Abstract:Journal of Interpersonal Violence, Ahead of Print. About one-third of sexual minority men (SMM) report intimate partner violence (IPV) victimization, and one-fourth report IPV perpetration, in their lifetime. IPV disclosure and help seeking are key processes of coping with or managing exposure to abuse. We surveyed 500 SMM residing across the United States who enrolled in project EROS (Empowering Relationships and Opportunities for Safety). In all, 201 participants reported any type of IPV in the past 6 months (including 109 who reported both victimization and perpetration); these men were then categorized as experiencing balanced bidirectional (n = 53), predominantly victimization (n = 113), and predominantly perpetration (n = 35) IPV. In each of these groups, close to one-third (25.7%–34.5%) had disclosed their IPV experience and one-fifth (13.2%–20.0%) had sought help; however, within the perpetration group, none of the 16 participants who only reported perpetration (the other 19 reported some victimization) disclosed their IPV. Group sizes allowed us to examine correlates of IPV disclosure and help seeking among the bidirectional and victimization groups, from among demographic, psychosocial, relational, and IPV characteristics, using bivariate statistics and multiple logistic regression. Greater IPV victimization was positively correlated with disclosure in both groups, and it was the lone independent correlate in multiple regression analysis (in the victimization group). Greater IPV victimization was also a strong positive and independent correlate of help seeking in multiple regression analysis within the victimization group, as was greater partner assertiveness in conflict resolution. There were no independent correlates of disclosure and help seeking within the bidirectional group. These findings highlight the need for efforts to empower SMM to seek support and services when exposed to IPV, and to do so early in the occurrence of IPV.

Evaluation of traditional risk factors for intimate partner violence among sexual and gender minority youth.

Abstract:Objective: Sexual and gender minority youth assigned female at birth (SGM-AFAB) experience higher rates of intimate partner violence (IPV) than heterosexual and cisgender youth. To inform efforts to reduce these disparities, we explored whether IPV risk factors identified in the general population are associated with IPV among SGM-AFAB young people. Method: Using multiwave longitudinal data from 400 SGM-AFAB youth (Ages 16–20 at baseline), we estimated between- and within-persons effects of demographic/contextual characteristics (gender, sexual identity, race/ethnicity, socioeconomic status), developmental/background factors (childhood violence), and psychological/behavioral factors (antisocial behavior, depression, problematic alcohol, and cannabis use) on a range of IPV experiences (victimization and perpetration of psychological, physical, sexual, and SGM-specific IPV). Results: In this SGM-AFAB sample, IPV experiences were associated with many traditional risk factors identified in the general population, including race, economic stress, childhood violence, antisocial behavior, depression, and use of substances (particularly cannabis). In contrast to previous research, we did not find that SGM youth with transgender or gender nonbinary identities, or with bi- or pan-sexual identities, were at greater risk for IPV than other SGM youth. Very few putative risk factors were associated with SGM-specific IPV. Conclusions: Findings suggest SGM youth could benefit from IPV prevention approaches that target common risk factors at multiple ecological levels (policies to reduce poverty and racism, parenting programs, interventions to reduce mental health and substance use problems). Continued research is needed to explore how the risk for IPV among SGM-AFAB youth may vary by gender identity, sexual identity, and stigma-based experiences.

Disclosure of Intimate Partner Violence to Informal Social Support Network Members

Abstract:This article presents a review of the published literature to date on rates, experiences, and correlates of victims’ disclosure of or help seeking for intimate partner violence to informal social support network members (e.g., friends, family, classmates, and coworkers). Research indicates that the majority of individuals disclose to at least one informal support and that victims’ disclosure is associated with a number of demographic (e.g., victims’ sex, age, race), intrapersonal (e.g., victims’ feelings of shame/embarrassment, perception of control over abuse), and situational (e.g., violence frequency and severity, if abuse is witnessed) factors. Following disclosure, victims experience a wide range of positive (e.g., believing the victim’s reports, validating the victim’s experiences) and negative (e.g. disbelieving, blaming the victim) social reactions, with positive reactions rated as the most common and most helpful forms of support by victims. Finally, a review of psychological correlates associated with reactions to disclosure indicates that positive social reactions are associated with more psychological health benefits and fewer negative health symptoms, whereas negative social reactions were associated with increased negative psychological health symptoms. Future research methodologies and implications for violence prevention, intervention, and policy are discussed.
Common and Unique Risk Factors for Intimate Partner Violence Victimization Among Sexual and Gender Minority Individuals Assigned Female at Birth

