Abstract:Rationale and objectiveA causal relationship concerning coffee intake and diabetic nephropathy (DN) is controversial. We conducted a Mendelian randomization study to assess the causal nature of these associations.Methods40 independent single nucleotide polymorphisms (SNPs) associated with coffee intake were selected from the UK Biobank study. Summary-level data for diabetic nephropathy were obtained from publicly available genome-wide association studies (GWAS) and the FinnGen consortium. Inverse variance weighted (IVW), MR-Egger, and weighted median (WM) methods were used to examine a causal association. Sensitivity analyses included Cochran’s Q test, the intercept of MR-Egger, MR-PRESSO, and the Outlier method. Leave-One-Out sensitivity analyses were also conducted to reduce the heterogeneity.ResultsOur current study demonstrated positive associations of genetically predicted coffee intake with diabetic nephropathy (OR=1.939; P = 0.045 and type 2 diabetes with renal complications (OR = 2.787, P= 0.047). These findings were robust across several sensitivity analyses.ConclusionsThis study found a positive correlation between coffee consumption and the risk of diabetic nephropathy using genetic data. For a more accurate and trustworthy conclusion, subgroup analysis on coffee intake, including preparing method, variety of coffee, and quantity, is required.
Habitual Coffee Consumption and Risk of Cognitive Decline/dementia: A Systematic Review and Meta-Analysis of Prospective Cohort Studies.

Abstract:Objective: Findings from epidemiologic studies of coffee consumption and risk for cognitive decline or dementia are inconclusive. The aim of this study was to conduct a meta-analysis of prospective studies to assess the association between coffee consumption and the risk for cognitive decline and dementia.Methods: Relevant studies were identified by searching PubMed and Embase databases between 1966 and December 2014. Prospective cohorts that reported relative risk (RRs) and 95% confidence intervals (CIs) for the association of coffee consumption with dementia incidence or cognitive changing were eligible. Study-specific RRs were combined by using a random-effects model.Results: Eleven prospective studies, including 29,155 participants, were included in the meta analysis. The combined RR indicated that high coffee consumption was not associated with the different measures of cognitive decline or dementia (summary RR, 0.97; 95% CI, 0.84-1.11). Subgroup analyses suggested a significant inverse association between highest coffee consumption and the risk for Alzheimer disease (summary RR, 0.73; 95% CI, 0.55-0.97). The dose-response analysis, including eight studies, did not show an association between the increment of coffee intake and cognitive decline or dementia risk (an increment of 1 cup/d of coffee consumed; summary RR, 1.00; 95% CI, 0.98-1.02).Conclusions: The present study suggests that higher coffee consumption is associated with reduced risk for Alzheimer disease. Further randomized controlled trials or well-designed cohort studies are needed to determine the association between coffee consumption and cognitive decline or dementia. (C) 2016 Elsevier Inc. Coffee, tea, and caffeine consumption and prevention of late-life cognitive decline and dementia: A systematic review

Abstract:A prolonged preclinical phase of more than two decades before the onset of dementia suggested that initial brain changes of Alzheimer's disease (AD) and the symptoms of advanced AD may represent a unique continuum. Given the very limited therapeutic value of drugs currently used in the treatment of AD and dementia, preventing or postponing the onset of AD and delaying or slowing its progression are becoming mandatory. Among possible reversible risk factors of dementia and AD, vascular, metabolic, and lifestyle-related factors were associated with the development of dementia and late-life cognitive disorders, opening new avenues for the prevention of these diseases. Among diet-associated factors, coffee is regularly consumed by millions of people around the world and owing to its caffeine content, it is the best known psychoactive stimulant resulting in heightened alertness and arousal and improvement of cognitive performance. Besides its short-term effect, some case-control and cross-sectional and longitudinal population-based studies evaluated the long-term effects on brain function and provided some evidence that coffee, tea, and caffeine consumption or higher plasma caffeine levels may be protective against cognitive impairment/decline and dementia. In particular, several cross-sectional and longitudinal population-based studies suggested a protective effect of coffee, tea, and caffeine use against late-life cognitive impairment/decline, although the association was not found in all cognitive domains investigated and there was a lack of a distinct dose-response association, with a stronger effect among women than men. The findings on the association of coffee, tea, and caffeine consumption or plasma caffeine levels with incident mild cognitive impairment and its progression to dementia were too limited to draw any conclusion. Furthermore, for dementia and AD prevention, some studies with baseline examination in midlife pointed to a lack of association, although other case-control and longitudinal population-based studies with briefer follow-up periods supported favourable effects of coffee, tea, and caffeine consumption against AD. Larger studies with longer follow-up periods should be encouraged, addressing other potential bias and confounding sources, so hopefully opening new ways for diet-related prevention of dementia and AD.

