Abstract:Objective: To describe disability-related performance and inequality nationwide in Brazil, and the changes that took place between 2012 and 2019 after the introduction of Programme for Improving Primary Care Access and Quality (PMAQ). Methods: We derived scores for disability-related care and accessibility of primary healthcare facilities from PMAQ indicators collected in round 1 (2011-2013), and round 3 (2015-2019). We assessed how scores changed after the introduction of PMAQ. We used census data on per capita income of local areas to examine the disability-specific care and accessibility scores by income group. We undertook ordinary least squares regressions to examine the association between PMAQ scores and per capita income of each local area across implementation rounds. Results: Disability-related care scores were low in round 1 (18.8, 95%CI 18.3-19.3, out of a possible 100) and improved slightly by round 3 (22.5, 95%CI 22.0-23.1). Accessibility of primary healthcare facilities was also poor in round 1 (30.3, 95%CI 29.8-30.8) but doubled by round 3 (60.8, 95%CI 60.3-61.3). There were large socioeconomic inequalities in round 1, with both scores approximately twice as high in the richest compared to the poorest group. Inequalities weakened somewhat for accessibility scores by round 3. These trends were confirmed through regression analyses, controlling for other area characteristics. Disability-related and accessibility scores also varied strongly between states in both rounds. Conclusions: People with disabilities are being left behind by the Brazilian healthcare system, particularly in poor areas, which will challenge the achievement of universal health coverage.

Abstract:Activated CD4-positive T cells are essential in the early stages of arteriosclerotic lesion development after cardiac transplantation. Besides its parenchymal effects, transforming growth factor-beta1 (TGF-beta1) mediates immunosuppressive effects on proliferation and activation of CD4 cells. This study was designed to assess immune contributions of TGF-beta1 to arteriosclerosis by comparing the effect of TGF-beta1-deficient and -competent infiltrating inflammatory cells on the development of intimal thickening in a heterotopic mouse transplant model (CBA to C57B6). Transplant arteriosclerosis was evaluated in cardiac grafts placed into knockout recipients heterozygous for TGF-beta1 (n=7) and was compared with those placed into wild-type recipients (n=11). At 55 days, allografts in TGF-beta1-deficient recipients had increased concentric intimal thickening. Computer-assisted analysis of all elastin-positive vessels (n=173) showed significantly increased luminal occlusion (67.8+/-5.6%) in grafts from TGF-beta1-deficient recipients compared with wild-type recipients (47.4+/-4.1%, P=0.003). To determine whether TGF-beta1 deficiency altered CD4 activation patterns, we studied intragraft cytokine expression. Using 32P-reverse-transcriptase polymerase chain reaction assays, we show that TGF-beta1-deficient recipients had an increased expression of the transcription factor STAT 4, interferon gamma, and interleukin-2 (Th1-type response) and unaltered or reduced expression of the transcription factor STAT 6, interleukin-4, and interleukin-10 (Th2-type response). Hence, when present, immune sources of TGF-beta1 attenuate transplant arteriosclerosis. This effect is associated with attenuation of Th1 forces.
National Prevalence of Disability and Clinical Accommodations in Medical Education

Abstract:Background: This study aimed to evaluate and report the national prevalence of disability across undergraduate medical education (UME) and examine differences in the category of disability, and accommodation practices between allopathic (MD)- and osteopathic (DO)-granting programs. Methods: Between May 20 and June 30, 2020, 75% of institutional representatives at eligible DO schools responded to a web-based survey. The survey assessed the aggregate prevalence of disabled DO students, prevalence of DO students by category of disability, and prevalence of accommodations granted. Descriptive statistics were used to summarize results. Using 2019 MD data, comparisons were made between MD and DO programs to calculate overall prevalence and differences in accommodation practices across undergraduate medical education. Results: DO-granting programs reported a disability prevalence of 4.27% of the total enrollment. Attention-deficit/hyperactivity disorder (ADHD), psychological disabilities, and chronic health disabilities were reported most frequently. DO-granting programs reported higher rates of ADHD than the MD-granting program. The national pooled prevalence of disability across MD- and DO-granting programs was 4.52%. MD-granting programs reported a higher number of students with disabilities and higher rates of psychological disabilities when compared with DO-granting programs. One hundred percent of DO students disclosing disability received some form of accommodation. General clinical accommodations were more frequently provided in MD-granting programs when compared to DO-granting programs. Conclusions: This study provides the first comprehensive prevalence of US medical student disability and accommodations. Additionally, these data may serve as a benchmark for DO programs, with implications for curricular development, instructional planning and disability support, and resource allocation in medical education.
Disability inclusion in the Brazilian health system: results of a health system assessment

