Abstract:A retrospective study was conducted based on the clinical data of 23 patients who underwent laparoscopic liver resection with tumor in the difficult part of right liver.Localization and edge of the lesion and Glisson system of the liver segment resection required were all marked by laparoscopic ultrasound, followed by clamping and resecting with cut-ultrasound aspiration (CUSA) the inflow pedicle of liver segment.Total laparoscopic liver resection was successfully carried out in all patients with reduced operation time and blood loss, and fast recovery.There was no perioperative death.Intraoperative ultrasound combined with CUSA in laparoscopic hepatectomy for tumors located in the difficult segments of right liver is a feasible, safe and repeatable therapeutic option.

Abstract:Abstract Background Three-dimensional (3D) models are increasingly used to help surgeons, guiding them through the complex hepatic vasculobiliary anatomy. The biliary tract is a relatively untapped territory with only a few case reports described in medical literature. Our aim is to present an innovative 3D reconstruction methodology for biliary imaging and surgical planning, applied to a case of iatrogenic biliary stricture, with fusion of segmented CT and MRI images. Case presentation A selected case of Bismuth type III iatrogenic biliary stenosis for 3D planning. CT and MR studies were acquired with dedicated protocols for segmentation. Two radiologists performed segmentation and 3D model post-processing, fusing both imaging techniques to faithfully render the anatomical structures. Measurements of anatomical landmarks were taken in both the CT/MRI and the 3D model to assure its accuracy and differences in measurement were calculated. The 3D model replicates anatomical structures and pathology with high accuracy, with only 2.2% variation between STL, CT and MRI measurements. The model was discussed with the surgical team and used in the surgical planning, improving confidence in this delicate procedure, due to the detailed prior knowledge of the patient's anatomy. Conclusion Three-dimensional reconstructions are a rapidly growing area of research with a significant impact in the personalized and precision medicine. The construction of 3D models that combine vascular and biliary anatomy, using different imaging techniques, respectively CT and MRI, will predictably contribute to a more rigorous planning of complex liver surgeries.

Optimizing Surgical Plans for Parenchyma-Sparing Liver Resections through Contour-Guided Resection and Surface Approximation

Abstract:Objective: This study introduces a novel method for defining virtual resections in liver cancer surgery, aimed at enhancing the adaptability of parenchyma-sparing resection (PSR) plans. By comparing these with traditional anatomical resection (AR) plans, we explore the potential for optimization in surgical planning. Methods: Leveraging contours and spline surface approximations directly from the liver's surface, our method aligns closely with actual surgical procedures, offering a more realistic representation of curved resection paths. This technique, tested against 14 cases from the OSLO-COMET study, incorporates surface deformation for versatile plan modeling, comparing volumetric outcomes of PSR and AR. Results: The study highlights significant benefits of PSR over AR, including reduced resected volume ($32.71 \pm 13.80$ ml for PSR vs. $249.53 \pm 135.23$ ml for AR, $p <0.0001$) and higher remnant liver volume ($1922.77 \pm 442.86$ ml for PSR vs. $1716.87 \pm 403.00$ ml for AR, $p <0.0001$). PSR also showed a considerably higher remnant percentage ($98.16 \pm 0.81%$) compared to AR ($87.40 \pm 6.49%$, $p <0.0001$). Conclusion: The proposed approach is able to define virtual resections accommodating a wide variety of resections (i.e., PSR and AR). Careful surgical planning using virtual resections can optimize the resection strategy. Significance: This study presents a novel computer-aided planning system for liver surgery, demonstrating its efficacy and flexibility for definition of virtual resections. Virtual surgery planning can be used for optimization of resection strategies leading to increased preservation of healthy tissue.