Abstract:Heat shock affects the oocyte developmental competence and embryonic gene expression. We found that the chromatin organisation of embryos derived from heat-shocked oocytes can also be affected (Camargo et al. 2015 Reprod. Fert. Dev. 27, 132). This study aimed to evaluate whether Scriptaid, a histone deacetylase inhibitor able to modulate the chromatin structure, could influence the development of embryos derived from heat-shocked oocytes. Bovine oocytes were in vitro-matured under conventional temperature (38.5°C) for 24h (non-heat-shock; NHS group) or under 41.5°C for 12h followed by 38.5°C for 12h (heat-shock; HS group). In vitro fertilization was performed under 38.5°C with 5% CO2 in air for 20h. Right after the end of fertilization the presumptive zygotes from the NHS or HS groups were denuded and randomly exposed to 500 nM Scriptaid for 0, 12, or 24h, comprising 6 treatments as followa: NHS-0h (NHS without Scriptaid, n=185); NHS-12h (NHS plus Scriptaid for 12h, n=178); NHS-24h (NHS plus Scriptaid for 24h, n=177); HS-0h (HS without Scriptaid, n=187); HS-12h (HS plus Scriptaid for 12h, n=180); and HS-24h (HS plus Scriptaid for 24h, n=183). After Scriptaid exposure, zygotes were cultured in CR2aa plus 2.5% FCS at 38.5°C with 5% CO2, 5% O2, and 90% N2. Cleavage rate was calculated on Day 2 (44h post-fertilization) and blastocyst rates on Day 7 and 8 post-fertilization. Six replicates were carried out and data was analysed by logistic regression (Proc Logistic, SAS 9.2, SAS Institute Inc., Cary, NC). Significant data were interpreted as odd ratios considering the 95% confidence interval. Values are shown as mean±standard error of the mean. There was no difference (P>0.05) among NHS treatments (NHS-0h, NHS-12h, and NHS-24h) as well as among HS treatments (HS-0h, HS-12h, and HS-24h) for cleavage and blastocyst rates at Day 7. At Day 8, however, the blastocyst rate in the NHS group decreased (P<0.05) as the time of zygote exposure to Scriptaid increased to 24h (33.9±2.8 and 24.2±1.6% for NHS-0h and NHS-24h, respectively), whereas no difference (P>0.05) was found in the HS group (20.7±1.5, 21.2±1.6, and 20.5±2.4% for HS-0h, HS-12h, and HS-24h, respectively). Comparison between NHS and HS treatments showed that cleavage and blastocyst rates at Day 7 and 8 of NHS-0h (88.7±2.8, 30.1±1.5, and 33.9±2.8%, respectively) were superior (P<0.05) to HS-0h (79.3±3.2, 16.9±1.0, and 20.7±1.5%, respectively). Differences (P<0.05) between NHS-12h and HS-12h on blastocyst rates at Day 7 (32.8±3.8 v. 20.6±1.7%, respectively) and at Day 8 (31.7±2.7 v. 21.2±1.6%, respectively) were also found. However, no difference (P>0.05) between NHS-24h and HS-24h was found. We showed that Scriptaid for 24h right after IVF has a negative impact on further development of zygotes derived from oocytes matured under conventional temperature (NHS group), in contrast to zygotes derived from oocytes matured under high temperature (HS group). We concluded that the effect of Scriptaid on embryo development is influenced by the temperature during oocyte maturation.
Genetically Predicted Coffee Consumption and Risk of Alzheimer's Disease and Stroke

