response to this catecholamine. Although epinephrine produced pronounced stimulation of α-amylase secretion, glucose-6-14C oxidation, leucine-U-14C oxidation, and inhibition of leucine-U-14C incorporation into rat parotid protein, no measurable effects were produced by adrenochrome. The present results strongly indicate that epinephrine rather than adrenochrome or other oxidation products is primarily responsible for the alterations in α-amylase secretion and metabolism observed with rat parotid in vitro.

Abstract:The "Dental students training to address the needs of each individual patient: enhancement of ability to make a comprehensive diagnosis and treatment plans with high ethical standards and good communication skills", project launched at Tokyo Dental College was adopted by the Ministry of Education, Culture, Sports, Science and Technology as part of its "Program for Promoting University Education and Student Support, Theme A: Program for Promoting University Education Reform" in 2009. One of the main goals of this subject is "the establishment of Patient Community". Patient Community members allowed students to gain a more realistic experience of clinical practice than simulated patients. The Patient Community consists of patients and members of the parents' association who have agreed to cooperate for the advancement of dental education, becoming involved in dental student education through Communication Studies, which are held for first- to fourth-year students. Patient Community members were recruited at the open lectures (15 times, between July 10, 2010 and November 30, 2011). The Patient Community comprised 24 members, including 8 men and 16 women by November 30, 2011. The cumulative number of attendees in Communication Studies (I-IV, 6 times) was 35, including 13 men and 22 women. Fourteen people applied for admission on the day of the open lecture. Seven people signed up between 1 and 7 days after the open lecture. On the other hand, only 3 people applied within 8 to 9 days after the open lecture. However, interestingly, the ratio of the attendance for Communication Studies by Patient Community members who applied 8 to 9 days after the open lecture was higher than that of members who applied for admission on the day of the open lecture. Since the number of Patient Community members is insufficient for the purposes of the Patient Community, it is necessary to think about how recruitment methods can be made more effective and how such open lectures should be conducted in future.
Improving the effectiveness of embedded behavioral health personnel through situational judgment training

Abstract:Embedding behavioral health (EBH) personnel into operational units has emerged as a major trend within the US military. These positions require skillsets in addition to those needed in a clinic setting. Little or no empirically based training has yet been developed to ensure preparedness to serve in EBH roles. Based on the demonstrated effectiveness of situational judgment tests (SJT) in training for other domains, we developed and evaluated 60 SJT scenarios for use in the US Air Force's formal EBH training course. SJTs were based on real-world experiences of seasoned EBH professionals, and were used for structured, scenario-based discussions during training as well as a 30-item pre- and posttest to assess learning. Use during training was associated with significantly improved scores in six EBH competencies (ethical decision-making, leadership consultation, balancing professional relationships, unit engagement, triage to correct services, and teaming). In a follow-up survey, students reported scenario-based discussions were very helpful, and reported improved abilities and confidence. This study demonstrates the potential utility of the SJT methodology to prepare mental health professionals to serve in EBH positions. Further development and possible applications are discussed, such as expansion to other training venues, job candidate screening, and assessment for readiness for EBH practice.

Abstract:The article describes the development and testing of the SJT-ICC situational judgment test designed to measure intercultural competence of a teacher in the form of behavioral preferences in the professional field of interaction with students and their parents. This approach makes it possible to assess behavioral aspects of intercultural competence in a particular professional area more effectively than survey methods. The SJT-ICC consists of 18 cases from pedagogical practice and four possible behavioral options for the teachers, from which the subject must choose the most appropriate one. These situations were prepared on the basis of qualitative research in the format of in-depth interviews centered around difficult, critical situations that a teacher encounters when working with children and par¬ents from other cultures (n=53). The assessment of the intercultural competence of the answer options was made on the basis of an expert survey of 23 special-ists.1367 teachers took part in the research to validate and check the consisten¬cy of the SJT-ICC. The outcome of the categorical principal component analysis (CatPCA) was a one-scale structure of the SJT-ICC which was confirmed by the confirmatory factor analysis. The relationship between the results of the SJT-ICC and the scales used for validation is predictable: intercultural competence is associated with indicators of professional success of a teacher working with students of other cultures. It is concluded that the SJT-ICC can be effectively used to assess the prerequisites for intercultural success among Russian teachers.
Measuring Teachers’ Social-Emotional Competence: Development and Validation of a Situational Judgment Test