Using multiple images and contours for deformable 3D-2D registration of a preoperative CT in laparoscopic liver surgery

Abstract:Purpose: Laparoscopic liver resection is a challenging procedure because of the difficulty to localise inner structures such as tumours and vessels. Augmented reality overcomes this problem by overlaying preoperative 3D models on the laparoscopic views. It requires deformable registration of the preoperative 3D models to the laparoscopic views, which is a challenging task due to the liver flexibility and partial visibility. Methods: We propose several multi-view registration methods exploiting information from multiple views simultaneously in order to improve registration accuracy. They are designed to work on two scenarios: on rigidly related views and on non-rigidly related views. These methods exploit the liver's anatomical landmarks and texture information available in all the views to constrain registration. Results: We evaluated the registration accuracy of our methods quantitatively on synthetic and phantom data, and qualitatively on patient data. We measured 3D target registration errors in mm for the whole liver for the quantitative case, and 2D reprojection errors in pixels for the qualitative case. Conclusion: The proposed rigidly related multi-view methods improve registration accuracy compared to the baseline single-view method. They comply with the 1 cm oncologic resection margin advised for hepatocellular carcinoma interventions, depending on the available registration constraints. The non-rigidly related multi-view method does not provide a noticeable improvement. This means that using multiple views with the rigidity assumption achieves the best overall registration error.

Abstract:New sphere decoding and synchronization algorithms for multiple-input-multiple-output (MIMO) orthogonal frequency-division multiplexing (OFDM) systems are proposed in this paper. In particular, an iterative list branch-and-bound (BB) algorithm based on the basic BB algorithm is described to obtain a candidate list to compute soft information that is used in the iterative detector. Furthermore, an improved algorithm that uses prior information from the preceding iteration to calculate the lower bound is proposed, and the candidate list is updated every iteration. To obtain a complete modem architecture, we propose an efficient expectation-maximization (EM)-based iterative algorithm for synchronization and channel estimation to interface with the proposed list-sphere-decoding detector, and we investigate the performance of the designed MIMO-OFDM modem on a realistic fading channel. The obtained performance results show that it is possible to practically design a performing MIMO-OFDM modem with high spectral efficiency, i.e., 8 bit/s/Hz with a 4times4 16-QAM MIMO-OFDM system.
Comprehensive evaluation of liver resection procedures: surgical mind development through cognitive task analysis

Abstract:Background: Liver resection is a complex procedure for trainee surgeons. Cognitive task analysis (CTA) facilitates understanding and decomposing tasks that require a great proportion of mental activity from experts. Methods: Using CTA and video-based coaching to compare liver resection by open and laparoscopic approaches, we decomposed the task of liver resection into exposure (visual field building), adequate tension made at the working plane (which may change three-dimensionally during the resection process), and target processing (intervention strategy) that can bridge the gap from the basic surgical principle. Results: The key steps of highly-specialized techniques, including hanging maneuvers and looping of extra-hepatic hepatic veins, were shown on video by open and laparoscopic approaches. Conclusions: Familiarization with laparoscopic anatomical orientation may help surgeons already skilled at open liver resection transit to perform laparoscopic liver resection smoothly. Facilities at hand (such as patient tolerability, advanced instruments, and trained teams of personnel) can influence surgical decision making. Application of the rationale and realizing the interplay between the surgical principles and the other paramedical factors may help surgeons in training to understand the mental abstractions of experienced surgeons, to choose the most appropriate surgical strategy effectively at will, and to minimize the gap.

Abstract:Three-dimensional liver modeling can be a useful tool when planning the preoperative strategy in liver surgery. The present study aims to review our case series of patients requiring complex hepatic resections for primary and secondary liver tumors, and for whom 3D models were built, to add further evidence in this direction. All consecutive patients undergoing complex liver resection were enrolled. Cross-sectional triphasic CT images were obtained for each patient. DICOM images were processed, and full virtual 3D models were generated. The additional details provided by 3D models were employed to better understand the anatomy, to define the most adequate surgical pathway, and, in case, to switch to a different surgical procedure. From January 2020 to September 2022, 11 complex hepatic resections requiring 3D reconstruction technology were performed. Eight 3D models scored ≥ 15 points in the quality control system. A detailed analysis of each case was reported. In three cases (27%) 3DVT helped to understand the anatomy and/or to detect vascular abnormalities. In six cases (54.5%) 3DVT led to a variation of the surgical planning. 3DVT may be helpful in planning preoperatively the most appropriate surgical procedure. Further large-scale, well-designed studies are needed to prove its true effectiveness in HPB surgical oncology.