Abstract:Background: Observational studies have reported that coffee consumption was associated with Alzheimer's disease (AD) and stroke risk. However, the results are inconclusive. Objective: We aimed to evaluate whether genetically predicted coffee consumption is associated with AD and stroke using Mendelian randomization (MR) design. Methods: Summary-level data for AD (n = 5 4,162), ischemic stroke (n = 440,328), and intracerebral hemorrhage (ICH, n = 3,026) were adopted from publicly available databases. Summary-level data for coffee consumption were obtained from two genome-wide association studies, comprising up to 375,833 subjects. Results: Genetically predicted coffee consumption (cups/day) was associated with an increased risk of AD (OR = 1.26, 95% CI = 1.05-1.51). Moreover, genetically predicted 50% increase of coffee consumption was associated with an increased risk of ICH (OR: 2.27, 95% CI: 1.08-4.78) but a decreased risk of small vessel stroke (OR: 0.71, 95% CI: 0.51-0.996). Estimate for AD and ICH in FinnGen consortium is directionally consistent. Combined analysis of different databases further confirmed that genetically predicted coffee consumption was associated with an increased risk of AD and ICH. In the multivariable MR analysis, genetically predicted coffee consumption retained a stable effect with AD and ICH when adjusting for smoking (p < 0.05), while the association with AD attenuated when adjusting for alcohol use. Conclusion: Our results indicate that genetically predicted coffee consumption may be associated with an increased risk of AD and ICH. The underlying biological mechanisms warrant further study.
Higher Coffee Consumption Is Associated With Reduced Cerebral Gray Matter Volume: A Mendelian Randomization Study

Abstract:Background: Recently published two-sample Mendelian randomization (MR) studies showed that genetically predicted coffee consumption may be associated with increased risk of Alzheimer's disease and intracerebral hemorrhage but associated with a decreased risk of small vessel ischemic stroke. We aimed to investigate the effects of genetically predicted coffee consumption on magnetic resonance imaging (MRI) markers of cerebral small vessel disease and brain volume using the two-sample MR method. Methods: Twelve single nucleotide polymorphisms (SNPs) in up to 375,833 individuals were used as genetic instruments for cups consumed per day of coffee. Another four SNPs from an independent sample were used to perform the replication analysis. Three SNPs in up to 45,821 individuals were used as genetic instruments for high coffee consumption vs. low/no coffee consumption. Results: Mendelian randomization analysis showed that coffee consumption (cups/day) was inversely associated with gray matter volume (beta = -0.371, 95% CI = -0.596 to -0.147, p = 0.001). Replication analysis and multivariable analyses after adjusting for other risk factors confirmed the effect. High coffee consumption was also suggestively associated with decreased gray matter volume (beta = -0.061, 95% CI = -0.109 to -0.013, p = 0.013) compared with low/no coffee consumption. All analyses did not find an effect of coffee consumption on other outcomes including white matter hyperintensity volume, mean diffusivity, fractional anisotropy, brain microbleed, total brain volume, white matter volume, and hippocampus volume. Conclusion: This two-sample MR study showed that genetically predicted higher coffee consumption is causally associated with reduced gray matter volume of the brain.
Association of Coffee Consumption and Striatal Dopamine Transporter Binding in Patients with Parkinson’s Disease and Healthy Controls

Abstract:Abstract Emerging evidence indicates that coffee consumption significantly reduces the risk for developing Parkinson disease (PD). We aimed to investigate the role of chronic coffee consumption in nigrostriatal dopaminergic neurodegeneration using 123I-FP-CIT single photon emission computed tomography. Overall, 138 PD patients and 75 healthy controls (HC) were enrolled from the Parkinson’s Progression Markers Initiative cohort. Demographic, clinical and striatal dopamine transporter (DAT) characteristics were compared across subgroups, including current, ever and never coffee consumers in PD patients and HC, respectively. Furthermore, we included the factors that may have influenced the loss of nigrostriatal dopaminergic neurons in multiple linear regression analyses to identify significant contributing factors to striatal DAT binding in each striatal region. In PD patients, current consumers had significantly lower DAT binding in the bilateral caudate than ever/never consumers. In HC, current consumers had significantly lower DAT binding in the bilateral caudate and right putamen than ever/never consumers. Cups per day were negatively correlated with the left caudate DAT binding in current consumers of PD patients. In addition, multiple linear regression analyses showed current coffee consumption remained as an independent predictor of a decrease in striatal DAT binding. Striatal DAT downregulation caused by chronic effects of caffeine may provide evidence for the favorable implication of caffeine to PD pathologies. Furthermore, our study firstly suggests that the chronic effects of caffeine on striatal DAT may fade and disappear after quitting coffee.