Abstract:Background: People with disabilities face more barriers accessing healthcare and, on average, experience worse health outcomes. Strengthening health access for people with disabilities requires coordinated action across the health system. The Missing Billion Inclusive Health System Framework is a new tool to support policy makers assess levels of disability inclusion within health systems. In this study we use the framework within the Unified Health System in Brazil. We consider the relevance and feasibility of the indicators, as part of further testing and refining the framework. Methods: Information sources, used to complete the assessment, included Brazilian laws and policies, publically available data, published literature and interviews with people with disabilities and service providers. A workshop with stakeholders was held to co-develop key recommendations. Findings: Overall, the framework was comprehensive and feasible to complete. It highlighted key strengths in terms of disability inclusion in the Brazilian health system as well as gaps and leverage points for action. Interpretation: The Missing Billions framework can identify progress and opportunities to strengthen disability inclusion in health systems. In Brazil, key promotive factors include supportive policies, leadership and financing structures. There are also opportunities for strengthening data and evidence, healthcare worker training on disability and health service accessibility. Actions must be centered on, and informed by, people with disabilities.

Abstract:Abstract Introduction In Brazil, situations of discrimination and violence experienced by the LGBTQIA+ community in health care services and educational institutions constitute programmatic vulnerability and have repercussions for their health-disease-care process. In the context of health education, topics related to sexual and gender minorities are not effectively explored, and it is not a mandatory part of the curriculum of medical schools. Objective To analyze the perception of medical students who identify as LGBTQIA+ about homophobia and transphobia. Methods Qualitative cross-sectional study. An online questionnaire was answered anonymously by LGBTQIA+ medical students. They are in the mandatory internship curriculum of Gynecology and Obstetrics in a public maternity school in northeastern Brazil. Responses were categorized, coded, analyzed, and interpreted using Bardin's content analysis method. Results Seven LGBTQIA+ medical students participated. Regarding sexual identity, 3 identified themselves as cisgender women, three as cis men, and one person as "questioning" of these five are homosexual, two bisexual, and one asexual. When asked about the qualification of health service professionals regarding LGBT+ people, six respondents commented on the lack of technical training of the medical team. A student believes that most physicians are knowledgeable about the topic and respect LGBT people. All interviewees have already witnessed, as victims or witnesses, situations of homophobia or transphobia. Three of the participants experienced circumstances that involved professionals providing care to patients. Among the actions mentioned to improve the qualification of professionals in serving the LGBT+ community, most participants suggested training,, and mandatory classes on the subject. Conclusions There was a significant perception of prejudice against the LGBTQIA+ population in transdisciplinary care and medical training. The solutions embrace the inclusion of topics on sexual and gender minorities in the curriculum of medical schools, as well as spaces and welcoming strategies for students who have experienced LGBTphobia. Abstract:In 2021, 28.16% of Brazilians were aged 0–19 years, highlighting the critical need for medical care in diagnosing and treating cerebral palsy (CP). In developing countries, CP prevalence reaches 7/1000 live births, emphasizing the importance of healthcare access, which influences diagnosis and prognosis. This study aimed to analyze the correlation between health infrastructure across different Brazilian regions and its impact on hospitalization and mortality rates among children with CP and other paralytic syndromes. An ecological time-series design was used, analyzing secondary population-based data from DATASUS, covering January 2018 to December 2021. The data included healthcare facilities, physicians, hospitalizations, and deaths for individuals aged 0–19 years, with analysis conducted using Microsoft Excel. The results revealed significant disparities in healthcare infrastructure, particularly in the northern and northeastern regions. A notable correlation was found between healthcare inequalities and hospitalizations and mortality rates, with the northern region showing significant results (p = 0.03 for hospitalizations and p = 0.02 for mortalities). This study underscores significant regional disparities in healthcare access in Brazil, contributing to variations in hospitalization rates for children and adolescents, especially in the northern region.

The perception of second language sounds in early bilinguals: new evidence from an implicit measure.