Abstract:Sexual and gender minority youth assigned female at birth (SGM-AFAB) are at disproportionately high risk for intimate partner violence victimization (IPVV), yet remain understudied. Using two time points of data collected from 367 SGM-AFAB young people (aged 16–31 years), we tested whether common, general population risk factors (childhood violence, depression, alcohol and cannabis use, and low social support) and unique stigma-related factors (enacted stigma, microaggressions, and internalized stigma) prospectively predicted psychological, physical, sexual, and identity abuse IPVV in the following 6 months. Results indicated that some traditional risk factors, including child abuse, depression, cannabis use, and low social support, raise IPVV risk among SGM-AFAB youth. Microaggressions and internalized stigma represent additional, unique IPVV risk factors in this population. SGM-affirmative efforts to prevent IPVV should address these common and SGM-specific risk factors.

Abstract:Background: Almost half of all women in the US experience intimate partner violence (IPV) in their lifetime. The US Preventive Services Task Force recommends IPV screening paired with intervention for women of reproductive age. We aim to understand clinical practices and policies that are beneficial, detrimental, or insufficient to support survivors of IPV in a safety-net healthcare system. Methods: We sampled 45 women who were 18-64 years old, had experienced IPV within the prior year and were patients in the San Francisco Health Network. We conducted in-depth, semi-structured interviews to elicit their perspectives on disclosing IPV and obtaining support within the healthcare system. We analyzed our data using thematic analysis and grounded theory practices informed by ecological systems theory. Findings: We identified four themes regarding factors that impeded or facilitated discussing and addressing IPV across interpersonal and systemic levels relating to relationship-building, respect, autonomy and resources. (1) Interpersonal barriers included insufficient attention to relationship-building, lack of respect or concern for survivor circumstances, and feeling pressured to disclose IPV or to comply with clinicians' recommended interventions. (2) Interpersonal facilitators consisted of patient-centered IPV inquiry, attentive listening, strength-based counseling and transparency regarding confidentiality. (3) Systemic barriers such as visit time limitations, clinician turn-over and feared loss of autonomy from involvement of governmental systems leading to separation from children or harm to partners, negatively affected interpersonal dynamics. (4) Systemic facilitators involved provision of resources through IPV universal education, on-site access to IPV services, and community partnerships. Conclusions: Women experiencing IPV in our study reported that relationship-building, respect, autonomy, and IPV-related resources were essential components to providing support, promoting safety, and enabling healing in the healthcare setting. Successful trauma-informed transformation of healthcare systems must optimize interpersonal and systemic factors that improve survivor wellbeing while eliminating barriers.

