Abstract:The forearm extensor muscle and hand extensor tendons are composed of several structures with unique anatomy identified with high morphological variability. During a routine dissection of a 74-year-old donated male cadaver, the right hand was isolated for educational purposes. After carefully dissecting the structures, an accessory muscle was identified. The accessory muscle corresponded to the extensor digitorum brevis manus (EDBM) inserted into the extensor indicis (EI) tendon, which was typically identified. According to the current literature, this occurrence corresponds to a rare variant, with 0.36-0.38% prevalence between the cadaveric studies. Knowledge of the hand extensor musculature is essential for orthopedics and plastic surgeons operating in the region to avoid iatrogenic injury.

Abstract:A 78-year-old male cadaver showed bilateral anomalous muscles on the dorsum of the hand. An extensor digitorum brevis manus was noted on the dorsum of the right hand. It originated from the distal end of the radius and the radiocarpal joint ligaments and inserted into the metacarpophalangeal joint of the third digit. On the dorsum of the left hand, an extensor digiti medii proprius was identified. It originated from the distal third of the ulna near the extensor indicis proprius and the interosseous membrane and inserted into the metacarpophalangeal joint of the third digit. Awareness of these combined muscular variation would be helpful in understanding the identification of digital extensors and in requiring careful consideration for the reconstruction surgery of the hand.
An accessory head of the extensor indicis: a rare case report

Abstract:Purpose: A deep knowledge of the variations of the posterior forearm musculature is crucial for assessing and diagnosing conditions in this region. Extensor indicis (EI) is one of the muscles in this region, which exhibits diverse anatomical variations. This report documents an extremely unusual form of the EI with an accessory head on the dorsum of the hand. Methods: During routine dissection, an extremely rare presentation of the EI was found in the left forearm of a 94-year-old female cadaver. Results: This unusual EI consisted of two muscle bellies. The traditional belly originated from the distal two-thirds of the ulna. The muscle became tendinous around the carpal area, distal to the extensor retinaculum. The tendon was subsequently joined by an accessory muscle belly originating from the distal radioulnar ligament. The EI tendon inserted onto the dorsal expansion of the index finger, ulnar to that of the extensor digitorum. The posterior interosseous nerve innervated the muscle. Conclusion: Herein, we report an extremely rare form of the EI. To our knowledge, EI with an accessory head has only been reported rarely over the past 200 years. Moreover, our report appears to be the first case with photographic details of this anatomical variation. Clinicians should be aware of this variation for proper diagnosis and treatment.

Abstract:Abstract The Extensor Indicis muscle is one among the deep muscles of the extensor compartment of the forearm which takes origin from the Posterior surface of the shaft of the ulna below the origin of the extensor pollicislongus, and from the Inter-osseous membrane. It gets inserted close to the head of second metacarpal bone joining with the index tendon of extensor digitorum on its ulnar side and enters the formation of dorsal digital expansion of the index finger. This muscle helps in the Extension of the index finger and also to extend the wrist. During our routine dissection for 1 st BAMS students, in the dissection hall of KAHER’s Shri B.M. Kankanawadi Ayurveda Mahavidyalaya, Belagavi, of a 55 year old male cadaver, variation in the origin of Extensor Indicis muscle was observed. The extensor Indicis muscle was found arising from the Lunate carpal bone and Dorsal Radiocarpal ligament andit further continued as short belly and had its distal attachment on the proximal phalanx of the index finger. Abnormal origin of Extensor IndicisMuscle may possibly be associated with pain and swelling at the back of the hand. This pain may be misinterpreted with other clinical conditions like synovial cyst and ganglion. Knowledge of such rare variations isuseful to clinician and surgeons for therapeutic and surgical procedures.

Abstract:This report describes a new configuration of the extensor digitorum longus (EDL) with two heads and two main tendons divided into four slips. During routine cadaver dissection, unilateral (right) EDL muscle belly and tendon variations were identified. The morphometric measurements of the EDL were conducted using Image J software. Two bellies and two tendons of the EDL muscle were observed in the right leg of an 87-year-old male cadaver. The second belly of the EDL muscle originates 2 cm below the first belly. The tendons originate at the ends of the muscle bellies. First and second tendons split into two more slips after passing through the same tunnel below the extensor retinaculum. The first tendon was divided into two tendon slips attached to the extensor expansion of the second and third toes, whereas the second tendon was divided into two tendon slips attached to the extensor expansion of the fourth and fifth toes on the dorsum of the foot. The width of the muscle belly for the tendons of 2nd and 3rd toes was 1.10 ± 0.06 cm, while it was 1.39 ± 0.04 cm for that of the 4th and 5th toes. The developmental variations in the EDL muscles can be asymptomatic. This variant of the EDL muscle may cause entrapment under the extensor retinaculum, potentially restricting ankle dorsiflexion. This limitation can also affect walking. Consequently, paying attention to this variance is crucial for surgical planning and interpretation of radiological scans.

