Abstract:Septic arthritis of the wrist can have severe deleterious effects on cartilage and bone if not promptly addressed. Expedient diagnosis and early medical intervention are important. The most effective strategy involves immediate arthrocentesis of the infected joint, enabling precise antibiotic selection based on joint fluid analysis. Diagnostic imaging is important in excluding fractures and identifying abscesses. This review explores the etiologic factors underlying septic wrist joint, identifying risk factors, and delineating optimal diagnosis and treatment approaches. The overarching goal is to impart valuable insights and guidance in the management of septic wrist joint, ensuring the highest quality patient care and optimal clinical outcomes.

Abstract:Background and Objectives: Hand septic arthritis is a potentially debilitating condition that can significantly affect patient functionality and quality of life. Understanding the demographic, clinical, and microbiological characteristics of this condition is crucial for its effective treatment and management. This study aimed to analyze the demographic and clinical profiles of patients with hand septic arthritis, to identify common microbial pathogens, and to evaluate the impact of various factors on clinical course and treatment outcomes. Material and Methods: This cross-sectional retrospective study examined patients diagnosed with septic arthritis of the hand, focusing on their demographic data, clinical presentation, causative organisms, treatment methods, and outcomes. Data on age, sex, cause of infection, affected sites, surgical interventions, microbiological findings, and patient outcomes were also collected. Results: This study found a higher prevalence of septic arthritis in males and identified bite as the predominant cause. Staphylococcus aureus is the most common pathogen. A large number of patients did not exhibit bacterial growth, and bacterial resistance did not significantly affect the outcome. Outcomes were statistically influenced by the timing of medical presentation and the presence of comorbidities. Conclusions: Early diagnosis and intervention are critical for effective management of hand septic arthritis. This study underscores the need for a comprehensive approach that considers patient demographic and clinical characteristics to optimize treatment outcomes. Awareness and preventive measures are essential to reduce the incidence and severity of this condition.

Abstract:Management of joint infection is an evolving topic. This article reviews the literature on the management of native and prosthetic joint infection and suggests some areas of improvement in short- and long-term management which could lead to better patient outcomes. Surgical management is the mainstay of treatment for native or prosthetic knee infection and aspiration should only be used for diagnostic purposes. A multidisciplinary team approach and compliance with national guidelines, alongside referral networks and pooling of expertise, should be mandatory to improve patient outcomes.

Abstract:Background: Septic arthritis is a common orthopedic emergency. Immediate establishment of the diagnosis and administration of an adequate therapy is paramount in minimizing morbidity and mortality in this severe condition. Objective: The aim of the present review was to evaluate the existing evidence in order to give an overview on current best practice in diagnostics and treatment of septic arthritis in adults and children. Results: Joint infections result from either hematogenous spread or direct inoculation of bacteria into the joint, mostly iatrogenically. Predisposing risk factors include recent orthopedic joint surgery, i. v. drug abuse, pre-existing inflammatory and degenerative joint diseases and old age. Although pathogens differ in different populations and age groups Staphylococcus aureus is the single most frequently isolated causative organism, followed by streptococci. Although diagnosis is based on an integration of medical patient history, clinical and laboratory findings and imaging studies, joint fluid analysis remains the mainstay in establishing a valid diagnosis. The range of differential diagnostics is broad and includes non-infectious inflammatory joint diseases, such as gout or reactive arthritis. Once a diagnosis has been established treatment should be started immediately. Treatment is based on adequate antibiotic therapy and joint drainage until dryness. There is a paucity of studies on the optimal antibiotic regimen, route of application and duration of therapy. Moreover, no high-quality studies exist on the optimal mode of joint drainage. While superiority has yet to be shown, operative treatment in terms of arthroscopic lavage must be considered the standard of care in Germany. Finally, despite promising results in children, the role of corticosteroids as an adjunct to antibiotic treatment in adults has yet to be clarified.