Abstract:Teachers' social-emotional competence is considered important in order to master the social and emotional challenges inherent in their profession and to build positive teacher-student relationships. In turn, this is key to both teachers' occupational well-being and positive student development. Nonetheless, an instrument assessing the profession-specific knowledge and skills that teachers need to master the social and emotional demands in the classroom is still lacking. Therefore, we developed the Test of Regulation in and Understanding of Social Situations in Teaching (TRUST), which is a theory-based situational judgment test measuring teachers' knowledge about strategies for emotion regulation and relationship management in emotionally and socially challenging situations with students. Results from three studies (N = 166 in-service teachers, N = 73 in-service teachers, N = 107 pre-service teachers) showed satisfactory internal consistency for both the emotion regulation and relationship management subtests. Furthermore, confirmatory factor analyses supported the differentiation between the two facets of social-emotional competence. Regarding convergent validity, results from Study 3 revealed a positive association between the profession-specific TRUST and pre-service teachers' general emotional intelligence. Furthermore, small to moderate correlations with the Big Five personality traits provided evidence for the discriminant validity of TRUST. In Studies 1 and 2, we found evidence for a correlation with external criteria, that is, teachers with higher test scores reported providing more emotional support for students and having better teacher-student relationships. For teachers' occupational well-being, we found a link with symptoms of depersonalization and job satisfaction, but none for emotional exhaustion. We will discuss the use of TRUST in research, for the evaluation of interventions, in teacher education, and professional development and will illustrate ideas for enhancing the tool.

Utility of the Jefferson teamwork observation guide for measuring collaborative practice competencies virtually, in-person, and across health professions: A Rasch Analysis

Abstract:Modern healthcare increasingly requires interprofessional teams to collaborate both in person and virtually to effectively achieve common goals. To prepare students for interprofessional collaborative practice (CP) universities need evaluation tools that can validly and reliably measure students' CP competencies after online and in-person interprofessional education. The Jefferson Teamwork Observation Guide® (JTOG) is a 360-degree evaluation tool previously validated to measure nationally-defined CP competencies. The psychometrics of the Individual JTOG have been examined in a sample of interprofessional healthcare students after online interprofessional education. The present study examined the psychometric properties of the Individual JTOG in 709 students after in-person interprofessional education using Rasch Modeling and compared results across collaborative settings and student professions. Results indicated that item and person statistics, unidimensionality, scaling performance, and local independence of the Individual JTOG were comparable between online and in-person samples, suggesting it is consistent in its measurement of CP competencies across collaborative settings. Psychometric properties were strong, but ceiling effects were present. Minor deviations were found in the Individual JTOG's unidimensionality between professional groups. The Values and Ethics construct was more strongly separated from others for nursing than other health professions. Recommendations for future research and possible adaptations to the instrument are discussed.

Abstract:Objective: The medical profession seeks to hire and train individuals who consistently meet and/or exceed both job and cultural expectations. Resident selection is often not structured to meet this goal. The objective of this quality improvement project was to evaluate a classic unscripted interview process (OI) in conjunction with a structured, scripted interview process (SI) developed using an established hiring methodology from industry not yet utilized in health care. Qualitative questions we sought to answer: (1) Can SI be practically applied to the selection of residents? (2) Is there a significant difference in the relative position of applicants between the OI and SI rank lists? (3) Qualitatively, does SI help the evaluation/discussion of the affective domain? Methods: Design: Prospective qualitative comparison of OI versus SI. Setting: Dartmouth Hitchcock Medical Center, Lebanon, NH. Participants: Applicants were assessed by OI and SI. SI factors were selected based on a job profile. Interview scripts were created from validated behavioral and attitudinal questions. Online assessments assessed 2 important attributes - adaptability and values. Rank lists were compared for relative rank position of applicants. Feedback from faculty was obtained. Results: Fifty-two applicants. Critical attributes were self-management, integrator-synthesizer, versatility, communication, and achievement. Absolute mean difference in rank/applicant was 9.8 (standard deviation 8.9, Range 0-36) positions. Comparing the top 20 candidates of each rank list, 40% of those applicants were only on one list. Faculty felt that applicants were given a greater opportunity to show "who they are." Conclusions: In conjunction with OI, an industry proven methodology was practically applied to define and select for high performance for the authors' specific institution. Comparing OI and SI resulted in substantial differences in rank lists. This initiative seemed to provide a structure to evaluate values and motivations that are inherently difficult to assess. Faculty felt SI in conjunction with OI gave a greater chance for applicants to show "who they are."
Effect of ketamine on thermoregulation in rats.

Abstract:Intraperitoneal administration of ketamine produced dose-dependent hypothermia at the ambient temperatures (Ta) of both 8 and 23 degrees C in unanesthetized rats. At a Ta of 8 degrees C, the hypothermia was brought about solely by a decrease in metabolic heat production. There were no changes in either the tail skin temperature (Ttail) or the sole skin temperature (Tsole). At a Ta of 23 degrees C, the hypothermia was due to an increase in Ttail, and increase in Tsole, and a decrease in metabolic heat production. However, at a Ta of 31 degrees C, there were no changes in rectal temperature in response to ketamine application, since neither heat production nor skin temperatures (e.g., Ttail and Tsole) was affected by ketamine at this Ta. The data indicate that the effect of the drug treatment may be to decrease heat production and (or) increase heat loss.