Clinical application of a three-dimensional reconstruction technique for complex liver cancer resection

Abstract:ObjectiveTo explore the utility of three-dimensional (3D) visualization technology in liver resection for patients with complex liver cancer.MethodsIn this retrospective cohort study, we collected and analyzed clinic pathological data from 105 patients who underwent complicated liver cancer resection at the authors’ unit between January 2014 and June 2019. Observation indicators included general demographic information, operative time, intraoperative blood loss, blood transfusion volume, postoperative liver function, complication rate, hospital stay, and in-hospital mortality.ResultsCompared with the complex liver cancer control group, operative time (257.1 ± 63.4 min versus [vs] 326.6 ± 78.3 min; P < 0.001), intraoperative blood loss (256.4 ± 159.1 mL vs 436.1 ± 177.3 mL; P < 0.001), blood transfusion volume (213.3 ± 185.2 mL vs 401.6 ± 211.2 mL; P < 0.001), and length of hospital stay (9.7 ± 3.1 days vs 11.9 ± 3.3 days; P = 0.001) were significantly reduced in the complex liver cancer reconstruction group. Although there was no statistical difference in total postoperative complication rate between the two groups, the incidence of serious postoperative complications in the reconstruction group was significantly lower than that in the control group (3/54 [5.6%] vs 10/51 [19.6%], respectively; P = 0.038). Regarding laboratory investigations, the time to recovery of liver function in the complex liver cancer reconstruction group was shorter than that in the complex liver cancer control group.ConclusionThe use of 3D visualization technology was highly influential in formulating meticulous, individualized surgical strategies for complex liver cancer liver resection with safety and reduced perioperative risk.
3D reconstruction in complex parenchymal sparing liver surgery

Abstract:Background: Prognosis of stage IV colorectal cancer is related to control of liver metastasis. As of now, surgery provides survival advantage for patients affected by resectable colorectal liver metastases (CRLM), with parenchymal sparing strategies representing the most accepted strategy {[1]. In this setting, 3D reconstruction programs represent the newest available technological leap to improve anatomical accuracy [2]. Despite being quite expensive, 3D models have proved themselves as helpful adjunctive tools to enhance pre-operative strategy [3] in complex liver procedures, even in the eyes of expert hepatobiliary surgeons [4]. Methods: We present a video describing the practical use of a custom-made 3D model, acquired following specific quality criteria [2], for a case of bilateral CLRM after neoadjuvant chemotherapy. Results: In our reported case and as described in the video, pre-operative visualization of 3D reconstructions altered significantly the pre-operative surgical plan. First, following the principles of parenchymal sparing surgery, challenging atypical resections of metastatic lesions close to main vessels (right posterior branch of the portal vein, inferior vena cava) were preferred to anatomic resections/major hepatectomies, allowing the highest projected future liver remnant volume possible (up to 65%) amongst different available strategies. Secondly, the order of hepatic resections was planned to follow a decreasing degree of difficulty, in order minimize the effect of blood redistribution after previous resections during parenchymal dissection (thus starting from atypical resections close to main vessels, followed by anatomical resections and atypical resections of superficial resections). In addition, the availability of the 3D model in the operating room was crucial in the surgical field to guide safe surgical pathways, especially during atypical resections of lesions close to the main vessels: detection and navigation were further enhanced thanks to tools of augmented reality that allowed the surgeon to manipulate the 3D model through a touchless sensor in a dedicated screen in the operating room and to replicate a mirroring snapshot of the surgical field, without compromising sterility nor the surgical set-up. In the setting of these complex liver procedures, the application of 3D printed models has been described [4]; when available, 3D printed models, particularly useful in the pre-operative phase when explaining the procedure to patients and relatives, have been reported to have comparable significant impact, with feedback from expert hepatobiliary surgeons that is very similar to the one we are reporting in our experience [4]. Conclusion: Routine use of 3D technology does not claim to revolutionize the world of traditional imaging but may be impactful in helping the surgeon visualize the anatomy of that specific individual in a dynamic and three-dimensional way that is similar to the surgical field, thus improving multidisciplinary preoperative planning and intraoperative navigation during complex liver surgery.
The Role and Significance of Digital Reconstruction Technique in Liver Segments Based on Portal Vein Structure