Abstract:A compact printed monopole Ultra-Wideband antenna with three subband notches is proposed and analyzed in this paper. The proposed antenna has the bandwidth from 3 GHz to over 11 GHz. By using two simple types of slot, triple subband-notched characteristic is obtained. A simple straight slot is etched on the radiating patch to acquire the subband notch at 3.3–3.7 GHz for WiMAX band. In addition, three simple semicircular slots are etched on the radiating patch to achieve another two subband notches at 5.15–5.825 GHz for WLAN and 7.25–7.75 GHz for downlink of X-band satellite communication systems. The antenna is successfully simulated and fabricated. The designed antenna has a compact size of 25 × 29 mm2 and a nearly omnidirectional radiation pattern in the whole bandwidth.

Abstract:Abstract Background/Aims Hand Osteoarthritis (HOA) is a condition causing significant pain and disability in those affected. Treatments include pharmacological/non-pharmacological approaches, including splinting, pain management with non-steroidal anti-inflammatory drugs (NSAIDs), intra-articular corticosteroid (ICS) injections and surgery. There may be variability in ICS use, since few trials have compared efficacy of ICS over placebo. We conducted a UK-wide study of practising clinicians regarding their management of HOA. Methods An online survey was conducted from September 2022 - May 2023. Practising clinicians who are members of their respective societies were contacted by email. Participants provided information about their job role, region of work, nature of their clinical practice, subtypes of HOA managed and preferred management options. Results A total of 132 complete responses were obtained from the survey. Responding clinicians included Consultant Rheumatologists (43.2%), Consultant Hand Surgeons (34.8%), Rheumatology Specialist Nurses (6.1%), Rheumatology trainees (7.4%), Occupational Therapists (4.6%), Consultant Radiologists (1.5%), Physiotherapists (0.8%), General Practitioners (0.8%) and Physician Associates (0.8%). Responses were obtained from London (25%), South East (12.9%), South West (4.5%), East of England (6.8%), Midlands (12.1%), North West (3.8%), North East (3.8%), Yorkshire and the Humber (28%), Scotland (0.8%) and Wales (2.3%). There were 48.5% of respondents working in tertiary care services, 37.1% in district general hospitals, 13.6% across multiple sites in secondary care and 0.8% in primary care. Ninety-eight per cent of responders were seeing patients with HOA and the most common number of patients seen with HOA was 1-10 per month, reported by 40.3% of respondents. The most common type of HOA observed was first CMC joint or DIP/PIP joint involvement (more than 50%), with a lower proportion for erosive HOA (up to 10%). More than 90% of respondents reported that NSAIDs/hand therapy/intra-articular steroid injections/surgery were the treatments most likely to be offered. Eighty per cent of respondents reported an awareness of splinting, 31% were aware of taping. Regarding ICS, 62.9% performed ICS injections and 37.1% preferred injections under ultrasound (US) guidance. Reasons for preferring injections in clinic included time taken for US guided injections due to pressure on radiology services. Some clinicians preferred US guided confirmation of inflammation before proceeding to injection. Conclusion We obtained a high response rate from clinicians managing HOA. The most common HOA subtype observed was the first CMC, PIP/DIP joints. Recent EULAR (European Alliance of Associations for Rheumatology) recommendations state that ‘intra-articular injections of glucocorticoids should not generally be used in hand OA’, but can be considered in patients with painful PIP/DIP joints. We found high use of ICS injections as an adjunct to other therapies. Injection in clinic was preferred due to pressures on Radiology services. Abstract:Purpose: To report the short-term outcomes and complications of patients undergoing 23-gauge transconjunctival sutureless pars plana vitrectomy. Methods: Retrospective, consecutive, noncomparative case series in which 100 eyes of 100 patients underwent elective 23-gauge pars plana vitrectomy for a variety of surgical indications. All patients were examined on postoperative day 1 and then approximately 1 and 4 weeks later. Demographic and ophthalmic data were recorded. Results: Fifty-two men and 48 women (mean age = 65 years) were observed for a mean of 26 weeks. Mean preoperative visual acuity was 20/842 and mean final postoperative visual acuity was 20/429. Postoperative visual acuity improved in 68% of patients, worsened in 16%, and remained unchanged in 16%. Mean preoperative intraocular pressure was 15 mmHg and mean postoperative intraocular pressure at final visit was 14 mmHg. No sutures were required to close sclerotomies and no intraoperative complications were observed, though one eye required conversion to 20-gauge pars plana vitrectomy during silicone oil removal. Postoperative complications included retinal detachment, cataract progression, vitreous hemorrhage, persistent macular hole, new macular hole, phthisis, posterior capsular opacification, and severe chemosis. Conclusions: Twenty-three-gauge pars plana vitrectomy demonstrates short-term visual outcomes and complication rates that are comparable to those reported with 20- and 25-gauge systems.