Abstract:Journal of Interpersonal Violence, Ahead of Print. Within the lesbian, gay, bisexual, transgender, and queer (LGBTQ) community, people of color (POC) disproportionately experience intimate partner violence (IPV). While shared decision-making (SDM)—a model of patient-provider communication—about IPV could benefit LGBTQ POC, its unique challenges merit consideration. This study identifies key factors affecting SDM between LGBTQ POC and healthcare providers surrounding IPV. LGBTQ POC participants (n = 217) in Chicago and San Francisco completed surveys about demographic information, healthcare utilization, and IPV history. Individual interviews and focus groups were then conducted with a Chicago-based subset of participants (n = 46) who identified as LGBTQ IPV survivors of color. Descriptive analyses were conducted of survey responses while focus group and interview transcripts were analyzed and thematically coded. Although 71% of survey participants experienced IPV, only 35% were asked about IPV in healthcare interactions within the previous year. Focus group and interview participants endorsed encounter-, patient-, and provider-centered factors affecting SDM around IPV. When IPV was discussed, patient-provider trust was essential while concordance of identities could either encourage or discourage IPV disclosure. Patients were hesitant to disclose IPV if they had never discussed their LGBTQ identity with their provider or thought providers would ignore their preferences for addressing IPV. Deterrents to SDM included providers denying the prevalence of IPV among LGBTQ individuals or lacking resources to support LGBTQ IPV survivors of color. This study highlights the identity-driven barriers that LGBTQ POC face in discussing IPV with providers. Utilizing SDM to discuss IPV with LGBTQ POC can better address the diverse health needs of this community. However, its success requires that providers acknowledge the diversity of experiences among this population, promote LGBTQ-inclusive practices, and identify resources welcome to LGBTQ POC.

Abstract:Although Asian women immigrants to the USA rarely disclose intimate partner violence, local research indicates that among them domestic abuse is prevalent. This study aimed to determine the main psychosocial barriers and enablers to disclosure among Asian-American women in California, and whether barriers outweighed benefits. We used a novel qualitative methodology of indirect and direct questioning with sixty married women from four ethnicities (Korean, Chinese, Thai and Vietnamese). Overall, barriers to disclosure were more compelling and tangible than enablers, particularly among Mandarin Chinese and Korean speakers. Five main barriers were found: victim-blaming, beliefs in female inferiority and male dominance, familial shame, individual shame and fear of undesirable consequences. Only extreme violence and the need to protect children from harm were seen as warranting disclosure. As a result, health and other providers' encouragement of disclosure is unlikely to be sufficient to achieve behavioural change. Abused Asian immigrant women need anonymous ways of obtaining professional counselling, information and resources. In addition, community-level awareness programmes in Asian languages are needed to reduce victim-blaming and misinformation.

Abstract:Risks of intimate partner violence (IPV) often are higher among immigrant women, due to dependency, language barriers, deportation fears, cultural beliefs, and limited access to services. In the United States, Asian immigrant women experiencing IPV often are reluctant to disclose abuse. Viewing videos that depict IPV survivors who have successfully obtained help might encourage disclosure. After conducting formative research, we created brief videos in four Asian languages (Korean, Mandarin Chinese, Thai, and Vietnamese) for use in primary care clinic consultation rooms. We then conducted in-depth interviews with 60 Asian immigrant women in California to get their perspectives on how helpful the videos might be in achieving disclosure. Most participants believed the videos would promote disclosure in clinics, although those who had been abused seemed more skeptical. Many had stereotyped views of victims, who they felt needed to be emotive to be credible. Videos should be upbeat, highlighting the positive outcomes of escaping violence and showing clearly each step of the process. Various types of IPV should be described, so that women understand the violence is not exclusively physical. Victims would need reassurance that they will not be arrested, deported, or forced to leave their abusers. Discussing the benefits of escaping violence to children could be influential. Victims also must be convinced that providers are trustworthy, confidential, and want to help. To assist immigrant populations to disclose IPV to a health provider, videos need to be culturally relevant, explain various types of violence, allay fears, and show clear processes and benefits.
Exploring help-seeking experiences of Chinese immigrant survivors of intimate partner violence in the U.S.