Abstract:Previous studies have suggested that nonnative (L2) linguistic sounds are accommodated to native language (L1) phonemic categories. However, this conclusion may be compromised by the use of explicit discrimination tests. The present study provides an implicit measure of L2 phoneme discrimination in early bilinguals (Catalan and Spanish). Participants classified the 1st syllable of disyllabic stimuli embedded in lists where the 2nd, task-irrelevant, syllable could contain a Catalan contrastive variation (/epsilon/-/e/) or no variation. Catalan dominants responded more slowly in lists where the 2nd syllable could vary from trial to trial, suggesting an indirect effect of the /epsilon/-/e/ discrimination. Spanish dominants did not suffer this interference, performing indistinguishably from Spanish monolinguals. The present findings provide implicit evidence that even proficient bilinguals categorize L2 sounds according to their L1 representations.

Abstract:Abstract The study of the association of social variables with the prevalence of impairments can provide subsidies for more adequate care and health policies for the most needy people by incorporating social aspects. This article aims to estimate the prevalence of diverse types of impairments, the degree of difficulty, limitations, and the need for help they cause and attest whether this prevalence differ by educational attainment in individuals aged 20 years or older. This is a populational cross-sectional study (2015 Health Survey of São Paulo-ISA Capital). Data from 3184 individuals were analyzed via educational attainment as exposure variable and outcome variables related to visual, hearing, intellectual, and mobility impairments. 19.9% of participants had visual, 7.8%, hearing, 2.7%, intellectual, and 7.4%, mobility impairments. Mobility and intellectual impairments limited participants’ daily activities the most, 70.3% and 63.3%, respectively; who, thus, needed the most help: 48.9% and 48.5%, respectively. Lower schooling was associated with a higher prevalence of impairments, greater need for help due to visual and intellectual impairments, and greater limitations due to hearing and visual impairments.

Prevalence of neonatal screening and associated factors in Brazil: a comparison of the 2013 and 2019 National Health Surveys

Abstract:This study analyzed the prevalence of complete neonatal screening (CNS) of children aged under 2 years in Brazil and associated factors using data from the 2013 (n=4,442) and 2019 (n=5,643) national health surveys. We conducted a cross-sectional study to compare prevalence of CNS (eye, ear and heel prick tests) adopting 95% confidence intervals (95%CI) and a 5% significance level. Crude and adjusted Poisson regression was performed to estimate prevalence ratios (PR) and 95%CI to assess the association between socioeconomic, demographic and health variables and CNS. There was a statistically significant increase in CNS prevalence, from 49.2% (95%CI: 47.1-51.3) in 2013 to 67.4% (95%CI: 65.5-69.3) in 2019. However, large disparities persist across states and between sociodemographic groups. In both years, CNS prevalence was lowest among brown and black children, those from families in the three lowest income quintiles, children without health insurance, those from families registered in the Family Health Strategy and children living in the North, cities outside the state capital/metropolitan regions and rural areas. Despite the increase in prevalence of CNS, deep individual and contextual inequalities persist, posing challenges for health policies.

Abstract:In Brazil, the Unified Health System (SUS) is the national health system that offers free medical services to all citizens including all levels of treatment and prevention of diseases, subsidized by the government. Although SUS is universally offered, people with intellectual and developmental disabilities (I/DD) face challenges regarding healthcare. This article aims to present a description of the healthcare framework for people with I/DD in Brazil, as well as to characterize the Care Network for Person with Disabilities. This is a descriptive study that reports aspects of the structure and functioning of the health system in the country. In addition, we summarize core data from the National Health Survey that characterizes the health condition of people with I/DD in the country. Although Brazil has promoted legislation in favor of the health of people with I/DD and other disabilities, the obstacle is the implementation. Overall, there have been advances in SUS efficacy. However, challenges include long waiting times for diagnosis, habilitation/rehabilitation, inadequate transportation, and insufficient staff training on disability and I/DD, particularly. In this sense, it is necessary to monitor laws and inclusive actions so that the principles of the SUS are actually applied.