Abstract:Background: Septic arthritis is an accute bacterial infection of a synovial joint. It is an orthopaedic emergency that can lead to morbidity or mortality if not properly treated. the fundamental issues in the management of septic arthritis include the duration of antibiotic therapy, the mode of joint drainage and the role of physiotherapy. There is a paucity of local data on septic arthritis in Nigeria. The study wa carried out at the National Orthopoedic Hospital, Enugu; a regional trauma and orthopaedic center with wide catchments area covering at least three geopolitical zones of Nigeria. The objective of the study is to describe the pattern and distribution of non-gonococcal septic arthritis, the causative organisms, and the outcome of management of this condition at Enugu, Nigeria and make recommendations on the antibiotic therapy. Method: A retrospective study of all the patients that presented at the National Orthopaedic Hospital Enugu with Non-gonococcal septic arthritis between January 1997 and December, 2006 was done. The patient's case notes were retrieved from the Medical Record Department. Information extracted and analyzed included demographic data, joints affected, cultured organisms, antibiotic sensitivity pattern, duration of parenteral and oral antibiotics therapy, complications and follow-up period. Patients with incomplete records, immune-compromised patients and those with subjacent osteomyelitis were excluded from the study. Result: Forty-three patients were seen within the period and 40 had analyzable data. The age range was 1 month to 39 years, with a mean age of 10.2 years. Twenty-one patients (52.5%) were males and 19 (47.5%) were females. The hip joint was the most commonly affected (47.5%). Staphylococcus aureus was the most common organism (50%), followed by Coliformn (42.5%). Most patients (75%) had parenteral antibiotics for 3-5 days, while 77.5% of patients received oral antibiotics for 2-4 weeks. All the patients had arthrotomy and joint irrigation within 48 hours of admission. Complicatins were recorded in 11 patients (27.5%). Fixed flexion deformity was the commonest complication (17.5%). No mortality was recorded. Conclusion: Septic arthritis is an orthopaedic emergency. Early diagnosis and prompt treatment with appropriate ontibiiotics and surgical drainage are the keys to a successful outcome. In our environment (Enugu), the coliforms ore competing favourably with stophylococcus oaureus as causative agents of septic arthritis. Short term parenteral antibiotics of 3-5 days seems to be as effective as the 1-2 weeks therapy in the non-immune compromised patients and in cases not complicated by juxtarticular osteomyehlitis or presence of prosthetic implants.

Abstract:The sudden improvement in long-term schizophrenic patients after treatment with clozapine is frequently disconcerting to their families. In many cases the patients appear to have increased hostility toward their families because of the alleviation of negative symptoms such as apathy and blunted affect. The authors believe that psychoeducational intervention can help families adjust to the change in their family members and describe such intervention with three families. They emphasize the importance of recognizing that a patient's improvement can be a stressor in the family relationship and the need for devising appropriate interventions.

Abstract:Septic arthritis is an orthopaedic emergency associated with poor prognosis in cases with delayed treatment. The standard routes through which the infection spreads are hematogenous and direct entry. Any delay in medicine could mean the patient facing severe joint destruction, limitation in joint range, and inability to do activities of daily living. Septic arthritis is treated with a multidisciplinary approach in which physiotherapy is essential in making patients functionally independent. This article discusses a 58-year-old male patient with pain and swelling in the right knee joint and difficulty doing activities like walking, squatting and climbing stairs. On further investigations and diagnostic arthroscopy, he was diagnosed with septic/pyogenic arthritis caused by Staphylococcus aureus in the right knee. The patient was being treated with antibiotics. Along with it, patient-tailored physiotherapy rehabilitation, including, but not limited to, strengthening, range of motion (ROM) exercises, endurance training, etc., was also given, which proved highly effective at enhancing the patient's functional independence and quality of life. The outcome measure used in this report is the Knee Injury and Osteoarthritis Outcome Score (KOOS).