Abstract:Background Limited and conflicting evidence exists on the associations between tea, coffee, and caffeine intake and risk of dementia and Alzheimer's Disease (AD). This meta-analysis aimed to elucidate these associations and quantify potential dose-response relationships. Methods PubMed, EMBASE, and Web of Science were searched up to 11 June 2024 for the cohort studies. Random effects models calculated pooled relative risks (RR) and 95% confidence intervals (CI), with dose-response relation assessed using restricted cubic splines. The Grading of Recommendations Assessment Development and Evaluation (GRADE) tool was used to assess the risk of bias. Results Our analysis encompassed 38 cohorts, totaling 751,824 participants and 13,017 dementia and 17,341s AD cases. For dementia, compared with the lowest category, the pooled RRs (95% CI) in the highest category of tea, coffee, and caffeine were 0.84 (0.74-0.96, n=6), 0.95 (0.87-1.02, n=9), and 0.94 (0.70-1.25, n=5), with all rated as low certainty in GRADE. For AD, the pooled RRs (95% CI) in the highest category of tea, coffee, and caffeine compared to the lowest category were 0.93 (0.87-1.00, n=6), 1.01 (0.90-1.12, n=10), and 1.34 (1.04-1.74, n=2), with certainty ratings of low, low, and very low, respectively. Dose-response analysis indicated a non-linear relationship coffee intake (Poverall=0.04 and Pnonlinear=0.01) and dementia risk, showing that the protective association for risk of dementia 1 to 3 cups/day of coffee intake. A linear association of tea intake and risk of dementia, with significantly decreased risk of dementia for each 1 cup/day increase in tea consumption (0.96, 95% CI 0.94-0.99, Poverall = 0.01 and Pnonlinear = 0.68). Conclusion Increased tea consumption was associated with decreased risk of dementia and AD, and a non-linear relation was found between coffee and dementia, supporting public health recommendations for dementia prevention.

Abstract:Background: Coffee and tea consumption has been linked to dementia. However, it remained unknown how sex and vascular risk factors modify the association. We aimed to investigate the association of coffee and tea consumption with dementia and whether sex and vascular comorbidities modified the association. Methods: We included 278 elderly patients with Alzheimer's disease (AD) and 102 patients with vascular dementia (VaD) from three hospitals; controls (N = 468) were recruited during the same period. We collected the frequency and amount of coffee and tea consumption and the presence of vascular comorbidities. The multinomial logistic regression model was utilized to evaluate the association of coffee and tea consumption with dementia, stratified by sex and vascular comorbidities. Results: Different combinations and quantities of coffee and tea consumption protected against AD and VaD. Consumption of ≥3 cups of coffee or tea per day was protective against AD [adjusted odds ratio (aOR) = 0.42; 95% confidence interval (CI) = 0.22-0.78)] and VaD (aOR = 0.42; 95% CI = 0.19-0.94). Stratified analyses showed that the protective effects of a higher quantity of coffee and tea against AD were more pronounced among females and individuals with hypertension. Consumption of either coffee or tea was associated with a decreased risk of VaD among diabetic participants (aOR = 0.23; 95% CI = 0.06-0.98). Hyperlipidemia modified the association of coffee or tea consumption on the risk of AD and VaD (both Pinteraction < 0.01). Conclusion: The risk of AD and VaD was lower with increased consumption of coffee and tea; the impact differed by sex and vascular comorbidities including hypertension, hyperlipidemia, and diabetes.