Abstract:ObjectiveThis study aimed to compare mental health, quality of life, empathy, and burnout in medical students from a medical institution in the USA and another one in Brazil.MethodsThis cross-cultural study included students enrolled in the first and second years of their undergraduate medical training. We evaluated depression, anxiety, and stress (DASS 21), empathy, openness to spirituality, and wellness (ESWIM), burnout (Oldenburg), and quality of life (WHOQOL-Bref) and compared them between schools.ResultsA total of 138 Brazilian and 73 US medical students were included. The comparison between all US medical students and all Brazilian medical students revealed that Brazilians reported more depression and stress and US students reported greater wellness, less exhaustion, and greater environmental quality of life. In order to address a possible response bias favoring respondents with better mental health, we also compared all US medical students with the 50% of Brazilian medical students who reported better mental health. In this comparison, we found Brazilian medical students had higher physical quality of life and US students again reported greater environmental quality of life. Cultural, social, infrastructural, and curricular differences were compared between institutions. Some noted differences were that students at the US institution were older and were exposed to smaller class sizes, earlier patient encounters, problem-based learning, and psychological support.ConclusionWe found important differences between Brazilian and US medical students, particularly in mental health and wellness. These findings could be explained by a complex interaction between several factors, highlighting the importance of considering cultural and school-level influences on well-being.

Diagnostic Practices of Autism Spectrum Disorders in Brazil

Abstract:In Brazil, in spite of over half a million students with special needs in primary education, little research has been conducted about ASD diagnostic practices. This involves learning about the training that professionals are receiving, what diagnostic practices are being implemented on the field, and whether those are being contextualized according to the Brazilian culture. Students with special needs are directly affected by the results of diagnostic practices, since identifying the individuals is one of the first steps. Lack of understanding about that stage may lead to the problematic realities of underidentification or overidentification, exclusion of children who should receive that special intervention and inclusion of some who do not need them. Precision in diagnosis, that considers cultural factors, is a highly desired and continuous goal for special educators. Therefore, students with ASD are directly affected by the results of this research. The purpose of this study is 1) to determine the knowledge and training of professionals that diagnose Autism Spectrum Disorder in Brazil (Audiologists, Neurologists, Pediatricians, Psychologists, Psychiatrists, and others), 2) to determine if those professionals are using diagnostic practices similar to the US, 3) to investigate which procedures and instruments are used in the diagnosis of a child suspected of having an Autism Spectrum Disorder in Brazil; and 4) to determine what happens after diagnosis. This research can be placed within the context of studies focused on three complementary areas: conceptual, historical, and cross-cultural. The first area is related to the overall conceptual understanding of cultural factors in the diagnosis of ASD. The second one considers the historical influences in the diagnostic practices of children with ASD in Brazil, including government policies. The last one adopts a cross-cultural approach to the study of autism with emphasis on comparative studies. The survey instrument specifically designed for this study is The Autism Spectrum Disorders Assessment Survey comprised of thirteen questions (single and multiple choice items, and Likert-type items) designed to collect information in regards to demographic characteristics of the participant, description of the diagnostic team, procedures, instruments used for diagnosis, and knowledge about characteristics necessary to identify a child as having an ASD. The survey received 236 responses from professionals that diagnose Autism Spectrum Disorders from every state and the Federal District in Brazil. The results of this research suggest the need to invest in the training of professionals. Another related suggestion is to develop specific criteria and common diagnostic protocol for professionals in Brazil. Finally, providing access to inclusive education to people with autism would also provide them a more adequate opportunity for development.
ANALYSIS OF MIGRATION BETWEEN STATES FOR HOSPITAL ADMISSION OF PATIENTS WITH CEREBRAL PALSY IN BRAZIL IN 2019