Abstract:Objective: To study the segment of liver according to the large amount of three-dimensional(3D) reconstructive images of normal human livers and the vascular system, and to recognize the basic functional liver unit based on the anatomic features of the intrahepatic portal veins. Methods: The enhanced CT primitive DICOM files of 1 260 normal human livers from different age groups who treated from October 2013 to February 2017 provided by 16 hospitals were analyzed using the computer-aided surgery system.The 3D liver and liver vascular system were reconstructed, and the digital liver 3D model was established.The vascular morphology, anatomical features, and anatomical distributions of intrahepatic portal veins were statistically analyzed. Results: The digital liver model obtained from the 3D reconstruction of CAS displayed clear intrahepatic portal vein vessels of level four.Perform a digital liver segments study based on the analysis of level four vascular distribution areas.As the less anatomical variation of left hepatic portal vein, the liver was classified into four types of liver segmentation mainly based on right hepatic portal vein.Type A was similar to Couinaud or Cho's segmentation, containing 8 segments(537 cases, 42.62%). Type B contained 9 segments as there are three ramifications of right-anterior portal vein(464 cases, 36.82%). The main difference for Type C was the variation of right-posterior portal vein which was sector shape(102 cases, 8.10%). Type D contained the cases with special portal vein variations, which needs three-dimensional simulation to design individualized liver resection plan(157 cases, 12.46%). These results showed that there was no significant difference in liver segmental typing between genders(χ(2)=2.179, P=0.536) and did not reveal any significant difference in liver segmental typing among the different age groups(χ(2)=0.357, P=0.949). Conclusions: The 3D digital liver model can demonstrate the true 3D anatomical structures, and its spatial vascular variations.The observation of anatomic features, distribution areas of intrahepatic portal veins and individualized liver segmentation achieved via digital medical 3D visualization technology is of great value for understand the complexity of liver anatomy and to guide the precise hepatectomy.

Abstract:We, herein, describe a case of complex parenchyma-sparing hepatectomy for multiple bilobar colorectal liver metastases (CRLMs). A 61-year-old woman, previously operated for an occlusive adenocarcinoma of the transverse colon and undergoing adjuvant chemotherapy, developed metachronous bilobar CRLMs. After administration of a II line chemotherapy with partial response, she was referred to our hospital. The pre- and post-treatment imaging recognized seven liver lesions, with a bilobar distribution involving segments 3, 4, 5, 8, and 7. After multidisciplinary team evaluation, the surgical strategy was planned by means of three-dimensional reconstruction and simulation software. The planned and executed procedure consisted of a unique transection plane comprising partial resection of segments 3-4-5-8-7, thus removing the central transversal portion of the liver. Accurate preoperative planning and intraoperative ultrasound for resection guidance allowed us to achieve a complex parenchymal sparing procedure in an advanced disease that would be usually candidate for major resection and staged hepatectomy.

Abstract:Background: Robotic liver surgery represents the most recent evolution in the field of minimally-invasive liver surgery. For planning and guidance of liver resections, surgeons currently rely on preoperative 2-dimensional (2D) CT and/or MR imaging and intraoperative ultrasonography. Translating 2D images into digital 3-dimensional (3D) models may improve both preoperative planning and surgical guidance. The da Vinci® robotic surgical system is a platform suitable for the integration of multiple imaging modalities into one single view. In this study, we describe multimodal imaging options and introduce the Robotic Liver Surgery Cockpit; Methods: in-house developed software was used and validated for segmentation and registration to create a virtual reality 3D model of the liver based on preoperative imaging. The accuracy of the 3D models in the clinical setting was objectively assessed in 15 patients by measuring tumor diameters and subjectively with a postoperative conducted questionnaire; Results: Implementation and applicability of the 3D model in the surgical cockpit was feasible in all patients and the quality of the 3D reconstructions was high in 14 (93%) of cases. Tumor diameters measured on CT and/or MR imaging were comparable to automated measurements using the segmentation software and 3D models; Conclusions: the 3D model was successfully incorporated in the robotic surgery console as part of a multimodality imaging platform and aided the surgeon in planning and guidance of the resection. Future studies should focus on further automation of 3D rendering and progress into augmented reality.