Abstract:Caffeine intake has been associated with a decreased risk of Parkinson's disease (PD) in men but the effect in women is less clear, and appears to be modified by use of post-menopausal estrogens. In a nested case–control study within the Nurses Health Study (NHS) and the Health Professionals Follow-up Study (HPFS), we examined associations between single nucleotide polymorphisms (SNPs) of caffeine metabolizing genes (CYP1A2 and NAT2) and estrogen receptors (ESR1 and ESR2), their interaction with caffeine intake and hormone replacement therapy (PMH) use (collected prospectively) and risk of PD. We matched 159 female cases to 724 controls and 139 male cases to 561 controls on birth year, source of DNA (blood or buccal smear), age and sex. The CYP1A2 rs762551 polymorphism (lower enzyme inducibility) was marginally associated with an increased risk of PD (RR, for increasing number of minor alleles=1.34; 95% CI 1.02, 1.78 in women, but not in men. None of the NAT2 (classified as slow vs. fast acetylator), ESR1 or ESR2 polymorphisms were significantly associated with an altered risk of PD. Marginally significant interactions were observed between caffeine intake and the ESR1 polymorphism rs3798577 (p=0.07) and ESR2 polymorphism rs1255998 (p=0.07). The observed increased risk of PD among female but not male carriers of the rs762551 polymorphism of CYP1A2 and the interactions of caffeine with ESR1 rs3798577 and ESR2 rs1255998 may provide clues to explain the relationship between gender, caffeine intake, estrogen status and risk of PD and need to be replicated.

Abstract:SummaryBackground & aimThe association between habitual coffee or caffeine consumption and age at onset (AAO) of Huntington's disease (HD) is unclear. We employed Mendelian randomization to investigate the causal relationship between coffee consumption and AAO of HD.MethodsThe instrumental variable included 14 independent genetic variants associated with coffee consumption was selected from a genome-wide association study (GWAS) meta-analysis of 375,833 individuals of European ancestry. Genetic association estimates for AAO of HD were obtained from the Genetic Modifiers of Huntington's Disease Consortium GWAS meta-analysis including 9,064 HD patients of European ancestry. The inverse variance weighted method was used to evaluate the causal estimate and a comprehensive set of analyses tested the robustness of our results.ResultsGenetically predicted higher coffee consumption was associated with an earlier AAO of HD (β = −1.84 years, 95% confidence interval = −3.47 to −0.22, P = 0.026). Results were robust to potential pleiotropy and weak instrument bias.ConclusionsThis genetic study suggests high coffee consumption is associated with an earlier AAO of HD. Coffee is widely consumed and thus our findings, if confirmed, offers a potential way to delay the onset of this debilitating autosomal dominant disease.

Abstract:Most individuals adjust their caffeine intake according to the objective and subjective effects induced by the methylxanthine. However, to reach the desired effects, the quantity of caffeine consumed varies largely among individuals. It has been known for decades that the metabolism, clearance, and pharmacokinetics of caffeine is affected by many factors such as age, sex and hormones, liver disease, obesity, smoking, and diet. Caffeine also interacts with many medications. All these factors will be reviewed in the present document and discussed in light of the most recent data concerning the genetic variability affecting caffeine levels and effects at the pharmacokinetic and pharmacodynamic levels that both critically drive the level of caffeine consumption. The pharmacokinetics of caffeine are highly variable among individuals due to a polymorphism at the level of the CYP1A2 isoform of cytochrome P450, which metabolizes 95% of the caffeine ingested. Moreover there is a polymorphism at the level of another critical enzyme, <i>N</i>-acetyltransferase 2. At the pharmacodynamic level, there are several polymorphisms at the main brain target of caffeine, the adenosine A2A receptor or ADORA2. Genetic studies, including genome-wide association studies, identified several loci critically involved in caffeine consumption and its consequences on sleep, anxiety, and potentially in neurodegenerative and psychiatric diseases. We start reaching a better picture on how a multiplicity of biologic mechanisms seems to drive the levels of caffeine consumption, although much more knowledge is still required to understand caffeine consumption and effects on body functions.