Abstract:Background Cerebral Palsy (CP) is defined as a non-progressive neurological condition of the motor development caused by disorders that occurred in the developing brain, leading to limitations in several systems. Providing integral health assistance means offering health services at all levels of care, meeting the existing needs of the population. However, the low quality of the services and accessibility problems demonstrate that the care model of the country can sometimes be inefficient. Asymmetries and flaws in the effectiveness of the current model often make it necessary for parents/guardians of patients with CP to search hospital care in Federative Units (FUs) other than the FU of residence. Objectives Analyze the number of patients with Cerebral Palsy assisted who sought hospital care in Federative Units different from the FU of residence. Methods Descriptive study based on data from the information systems of the Ministry of Health on hospital admissions on a nationwide level. Data obtained were authorizations for hospitalization, FU where the establishment is located, FU where the patient resides and an indicator of care at a FU other than the location of residence. The FUs were also grouped in macroregions. Data were analyzed using the R software and presented in frequency measures. Results A total of 8,785 inpatient admissions from patients with CP were registered in Brazil during 2019. The southeast region had the highest number of records, and the north had the lowest. Of the total number of inpatient admissions records, about 4.5% were from patients treated in a FU different from their FU of residence. ¼ of these records belonged to the Midwest region, which is the region that receives most patients from other locations; this region received patients from all regions of the country and from almost all states. The northern region did not register care for patients from any of the other regions; and the southern region registered only one. In the analysis by state, it was observed that, in general, states rarely receive patients residing in other states. The exception was the Federal District, which recorded that 65% of hospitalizations of patients with CP in establishments located in the district are residents of other states; Goiás is responsible for 35% of these admissions. Conclusions The Brazilian states do not receive many patients coming from other FU, and when they do, they are normally from close states. The North did not register attendances of patients not residing in the region; however, the other regions of the country admitted patients from the northern region. Unlike other states, the Federal District mainly serves patients residing in other locations. Implications It is possible that there is a difference in quality and aptitude of assistance for patients with CP between regions and states of Brazil, which leads these patients to be cared for in health facilities located in a FU different from their residence. These data can indicate which states need the attention of health managers, and actions that identify points of improvement for the service capacity.

Overweight and obesity among Brazilian healthcare university students: prevalence and associated factors

Abstract:Objective: Obesity is an important factor for cardiovascular and metabolic events. Thus, this study aimed to assess the prevalence of overweight/obesity and the associated factors among healthcare university students.Materials and methods: A cross-sectional university-based study of 2,245 healthcare university students in the Midwest region of Brazil was conducted in 2018. Overweight and obesity were defined as body mass index (BMI) ≥ 25 kg/m2 and BMI ≥ 30 kg/m2, respectively. Demographic, socioeconomic, behavioral, family and comorbidities characteristics were investigated. Poisson regression was used for the multivariable analysis. All analysis was stratified by sex.Results: The mean age of the sample was 22.7 years (standard deviation = 4.1) and 69.5% of the students was female. The prevalence of overweight and obesity were 13.9% (95% confidence interval [CI]: 12.2- 15.6) and 4% (95% CI: 3.0-4.9) for women and 34.5% (95% CI: 30.9-38.1) and 11.3% (95% CI: 8.9-13.6) for men, respectively. After multivariate adjustment, the prevalence of overweight/obesity was at least 70% higher in female students aged 24 years or older (prevalence ratio [PR] = 1.73; 95% CI: 1.24-2.41) and those who smoke (PR = 1.95; 95% CI: 1.66-3.02). Additionally, female students with a family history of obesity (PR = 2.01; 95% CI: 1.46-2.77) or with two or three comorbidities (PR = 2.09; 95% CI: 1.43-3.04) exhibited a significantly higher probability for overweight/obesity. Similar findings were observed in male students, but with smaller effect sizes.Conclusion: This study revealed a high prevalence of overweight/obesity among healthcare university students, especially in men. It was related to the socio-demographic and family characteristics rather than behavioral factors.

Abstract:PURPOSE: The number of medical schools in Brazil has increased in recent years; however, vacancies for specialization in ophthalmology probably have not kept up with the growing demand. This study wants to estimate the increase in medical schools, the demand for ophthalmology specialization, and evaluate learning opportunities in ophthalmology.METHODS: This was a retrospective study with research from the Ministry of Education and Brazilian Council of Ophthalmology database from 2002 to 2021. These data were checked through 120 notices published by the institutions in 2021.RESULTS: The number of medical school vacancies increased by 370%, whereas the number of certified ophthalmology vacancies increased by 64%. There was an 11.4% misalignment between the Brazilian Council of Ophthalmology data in the Ministry of Education.CONCLUSION: The proportion of medical graduates has increased much more than opportunities for ophthalmology specialization. The effect on the search for unaccredited specialization positions is unknown, and policies for monitoring the specialization of ophthalmology vacancies should be established.