Abstract:Septic arthritis of the temporomandibular joint (SATMJ), primarily caused by bacterial infections, is a rare condition with a diverse etiology that is inadequately documented in the literature, resulting in the absence of standardized treatment protocols. Its nonspecific clinical presentation often leads to misdiagnosis as other temporomandibular disorders, delaying diagnosis and treatment and potentially causing severe complications in the absence of established therapeutic guidelines. The main objective of this article is to report a case of a 61-year-old female with diabetes who was undergoing prolonged corticosteroid therapy and presented with pain, swelling in the right pre-auricular area, and progressive limitation in mouth opening, with no history of facial trauma, where the early diagnosis and isolation of Staphylococcus aureus after a single-port arthrocentesis prompted the timely adjustment of the treatment regimen, significantly influencing the outcome by mitigating the risk of complications. Additionally, this report includes a comprehensive literature review, highlighting the crucial importance of this prompt intervention to achieve a favorable clinical outcome.
Arthroscopic Debridement Enhanced by Intra-Articular Antibiotic-Loaded Calcium Sulphate Beads for Septic Arthritis of a Native Knee Following Iatrogenic Joint Injection: A Case Report

Abstract:Septic arthritis (SA) represents an orthopedics urgency and mainly affects the knee joint. Due to its devastating effects on cartilage, immediate management is crucial. SA is characterized by an annual incidence of 2 to 10 cases per 100,000 individuals, with mortality rates fluctuating between 0.5% and 15%, with a substantially higher mortality rate observed in older people (15%) in contrast to younger cohorts (4%). The etiology of septic arthritis is multifactorial: a spectrum of Gram-positive and Gram-negative bacteria can contribute to the development of this condition, especially Staphylococcus aureus. The treatment involves urgent (arthroscopic or arthrotomic) debridement associated with adequate antibiotic therapy. Intra-articular antibiotic carriers can also be used to increase their local concentration and effectiveness. The case of a 67-year-old woman affected by knee SA from methicillin-susceptible S. aureus is presented. She was treated with an arthroscopic debridement enhanced by intra-articular antibiotic-loaded calcium sulphate beads, together with antibiotic therapy. At 2-year follow up, the infection had been eradicated and the patient fully recovered. This is the first description, to our knowledge, in the English literature, of the use of antibiotic-loaded calcium sulphate beads as an adjuvant in the surgical treatment of SA of a native knee joint.
Microbiological Differences Among Age Groups in Patients Diagnosed With Periprosthetic Joint Infection: A Database Analysis of 2,392 Patients

Abstract:Introduction: Periprosthetic joint infection (PJI) after total joint arthroplasty (TJA) poses substantial economic and quality-of-life challenges. With the rising incidence of hip and knee arthritis globally, understanding the changing profile of PJIs across different age groups becomes crucial. While various studies have explored risk factors, the influence of age on PJI remains debated, with potential bimodal relationships. This study aims to investigate the causative organisms of PJIs in patients of different age groups undergoing TJA. Methods: Conducted as a retrospective cohort study at a high-volume PJI referral center, the study adhered to Strengthening the Reporting of Observational Studies in Epidemiology guidelines. Data spanned from January 1, 2001, to December 31, 2022, including patients with documented PJI undergoing revision TJA. Patients were categorized into age quintiles, and outcomes analyzed included causative organisms, difficult-to-treat infections, antimicrobial resistance, and Gram stain characteristics. Statistical analyses used descriptive statistics, chi-square tests, and sensitivity analyses for hip and knee patients separately. Results: The study comprised 2,392 patients, with 60.7% undergoing hip arthroplasty and 39.3% undergoing knee arthroplasty. 1,080 women (45.2%) and 1,312 men (54.8%) were included. Older patients were markedly more likely to have gram-negative infections and atypical infections. Patients in the youngest age group had the lowest rates of methicillin-resistant Staphylococcus aureus infection. Results were similar between hip and knee PJIs. Conclusions: The study reveals age-related variations in the characteristics of PJIs after TJA, emphasizing higher risks of atypical and resistant infections in older patients. These findings underscore the importance of tailored preventive measures and potential considerations for adjunctive or prolonged antibiotic therapies, especially in the elderly population. Recognizing the unique infection patterns in older patients may inform better prevention and treatment strategies, with implications for enhanced patient care and outcomes. Future directions should focus on patient-specific strategies for preventing and treating PJIs, particularly in high-risk populations.