Abstract:Palmaris profundus is an aberrant muscle of forearm and wrist anatomy. It has no discernible function, but its tendon has been implicated as a cause of carpal tunnel syndrome. Previously, all cases of palmaris profundus in the literature have been encountered during either open surgery or cadaveric dissection. We report a case of palmaris profundus encountered during attempted single-portal endoscopic carpal tunnel release, necessitating conversion to an open approach. There was a unique point of tendon insertion onto the undersurface of the transverse carpal ligament, more proximal than what has been previously described in the literature. There were other anomalies present as well, including a persistent median artery and bifid median nerve. Given the volar position of the structure, its proximal point of insertion, and its minimal bulk, we did not feel that this was the cause of our patient's carpal tunnel syndrome.

Abstract:An interesting case of unusual unilateral variant of palmaris longus (PL) tendon of forearm was noticed by us. We found two bellies of PL as well as their different insertions. These observations will help in understanding morphological variations of this muscle and its clinical implications. PL is a fusiform muscle in the superficial flexor group of muscles of forearm. It originates from medial epicondyle of humerus by common flexor tendon. We found PL having one origin, i.e., from medial epicondyle from common tendinous origin of flexor muscles and then it divided to form two bellies having two long tendons distally. Understanding of presence or absence or anomalies of PL is not only important for medical professionals but also for evolutionary biologists. Awareness of anatomy and variations of flexor tendons is important for health care practitioners for the correct diagnosis and management of pain, disease, and trauma of forearm and hand.

Abstract:Background A true accessory extensor tendon of the thumb seems to be rare. We found a variation of the accessory extensor pollicis longus (EPL) tendon while performing an emergency surgical procedure. Identification of this accessory tendon during emergency surgery has not been previously reported. Case presentation: A 43-year-old man presented to our hospital after sustaining a work-related injury involving a saw. During the operation, the tendons of the EPL, accessory EPL, and extensor pollicis brevis were severed. The lacerated tendon and tendon sheath were repaired, and the wrist and thumb were positioned in extension. The patient’s postoperative activity returned to normal without the need for a second tendon release operation. Conclusions Surgeons should be aware of this anatomic anomaly of the EPL tendon. Presentation of our case increases clinicians’ chances of preoperatively detecting this anomaly, which can improve surgical outcomes.

Abstract:An 89-year-old Caucasian male cadaver with prostate cancer demonstrated bilateral persistence of the median artery and bifid median nerve (BMN) during upper limb dissection. The persistent median artery (PMA) originated from the common interosseous artery and coursed alongside the median nerve. Proximal to the carpal tunnel, the median nerve bifurcated into medial and lateral branches. To our knowledge, this is the first documented case of a bilateral PMA and BMN. While the majority of existing literature focuses on a unilateral PMA or unilateral BMN, bilateral occurrences of either variation are rare. This report presents a novel finding by documenting the simultaneous presence of a bilateral PMA and BMN.
A Rare Case of Extensor Digitorum Communis Rupture Due to Pancarpal Arthritis

Abstract:We report the case of a 71-year-old man who presented to our clinic with extensor digitorum communis (EDC) tendon rupture associated with pancarpal arthritis. He presented with a history of prolonged chainsaw usage. Later that day upon waking up, he noticed an inability to extend his small and ring fingers. On examination, the EDC of the ring and small fingers showed zero power. Radiographs of the wrist joint showed pancarpal arthritis with a dorsally displaced lunate along with distal radio-ulnar joint (DRUJ) osteoarthritis (OA). During surgery, a sharp posterior lunate prominence was noticed to be the cause of the attrition and rupture of EDC. The DRUJ surface was relatively smooth. Proximal row carpectomy and transfer of extensor indicis proprius (EIP) to EDC reverse end-to-side were done. Postoperatively, the patient gained full extension. There are no other similar cases reported in the literature.
Predicting Tendon Tissue Grafting Source From the Extensors of Long Fingers: A Systematic Review of Cadaveric Studies

