Abstract:Gait modifications are effective in reducing the first peak knee abduction moment (PKAM), a surrogate for knee loading. Reliance on 3D motion capture currently restricts these modifications to the laboratory. Therefore, our purpose was to test the feasibility of a novel wearable biofeedback system to train (1) toe-in and trunk lean modifications and (2) combined toe-in and trunk lean modifications to reduce PKAM during overground walking outside of the laboratory. Twelve healthy participants practiced modifications in a university hallway directly after performing five normal walking trials. The wearable feedback system provided real-time haptic biofeedback during training trials to inform participants if they were within the prescribed modification range (7-12deg greater than baseline). Participants were instructed to move to the next modification only once they felt comfortable and could perform it with minimal errors. Following training, five trials of each modification were immediately performed in the gait laboratory without feedback. All participants successfully modified their foot progression and trunk angle using the wearable system. At post-test, PKAM decreased from baseline by 62%, 55%, and 28% during combined, trunk leanand toe-in gait, respectively. The wearable feedback system was effective to modify participants' foot and trunk angle by the prescribed amount, resulting in reduced PKAM during all modifications at post-test. Participants were also able to perform a combined modification, although it took longer to report feeling comfortable doing so. This study demonstrates that a wearable feedback system is feasible to modify kinematic parameters and train gait modifications outside the laboratory.
Stair ascent and descent biomechanical adaptations while using a custom ankle-foot orthosis

Abstract:The ability to navigate stairs step-over-step is an important functional outcome following severe lower leg injury and is difficult for many patients. Ankle-foot orthoses, such as the Intrepid Dynamic Exoskeletal Orthosis (IDEO), are often prescribed to improve function. This study compared stair climbing mechanics between IDEO users and able-bodied control participants. Thirteen IDEO users who sustained severe lower leg injury and 13 controls underwent biomechanical gait analysis. Participants ascended and descended a 16-step instrumented staircase without handrail use at a controlled cadence of 80 steps/min. Peak joint angles, moments, powers, and ground reaction forces, and integrated mechanical work were calculated. Independent t-tests with Bonferroni-Holm corrections were used to compare controls to IDEO and sound limbs. Reduced ankle range of motion on the IDEO limb resulted in compensatory strategies while ascending or descending stairs. During ascent, IDEO users had greater bilateral hip power during pull-up (p<0.007) to compensate for the IDEO limb׳s reduced ankle dorsiflexion (p<0.001) and knee extensor moment (p=0.001) while it was leading, and reduced ankle plantarflexor power while it was trailing (p<0.001). During stair descent, when the IDEO limb had was trailing, it had less ankle dorsiflexion during controlled lowering (p<0.001), resulting in greater vertical ground reaction force (p=0.005) and greater ankle and knee power absorption (p<0.001). Reduced IDEO limb ankle power absorption during weight acceptance (p<0.001) resulted in a large knee extensor moment (p<0.001) on the trailing sound limb to lower the body. Despite gait deviations, IDEO users were able to climb stairs step-over-step unassisted.
Angular Kinematics at Minimum Toe Clearance in People With Transtibial Amputation Using Articulated and Nonarticulated Prosthesis

Abstract:Subjects with unilateral transtibial amputation exhibit altered minimum toe clearance (MTC) depending on the ankle prosthesis used. It has been suggested that a limited prosthetic ankle angle could be the cause of the change. The aim of this study was to investigate the alterations in kinematics in the joints responsible for the changes in MTC when using an articulating hydraulic ankle (AHA) prosthesis compared to a nonarticulating ankle (NAA) prosthesis. Twelve participants with unilateral transtibial amputation walked at their self-selected speed on a 10 m pathway. They used both the same AHA and NAA prosthetic models and the prosthetic characteristics were unchanged except for the ankle mechanisms and, consequently, its mass. Data from MTC and hip, knee, and ankle angles in the sagittal, frontal, and transversal plane at the time of MTC were statistically analyzed with a paired sample t-test. The AHA prosthesis showed significantly higher MTC mean (AHA=24.7 ± 9.6 mm versus NAA=17.4 ± 5.2 mm, P<0.01) and variability (13.4 ± 9.6 mm versus 6.7 ± 4.2 mm, P=0.03) on the prosthetic limb than the NAA. A higher mean MTC could be explained by an increase in ankle angle dorsiflexion (AHA=-1.2 ± 2.6 deg versus NAA=-2.9 ± 1.5 deg, P=0.01), while the variability of the prosthetic MTC appears to be influenced by changes in prosthetic mass. The results of this study suggest that ankle dorsiflexion during swing and the mass of the prosthesis have a direct influence in mean MTC and its variability, respectively.
Effects of toe stiffness on ankle kinetics in a robotic transtibial prosthesis during level-ground walking