Abstract:Abstract Purpose Spontaneous sternoclavicular joint infection (SSCJI) is a rare and poorly understood disease process. This study aims to identify factors guiding effective management strategies for SSCJI by using data mining. METHODS An IRB-approved retrospective review of patients from two large hospitals (2010 to 2022) was conducted. Spontaneous sternoclavicular joint infection is defined as a joint infection without direct trauma or radiation, direct instrumentation, or contiguous spread. An interdisciplinary team consisting of thoracic surgeons, radiologists, infectious disease specialists, orthopedic surgeons, hospital information experts, and systems engineers selected relevant variables. Small set data mining algorithms, utilizing systems engineering, were employed to assess the impact of variables on patient outcomes. RESULTS A total of 73 variables were chosen and 54 analyzed against 11 different outcomes. Forty-seven patients (mean age, 51 (22–82); 77% male) met criteria. Among them, 34 underwent early joint surgical resection (<14 days), 5 patients received delayed surgical intervention (>14 days), and 8 had antibiotic-only management. The antibiotic-only group had comparable outcomes. Indicators of poor outcomes were soft tissue fluid >4.5 cm, previous SSCJI, moderate/significant bony fragments, HgbA1c >13.9%, and moderate/significant bony sclerosis. CONCLUSIONS This study suggests that targeted antibiotic-only therapy should be considered initially for spontaneous sternoclavicular joint infection cases while concurrently managing comorbidities. Patients displaying indicators of poor outcomes or no symptomatic improvement after antibiotic-only therapy should be considered for surgical joint resection (central image).

The aim of this study was to test if a locus previously mapped to a region on chromosome 17 qter in patients with epidermodysplasia verucciformis (EV) and psoriasis and considered to be responsible for an increased susceptibility to HPV 5, also is linked to increased HPV susceptibility in cervical cancer in situ. We also wanted to test whether HPV 16 positivity cluster in families with cervical cancer. DNA was extracted from formalin fixed biopsies of 224 affected from 77 families diagnosed with cervical cancer in situ. Two microsatellite markers (D17S939 and D17S802) containing the locus were genotyped and linkage analysis was performed. No linkage was found to any of the two markers, neither when considering all cancer cases as affected nor when only considering HPV 16 infected cancer cases as affected in the analysis. We conclude that the susceptibility locus for HPV 5 infections associated with EV and psoriasis does not seem to affect susceptibility to HPV 16, frequently detected in cervical cancer. Also, positivity for HPV 16 did not show a significant clustering in families.
Management of periprosthetic joint infection of the shoulder: A narrative review

Abstract:Evidence for management of shoulder periprosthetic joint infection (PJI) has traditionally originated from the hip and knee literature. The differing microbiome, anatomy and implants used in the shoulder mean this evidence is not always directly transferrable. The 2018 Philadelphia International Consensus Meeting for the first-time produced evidence-based guidelines and diagnostic criteria relating specifically to PJI of the shoulder. These guidelines and criteria recognize the pathogenicity of lower virulence organisms in the shoulder which often means clinical presentation is less obvious than other joints. The role of Cutibacterium acnes in shoulder PJI is the subject of increasing basic science and clinical research and advances in microbiological research may help to understand the pathology behind shoulder infections. There is new evidence that outcomes after revision shoulder arthroplasty are dependent on the virulence of the causative organism. An individualised approach to treatment considering host factors, organism, soft tissues and bone stock is recommended. Debate continues in the literature regarding the indications of one- or two-stage revision and the latest evidence is discussed and synthesized in this review article. We advocate careful multidisciplinary team decision making for cases of shoulder PJI and recognize a limited role for debridement and implant retention in acute shoulder PJI (<6 weeks). There appears to be a role for one-stage revision in lower risk cases with low virulence organisms but caution against its' universal adoption. In higher risk or complex cases, there remains a clear role for two-stage revision arthroplasty, and we detail the specifics of this protocol and procedure from our tertiary shoulder and elbow unit.