Abstract:Objective: Immigrant women are vulnerable to intimate partner violence (IPV), and differences in immigration history, language, and culture impact their trauma responses. Although Chinese are the largest Asian immigrant subgroup in the U.S., little IPV research has specifically focused on this population. Therefore, we aimed to gain a better understanding of help-seeking experiences of Chinese immigrant women who had IPV. Method: Twenty female Chinese immigrant survivors of recent IPV completed a Mandarin-language qualitative phone interview about their IPV experiences, help-seeking behaviors, and barriers to services. Interviews were analyzed using thematic analyses. Results: Women in our sample relied on family, friends, and self-help strategies to cope with IPV. They experienced feelings of shame, stigma, and "losing face," and had little social support. They also faced significant sociocultural and immigration-specific problems with formal services (e.g., culturally incongruent therapists, language, financial and transportation barriers, concerns about partner's reputation and legal status), lacked knowledge about available services, and rarely engaged with them. Conclusions: Abused Chinese immigrant women may be exceptionally isolated, and culturally specific barriers may substantially impact both their understanding of their IPV experiences and their use of services. Practitioners need training and support to engage survivors in culturally competent ways. While safety planning and other formal IPV services are critical to safety and trauma recovery, these survivors underutilize them, and community outreach is needed to U.S. Chinese communities to raise awareness regarding available IPV resources and services. Finally, research is needed to develop and test culturally competent, evidence-based interventions for this isolated and vulnerable population. A Mixed Methods Study of Barriers to Help-Seeking for Intimate Partner Aggression in the LGBTQIA+ Community

Abstract:Journal of Interpersonal Violence, Ahead of Print. People in the LGBTQIA+ community (i.e., lesbian, gay, bisexual, transgender, queer, intersex, asexual, and other gender/sexual minorities) experience greater rates of intimate partner aggression (IPA) than the general population and have fewer help-seeking pathways available. The current research examined the extent to which LGBTQIA+ people's perceptions of barriers to help-seeking were associated with perceptions of societal heteronormativity—the belief that being cisgender and heterosexual is the norm—and whether the source of support was formal (e.g., police, counselors) versus informal (e.g., friends, family). The current research was conducted in two parts. In the first part of the study (Study 1a), structural equation modeling indicated a significant positive association between perceived societal heteronormativity and self-focused barriers (e.g., feeling too ashamed or guilty to seek help) but not with other-focused barriers (e.g., expecting unfair treatment). Instead, LGBTQIA+ people perceived greater other-focused barriers when considering formal compared to informal sources of support. In the second part of the study (Study 1b), we interviewed 10 LGBTQIA+ people about barriers to help-seeking for IPA. A reflexive thematic analysis identified four themes: (1) Who can hold the status of being a "victim"?; (2) The heightened importance of autonomy; (3) Formal supports need LGBTQIA+ competency; and (4) Judged by the outside in. The themes illustrated unique barriers experienced by LGBTQIA+ people when judging possible harm, choosing whether to seek help, and actual help-seeking. Altogether, current help-seeking pathways for IPA are generally inaccessible to people in the LGBTQIA+ community. IPA interventions for the LGBTQIA+ community require awareness of stigma, improved education for informal and formal support pathways, and the development of community-led interventions.

The Role of Identity and Gender Beliefs in Self-Identification of Abuse for Male Victims of IPV

Men experience numerous barriers in seeking help or resources after intimate partner violence (IPV) victimization, with one barrier being a reluctance or lack of ability to identify that they have been victims of IPV. This study examines factors relating to male victims of IPV self-identification of abuse. Using a gender socialization approach, demographics and facets of masculine honor ideology were tested to see whether they were significantly related to self-identifying as experiencing abuse in their relationship. Using a sample of 289 men, the frequency of individuals who self-identified as someone who had been in an abusive relationship was compared to the number of individuals who indicated IPV victimization on the Revised Conflict Tactics Scale (CTS2), and percentages of correct identification were calculated. In addition, a binary logistic regression was run to examine factors that were related to someone identifying as a victim of IPV compared to those that were not. When directly asked if they had ever experienced abuse in a romantic relationship, a total of 41 (14.2%) men self-identified as having been a victim of IPV. However, when examining scores on the CTS2, 69 (23.9%) reported some sexual IPV, 201 (69.6%) indicated psychological abuse, and 59 (20.4%) indicated physical abuse. Subscales of the Masculine Honor Belief Scale were not found to have a significant relationship with self-identification, but differences were found among types of IPV and demographics. Specifically, men who identify as LGBTQ+ were significantly more likely to identify when they experienced IPV victimization. Barriers to male self-identification and treatment are discussed.