Abstract:Robotic transtibial prostheses are gaining popularity, as they significantly improve amputees’ locomotion. Existing studies mainly concentrate on the importance of ankle joints in these prostheses. However, there are few studies that focus on the effects of prosthetic toe joints. Different from human feet, most prosthetic feet are single-segment structures without toe joints. In order to develop more human-like prostheses with ankle and toe joints, it is of great importance to investigate the effects of toe stiffness on ankle kinetics, as it contributes to the design and control optimizations. In this paper, we analyze the effects of toe stiffness on ankle kinetics in a robotic transtibial prosthesis we have built recently. Dynamic models of the prosthesis’ ankle and toe joints are built. The imposed plantar pressures and objective joint angles of these models are obtained from locomotion experiments on able-bodied subjects. Simulation results indicate when the toe joint is fixed, the ankle energy consumption in one gait cycle is almost 1.2 times of that when the toe joint is driven to track the able-bodied toe motion. Experiments of the prosthesis prototype are carried out on an amputee subject with a transtibial amputation. The results show that compared with the fixed toe joint either the active toe joint or the passive toe joint can reduce the ankle output moment and power consumption. This means adding a toe joint to the foot can reduce the designing requirements of the ankle joint. In addition, the active toe joint can help the prosthesis perform more human-like behaviors compared with the passive toe joint. On the other hand, the passive toe joint can simplify the prosthesis structures and reduce the total weight compared with the active toe joint.
Variation in acclimation of photosynthesis in Trifolium repens after eight years of exposure to Free Air CO2 Enrichment (FACE).

Abstract:The initial stimulation of photosynthesis observed on elevation of [CO2] in grasslands has been predicted to be a transient phenomenon constrained by the loss of photosynthetic capacity due to other limitations, notably nutrients and sinks for carbohydrates. Legumes might be expected partially to escape these feedbacks through symbiotic N2 fixation. The Free-Air Carbon dioxide Enrichment (FACE) experiment at Eschikon, Switzerland, has been the longest running investigation of the effects of open-air elevation of [CO2] on vegetation. The prediction of a long-term loss of photosynthetic capacity was tested by analysing photosynthesis in Trifolium repens L. (cv. Milkanova) in the spring and autumn of the eighth, ninth and tenth years of treatment. A high and low N treatment also allowed a test of the significance of exogenous N-supply in maintaining a stimulation of photosynthetic capacity in the long-term. Prior work in this Free Air CO2 Enrichment (FACE) experiment has revealed that elevated [CO2] increased both vegetative and reproductive growth of T. repens independent of N treatment. It is shown here that the photosynthetic response of T. repens was also independent of N fertilization under both current ambient and elevated (600 micro mol mol-1) [CO2]. There was a strong effect of season on photosynthesis, with light-saturated rates (Asat) 37% higher in spring than in autumn. Higher Asat in the spring was supported by higher maximum Rubisco carboxylation rates (Vc,max) and maximum rates of electron transport (Jmax) contributing to RuBP regeneration. Elevated [CO2] increased Asat by 37% when averaged across all measurement periods and both N fertilization levels, and decreased stomatal conductance by 25%. In spring, there was no effect of elevated [CO2] on photosynthetic capacity of leaves, but in autumn both Vc,max and Jmax were reduced by approximately 20% in elevated [CO2]. The results show that acclimation of photosynthetic capacity can occur in a nitrogen-fixing species, in the field where there are no artificial restrictions on sink capacity. However, even with acclimation there was a highly significant increase in photosynthesis at elevated [CO2].
Lower limb joint biomechanics-based identification of gait transitions in between level walking and stair ambulation