Abstract:Background: The use of tendon tissue for transfer and grafting is a common practice in hand surgery. Many local tendons are usually used, mainly that of the palmaris longus muscle. However, this tendon could be absent in up to 46%. Extra slips of the extensor digitorum communis (EDC) and extra tendons such as the extensor indicis (EI) and the extensor digiti minimi (EDM) could be valuable sources for tendon tissue. Methods: A systematic review was conducted to quantify the proportion of extra slips in relation to each tendon and extra tendons in relation to each long finger. The aim is to better predict the location of tendon tissue sources on the dorsum of the hand in reconstructive surgery. Results: Based on 39 studies including 7847 hands, the cumulative frequencies of extra slips and extra tendons and the association between double-slip frequency and ancestry were as follows: (1) the little finger showed the highest cumulative frequency of extra slips (96.54%), followed by the ring (37.5%), long (30.6%), and index (9.5%) fingers; (2) the index with its EI and its variants and the little finger with its EDM bear by far the highest cumulative frequency of extra tendons (≈100%); and (3) double slips are found to be rare in Indian populations, most prevalent in Japanese populations, more prevalent in the index and little fingers of Caucasians, and more prevalent in the ring fingers of Middle Eastern populations. Conclusions: Knowledge of the frequencies of extra slips for each long extensor tendon and extra tendons for each long finger along with their prevalence in different populations would improve: (1) location prevision of tendon source for tendon grafting; and (2) surgical planning while supporting a patient-centered approach. Evidence-based hand anatomy would have a major potential to contribute to the practice of an evidence-based hand surgery. Predicting the possible tendon sources that could be present on the dorsum of the hand is thought to be very valuable for hand surgeons. In addition and from an evolutionary perspective, we hypothesized that the observed significantly higher frequency values of the double-slip and triple-slip types of the EDC of the ring finger, EDC of the little finger, and EDM might indicate a natural selection tendency for a higher independence of the former digits in the future evolution of the human hand.

Abstract:<h3 class="Heading">Background</h3><p class="Para">Extensor pollicis longus (EPL) tendon rupture is a debilitating condition that presents with impaired interphalangeal joint (IPJ) extension. Standard descriptions of EPL function state that it is the sole thumb IPJ extensor. It has been suggested that the intrinsic muscles of the thumb, acting through a dorsal extensor expansion (DEE), are also capable of producing IPJ extension which can result in a missed diagnosis of EPL rupture. It remains controversial whether the thumb possesses a DEE analogous to the DEE of the fingers. The aim of this study was to determine if a DEE of the thumb exists and to establish if intrinsic hand muscles insert into it to produce IPJ movement.</p><h3 class="Heading">Methods</h3><p class="Para">Nine cadaveric hands were dissected, and muscle function was assessed.</p><h3 class="Heading">Results</h3><p class="Para">A DEE was observed in all thumbs and its structure closely resembled the DEE of the fingers. The DEEs had clear margins that received contributions from intrinsic muscles of the thumb and EPB. Applying traction to the intrinsic muscle attachment consistently resulted in IPJ extension.</p><h3 class="Heading">Conclusions</h3><p class="Para">A DEE is present in the thumb that structurally and functionally resembles that of the fingers. Traction on the EPL tendon produced a compound movement including IPJ and metacarpophalangeal joint extension and supination of the entire first ray around the carpometacarpal joint. This compound movement is, in effect, the antagonistic movement to thumb opposition. This explains the profound loss of function associated with EPL rupture and suggests a more accurate test for EPL function than simply assessing for active IPJ extension.</p><p class="Para">Level of evidence: Level IV, diagnostic study.</p>
A very rare case of the accessory palmaris longus muscle and clinical significance

Abstract:The palmaris longus muscle is located in the forearm region. It morphological variability was noted during standard anatomical dissection of the upper limb. The muscle was characterized by a normal course, i.e. originating from the medial humeral epicondyle and inserting to the palmaris aponeurosis, but a small additional tendon attached to the flexor retinaculum was observed in its distal part. An accessory palmaris longus muscle was also observed nearby. Interestingly, this accessory muscle was reversed, and the first part was not muscular, but tendinous, represented by two tendons originating from the common muscular mass attached to the medial epicondyle of the humerus; these later connected together, creating one muscle belly distally attached to the flexor retinaculum. This additional structure was innervated by a neural branch from the median nerve and the ulnar artery was responsible for blood supply. The course of the median nerve is also clinically important, because before entering the carpal tunnel, it was located directly under the accessory palmaris longus muscle. In turn, the ulnar artery passed through a special hole created by the flexor digiti minimi brevis and flexor retinaculum, before passing under the palmaris brevis muscle.