Abstract:Supportive care for survivors of intimate partner violence (IPV) remains limited in primary care settings. Low-income and Spanish-speaking survivors of IPV are even more disadvantaged, given the dearth of linguistically and culturally appropriate interventions for IPV. We conducted semi-structured individual interviews with 17 healthcare workers, including physicians, nurses, and social workers, to describe how healthcare workers serving primarily low-income, Latina populations are currently screening and responding to IPV disclosure, and to explore the acceptability of integrating an interactive, personalized safety decision aid application-myPlan app-into the clinic setting. Despite recognition of IPV as a problem, none of the clinical sites had a protocol to guide screening and response to IPV disclosure. Screening practices varied across the sites, sometimes conducted by medical assistants prior to the provider visit and other times by the physician or nurse provider. When IPV was disclosed, it was often during assessment for a presenting problem such as poor sleep or anxiety. Most healthcare workers felt that clinical and community resources were limited for their patients experiencing IPV. The "warm hand-off" to a social worker was the most common response strategy when possible; otherwise, women were given information about available resources such as hotlines and safe houses. We discuss structural, family, and individual barriers to accessing safety resources for underserved women and review how an easily accessible safety decision app, such as myPlan, could be a resource for women to safely tailor an action plan for her situation.
A Longitudinal Study of IPV Victimization Among Sexual Minority Youth

Abstract:Although intimate partner violence (IPV) is highly prevalent among lesbian, gay, bisexual, and transgender (LGBT) youth, little is known regarding its developmental patterns, risk factors, or health-related consequences. We examined IPV victimization in an ethnically diverse community-based convenience sample of 248 LGBT youth (aged 16-20 at study outset) who provided six waves of data across a 5-year period. Results from multilevel models indicated high, stable rates of IPV victimization across this developmental period (ages 16-25 years) that differed between demographic groups. Overall, 45.2% of LGBT youth were physically abused and 16.9% were sexually victimized by a dating partner during the study. Odds of physical victimization were 76% higher for female than for male LGBT youth, 2.46 times higher for transgender than for cisgender youth, and 2 to 4 times higher for racial-ethnic minorities than for White youth. The prevalence of physical IPV declined with age for White youth but remained stable for racial-ethnic minorities. Odds of sexual victimization were 3.42 times higher for transgender than for cisgender youth, 75% higher for bisexual or questioning than for gay or lesbian youth, and increased more with age for male than female participants. Within-person analyses indicated that odds of physical IPV were higher at times when youth reported more sexual partners, more marijuana use, and lower social support; odds of sexual IPV were higher at times when youth reported more sexual partners and more LGBT-related victimization. In prospective analyses, sexual IPV predicted increased psychological distress; both IPV types marginally predicted increased marijuana use.

Abstract:Intimate partner violence (IPV), inclusive of all forms of abuse, is an ongoing public health and criminal-legal issue that transcends social boundaries. However, there is a lack of equitable representation of diverse populations who experience IPV in the literature. To garner a holistic knowledge of diverse IPV survivor populations' experiences with seeking help from the police, the current review utilized a qualitative research synthesis methodology to explore police interactions among six IPV survivor populations that are underrepresented in the current literature: women with substance use issues, immigrant women, women in rural localities, heterosexual men, racially/ethnically minoritized women, and sexual minority women. Seven electronic databases were searched to identify peer-reviewed articles on IPV survivors' narrative descriptions (qualitative or mixed-methods) of their encounters with law enforcement. The final analysis included 28 studies that were then coded with an iterative coding strategy. The analysis uncovered the following themes: (a) revictimization by the police, (b) police negligence, (c) discrimination, (d) cultural differences, and (e) positive experiences. These themes demonstrated that while some experiences with law enforcement were shared between under-researched survivor groups, some experiences were explicitly tied to some aspects of survivors' identities. Recognizing the potential law enforcement has to support survivors, the findings of the current review reiterate the need for ongoing efforts to improve law enforcement knowledge and overall response to IPV, especially for diverse populations of IPV survivors.