Abstract:Lower limb exoskeletons and lower limb prostheses have the potential to reduce gait limitations during stair ambulation. To develop robotic assistance devices, the biomechanics of stair ambulation and the required transitions to level walking have to be understood. This study aimed to identify the timing of these transitions, to determine if transition phases exist and how long they last, and to investigate if there exists a joint-related order and timing for the start and end of the transitions. Therefore, this study analyzed the kinematics and kinetics of both transitions between level walking and stair ascent, and between level walking and stair descent (12 subjects, 25.4 yrs, 74.6 kg). We found that transitions primarily start within the stance phase and end within the swing phase. Transition phases exist for each limb, all joints (hip, knee, ankle), and types of transitions. They have a mean duration of half of one stride and they do not last longer than one stride. The duration of the transition phase for all joints of a single limb in aggregate is less than 35% of one stride in all but one case. The distal joints initialize stair ascent, while the proximal joints primarily initialize the stair descent transitions. In general, the distal joints complete the transitions first. We believe that energy- and balance-related processes are responsible for the joint-specific transition timing. Regarding the existence of a transition phase for all joints and transitions, we believe that lower limb exoskeleton or prosthetic control concepts should account for these transitions in order to improve the smoothness of the transition and to thus increase the user comfort, safety, and user experience. Our gait data and the identified transition timings can provide a reference for the design and the performance of stair ambulation- related control concepts.

Comparing the lower limb joint biomechanics of the Power Knee, C-Leg and Rheo Knee during ramp and stair ambulation

Abstract:One of the most significant developments in prosthetic knee technology has been the introduction of the Microprocessor-Controlled Prosthetic Knee (MPK). However, there is a lack of consensus over how different types of MPKs affect performance in different ambulation modes. In this study, we investigated the biomechanical differences in ramp and stair maneuvers when an individual with transfemoral amputation wears three commercial MPKs: the Össur Power Knee, the Össur Rheo Knee and the Ottobock C-Leg 4. The primary outcome variable for this study was the lower limb biological joint work, inclusive of the intact leg and prosthetic side hip. We hypothesized that (1) the Power Knee would result in lower biological work during ascent activities than the C-Leg and Rheo, both passive MPKs, and (2) the C-Leg and Rheo would result in lower biological work during descent activities than the Power Knee. During ramp ascent, the C-Leg was associated with lower biological joint work (p < 0.05) than the Power Knee. However, this relationship did not hold during stair ascent, where the Power Knee showed advantages for stair ascent with net reductions in biological joint work of 14.1% and 23.3% compared to the Rheo and C-leg, respectively. There were no significant differences in biological joint work between the knees during ramp and stair descent, indicating that choice of MPK may not be as important for descent activities. Our results demonstrate that differences are present between different types of MPKs during ascent activities which could prove useful in the prescription of these devices.
The biomechanical influence of transtibial Bone-Anchored limbs during walking