Abstract:Purpose Spasticity management in finger flexors (flexor digitorum profundus and superficialis and flexor pollicis longus) is a challenge. Recent studies demonstrated the short- and long-term efficacy of selective and hyperselective neurectomy for the spastic upper limb. However, hyperselective neurectomy of flexor digitorum profundus and flexor digitorum superficialis branches was incomplete, without impairing their muscular body and function. This cadaveric study describes a novel medial approach in the forearm, to reach all the muscular branches: flexor digitorum superficialis and profundus and flexor pollicis longus. Material and Methods Fourteen cadaveric fresh frozen upper limbs were used. The feasibility of the medial surgical approach was studied, as well as the number, length and point of emergence of the muscular branches from the median and ulnar nerves to the flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis. Results The medial approach to the forearm gave access to all the muscular branches from the median and ulnar nerves to the flexor pollicis longus, flexor digitorum superficialis and flexor digitorum profundus, in all cases. A Martin Gruber communicating branch was found in 7 cases out of 14. Conclusion The medial approach to the forearm gave access to all the muscular branches from the median and ulnar nerve to the flexor pollicis longus, flexor digitorum superficialis and flexor digitorum profundus, without extensive transmuscular dissection of the pronator teres or flexor digitorum superficialis muscles. This approach opens the way for selective neurectomy of the flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis muscles. Level of evidence : IV.

A Retrospective Hospital-Based Study to Evaluate the Results of Extensor Tendon Injuries of the Hand, Wrist, and Forearm Treated Surgically in a Tertiary Care Hospital

Abstract:Introduction The important factors determining the treatment of extensor tendon injuries include the anatomical zone, type of injury, mode of injury, chronicity, and pathology of the adjacent tissues (principally the skin, bone, and joints). Very few studies have collectively studied the outcomes of all the wrist, forearm, and hand extensors. Hence, the major aim of this study was to evaluate the results of extensor tendon injuries of the hand, wrist, and forearm that were treated surgically. Methodology This was a hospital-based retrospective study done in a tertiary teaching hospital in South India. A total of 30 patients (23 males, seven females) were included in the study. All the cases of extended tendon injuries of the wrist, hand, and forearm were treated surgically, and those willing to participate were included in the study after obtaining institutional ethics committee approval. Results The study included 30 patients, predominantly males (76.66%). The majority were aged 31-40 years (33.33%). Occupational injuries were the most common cause (36.66%), followed by road traffic accidents (RTAs) and glass cuts (26.66%). Right-sided injuries were more frequent (56.66%), with zone VI being the most affected (43.33%). Extensor digitorum communis was the most injured tendon (40%). Various suture techniques were used, including horizontal mattress and modified Kessler's. Complications occurred in four patients, including hematoma and surgical site infections. Functional outcomes, assessed by Miller's Criteria, indicated extension lag and flexion loss as key recovery measures. Conclusion Hand function is essential for daily life activities, and optimal repair and reconstruction of extensor tendon injuries are crucial to avoid functional disability. While the present study demonstrated positive outcomes, further research with larger sample sizes and more rigorous designs is needed to validate these findings and improve treatment strategies for hand injuries.