Abstract:Introduction: Three-dimensional liver modeling can lead to substantial changes in choosing the type and extension of liver resection. This study aimed to explore whether 3D reconstruction helps to better understand the relationship between liver tumors and neighboring vascular structures compared to standard 2D CT scan images. Methods: Contrast-enhanced CT scan images of 11 patients suffering from primary and secondary hepatic tumors were selected. Twenty-three experienced HBP surgeons participated to the survey. A standardized questionnaire outlining 16 different vascular structures (items) having a potential relationship with the tumor was provided. Intraoperative and histopathological findings were used as the reference standard. The proper hypothesis was that 3D accuracy is greater than 2D. As a secondary endpoint, inter-raters' agreement was explored. Results: The mean difference between 3D and 2D, was 2.6 points (SE: 0.40; 95 % CI: 1.7-3.5; p < 0.0001). After sensitivity analysis, the results favored 3D visualization as well (mean difference 1.7 points; SE: 0.32; 95 % CI: 1.0-2.5; p = 0.0004). The inter-raters' agreement was moderate for both methods (2D: W = 0.45; 3D: W = 0.44). Conclusion: 3D reconstruction may give a significant contribution to better understanding liver vascular anatomy and the precise relationship between the tumor and the neighboring structures.

Abstract:Objective: To explore the application value of three-dimensional reconstruction of liver based on computer software in preoperative evaluation of precise hepatectomy for complex primary liver cancer. Methods: The clinical data of patients in Peking University People's Hospital Hepatobiliary surgery with complicated primary liver cancer from September 17, 2019 to December 20, 2020 were analyzed retrospectively. The liver volume of patients was calculated, the size and location of tumor were described, and the relationship between tumor and intrahepatic bile duct, portal vein, hepatic artery and inferior vena cava was described. Preoperative decision-making and treatment plan were made to compare the compliance of the actual operation plan with the planned operation plan under the guidance of three-dimensional reconstruction. Results: Among all 10 patients, there were 6 males and 4 females, aged 36 to 75 years. There were 6 cases of Child-Pugh grade A and 4 cases of B liver function. There were 5 cases of portal vein variation and 5 cases of hepatic artery variation. The range and M(Q1,Q3)of actual liver volume measured by 3D reconstruction in 10 patients was 895- 2 477 cm3 and [1 444(1 001, 1 854)] cm3;the nodule volume was 7-1 808 [133(50, 566)] cm3;the nodule volume in the liver was 0.8%-73.0% [11.0(3.3,32.7)]% and the number of suspected lesions was 1-7. In the end, 6 patients received surgical treatment;3 received chemotherapy, and 1 received conservative treatment. The preoperative three-dimensional reconstruction of liver anatomy and tumor location of the patients receiving surgical treatment were basically consistent with the intraoperative situation. There was no case of liver failure or death during perioperative period. Conclusion: In the preoperative evaluation of precise hepatectomy for complex primary liver cancer, 3D reconstruction can improve the rationality of treatment and the surgeon's understanding of the anatomical characteristics of the liver, so as to select the best treatment for patients, which has great application value.

Abstract:Liver resection has been established currently as an effective and standard treatment for patients suffering from both benign and malignant hepatobiliary diseases. Although substantial improvement in perioperative mortality rate and morbidity resulting from appropriate candidates selection, advanced surgical techniques and enhanced perioperative care, hepatectomy is still burdened by about 5% mortality rate and some lethal postoperative complications, especially postoperative liver insufficiency and failure. Various approaches have been advocated to minimize stress and insult on patients due to operative procedures. It becomes important to preserve remnant hepatic function as much as possible to improve the outcome of hepatectomy. Minimally invasive concept and fast track surgery are crucial breakthrough in the natural history of surgery and have been employed in liver resection. To safely and accurately perform hepatic resection, owing to our experiences with recent advances in surgical techniques and perioperative administration for liver resection, a novel strategy, "precise hepatectomy" originating from minimally invasive surgery has been developed, which includes precise preoperative planning, sophisticated intraoperative techniques and careful postoperative management. This strategy is characteristic by involvement of minimally invasive concept in overall therapy, from preoperative assessment to postoperative care, optimization of a series of advanced techniques and proper employment of surgical instruments in light of actual individual information. However, further prospective studies, especially randomized controlled trials in high volume centers, remain essential to compare the safety and therapeutic efficacies between precise hepatectomy and conventional surgical procedures.
Laparoscopic Extended Segmentectomy VIII Guided by Three-Dimensional Reconstruction and Hepatic Veins with a Cranio-Caudal Approach