Abstract:Individuals with unilateral transtibial amputation (TTA) using socket prostheses demonstrate asymmetric joint biomechanics during walking, which increases the risk of secondary comorbidities (e.g., low back pain (LBP), osteoarthritis (OA)). Bone-anchored limbs are an alternative to socket prostheses, yet it remains unknown how they influence multi-joint loading. Our objective was to determine the influence of bone-anchored limb use on multi-joint biomechanics during walking. Motion capture data (kinematics, ground reaction forces) were collected during overground walking from ten participants with unilateral TTA prior to (using socket prostheses) and 12-months after bone-anchored limb implantation. Within this year, each participant completed a rehabilitation protocol that guided progression of loading based on patient pain response and optimized biomechanics. Musculoskeletal models were developed at each testing timepoint (baseline or 12-months after implantation) and used to calculate joint kinematics, internal joint moments, and joint reaction forces (JRFs). Analyses were performed during three stance periods on each limb. The between-limb normalized symmetry index (NSI) was calculated for joint moments and JRF impulses. Discrete (range of motion (ROM), impulse NSI) dependent variables were compared before and after implantation using paired t -tests with Bonferroni-Holm corrections while continuous (ensemble averages of kinematics, moments, JRFs) were compared using statistical parametric mapping ( p < 0.05). When using a bone-anchored limb, frontal plane pelvic (residual: pre = 9.6 ± 3.3°, post = 6.3 ± 2.5°, p = 0.004; intact: pre = 10.2 ± 3.9°, post = 7.9 ± 2.6°, p = 0.006) and lumbar (residual: pre = 15.9 ± 7.0°, post = 10.6 ± 2.5°, p = 0.024, intact: pre = 17.1 ± 7.0°, post = 11.4 ± 2.8°, p = 0.014) ROM was reduced compared to socket prosthesis use. The intact limb hip extension moment impulse increased (pre = -11.0 ± 3.6 Nm*s/kg, post = -16.5 ± 4.4 Nm*s/kg, p = 0.005) and sagittal plane hip moment impulse symmetry improved (flexion: pre = 23.1 ± 16.0 %, post = -3.9 ± 19.5 %, p = 0.004, extension: pre = 29.2 ± 20.3 %, post = 8.7 ± 22.9 %, p = 0.049). Residual limb knee extension moment impulse decreased compared to baseline (pre = 15.7 ± 10.8 Nm*s/kg, post = 7.8 ± 3.9 Nm*s/kg, p = 0.030). These results indicate that bone-anchored limb implantation alters multi-joint biomechanics, which may impact LBP or OA risk factors in the TTA population longitudinally.

Abstract:Background Prosthetic foot stiffness, which is typically invariable for commercially available prosthetic feet, needs to be considered when prescribing a prosthetic foot. While a biological foot adapts its function according to the movement task, an individual with lower limb amputation may be limited during more functionally demanding gait tasks by their conventional energy storing and return prosthetic foot. Research Question How do changes in prosthetic foot stiffness during incline walking affect biomechanical measures as well as perception of participants. Methods Kinetic and kinematic data were collected during incline walking, for five participants with trans-tibial amputation. A mixed model analysis of variance was used to analyse the effects of changing the stiffness during incline walking, using a novel variable-stiffness unit built on a commercially available prosthetic foot. Biomechanical results were also analysed on an individual level alongside the participant feedback, for a better understanding of the various strategies and perceptions exhibited during incline walking. Results Statistically significant effects were only observed on the biomechanical parameters directly related to prosthetic ankle kinematics and kinetics (i.e., peak prosthetic ankle dorsiflexion, peak prosthetic ankle power, dynamic joint stiffness during controlled dorsiflexion). Participant perception during walking was affected by changes in stiffness. Individual analyses revealed varied perceptions and varied biomechanical responses among participants. Significance While changes in prosthesis mechanical properties influenced the amputee's experience, minimal immediate effects were found with the overall gait pattern. The reported inter-participant variability may be due to the person's physical characteristics or habitual gait pattern, which may influence prosthesis function. The ability to vary prosthetic foot stiffness during the assessment phase of setting up a prosthesis could provide useful information to guide selection of the appropriate prosthetic device for acceptable performance across a range of activities.

The effect of foot orientation modifications on knee joint biomechanics during daily activities in people with and without knee osteoarthritis