Abstract:Introduction: We investigated the link between habitual caffeine intake with memory impairments and cerebrospinal fluid (CSF) biomarkers in mild cognitive impairment (MCI) and Alzheimer's disease (AD) patients. Methods: MCI (N = 147) and AD (N = 116) patients of the Biomarker of AmyLoid pepTide and AlZheimer's diseAse Risk (BALTAZAR) cohort reported their caffeine intake at inclusion using a dedicated survey. Associations of caffeine consumption with memory impairments and CSF biomarkers (tau, p-tau181, amyloid beta 1-42 [Aβ1-42], Aβ1-40) were analyzed using logistic and analysis of covariance models. Results: Adjusted on Apolipoprotein E (APOE ε4), age, sex, education level, and tobacco, lower caffeine consumption was associated with higher risk to be amnestic (OR: 2.49 [95% CI: 1.13 to 5.46]; p = 0.023) and lower CSF Aβ1-42 (p = 0.047), Aβ1-42/Aβ1-40 (p = 0.040), and Aβ1-42/p-tau181 (p = 0.020) in the whole cohort. Discussion: Data support the beneficial effect of caffeine consumption to memory impairments and CSF amyloid markers in MCI and AD patients. Highlights: We studied the impact of caffeine consumption in the BALTAZAR cohort. Low caffeine intake is associated with higher risk of being amnestic in MCI/AD patients. Caffeine intake is associated with CSF biomarkers in AD patients.

Abstract:BACKGROUND: Coffee intake may lower prostate cancer risk and progression, but postdiagnosis outcomes by caffeine metabolism genotype are not well characterized.OBJECTIVE: To evaluate associations between coffee intake, caffeine metabolism genotype, and survival in a large, multicenter study of men with prostate cancer.DESIGN, SETTING, AND PARTICIPANTS: Data from The PRACTICAL Consortium database for 5727 men with prostate cancer from seven US, Australian, and European studies were included. The cases included had data available for the CYP1A2 -163C>A rs762551 single-nucleotide variant associated with caffeine metabolism, coffee intake, and >6 mo of follow-up.OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Multivariable-adjusted Cox proportional hazards models across pooled patient-level data were used to compare the effect of coffee intake (categorized as low [reference], high, or none/very low) in relation to overall survival (OS) and prostate cancer-specific survival (PCSS), with stratified analyses conducted by clinical disease risk and genotype.RESULTS AND LIMITATIONS: High coffee intake appeared to be associated with longer PCSS (hazard ratio [HR] 0.85, 95% confidence interval [CI] 0.68-1.08; p = 0.18) and OS (HR 0.90, 95% CI 0.77-1.07; p = 0.24), although results were not statistically significant. In the group with clinically localized disease, high coffee intake was associated with longer PCSS (HR 0.66, 95% CI 0.44-0.98; p = 0.040), with comparable results for the group with advanced disease (HR 0.92, 95% CI 0.69-1.23; p = 0.6). High coffee intake was associated with longer PCSS among men with the CYP1A2 AA (HR 0.67, 95% CI 0.49-0.93; p = 0.017) but not the AC/CC genotype (p = 0.8); an interaction was detected (p = 0.042). No associations with OS were observed in subgroup analyses (p > 0.05). Limitations include the nominal statistical significance and residual confounding.CONCLUSIONS: Coffee intake was associated with longer PCSS among men with a CYP1A2 -163AA (*1F/*1F) genotype, a finding that will require further replication.PATIENT SUMMARY: It is likely that coffee intake is associated with longer prostate cancer-specific survival in certain groups, but more research is needed to fully understand which men may benefit and why.

Abstract:In light of the fact that the number of elderly citizens in society is steadily increasing, the search for dietary factors which might prolong mental agility is growing in significance. Coffee, together with its main ingredient, caffeine, has been the focus of much attention from various researchers, as data on its beneficial effects on human health continue to accumulate. Most reports indicate that moderate coffee consumption may in fact lower the risk for common neurodegenerative conditions, i.e. Alzheimer's and Parkinson's diseases. Regardless, due to their complex pathogenesis as well as methodology of scientific research, the exact impact of coffee consumption remains to be fully elucidated. At present, it seems safe to inform the general public that coffee drinkers need not fear for their health. Possibly, in the future experts will recommend drinking coffee not only to satisfy individual taste preferences but also to decrease age-related mental deterioration.