Abstract:Introduction: Minimally invasive resection of segment VIII is a technically challenging procedure, made even more challenging when the resection is extended to segment IV and/or segment VII. Parenchymal-sparing resections are frequently used in the management of liver metastases but expose to the risk of R1 resection, especially with a minimally invasive approach. Preoperative surgical planning with 3D reconstruction and intraoperative guidance with hepatic vein is helpful for laparoscopic oncological liver resection.1-3 PATIENT AND METHODS: We present the case of a 58-year-old female with three metachronous liver metastases from epidermoid anal cancer. The disease was stable 6 months after cessation of chemotherapy. Metastases were mainly located in segment VIII (with a large segment VIII dorsal) but also in the territory of glissonian pedicles from segments IV and VII. Prior to surgery, three-dimensional (3D) reconstruction showed that a segmentectomy VIII would not be sufficient to have a safety margin and showed the relation between metastases and hepatic veins. Transection of the liver was performed with an ultrasonic dissector. Exposure of the hepatic veins was performed by gently pulling of the hepatic tissue from the vein, using the nonactive blade of the ultrasonic device. Activation of ultrasonic energy was performed only for sealing and dividing small collateral veins. Three transection lines were necessary. The posterior transection line, in segment VII, was determined with intraoperative ultrasound (IOUS), at 1 cm below the metastasis. The liver was transected superficially only. The medial transection line, in segment IV, was determined with IOUS, at 1 cm on the left of the metastasis, parallel to the middle hepatic vein. Finally, the inferior transection line, between segment V and segment VIII, was approximately determined with IOUS, vertically aligned with the hepatic vein of segment V. The transection line was further corrected after clamping the glissonian pedicle of segment VIII, according to fluorescence. The surgical procedure began with the mobilization of the right liver, including division of the hepato-caval ligament, followed by the superficial transection of the posterior margin in segment VII. Then, transection of segment IV was performed near the termination of the middle hepatic vein, which was further exposed with a cranio-caudal approach to minimize the risk of vein injury. The hepatic vein of segment V was then used as a landmark for the identification of the Glissonian pedicle of segment VIII, which was transected.4 Termination of the right hepatic vein (RHV) was then identified, and the ventral branch of the RHV was transected. The dorsal branch of the RHV was exposed with a cranio-caudal approach. Finally, transection of segment VII was performed toward the transection line made initially. Results: Operative time was 360 min with 450 mL blood loss. The Pringle maneuver was used during 148 min. The patient was discharged on the seventh postoperative day. Pathological examination confirmed R0 resection, with 20-60% necrosis of the three liver metastases. The resected liver weight was 225 g. Six months after liver resection, the patient had a recurrence in a celiac lymph node, which was treated by radiotherapy. Fifteen months after liver resection, the patient is free of disease without active treatment. Conclusion: Preoperative virtual hepatectomy facilitates surgical planning by increasing the understanding of the tumors-vessels relationship. Intraoperative hepatic vein guidance with a cranio-caudal approach enables to follow preoperative surgical planning and to perform safe complex laparoscopic liver resection.

Abstract:ABSTRACT— Amyloid protein in Icelandic patients with hereditary cerebral amyloid angiopathy (CAA) is a variant of cystatin C. Immunoreactivities of the cystatin C and other amyloid proteins were investigated in CAA and other senile amyloid deposits in the Japanese sporadic aged cases including patients with dementia of Alzheimer type, and compared with those in Icelandic hereditary CAA. Compared with positive reaction of cystatin C in Icelandic hereditary CAA, no immunoreactivity of cystatin C was found in senile amyloid deposits of the Japanese aged including CAA. Immunoreactivity of the amyloid β protein was negative in Icelandic hereditary CAA, for which CAA and senile plaque amyloid in the Japanese senile brains were positive. Our data suggest that the cystatin C amyloid would be present only in the hereditary CAA, but not in the CAA and other senile amyloid deposits of the sporadic aged cases.