Abstract:Background: Altered gait could influence knee joint moment magnitudes and cumulative damage over time. Gait modifications have been shown to reduce knee loading in people with knee osteoarthritis during walking, although this has not been explored in multiple daily activities. Therefore, this study investigated the effect of different foot orientations on knee loading during multiple daily activities in people with and without knee osteoarthritis. Methods: Thirty people with knee osteoarthritis and twenty-nine without (control) performed walking, stair ambulation and sit-to-stand across a range of foot progression angles (neutral, toe-in, toe-out and preferred). Peak knee adduction moment, knee adduction moment impulse and knee pain were compared across a continuous range of foot orientations, between activities, and groups. Findings: Increased foot progression angle (more toe-in) reduced 1st peak knee adduction moment across all activities in both knee osteoarthritis and control (P < 0.001). There was a greater reduction in knee adduction moment in the control group during walking and stair ambulation (P ≤ 0.006), where the knee osteoarthritis group already walked preferably less toe-out than the control group. Under preferred condition, stair descent had the greatest knee loading and knee pain compared to other activities. Interpretation: Although increased foot progression angle (toward toe-in) appeared to be more effective in reducing knee loading for all activities, toe-in modification might not benefit stair ambulation. Future gait modification should likely be personalised to each patient considering the individual difference in preferred gait and knee alignment required to shift the loading medially or laterally.
There are unique kinematics during locomotor transitions between level ground and stair ambulation that persist with increasing stair grade

Abstract:Human ambulation is typically characterized during steady-state isolated tasks (e.g., walking, running, stair ambulation). However, general human locomotion comprises continuous adaptation to the varied terrains encountered during activities of daily life. To fill an important gap in knowledge that may lead to improved therapeutic and device interventions for mobility-impaired individuals, it is vital to identify how the mechanics of individuals change as they transition between different ambulatory tasks, and as they encounter terrains of differing severity. In this work, we study lower-limb joint kinematics during the transitions between level walking and stair ascent and descent over a range of stair inclination angles. Using statistical parametric mapping, we identify where and when the kinematics of transitions are unique from the adjacent steady-state tasks. Results show unique transition kinematics primarily in the swing phase, which are sensitive to stair inclination. We also train Gaussian process regression models for each joint to predict joint angles given the gait phase, stair inclination, and ambulation context (transition type, ascent/descent), demonstrating a mathematical modeling approach that successfully incorporates terrain transitions and severity. The results of this work further our understanding of transitory human biomechanics and motivate the incorporation of transition-specific control models into mobility-assistive technology.
The effects of slope-adaptive prosthetic feet on sloped gait performance and quality in unilateral transtibial prosthesis users: A scoping review

Abstract:Introduction: In non-impaired human locomotion, sagittal-plane slope adaptation of the foot-ankle complex is a volitional function driven by neuromotor control to support upright posture and forward ambulation. Loss of this adaptation due to transtibial amputation can lead to instability and compensatory motions as most commercially-available prosthetic feet do not permit automatic slope adjustments. A selection of slope-adaptive feet (SAF) have been developed to promote biomimetic ankle motion while ambulating over slopes. This review evaluated the current literature to assess the effects of SAF prostheses on sloped gait performance in unilateral transtibial prosthesis users. Methods: Four databases (PubMed, Embase, CINAHL, IEEE Xplore) were searched on April 28, 2022, for relevant articles. Search keywords covered the general terms "transtibial," "amputation," "slope," "adaptive," and "gait", and included articles comparing a SAF prosthesis to a non-SAF prosthesis condition. Data were extracted for analysis and results were grouped according to outcomes to identify trends and aid interpretation of slope adaptation effects on gait. Results: Of the 672 articles screened, 24 met the selection criteria and were included in this review, published between 2009 and 2022. The non-SAF condition included dynamic response feet and SAF prostheses with the adaptability function inactive. Outcomes included biomechanical variables (joint dynamics, gait symmetry, toe clearance), clinical outcome measures, and energy expenditure. All SAF demonstrated some form of foot-ankle slope gradient adaptability, but effects on other joint dynamics were inconsistent. Minimum toe clearance during incline and decline walking was greater when using SAF compared to non-SAF in all reporting studies. Conclusions: Results generally suggest improvements in gait quality, comfort, and safety with use of SAF compared to non-SAF during slope walking. However, variations in tested SAF and walking gradients across studies highlight the need for research to elucidate walking condition effects and advantages of specific designs. Clinical relevance: Slope-adaptive prosthetic feet may improve user gait quality and comfort and enhance gait safety by increasing minimum toe clearance. Patients who encounter slopes regularly should be considered as potential users of SAF if indicated appropriately.