Abstract:Background: Extensor tendon injuries are a common condition that hand surgeons must be prepared to treat. The area of extensor tendon injury can traditionally be broken down into 9 zones. Physical examination is the best way to diagnose extensor tendon injury with a loss of active extension in the injured digit. The tenodesis effect may be utilized to aid in diagnosis: wrist flexion should cause passive extension at the metacarpophalangeal, proximal interphalangeal, and distal interphalangeal joints if the extensor tendon is in continuity. Description: Repair of extensor tendon injuries depends on the zone of injury and the thickness of the tendon, which determines its ability to hold core sutures. For zone-I and II injuries, several "figure of 8" buried sutures can be utilized or a running type of suture may be employed. For zone-III to VII injuries, 1 or 2 core sutures and a supplementary running suture can be utilized. Alternatives: Several alternative techniques have been previously described. These include variations in the number of core strands, repair configuration, and suture caliber, as well as the use of epitendinous repair. Alternative treatments also include nonoperative treatment, which is typically reserved for partial tendon injuries and for patients who are unable to tolerate a surgical procedure. Rationale: The techniques that we describe are tailored to the thickness of the tendon. Running sutures are applicable to any zone, whereas core sutures are best utilized in zones III through VII. The running interlocking horizontal mattress technique has been found to be stiffer and faster to accomplish compared with the other techniques, and was found to have good to excellent clinical results in a study of lacerations in zones IV and V1,2. Expected outcomes: Repair of extensor tendon lacerations has demonstrated good long-term outcomes if performed in a timely manner after injury. A previous study showed good to excellent function in up to 64% of acute extensor tendon repairs, more fingers lost the ability to flex fully than lost the ability to extend3. Systematic reviews have suggested that dynamic rehabilitation may not provide superior long-term benefit compared with static splinting4. Important tips: The Elson test should be performed under a digital nerve block in order to properly assess the integrity of the central slip.In distal zone I and III injuries, suture anchors or bone tunnels may be utilized when there is no remaining tendon on the distal end of the laceration.When preparing the tendon ends for repair, it is important to handle the tendon delicately-preferably through the cut end of the tendon rather than the tendon itself.In zone-VII injuries, the injured tendon may lie beneath the extensor retinaculum. In such cases, windowing of the extensor retinaculum may decrease adhesion formation and facilitate repair. Abstract:A 57-year-old female cadaver donated for research contained an accidental bilateral presence of the psoas minor (PMn) muscle. Bilateral PMn is a rare finding but it may be associated with other anatomical variations or abnormalities in the abdomen or pelvis. In some cases, the muscle may be involved in the development of certain medical conditions or may compress nearby structures, leading to symptoms such as pain or discomfort. Overall, bilateral PMn is a relatively uncommon anatomical variation that may be of interest to anatomists or medical professionals. A thorough understanding of this muscle is essential, particularly when making a clinical diagnosis and carrying out procedures in the iliac area. Extensor carpi radialis brevis motor branch transfer to the deep branch of the ulnar nerve to repair intrinsic hand palsy in neonatal lower brachial plexus and spinal cord injury: A case report

Abstract:Concomitant lower neonatal brachial plexus palsy (Klumpke) and spinal cord injury is extremely rare but with a clearly established mechanism of injury pattern. No successful surgical techniques have been reported to date to restore intrinsic hand function. We report a case of successful transfer of the extensor carpi radialis brevis motor branch to the deep branch of the ulnar nerve to repair intrinsic hand palsy. Three-month-old boy with the diagnosis of left Klumpke paralysis and thoracic spinal cord injury associating left Horner's sign, intrinsic minus deformity of all the digits, and thenar muscle paralysis in the upper limb. Both lower limbs were fully paralyzed. Cervical magnetic resonance imaging (MRI) revealed spinal cord narrowing from T1 to T5 and pseudo-meningoceles involving the left C8 through T3 roots. Since no spontaneous recovery was apparent by 6.5 months and surgical exploration showed pronator quadratus denervation, the ECRB motor branch deep branch was transferred to the ulnar nerve (DBUN) with interposed a 7.5 cm-long sural nerve graft. By 18 months post-operatively, all the digits showed complete active IP extension. Thirty-six months after surgery, no signs of first dorsal interosseous nerve or thenar muscle reinnervation were present, thus an extensor carpi ulnaris opponensplasty was performed. ECRB motor branch might be a valuable tool to restore finger intrinsic function in these uncommon cases.

Abstract:Background C7 - T1 palsy results in complete loss of finger motion and poses a surgical challenge. This study investigated the anatomy of the radial nerve in the elbow and forearm and the feasibility of intraplexus nerve transfer to restore thumb and finger extension.Methods The radial nerves were dissected in 28 formalin-fixed upper extremities. Branching pattern, length, diameter, and number of myelinated fibers were recorded.Results Commonly, the branching pattern (from proximal to distal) was to the brachioradialis, extensor carpi radialis longus, superficial sensory proximal to the lateral epicondyle, extensor carpi radialis brevis, supinator, extensor digitorum communis, extensor digiti minimi, extensor carpi ulnaris, abductor pollicis longus, extensor pollicis brevis, extensor pollicis longus, and extensor indicis distal to the lateral epicondyle.Conclusions Branches to the brachioradialis, extensor carpi radialis longus, and supinator can be transferred to the posterior interosseous nerve to restore hand movement in patients with C7 - T1 brachial plexus palsies; the supinator branch is probably the best choice in this regard.