Abstract:A 76-year-old male with local recurrence after abdominosacral resection of recurrent rectal cancer was admitted to our hospital. Computed tomography (CT) and magnetic resonance imaging (MRI) indicated a recurrent tumor at the back of the prostate and seminal vesicles up to the resected sacral (S3) stump. However, positron emission tomography (PET)/CT fusion images showed no 18F-2-fluoro-2-deoxy-D-glucose (FDG) uptake at the pre-sacral region, which suggested postoperative scarring of the tissue. Based on the PET/CT diagnosis, we performed abdominoperineal en bloc resection of the recurrent tumor with bladder, seminal vesicles, prostate, internal obturator muscle, ischial spine, and gluteal muscle without resection of the sacrum. Specimen revealed no circumferential resection margin involvement by tumor on pathologic examination. The patient was discharged from the hospital on postoperative day 23 without major complications.
Chronic Pain in People with Physically Disabling Conditions: A Review of the Application of Biopsychosocial Models

Abstract:There are a wide range of conditions which can result in physical disabilities, affecting more than a billion people or approximately 15% of the population worldwide (World Health Organisation, 2011). Disability is an umbrella term for the restrictions and impairments arising from the interaction between an individual with a health condition and the environment (World Health Organisation, 2011). The physical conditions from which disability arises are diverse and heterogeneous, but include both diseases, such as multiple sclerosis, as well as traumatic and non-traumatic injuries, including spinal cord injury and brain injuries. Research concerning the occurrence of chronic pain (defined as pain persisting beyond the period of healing, usually operationalized as three or six months) among people with physical disabilities, and the impact upon those individuals affected, is relatively limited compared to that focusing on primary pain conditions, such as low back pain. Within the available literature the focus is largely biomedical, with the majority of available research exploring biological factors and medical interventions. A great deal has been gained in the management of primary pain conditions through investigating psychological and social factors, and developing interventions such as cognitive behavioural pain management programs to target these factors. This chapter will describe the current understanding of the role of psychological and social factors in understanding the experience of chronic pain in the context of a physically disabling condition, and the use of interventions addressing these factors in this population.

Abstract:Introduction Intimate partner violence (IPV) involves an individual committing acts intended to harm or intimidate a current or former romantic partner. The COVID‐19 pandemic and subsequent stay‐at‐home orders often trapped victims with perpetrators and intensified IPV. Although sexual and gender diverse people disproportionately experience IPV compared to cisgender, heterosexual people, their experiences are not well documented in the Canadian context. This study aimed to explore the experiences of Two‐Spirit, lesbian, gay, bisexual, transgender, queer, intersex, asexual, and other sexual and gender diverse (2S/LGBTQIA+) people with IPV in Ontario and how the COVID‐19 pandemic affected their IPV experiences. Methods We conducted in‐depth, semi‐structured interviews with self‐identified 2S/LGBTQIA+ people who experienced IPV on/after March 15, 2020. We audio‐recorded and transcribed all interviews and coded the transcripts for content and themes using inductive and deductive techniques. Results Our 20 participants experienced physical, psychological, sexual, and financial abuse. Technology‐facilitated violence extended abuse geographically and temporally. IPV experiences were associated with negative mental health outcomes that were intensified by the COVID‐19 pandemic. Participants struggled to see themselves as legitimate victims of IPV. Although participants regretted being victims of violence, many saw their abusive relationship(s) as a learning experience to inform future relationships. Discussion Our findings suggest that 2S/LGBTQIA+ people may experience unique forms of identity abuse and may have difficulty recognizing their IPV experiences as abuse. Ensuring that comprehensive sexual health education is trauma‐informed, anti‐oppressive, and includes information about healthy relationship dynamics, 2S/LGBTQIA+ relationships, and IPV is critical.