Abstract:Passive prosthetic legs require undesirable compensations from amputee users to avoid stubbing obstacles and stairsteps. Powered prostheses can reduce those compensations by restoring normative joint biomechanics, but the absence of user proprioception and volitional control combined with the absence of environmental awareness by the prosthesis increases the risk of collisions. This article presents a novel stub avoidance controller that automatically adjusts prosthetic knee/ankle kinematics based on suprasensory measurements of environmental distance from a small, lightweight, low-power, low-cost ultrasonic sensor mounted above the prosthetic ankle. In a case study with two transfemoral amputee participants, this control method reduced the stub rate during stair ascent by 89.95% and demonstrated an 87.50% avoidance rate for crossing different obstacles on level ground. No thigh kinematic compensation was required to achieve these results. These findings demonstrate a practical perception solution for powered prostheses to avoid collisions with stairs and obstacles while restoring normative biomechanics during daily activities.

Abstract:In case of transtibial amputation, the deficit resulting from the loss of the lower limb can be partly compensated with a prosthetic foot and adapted rehabilitation. New prosthetic feet have been developed for transtibial amputees to mimic ankle adaptability to varying terrain. Among them, Microprocessor Prosthetic Ankles (MPA) have a microprocessor to control an electric or a hydraulic actuator to adapt ankle kinematics in stairs and slopes. The objective is to investigate parameters extracted from the moment-angle curve (MAC) and use them to compare 3 MPA during level and slope locomotion against energy storing and return (ESR) foot. Five persons with lower limb transtibial amputation successively fitted with 3 MPA (PropriofootTM, ElanTM, MeridiumTM) compared to their ESR foot. The participants had 2 weeks of adaptation before data acquisition and then a 3 week wash-out period. Range of motion, equilibrium point, hysteresis, late stance energy released, and quasi-stiffness were computed on level ground and 12% slope (upward and downward) thanks to the MAC at the ankle. The study shows the relevance of MAC parameters to evaluate the behavior of MPA. In particular, compared to ESR, all MPA tested in the present study demonstrated a better angle adaptation between walking conditions but a decrease of available energy for the propulsion. Among MPA, main results were: (i) for the PropriofootTM: an adaptation of the ankle angle without modification of the pattern of the MAC (ii) for the ElanTM: a limited adaptation of the range of motion but a modification of the energy released (iii) for the MeridiumTM, the highest adaptation of the range of motion but the lowest available energy of propulsion. One of the main findings of the research is to show and quantify the relationship between range of motion and energy available when using different prosthetic feet in different walking conditions.

Abstract:Although emerging powered prostheses can enable people with lower-limb amputation to walk and climb stairs over different task conditions (e.g., speeds and inclines), the control architecture typically uses a finite-state machine to switch between activity-specific controllers. Because these controllers focus on steady-state locomotion, powered prostheses abruptly switch between controllers during gait transitions rather than continuously adjusting leg biomechanics in synchrony with the users. This paper introduces a new framework for powered prosthesis control by modeling the lower-limb joint kinematics over a continuum of variable-incline walking and stair climbing, including steady-state and transitional gaits. Steady-state models for walking and stair climbing represent joint kinematics as continuous functions of gait phase, forward speed, and incline. Transition models interpolate kinematics as convex combinations of the two steady-state models, with an additional term to account for kinematics that fall outside their convex hull. The coefficients of this convex combination denote the similarity of the transitional kinematics to each steady-state mode, providing insight into how able-bodied individuals continuously transition between ambulation modes. Cross-validation demonstrates that the model predictions of untrained kinematics have errors within the range of physiological variability for all joints. Simulation results demonstrate the model's robustness to incline estimation and mode classification errors.
Influence of Walking Over Unexpected Uneven Terrain on Joint Loading for Individuals with Transtibial Amputation