Abstract:The access of People with Disabilities (PwD) to specialized care services was analyzed on the basis of the availability-accommodation and adequacy dimensions. This is a case study with a qualitative approach and triangulation of sources based on documentary research, data from the Health Information Systems and semi-structured interviews with managers, health professionals and PwD. There was an expansion of rehabilitation services in Recife, although it was not possible to analyze the production capacity of such services. The findings point to the existence of architectural and urban barriers and insufficient resources in the services studied. Furthermore, there is a long waiting time for specialized care and difficult access to assistive technologies. It was also observed that professionals have low qualifications to meet the needs of PwD and a process of permanent education in different levels of complexity has not been instituted for workers. The conclusion drawn is that the institution of the Municipal Policy of Comprehensive Health Care for the PwD was insufficient to guarantee access to health services with continuity of care, considering the permanence of the fragmentation of the care network, thus violating the right to health of this segment.

Abstract:Abstract Worldwide, more than one billion people live with a disability (World Health Organization. (2011). World report on disability . People with a disability (PWD) have unmet needs with the delivery of health care, secondary to nursing student’s negative attitudes, knowledge, and skill deficits. This integrative literature review evaluated nursing student’s education and attitudes towards PWD. Deficient education, attitudes, knowledge, and skills are addressed. Combined search terms utilized included: healthcare students; healthcare education; nursing students; clinical experience; clinical rotation; attitude of health care students; and exposure to PWD. Thirty articles were analyzed with common themes identified. Abstract:Introduction Sexual dysfunctions remain inadequately addressed in medical education, despite their prevalence and adverse impact on quality of life. In Brazil, there are scarce university-affiliated healthcare centers that focus on sexual health. These services play a critical role in diagnosing and managing sexual disorders and can be pivotal in the training of healthcare professionals, as well as in the advancement of research and outreach in sexual medicine. Objective To describe the experience of establishing two sexual health centers in university-affiliated hospitals in Northeastern Brazil. Method This is an experiential report detailing the establishment of two distinct sexual health services. Results Both sexual health services started in different general hospitals in 2021, founded by a male and a female psychiatrists, authors of this paper. The initiative was prompted by the lack of dedicated sexual health services and the awareness of substantial patient distress stemming from sexual issues. Activities include patient consultations, case discussions, sexology seminars, and research project development, involving medical students and resident physicians, integrated into the psychiatry residency curriculum Referrals come mainly from other hospital specialties, but external referrals are also accepted. Collaboration with other healthcare professionals occurs through the analysis of clinical cases. Conclusion The establishment of sexual health services within university-affiliated hospitals has a positive impact on patient care and contributes to the study of sexual medicine, an area often overlooked in medical education. Improvements are needed in patient recruitment, counter-referral procedures, and the systematic incorporation of medical student rotations. While these services hold promise, they require greater integration with other healthcare services and a closer collaboration with undergraduate medical education. Financing No conflict.

Abstract:Abstract Background The prevalence and burden of musculoskeletal (MSK) conditions are growing around the world, and low back pain (LBP) is the most significant of the five defined MSK disorders in the Global Burden of Disease (GBD) study. LBP has been the leading cause of non-fatal health loss for the last three decades. The objective of this study is to describe the current status and trends of the burden due to LBP in Brazil based on information drawn from the GBD 2017 study. Methods We estimated prevalence and years lived with disability (YLDs) for LBP by Brazilian federative units, sex, age group, and age-standardized between 1990 and 2017 and conducted a decomposition analysis of changes in age- and sex-specific YLD rates attributable to total population growth and population ageing for the purpose of understanding the drivers of changes in LBP YLDs rates in Brazil. Furthermore, we analyzed the changes in disability-adjusted life years (DALYs) rankings for this disease over the period. Results The results show high prevalence and burden of LBP in Brazil. LBP prevalence increased 26.83% (95% UI 23.08 to 30.41) from 1990 to 2017. This MSK condition represents the most important cause of YLDs in Brazil, where the increase in burden is mainly related to increase in population size and ageing. The LBP age-standardized YLDs rate are similar among Brazilian federative units. LBP ranks in the top three causes of DALYs in Brazil, even though it does not contribute to mortality. Conclusions Findings from this study show LBP to be the most important cause of YLDs and the 3rd leading cause of DALYs in Brazil. The Brazilian population is ageing, and the country has been experiencing a rapid epidemiological transition, which generates an increasing number of people who need chronic care. In this scenario, more attention should be paid to the burden of non-fatal health conditions.