Abstract:Abstract Individuals with transtibial amputation (TTA) experience asymmetric lower-limb loading which can lead to joint pain and injuries. However, it is unclear how walking over unexpected uneven terrain affects their loading patterns. This study sought to use modeling and simulation to determine how peak joint contact forces and impulses change for individuals with unilateral TTA during an uneven step and subsequent recovery step and how those patterns compare to able-bodied individuals. We expected residual limb loading during the uneven step and intact limb loading during the recovery step would increase relative to flush walking. Further, individuals with TTA would experience larger loading increases compared to able-bodied individuals. Simulations of individuals with TTA showed during the uneven step, changes in joint loading occurred at all joints except the prosthetic ankle relative to flush walking. During the recovery step, intact limb joint loading increased in early stance relative to flush walking. Simulations of able-bodied individuals showed large increases in ankle joint loading for both surface conditions. Overall, increases in early stance knee joint loading were larger for those with TTA compared to able-bodied individuals during both steps. These results suggest that individuals with TTA experience altered joint loading patterns when stepping on uneven terrain. Future work should investigate whether an adapting ankle-foot prosthesis can mitigate these changes to reduce injury risk.

Abstract:After a transtibial amputation, the prosthetic foot aims at replacing the missing ankle joint. Due to alteration of proprioception and mobility, the static balance of amputees is challenging. The stiffness of most of the usual prosthetic feet cannot adapt according to the situation. Thus, the control of the user’s balance is closely related to the ankle stiffness value. The aim of this study is to evaluate both the impact of the ankle stiffness and the visual system on static balance. In order to avoid bias relative to different levels of residual proprioception among individuals, the study has been carried out on healthy subjects wearing lower limb prosthetic simulators under each foot. This configuration could be considered as a relevant model to isolate the effect of the stiffness. Eleven subjects wearing prosthetic feet with different modules were asked to remain as static as possible both with open eyes (OE) and closed eyes (CE). The center of pressure (COP) displacements and the joint angles range of motion (ROM) were experimentally assessed. The length of the major axis of the COP 95% confidence ellipse was projected on the antero-posterior direction (AP range). Linear regression models of the AP range and joint angles ROM as a function of the situation (OE and CE) and of the normalized ankle stiffness were created. A one-way analysis of variance test was performed on the model of the AP range. Linear regression coefficients and 95% confidence intervals (CI) were calculated between the AP range and the normalized ankle stiffness and between the joint angles ROM and the normalized ankle stiffness both in OE and CE. This study confirmed that static balance decreases when ankle stiffness decreases. The results also showed that a visual system alteration amplifies more significantly the decrease of static balance of people wearing prosthetic feet and has no significant influence on non-amputated subjects. The slope of the linear regression for the AP range according to the normalized ankle stiffness was equal to −9.86 (CI: −16.03, −3.69) with CE and −2.39 (CI: −4.94, 0.17) with OE. Both the normalized ankle stiffness and the visual system had a significant impact on the AP range (pvalue<0.05). The ankle stiffness is an interesting parameter as it has a high impact on the gait and on the static balance of the users and it must be controlled to properly design prosthetic feet.

Abstract:To enhance human mobility, training interventions and assistive lower limb wearable robotic designs must draw insights from movement tasks from daily life. This study aimed to analyze joint peak power, limb and joint work, and muscle activity of the lower limb during a series of stair ambulation conditions. We recruited 12 subjects (25.4±4.5 yrs, 180.1±4.6 cm, 74.6±7.9 kg) and studied steady gait and gait transitions between level walking, stair ascent and stair descent for three staircase inclinations (low 19°, normal 30.4°, high 39.6°). Our analysis revealed that joint peak power, limb and joint work, and muscle activity increased significantly compared to level walking and with increasing stair inclination for most of the conditions analyzed. Transition strides had no increased requirements compared to the maxima found for steady level walking and steady stair ambulation. Stair ascent required increased lower limb joint positive peak power and work, while stair descent required increased lower limb joint negative peak power and work compared to level walking. The most challenging condition was high stair inclination, which required approximately thirteen times the total lower limb joint positive and negative net work during ascent and descent, respectively. These findings suggest that training interventions and lower limb wearable robotic designs must consider the major increases in lower limb joint and muscle effort during stair ambulation, with specific attention to the demands of ascent and descent, to effectively improve human mobility.
